Abstract Background Sarcopenia is highly prevalent in heart failure (HF) patients, and the involvement of biomarkers in its pathophysiology is suggested, but little has been studied concerning HF sarcopenic patients. Objectives To evaluate the association between inflammatory and humoral markers with sarcopenia, as well as the impact of sarcopenia on quality of life and functional capacity in older HF patients. Methods In this cross-sectional study, 90 outpatient HF patients, aged ≥ 60 years, were evaluated for sarcopenia (EWGSOP2 diagnostic criteria), inflammation (high-sensitive C-reactive protein [hs-CRP], Interleukin-6 [IL-6], tumor necrosis factor alpha [TNF-α]) and humoral markers (total testosterone and insulin-like growth factor-1 [IGF-1]), physical activity (International Physical Activity Questionnaire), quality of life (Minnesota Living with Heart Failure Questionnaire), and functional capacity (6-minute walk test). The adopted level of significance was p<0.05. Results Patients had a mean age of 69.4 ± 7.2 years, 67.8% were male, with left ventricular ejection fraction (LVEF) of 35.9 ± 11.9% and 22 (24.4%) were sarcopenic. Age (73.1 ± 8.1 and 68.3 ± 6.5 years; p= 0.006), body mass index (BMI) (23.1 ± 2.8 and 28.2 ± 4.2 kg/m2; p <0.001), and LVEF (29.9 ± 8.8 and 37.9 ± 12.1%; p= 0.005) were different between groups with and without sarcopenia, respectively. After adjusting for age, ethnicity, BMI, LVEF, and the use of angiotensin converting enzyme inhibitors/angiotensin receptor blockers, sarcopenia was associated with higher serum levels of IL-6 and worse functional capacity. Conclusion In HF patients, sarcopenia was associated with IL-6 levels and functional capacity.

Resumo Fundamento Sarcopenia é altamente prevalente em pacientes com insuficiência cardíaca (IC), e o envolvimento de biomarcadores em sua fisiopatologia é sugerido, mas poucos estudos foram realizados em relação a pacientes sarcopênicos com IC. Objetivos Avaliar a associação entre marcadores inflamatórios e humorais e sarcopenia, bem como o impacto da sarcopenia na qualidade de vida e na capacidade funcional em pacientes mais velhos com IC. Métodos Neste estudo transversal, 90 pacientes ambulatoriais com IC, com idade ≥60 anos, foram avaliados quanto a sarcopenia (critérios diagnósticos EWGSOP2), inflamação (Proteína C reativa de alta sensibilidade [PCR-as], Interleucina-6 [IL-6], fator de necrose tumoral alfa [TNF-α]) e marcadores humorais (testosterona total e fator de crescimento semelhante à insulina tipo 1 [IGF-1]), atividade física (Questionário internacional de atividade física), qualidade de vida ( Minnesota Living with Heart Failure Questionnaire – Questionário Minnesota sobre conviver com a insuficiência cardíaca), e capacidade funcional (teste de caminhada de 6 minutos). O nível de significância estatística adotado foi p <0,05. Resultados Os pacientes tinham uma média de idade de 69,4 ± 7,2 anos, 67,8% eram do sexo masculino, com fração de ejeção ventricular esquerda (FEVE) de 35,9 ± 11,9% e 22 (24,4%) eram sarcopênicos. Idade (73,1 ± 8,1 e 68,3 ± 6,5 anos; p= 0,006), índice de massa corporal (IMC) (23,1 ± 2,8 e 28,2 ± 4,2 kg/m2; p <0,001), e FEVE (29,9 ± 8,8 e 37,9 ± 12,1%; p= 0,005) eram diferentes nos grupos com e sem sarcopenia, respectivamente. Depois de normalizar em relação à idade, etnia, IMC, FEVE, e o uso de inibidores da enzima conversora de angiotensina/bloqueadores de receptor de angiotensina, a sarcopenia foi associada a níveis séricos de IL-6 mais altos e capacidade funcional pior. Conclusão Em pacientes com IC, a sarcopenia foi associada aos níveis de IL-6 e à capacidade funcional.

Trata-se de estudo de métodos mistos, com estratégia incorporada concomitante, realizado com 134 nutricionistas que prestam assistência a estudantes com NAE. Utilizou-se questionário enviado por correio eletrônico como instrumento de coleta de dados, que foi avaliado por meio da análise de conteúdo temática e por estatística descritiva. Os resultados indicaram que o atestado ou laudo médico para oferta da alimentação especial foram os principais documentos comprobatórios; a licitação tipo pregão foi o meio de obtenção de alimentos especiais; os manipuladores de alimentos predominaram no recebimento de orientação referente às NAE; falta de produtos e fornecedores foram indicados como dificuldades; e garantia de atendimento destacada como facilidade na gestão das NAE. Quanto às características da gestão, destacam-se a ausência de solicitação de ficha técnica (79,9%), de oferta de alimentação especial (62%), de material informativo (92,5%) e da disponibilização de utensílios (63,4%). A assistência nutricional nos municípios catarinenses possui mecanismos de gestão diversos, que atendem às necessidades dos estudantes com NAE, mas ainda possui algumas fragilidades que dificultam a efetivação da assistência integral.

The nutritional assistance in cities has different management mechanisms, which meet the needs of students with SFN, but still have some weaknesses that hinder the implementation of comprehensive care.

Introduction: Sulforaphane (SFN) is an isothiocyanate derived from glucoraphanin (GRA), which is found in great amounts especially in broccoli. Its consumption has been reported to be associated with a lower risk of myocardial infarction and cancer development. Additionally, its effects have been studied in neurodegenerative diseases, diabetes, and atherosclerosis, most of the times using animal models and cell cultures. Objectives: Given the promising results of SFN, this review aimed to investigate evidence documented in human intervention studies with broccoli, GRA and SFN. Methods: A search was performed on PubMed and Virtual Health Library databases by two independent researchers using the descriptors “broccoli” or “glucora-phanin” or “sulforaphane”, which should appear on the study’s title or abstract. This review included randomized clinical trials performed in humans that were published in English and Portuguese from 2003 to 2013 and that considered clinical and molecular parameters of cell damage as outcomes of interest. Results: Seventeen studies were selected, and the predominant type of intervention was broccoli sprouts. More consistent results were obtained for the clinical parameters blood glucose and lipid profile and for molecular parameters of oxidative stress, indicating that there was an improvement in these parameters after intervention. Less solid evidence was found with regard to decreased inflammation, Helicobacter pylori colonization, and protection against cancer. Conclusion: Although being relevant, the evidence for the use of broccoli, GRA and SFN in humans are limited;thus, further intervention studies are needed to evaluate outcomes more consistently and reach better grounded conclusions.

Introducción: El sulforafano (SFN) es un isotiocianato derivado de la glucorafanina (GRA), encontrada en gran cantidad especialmente en el brócolis. Su consumo está asociado a un menor riesgo de infarto del miocardio y de cáncer. Además, sus efectos están siendo estudiados en enfermedades neurodegenerativas, diabetes y aterosclerosis, casi siempre utilizando modelos animales y cultivos celulares. Objetivos: Debido a los resultados prometedores del compuesto SFN, esta revisión buscó investigar evidencias ya documentadas en intervenciones con brócoli, GRA y SFN en humanos. Métodos: Se realizó una búsqueda en las bases de datos PubMed y Biblioteca Virtual en Salud, por dos investigadores independientes, utilizando los descriptores “broccoli” o “glucoraphanin” o “sulforaphane”, que debían constar en el título o resumen del trabajo. Se incluyeron ensayos clínicos randomizados realizados en humanos, publicados en inglés y portugués entre 2003 y 2013, y que consideraron como desenlaces de interés parámetros clínicos y moleculares de daño celular. Resultados: Se seleccionaron 17 estudios y el tipo de intervención predominante fueron brotes de brócoli. Los resultados más consistentes fueron obtenidos con los parámetros clínicos glicemia y perfil lipídico y los parámetros moleculares de estrés oxidativo, que presentaron mejora después de la intervención. Se encontraron evidencias menos sólidas respeto a la disminución de la inflamación, de la colonización por Helicobacter pylori y protección contra cáncer. Conclusión: Aunque relevantes, las evidencias del uso de brócoli, GRA y SFN en humanos son limitadas, siendo necesarios más estudios de intervención para avaluar los desenlaces de forma más consistente y producir conclusiones mejor fundamentadas.

Background: Nutritional factors have a significant influence on the prognosis of patients with heart failure (HF). Objective: The goal of the present study was to assess the food intake of stable patients with HF. Methods: Patients of both genders aged over 18 years with a confirmed diagnosis of HF were recruited and matched with healthy individuals for age, sex and BMI. Food records and weighing were used to assess participant nutritional intake. DRIs and NCEP-ATP III recommendations were used to evaluate the adequacy of nutritional intake. Results: Sixty-five percent of the 40 patients in the sample and 48% of the 25 control subjects were men. The mean age in both groups was 54±8 years and mean BMI was categorized as overweight. Carbohydrate, trans fatty acid and sodium intake were higher in the HF group as compared to control subjects (p=0.006, p<0.001 and p=0.029). A positive association was found between a diagnosis of HF and excess carbohydrate intake (p=0.038). Patients with HF were found to consume 130% of the recommended dietary allowance for trans fatty acids, and participants in both groups consumed only 50% of the recommended amounts of ω-3 and ω-6 fatty acids. Similar findings were obtained for calcium and potassium. Participants in both groups consumed only 5% of recommended daily vitamin D levels. Mean sodium intake was approximately 200% of the recommended dietary allowance, and was found to be significantly higher among patients in the HF group (p=0.042). Conclusion: The present study demonstrated an inadequate intake of macro- and micronutrients such as sodium, trans fatty acids, ω-3 and ω-6 fatty acids, carbohydrates, calcium, potassium and vitamin D in patients with HF.

Introducción: Factores nutricionales tienen una influencia significativa en el pronóstico de pacientes con insuficiencia cardíaca (IC). Objetivos: Evaluar la ingesta de alimentos de pacientes con IC estable. Métodos: Pacientes de ambos sexos, mayores de 18 años, con diagnóstico confirmado de IC fueron reclutados y emparejados por edad, sexo y IMC con individuos sanos. La ingesta nutricional fue evaluada mediante el registro alimentario y pesaje de acuerdo con las recomendaciones de las DRIs y NCEP-ATP III para la evaluación de la adecuación de la ingesta nutricional. Resultados: El 65% de los 40 pacientes con IC y el 48% de los 25 individuos control eran hombres. La edad media en los grupos fue de 54�±8 anos y los valores del IMC fueron indicativos de sobrepeso. La ingesta de carbohidratos, ácidos grasos trans y sodio fue mayor en el grupo IC que en el grupo control (p=0,006, p<0,001 y p=0,029). Se encontró una asociación positiva entre IC y consumo excesivo de carbohidratos (p=0,038). El grupo IC consumió el 130% de los valores diarios recomendados de ácidos grasos trans, y ambos grupos consumieron el 50% de la cantidad recomendada de ácidos grasos ω-3 y ω-6. Resultados similares se obtuvieron para el calcio y el potasio. La ingesta de vitamina D correspondió al 5% los valores recomendados en ambos grupos. La media de los niveles de ingesta de sodio representó aproximadamente el 200% de la cantidad recomendada y fue significativamente mayor entre los pacientes con IC (p=0,042). Conclusiones: El presente estudio demostró un consumo inadecuado de macro y micronutrientes, tales como sodio, ácidos grasos trans, ω-3 y ω-6, carbohidratos, calcio, potasio y vitamina D, en pacientes con IC.

OBJETIVO: Avaliar a adequação do manejo nutricional do paciente séptico a diretrizes de nutrição enteral para pacientes críticos. MÉTODOS: Estudo de coorte prospectivo com 92 pacientes sépticos, idade ≥18 anos, internados em unidade de terapia intensiva, em uso de nutrição enteral, avaliados segundo diretrizes para pacientes críticos quanto à nutrição enteral precoce, adequação calórica e proteica, e motivos para não início da nutrição enteral precoce bem como de interrupção da mesma. Escores prognósticos, tempo de internação, evolução clínica e estado nutricional também foram analisados. RESULTADOS: Pacientes com idade média de 63,4±15,1 anos, predominantemente masculinos, diagnóstico de choque séptico (56,5%), tempo de internação na unidade de terapia intensiva de 11 (7,2 a 18,0) dias, escores SOFA de 8,2±4,2 e APACHE II de 24,1±9,6 e mortalidade de 39,1%. Em 63% dos pacientes, a nutrição enteral foi iniciada precocemente. Cerca de 50% atingiu as metas calóricas e proteicas no 3º dia de internação na unidade de terapia intensiva, percentual que foi reduzido para 30% no 7º dia. Motivos para início da nutrição enteral tardia foram complicações do trato gastrintestinal (35,3%) e instabilidade hemodinâmica (32,3%). Procedimentos foram o motivo mais frequente para interrupção da nutrição enteral (44,1%). Não houve associação entre a adequação às diretrizes com estado nutricional, tempo de internação, gravidade ou evolução. CONCLUSÃO: Embora expressivo o número de pacientes sépticos que iniciaram a nutrição enteral precocemente, metas calóricas e proteicas no 3º dia da internação foram atingidas apenas pela metade destes, percentual que diminui no 7º dia.

OBJECTIVE: Evaluate the compliance of septic patients' nutritional management with enteral nutrition guidelines for critically ill patients. METHODS: Prospective cohort study with 92 septic patients, age ≥18 years, hospitalized in an intensive care unit, under enteral nutrition, evaluated according to enteral nutrition guidelines for critically ill patients, compliance with caloric and protein goals, and reasons for not starting enteral nutrition early or for discontinuing it. Prognostic scores, length of intensive care unit stay, clinical progression, and nutritional status were also analyzed. RESULTS: The patients had a mean age of 63.4±15.1 years, were predominantly male, were diagnosed predominantly with septic shock (56.5%), had a mean intensive care unit stay of 11 (7.2 to 18.0) days, had 8.2±4.2 SOFA and 24.1±9.6 APACHE II scores, and had 39.1% mortality. Enteral nutrition was initiated early in 63% of patients. Approximately 50% met the caloric and protein goals on the third day of intensive care unit stay, a percentage that decreased to 30% at day 7. Reasons for the late start of enteral nutrition included gastrointestinal tract complications (35.3%) and hemodynamic instability (32.3%). Clinical procedures were the most frequent reason to discontinue enteral nutrition (44.1%). There was no association between compliance with the guidelines and nutritional status, length of intensive care unit stay, severity, or progression. CONCLUSION: Although the number of septic patients under early enteral nutrition was significant, caloric and protein goals at day 3 of intensive care unit stay were met by only half of them, a percentage that decreased at day 7.

OBJETIVO: Identificar valores de ângulo de fase em pacientes sépticos, por meio de bioimpedância elétrica, buscando associação com variáveis clínicas e bioquímicas, bem como comparação com valores de ângulo de fase de referência. MÉTODOS: Estudo de coorte, com 50 pacientes sépticos, idade ≥18 anos, internados em unidade de terapia intensiva, avaliados quanto a índices prognósticos (APACHE II e SOFA), evolução clínica (mortalidade, gravidade da sepse e tempo de internação na unidade de terapia intensiva), parâmetros bioquímicos (albumina e proteína C-reativa) e ângulo de fase. RESULTADOS: A média de idade dos pacientes estudados foi de 65,6±16,5 anos, a maioria do gênero masculino (58%) e apresentando choque séptico (60%). A média dos escores APACHE II e SOFA foi de 22,98±7,1 e 7,5±3,4, respectivamente, o tempo de internação na unidade de terapia intensiva dos pacientes que sobreviveram foi de 9 dias (5 a 13) e a taxa de mortalidade foi de 30%. A média do ângulo de fase da amostra total foi de 5,4±2,6° e menor no gênero feminino (p=0,01). Não houve associação entre ângulo de fase e a gravidade da sepse, mortalidade, gênero e idade, assim como não houve correlação entre ângulo de fase, tempo de internação e parâmetros bioquímicos. Comparativamente a dados em população saudável, os valores de ângulo de fase, a depender da idade e gênero, apresentaram-se 1,1 a 1,9 vezes inferiores. CONCLUSÃO: O ângulo de fase médio de pacientes sépticos foi inferior aos valores referência para população saudável, não havendo correlação e associação com as variáveis clínicas e bioquímicas, o que poderia ser atribuído a homogeneidade da amostra.

OBJECTIVE: To calculate the values of the phase angle of septic patients using bioelectrical impedance analysis, correlate the values with clinical and biochemical variables, and compare them to reference values. METHODS: Cohort study conducted with 50 septic patients aged ≥18 years old, admitted to intensive care units, and assessed according to prognostic indexes (APACHE II and SOFA), clinical progression (mortality, severity of sepsis, length of stay in intensive care unit), biochemical parameters (albumin and C-reactive protein), and the phase angle. RESULTS: The average age of the sample was 65.6±16.5 years. Most patients were male (58%) and suffering from septic shock (60%). The average APACHE II and SOFA scores were 22.98±7.1 and 7.5±3.4, respectively. The patients who survived stayed nine days on average (five to 13) in the intensive care unit, and the mortality rate was 30%. The average value of the phase angle was 5.4±2.6° in the total sample and was smaller among the females compared with the males (p=0.01). The phase angle measures did not exhibit an association with the severity of the sepsis, mortality, gender, and age or correlate with the length of hospitalization or the biochemical parameters. The participants' phase angle values adjusted per gender and age were 1.1 to 1.9 times lower compared with the values for a normal population. CONCLUSION: The average value of the phase angle of septic patients was lower compared with the reference values for a healthy population. The phase angle measures did not exhibit association with the clinical and biochemical variables, which might be explained by the sample homogeneity.
