Abstract Objective To evaluate clinical characteristics and outcomes of COVID-19 patients infected with HIV, and to compare with a paired sample without HIV infection. Methods This is a substudy of a Brazilian multicentric cohort that comprised two periods (2020 and 2021). Data was obtained through the retrospective review of medical records. Primary outcomes were admission to the intensive care unit, invasive mechanical ventilation, and death. Patients with HIV and controls were matched for age, sex, number of comorbidities, and hospital of origin using the technique of propensity score matching (up to 4:1). They were compared using the Chi-Square or Fisher's Exact tests for categorical variables and the Wilcoxon for numerical variables. Results Throughout the study, 17,101 COVID-19 patients were hospitalized, and 130 (0.76%) of those were infected with HIV. The median age was 54 (IQR: 43.0;64.0) years in 2020 and 53 (IQR: 46.0;63.5) years in 2021, with a predominance of females in both periods. People Living with HIV (PLHIV) and their controls showed similar prevalence for admission to the ICU and invasive mechanical ventilation requirement in the two periods, with no significant differences. In 2020, in-hospital mortality was higher in the PLHIV compared to the controls (27.9% vs. 17.7%; p = 0.049), but there was no difference in mortality between groups in 2021 (25.0% vs. 25.1%; p > 0.999). Conclusions Our results reiterate that PLHIV were at higher risk of COVID-19 mortality in the early stages of the pandemic, however, this finding did not sustain in 2021, when the mortality rate is similar to the control group.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

ABSTRACT The objective was to determine the optimal dehydrated passion fruit pulp replacing ground corn in diets for goat kids. We used 24 crossbred castrated goat kids with initial body weight of 26.6±0.3 kg. The experimental period was 23 days, with 15 days of adjustment and acclimatization (mostly to the installation and diet). Kids were distributed in a randomized experimental design with four treatments of 0, 80, 160, or 240 g/kg of dehydrated passion fruit residue as a supplementary inclusion in the dry matter (DM) content of the diet. The statistical model considered the fixed effect of the included amount of residue from dehydrated passion fruit. Analysis of variance was conducted using the PROC GLM of SAS. Intakes of DM and total digestible nutrients (TDN) were not affected by residue inclusion. However, neutral detergent fiber (NDF) intake increased linearly. The inclusion linearly decreased intake of ether extract (EE) and non-fibrous carbohydrates (NFC). Crude protein (CP) intake was quadratically affected, with a greater value under treatment of 160 g/kg passion fruit. Dry matter and NFC digestibility decreased linearly with the amount of passion fruit included in diet. Fecal N and total N excretion values (in g/d and %N intake) increased linearly. Nitrogen balance and N retained values showed a quadratic increase, with greater values under the 160 g/kg inclusion. Time spent ruminating, DM and NDF eating efficiency, and g DM/bolus chewed were affected quadratically by inclusion of passion fruit residue. The replacement of ground corn with dehydrated passion fruit residue at a level of 160 g/kg is recommended for diets of goat kids to improve their NDF and CP intake and their N balance without affecting DM and TDN intakes.

RESUMO A produção de mudas de tomateiro é conduzida em substratos comerciais com propriedades adequadas para a boa formação da parte aérea e da raiz. O composto pós-cultivo de cogumelos (“Spent Mushroom Substrate” ou SMS) apresenta algumas vantagens em relação aos substratos comerciais para a qualidade das mudas de hortaliças, que podem ser proporcionados pela presença de uma rica microbiota, trazendo maior equilíbrio e maior competição com microrganismos patogênicos, além do controle biológico de patógenos e nematoides. É importante conhecer a microbiota presente nesse material e sua relação com a planta, para que essa associação possa ser feita da melhor maneira possível. Este trabalho teve como objetivo identificar a microbiota presente na rizosfera de mudas de tomate produzidas em SMS de cogumelos das espécies Agaricus subrufescens e Agaricus bisporus, acrescidos ou não de substrato comercial. A microbiota foi analisada por DGGE e amostras representativas de DNA foram sequenciadas para identificação das espécies. Entre os eucariotos, foram encontradas espécies de Chaetomium globosum, Arthrobotrys amerospora, predominantes no SMS de A. subrufescens, e Geastrum floriforme, Chrysosporium chiropterum, Pleurotus sp., e Trichosporon sp. no SMS de A. bisporus. Os procariontes apresentam maior diversidade, sugerindo um grande número de espécies ainda não classificadas ou cujas sequências ainda não estão disponíveis.
ABSTRACT The production of tomato seedlings is conducted on commercial substrates with adequate properties for the good formation of the aerial part and root. The Spent Mushroom Substrate, or SMS, presents advantages over commercial substrates regarding the quality of the vegetable seedlings, which may be provided by the presence of a rich microbiota, bringing higher balance and competition with pathogenic microorganisms, in addition to the biological control of pathogens and nematodes. It is important to know the microbiota present in this material and its relation to the plant, in order for this association to occur in the best manner possible. This work had the objective of identifying the microbiota present in the rhizosphere of tomato seedlings produced in SMS of Agaricus subrufescens and Agaricus bisporus mushrooms, added or not with commercial substrate. The microbiota was analyzed by DGGE and the representative samples were sequenced in order to identify the species. Among the eukaryotes, the Chaetomium globosum, Arthrobotrys amerospora species were predominant in the A. subrufescens SMS, and Geastrum floriforme, Chrysosporium chiropterum, Pleurotus sp., and Trichosporon sp. in the A. bisporus SMS. Prokaryotes are the most diverse group, suggesting a large number of species not yet classified or which sequences are not yet available.

Seasonal variation in the reproductive activity of male goats raised under tropical climate conditions

ABSTRACT This study analyzed seasonal variations in the testes, the concentration of sex hormones, the parameters of fresh and thawed semen, and the sexual behavior of male Alpine goats from a temperate region in a tropical climate and possible interference with fertility. The maximum and minimum temperature and luminosity were recorded daily, while seminal, hormonal, and behavioral assessments were carried out every fortnight. The maximum and minimum temperature (°C) and luminosity (h) were recorded daily always at 17.00 h. The scrotal circumference (cm), testicular volume (mL), volume (mL), appearance (creamy, milky, aqueous) and coloration (white, white-yellowish, and yellowish) seminal, turbulence or mass movement (0 to 5), progressive spermatic motility (0 to 100%), spermatic force (0 to 5), concentration (spermatozoids/mL), spermatic pathologies, hypoosmotic test (%), serum levels of FSH (mUI/mL), LH (mUI/mL), testosterone (ng/mL), and sexual behaviors were carried out every fortnight. There was a difference between the scrotal circumference evaluated monthly, testicular volume, volume and concentration of fresh semen, sperm vigor of the thawed semen, serum levels of testosterone, FSH and LH, and some sexual behaviors. Thus, the changes that occur in the quantity and quality of sperm, in the hormonal profile, and in sexual behaviors should not be regarded as an impediment to the use of male Alpine goats in tropical climates throughout the year. These variations do not lead to changes in the semen that may compromise the fertility of these animals.

Femtosecond-assisted intrastromal corneal ring implantation for keratoconus treatment: a comparison with crosslinking combination

Objetivo: Comparar os resultados visuais, astigmatismo corneano e ceratometria em pacientes com ceratocone submetidos a implante de anel corneano intraestromal (ICRSI) e quando em combinação com radiação ultravioleta associado ao crosslinking do colágeno corneano mediada pela riboflavina (CXL). Métodos: Comparou-se retrospectivamente pacientes com ceratocone submetidos somente a implante de anel corneano intraestromal (grupo 1) versus o mesmo procedimento associado ao crosslinking em um período de 2 anos. Avaliou-se acuidade visual com correção, equivalente esférico, ápice do cone na topografia e adaptação com lentes de contato pré e pós operatórios. Resultados: O estudo avaliou 32 olhos de 31 pacientes. Em 10 casos (31%) foi realizado crosslinking corneano, não havendo complicações ou necessidade de reposicionamento do anel. Acuidade visual corrigida pré e pós-operatória, componentes esférico e cilíndrico da refração e valores de ceratometria media diminuíram significativamente em ambos os grupos. Após o implante, nenhum paciente apresentou acuidade visual pior que 20/60 e 78% apresentaram acuidade corrigida melhor ou igual a 20/40 (72% do grupo 1 e 90% do grupo 2). Observou-se diminuição no valor do equivalente esférico no grupo 1 (de -5,89 ± 3,37 pré-operatório para -2,65 ± 2,65 pós-operatório; p<0,05) e no grupo 2 (de -6,91 ± 1,93 pré-operatório para -2,11 ± 3,01 pós-operatório; p<0,05). Conclusão: Ambas as técnicas podem ser consideradas seguras e eficazes na melhora da acuidade visual e equivalente esférico, diminuição do ápice de curvatura do cone na análise topográfica e na redução de dioptrias a serem corrigidas no pós-operatório de pacientes com ceratocone.
Purpose: To compare visual outcomes, corneal astigmatism, and keratometric readings in patients with keratoconus who underwent intrastromal corneal ring implantation (ICRSI) alone with those who underwent ICRSI combined with ultraviolet A riboflavin-mediated corneal collagen crosslinking (CXL). Methods: Pre- and post-operative best-corrected distance visual acuity (BCDVA), spherical error, cylindrical error, and mean keratometry were retrospectively compared over a period of 2 years in patients with keratoconus who underwent only ICRSI (group 1) versus those in patients who underwent combined ICRSI-CXL (group 2). Results: Thirty-two eyes of 31 patients were evaluated. CXL was performed in 10 cases (31%), and there were no complications or need for ring repositioning. BCDVA improved from 0.54 to 0.18 in the group 1 and from 0.56 to 0.17 in the group 2. Spherical and cylindrical errors and mean keratometry values significantly decreased in both groups. No patient postoperatively had visual acuity (VA) of less than 20/60 on refraction, and 78% exhibited VA better than or equal to 20/40 with spectacles (72% of group 1 and 90% of group 2). Improvement in the spherical equivalent (SE) value was observed in the group 1 (from -5.89 ± 3.37 preoperatively to -2.65 ± 2.65 postoperatively; p<0.05) and group 2 (from -6.91 ± 1.93 preoperatively to -2.11 ± 3.01 postoperatively; p<0.05). Conclusion: Both techniques can be considered safe and effective in improving VA and refractive SE values, in decreasing the curvature of the cone apex in the topographical analysis, and in decreasing corrected diopters postoperatively in patients with keratoconus.

Dengue é atualmente um importante problema de saúde pública. O vírus da dengue (DENV) é classificado em quatro sorotipos distintos, DENV 1-4. Após 28 anos de ausência, o DENV-4 foi detectado novamente no Brasil em 2010 no Estado de Roraima, e um ano depois, o vírus foi identificado em outros estados do norte do país, Amazonas e Pará, seguido pelos estados do Rio de Janeiro e São Paulo. Em Minas Gerais, o primeiro caso confirmado de DENV-4 ocorreu no município de Frutal em 2011 e, desde então, o sorotipo foi isolado em um número crescente de pacientes. Apesar do DENV-2 estar associado a um maior risco de formas graves e morte, o DENV-4 também tem sido associado a casos graves e a risco aumentado de manifestações hemorrágicas. Neste relato, descrevemos o primeiro caso fatal confirmado por DENV-4 no Brasil. A paciente era uma menina de 11 anos do município de Montes Claros, no norte de Minas Gerais, Brasil. Apresentava púrpura trombocitopênica idiopática e evoluiu de forma fulminante durante a infecção por dengue, com óbito associado a complicações hemorrágicas. O diagnóstico foi confirmado pela detecção de anticorpos IgM específicos para dengue, por método imunoenzimático, e por semi-nested RT-PCR. Médicos e outros profissionais de saúde devem estar cientes que infecções por DENV-4 também podem resultar em formas graves da doença com complicações hemorrágicas e óbito, principalmente em pacientes com comorbidades.
Dengue is currently a major public-health problem. Dengue virus (DENV) is classified into four distinct serotypes, DENV 1-4. After 28 years of absence, DENV-4 was again detected in Brazil in 2010 in Roraima State, and one year later, the virus was identified in the northern Brazilian states of Amazonas and Pará, followed by Rio de Janeiro and São Paulo. In Minas Gerais, the first confirmed case of DENV-4 occurred in the municipality of Frutal in 2011 and has now been isolated from a growing number of patients. Although DENV-2 is associated with the highest risk of severe forms of the disease and death due to the infection, DENV-4 has also been associated with severe forms of the disease and an increasing risk of hemorrhagic manifestations. Herein, the first fatal case of confirmed DENV-4 in Brazil is reported. The patient was an 11-year-old girl from the municipality of Montes Claros in northern Minas Gerais State, Brazil. She had idiopathic thrombocytopenic purpura as a comorbid condition and presented with a fulminant course of infection, leading to death due to hemorrhagic complications. Diagnosis was confirmed by detection of Dengue-specific antibodies using IgM capture enzyme-linked immunosorbent assay and semi-nested RT-PCR. Primary care physicians and other health-care providers should bear in mind that DENV-4 can also result in severe forms of the disease and lead to hemorrhagic complications and death, mainly when dengue infection is associated with coexisting conditions.

OBJETIVO: avaliar os aspectos epidemiológicos, conduta, morbidade e resultados do tratamento trauma hepático. MÉTODOS: estudo retrospectivo de doentes com mais de 13 anos de idade admitidos em um hospital universitário de 1990 a 2010, submetidos ao tratamento cirúrgico ou não operatório (TNO). RESULTADOS: foram admitidos 748 pacientes com trauma hepático. O mecanismo de trauma mais frequente foi o trauma penetrante (461 casos; 61,6%). O trauma fechado ocorreu em 287 pacientes (38,4%). De acordo com o grau de lesão hepática (AAST-OIS), no trauma fechado foi observada uma predominância dos graus I e II e no trauma penetrante, uma predominância do grau III. O TNO foi realizado em 25,7% dos pacientes com trauma hepático contuso. Entre os procedimentos cirúrgicos, a sutura foi realizada com maior frequência (41,2%). A morbidade relacionada ao fígado foi 16,7%. A taxa de sobrevida para pacientes com trauma hepático fechado foi 73,5% e no trauma penetrante de 84,2%. A mortalidade no trauma complexo foi 45,9%. CONCLUSÃO: o trauma permanece mais incidente nas populações mais jovens e no sexo masculino. Houve uma redução dos traumas hepáticos penetrantes. O TNO se mostrou seguro e efetivo, e, frequentemente, foi empregado para tratar os pacientes com trauma hepático penetrante. A morbidade foi elevada e a mortalidade foi maior em vítimas de traumas contusos e em lesões hepáticas complexas.
OBJECTIVE: To evaluate the epidemiological aspects, behavior, morbidity and treatment outcomes for liver trauma. METHODS: We conducted a retrospective study of patients over 13 years of age admitted to a university hospital from 1990 to 2010, submitted to surgery or nonoperative management (NOM). RESULTS: 748 patients were admitted with liver trauma. The most common mechanism of injury was penetrating trauma (461 cases, 61.6%), blunt trauma occurring in 287 patients (38.4%). According to the degree of liver injury (AAST-OIS) in blunt trauma we predominantly observed Grades I and II and in penetrating trauma, Grade III. NOM was performed in 25.7% of patients with blunt injury. As for surgical procedures, suturing was performed more frequently (41.2%). The liver-related morbidity was 16.7%. The survival rate for patients with liver trauma was 73.5% for blunt and 84.2% for penetrating trauma. Mortality in complex trauma was 45.9%. CONCLUSION: trauma remains more common in younger populations and in males. There was a reduction of penetrating liver trauma. NOM proved safe and effective, and often has been used to treat patients with penetrating liver trauma. Morbidity was high and mortality was higher in victims of blunt trauma and complex liver injuries.
