OBJETIVO: Definir qual a melhor potência a ser empregada de forma a atingir a profundidade necessária para ablação com o menor número de sintomas pós-procedimento. MÉTODO: Foram estudados 28 pacientes com esôfago de Barrett, após tratamento cirúrgico do refluxo ou em uso de bloqueadores de bomba de prótons, submetidos à ablação endoscópica, randomizados em dois grupos com potências diferentes - 50 ou 70W. Imediatamente após, foram realizadas biópsias endoscópicas das áreas fulguradas. A sintomatologia foi avaliada por questionário telefônico. RESULTADOS: Não houve diferença entre os grupos quanto a idade, a extensão do esôfago de Barrett, a porcentagem da circunferência esofagiana coagulada e a duração dos sintomas. A dor foi o sintoma predominante e a disfagia ocorreu de forma transitória. Houve correlação negativa moderada entre número de sintomas e potência (potência mais baixa com maior número de sintomas), porém sem diferença significativa. Em 40% dos casos em que se utilizou menor potência obteve-se fragmentos que atingiram apenas a porção superficial da mucosa, o que só ocorreu em 10% dos casos no grupo de 70W. Não foi observada diferença significativa entre a potência utilizada ou o acometimento da muscular da mucosa e o número de sintomas. CONCLUSÕES: A utilização de potência de 70W durante a coagulação do esôfago de Barrett com plasma de argônio sugere associação com menor incidência de metaplasia colunar especializada residual abaixo do epitélio escamoso neoformado.

OBJECTIVE: To establish the ideal power to be employed in order to get the effective ablation and the lowest rate of symptoms at argon plasma thermocoagulation in Barrett's esophagus (BE). METHODS: Twenty-eight asymptomatic patients with BE, were randomly divided in two groups of different ablation powers, 50W or 70W. After endoscopic ablation and biopsies from the treated area for histological analyses, symptoms were evaluated through a questionnaire answered by phone. RESULTS: Thirteen patients without specialized columnar metaplasia were excluded and the remaining fifteen patients, seven men (46,7%) and eight women (53,3%), with an average age of 53 years +10,4, composed the two groups: 10 patients at the 70W power and 5 at the 50W power group. There was no significant difference between the groups regarding age, BE extent, percentage of coagulated esophageal circumference and the duration of symptoms. Pain was the most important symptom, with a mean duration of 10,3 + 9,7 days. When power was compared to symptoms, although not statistically significant, a moderate negative correlation was noted. Endoscopic biopsies showed ablation restricted to the mucosa's superficial layer in 40% of the cases in the lower power group, and only 10% in the higher power group, although deeper layers of the mucosa were compromised. There were no statistical significant differences when comparing the different powers to the penetration through the mucosa's layers and the symptoms. CONCLUSION: There are evidences that the 70W potency argon plasma coagulation for BE leads to a lower incidence of residual specialized columnar metaplasia under the new scamous epithelium.

Racional - A síndrome hepatopulmonar caracteriza-se pela presença de hipoxemia arterial resultante de dilatações vasculares intra-pulmonares em portadores de doença hepática crônica e/ou hipertensão portal. A síndrome, que resulta em hipoxemia arterial por vezes grave, pode alcançar completa resolução após o transplante hepático. Objetivo - Determinar a prevalência da síndrome hepatopulmonar em portadores de cirrose hepática, candidatos a transplante de fígado no Serviço de Transplante Hepático do Hospital de Clinicas da Universidade Federal do Paraná, Curitiba, PR. Pacientes e Métodos - Cinqüenta e quatro pacientes adultos (idade > ou = 18 anos) com cirrose hepática avançada foram submetidos a análise dos gases arteriais em ar ambiente na posição sentada. Os pacientes com hipoxemia arterial (PaO2 < 70 mm Hg) foram avaliados com ecocardiografia com microbolhas e estudo da função pulmonar. Resultados - Sete dos 54 pacientes (12,9%) apresentaram hipoxemia arterial em ar ambiente, embora apenas um deles referisse dispnéia aos esforços. Os sete pacientes apresentavam disfunção hepática variável de acordo com a classificação de Child (A = 1, B = 4, C = 2). Em todos os sete pacientes a ecocardiografia com microbolhas foi positiva, caracterizando a presença de síndrome hepatopulmonar. O estudo da função pulmonar não revelou anormalidades nos sete pacientes hipoxêmicos. Conclusão - A prevalência da síndrome hepatopulmonar em candidatos a transplante hepático é elevada. Essa síndrome nem sempre se acompanha de sintomas respiratórios, devendo ser rastreada rotineiramente no processo de seleção dos candidatos a transplante hepático.

Background - Hepatopulmonary syndrome is an important clinical problem associated with chronic liver disease. Liver transplantation can result in complete resolution of the arterial hypoxemia associated with this syndrome, even in its most severe presentation. Aim - To determine the prevalence of hepatopulmonary syndrome in adult liver transplant candidates. Patients and Methods - Fifty-four consecutives adult patients ( > or = 18 years) with severe liver disease waiting for liver transplant were screenned for arterial hypoxemia (PaO2 <70 mm Hg) while they were seated, at rest, and breathing room air. Patients with arterial hypoxemia underwent contrast-enhanced two-dimensional echocardiography and pulmonary function testing. Hypoxemia (PaO2 <70 mm Hg) was present in 7 of 54 patients (12,9%), although only 1 of them complained of dyspnea. The Child's classification of the patients were: A = 1, B = 4, and C = 1. All seven hypoxemic patients had positive contrast-enhanced two-dimensional echocardiography, consistent with hepatopulmonary syndrome. Conclusion - The prevalence of hepatopulmonary syndrome in adult liver transplant candidates is elevated. The screenning for hepatopulmonay syndrome should be part of the routine evaluation of liver transplant candidates even in the absence of pulmonary symptoms.

Racional - O hepatocarcinoma é uma das doenças malignas mais comuns em todo o mundo. O transplante hepático é boa opção terapêutica para os pacientes com hepatocarcinoma em fase inicial, alcançando índices de sobrevida semelhantes aos encontrados nos pacientes cirróticos transplantados sem malignidade hepática. Objetivo - Avaliar a evolução de pacientes cirróticos com hepatocarcinoma submetidos a transplante hepático no Serviço de Transplante Hepático do Hospital de Clínicas da Universidade Federal do Paraná, Curitiba, PR. Métodos - Estudo retrospectivo dos pacientes cirróticos com hepatocarcinoma submetidos a transplante ortotópico de fígado no período de setembro de 1991 a setembro de 2000. O diagnóstico do tumor foi estabelecido durante os exames pré-transplante em cinco doentes e foi achado incidental no fígado nativo em três. Nos pacientes com diagnóstico pré-operatório de hepatocarcinoma adotou-se como critério de elegibilidade para o transplante, a presença de tumor único de diâmetro <FONT FACE=Symbol>£</FONT> 5 cm ou até três lesões com diâmetro <FONT FACE=Symbol>£</FONT> 3 cm cada, ausência de invasão tumoral da veia porta ou de metástases extra-hepática. Foram avaliados como parâmetros principais a sobrevida do paciente e a ocorrência de recidiva tumoral após o transplante. Resultados - A principal causa de doença hepática pré-transplante foi a hepatite por vírus C (50%). No exame do fígado explantado, a maioria dos pacientes (6/8, 75%) tinha lesão única; um paciente tinha dois nódulos e em outro detectou-se hepatocarcinoma multifocal incidentalmente no fígado nativo. O tamanho do tumor variou de 0,2 a 5,0 cm. Nenhum dos casos apresentou invasão vascular ou linfonodal. Todos os pacientes permaneceram vivos e livres de recurrência tumoral durante o tempo do estudo, sendo a mediana de seguimento de 18,5 meses (variando de 5-29 meses). Conclusão - O transplante hepático é boa opção terapêutica nos pacientes cirróticos com hepatocarcinoma em fase inicial. Com seleção adequada, o transplante hepático oferece excelentes índices de sobrevida livre de recurrência tumoral.

Background - Hepatocellular carcinoma is one of the most common malignancies worldwide. Liver transplantation has emerged as a good option for early-stage hepatocellular carcinoma yielding survival rates as good as for recipients without this type of tumor. Objective - To assess the outcome of cirrhotic patients with hepatocellular carcinoma undergoing liver transplantation at the Liver Transplantation Service of the "Hospital de Clínicas", Federal University of Paraná, Curitiba, PR, Brazil. Methods - Retrospective study of cirrhotic patients with hepatocellular carcinoma undergoing orthotopic liver transplantation at the mentioned Institution between September 1991 and September 2000. The diagnosis of hepatocellular carcinoma was established during the pretransplant workup in five patients and the tumor was an incidental finding in the native liver in three. The indication for liver transplantation was restricted to solitary tumor equal to or less than 5 cm or up to 3 nodules, with each nodule measuring less than 3 cm, and no evidence of vascular invasion or extrahepatic spread. Patient survival and evidence of tumoral recurrence posttransplant were evaluated. Results - The most common cause for pretransplantation liver disease was hepatitis C virus (50%). On examination of the explanted liver, the majority of patients (6/8, 75%) had a single lesion; one patient had two nodules and one had a multifocal hepatocellular carcinoma found incidentally in the native liver. Tumor size ranged from 0,2 to 5,0 cm. All cases had neither vascular invasion nor linfonodal envolvement. All patients remained alive and free of tumor recurrence at the time of the study with a mean follow-up of 18,5 months (range, 5-29 months). Conclusion - Liver transplantation is a good therapeutic option for early stage hepatocellular carcinoma arising in cirrhotic patients. With proper selection, liver transplantation can offer excelent survival rates free of tumor recurrence.
