ABSTRACT Objective: To construct, validate, and apply a questionnaire to assess and characterize the practice of self-medication performed by parents or children's guardians undergoing dental care. Material and Methods: A questionnaire was constructed, validated, and applied through personal interviews in the teaching clinics of two higher education institutions in the Federal District. Absolute and relative frequencies of categorical variables were calculated using descriptive statistics. Quantitative data were presented as mean and standard deviation. The chi-square test measured the association between the studied variables and self-medication. Results: One hundred and five participants were interviewed between August 2019 and November 2020. The average age of the participants was 37 (± 9) years, and most were mothers of the children who attended (78%) and users of the public health system (94.2%). The average family income was 1.88 minimum wage, and the most prevalent educational level was complete high school (40%). Conclusion: Despite the high level of knowledge about medication safety in children, self-medication was practiced in 50% of them. Difficulty in accessing dental treatment was mentioned by most as a justification. Self-medication was associated with dental pain, continuous medication use, and family self-medication habits.

Abstract Background: Patients using immunosuppressive drugs may have unfavorable results after infections. However, there is a lack of information regarding COVID 19 in these patients, especially in patients with rheumatoid arthritis (RA). Therefore, the aim of this study was to evaluate the risk factors associated with COVID 19 hospitalizations in patients with RA. Methods: This multicenter, prospective cohort study is within the ReumaCoV Brazil registry and included 489 patients with RA. In this context, 269 patients who tested positive for COVID 19 were compared to 220 patients who tested negative for COVID 19 (control group). All patient data were collected from the Research Electronic Data Capture database. Results: The participants were predominantly female (90.6%) with a mean age of 53 ±12 years. Of the patients with COVID 19, 54 (20.1%) required hospitalization. After multiple adjustments, the final regression model showed that heart disease (OR =4.61, 95% CI 1.06–20.02. P < 0.001) and current use of glucocorticoids (OR =20.66, 95% CI 3.09–138. P < 0.002) were the risk factors associated with hospitalization. In addition, anosmia was associated with a lower chance of hospitalization (OR =0.26; 95% CI 0.10–0.67, P < 0.005). Conclusion: Our results demonstrated that heart disease and the use of glucocorticoids were associated with a higher number of hospital admissions for COVID 19 in patients with RA. ABSTRACT The objective was to evaluate the inclusion of chitosan (CHI) and technical cashew nut shell liquid (CNSLt) as natural feed additives in cattle diets on nutrient digestibility, ruminal fermentation, and in vitro gas production kinetics. We conducted a completely randomized design with 5×4 factorial arrangement, with 20, 35, 50, 65, and 100% Tifton 85 hay and four additives, monensin (200 mg/kg DM), CNSLt (500 mg/kg DM), CHI (500 mg/kg DM), and CNSLt+CHI (500 mg/kg DM/each). Dry matter (DM) and organic matter (OM) digestibility showed a linear reduction according to forage levels. The highest DM digestibility was observed with CHI on cattle diets. Inclusion of CHI increased DM digestibility. The highest in vitro organic matter and crude protein (CP) digestibilities were observed for CNSLt+CHI. The in vitro dry matter digestibility increased linearly with concentrate in the diet. There was interaction of forage:concentrate ratio and the additives for neutral detergent fiber, acid detergent fiber and hemicellulose digestibility. Chitosan, CNSLt, and CNSLt+CHI promoted the lowest acetate:propionate ratio compared with monensin. Total gas production showed interaction of the forage:concentrate ratio and additives. Lag time was lowest with CNSLt+CHI. Chitosan and CNSLt can be considered alternative fermentation modulators to ionophores by improving nutrient digestibility and increasing ruminal propionate concentrations.

Abstract The new coronavirus pandemic (COVID-19) is a global problem that is having severe impacts on health systems worldwide. One particular characteristic of this virus is its high transmission rate, which has led to a high demand for personal care materials such as masks, gloves, and alcohol for asepsis. Seventy percent is the ideal concentration for the ethanol used in hand sanitizers; however, this concentration can be expressed in different ways, and, consequently, the different solutions will have different antiseptic activity. In this manuscript we comment on some characteristics of alcohol-based preparations and the different ways of expressing the concentrations.

RESUMO Introdução Os treinamentos com simuladores para videocirurgia têm sido propostos como ferramentas de ensino, permitindo avaliação formativa de alunos em condições mais controladas e seguras. Objetivo Validar um programa estruturado de treinamento de endossuturas para residentes de cirurgia, por meio de duas escalas estruturadas de aquisição de habilidades laparoscópicas e da percepção dos participantes. Métodos Estudo experimental longitudinal quantitativo, que contou com a participação de 12 residentes de cirurgia provenientes de quatro hospitais distintos. O treinamento consistia na confecção de endossuturas com simuladores de vídeo em sete sessões num período de sete meses. A avaliação da progressão da competência técnica dos alunos foi realizada por três avaliadores, utilizando-se o Objective Structured Assessement of Technical Skills (Osats) e a Escala de Progressão de Proficiência (EPP) em Endossuturas. Os residentes usaram um questionário com escala de Likert de 1 a 5 para avaliar o programa de treinamento realizado quanto à aquisição de habilidades específicas. Foram feitos os testes de Anova e Qui-Quadrado para comparação entre variáveis. As correlações e associações entre duas variáveis numéricas foram verificadas por meio de regressão linear simples e múltipla, quando foram incluídos os fatores determinantes além do número de sessões de treinamento. Foi calculado o ranking médio para análise da escala de Likert. Foram consideradas significativas as comparações com valor de p ≤ 0,05. Este estudo foi aprovado pelo Comitê de Ética em Pesquisa da instituição. Resultados Quanto ao desempenho dos residentes observado pelos avaliadores por meio do Osats, observou-se melhora estatisticamente significante na pontuação em relação aos ítens: respeito ao tecido (p = 0,006), tempo e movimento (p = 0,001), conhecimento e manuseio dos instrumentos (p = 0,002), fluxo da cirurgia (p = < 0,001), conhecimento do procedimento específico (p = 0,001), pelo teste Qui-Quadrado. Em relação ao número total de pontos pela EPP e à nota média nos testes pelo Osats modificado, o total e a nota média nos testes foram significantes (< 0,001 e < 0,001), utilizando-se o teste Kruskal-Wallis ao longo dos meses de treinamento. Na percepção dos alunos, o programa de treinamento contribuiu para o desenvolvimento de habilidades cirúrgicas, tendo como itens mais bem avaliados o manuseio de pinças e porta-agulhas e a confecção de nós. Os itens mais mal avaliados foram a hapticidade e a passada de alça. Demonstrou-se ganho significativo na aquisição de competência técnica pelos residentes durante o curso, percebido por meio dos dois instrumentos de avaliação utilizados (Osats e EPP). Conclusão Houve ganho significativo de habilidades pelos residentes de cirurgia ao longo dos sete meses do programa de treinamento simulado, demonstrado pelos dois instrumentos de avaliação estruturada e pela percepção dos residentes.

ABSTRACT Introduction Training with simulators for video-surgery has been proposed as teaching tools, allowing formative evaluation of students under more controlled and safe conditions. Objective To validate structured program of endosuture training for surgical residents, through two structured scales of acquisition of laparoscopic skills and participant’s perception. Methods This was a quantitative longitudinal experimental study involving 12 residents of surgery from four different hospitals. The training consisted of making endorsements with video simulators in seven sessions over a period of seven months. The evaluation of the progression of the technical competence of the students was carried out by three evaluators, using the Objective Structured Assessmen of Technical Skills (Osats) and the Progression of Proficiency Scale (EPP) in Endorsements. Residents used a Likert scale to evaluate the training program. Anova and chi square tests were performed to compare variables. Correlations and associations between two numerical variables were verified through simple and multiple linear regression, when the determinants were included in addition to the number of training sessions. The Average Ranking was calculated for Likert scale analysis. Comparisons with p value ≤ 0.05 were considered significant.This study was approved by the Research Ethics Committee of the institution. Results There was a significant gain in the acquisition of technical competence by the residents during the course, perceived through the two evaluation instruments used (Osats and EPP). As regards the performance of the residents observed by the evaluators through the Osats, there was a statistically significant improvement in the score in relation to the items: Respect to the tissue (p = 0.006), Time and movement (p = 0.001), Knowledge and handling of instruments p = 0.002), Circulation flow (p = 0.001), Knowledge of the specific procedure (p = 0.001), by the chi-square test. In relation to the total number of points by EPP and the mean score in the tests by the modified Osats, the total and the mean score in the tests were significant (< 0.001 and < 0.001), using the Kruskal-Wallis test, over the months training. In the students’ perception, the training program contributed to the development of surgical skills, with the best evaluated items being the handling of tweezers and needle holders and the knitting. The worst evaluated items were the hapticity and the gait step. Conclusion There was a significant gain in skills by the residents of surgery during the seven months of simulated training, demonstrated in two structured assessment instruments and by the residents’ perception.

Aborto infeccioso é uma causa significativa de falhas reprodutivas e perdas econômicas na bovinocultura. O objetivo deste estudo foi detectar ácidos nucleicos de vários agentes infecciosos reconhecidos como causadores de aborto, incluindo-se Arcanobacterium pyogenes, Herpesvirus bovino tipo 1, Brucella abortus, Campylobacter fetus subsp. venerealis, Chlamydophila abortus, Leptospira sp., Listeria monocytogenes, Salmonella sp., Mycoplasma bovis, Mycoplasma bovigenitalium, Neospora caninum e Tritrichomonas foetus. Homogenados de tecidos de 42 fetos e tecidos incluídos em parafina de 28 fetos e 14 placentas/endométrio foram incluídos neste estudo. Brucella abortus foi detectada em 14,2% (12/84) das amostras. DNA de Salmonella sp. foi amplificado de dois fetos e houve um feto positivo para Neospora caninum e outro para Listeria monocytogenes. Essa metodologia baseada em PCR resultou na identificação da etiologia em 19% das amostras ou 20% se considerados somente os tecidos fetais.

Infectious abortion is a significant cause of reproductive failure and economic losses in cattle. The goal of this study was to detect nucleic acids of several infectious agents known to cause abortion including Arcanobacterium pyogenes, Bovine Herpesvirus 1, Brucella abortus, Campylobacter fetus subsp. venerealis, Chlamydophila abortus, Leptospira sp., Listeria monocytogenes, Salmonella sp., Mycoplasma bovis, Mycoplasma bovigenitalium, Neospora caninum, and Tritrichomonas foetus. Tissue homogenates from 42 fetuses and paraffin-embedded tissues from 28 fetuses and 14 placentas/endometrium were included in this study. Brucella abortus was detected in 14.2% (12/84) of the samples. Salmonella sp. DNA was amplified from 2 fetuses, and there was one positive for Neospora caninum, and another for Listeria monocytogenes. This PCR-based approach resulted in identification of the etiology in 19% of samples, or 20% if considered fetal tissues only.

A surdez súbita sensorioneural é uma perda auditiva súbita ou rapidamente progressiva. Na maioria dos casos a etiologia não é descoberta. Uma das causas possíveis de surdez súbita é a hemorragia intralabiríntica que, antes do surgimento da ressonância magnética, não era corretamente diagnosticada. O objetivo deste trabalho é relatar um caso de surdez súbita causada por hemorragia intralabiríntica e realizar uma revisão da literatura sobre este assunto.

Sudden sensorineural hearing loss is relatively frequent. In most cases, the etiology is not discovered. One of the possible causes for sudden deafness is inner labyrinth bleeding, which was difficult to diagnose before the advent of magnetic resonance imaging. The purpose of this paper is to report a case of sudden hearing loss caused by a labyrinthine hemorrhage, and to present a review of the literature on this topic.

A otospongiose é uma osteodistrofia do osso temporal caracterizada pela reabsorção e neoformação óssea desordenada. Clinicamente, caracteriza-se por disacusia condutiva, neurossensorial, e/ou mista, progressiva e zumbidos. O início dos sintomas ocorre entre 30 e 40 anos de idade sendo rara sua manifestação na infância. Descrevemos o caso de um paciente de 11 anos de idade, com quadro de hipoacusia unilateral progressiva há 5 anos. O exame otorrinolaringológico revelou mancha rubra de Schwartze em orelha esquerda. A audiometria, imitanciometria e a tomografia computadorizada demonstraram características sugestivas de otospongiose. Realizamos uma revisão dos aspectos clínicos, diagnósticos e da conduta terapêutica da otospongiose na infância.

Otospongiosis is an osteodystrophy of the temporal bone, characterized by disordered neoformation and deposition of bone, characterized by the presence of a progressive conductive, sensorineural or mixed hearing loss and tinnitus. Typically, otospongiosis presents as a slowly progressive conductive hearing loss in the third to fourth decade of life. Uncommonly children and adolescents may also have conductive or sensorineural hearing loss caused by otosclerosis. We describe a case of an 11-year-old patient, with progressive unilateral conductive hearing loss for 5 years. The otoscopic examination revealed a positive Schwartz's sign in the left ear. Audiometry, impedanciometry and CT scan showed characteristics that suggested otospongiosis. We reviewed clinical aspects, diagnosis and the therapeutic approach for otospongiosis in children.
