Abstract Objective to understand the perceptions of councilors about the functioning of councils for the rights of older people in the state of Rio Grande do Norte. Method This is an analytical cross-sectional study with a quantitative approach carried out with municipal councilors for the rights of older people in the state of Rio Grande do Norte. 109 councilors participated, with a predominance of full members, female, from government representation and with a higher education level. Results Several challenges were identified for the performance and operation of the councils, among which stand out: isolated actions, low participation of government members and representation of civil society, little time for discussion in meetings and the need for valid instruments to monitor the actions of the councils. Low technical knowledge of the councilors and aversion to divergences and political positions were also evidenced. Conclusion it is essential to ensure intersectoral policies, greater independence from the executive and, above all, greater training of councilors and democratic political strengthening so that they can not only promote, but exercise a socio-political protagonism based on a collective construction and social representation.

Resumo Objetivo compreender as percepções dos conselheiros acerca do funcionamento dos conselhos dos direitos da pessoa idosa no estado do Rio Grande do Norte. Método Trata-se de um estudo de natureza transversal analítica com abordagem quantitativa realizado com os conselheiros municipais dos direitos da pessoa idosa do estado do Rio Grande do Norte. Participaram 109 conselheiros, havendo predominância de membros titulares, do sexo feminino, de representação governamental e com nível de escolaridade superior. Resultados Foram apontados diversos desafios para atuação e funcionamento dos conselhos, dentre os quais destacam-se: ações isoladas, baixa participação de membros governamentais e representação da sociedade civil, pouco tempo de discussão nas reuniões e necessidade de instrumentos válidos para monitorar as ações dos conselhos. Evidenciaram-se também baixo conhecimento técnico dos conselheiros e aversão às divergências e posicionamentos políticos. Conclusão é imprescindível assegurar políticas intersetoriais, maior independência perante o executivo e, prioritariamente, maior capacitação dos conselheiros e fortalecimento político democrático para que possam não só promover, mas exercer um protagonismo sociopolítico a partir de uma construção coletiva e representatividade social.

Abstract Objective to identify the prevalence of multimorbidity in elderly people living in rural areas and associated sociodemographic, behavioral and clinical-therapeutic factors. Method cross-sectional study, conducted with randomly selected elderly residents in the rural area of Araçagi, Paraíba, Brazil, linked to the Family Health Strategy. The dependent variable of the study was multimorbidity, understood as the presence of two or more chronic conditions in a single individual. Data were collected through a sociodemographic questionnaire and a form about self-reported health problems, addressing 32 conditions. Univariate and bivariate statistics and Poisson regression were used in the data analysis, considering significant when p-value <0.05. Results 360 elderly subjects participated in the study, with a 54.2% (95%CI: 49,0–59,3) prevalence of multimorbidity. After regression, it was identified that female gender (PR=1,16; 95%CI: 1,09–1,25), age ≥70 years (PR=1,08; 95%CI: 1,01–1,15), overweight (PR=1,19; 95%CI: 1,10–1,29), access to treated and piped water (PR=1,09; 95%CI: 1,00–1,18), smoking history (PR=1,10; 95%CI: 1,03–1,17), not using alcohol (PR=1,13; 95%CI: 1,05–1,22), chronic pain (PR=1,18; 95%CI: 1,10–1,26), hospital as first choice of health service (PR=1,12; 95%CI: 1,03–1,21) and medical consultation in the last year (PR=1,19; 95%CI: 1,11–1,27) were factors associated with multimorbidity. Conclusion the study revealed a high prevalence of multimorbidity and its associated factors. Finally, it becomes feasible to plan measures that improve the health condition of these individuals and think of possibilities to promote healthy aging.

Resumo Objetivo identificar a prevalência de multimorbidade em pessoas idosas residentes na zona rural e os fatores sociodemográficos, comportamentais e clínico-terapêuticos associados. Método estudo transversal, realizado com idosos residentes na zona rural de Araçagi, Paraíba, Brasil, vinculados à Estratégia Saúde da Família e selecionados aleatoriamente. A variável dependente do estudo foi a multimorbidade, entendida como a presença de duas ou mais condições crônicas em um só indivíduo. Os dados foram coletados através de um questionário sociodemográfico e um formulário sobre problemas de saúde autorreferidos, abordando 32 condições. Na análise dos dados, utilizou-se a estatística univariada, bivariada e regressão de Poisson. Resultados participaram do estudo 360 idosos com prevalência de multimorbidade de 54,2% (IC95%: 49,0–59,3). Após regressão, identificou-se que sexo feminino (RP=1,16; IC95%: 1,09–1,25), idade ≥ 70 anos (RP=1,08; IC95%: 1,01–1,15), sobrepeso (RP=1,19; IC95%: 1,10–1,29), acesso a água tratada e encanada (RP=1,09; IC95%: 1,00–1,18), histórico de tabagismo (RP=1,10; IC95%: 1,03–1,17), não fazer uso de álcool (RP=1,13; IC95%: 1,05–1,22), dor crônica (RP=1,18; IC95%: 1,10–1,26), hospital como primeira opção de serviço de saúde (RP=1,12; IC95%: 1,03–1,21) e consulta médica no último ano (RP=1,19; IC95%: 1,11–1,27) eram fatores associados à multimorbidade. Conclusão o estudo revelou alta prevalência de multimorbidade e seus fatores associados. Por fim, torna-se viável o planejamento de medidas que melhorem a condição de saúde desses indivíduos e pensar em possibilidades de promover um envelhecimento saudável.

Resumo Objetivo Discutir as medidas de prevenção da covid-19 no contexto da vulnerabilidade das pessoas idosas institucionalizadas e analisar o apoio social ofertado às Instituições de Longa Permanência para Idosos durante a pandemia. Método Pesquisa qualitativa realizada com trabalhadores de 24 instituições filantrópicas do Rio Grande do Norte. Os conceitos de vulnerabilidade em saúde e apoio social embasaram a organização e discussão dos dados submetidos à análise temática. Resultados As instituições adotaram, parcialmente, as medidas de prevenção recomendadas pela Agencia Nacional de Vigilância Sanitária, em evidência: cancelamento de visitas, uso de Equipamentos de Proteção Individual e limpeza dos ambientes. Em sua maioria, as instituições priorizaram o controle da transmissão viral, secundarizando a redução dos impactos sociopsicológicos referentes ao distanciamento e isolamento social. Foram observadas medidas não recomendadas e sem evidência científica, como o uso de ivermectina. Ainda, a atuação das redes de assistência social e de saúde se efetivou de forma mais integrada, melhorando o apoio social oferecido às instituições na perspectiva da pandemia. O Sistema Único de Saúde se destacou pelas recomendações sanitárias, oferta de insumos e atenção da Estratégia Saúde da Família, enquanto o Sistema Único de Assistência Social atuou de maneira menos expressiva. Conclusão Em geral, as medidas adotadas foram insuficientes para a prevenção da covid-19 diante das suscetibilidades dos idosos institucionalizados. Embora a pandemia tenha ampliado a rede de apoio social e a visibilidade das Instituições de Longa Permanência para Idosos, tornam-se necessários maiores investimentos do poder público para efetivar a redução de vulnerabilidade dessas pessoas idosas.

Abstract Objective Discuss covid-19 prevention measures in the context of the vulnerability of institutionalized elderly people and analyze the social support offered to Long Stay Institutions for the Elderly during the pandemic. Method Qualitative research carried out with workers from 24 philanthropic institutions in Rio Grande do Norte. The concepts of vulnerability in health and social support supported the organization and discussion of data submitted to thematic analysis. Results The institutions partially adopted the prevention measures recommended by the National Health Surveillance Agency, in evidence: cancellation of visits, use of Personal Protective Equipment and cleaning of environments. For the most part, institutions prioritized the control of viral transmission, putting the reduction of socio-psychological impacts related to social distancing and isolation in the background. Measures not recommended and without scientific evidence were observed, such as the use of ivermectin. Also, the performance of social assistance and health networks was carried out in a more integrated way, improving the social support offered to institutions in the perspective of the pandemic. The Unified Health System stood out for its sanitary recommendations, supply of supplies and attention from the Family Health Strategy, while the Unified Social Assistance System acted in a less expressive way. Conclusion In general, the measures adopted were insufficient to prevent covid-19 in view of the susceptibilities of institutionalized elderly people. Although the pandemic has expanded the social support network and the visibility of Long-Stay Institutions for the Elderly, greater investments by the government are necessary to effectively reduce the vulnerability of these elderly people.

Resumo O artigo tem por objetivo contribuir na identificação das condições e tendências de acesso e utilização dos serviços da atenção primária em saúde (APS) pelos idosos brasileiros nos anos de 2008, 2013 e 2019. Um estudo transversal em painéis com análise descritiva dos percentuais e intervalos de confiança das variáveis elencadas, no qual foram resgatados os dados relativos à população idosa investigada no Suplemento Saúde da Pesquisa Nacional por Amostra de Domicílios (PNAD) do ano 2008 e da Pesquisa Nacional de Saúde (PNS) 2013 e 2019. Inicialmente identificou-se que, apesar da APS ter sido o principal serviço procurado mediante a necessidade de assistência, houve uma tendência de redução para esta procura em todas as regiões brasileiras, principalmente entre as mulheres idosas, na faixa etária de 60-69 anos e na população de raça/cor branca. Verificou-se um aumento de 15,2% no quantitativo de domicílios cadastrados na Unidade de Saúde da Família; 4,5% na procura por lugar, serviço ou profissional de saúde; e de 31,4% na população atendida. Os principais fatores de não atendimento foram não conseguir vaga/senha e não ter médico atendendo. E, dentre os investigados, verificou-se uma tendência de crescimento entre os que tinham o diagnóstico de hipertensão arterial sistêmica, diabetes mellitus e multimorbidades.

We performed a cross-sectional panel study with a descriptive analysis of the percentages and confidence intervals of the variables listed, in which data on the elderly population investigated in the Health Supplement of the 2008 National Household Sample Survey (PNAD) and the 2013 and 2019 National Health Surveys (PNS) were obtained. We initially identified that, while PHC was the primary service sought for care needs, this demand tended to decline in all Brazilian regions, among older women, the 60-69 years age group, and whites. The number of households registered with the USF increased 15.2%; the search for a place, service, or health professional by 4.5%; and the population served by 31.4%. The main factors of non-attendance were not being able to get a vacancy/service ticket and doctor unavailability. A growing trend was observed among those diagnosed with Systemic Arterial Hypertension, Diabetes Mellitus, and multimorbidities among those investigated.

Resumo Objetivo caracterizar as denúncias de violência contra pessoas idosas no Disque 100 no Rio Grande do Norte, Brasil, entre dezembro de 2018 e novembro de 2019. Métodos estudo transversal no qual foram analisados descritivamente os dados contidos nos extratos das denúncias de violência efetuadas no Disque 100 e, posteriormente, realizada uma análise espacial exploratória. Foram analisados os municípios de residência das vítimas, tipos de violência, local de ocorrência e os encaminhamentos para a rede de garantia de direitos. Além disso, idade, sexo e raça/cor das vítimas; e idade, sexo, raça/cor dos suspeitos e parentesco com a vítima Resultados No total, 878 denúncias contra 1.014 pessoas idosas foram registradas, a maioria de Natal, RN (46,9%). As denúncias mais prevalentes foram: negligência (77%), violência psicológica (44,9%) e abuso financeiro (19%) e ocorreram predominantemente na residência das vítimas (95,3%), atingindo principalmente as mulheres (66,9%). Os suspeitos das violências foram na maioria os filhos (62,1%) e do sexo feminino (49,3%). Conclusão Os resultados sugerem uma fragilidade do apoio/núcleo familiar e a necessidade da atenção do Estado para o cuidado nesse âmbito. Ademais, o fato de que as principais formas de violência nem sempre deixam marcas físicas alerta a população para a sua contribuição na identificação da violência contra a pessoa idosa. Finalmente, os resultados sugerem o desconhecimento do Disque 100 pelos indivíduos que residem no interior, fazendo-se necessário melhor divulgação dos canais de comunicação de violência para esse público, para uma análise mais realista dessa problemática no estado e um enfrentamento mais efetivo.

Abstract Objective to characterize the reports of violence against older people made via the Dial 100 service in the state of Rio Grande do Norte (RN), Brazil, between December 2018 and November 2019. Methods a cross-sectional study involving a descriptive analysis of data contained in excerpts from reports of violence made via the Dial 100 service was conducted and an exploratory spatial analysis carried out. The cities of residence of the victims, types of violence, place of occurrence and referrals to the Network for Securing Rights were analyzed, along with age, sex and race/color of both victims and suspected perpetrators, besides the relationship between them. Results In total, 878 reports of violence against 1,014 older people were registered, the majority in the city of Natal-RN (46.9%). The most prevalent complaints were neglect (77%), psychological violence (44.9%) and financial abuse (19%). Acts of violence were perpetrated predominantly in victims’ homes (95.3%) and affected mainly women (66.9%). Most suspected perpetrators were children of the victim (62.1%) and female (49.3%). Conclusion The results suggest a weakness in the family support/nucleus and the need for state assistance to deliver care in this context. In addition, the fact that the main forms of violence do not always leave physical evidence highlights the role of the population in helping to identify violence against the older population. Finally, the results suggest a lack of awareness about the Dial 100 service among residents in the interior of the state, pointing to the need for greater dissemination of the channels for denouncing violence against this group, allowing a more accurate analysis of the problem in the state and more effective actions to tackle the issue.

RESUMO: Quinze por cento da população mundial apresenta algum tipo de deficiência. Isso pode ser caracterizado por vários tipos de condições, cada uma com sua complexidade específica. Assim sendo, esta revisão da literatura objetiva identificar recentes estudos científicos, no quinquênio 2014-2019, que descrevem cuidados psicoterapêuticos com pessoas com deficiência. Foram consultados: PubMed, EMBASE, Web of Science, PsycINFO (APA), BVS (Bireme/LILACS) e SciELO. Encontrou-se um total de 1.330 artigos. Após dois processos de seleção, 26 pesquisas corresponderam aos critérios de inclusão. A metodologia de pesquisa predominante foi ensaio clínico randomizado, seguido por estudos de caso e estudos descritivos/qualitativos. Pessoas com deficiência intelectual e/ou física, deficiência visual e/ou auditiva e autismo foram as populações mais estudadas. As técnicas comportamentais prevalecem nos estudos incluídos, com menor incidência de técnicas psicodinâmicas. A psicoterapia favorece inclusive o enfrentamento de situações adversas impostas na vida de qualquer ser humano. Pessoas com deficiência também podem apresentar necessidades psicoterápicas, uma vez que, na realidade de suas vidas, incluem complexidades além de sua deficiência. Existem fortes evidências para o uso da psicoterapia como recurso eficaz para a elaboração terapêutica de problemas relacionados à saúde, à educação e à vida social de pessoas com deficiência, independentemente do tipo de intervenção psicoterápica.

ABSTRACT: Fifteen percent of the world’s population has some form of disability. This can be characterized by several types of conditions, each with its specific complexity. Thus, this literature review aims to identify recent scientific studies, in the 2014-2019 five-year period, which describe psychotherapeutic care for people with disabilities. PubMed, EMBASE, Web of Science, PsycINFO (APA), BVS (Bireme/LILACS) and SciELO were consulted. It was found a total of 1,330 articles. After two selection processes, 26 research met the inclusion criteria. The predominant research methodology was the randomized clinical trial, followed by case studies and descriptive/qualitative studies. People with intellectual and/or physical disabilities, visual and/or hearing disabilities and autism were the most studied populations. Behavioral techniques prevail in the studies included, with a lower incidence of psychodynamic techniques. Psychotherapy also favors coping with adverse situations imposed on the life of any human being. People with disabilities may also have psychotherapeutic needs, since, in the reality of their lives, they also include complexities in addition to their disability. There is strong evidence for the use of psychotherapy as an effective resource for the therapeutic elaboration of problems related to health, education and social life of people with disabilities, regardless of the type of psychotherapeutic intervention.

RESUMO Objetivo: Avaliar as práticas alimentares em menores de um ano de idade, de acordo com as políticas de alimentação e nutrição. Métodos: Estudo transversal descritivo, com dados secundários da Chamada Neonatal no Estado do Rio Grande do Norte. A amostra analisada foi de 837 pares mãe/filho menor de um ano de idade, e observou-se a prevalência de dados do Aleitamento Materno Exclusivo (AME), na primeira hora de vida, parcial e total, assim como dos alimentos ingeridos pelas crianças nas últimas 24 horas anteriores à entrevista. Por meio da análise de probitos, estimaram-se as probabilidades de consumo dos alimentos por idade da criança, em dias. Resultados: A prática de aleitamento na primeira hora de vida foi relatada por 64,8% (IC95% 62,4-70,8) das mães entrevistadas e, ao final do primeiro ano de vida, 60% (IC95% 56,41-63,07) das crianças ainda estavam sendo amamentadas. A mediana de aleitamento exclusivo foi de 63 dias (IC95% 60-67). Água ou chá, alimentos lácteos, frutas, legumes e verduras foram introduzidos precocemente, com medianas menores que 180 dias. Observou-se por análise de probitos que o consumo de leite materno tendeu a diminuir e o de alimentos a aumentar, de acordo com a idade da criança, com aumento exponencial do grupo “alimentos não saudáveis”. Conclusões: Apesar de a maioria das crianças ser amamentada até um ano de vida, poucas estavam em aleitamento exclusivo. Alimentos foram introduzidos precocemente, com aumento do consumo dos não saudáveis, resultando em inadequação da qualidade alimentar frente ao preconizado pelas políticas públicas de alimentação e nutrição.

ABSTRACT Objective: To evaluate the feeding practices for infants under one year of age, according to food and nutrition policies. Methods: This is a descriptive cross-sectional study based on secondary data from the Chamada Neonatal project (research on prenatal, childbirth, and infant care) in the state of Rio Grande do Norte. The sample analyzed comprised 837 mother/child (under one year of age) pairs. We found a prevalence of data on exclusive breastfeeding (EBF) in the first hour of life - partial and total -, as well as on food consumed by children 24 hours prior to the interview. We estimated the probability of consumption according to the child’s age in days using the probit analysis. Results: Among the interviewed mothers, 64.8% (95%CI 62.4-70.8) declared breastfeeding in the first hour of life, and 60% (95%CI 56.41-63.07) of the children were still breastfed at the end of their first year of life. The median duration of EBF was 63 days (95%CI 60-67). Water or tea, dairy products, fruits, and vegetables were introduced early, with medians lower than 180 days. The probit analysis revealed that the consumption of breast milk tended to decrease and food intake to increase as the child gets older, with exponential growth in the “unhealthy food” group. Conclusions: Although most children were breastfed up to one year of life, few did so exclusively. Foods were introduced early, with increased consumption of unhealthy ones, resulting in inadequate dietary quality according to recommendations from food and nutrition public policies.

RESUMO OBJETIVO Objetivou-se realizar uma análise das internações dos idosos por condições sensíveis à atenção primária (ICSAP) e fatores contextuais associados em idosos com 60 anos ou mais, residentes em municípios da Região Nordeste. MÉTODOS Caracterizou-se por ser um estudo ecológico utilizando dados do Sistema de Informação Hospitalar (SIH) e do Sistema de Informação da Atenção Básica (Siab) referentes a idosos com 60 anos ou mais. RESULTADOS A taxa total de internação foi de 527,524, sendo em maior quantidade aquelas por insuficiência cardíaca, seguidas das doenças cerebrovasculares e, em terceiro, as gastroenterites infecciosas. Analisando as taxas de ICSAP com os fatores contextuais, todas foram significativas. Em relação à cobertura da atenção básica, ocorreu uma similaridade entre eles, e para a taxa do número de consultas entre idosos, apesar do maior número destas nos municípios com maiores taxas de internação, não existiu diferença significativa entre eles. CONCLUSÃO Concluímos que os fatores contextuais interferem nas condições dessa internação, necessitando, além da melhoria da atenção primária, uma melhoria nas condições de vida da população idosa.

SUMMARY OBJECTIVE The objective of this study was to analyze the hospitalizations of the elderly for conditions sensitive to primary care (ICSAP) and associated contextual factors, referring to elderly people aged 60 and over, living in municipalities in the Northeast region. METHODS Characterized as being an ecological study using data from the Hospital Information System (SIH) and the Basic Attention Information System (SIAB) referring to elderly people aged 60 and over. RESULTS The total hospitalization rate was 527,524, with the highest number due to heart failure, followed by cerebrovascular diseases, and infectious gastroenteritis. Analyzing the ICSAP rates with the contextual factors, all were significant. Regarding the coverage of basic care, a similarity occurred between them, and for the rate of the number of consultations among the elderly, despite the greater number of these in the municipalities with higher hospitalization rates, there was no significant difference between them. CONCLUSION We conclude that the contextual factors interfere in the conditions of this hospitalization, necessitating, besides the improvement of primary care, an improvement in the living conditions of the elderly population.

Abstract: The distribution of harms to health varies spatially determined by the socioeconomic conditions of the environment. This research aimed to assess the spatial distribution of dental caries in 12-year-old children and their correlation with socioeconomic indicators in Brazilian states. The sample of this ecological study comprised all the 26 Brazilian states and the Federal District. Thematic and correlation maps were constructed in order to assess the spatial dependency, as well as the correlation between dental caries and socioeconomic factors. The results showed that the states with the worst DMFT indexes were located in the north and northeast, showing spatial autocorrelation. These regions also had the worst results for the following variables: poverty, illiteracy, education, and income. The bivariate analysis showed that household income and education level had negative spatial correlation with the DMFT index, while illiteracy and poverty rates showed positive correlation. Despite advances in the decline of DMFT index in recent years, there is still an inequity in the distribution of the caries disease.

RESUMO: Objetivo: Analisar as tendências das taxas de incidência da intoxicação por agrotóxicos nas regiões brasileiras, de acordo com sexo e circunstância da intoxicação, no período de 2001 a 2014. Método: Estudo de séries temporais, com dados do Sistema de Informação de Agravos de Notificação (SINAN). As taxas de incidência foram calculadas por meio da razão entre o número de casos novos confirmados de intoxicações por agrotóxicos e a população residente no mesmo período e local. Foram realizados análise de regressão polinomial e testes de Mann-Whitney e Kruskal-Wallis. Quando encontradas diferenças significativas, os testes foram seguidos pela penalização de Bonferroni com a finalidade de identificar onde residia a diferença. Resultados: No Brasil, foram registrados 80.069 casos de intoxicação nesse período. Houve um crescimento linear de notificações de intoxicação por agrotóxico, cuja taxa de tendência de crescimento encontrada foi de 0,377 por 100 mil habitantes/ano. As regiões Sul e Centro-Oeste apresentam as maiores taxas de intoxicação. Em relação ao sexo, não foram encontradas diferenças significativas (p < 0,347), sendo a tentativa de suicídio a circunstância de intoxicação mais significativa (p < 0,001). Conclusão: A incidência de intoxicação por agrotóxico no Brasil segue em aumento no século XXI.

ABSTRACT: Objective: To analyze trends in pesticide poisoning incidence rates in Brazilian regions, according to sex and circumstances of poisoning, between 2001 and 2014. Method: Ecological time-series study, with data from the national Notifiable Diseases Information System (SINAN). The incidence rates were calculated by dividing the number of confirmed new cases of pesticide poisoning by the total resident population in the same period and location. Both Polynomial regression analysis and Mann-Whitney and Kruskal-Wallis tests were performed. When significant differences were found, these tests were followed by the Bonferroni penalty, in order to identify the difference more precisely. Results: In Brazil, 80,069 notified poisoning cases were recorded from 2001 to 2014. There was a steadily increasing growth of pesticide poisoning in this population, whose growth trend was 0,377 for 100,000 inhabitants per year. The highest incidence of poisoning occurred in the South and Midest Regions. Regarding sex, no significant differences were found (p < 0,347), and attempted suicide was the most significant circumstance of poisoning (p < 0,001). Conclusion: The incidence of pesticide poisoning in Brazil has been continuously increasing in the twenty-first century.

RESUMO: Objetivo: Analisar as tendências das taxas de incidência da intoxicação por agrotóxicos nas regiões brasileiras, de acordo com sexo e circunstância da intoxicação, no período de 2001 a 2014. Método: Estudo de séries temporais, com dados do Sistema de Informação de Agravos de Notificação (SINAN). As taxas de incidência foram calculadas por meio da razão entre o número de casos novos confirmados de intoxicações por agrotóxicos e a população residente no mesmo período e local. Foram realizados análise de regressão polinomial e testes de Mann-Whitney e Kruskal-Wallis. Quando encontradas diferenças significativas, os testes foram seguidos pela penalização de Bonferroni com a finalidade de identificar onde residia a diferença. Resultados: No Brasil, foram registrados 80.069 casos de intoxicação nesse período.
