Resumen Objetivo: Describir las preguntas empleadas para valorar la exposición al humo ambiental de tabaco (HAT) en las encuestas de salud en España. Método: Extracción y análisis de literales de las preguntas sobre HAT de las encuestas de salud de España identificadas en las páginas web del Ministerio de Sanidad, Plan Nacional sobre Drogas y Consejerías de Sanidad de las comunidades autónomas. Resultados: Tres encuestas de ámbito nacional valoran la exposición al HAT, con variabilidad en las preguntas, las respuestas o el periodo de recuerdo. Cataluña en 2022, Galicia en 2018 y el País Vasco en 2018 valoraron la exposición de forma detallada. Conclusiones: Las preguntas orientadas a valorar la exposición autodeclarada al HAT son dependientes de la encuesta. Es necesario disponer de un conjunto de preguntas que permitan valorar la exposición de manera homogénea en las encuestas de salud.

Abstract Objective: To describe the questions used to assess exposure to secondhand smoke (SHS) in Spanish health surveys. Method: Extraction and analysis of the literals of the questions on SHS in the health surveys in Spain identified on the website of the Ministry of Health, the National Plan on Drugs and Health Departments of the autonomous communities. Results: Three nationwide surveys assessed SHS exposure, with variability in questions, responses, and recall periods. Catalonia in 2022, and Galicia and the Basque Country in 2018, assessed exposure in detail. Conclusions: Questions assessing self-reported exposure to SHS are survey-dependent. There is a need for a set of questions to assess exposure in a homogeneous way in health surveys.

ABSTRACT BACKGROUND: Intrahepatic cholangiocarcinoma (iCCA) was considered a contraindication for liver transplantation. However, recent studies have shown that highly selected cases of patients with a good response to neoadjuvant therapy may achieve acceptable survival rates when following liver transplantation. AIMS: To present two cases of patients with iCCA, without extrahepatic disease, who underwent living donor liver transplantation after receiving neoadjuvant chemotherapy. METHODS: Two cases of patients with histopathological diagnosis of locally advanced iCCA, ineligible for resection and without evidence of extrahepatic disease, are presented. RESULTS: These patients underwent at least nine sessions of neoadjuvant chemotherapy, including Gemcitabine and Cisplatin, with or without the addition of immunobiological agents, resulting in a radiological tumor response. They subsequently underwent living donor liver transplantation. The average follow-up time was 15 months, with no clinical or radiological signs of disease. CONCLUSIONS: In well-selected patients without extrahepatic disease, living donor liver transplantation represents a potential therapeutic option for iCCA.

RESUMO RACIONAL: O colangiocarcinoma intra-hepático (CCi) foi considerado uma contraindicação para transplante hepático. No entanto, estudos recentes têm demonstrado que casos altamente selecionados de pacientes com boa resposta à terapia neoadjuvante podem ter uma taxa de sobrevida aceitável ao serem submetidos a transplante hepático. OBJETIVOS: Apresentar dois casos de pacientes com colangiocarcinoma intra-hepático sem doença extra-hepática submetidos a transplante hepático de doador vivo, após quimioterapia neoadjuvante. MÉTODOS: Dois pacientes com diagnóstico histopatológico de Cci, localmente avançado, inelegíveis para ressecção e sem evidência de doença extra-hepática. RESULTADOS: Esses pacientes foram submetidos a pelo menos nove sessões de quimioterapia neoadjuvante incluindo Gemcitabina e Cisplatina com ou sem adição de agentes imunobiológicos e com resposta tumoral radiológica. Em seguida, foram submetidos a transplante hepático de doador vivo. O tempo médio de acompanhamento foi de 15 meses, sem sinais clínicos ou radiológicos de doença. CONCLUSÕES: Em pacientes bem selecionados, sem doença extra-hepática, o transplante de fígado de doador vivo é uma opção terapêutica potencial para colangiocarcinoma intra-hepático.

Resumen La Sociedad Argentina de Periodoncia ha formulado un Comentario, analizando los condicionantes del contexto nacional, para determinar si las recomendaciones generadas en la Guía de Práctica Clínica de nivel S3 de la Federación Europea de Periodoncia para el tratamiento de la periodontitis en estadios I-III, podrían ser aplicadas para dar respuesta a la demanda de la población, en lo referido a la salud periodontal.

Abstract The Argentine Society of Periodontics has formulated a Commentary, analyzing the determining factors of the national context, to determine if the recommendations generated in the Level S3 Clinical Practice Guideline of the European Federation of Periodontics for the treatment of periodontitis in stages I-III could be applied to respond to the demand of the population, regarding periodontal health.

Abstract This study aimed to evaluate the contribution of oral and maxillofacial pathology laboratories (OMPLs) in Brazilian public universities to the diagnosis of lip, oral cavity, and oropharyngeal squamous cell carcinoma (SCC). A cross-sectional study was performed using biopsy records from a consortium of sixteen public OMPLs from all regions of Brazil (North, Northeast, Central-West, Southeast, and South). Clinical and demographic data of patients diagnosed with lip, oral cavity, and oropharyngeal SCC between 2010 and 2019 were collected from the patients’ histopathological records. Of the 120,010 oral and maxillofacial biopsies (2010-2019), 6.9% (8,321 cases) were diagnosed as lip (0.8%, 951 cases), oral cavity (4.9%, 5,971 cases), and oropharyngeal (1.2%, 1,399 cases) SCCs. Most cases were from Brazil’s Southeast (64.5%), where six of the OMPLs analyzed are located. The predominant profile of patients with lip and oral cavity SCC was Caucasian men, with a mean age over 60 years, low schooling level, and a previous history of heavy tobacco consumption. In the oropharyngeal group, the majority were non-Caucasian men, with a mean age under 60 years, had a low education level, and were former/current tobacco and alcohol users. According to data from the Brazilian National Cancer Institute, approximately 9.9% of the total lip, oral cavity, and oropharyngeal SCCs reported over the last decade in Brazil may have been diagnosed at the OMPLs included in the current study. Therefore, this data confirms the contribution of public OMPLs with respect to the important diagnostic support they provide to the oral healthcare services extended by the Brazilian Public Health System.

Abstract Objective To identify how healthcare professionals' actions for fall prevention in elderly individuals in both the hospital and home settings influence adaptation and continuity of home-based care. Method An integrative review conducted with a search in the Virtual Health Library (VHL), the Scientific Electronic Library Online (SciELO), the United States National Library of Medicine (PubMed), EBSCO, SCOPUS, and Web of Science. The search used the following Medical Subject Headings (MeSH) terms: "Aged," "Accidental Falls," "Patient Discharge," "Discharge, Patient," "Discharges, Patient," "Patient Discharges," "Discharge Planning," "Discharge Plannings," "Planning, Discharge," and "Plannings, Discharge." Original articles from the past five years were selected, with no language restrictions, and that addressed the research question. Article search and selection were conducted independently and blind by two reviewers between November and December 2022. Results The final sample included eight studies. Six were conducted in Australia, seven were in the English language, and the year 2019 had the highest number of publications. Fall prevention actions were primarily implemented by physiotherapists and occupational therapists through health education and digital videos for guidance related to home modifications. These actions had a positive impact on the motivation, engagement, and awareness of the elderly population and their caregivers. Conclusion The actions of healthcare professionals facilitated greater engagement for the continuity of care and the control of fall risks.

Resumo Objetivo Identificar como as ações dos profissionais de saúde para prevenção de quedas em pessoas idosas no contexto hospitalar e domiciliar, influenciaram na adaptação e continuidade do cuidado em domicíli Método Revisão integrativa com busca no portal da Biblioteca Virtual em Saúde (BVS); Biblioteca Scientific Electronic Library Online (SciELO); United States National Library of Medicine (PubMed); EBSCO, SCOPUS; e Web of Science, a partir dos descritores Medical Subject Heading: “Aged”, “Accidental Falls", “Patient Discharge”, “Discharge, Patient”, “Discharges, Patient”, “Patient Discharges”, “Discharge Planning”, “Discharge Plannings”, “Planning, Discharge”, “Plannings, Discharge. Foram selecionados artigos originais dos últimos cinco anos, sem restrição de idioma, disponíveis eletronicamente e que respondessem à questão de investigação. A busca e seleção dos artigos foram realizadas de forma independente e cega por dois revisores entre novembro e dezembro de 2022. Resultados Obteve-se como amostra final oito estudos. Seis foram realizados na Austrália, sete estavam no idioma inglês e o ano de 2019 obteve maior número de publicação. As ações para prevenção de quedas foram implementadas majoritariamente por fisioterapeutas e terapeutas ocupacionais mediante educação em saúde e vídeos digitais para orientações relacionadas à modificação do domicílio. Essas reverberaram positivamente na motivação, engajamento e consciencialização da população idosa e seus cuidadores. Conclusão As ações dos profissionais de saúde possibilitaram maior engajamento para a continuidade do cuidado e controle do risco de quedas.

ABSTRACT Objectives: to raise reflections on the need for health services and professionals to implement sustainable actions, aiming at their own survival and that of the planet. Methods: reflective essay based on international reports regarding the impact of climate change on people’s health and the role of institutions in this context. Results: the article focused on three fundamentals: climate change continues to be a threat to the health and well-being of all beings on Earth; the institutions that should contribute to health are great agents of contamination of the environment and emission of gases that aggravate the greenhouse effect; and there are several benefits for health institutions to act sustainably. Final Considerations: we cannot wait any longer; we must develop policies and management models aimed at environmentally responsible, economically viable, and socially more collaborative healthcare.

RESUMO Objetivos: suscitar reflexões acerca da necessidade de os serviços e profissionais de saúde implementarem ações sustentáveis visando à própria sobrevivência e do planeta. Métodos: ensaio reflexivo embasado em relatórios internacionais com relação ao impacto das mudanças climáticas sobre a saúde das pessoas e ao papel das instituições nesse contexto. Resultados: são enfocados três fundamentos: as mudanças climáticas continuam a ser uma ameaça para a saúde e bem-estar de todos os seres da Terra; as instituições que deveriam contribuir para a saúde são grandes agentes de contaminação do meio ambiente e de emissão dos gases que agravam o efeito estufa; e há vários benefícios para as instituições de saúde agirem de maneira sustentável. Considerações Finais: não podemos esperar mais; temos de desenvolver políticas e modelos de gestão voltados à uma assistência à saúde ambientalmente responsável, economicamente viável e socialmente mais colaborativa.

RESUMEN Objetivos: suscitar las reflexiones acerca de la necesidad de los servicios y profesionales de salud implementar acciones sostenibles objetivando a la propia supervivencia y del planeta. Métodos: ensayo reflexivo basado en informes internacionales con relación al impacto de los cambios climáticos sobre la salud de las personas y al rol de las instituciones en eso contexto. Resultados: están enfocados tres fundamentos: los cambios climáticos continúan a ser una amenaza para la salud y bienestar de todos los seres de la Tierra; las instituciones que deberían contribuir para la salud son grandes agentes de contaminación del ambiente y de emisión de los gases que agravan el efecto invernadero; y hay diversos beneficios para las instituciones de salud actuaren de manera sostenible. Consideraciones Finales: no podemos esperar más; tenemos de desarrollar políticas y modelos de gestión vueltos a una asistencia a la salud medioambiental responsable, económicamente viable y socialmente más colaborativa.

ABSTRACT BACKGROUND: Liver transplantation represents the best therapeutic modality in end-stage chronic liver disease, severe acute hepatitis, and selected cases of liver tumors. AIMS: To describe a double retransplant in a male patient diagnosed with Crohn's disease and complicated with primary sclerosing cholangitis, severe portal hypertension, and cholangiocarcinoma diagnosed in the transplanted liver. METHODS: A 48-year-old male patient diagnosed with Crohn's disease 25 years ago, complicated with primary sclerosing cholangitis and severe portal hypertension. He underwent a liver transplantation in 2018 due to secondary biliary cirrhosis. In 2021, a primary sclerosing cholangitis recurrence was diagnosed and a liver retransplantation was indicated. Recipient's hepatectomy was very difficult by reason of complex portal vein thrombosis requiring extensive thromboendovenectomy. Intraoperative ultrasound with liver doppler evaluation was performed. Two suspicious nodules were incidentally diagnosed in the donor's liver and immediately removed for anatomopathological evaluation. RESULTS: After pathological confirmation of carcinoma, probable cholangiocarcinoma, at frozen section, the patient was re-listed as national priority and a new liver transplantation was performed within 24 hours. The patient was discharged after 2 weeks. CONCLUSIONS: The screening for neoplasms in donated organs should be part of our strict daily diagnostic arsenal. Moreover, we argue that, for the benefit of an adequate diagnosis and the feasibility of a safer procedure, the adoption of imaging tests routine for the liver donor is essential, allowing a reduction of the costs and some potential risks of liver transplant procedure.

RESUMO RACIONAL: O transplante de fígado representa a melhor modalidade terapêutica na doença hepática crônica terminal, hepatite aguda grave e casos selecionados de tumores hepáticos. OBJETIVOS: Descrever um retransplante duplo em paciente do sexo masculino, diagnosticado com doença de Crohn e complicado com colangite esclerosante primária, hipertensão portal grave e colangiocarcinoma diagnosticado no fígado transplantado. MÉTODOS: Paciente do sexo masculino, 48 anos, diagnosticado com doença de Crohn há 25 anos e complicado com colangite esclerosante primária e hipertensão portal grave. Foi submetido a um transplante de fígado em 2018 devido a cirrose biliar secundária. Em 2021, foi diagnosticada recidiva de colangite esclerosante primária e indicado retransplante hepático. A hepatectomia do receptor foi de alta complexidade devido à trombose complexa da veia porta, exigindo extensa tromboendovenectomia. Foi realizada ultrassonografia intraoperatória com doppler hepático. Dois nódulos suspeitos foram diagnosticados incidentalmente no fígado do doador e imediatamente removidos para avaliação anatomopatológica. RESULTADOS: Após confirmação patológica de carcinoma, provável colangiocarcinoma, pela congelação, o paciente foi relistado como prioridade nacional, e novo transplante hepático foi realizado em 24 horas. O paciente teve alta após 2 semanas. CONCLUSÕES: O rastreamento de neoplasias em órgãos doados deve fazer parte de nosso estrito arsenal diagnóstico diário. Além disso, defendemos que, em benefício de um diagnóstico correto e da viabilidade de um procedimento mais seguro, a adoção de uma rotina de exames de imagem é essencial em doadores hepáticos, permitindo a redução dos custos e alguns riscos potenciais do procedimento de transplante hepático.

RESUMEN Fundamentos: La violencia en la pareja que se produce en adolescentes se conoce con el nombre de violencia en el noviazgo. Actualmente, es un problema generalizado en la adolescencia y existe un gran desconocimiento al respecto de las creencias/actitudes que están ayudando a perpetuar este fenómeno. El objetivo de este estudio fue conocer la percepción de los adolescentes sobre esta violencia y estimar la frecuencia observada de exposición a diferentes ítems de violencia en el noviazgo, valorando las diferencias según sexo y nivel educativo. Métodos: Se realizó en 2022 un estudio transversal en cuatro centros educativos públicos de Galicia (España) que impartían Educación Secundaria Obligatoria y Bachillerato. El muestreo de los centros fue de conveniencia. Se invitó a los estudiantes a contestar un cuestionario anónimo. Se estimó la frecuencia observada de exposición del estudiantado a diferentes ítems de violencia en el noviazgo y de identificación de la violencia. Para comparar las proporciones en función del sexo y del nivel educativo se empleó la prueba exacta de Fisher. Resultados: Se incluyeron 410 estudiantes. El 99% de las mujeres percibieron que no es normal controlar la vestimenta de la pareja frente al 88% de los hombres; en relación con el control de las amistades, estos porcentajes fueron del 87,6% y el 73,1%, en mujeres y hombres, respectivamente. Con respecto a las críticas a la pareja, estos porcentajes fueron del 54,7% frente al 67,9%, respectivamente en mujeres y hombres. El 46,8% del estudiantado admitió conocer casos que enviaban varios mensajes al día para saber qué estaba haciendo la pareja. El 21,7% refirió conocer casos que sentían miedo de su pareja. Conclusiones: La percepción de violencia en el noviazgo es mayor por parte de las mujeres. Las mayores diferencias entre hombres y mujeres se observan en los ítems del dominio control.

ABSTRACT Background: The violence produced in the context of dating is known as dating violence. Currently, it is a widespread problem in adolescence and there is a great lack of knowledge about the beliefs/attitudes that help perpetuate this phenomenon. This study aimed to assess how adolescents perceive dating violence. Additionally, to estimate the observed frequency of exposure of adolescents to different dating violence items, assessing the differences according to sex and educational level. Methods: A cross-sectional study was carried out in 2022 among high school students coming from Spain (Galician Region) The data was collected by means of an anonymous online questionnaire. A descriptive analysis of the data obtained was conducted. The observed frequency of adolescents' exposure to different items of dating violence and identification of violence was estimated. Fisher's exact test was used to compare the proportions according to sex and educational level. Results: 410 students were included. 99% of the women perceived that it is not normal to control their partner's clothing compared to 88% of the men; in relation to the control of friendships these percentages were 87.6% and 73.1% and with respect to criticism of the partner 54.7% vs 67.9%, respectively in women and men. 46.8% of the students admitted knowing of cases in which they sent several messages a day to find out what their partner was doing. 21.7% referred to be aware of cases who felt fear of the partner. Conclusions: The perception of dating violence by women is higher. The largest differences between men and women are in the items of the control domain.

Resumen Objetivo: Caracterizar la prevalencia de actividad física en la población ≥ 16 años integrando sedentarismo e inactividad física. Método: Se realizó un estudio transversal con datos del Sistema de Datos de Conductas de Riesgo de Galicia (n = 12.928). La población se clasificó en cuatro categorías: físicamente activa (activa y no sedentaria), activa pero sedentaria (activa y sedentaria), poco activa (inactiva y no sedentaria) y extremadamente sedentaria (inactiva y sedentaria). Se calcularon las prevalencias para cada categoría y se ajustó un modelo de regresión logística multinomial. Resultados: El 58,0% de la población era físicamente activa y el 42,0% restante presentaba algún grado de sedentarismo o inactividad física. Los hombres eran más propensos a ser extremadamente sedentarios. El riesgo de sedentarismo disminuía con la edad, y los trabajadores o estudiantes eran propensos a un mayor riesgo de sedentarismo extremo en comparación con los que declaraban otra condición. Los comportamientos extremadamente sedentarios también fueron más frecuentes en personas con niveles educativos más altos, personas que vivían solas, residentes en entornos urbanos y fumadores. Conclusiones: La evaluación conjunta del sedentarismo y la inactividad física proporciona una imagen más completa y realista de los comportamientos de la población relacionados con la actividad física. Dado que el sedentarismo es el comportamiento más prevalente, este estudio recomienda que se realicen intervenciones en el ámbito laboral y académico.

Abstract Objective: To characterize the prevalence of physical activity in the population aged ≥ 16 years integrating sedentarism and physical inactivity. Method: A cross-sectional study using data from the Galician Risk Behavior Data System (n = 12,928) was conducted. The population was classified into four categories: physically active (active and non-sedentary), active but sedentary (active and sedentary), lightly active (inactive and non-sedentary), and extremely sedentary (inactive and sedentary). Prevalences were calculated for each category and a multinomial logistic regression model was fitted. Results: 58.0% of the population was physically active and the remaining 42.0% presented some degree of sedentarism and/or physical inactivity. Men were more likely to be extremely sedentary. The risk of sedentarism decreased with age, and workers and/or students were prone to a higher risk of extreme sedentarism in comparison to those who reported other status. Extremely sendentary behaviors were also more common in people with higher educational levels, individuals living alone, residents of urban environments, and smokers. Conclusions: Joint evaluation of sedentarism and physical inactivity provides a more comprehensive and realistic picture of population behaviors related with physical activity. Since sedentarism is the most prevalent behavior, this study recommends that interventions be conducted at work and at academic environments.

Resumen Objetivo: Analizar la carga del consumo de tabaco en la mortalidad y en los años de esperanza de vida perdidos en población ≥35 años en Brasil en el periodo 1996-2019, e identificar cambios de tendencia en la mortalidad atribuida. Método: Para estimar la mortalidad atribuida se aplicó un método independiente de prevalencia que asume la tasa de mortalidad por cáncer de pulmón como indicador indirecto del riesgo acumulado aso- ciado al consumo de tabaco. La mortalidad atribuida se estima a partir de la tasa de mortalidad por cáncer de pulmón y aplicando riesgos relativos de cinco cohortes estadounidenses. Se presentan la mortalidad atribuida, las tasas de mortalidad atribuida brutas y estandarizadas en global, por sexo, edad y causas de muerte. Se analizó la tendencia aplicando modelos de regresión joinpoint. Se calcularon los años de esperanza de vida perdidos a causa del tabaco. Resultados: Entre 1996 y 2019 el consumo de tabaco causó 2.389.831 muertes en Brasil. Las enfermedades cardiometabólicas fueron la primera causa de muerte en mujeres en todo el periodo y en hombres hasta 2015. Desde 2006, las tasas de mortalidad atribuida en los hombres, con independencia de la edad, muestran una tendencia decreciente, mientras que en las mujeres la evolución es diferente. Los años de esperanza de vida perdidos muestran un ligero descenso desde los primeros trienios y son más en los hombres. Conclusiones: El 8,5% de la mortalidad total en Brasil durante el periodo 1996-2019 se atribuye al consumo de tabaco. Es importante monitorizar la carga del consumo de tabaco en la mortalidad para fortalecer o implantar intervenciones frente al tabaquismo en Brasil.

Abstract Objective: To analyze the burden of tobacco consumption on mortality and years of life expectancy lost in population ≥35 years in Brazil in the period 1996-2019 and to identify trend changes in smoking- attributable mortality. Method: An independent prevalence method using the lung cancer mortality rate as a proxy for cumulative smoking risk was used to estimate smoking-attributable mortality. Smoking-attributable mortality is estimated from the lung cancer mortality rate and applying relative risks from 5 US cohorts. Smoking- attributable mortality, crude and standardized attributed mortality rates are presented overall, by sex, age and causes of death. Trend analysis was performed by applying joinpoint regression models. Years of life expectancy lost due to tobacco were calculated. Results: Tobacco consumption caused 2,389,831 deaths in Brazil between 1996-2019. Cardiometabolic diseases were the leading cause of death in women throughout the period and in men until 2015. Since 2006, smoking-attributable mortality rates in men, regardless of age, show a decreasing trend while in females the evolution is different. The years of life expectancy lost show a slight decrease since the first triennia and are higher in men. Conclusions: In Brazil, the 8.5% of total mortality between 1996-2019 is attributed to tobacco consumption. It is important to monitor the burden of the tobacco consumption on mortality in order to strengthen or implement interventions against smoking in Brazil.

Resumen Objetivo: Conocer si en España los planes de salud autonómicos incluyen objetivos y medidas específicas relacionados con la actividad física. Método: Se realizó una búsqueda en las páginas web institucionales para identificar el plan de salud más reciente de cada comunidad autónoma. Se extrajo la información que incluían los planes de salud sobre objetivos y medidas, y grupos poblacionales a los que se dirigían dichas medidas. La búsqueda se completó el 24 de enero de 2022. Resultados: Se identificaron 16 planes de salud, de los cuales seis están vigentes en 2022; de estos, 15 incorporaban objetivos y medidas específicas sobre actividad física. La mayor parte de las acciones van destinadas a población general, población infanto-juvenil y personas mayores. Los planes de Cantabria, la Comunidad Valenciana y el País Vasco incluyen la actividad física de forma más extensa, proponiendo medidas para siete de los ocho grupos poblacionales identificados. Conclusiones: La mayor parte de los planes de salud incluyen medidas en relación con la actividad física y las dirigen a dos o más grupos poblacionales. La heterogeneidad en cuanto a objetivos, medidas específicas y grupos poblacionales es alta. Este estudio puede ayudar a las comunidades autónomas a revisar su plan de salud, actualizarlo y mejorarlo en materia de actividad física. Es conveniente que los diferentes planes de salud tengan un marco de propuestas homogéneo relacionado con la promoción de la actividad física.

Abstract Objective: To identify if the regional government health plans in Spain include specific objectives and measures related to physical activity. Method: Institutional websites were scanned to identify the most recent health plan of each regional government. The information included in the health plans on objectives and measures and the population groups targeted by these measures was extracted. The search was completed on 24 January 2022. Results: Sixteen health plans were identified of which six are in force in 2022; 15 of these have incorporated specific objectives and measures on physical activity. Most of the actions are focus on general population, children-adolescents and elderly population. The plans of Cantabria, the Comunitat Valenciana and País Vasco cover physical activity more widely, proposing measures for seven of the eight identified population groups. Conclusions: Most of the health plans include measures related to physical activity and target two or more population groups. The objectives, specific measures and population groups were highly heterogeneous. This study could serve to prompt regional governments to review their health plans, update them and improve them in terms of physical activity. It seems advisable that the different health plans should have a homogeneous framework of proposals related to the promotion of physical activity.

ABSTRACT BACKGROUDS: Child and Adolescent Psychosocial Care Centers (Centros de Atenção Psicossocial, CAPSI) are dedicated centers for persistent psychiatric disorders, which provide an individualized therapeutic approach based on extra-hospital services. OBJECTIVES: We aimed to describe the clinico-epidemiological profiles of the patients seeking interventions at the CAPSIs. DESIGN AND SETTING: A cross-sectional study was conducted in two CAPSI in São Bernardo do Campo, SP, Brazil. One CAPSI is dedicated to the treatment of alcohol- and drug-related disorders, and the other to the treatment of other mental disorders. METHODS: In July 2017, we reviewed all active medical records of these two CAPSI, and collected the patients information including sex, race, education, type of referral, initial complaints, psychiatric diagnoses, and medication utilization. RESULTS: Of the 233 patients, 69.5% were male and 42.5% lived with their immediate family. Most of the patients were referred from other health services. Complaints on admission included agitation and aggressive behavior (30.9%). Autism spectrum disorder (ASD) was the most prevalent diagnosis (46.8%), followed by depressive disorder (13.8%). Of the patients, 81.5% were on regular medical follow-up and 70.3% were on a single medication only. CONCLUSION: Aggression complaints are the most prevalent in CAPSI, and diagnoses of ASD and psychotic disorders are more common. This situation differs from most CAPSI that present school complaints as the most prevalent, in which diagnoses of attention-deficit/hyperactivity disorder and conduct disorders are likely to be more frequent. The epidemiological profile of each CAPSI should guide the implementation of human and structural resources targeting the most prevalent complaints and diagnoses.

RESUMO Introdução: O reparo tecidual e a cicatrização de feridas cutâneas são processos complexos que envolvem uma série de eventos dinâmicos, incluindo coagulação, inflamação, formação de tecido de granulação, contração da ferida e remodelação tecidual. A necessidade de cuidar de pacientes com dificuldades na cicatrização de feridas é um desafio crescente e requer estratégias inovadoras. Uma abordagem que se destaca no tratamento dessas lesões é a laserterapia de baixa potência. Este estudo tem como objetivo demonstrar a aplicabilidade e os resultados de um protocolo de laserterapia de baixa potência no manejo da cicatrização de feridas cutâneas. Métodos: Trata-se de estudo retrospectivo de revisão de casos relatando o uso de laser no tratamento de feridas cutâneas em um centro único. Resultados: Avaliamos os resultados do uso de laserterapia de baixa potência e relatamos cinco casos de pacientes com feridas cutânea que não apresentaram melhora com o tratamento convencional. As lesões eram localizadas em sítios diversos, com lesão no tronco, membro superior e membro inferior. Os cinco casos apresentaram uma boa resposta com a terapia a laser, evoluindo com a cicatrização das feridas em um curto período de tempo, com período médio de cicatrização de 48 dias. Conclusão: Conclui-se que a laserterapia de baixa potência quando aplicada sobre feridas cutâneas sugere uma ação benéfica, promissora e tem um potencial para aumentar as opções terapêuticas disponíveis para o cirurgião, porém, como relatamos um número de cinco casos, há necessidade de mais estudos para verificar a eficiência do laser em feridas. ABSTRACT Introduction: Tissue repair and skin wound healing are complex processes that involve a series of dynamic events, including coagulation, inflammation, granulation tissue formation, wound contraction and tissue remodeling. The need to care for patients with difficulties in wound healing is a growing challenge and requires innovative strategies. One approach that stands out in the treatment of these injuries is low-power laser therapy. This study aims to demonstrate the applicability and results of a low-power laser therapy protocol in the management of skin wound healing. Methods: This is a retrospective case review study reporting the use of laser in the treatment of skin wounds in a single center. Results: We evaluated the results of the implementation of this low-power laser therapy protocol and reported five cases of patients with skin wounds that did not improve with conventional treatment. The lesions were located in different places, with lesions on the trunk, upper limb and lower limb. The five cases showed a good response to laser therapy, evolving with wound healing in a short period of time, with an average healing time of 48 days. Conclusion: It is concluded that low power laser therapy when applied to skin wounds suggests a beneficial, promising action and has the potential to increase the therapeutic options available to the surgeon, however as we report a number of five cases, further studies are needed for check laser efficiency on wounds. RESUMEN Objetivo: La necesidad de generar evidencia relacionada con la COVID-19, la aceleración del proceso de revisión por pares y publicación, y la competencia entre revistas, pueden haber influido en la calidad de los artículos sobre la COVID-19. El objetivo fue comparar las características de los artículos sobre la COVID-19 con las de los artículos no COVID-19 e identificar las variables en las que difieren. Método: Se realizó un estudio de casos y controles emparejados por revista. Los casos eran artículos COVID-19 y los controles eran artículos no COVID-19 publicados en el mismo periodo. Se seleccionaron revistas pertenecientes a cinco categorías diferentes del Journal Citations Reports. Dentro de cada revista seleccionada, se escogió un artículo COVID-19 (cuando había uno) y otro artículo no COVID-19. Se ajustaron modelos de regresión logística condicional. Resultados: Se incluyeron 81 artículos COVID-19 y 143 artículos no COVID-19. Los estudios observacionales descriptivos y los estudios observacionales analíticos tenían, respectivamente, una probabilidad 55 veces mayor (odds ratio [OR]: 55,12; intervalo de confianza del 95% [IC95%]: 7,41-409,84) y 19 veces mayor (OR: 19,28; IC95%: 3,09-120,31) de ser artículos COVID-19 y de tener un tamaño de muestra menor (OR: 7,15; IC95%: 2,33-21,94). Los artículos más citados desde su publicación tenían 5 veces más probabilidades de ser artículos COVID-19 (OR: 4,97; IC95%: 1,63-15,10). Conclusiones: Los artículos COVID-19 parecen tener mayor impacto bibliométrico, a pesar de tener menor calidad metodológica. Para asegurar la publicación de buena evidencia científica se debe preservar la calidad de los artículos relacionados con la COVID-19.

Our objective was to compare the characteristics of COVID-19 papers against those of non-COVID-19 papers and identify the variables in which they differ. Method: We conducted a journal-matched case-control study. Cases were COVID-19 papers and controls were non-COVID-19 papers published between March 2020 and January 2021. Journals belonging to five different Journal Citations Reports categories were selected. Within each selected journal, a COVID-19 paper (where there was one) and another non-COVID-19 paper were selected. Conditional logistic regression models were fitted. Results: We included 81 COVID-19 and 143 non-COVID-19 papers. Descriptive observational studies and analytical observational studies had, respectively, a 55-fold (odds ratio [OR]: 55.12; 95% confidence interval [95%CI]: 7.41-409.84) and 19-fold (OR: 19.28; 95%CI: 3.09-120.31) higher likelihood of being COVID-19 papers, respectively, and also a higher probability of having a smaller sample size (OR: 7.15; 95%CI: 2.33-21.94). COVID-19 papers had a higher probability of being cited since their publication (OR: 4.97; 95%CI: 1.63-15.10). Conclusions: The characteristics of COVID-19 papers differed from those of non-COVID-19 papers published in the first months of the pandemic. Resumen En los últimos años han salido a la luz muchos casos de mala conducta científica, algunos con importantes consecuencias, que han evidenciado las brechas que globalmente existen en cuanto a integridad científica. En España también se han dado casos notables de mala conducta científica en el ámbito de la investigación biomédica. Sin embargo, hasta el momento no se ha creado un organismo encargado de supervisar las fases de ejecución, análisis y publicación de las investigaciones biomédicas desde un punto de vista ético. Por lo tanto, en este contexto, consideramos que es necesaria la creación de una oficina que supervise la integridad científica en España y que actúe en caso de sospecha de mala conducta científica, llevando a cabo una investigación independiente y con capacidad sancionadora. La existencia de dicho organismo sería de especial importancia en el caso de las investigaciones financiadas con fondos públicos, ya que en ese caso el fraude en investigación supondría la malversación de dinero público. La creación de una oficina que realmente actúe frente a los casos detectados podría tener un efecto disuasorio sobre una potencial mala conducta de algunos investigadores, previniendo así situaciones de mala conducta científica.

Abstract In recent years, many cases of scientific misconduct have come to light, some with considerable consequences, highlighting the existing breaches in the scientific integrity globally. In Spain, there have also been high-profile cases of scientific misconduct. However, so far, no organism or agency has been created to monitor the execution, analysis and publication phases of biomedical research from an ethical point of view. Therefore, in this context, we consider that there is a need for the creation of an office which supervises research integrity in Spain which would act in cases of suspected scientific misconduct, carrying out an independent investigation and proposing public sanctions. The existence of such an organism would be of particular importance in the case of publicly funded research, since in that case research fraud would involve the misappropriation of public funds. The creation of an office that would act on detected cases could have a deterrent effect on potential misconduct by some researchers, thus preventing cases of scientific misconduct.
