Abstract There is scarcity of global data on suicide attempts (SA). The higher frequency of mental disorders places women at a higher risk for SA. Mental disorders (MD) are ranked first in the Global Burden of Diseases (GBD) in terms of years lived with disability (YLD) and common mental disorders (CMD) affect women the most. IN this light, the present study aimed to investigate the impact of CMD during pregnancy, as well as 6 to 9 years after birth, on SA in women who were registered in the Family Health Strategy from the town of Recife, Pernambuco, Brazil. This study consisted of two stages of a prospective cohort. 643 adult women were included. The prevalence of CMD in stage I (pregnancy) and in stage III (six to nine years after birth) were, respectively, 19.3% and 12.6%. The incidence of SA was 10.9%. The multivariate analysis demonstrated a cumulative effect of CMD for SA: only during pregnancy (OR 5.4; 95%CI 2.2-13.3); only in the third stage (OR 5.8; 95%CI 2.3-14.9); and in both stages (OR 6.0; 95%CI 2.5-14.4). The increase of CMD in women increases the chances of SA, hence the importance of implementing public policies for women’s health, especially for those with a history of mental disorders and unhealthy habits, and those who suffer violence.

Resumo Há escassez de dados globais sobre as tentativas de suicídio (TS). A maior frequência de adoecimento mental coloca as mulheres em maior risco de TS. Os transtornos mentais (TM) estão em primeiro lugar na carga global de doenças em termos de anos vividos com incapacidade (AVI). Entre os problemas de saúde mental que mais acometem mulheres estão os transtornos mentais comuns (TMC). O objetivo desta pesquisa foi investigar o impacto dos TMC na gravidez e seis a nove anos após o parto para a TS em mulheres cadastradas na Estratégia de Saúde da Família no Recife, Pernambuco, Brasil. O estudo engloba duas etapas de uma coorte prospectiva. Foram incluídas 643 mulheres adultas. A prevalência de TMC na etapa I (gestação) e sua incidência na etapa III (seis a nove anos após o parto) foram, respectivamente, 19,3% e 12,6%. A incidência da TS foi de 10,9%. A análise multivariada demonstrou efeito cumulativo dos TMC para a TS: TMC só na gestação (OR 5,4; IC95% 2,2-13,3); só na terceira etapa (OR 5,8; IC95% 2,3-14,9); e em ambas (OR 6,0; IC95% 2,5-14,4). O acúmulo dos TMC em mulheres aumenta a chance de TS, sendo importante a implementação de políticas públicas para a saúde das mulheres, principalmente com histórico de doença mental, hábitos não saudáveis e que sofrem violência.

Objective: to map the instruments for risk assessment of pressure ulcers in adults in critical situation in intensive care units; identify performance indicators of the instrument, and the appreciation of users regarding the instruments’ use/limitations. Method: a scoping review. We used the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews in the writing of the study. We carried out the searches in the EBSCOhost search tool for 8 databases, resulting in 1846 studies, of which 22 studies compose the sample. Results: we identified two big instrument groups: generalist [Braden, Braden (ALB), Emina, Norton-MI, RAPS, and Waterlow]; and specific (CALCULATE, Cubbin & Jackson, EVARUCI, RAPS-ICU, Song & Choi, Suriaidi and Sanada, and COMHON index). Regarding the predictive value, EVARUCI and CALCULATE presented better results for performance indicators. Concerning appreciation/limitations indicated by users, we highlight the CALCULATE scale, followed by EVARUCI and RAPS-ICU, although they still need future adjustments. Conclusion: the mapping of the literature showed that the evidence is sufficient to indicate one or more instruments for the risk assessment of pressure ulcers for adults in critical situation in intensive care units.

Objetivo: mapear los instrumentos para la evaluación del riesgo de lesiones por presión en adultos en situación crítica en una unidad de terapia intensiva; identificar los indicadores de desempeño de los instrumentos y la apreciación de los usuarios con respecto al uso/limitaciones de los instrumentos. Método: scoping review. Para redactar el estudio se utilizó la extensión Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews. La investigación se realizó mediante la herramienta de búsqueda EBSCOhost en 8 bases de datos, resultando 1846 estudios, de los cuales 22 conforman la muestra. Resultados: se identificaron dos grandes grupos de instrumentos: los generalistas [Braden, Braden (ALB), Emina, Norton-MI, RAPS y Waterlow]; y los específicos (CALCULATE, Cubbin & Jackson, EVARUCI, RAPS-ICU, Song & Choi, Suriaidi y Sanada y el índice COMHON). En cuanto al valor predictivo, EVARUCI y CALCULATE mostraron los mejores resultados de indicadores de desempeño. En cuanto a las apreciaciones/limitaciones señaladas por los usuarios, destaca la escala CALCULATE, seguida de la EVARUCI y la RAPS-ICU, aunque aún necesitan ajustes futuros. Conclusión: el mapeo mostró que las evidencias son suficientes para indicar uno o más instrumentos para la evaluación del riesgo de lesiones por presión en adultos críticos en una unidad de cuidados intensivos.

Resumo Introdução: A horizontalização do cuidado é uma forma de organização do trabalho em saúde, na qual existe uma equipe multiprofissional de referência que atua diariamente no serviço, em contraposição à forma de organização do trabalho em que profissionais têm uma carga horária distribuída por plantão. Objetivos: Compreender o processo de trabalho relacionado às atribuições do enfermeiro rotineiro na equipe multiprofissional. Métodos: Trata-se de um estudo descritivo, exploratório de cunho qualitativo, realizada com 48 profissionais de um hospital público no Distrito Federal, Brasil. A coleta de dados ocorreu no período de 10 de outubro a 30 de novembro de 2022. Os dados foram analisados por meio da análise de conteúdo de Bardin: pré-análise, exploração do material e tratamento dos resultados, inferência e interpretação. Resultados: Foi possível encontrar três núcleos de sentido que foram divididos em três categorias: 1- Inexistência de sistematização do trabalho do rotineiro; 2- Processo de trabalho na percepção da equipe; e 3- Necessidades de aprendizagem. Conclusões: Verificou-se que o papel do enfermeiro horizontal não está devidamente esclarecido, necessitando, portanto, de elaboração de protocolos institucionais que apresentem as atividades a serem desenvolvidas pela horizontal e pelo plantonista. No intuito de alinhar as condutas no cenário hospitalar, sugere-se ampla capacitação da equipe multiprofissional com abordagem no cuidado horizontal, preconizado pela Política Nacional de Atenção Hospitalar.

Abstract Introduction: The horizontalization of care is a form of organization of work in health, in which there is a multidisciplinary team of reference that works daily in the service, as opposed to the form of organization of work in which professionals have a workload distributed per shift. Objectives: Understanding the work process related to the duties of the horizontal nurse in the multidisciplinary team. Methods: This is a descriptive, exploratory qualitative study, carried out with 48 professionals from a public hospital. Data collection took place from October 10 to November 30, 2022. Data were analyzed using Bardin’s content analysis: pre-analysis, material exploration and treatment of results, inference and interpretation. Results: It was possible to find three nuclei of meaning that were divided into three categories: 1- Lack of routine work systematization; 2- Work process in the team's perception; and 3- Learning needs. Conclusions: It was verified that the role of the horizontal nurse is not properly clarified, therefore requiring the elaboration of institutional protocols that present the activities to be developed by the horizontal nurse and by the on-duty nurse. In order to align conducts in the hospital scenario, it is suggested extensive training of the multidisciplinary team with a horizontal care approach, recommended by the National Policy for Hospital Care.

Abstract Objective: To investigate the association between emotional and behavioral difficulties in schoolchildren and maternal educational practices. Methods: Participants and Setting: 631 mother-child pairs were evaluated in a poor urban district in Recife, northeastern Brazil. Cross-sectional study carried out between 2013 and 2014. It integrates a prospective cohort study designed to investigate the consequences of intimate partner violence that occurred during pregnancy, postpartum, and seven years after birth for the physical and mental health of women and their children. Maternal educational practices were assessed using the Parent-Child Conflict Tactics Scale (CTSPC – mother-child version) and the child’s behavioral and emotional disorders through the Strengths and Difficulties Questionnaire (SDQ) mother-version. The association was estimated through crude and adjusted prevalence ratios, using Poisson regression considering the 95% confidence interval. Results: The prevalence of violent maternal educational practices was 91.8% (572/631) being 89.7% (566/631) of psychological aggression and 75.6% of physical aggression, subdivided into corporal punishment (73.5%), physical maltreatment (35.8%) and severe physical maltreatment (1.7%). Corporal punishment (Adjusted PR 1.5; CI 95%: 1.1–2.1; p = 0.010) and severe physical maltreatment (Adjusted PR 1.9; CI 95%: 1.3–2.8; p = 0.002) were associated with emotional and behavioral difficulties in schoolchildren. Conclusion: The high prevalence of violent maternal educational practices, especially corporal punishment and severe physical maltreatment is associated with emotional and behavioral disorders in children. Therefore, it is necessary to interventions that promote parental support and effective use of non-violent discipline in conducting the educational process to establish healthier family relationships and to prevent/mitigate the impact of emotional and behavioral problems in children. © 2022 Sociedade Brasileira de Pediatria. RESUMO O artigo analisa aspectos da mudança da natureza jurídica de instituições privadas de assistência à saúde, de estabelecimentos “com” para “sem” fins lucrativos. Trata-se de uma pesquisa exploratória, apoiada no referencial de análise de políticas, com foco em dados secundários, provenientes do Sistema de Cadastro Nacional de Estabelecimentos de Saúde (SCNES), de 2012 a 2020, e estudo de caso. Os resultados apresentam aumento dessas entidades em todas as regiões do país e evidências de que se comportam como estabelecimentos com fins lucrativos. A mudança de natureza jurídica oculta um processo mais amplo de mercantilização implícita dos serviços de saúde, incentivado por políticas estatais e relacionado às isenções previstas em lei.

ABSTRACT The article analyzes aspects of the change in the legal nature of private healthcare from “for-profit” to “non-profit” entities. It is an exploratory research, supported by the policy analysis framework, focusing on secondary data from the Cadastro Nacional de Estabelecimentos de Saúde (National Registry of Health Facilities – CNES) from 2012 to 2020 and a case study. The results show an increase in these entities in all regions of the country and evidence that they behave like profit-oriented entities. The change in legal nature hides a broader process of implicit commodification of healthcare services, encouraged by state policies and related to exemptions provided by law.

Abstract The University Pharmacy Program (FU), from the Federal University of Rio de Janeiro (UFRJ), was created based on the need to offer a curricular internship to students of the Undergraduate Course at the Faculty of Pharmacy. Currently, it is responsible for the care of about 200 patients/day, offering vacancies for curricular internships for students in the Pharmacy course, it has become a reference in the manipulation of many drugs neglected by the pharmaceutical industry and provides access to medicines for low-income users playing an important social function. Research is one of the pillars of FU-UFRJ and several master and doctoral students use the FU research laboratory in the development of dissertations and theses. As of 2002, the Pharmaceutical Care extension projects started to guarantee a rational and safe pharmacotherapy for the medicine users. From its beginning in 1982 until the current quarantine due to the COVID-19 pandemic, FU-UFRJ has been adapting to the new reality and continued to provide patient care services, maintaining its teaching, research, and extension activities. The FU plays a relevant social role in guaranteeing the low-income population access to special and neglected medicines, and to pharmaceutical and education services in health promotion.

Abstract Hydrogels are used for wound treatment, as they may contain one or more active components and protect the wound bed. Papain is one of the active substances that have been used with this purpose, alongside urea. In this paper, carboxypolymethylene hydrogels containing papain (2% and 10% concentrations) and urea (5% concentration) were produced. Physical-chemical stability was performed at 0, 7, 15 and 30 days at 2-8ºC, 25ºC and 40ºC, as well as the rheological aspects and proteolytic activity of papain by gel electrophoresis. Clinical efficacy of the formulations in patients with lower limb ulcers was also evaluated in a prospective, single-center, randomized, double-blind and comparative clinical trial. The results showed 7-day stability for the formulations under 25ºC, in addition to approximately 100% and 15% of protein activity for 10% and 2% papain hydrogel, respectively. The rheological profile was non-Newtonian for the 10% papain hydrogel tested. There were no significant differences regarding the mean time for healing of the lesions, although 10% papain presented a better approach to be used in all types of tissue present in the wound bed.

Abstract The objective of this paper was to develop and evaluate two semi-solid pharmaceutical forms containing 0.1% tacrolimus: cream (CRT01) and gel (GLT01). For the evaluation of physicochemical stability, at times 0, 30, 60 and 90 days, at 23°C and at 40°C, High Performance Liquid Chromatography coupled with a Diode Array Detector (HPLC-DAD) was employed. This method was developed and validated for tacrolimus quantification. The occlusivity test and skin permeation assay were also performed, using an animal model (Wistar rats), and the CRT01 and GLT01 were compared to the 0.1% tacrolimus ointment (PFU01) obtained from the University Pharmacy, Federal University of Rio de Janeiro, Brazil. CRT01 and GLT01 presented a homogeneous aspect and consistency adequate for topical products, along with sensory characteristics above PFU01. They also presented adequate physicochemical stability for 90 days and a lower occlusive effect than PFU01 (p<0.05). CRT01 showed greater affinity for the skin when compared to PFU01 and GLT01, with low systemic absorption. The CRT01 semi-solid formulation was considered the most adequate one to treat patients with atopic dermatitis or other dermatologic inflammatory diseases, promoting rational use of tacrolimus.

Abstract Objective: To analyze the association between child exposure to intimate partner violence (CEIPV) committed against their mother and functional gastrointestinal disorders (FGIDs) in school-age children. Methods: A prospective cohort study was carried out that evaluated 626 mother-child pairs living in a poor urban area in the Northeast of Brazil. The mothers were asked about their children’s exposure to intimate partner violence. Holden’s classification was used, which investigates ten types of CEIPV from prenatal care. The number of exposures, age of onset, and the perpetrator of the exposure were also verified. The FGIDs in children were identified from the mother’s responses to the questionnaire about pediatric gastrointestinal symptoms, Roma III version parent-report, translated and cross-culturally adapted into Brazilian Portuguese. Results: FGIDs were more frequent among children exposed to violence than those not exposed (OR: 1.64; 95% CI: 1.04-2.58; p = 0.03). Likewise, in children who suffered four or more types of exposure (OR: 1.81; 95% CI: 1.10-2.96; p = 0.01), in which the exposure started in the first two years of life (OR: 1.93; 95% CI: 1.08-3.47; p = 0.02) and in those whose biological father was the perpetrator of the violence (OR; 1.62; 95% CI: 1, 05-2.48; p = 0.02). Conclusion: Corroborating the proposal of the biopsychosocial model, CEIPV increases the chance of occurrence of FGIDs in school-age children. Therefore, early identification and proposals for appropriate interventions are suggested both for the prevention of CEIPV and for the prevention and follow-up of FGIDs.

Abstract Oil-in-water photoprotective nanoemulsions (NEs) were developed using Babassu (BBS) lipophilic extract, nonionic surfactants, and low concentrations of organic sunscreens by ultrasonic processing. BBS extract was chosen due to its suitable physicochemical properties (acidity index, peroxide index, refraction index, and relative density) and predominance of saturated fatty acids, identified by gas chromatography-mass spectrometry (GC-MS), which promote biological activities and high oxidative stability. NEs were characterized by mean droplet size, morphology, polydispersity index (PdI), pH, and organoleptic properties, and the physical stability of the NEs was evaluated for 120 days at room temperature. The sun protection factor (SPF) was determined, and the photostability and in vitro cytotoxicity assays were performed for NEs. All NEs remained stable for 120 days, with a droplet size <150 nm and a monomodal distribution profile. The pH values were compatible with the skin’s pH. NE3 showed a spherical morphology, with a mean droplet size of 125.15 ± 0.16 nm and PdI of 0.145 ± 0.032. NE3 containing BBS extract and sunscreens presented an SPF of 35.5 ± 3.0, was photostable after 6 h of radiation and was non-cytotoxic to fibroblast cells. Thus, NE3 could be considered a promising formulation for developing synergic plant-extract sunscreen photoprotective products for the market.

Abstract The incorporation of antioxidants into sunscreens may provide additional skin photoprotection against the harmful photobiological effects of ultraviolet radiation. The present study evaluated the applicability of a screening approach to the assessment of the antioxidant and photoprotective properties of vitamin C, vitamin E, and coenzyme Q10 and then determined the performance of the most effective antioxidant in a sunscreen formulation. Antioxidant activity was assessed by the 2,2-diphenyl-1-picrylhydrazyl (DPPH) assay, 2,2`-azino-bis (3-ethylbenzothiazoline-6-sulfonic acid) (ABTS) assay, and oxygen radical absorbance capacity (ORAC) assay, and the photoprotective potential was investigated by the yeast photoprotection assay. The antioxidant with the best effect was incorporated into sunscreen formulations which were evaluated for 120 days regarding their in vitro photoprotective parameters. Vitamin C showed high antioxidant capacity as well as a photoprotective potential against simulated solar irradiation applied for times longer than 1 h. Although the Sun Protection Factor, UVA/UVB ratio and critical wavelength did not differed significantly (p<0.05) between the formulation blank and the formulations containing 0.5% or 1% vitamin C, formulations with vitamin C kept their photostability for 6 months. Consequently, the proposed screening approach seems to be promising for the development of an antisolar photostable formulation containing vitamin C as an antioxidant.

Resumo O 12º Congresso Internacional de Zoologia realizou-se em Lisboa, em 1935. Da sua memória concreta constam insígnias – estrela-do-mar – e caricaturas de zoólogos. A partir de investigação no Arquivo do Museu Nacional de História Natural e da Ciência/Universidade de Lisboa, pretendemos interrogar a cenografia material, pensada pelo zoólogo Artur Ricardo Jorge pela aproximação epistémica às potencialidades da biografia de objetos científicos. Esta nota de pesquisa pretende deixar a marca de um discurso científico traduzido em materialidades de circulação em espaço público, na Europa globalizante dos anos 1930, com referências de poder científico, no contexto do Estado Novo português, inaugurado, constitucionalmente, em 1933.

Abstract The material memory of the 12th International Congress of Zoology, held in Lisbon in 1935 includes insignias – the starfish – and caricatures of zoologists. Through an investigation of the archives at the University of Lisbon’s National Museum of Natural History and Science, we intend to investigate the material landscape as conceived by the zoologist Artur Ricardo Jorge by drawing epistemologically on the potentialities of the biography of scientific objects. This research note reveals a scientific discourse translated into materialities circulated in public spaces in the globalized Europe of the 1930s, with references from scientific power, in the context of Portugal’s Estado Novo regime, enshrined in its 1933 constitution.

Abstract Introduction The gastrointestinal stromal tumor (GIST) is the most common mesenchymal neoplasm of the gastrointestinal tract. Even though it can be found in any location of the digestive tract, the colorectal GIST is rare. With this study, we aim to review the current knowledge regarding the prognosis and management of colorectal GIST. Methods A literature search was conducted in PubMed, and 717 articles were collected. After analyzing these studies, 60 articles were selected to use in this review. Results The mitotic index, as well as tumor size and location were identified as good discriminators of prognosis in various studies. Surgery remains the only curative therapy for potentially resectable tumors. However, even after surgical resection, some patients develop disease recurrence and metastasis, especially those with highrisk tumors. Therefore, surgical resection alone might be inadequate for the management of all colorectal GISTs. The discovery of GIST’s molecular pathway led to a shift in its therapy, insofar as tyrosine kinase inhibitors became part of the treatment schemes for this tumor, revolutionizing the treatment’s outcome and prognosis. Discussion/Conclusion The controversy concerning colorectal GIST prognosis and treatment can be, in part, attributed to the limited number of studies in the literature. In this review, we gathered the most recent knowledge about the prognosis and management of GIST in this rare location and propose two algorithms for its approach. Lastly, we highlight the importance of an individualized approach in the setting of a multidisciplinary team.

The aim of this study is to analyze home food (in)security via intersectionality. The data are from the cross-sectional study Quality of the Urban Environment of Salvador - QUALISalvador, conducted from 2018 to 2020 in Salvador, Bahia State, Brazil. A structured questionnaire and the Brazilian Food Insecurity Scale were used. A total of 14,713 households were analyzed. The outcome variable was the situation of food security: mild, moderate, or severe food insecurity. The exposure of interest variable was the intersection of the variables self-declared race/color and gender: white man, white woman, black man, black woman. Multinominal logistic regression models were used to estimate the association between exposure of interest and outcome, adjusted for socioeconomic variables and stratified according to education level and per capita family income. Households lead by black women had a higher chance of experiencing mild (OR = 1.39; 95%CI: 1.15-1.68; p = 0.001) and moderate or severe (OR = 1.94; 95%CI: 1.49-2.52; p < 0.001) food insecurity in relation to households lead by white men. They also had a higher chance of experiencing moderate or severe food insecurity in all levels of education and in the ranges of up to 1/2 minimum wage and > 1 minimum wage. When the person responsible for the household was a black man, the greatest chance for this condition was in the > 1 minimum wage. Food insecurity in households lead by black women, even in socioeconomically favorable conditions, is revealed as one of the consequences of the structural interaction of racism and sexism.

Este trabalho objetiva analisar a (in)segurança alimentar domiciliar sob o olhar da interseccionalidade. Para isso, foram analisados 14.713 domicílios, utilizando-se um questionário estruturado e a Escala Brasileira de Insegurança Alimentar, além dos dados do estudo transversal Qualidade do Ambiente Urbano de Salvador - QUALISalvador, realizado entre 2018 e 2020 em Salvador, Bahia, Brasil. A variável desfecho foi a situação de segurança alimentar (insegurança alimentar leve, moderada ou grave) e a variável exposição de interesse foi o cruzamento de raça/cor e sexo autodeclarado (homem branco, mulher branca, homem negro, mulher negra). Modelos de regressão logística multinominal foram usados para estimar a associação entre a exposição de interesse e o desfecho, ajustada a partir de questões socioeconômicas estratificadas segundo escolaridade e renda familiar per capita. Diante disso, concluiu-se que domicílios chefiados por mulheres negras apresentaram maior chance de insegurança alimentar leve (OR = 1,39; IC95%: 1,15-1,68; p = 0,001) e moderada ou grave (OR = 1,94; IC95%: 1,49-2,52; p < 0,001) em relação aos domicílios chefiados por homens brancos, além de maior chance de insegurança alimentar moderada ou grave em todos os níveis de escolaridade e nas faixas de até 1/2 salário mínimo e > 1 salário mínimo. Quando os responsáveis foram homens negros, a maior chance se apresentou na faixa > 1 salário mínimo. A insegurança alimentar nos domicílios chefiados por mulheres negras, mesmo em condições socioeconomicamente favoráveis, revela-se como uma das consequências da interação estrutural do racismo e do sexismo.

El objetivo de este estudio fue analizar la (in)seguridad alimentaria de los hogares desde la perspectiva de la interseccionalidad. Los datos son del estudio transversal Calidad del Medio Ambiente Urbano de Salvador - QUALISalvador, realizado entre el 2018 y el 2020 en Salvador, Bahía, Brasil. Se utilizó un cuestionario estructurado y la Escala Brasileña de Inseguridad Alimentaria. Se analizaron 14.713 hogares. La variable desenlace fue la situación de seguridad alimentaria, inseguridad alimentaria leve, moderada o severa. La variable de exposición de interés fue el cruce de las variables autoinformadas raza/color y sexo: hombre blanco, mujer blanca, hombre negro, mujer negra. Se utilizaron modelos de regresión logística multinomial para estimar la asociación entre la exposición de interés y el desenlace, ajustados por variables socioeconómicas y estratificadas, según la escolaridad y la renta familiar per cápita. Los hogares encabezados por mujeres negras presentaron una mayor probabilidad de inseguridad alimentaria leve (OR = 1,39; IC95%: 1,15-1,68; p = 0,001) y moderada o severa (OR = 1,94; IC95%: 1,49-2,52; p < 0,001) con relación a los hogares encabezados por hombres blancos. También presentaron una mayor probabilidad de inseguridad alimentaria moderada o severa en todos los niveles de escolaridad y en los rangos de hasta 1/2 salario mínimo y > 1 salario mínimo. Cuando el responsable era un hombre negro, la mayor probabilidad para esta condición se presentó en el rango > 1 salario mínimo. La inseguridad alimentaria en los hogares encabezados por mujeres negras, incluso en condiciones socioeconómicamente favorables, se revela como una de las consecuencias de la interacción estructural del racismo y del sexismo.
