Association between multimorbidity, intensive care unit admission, and death in patients with COVID-19 in Brazil: a cross-section study, 2020

ABSTRACT BACKGROUND: Multimorbidity can influence intensive care unit (ICU) admissions and deaths due to coronavirus disease (COVID-19). OBJECTIVE: To analyze the association between multimorbidity, ICU admissions, and deaths due to COVID-19 in Brazil. DESIGN AND SETTING: This cross-sectional study was conducted using data from patients with severe acute respiratory syndrome (SARS) due to COVID-19 recorded in the Influenza Epidemiological Surveillance Information System (SIVEP-Gripe) in 2020. METHODS: Descriptive and stratified analyses of multimorbidity were performed based on sociodemographic, ventilatory support, and diagnostic variables. Poisson regression was used to estimate the prevalence ratios. RESULTS: We identified 671,593 cases of SARS caused by COVID-19, of which 62.4% had at least one morbidity. Multimorbidity was associated with male sex, age 60–70 and ≥ 80 years, brown and black skin color, elementary education and high school, ventilatory support, and altered radiologic exams. Moreover, all regions of the country and altered computed tomography due to COVID-19 or other diseases were associated with death; only the northeast region and higher education were associated with ICU admission. CONCLUSION: Our results showed an association between multimorbidity, ICU admission, and death in COVID-19 patients in Brazil.

Abstract Aim: This study aimed to explore the effects of a training program during the pre-season on aerobic and anaerobic performance, hemodynamics, autonomic variables, and sleep quality in youth soccer players. Methods: Nineteen athletes, with an average age of 17 ± 1 years, participated in the study. The multicomponent training protocol was divided into technical, tactical, and physical practice for four weeks. The cardiac autonomic modulation was obtained through an electrocardiogram and blood pressure values were measured by a sphygmomanometer. The athletes answered the Pittsburgh questionnaire that assessed sleep patterns and issues. The VO2max was analyzed using the Intermittent Recovery Test Yo-Yo level 1. The RAST test was used to assess anaerobic power. Results: There was improvement in Heart Rate Variability (HRV) indicated by the increase in indexes, mean square root of the differences between normal cycles (RMSSD), low frequency increase (LF) (p = 0.04; d = 0.70), high frequency decrease (HF) (p = 0.01; d = 1.02) and the LF / HF sympathovagal index (p = 0.03; d = 0.70), variables related to faster recovery. An improvement in the components of sleep duration (p = 0.03) and quality (p = 0.02) of baseline and post-intervention sleep was also observed. Conclusion: The four-week multicomponent protocol contributed to improving VO2max, improving fatigue rates, quality of sleep, and maximum power. Additionally, we observe that youth soccer athletes had physiological and hemodynamic adaptations that resulted in an improvement in cardiac autonomic modulation and sleep patterns after four weeks of training.

New species of the Corumbataia cuestae group (Siluriformes: Loricariidae) from the Rio Tocantins basin, with comments on its phylogenetic relationships

Resumo Uma nova espécie de Corumbataiaé descrita para o Rio Maranhão, na bacia do Rio Tocantins na região central do Brasil. A nova espécie é diagnosticada dos demais congêneres pela presença de uma pequena área nua na ponta do focinho; por possuir o abdômen coberto por pequenas placas formando um escudo que alcança as placas laterais mid-ventrais; perfil anterior da cabeça arredondado em vista dorsal; lábio inferior não alcançando a linha transversal da cintura peitoral; e presença de 28 ou 29 vértebras. Altos valores de divergência genética também suportam a validade dessa nova espécie. Nossa análise filogenética encontrou um subclado derivado em Corumbataia, aqui denominado grupo Corumbataia cuestae, composto pela nova espécie mais C. cuestae, C. tocantinensis, C. britskii, C. liliai e C. lucianoi. Esse grupo é definido por possuir uma crista conspícua de odontódeos hipertrofiados na cabeça; ausência de nadadeira adiposa ou série única de placas na posição da nadadeira adiposa; anastomose dos canais sensoriais infraorbital e ótico sobre o pterótico-supracleitro. Aqui, nós também restringimos a distribuição de C. tocantinensisà bacia do Rio Araguaia.
Abstract A new species of Corumbataia is described from Rio Maranhão, Rio Tocantins basin, central Brazil. The new species is distinguished from all congeners by the presence of a small, naked area on snout tip; by having the abdomen covered with small platelets forming a shield which reaches the lateral mid-ventral plates; by the anterior profile of the head rounded in dorsal view; by the lower lip not reaching the transversal line of the pectoral girdle; and by the presence of 28 or 29 vertebrae. High genetic divergence in mitochondrial cytochrome c oxidase subunit I (COI) further supports the validity of this new species. Our phylogenetic analysis shows a derived subclade in Corumbataia, herein named as the Corumbataia cuestae group, composed of the new species plus C. cuestae, C. tocantinensis, C. britskii, C. liliai, and C. lucianoi. This group is defined by having a conspicuous crest of hypertrophied odontodes on head; absence of the adipose fin or a single series of platelets at adipose-fin position; and anastomosis of the infraorbital and otic sensory canals over the pterotic-supracleithrum. Here we also restrict the distribution of C. tocantinensis to the Rio Araguaia basin.

Abstract Biflorin (6,9-dimethyl-3-(4-methylpent-3-en-1-yl) benzo[de]chromene-7,8-dione) is a promising substance that has been increasingly studied in the past decades due to its diverse pharmacological properties (i.e. antitumor, antioxidant, antiinflamatory, antimicrobial activity etc.). Aiming the comprehension of its antitumoral activity we investigated the cell proliferation and cytotoxicity habilities of biflorin against mice splenocytes Balb/c. Biflorin was able to stimulate mice splenocytes Balb/c in 48 h of incubation at a concentration of 20.2 µM. Its immunostimulation promoted the production of cytokines such as: TNF-α, IFN-γ, IL-2, IL-6 and IL-17, inducing the immune profile toward a Th1 response. Moreover, an original method which led to an excellent yield with less processing time compared to the methods described in the literature was developed to obtain biflorin, from sawdust of Capraria biflora L., Scrophulariaceae. This method shows a great potential of increasing the production of this pharmacological active compound.

A contingent valuation study of buriti ( Mauritia flexuosa L.f.) in the main region of production in Brazil: is environmental conservation a collective responsibility?

ABSTRACT The immature leaves of the buriti palm (Mauritia flexuosa) are widely harvested in the municipality of Barreirinhas, Maranhão, for the production of handicrafts, which are sold to locals and tourists. The increasing demand for these artisanal goods is stimulating the emergence of an informal market for immature buriti leaves, leading to an intensification of their extraction and resulting in negative effects on local buriti palm populations and the ecosystem. Thus, the objective of the present study was to assess the environmental value of the buriti palm tree based on the maximum willingness to pay (WTP) for its conservation, using the contingent valuation method. Among the respondents, 99.74% reported that the palm species should be protected and the majority of them (65.75%) agreed to pay for its conservation (annual WTP R$ 179.49 ± 222.05). Multivariate analysis revealed that the WTP was not influenced by the socio-economic profile of the respondents. The main reasons for non-WTP are related to dissatisfaction with the government and the belief that financial contributions would not solve problems of environmental damage. Overall, the evaluated population believes that environmental conservation is not an obligation of the government or that of the population, but is a collective responsibility.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Importantes agentes para defesa contra armas de guerra química são reativadores da acetilcolinesterase humana (huAChE) inibida por compostos organofosforados neurotóxicos (OP), e precisam de uma permeabilidade razoável pela barreira hematoencefálica (HB). Neste trabalho, oximas neutras, que penetram HB melhor do que as oximas catiônicas atualmente utilizadas como agentes de defesa, foram testadas como reativadores da huAChE inibida com paraoxon usando o método de Ellman modificado e pralidoxima (2-PAM) como padrão positivo. A oxima neutra mais ativa foi a 2-tiofenoaldoxima, que reativou 93% da huAChE inibida, sendo 12% mais eficiente do que a 2-PAM. Os resultados mostraram que oximas neutras simples possuem potencial para atuarem como antídotos para intoxicação com OPs neurotóxicos, sugerindo maior investigação no desenvolvimento de agentes de defesa neutros.
Important defense agents against chemical warfare weapons, which are reactivators of human acetylcholinesterase (huAChE) inhibited by neurotoxic organophosphorus compounds (OP), need a reasonable permeation of the hematoencephalic barrier (HB). In this work, neutral oximes, which permeate HB better than the cationic oximes currently used as defense agents, were tested as reactivators of huAChE inhibited with paraoxon using the modified Ellman test with pralidoxime (2-PAM) as positive standard. The most active neutral oxime was (thiophen-2-yl)aldoxime, which reactivated 93% of the inhibited huAChE and was 12% more effective than 2-PAM. The results showed that simple neutral oximes have potential to function as antidotes for intoxication with neurotoxic OPs, suggesting further research on the development of neutral defense agents.

As escalas de percepção de esforço de Borg e OMNI são amplamente validadas para identificação da intensidade do exercício. O estudo teve como objetivo testar a correspondência entre as categorias das Escalas de Borg (6-20) e OMNI-Ciclismo na prescrição de exercícios em cicloergômetro e propor uma nova tabela de conversão entre as escalas. Vinte seis homens (17-41 anos de idade), praticantes de Ciclismo Indoor, pedalaram por 3 minutos, em 6 cargas auto selecionadas correspondentes às categorias de percepção de esforço 9, 11, 13, 15, 17, 19 para Borg, em uma sessão e 2, 4, 5, 7, 8 e 10 para OMNI, em outra sessão. Ao final de cada estágio, a frequência cardíaca (FC) e a potência (W) foram registradas. Calculou-se a correlação de Pearson entre as escalas. A correspondência entre as categorias das escalas foi testada pela ANOVA para medidas repetidas, seguida do teste de Tuckey (p < 0,05). Foi encontrada alta correlação entre as duas escalas (r = 0,87; P < 0,05) e diferença significativa na FC para as três primeiras categorias. Quanto a W, encontrou-se diferença significativa apenas nas duas primeiras categorias. A conversão testada mostrou correlação significante. Entretanto, há diferenças significantes na FC para as três primeiras categorias e na W para as duas primeiras. A seguinte correspondência foi proposta: Borg 11, 13, 15, 17 e 19; para OMNI 2, 4, 7, 8 e 10.
The Borg and OMNI scales of perceived exertion have been widely validated for the identification of exercise intensity. The objective of this study was to test the agreement between the categories of the Borg (6-20) and OMNI-Cycle scales for the prescription of cycle ergometer exercise and to propose a new conversion table. Twenty-six male indoor cyclists (17-41 years) pedalled for 3 min at six self-selected increasing loads. These loads corresponded to categories 9, 11, 13, 15, 17, and 19 of the Borg scale in one session, and to categories 2, 4, 5, 7, 8, and 10 of the OMNI scale in the other session. At the end of each stage, heart rate (HR) and power (W) were recorded. Pearson's correlation coefficient between scales was calculated. Agreement between the categories of the scales was tested by ANOVA for repeated measures, followed by the Tukey test (p < 0.05). The correlation between the two scales was high (r = 0.87, p < 0.05). Significant difference between the two scales was observed for HR in the first three categories. W differed significantly only in the first two categories. The conversion tested showed a significant correlation. However, there were significant differences in HR in the first three categories and in W in the first two. The following agreement is proposed: Borg 11, 13, 15, 17, and 19 corresponding to OMNI 2, 4, 7, 8, and 10.

O impacto do uso de agrotóxicos sobre a saúde humana é um problema que tem merecido atenção da comunidade científica em todo o mundo, sobretudo nos países em desenvolvimento. O consumo de agrotóxicos na região sudeste do Brasil está estimado em 12kg de agrotóxico/trabalhador/ano. Em algumas áreas agrícolas do Estado do Rio de Janeiro, como na região da Microbacia do Córrego de São Lourenço, Nova Friburgo, o consumo de agrotóxico foi estimado em 56kg de agrotóxico/trabalhador/ano. Elevados níveis de contaminação humana e ambiental foram encontrados nesta região, como decorrência do uso extensivo destes agentes químicos. A avaliação do impacto sobre a saúde humana implica o conhecimento e a visualização da importância/magnitude relativa de cada uma das vias de contaminação. Inúmeros fatores, que, em geral, encontram-se inter-relacionados, contribuem para a situação encontrada na Microbacia do Córrego de São Lourenço e a forma mais adequada de se avaliar toda a dimensão deste problema é o uso de uma abordagem integrada.
Environmental and human difficulties caused by pesticides used in agriculture are important health issues worldwide, specially in the developing countries. Pesticide consumption in Southeast Brazil is estimated in 12kg/worker/year. In the São Lourenço Valley, Rio de Janeiro State, pesticide consumption is estimated at some 56kg/worker/year. High levels of human and environmental contamination caused by these products were found in the area. Many factors were shown as determinants of this situation, such as: pesticides with different formulations used with insufficient or even non-existent supervision or technical advice; intensive marketing activities place the responsibility for correct use and disposal of these chemicals on farm workers with low education levels typical of rural areas in Brazil, resulting in widespread human and environmental contamination. The evaluation of those loosely inter-related factors help shape the current situation in the São Lourenço Valley, indicating that the only way to really understand and assess the problem as a whole is through an integrative (transdisciplinary) approach.
