Resumo O impacto da infecção por Chlamydia trachomatis (CT) na fertilidade feminina ainda não está completamente estabelecido, uma vez que o nível de evidência associando esses fatores ainda é insignificante. Assim, o objetivo desta revisão é contribuir para uma melhor elucidação deste assunto. A base de dados eletrônica escolhida foi a Medline/PubMed, com a última pesquisa em 11 de maio de 2021. Utilizou-se como filtro a data de publicação, sendo selecionados os 5 anos anteriores. Foram usados os seguintes descritores: Chlamydia trachomatis E infertility; Chlamydia trachomatis E tubal alteration E infertility; Chlamydia E low pregnancy rates. Dos 322 estudos selecionados, 293 que não atenderam aos nossos critérios de elegibilidade foram excluídos. Posteriormente, retiramos sete estudos por não terem como foco principal a possível correlação entre infecção por CT e infertilidade feminina e três por tratarem de infecções sexualmente transmissíveis (ISTs) em geral. Além disso, dois estudos concebidos como revisões também foram excluídos. Portanto, incluímos 17 estudos em nossa análise qualitativa. Os autores realizaram pesquisas individualmente e analisaram criteriosamente os estudos selecionados. Como obtivemos as informações necessárias para nosso estudo por meio da leitura dos textos, nenhum contato foi feito com os autores. Esta revisão sistemática corrobora a hipótese de que a infecção por CT potencializa a infertilidade feminina, pois 76,47% dos estudos incluídos encontraram correlação positiva entre eles. Concluímos que existe uma associação importante entre infecção por CT e infertilidade feminina. Portanto, tornar os procedimentos de triagem por CT parte da rotina de investigação de infertilidade é relevante e justificável.

Abstract The impact of Chlamydia trachomatis (CT) infection on female’s fertility is not completely established yet, since the level of evidence associating these factors is still weak. Hence, the goal of the present review is to contribute to a better elucidation of this matter. The electronic database chosen was the Medline/PubMed, with the last survey on May 11, 2021. Publication date was used as a filter, with the previous 5 years having been selected. The following describers were used: chlamydia trachomatis AND infertility; chlamydia trachomatis AND tubal alteration AND infertility; chlamydia AND low pregnancy rates. From the 322 studies screened, 293 that failed to meet our eligibility criteria were excluded. Subsequently, we removed seven studies for not having the possible correlation between CT infections and female infertility as its main focus, and three for being about sexually transmitted infections (STIs) in general. Moreover, two studies designed as reviews were also excluded. Ergo, we included 17 studies in our qualitative analysis. The authors conducted research individually and analyzed carefully the studies selected. As we retrieved the information needed for our study through reading the texts, no contact was made with the authors of the studies selected. This systematic review corroborates the hypothesis that CT infection potentiates female infertility, as 76.47% of the included studies found a positive correlation between them. We conclude that there is an important association between CT infection and female infertility. Ergo, making CT screening part of the infertility investigation routine is relevant and has a reasonable justification.

RESUMO Quando o projeto parental é inviabilizado em decorrência da infertilidade e o casal recorre às Técnicas de Reprodução Assistida (TRA), a construção da paternidade pode ser afetada. Este estudo qualitativo, que teve como objetivo investigar a experiência paterna da gestação nesse contexto, foi realizado com 13 pais cujas companheiras engravidaram por meio de TRA e se encontravam no terceiro trimestre gestacional A análise de conteúdo das entrevistas revelou que os participantes estavam envolvidos com a gestação e com o bebê, bem como com aceitação da ideia de se tornar pai. Destaca-se que essa vivência foi permeada pelas repercussões da infertilidade e do tratamento, o que pode trazer dificuldades e especificidades para a paternidade. Nesse contexto, em que o desejo e a realização da paternidade sofrem entraves desde seu início, é importante que se possa oferecer aos futuros pais o apoio de profissionais da área da saúde mental.

ABSTRACT When plans to become parents cannot be realized due to infertility, and when the couple resort to Assisted Reproductive Technologies (ART), the construction of fatherhood may be affected. The objective of this qualitative study with 13 men whose partners conceived through ART and were in their third trimester of pregnancy was to investigate the experience of pregnancy according to the fathers’ perceptions. Content analysis of the interviews showed that the majority of participants was involved with pregnancy and the baby, and also accepted the idea of becoming a father. It is noteworthy that this experience was permeated by the repercussions of infertility and its treatment, which may lead to difficulties and specificities to fatherhood. In this context, in which the desire to become a father and the possibility to fulfill such a dream are difficult since the beginning, the authors suggest that these men may be supported by mental health professionals.

Neste estudo é apresentado um modelo experimental de defeito agudo em nervo periférico para avaliação da regeneração nervosa mediante técnica de tubulização associada à inoculação de células-tronco autólogas de medula óssea. Foram utilizados 12 coelhos Nova Zelândia albinos, submetidos à secção bilateral e ao afastamento de 5mm do nervo tibial e posterior reparo mediante utilização de câmara de silicone. Internamente à prótese de tubulização do nervo tibial esquerdo em todos os animais, foram inoculadas células-tronco autólogas de medula óssea, coletadas a partir do úmero. Como grupo controle (nervo tibial direito), mediante aplicação da mesma técnica de reparo, solução de NaCl 0,9% foi administrada internamente à prótese. Após 30 dias de observação, os animais foram eutanasiados e foi realizada a avaliação histológica dos segmentos nervosos por meio das colorações de hematoxilina-eosina, luxol fast blue e azul de toluidina. Com os resultados, foi possível concluir que o transplante de células-tronco autólogas associado à técnica de tubulização apresenta vantagens no processo de regeneração nervosa periférica.

This study presents an experimental model of an acute deffect in a peripheral nerve to evaluate neural regeneration using a tubulization technique associated with the inoculation of autologous stem cells from bone marrow. A total of 12 New Zealand white rabbits underwent a bilateral dissection of the tibial nerve followed by repair with silicone tubulization. On the left tibial nerve of all animals, the tube was filled with autologous bone marrow-derived stem cells collected from the humerus. For control, using the same repair technique, the tubes were filled with a NaCl solution in the right tibial nerve. After 30 days of observation, the animals were euthanized and a histological evaluation of the collected nerve segments was performed by staining with hematoxylin-eosin, luxol fast blue, and toluidine blue. From the results it is possible to conclude that the transplanted autologous stem cells associated with the tubulization technique present an advantage in the peripheral nerve regeneration process.

Durante o processo de criopreservação de sêmen, os espermatozóides sofrem alguns danos que resultam na diminuição da fertilidade deste. O presente estudo foi realizado com o objetivo de avaliar os efeitos da utilização combinada de duas curvas de congelamento com dois diluentes comerciais (FR-5® e Botu-Crio®) sobre a criopreservação de sêmen eqüino. Foram analisados 20 ejaculados de dois garanhões. As amostras foram avaliadas por microscopia de contraste de fase e microscopia de epifluorescência, observando-se a motilidade progressiva e total do sêmen pós-descongelamento e a integridade e a funcionalidade da membrana dos espermatozóides. A combinação entre curva automatizada e Botu-Crio® apresentou as maiores médias nas análises de motilidade total e progressiva, após o descongelamento. O diluente Botu-Crio®, isoladamente, preservou também as membranas destes, quando foram realizadas as análises de integridade utilizando teste com diacetato de carboxifluoresceína e iodeto de propídio e funcionalidade de membrana pelo teste hiposmótico.

During semen cryopreservation, sperm cells were submitted to several deleterious events leading to membrane damage which result in fertility decrease. This study was designed to compare the effects of two freezing techniques (conventional and automated), and the use of two commercial extenders as cryoprotectants (FR-5® and Botu-Crio®) on total and progressive motility, integrity and functionality of spermatic membranes during the cryopreservation of equine semen. Twenty ejaculates from two stallions were analyzed. The total and progressive motility of fresh and post-thawing semen samples were evaluated by patterns assays. Function of plasmatic membrane was measured by the hipoosmotic swelling test. Integrity of plasmatic membrane was evaluated using carboxifluorescein diacatate and iodidium propide fluorescent probes. There were significant differences between the two freezing techniques and/or between cryoprotectants for all assessed parameters. The combination of Botu-Crio® and automated curves showed better results on total and progressive post-thawing motility. The extender Botu-Crio®, alone, showed to better preserve the membrane integrity and function.

OBJETIVO: desenvolvimento da versão em português brasileiro do Endometriosis Health Profile Questionnaire (EHP-30), adaptação transcultural ao Brasil e avaliação das medidas psicométricas do EHP-30 Português em amostra brasileira. MÉTODOS: o instrumento original em inglês foi vertido para o português, seguindo diretrizes internacionais, passando por todas as etapas de tradução, retrotradução e comparação das versões para adaptação transcultural, validade de face e de conteúdo. O EHP-30 Português foi aplicado a uma amostra de 54 pacientes com diagnóstico de endometriose para análise de consistência interna, usando o alfa de Cronbach. A fidedignidade teste-reteste foi avaliada pelo coeficiente de correlação intraclasse (ICC). Para a avaliação de validade de construto convergente foi testada correlação entre o EHP-30 Português, WHOQOL-Bref e o Inventário de Depressão de Beck (BDI). RESULTADOS: a avaliação da consistência interna apresentou valores de α=0,8 a 0,9, sugerindo homogeneidade entre as questões. A fidedignidade teste-reteste apresentou ICC de 0,8 a 0,9, demonstrando estabilidade do instrumento. Na validação de construto, demonstraram-se fortes correlações da escala de auto-imagem do EHP-30 com os domínios físico (-0,6) e psicológico (-0,6) do WHOQOL-Bref e da escala de suporte social com o BDI (0,5), evidenciando-se, assim, boa correlação com outros instrumentos de avaliação de qualidade de vida. CONCLUSÕES: o EHP-30 Português mostrou ser um instrumento de fácil e rápida aplicação e bem aceito pelas pacientes, apresentando bom desempenho psicométrico, com medidas de fidedignidade adequadas (consistência interna e fidedignidade teste-reteste) e validade de construto. Estes resultados demonstram que o EHP-30 Português é um instrumento adequado para avaliação de qualidade de vida em mulheres brasileiras com endometriose em ambiente clínico e de pesquisa.

PURPOSE: the objective of the present study was the development of the Brazilian Portuguese version of Endometriosis Health Profile Questionnaire (EHP-30), the cross-cultural adaptation to Brazil and the evaluation of EHP-30 Portuguese psychometric measures in a Brazilian sample. METHODS: the original instrument in English was translated into Portuguese following international guidelines, going through all stages of translation, back-translation and comparison of the versions for cross-cultural adaptation, face and content validity. A sample of 54 patients with endometriosis was used for internal consistency analyses using the Cronbach alpha. Test-retest reliability was evaluated through the intraclass correlation coefficient (ICC). In order to evaluate the convergent construct validity, the correlation between EHP-30 Portuguese and WHOQOL-Bref and Beck Depression Inventory (BDI) was tested. RESULTS: internal consistency presented alpha values of 0.8 to 0.9 suggesting homogeneity between questions. The test-retest reliability presented ICC of 0.8 to 0.9 showing instrument stability. In the construct validation, strong correlations were demonstrated of the EHP-30 Portuguese self-image scale with physical (-0.6) and psychological domains (-0.6) of WHOQOL-Bref, and EHP-30 Portuguese social support scale with BDI (0.5), confirming good correlation with other quality of life evaluation instruments. CONCLUSIONS: the EHP-30 Portuguese was found to be an easy, quickly applied instrument, and well-accepted by the patients. It presented good psychometric properties with appropriate reliability measures (internal consistency and test-retest reliability) and construct validity. These results show that EHP-30 Portuguese is an adequate instrument for quality of life evaluation in Brazilian women with endometriosis, both in clinical and research setting.

OBJETIVO: Determinar fatores preditivos do achado de espermatozóides móveis em casos de azoospermia. MÉTODOS: Sessenta pacientes com diagnóstico de azoospermia tiveram avaliado seu volume testicular e dosagens séricas de hormônio folículo estimulante (FSH), hormônio luteinizante (LH), prolactina e testosterona. Os pacientes foram submetidos à biópsia testicular bilateral com anestesia local. As amostras obtidas foram submetidas à avaliação no laboratório de biologia de reprodução, sendo classificadas em: ausência de espermatozóides, presença de espermatozóides móveis e imóveis, e à avaliação histológica. Foram comparados: idade, valores hormonais, volume testicular e histologia com o achado de espermatozóides móveis. Foi considerado significativo um P < 0,05. RESULTADOS: Quarenta e cinco pacientes possuíam azoospermia não-obstrutiva. Foi encontrada uma diferença significativa entre os grupos com achado de espermatozóides móveis e o grupo com ausência de espermatozóides quanto aos valores de FSH (P=0,037 ANOVA one-way). Foi construída uma curva ROC que determinou valores abaixo de 16,05 UI/L (sensibilidade de 76,2%, especificidade de 67,7%) como preditivos do achado de espermatozóides móveis. Não foram encontradas outras diferenças estatísticas. CONCLUSÕES: O FSH possui boa acurácia na determinação da presença de espermatozóides móveis na biópsia testicular de pacientes azoospérmicos. O exame físico, os valores de testosterona, LH e prolactina não podem ser utilizados na predição do sucesso da biópsia testicular para fins de reprodução assistida.

OBJECTIVE: To define predictive factors of mobile spermatozoa recovery in azoospermic patients. METHODS: Testicular volume, serum follicle stimulating hormone (FSH), luteinizing hormone (LH), prolactin (PRL) and testosterone levels were assessed in 60 azoospermic patients. Patients underwent bilateral testicular biopsy with local anesthesia. Samples were classified according to absence of spermatozoa, presence of motile and nonmotile spermatozoa, and histological findings. Age, hormone levels, testicular volume and histology with motile spermatozoa recovery were compared. P < 0.05 was considered significant. RESULTS: Non-obstructive azoospermia was diagnosed in 45 patients. Significant differences were detected between the group with motile spermatozoa recovery and the group with absence of spermatozoa in terms of FSH levels (P=0.037 ANOVA one-way). A ROC curve was used to define FSH values below 16.05 IU/L (sensitivity: 76.2%, specificity: 67.7%) as predictive factors for motile spermatozoa recovery. Other statistical differences were not detected. CONCLUSIONS: FSH levels below 16.05 IU/L showed good accuracy to predict the presence of motile spermatozoa in the testicular biopsy of azoospermic patients. Physical examination, testosterone levels, LH and prolactin were not useful as predictive factors in the present study.
