ABSTRACT Ethical dilemmas regarding end-of-life care are the most delicate and important in contemporary medical practice; this care goes beyond the field of bioethics due to legal, social, cultural and religious implications. Regardless of the universality of bioethical postulates, they must be contextualized taking into account the characteristics of each nation. The demographic and epidemiological panorama of Cuba makes the problem behave similarly to the developed world, which motivated us to make a theoretical approach to the practice of limiting the therapeutic effort. For this purpose, we start from the ethical principles of Cuban nationality, the social-demographic scenario, the current legal framework and the health policy in the context of the economic transformations that the country is experiencing. This therapeutic strategy is not related to euthanasia and it allows dysthanasia to be removed from the clinical setting. At the same time, it constitutes a starting point towards palliative care with an economic impact and in the efficient organization of potentially beneficial health services. There is a favorable movement in the national literature regarding this medical practice; but the main weakness in the country is the lack of a legal framework that condemns dysthanasia and it recognizes the limitation of therapeutic effort as the measure that avoids it.

RESUMEN Los dilemas éticos respecto al cuidado al final de la vida son los más delicados e importantes en la práctica médica contemporánea; rebasan el campo de la bioética por sus implicaciones legales, sociales, culturales y religiosas. Independientemente de la universalidad de los postulados bioéticos, estos deben ser contextualizados atendiendo las características propias de cada nación. El panorama demográfico y epidemiológico de Cuba hace que la problemática tenga un comportamiento similar al mundo desarrollado, lo cual nos motivó a realizar un acercamiento teórico a la práctica de la limitación del esfuerzo terapéutico. Para ello partimos de los principios éticos de la nacionalidad cubana, el escenario social-demográfico, el marco legal vigente y la política de salud en el contexto de las transformaciones económicas que vive el país. Esa estrategia terapéutica no guarda relación alguna con la eutanasia y permite retirar del escenario clínico la distanasia. A la vez, constituye un punto de partida hacia los cuidados paliativos con impacto en lo económico y en el ordenamiento eficiente de los servicios de salud potencialmente beneficioso. Existe en la literatura nacional un movimiento favorable respecto a esa práctica médica; pero la principal debilidad en el país es la falta de un marco legal que condene la distanasia y reconozca la limitación del esfuerzo terapéutico como la medida que la evita.

ABSTRACT The autopsy is the most complete study of the patient and the disease; from its results derive a series of scientific-educational events in medical education, which means that general clinical-pathological meetings and fresh pieces are essential for the training of health professionals, because they contribute to the quality of care of the services, they reduce the clinical-pathological discrepancies to the minimum degree and point out the deviations of the treatments and the possible appearance of new or unexpected diseases. The exchange carried out is of great medical care, teaching and scientific importance, that is why it is necessary to insist on the teaching methods to be used in order to continue raising their quality in the acquisition of knowledge and skills needed to develop the scientific thinking that links the school with life.

RESUMEN La autopsia es el estudio más completo del enfermo y la enfermedad; de sus resultados derivan una serie de eventos científico-docentes en la enseñanza médica, lo que significa que las reuniones clínico-patológicas generales y de piezas frescas, son indispensables para la formación de los profesionales sanitarios, porque contribuyen a la calidad asistencial de los servicios, reducen al grado mínimo las discrepancias clínico-patológicas y señalan las desviaciones de los tratamientos y la posible aparición de patologías nuevas o no esperadas. El intercambio realizado tiene gran importancia asistencial, docente y científica, por lo que se hace necesario insistir en los métodos de enseñanza a utilizar con el objetivo de continuar elevando su calidad en la adquisición de conocimientos y habilidades necesarios para desarrollar el pensamiento científico que vincula la escuela con la vida.

ABSTRACT Background: the control of transmittable diseases contributes to the illegitimate use of biological agents, initially for war or criminal purposes and currently for terrorist purposes. Objective: to describe the problem of the use of biological weapons, its different edges and in its current form for terrorist purposes and the aspects that prohibit its use according to current international regulations. Methods: a bibliographic review was made with the bibliographic analysis method with a search conducted between 2000 and 2017, where specialized databases were consulted and 31 scientific publications on the subject were selected. Development: an analysis on the issue of biological warfare was made based on the agents used and the threat that constitutes their use, since they cause diseases and cause death by affecting the health of living beings and the environment. They are harmful and include knowledge of science, technology, engineering and involve other areas, such as economics, law, the legal framework that limits or not the obtaining of materials and the psychological aspects of such attacks. The technological advances in Biology and Genetic engineering of recent decades make it possible to modify and create new microorganisms that are more resistant to treatment and in particular in the search for the inoculation of diseases such as cancer. The biodefense is the set of measures for the prevention and action facing a sanitary alert where the use of the biological agents with war aims is implied. Conclusions: biological agents are used as weapons to cause damage in circumstances of biological warfare and bioterrorism, as it is potentially the most destructive weapon known to humanity that involves political, economic, scientific-technological, legal and psychological aspects, which is why biodefense protects people and the environment with the use of sanitary measures against above-mentioned agents.

RESUMEN Fundamento: el control de enfermedades transmisibles contribuye al empleo ilegítimo de los agentes biológicos, de manera inicial con fines bélicos o criminales y en la actualidad con fines terroristas. Objetivo: describir la problemática del empleo de las armas biológicas, sus diferentes aristas y en su forma actual con fines terroristas y los aspectos que prohíben su uso según la normativa internacional vigente. Métodos: se realizó una revisión bibliográfica con el método de análisis bibliográfico con una búsqueda realizada entre los años 2000 al 2017, donde se consultaron bases de datos especializadas y se seleccionaron 31 publicaciones científicas sobre el tema. Desarrollo: se realizó un análisis del tema de la guerra biológica basado en los agentes utilizados y la amenaza que constituye su empleo, pues provocan enfermedades y causan la muerte al afectar la salud de los seres vivos y el medio ambiente. Son dañinas e incluyen conocimientos de ciencia, tecnología, ingeniería e involucran otras áreas, como la economía, el derecho, el marco jurídico que limita o no la obtención de materiales y los aspectos psicológicos ante tales ataques. Los avances tecnológicos en Biología e Ingeniería genética de las últimas décadas posibilitan modificar y crear nuevos microorganismos más resistentes al tratamiento y en particular en la búsqueda de inoculación de enfermedades como el cáncer. La biodefensa es el conjunto de medidas para la prevención y actuación frente a una alerta sanitaria donde está implicado el uso de los agentes biológicos con fines bélicos. Conclusiones: los agentes biológicos se utilizan como armas para ocasionar daños en circunstancias de guerra biológica y bioterrorismo, por ser de forma potencial el arma más destructiva conocida por la humanidad que implica aspectos políticos, económicos, científico-tecnológicos, jurídicos y psicológicos, por lo que la biodefensa protege a las personas y el medio ambiente con el uso de medidas sanitarias contra dichos agentes.

Perioperative hypertension is defined as blood pressure values higher than 140/90 mm Hg in pre-, trans- or post-operatory. Its incidence as a reason for cancellation or delay of surgical procedure motivated this research. An observational cross-sectional study was aimed at characterizing perioperative hypertension at «Cmdte. Manuel Fajardo Rivero» Hospital of Villa Clara from January, 2010 to December, 2014. Universe was constituted by patients who required major surgery; a simple random sample of the 30 % was selected. Perioperative hypertension was more common in women (64.6 %); patients with history of blood pressure and diabetes mellitus had 7.6 and 7.8 more probabilities of suffering from perioperative hypertension than those without a family history of these diseases; it was 14.4 times more frequent in patients undergoing emergency surgery and in the 5.8 operated with general anesthesia. Evaluated variables were associated with perioperative hypertension in the studied patients, which will allow us to improve medical intervention with a preventive approach

La hipertensión perioperatoria se define como las cifras tensionales superiores a 140/90 mm/Hg en el pre-, trans- o postoperatorio. Su incidencia como causa de dilación o suspensión del acto quirúrgico motivó esta investigación. Se realizó un estudio observacional, de corte transversal, cuyo objetivo fue caracterizar la hipertensión perioperatoria en el Hospital «Cmdte. Manuel Fajardo Rivero» de Villa Clara, entre enero de 2010 y diciembre de 2014. El universo estuvo constituido por pacientes que requerían cirugía mayor; se seleccionó una muestra simple aleatoria del 30 %. La hipertensión perioperatoria fue más común en las mujeres (64,6 %); los pacientes que tenían antecedentes de hipertensión arterial y diabetes mellitus presentaron 7,6 y 7,8 veces más probabilidades de padecer de hipertensión perioperatoria que aquellos que no los tenían, y fue 14,4 veces más frecuente en operados de urgencia y en el 5,8 con anestesia general. Las variables evaluadas se asociaron a la hipertensión perioperatoria en los pacientes estudiados, lo que permitirá mejorar la actuación médica con enfoque preventivo

Resumen Objetivo: Determinar la prevalencia de Síntomas Físicos Medicamente No Explicables y las características y uso de los servicios de salud entre el grupo de pacientes con Síntomas Físicos Medicamente No Explicables, y el grupo con otras enfermedades. Métodos: Estudio transversal, retroprospectivo y multicéntrico. Se incluyeron a 1,043 pacientes mayores de 18 años, en 30 unidades de atención primaria de una institución gubernamental en salud, en 11 estados de la República Mexicana, atendidos por 39 médicos familiares. Se estimó la prevalencia de Síntomas Físicos Medicamente No Explicables y se compararon los dos grupos con y sin estos síntomas, en cuanto al uso de medicamentos, estudios de laboratorio, de gabinete, incapacidades y referencias en los últimos seis meses. El grupo de Síntomas Físicos Medicamente No Explicables fue diagnosticado por el Patient Health Questionnaire (son los síntomas físicos más comúnmente referidos por estos pacientes en el primer nivel de atención), además de criterios diagnósticos de Reid et al. Se excluyeron urgencias o con enfermedad terminal. Se utilizó prueba Chi cuadrada con p <0.05 para significancia estadística. Resultados: El 7.0% (73) se diagnosticó como Síntomas Físicos Medicamente No Explicables, la mayoría mujeres (91.8%); el síntoma predominante pertenece al sistema gastrointestinal con 76.7% (56). Este grupo demandó mayor uso de estudios de gabinete y referencias a otros servicios (media 1.1 vs. 0.5; p <0.0001 y 0.8 vs 0.6; p <0.01, respectivamente). Conclusiones: La prevalencia de Síntomas Físicos Medicamente No Explicables fue baja, pero con impacto significativo en el uso de algunos servicios de salud. Esto pudiera representar un mayor costo comparado con otro grupo de pacientes.

Abstract Objective: To determine the prevalence of medically unexplained physical symptoms and the characteristics and use of health services in a group of patients with medically unexplained physical symptoms and a group of patients with other illnesses. Methods: This was a cross-sectional, retrospective and multicenter study. We included 1,043 patients over 18 years of age from 30 primary care units of a government health institution, in 11 states of Mexico, attended by 39 family physicians. The prevalence of medically unexplained physical symptoms was determined and both groups with or without symptoms were compared with regard to drug use, laboratory and other studies, leaves of absence, and referrals in the last six months. The group with medically unexplained physical symptoms was diagnosed using the Patient Health Questionnaire and the diagnostic criteria of Reid et al. Emergency or terminal illnesses were excluded. The chi square test was used with a statistical significance of p < 0.05. Results: Medically unexplained physical symptoms was diagnosed in 73 patients (7.0%). The majority were women (91.8%); their predominant symptom was from the gastrointestinal system in 56 (76.7%). This group had a greater use of clinical studies and referrals to other services (mean 1.1 vs. 0.5; p <0.0001 and 0.6 vs. 0.8; p < 0.01, respectively). Conclusions: The prevalence of medically unexplained physical symptoms was low, but with a greater impact on some health services. This could represent an overload in medical costs.
