Abstract Preeclampsia is a multifactorial condition associated with significant morbidity and mortality. Fluid therapy in these patients is challenging since volume expansion may precipitate pulmonary edema, and fluid restriction may worsen renal function. Furthermore, cardiac impairment may introduce an additional component to the hemodynamic management. This article reviews the repercussions of preeclampsia on renal and cardiovascular systems and the development of pulmonary edema, as well as to discuss fluid management, focusing on the mitigation of adverse outcomes and monitoring alternatives. The literature review was carried out using PubMed, Embase, and Google Scholar databases from May 2019 to March 2020. Papers addressing the subjects of interest were included regardless of the publication language. There is a current trend towards restricting the administration of fluids in women with non-complicated preeclampsia. However, patients with preeclampsia may experience hemorrhagic shock, requiring volume resuscitation. In this case, hemodynamic monitoring is recommended to guide fluid therapy while avoiding complications.

RESUMEN Bixa orellana L. es una planta nativa de Brasil, pero también está presente en otros países tropicales como Perú, Colombia, Ecuador, México, Indonesia, India y África Oriental. Es conocida popularmente como Urucum en Brasil. Esta revisión expone el potencial de los compuestos bioactivos derivados de B. orellana para tratar enfermedades debido a sus propiedades antimicrobianas y antioxidantes. Esta planta también está relacionada como un agente antiinflamatorio para el tratamiento de enfermedades pulmonares e incluso como gotas para los ojos para el enrojecimiento. Sus hojas se utilizan para el tratamiento de la mordedura de serpiente, diarrea, gonorrea, hepatitis, gastritis, diuréticos, antipiréticos y para enfermedades de la piel. Ese conocimiento popular ha fomentado la identificación de compuestos bioactivos en esa planta. Los compuestos β-criptoxantina, geranilgeraniol, luteína, procianidina B2, procianidina B3, isómero elagitanino y ácido elágico desoxihexosa inhibieron microorganismos patógenos como bacterias, hongos, protozoos y virus. En ese sentido, B. orellana es una fuente prometedora de compuestos que podrían aplicarse en la terapia antimicrobiana. Este trabajo de revisión puede ayudar a comprender e incentivar nuevas investigaciones para los descubrimientos de antimicrobianos que utilizan diferentes compuestos de B. orellana.

SUMMARY Bixa orellana L. is a native plant from Brazil, but it is also present in other tropical countries such as Peru, Colombia, Ecuador, Mexico, Indonesia, India and East Africa. It is popularly known as Urucum in Brazil. This review shows the potential of bioactive compounds derived from B. orellana to treat infectious diseases due their antimicrobial and antioxidant properties. This plant is also related as an antiinflammatory agent for treatment of pulmonary diseases, or even as eye drops for redness. Its leaves are used for treatment of snakebite, diarrhea, gonorrhea, hepatitis, gastritis, diuretic, antipyretic, and for skin disease. This popular knowledge has encouraged the identification of bioactive compounds in this plant. Compounds as β-cryptoxanthin, geranylgeraniol, lutein, procyanidin B2, procyanidin B3, ellagi tannin isomer and ellagic acid deoxyhexose have been described. These compounds inhibited pathogenic microorganisms such as bacteria, fungi, protozoan and viruses. In addition, some compounds with anti-inflammatory and antioxidant activities were also described. In this sense, B. orellana is a promising source of compounds that could be applied in antimicrobial therapy. This review work may help in the understanding and incentive of new research for antimicrobial discoveries using different B. orellana compounds.

OBJECTIVES: The aim of this study was to determine whether a restrictive compared to a liberal fluid therapy will increase postoperative acute kidney injury (AKI) in patients with severe preeclampsia. METHODS: A total of 46 patients (mean age, 32 years; standard deviation, 6.8 years) with severe preeclampsia were randomized to liberal (1500 ml of lactated Ringer’s, n=23) or restrictive (250 ml of lactated Ringer’s, n=23) intravenous fluid regimen during cesarean section. The primary outcome was the development of a postoperative renal dysfunction defined by AKI Network stage ≥1. Serum cystatin C and neutrophil gelatinase-associated lipocalin (NGAL) were evaluated at postoperative days 1 and 2. ClinicalTrials.gov: NCT02214186. RESULTS: The rate of postoperative AKI was 43.5% in the liberal fluid group and 43.5% in the restrictive fluid group (p=1.0). Intraoperative urine output was higher in the liberal (116 ml/h, IQR 69-191) than in the restrictive fluid group (80 ml/h, IQR 37-110, p<0.05). In both groups, serum cystatin C did not change from postoperative day 1 compared to the preoperative period and significantly decreased on postoperative day 2 compared to postoperative day 1 (p<0.05). In the restrictive fluid group, NGAL levels increased on postoperative day 1 compared to the preoperative period (p<0.05) and decreased on postoperative day 2 compared to postoperative day 1 (p<0.05). CONCLUSION: Among patients with severe preeclampsia, a restrictive fluid regimen during cesarean section was not associated with increased postoperative AKI.

Resumo Objetivo Há comprovação do importante papel das alterações no sistema imunológico no desencadeamento e manutenção da síndrome nefrótica idiopática (SNI). O objetivo deste estudo foi investigar a expressão das citocinas em populações de linfócitos de pacientes com SNI em comparação a indivíduos saudáveis e de acordo com a proteinúria. Métodos Este estudo transversal incluiu 44 pacientes com SNI e oito crianças saudáveis, pareados por idade e sexo (controles). Os pacientes foram subdivididos de acordo com a proteinúria: proteinúria persistente ou remissão parcial (PP ≥ 300 mg/24 h, n = 17) e proteinúria baixa ou remissão completa (PB < 300 mg/24 h, n = 27). A análise ex vivo de leucócitos no sangue periférico por citometria de fluxo foi feita utilizando marcadores de superfície para linfócitos T, TCD4, TCD8, células natural killer (NK), linfócitos NKT e B. As frequências das citocinas intracelulares foram analisadas nessas células. Resultados A frequência dos linfócitos B, células NK e células NKT foi menor em pacientes com SNI do que nos controles, ao passo que os pacientes com SNI apresentaram maior frequência de células CD4+fator de necrose tumoral (TNF)-α+ do que nos controles. Os linfócitos T citotóxicos que expressam interferon (IFN)-γ foram menores nos pacientes com SNI do que nos controles. Os pacientes com PP mostraram maiores frequências de linfócitos T CD4 que expressam IFN-γ e TNF-α que os controles. Os linfócitos CD8 que expressam TNF-α apresentaram aumento no grupo com PP, em comparação aos com PB e os controles, apesar de as células CD8+IFN-γ+ serem mais baixas nos pacientes com PB e nos controles. Conclusão Com relação ao nível de proteinúria, os pacientes com SNI apresentaram aumento na expressão de TNF-α nos linfócitos CD4 e expressão reduzida de IFN-γ nos linfócitos CD8. A persistência da proteinúria foi associada a maiores níveis de marcadores inflamatórios.

Abstract Objective: There is evidence of an important role of immune system changes in the triggering and maintenance of idiopathic nephrotic syndrome (INS). The aim of this study was to investigate the expression of cytokines in lymphocyte populations of patients with INS in comparison to healthy individuals, according to proteinuria. Methods: This cross-sectional study included 44 patients with INS and eight healthy children, matched for age and sex (controls). Patients were subdivided according to proteinuria: persistent proteinuria or partial remission (PP ≥ 300 mg/24 h, n = 17) and low proteinuria or complete remission (LP < 300 mg/24 h, n = 27). Ex vivo analysis of peripheral blood leukocytes by flow cytometry was performed using surface markers for T-lymphocytes, TCD4, TCD8, natural killer (NK) cells, NKT, and B-lymphocytes. Frequencies of intracellular cytokines were analyzed in these cells. Results: The frequencies of B-lymphocytes, NK cells, and NKT cells were lower in INS than in controls, whereas INS patients had a higher frequency of CD4+tumor necrosis factor (TNF)-α+ cells than controls. Cytotoxic-T-lymphocytes expressing IFN-γ were lower in INS than in controls. Patients with PP showed higher frequencies of CD4-T-lymphocytes expressing IFN-γ and TNF-α than controls. CD8-lymphocytes expressing TNF-α were increased in PP group when compared with LP and controls, while CD8+interferon (IFN)-γ+ cells were lower than in LP and in controls. Conclusion: Regardless the level of proteinuria, INS patients had increased expression of TNF-α in CD4-lymphocytes and reduced expression of IFN-γ in CD8-lymphocytes. Persistence of proteinuria was associated with higher levels of inflammatory markers.

Objetivo Realizar uma análise descritiva do desempenho de pacientes encaminhados a um hospital de uma instituição de ensino público, para avaliação do processamento auditivo, e correlacionar os achados desta avaliação à idade, queixas, resultados e às avaliações auditivas comportamental e eletrofisiológica.Métodos O estudo incluiu 159 indivíduos encaminhados pelo sistema público de saúde para avaliação do processamento auditivo. Todos os participantes realizaram audiometria tonal liminar, medidas de imitância acústica, testes comportamentais do processamento auditivo e avaliação eletrofisiológica da audição.Resultados A principal queixa referida foi a de dificuldade de aprendizagem e os testes que avaliam processamento temporal e escuta dicótica foram os que apresentaram maior prevalência de alteração. Em todos os testes eletrofisiológicos, o número de resultados normais foi superior aos alterados. A proporção de indivíduos normais e alterados, nos testes comportamentais e eletrofisiológicos, não diferiu em relação ao gênero. Houve correlação fraca entre fechamento auditivo e potencial evocado auditivo de média latência da orelha direita; potencial evocado auditivo de média latência da orelha esquerda e potencial evocado auditivo de média latência total; ordenação temporal e efeito eletrodo direito e P300; processamento temporal e potencial evocado auditivo de média latência da orelha direita; escuta dicótica e P300 e entre interação binaural e reflexo acústico das orelhas direita e esquerda.Conclusão A dificuldade de aprendizagem prevaleceu sobre as queixas dos participantes e as habilidades de processamento temporal e escuta dicótica apresentaram maior prevalência de alteração. A maioria dos participantes foi encaminhada para a avaliação do processamento auditivo pelo fonoaudiólogo. Os testes eletrofisiológicos apresentaram correlação fraca com os testes comportamentais.

Purpose To perform a descriptive analysis of the performance of patients referred to a public educational institution to take part in the assessment of auditory processing and to correlate the assessment findings to the variables of age and hearing complaints, also to correlate results to behavioral and electrophysiological assessments.Methods The study included 159 individuals that were referred to the public health system to take part in an assessment of auditory processing. All participants underwent pure-tone audiometry, acoustic immittance measures, behavioral tests of auditory processing, and electrophysiological hearing assessment.Results The main complaint was learning disability and the tests that presented higher prevalence of abnormal results were temporal processing and dichotic listening tests. In all electrophysiological tests the number of normal results was higher than the altered ones. The proportion of normal and abnormal individuals in behavioral and electrophysiological tests did not differ in relation to gender. There was weak correlation between auditory closure and right ear middle latency responses; between left ear middle latency responses and total middle latency responses; among temporal ordering and electrode effect and P300; between temporal processing and right ear middle latency responses; between dichotic listening and P300 and also between binaural interaction and right and left ears acoustic reflexes.Conclusion The most frequent complaint among participants of this study was learning disability. Temporal processing and dichotic listening skills showed higher prevalence of alteration in the assessment. Most participants were referred to assessment of auditory processing by the speech-language pathologist. Correlation between behavioral and electrophysiological assessments was weak.

Por isso, se faz importante o estudo dos agravos em saúde que poderão se intensificar em decorrência desses eventos. Portanto, por meio deste artigo, propõe-se estimar o custo representado pelos casos de dengue atribuídos ao desastre de 2011, em Nova Friburgo (RJ). Verificou-se que ocorreram 1.356 casos suspeitos de dengue, dos quais 937 foram confirmados. O custo total da doença pode ter variado, no mínimo, entre R$ 66 mil e R$ 499 mil, no cenário de menor valor de salário, sendo que cerca de 70% desse custo ocorreu entre os casos confirmados. O desastre provocou grandes mudanças no ambiente do município que, junto com os problemas de saneamento e limpeza urbana, que ocorreram em períodos posteriores ao evento, acabou por aumentar a disponibilidade de locais que pudessem servir de criadouro do mosquito, facilitando sua proliferação e aumentando a incidência da dengue, doença que na década anterior ao desastre teve baixa incidência no município. Isso ilustra o potencial que eventos como ao ocorrido em 2011, em Nova Friburgo, podem ter no incremento desses casos e, consequentemente, nos custos da doença.

Therefore, a study of the health scourges that may intensify due to these events is important. The scope of this article is to estimate the cost represented by the cases of dengue fever attributed to the 2011 disaster in Nova Friburgo (RJ). There were 1,356 suspected cases of dengue fever, 937 of which were confirmed. The total cost of the disease may have been between R$66,000 and R$499,000 taking the minimum salary as a benchmark, with approximately 70% of this amount being among the confirmed cases. The disaster caused extensive changes in the city's environment which, together with the urban sanitation and clearing up process that occurred in the post-event period, led to an increase in the number of potential mosquito breeding sites, facilitating their proliferation and increasing the number of cases of dengue fever. This was a disease that in the decade prior to the disaster recorded few cases of the disease in the municipality. This illustrates the potential of events like the one that occurred in 2011 in Nova Friburgo may have on the increase in the number of cases, and consequently on the cost of the disease.

INTRODUÇÃO: Meningite bacteriana aguda (MBA) permanece um problema de saúde pública no Brasil. Para avaliar a epidemiologia da MBA atendida no Hospital Giselda Trigueiro, Rio Grande do Norte, um estudo retrospectivo-descritivo foi realizado de 2005 a 2008. MÉTODOS: Dados clínicos e laboratoriais foram coletados do departamento de epidemiologia hospitalar e analisados. RESULTADOS: Dos 168 casos de MBA, 24,4%, 10,7% e 2,4% foram, respectivamente, causados por Streptococcus pneumoniae, Neisseria meningitidis e Haemophilus influenzae b e 5,4% por outras bactérias. A média da idade foi 22,48 ± 18,7 anos. CONCLUSÕES: Streptococcus pneumoniae foi o principal patógeno causador na população urbana jovem.

INTRODUCTION: Acute bacterial meningitis (ABM) remains a public health problem in Brazil. To evaluate the epidemiology of ABM cases at Giselda Trigueiro Hospital, Rio Grande do Norte, a descriptive retrospective survey was conducted covering 2005 to 2008. METHODS: Clinical and laboratory data were collected from the epidemiology department of the hospital and analyzed. RESULTS: Out of 168 ABM cases, 24.4%, 10.7%, and 2.4% were, respectively, caused by Streptococcus pneumoniae, Neisseria meningitidis and Haemophilus influenza b, and 5.4% by other bacteria. The mean age was 22.48 ± 18.7 years old. CONCLUSIONS: Streptococcus pneumoniae was the main causative pathogen in the young urban population.

OBJETIVOS: Determinar a incidência de retinopatia da prematuridade e avaliar os principais fatores de risco implicados no seu desenvolvimento. MÉTODOS: Estudo coorte retrospectivo de base hospitalar realizado no período de janeiro de 2004 a dezembro de 2006, no Hospital Universitário Onofre Lopes, Natal (RN) - Brasil. A amostra foi composta por 663 recém-nascidos, com idade gestacional < 36 semanas e/ou peso ao nascimento < 1.500 g e submetidas ao protocolo de retinopatia da prematuridade existente no ambulatório de oftalmologia do hospital. As variáveis estudadas foram: gênero, peso ao nascimento, idade gestacional, tempo de oxigenioterapia, ventilação mecânica, sepse e transfusão sanguínea. Os dados foram analisados por meio dos testes do qui-quadrado, exato de Fisher e da regressão logística múltipla. RESULTADOS: Entre os 663 prontuários, retinopatia da prematuridade ocorreu em 414 casos (62,4%). Do total da amostra, 338 (51,0%) eram do sexo masculino e 282 (42,5%) do feminino. As médias e os desvios-padrão do peso, da idade gestacional e do tempo de oxigenioterapia foram, respectivamente, 1.334,9 ± 345,6 g, 31,9 ± 2,3 semanas e 10,0 ± 14,0 dias. A incidência de retinopatia em prematuros no período foi de 62,4%, com 58,0% dos casos em 2004, 67,2% em 2005 e 63,0% em 2006. A análise de regressão logística múltipla demonstrou que o peso <1.000 g (p<0,001; ORaj=17,18; IC= 6,52-45,29) e entre 1.000 g a 1.500 g (p=0,002; ORaj=4,20; IC= 1,68-10,48), o tempo de oxigenioterapia >20 dias (p=0,022; ORaj=3,40; IC= 1,19-9,69) e a transfusão sanguínea (p=0,022; ORaj=2,06; IC= 1,11-3,83) são fatores independentes de risco para a doença. CONCLUSÕES: O estudo demonstra uma alta incidência da patologia no serviço. O baixo peso ao nascer, um tempo prolongado de oxigenoterapia, bem como a transfusão sanguínea são fatores associados ao desenvolvimento da retinopatia da prematuridade. Idade gestacional não é um dado confiável para a triagem dos neonatos realizada pelo setor.

PURPOSES: To determine the incidence of retinopathy of prematurity and assess the main risk factors involved in its development. METHODS: Retrospective cohort study carried out from January 2004 to December 2006, at University Hospital Onofre Lopes, Natal (RN) - Brazil. The sample was composed of 663 newborns, with less than or equal to 36 weeks of gestational age and/or birth weight less than or equal to 1,500 g, submitted to the protocol of retinopathy of prematurity in the ophthalmology department of the hospital. The variables were: gender, birth weight, gestational age, duration of oxygen therapy, mechanical ventilation, sepsis and blood transfusion. Data were analyzed through the chi-squared test, Fisher's exact test and logistic regression model. RESULTS: Of the 663 cases, retinopathy of prematurity occurred in 414 (62.4%). Of the total sample, 338 (51.0%) were male and 282 (42.5%) female. Mean and standard deviation of weight, gestational age and duration of oxygen therapy were, respectively, 1,334.9 ± 345.6 g, 31.9 ± 2.3 weeks and 10.0 ± 14.0 days. The incidence of retinopathy in premature newborns in the period was 62.4%, 58.0% of cases in 2004, 67.2% in 2005 and 63.0% in 2006. The multiple logistic regression analysis showed that the weight less than 1,000 g (p<0.001; adjOR=17.18, CI= 6.52-45.29) and between 1,000 g and 1,500 g (p=0.002; adjOR -4.20 CI= 1.68-10.48), the time of oxygen lounger than 20 days (p=0.022; adjOR=3.40, CI= 1.19-9.69) and blood transfusion (p=0.022; adjOR -2.06, CI= 1.11-3.83) are independent factors for the disease. CONCLUSIONS: The study showed a high incidence of the disease in the service. A low birth weight, a prolonged oxygen therapy and blood transfusion are factors associated with the development of retinopathy of prematurity. Gestational age is not a reliable parameter for screening of newborns in the service.
