RESUMO. Nota-se um crescente número de idosos que prestam cuidados a outros idosos. A sobrecarga e o estresse comumente existentes podem afetar de formas diferentes o desempenho cognitivo, a depender do contexto desses idosos cuidadores. Objetivo: Comparar o desempenho cognitivo, a sobrecarga e o estresse de idosos cuidadores de idosos com e sem indícios de alterações cognitivas. Métodos: Estudo transversal e quantitativo realizado com 205 idosos cuidadores de idosos com indícios de alterações cognitivas e 113 idosos cuidadores de idosos sem indícios de alterações cognitivas atendidos na Atenção Primária à Saúde. Eles foram avaliados quanto às características sociodemográficas, cognição, sobrecarga e estresse. Análises descritivas (teste de Kolmogorov-Smirnov) e comparativas foram realizadas (teste t de Student e χ² de Pearson). Resultados: Os idosos cuidadores de idosos com indícios de alterações cognitivas eram mais velhos, com menor escolaridade e maior percentual de horas diárias de cuidado em comparação aos idosos cuidadores de idosos sem indícios de alterações cognitivas. Já quanto ao desempenho cognitivo, as médias foram inferiores para todos os domínios. Além disso, esse mesmo grupo apresentou maior pontuação, com diferença estatisticamente significante, para o estresse percebido e para a sobrecarga. Conclusão: Os cuidadores de idosos com indícios apresentaram menor desempenho cognitivo e maiores níveis de sobrecarga e estresse. Tais achados norteiam o planejamento de intervenções voltadas aos idosos cuidadores na Atenção Primaria à Saúde.

Abstract The task of caring can negatively affect the physical and mental health; therefore, it is important to understand which factors are associated with burden in older caregivers of older adults. Objective: This study aimed to explore sociodemographic, clinical, and psychosocial factors associated with burden in older caregivers of older adults. Methods: This is a cross-sectional study developed with 349 older caregivers who were registered at a Family Health Unit of a city in the state of São Paulo, Brazil. Household interviews were conducted and data were collected on the sociodemographic (profile, family income), clinical (self-reported pain, sleep, frailty), and psychosocial (burden, family functioning, depressive symptoms, stress) characteristics of the caregivers as well as dependence on activities of daily living and cognition in the care recipients. Results: Women predominated in the sample (76.5%) and mean age was 69.5 years. The mean burden score was 18.06 points, with 47.9% above the cutoff of 16 points, denoting excessive burden. The bivariate model revealed associations between burden and financial insufficiency, family dysfunction, difficulty sleeping, pain, perceived stress, depressive symptoms, frailty, and multimorbidity among the caregivers as well as worse functional and cognitive performance in the care recipients. The controlled model revealed an association between burden and depressive symptoms (β=16.75; 95%CI 1.80–31.68). Conclusions: We identified an association between burden and depressive symptoms, underscoring the need for the planning and implementation of specific actions directed at caregivers in order to minimize the impact on health and to improve the quality of life.

RESUMO A tarefa de cuidar pode afetar negativamente a saúde física e mental, sendo importante compreender quais os fatores associados à sobrecarga em idosos cuidadores de idosos. Objetivo: Explorar os fatores sociodemográficos, clínicos e psicossociais associados à sobrecarga em idosos cuidadores de idosos. Métodos: Estudo transversal desenvolvido com 349 idosos cuidadores cadastrados nas unidades de Saúde da Família de um município do interior paulista. Foram realizadas entrevistas domiciliares e coletados dados sobre as informações sociodemográficas (perfil, renda familiar), clínicas (dor autorrelatada, sono, fragilidade) e psicossociais (sobrecarga, funcionalidade familiar, sintomas depressivos, estresse) do cuidador, bem como dados de funcionalidade e cognição dos receptores de cuidados. Resultados: Houve prevalência do sexo feminino (76,5%), com média de idade de 69,5 anos. A média do escore da sobrecarga foi 18,06 pontos, com 47,9% dos idosos cuidadores acima da nota de corte de 16 pontos. O modelo univariado revelou associação entre sobrecarga e percepção de insuficiência financeira, disfunção familiar, dificuldade de dormir, dor, estresse percebido, sintomas depressivos, fragilidade e multimorbidade, além de pior desempenho funcional e cognitivo dos idosos receptores de cuidados. No modelo controlado houve associação entre sobrecarga e sintomas de depressão (β=16,75; intervalo de confiança — IC95% 1,80–31,68). Conclusão: Identificamos associação entre sobrecarga e sintomas depressivos, de modo que é necessário planejar e implementar cuidados específicos a fim de minimizar o impacto na saúde dos cuidadores e favorecer a qualidade de vida.

Abstract Aims: To describe reference values for the Closed Kinetic Chain Upper Extremity Stability Test (CKCUEST) and Upper Quarter Y Balance Test (UQYBT) in young adults, and to determine whether there were differences in both tests based on gender and age. Intrarater reliability and measurement error were also assessed. Methods: Test-retest design with a sample of 146 young adults. The CKCUEST (number of touches, normalized score, and power) and the UQYBT (normalized reach in the medial, superolateral, and inferolateral direction, and the composite score) were used. Mean and standard deviation were calculated for both tests. A linear and a mixed regression model were applied to determine significant differences in test scores. Reliability was determined using Intraclass Correlation Coefficient (ICC) and error measurement through Standard Error of Measurement (SEM) and Smallest Detectable Change (SDC). Results: Normative values were established and divided by gender and age. Men presented greater scores regarding the number of touches, power score, superolateral and inferolateral reaches. Good intrarater reliability was found for both tests. In the CKCUEST, the SEM and SDC values were 1 and 3 touches. In the UQYBT, the SEM values ranged from 3 to 7 cm, while the SDC ranged from 8 to 19 cm. Conclusion: Normative data were provided and men presented greater scores than women. Good intrarater reliability was found and values of SEM and SDC were established. Clinicians must use both tests in clinical practice since different aspects of the upper quarter are assessed.

Resumo: Sementes de café são sensíveis à dessecação e utilizadas ou armazenadas com diferentes umidades, podendo interferir nos resultados da avaliação da qualidade. O objetivo do trabalho foi avaliar as alterações no perfil proteico em gel de eletroforese em sementes de café com diferentes umidades e submetidas ou não à liofilização. Foram utilizados dois lotes de café arábica, sendo um de sementes recém-colhidas e outro de sementes armazenadas. As sementes foram secadas até as umidades de 12, 15, 20, 25, 30, 35 e 40%. A qualidade fisiológica das sementes foi avaliada pelo teste de germinação, condutividade elétrica e os perfis das enzimas SOD, CAT, PO, GOT, MDH e EST e de proteínas resistentes ao calor. De forma geral, ocorre uma interferência na expressão dessas enzimas em função da presença de água livre nas sementes. Sementes úmidas apresentam baixa ou nenhuma expressão enzimática. O processo de liofilização permite a preservação da qualidade das sementes de café e não altera a funcionalidade das enzimas estudadas. Os perfis de isoenzimas do processo antioxidativo em sementes de café arábica é influenciado pela umidade inicial das sementes. O processo de liofilização das sementes garante maior sensibilidade na detecção da expressão dessas isoenzimas.

Abstract: Coffee seeds are sensitive to desiccation and are used or stored with different moisture content values, which may affect the results of quality assessment. The aim of this study was to evaluate the changes in protein profile in electrophoresis gel in coffee seeds with different moisture content values under freeze-drying and without freeze-drying. Two lots of arabica coffee seeds were used, one of newly-harvested seeds and another of stored seeds. The seeds were dried to the moisture content values of 12, 15, 20, 25, 30, 35, and 40%. The physiological quality of the seeds was assessed through the germination test, electrical conductivity, and the profiles of the enzymes SOD, CAT, PO, GOT, MDH, and EST and of heat-resistant proteins. In general, there is an effect on expression of these enzymes in accordance with the presence of free water in the seeds. Moist seeds have little to no enzyme expression. The freeze-drying process allows preservation of coffee seed quality and does not change the functionality of the enzymes studied. The isoenzyme profiles of the antioxidant process in arabica coffee seeds are affected by the initial moisture content of the seeds. The freeze-drying process of the seeds ensures greater sensitivity in detection of the expression of these isoenzymes.

ABSTRACT Chronic Chagas Cardiomyopathy (CCC) is the most prevalent type of myocarditis and the main clinical form of the Chagas disease, which has peculiarities such as focal inflammation, structural derangement, hypertrophy, dilation, and intense reparative fibrosis. Many cellular compounds contribute to CCC development. Galectin-3 is a partaker in inflammation and contributes to myocardial fibrosis formation. Some studies showed the connection between Galectin-3 and fibrosis in Chagas disease but are still inconclusive on the guidance for the early implementation of pharmacological therapy. This systematic review evaluated Galectin-3 as a biomarker for fibrosis intensity in CCC. Two independent reviewers have searched five databases (PubMed, EMBASE, Cochrane Library, Scopus, and Lilacs), using the following search terms: galectin-3, biomarkers, fibrosis, Chagas cardiomyopathy, and Chagas disease. Overall, seven studies met the inclusion criteria and made up this review. There were four trials conducted through animal model experiments and three trials with humans. Experimental data in mice indicate an association between Galectin-3 expression and fibrosis in CCC (75% of studies). Data from human studies showed no direct connection between myocardial fibrosis and Galectin-3 expression (80% of studies). Thus, human findings do not provide significant evidence indicating that Galectin-3 is related to fibrosis formation in Chagas disease. Based on the analyzed studies, it is suggested that Galectin-3 might not be a good fibrosis marker in CCC.

Resumo Objetivo Avaliar características sociodemográficas e obstétricas de mulheres com diabetes gestacional que mantêm hiperglicemia no período pós-parto (6–12 semanas pós-parto). Métodos Este é um estudo longitudinal de coorte com mulheres com diagnóstico de diabetes gestacional e/ou macrossomia fetal entre 1° de março de 2016 a 1° de março de 2017. As mulheres coletaram glicemia de jejum, teste de tolerância a glicose e hemoglobina glicada entre 6 a 12 semanas pós-parto. Os dados foram coletados de prontuários médicos e durante entrevista na primeira consulta de revisão pós-parto. Uma análise estatística foi realizada através do cálculo de frequências, porcentagens, teste do qui-quadrado, teste exato de Fisher, teste de Mann-Whitney e regressão multivariada de Poisson. A significância estatística adotada foi de 5%. Resultados Cento e vinte e duas mulheres foram incluídas. A maioria delas tinha menos de 35 anos de idade (70,5%), eram brancas, multíparas, e não tinham história de diabetes gestacional. Treze por cento das participantes desenvolveu hiperglicemia persistente. A análise univariada mostrou que os fatores relacionados com a persistência de hiperglicemia no período pós-natal foram: idade materna acima de 35 anos, sobrepeso, obesidade grau 1 e ganho de peso abaixo de 5 quilos. A análisemultivariada incluiu o diagnóstico no primeiro trimestre como fator de risco para hiperglicemia persistente. Conclusão Mulheres acima de 35 anos, obesidade, sobrepeso e diagnóstico de diabetes gestacional no primeiro trimestre estão relacionados com hiperglicemia persistente no período pós-parto.

Abstract Objective To evaluate the obstetric and sociodemographic characteristics of gestational diabetic women who maintained hyperglycemia in the postpartum period (6-12 weeks postpartum). Methods This is a longitudinal cohort study with women who have had gestational diabetes and/or macrosomic children between March 1st, 2016 and March 1st, 2017. Between 6 and 12 weeks after birth, women who had gestational diabetes collected fasting glycemia, glucose tolerance test, and glycated hemoglobin results. The data were collected from medical records and during an interview in the first postpartum consultation. A statistical analysis was performed using frequency, percentage, Chi- Squared test, Fisher exact test, Mann-Whitney test, and multivariate Poisson regression. The significance level adopted for the statistical tests was 5%. Results One hundred and twenty-two women were included. Most of the women were younger than 35 years old (70.5%), white, multiparous, and with no history of gestational diabetes. Thirteen percent of the participants developed persistent hyperglycemia. A univariate analysis showed that maternal age above 35 years, being overweight, having grade 1 obesity and weight gain under 5 kg was related to the persistence of hyperglycemia in the postpartum period. Conclusion Maternal age above 35 years, obesity and overweight, and the diagnosis of gestational diabetes in the first trimester of pregnancy are associated with hyperglycemia during the postpartum period.

RESUMO Objetivo: identificar a associação entre as características da rede de apoio e o desempenho cognitivo de cuidadores idosos e comparar características entre cuidadores e não cuidadores. Método: foram avaliados 85 idosos cuidadores e 84 idosos não cuidadores cadastrados em unidades da atenção primária de saúde quanto às características sociodemográficas, cognitivas e suporte social. Foi realizada análise de regressão linear múltipla. Resultados: entre os não cuidadores, foram encontradas associações significativas entre melhor desempenho cognitivo e receber apoio emocional/afetivo; cada aumento de um ponto no escore de apoio emocional e escore de suporte afetivo foi relacionado a um aumento de 0,43 pontos e 0,39 pontos na avaliação cognitiva, respectivamente. Entre os cuidadores mais velhos, cada aumento de um ponto na pontuação do apoio emocional teve relação com um aumento de 0,55 ponto na avaliação cognitiva. Conclusão: o fortalecimento das redes de apoio de cuidadores mais velhos e o incentivo a trocas satisfatórias de apoio social podem auxiliar na melhoria do desempenho cognitivo, o que pode impactar positivamente na saúde dos cuidadores.

ABSTRACT Objective: to identify the association between the characteristics of the support network and cognitive performance of older caregivers and compare characteristics between caregivers and non-caregivers. Methods: we evaluated 85 older caregivers and 84 older non-caregivers registered with primary care units regarding sociodemographic characteristics, cognition, and social support. Multiple linear regression analysis was performed. Results: among non-caregivers, significant associations were found between a better cognitive performance and receiving emotional/affectionate support; each one-point increase in the emotional support score and affectionate support score was related to a 0.43-point and 0.39-point increase in cognitive assessment, respectively. Among older caregivers, each one-point increase in the emotional support score was related to a 0.55-point increase in cognitive assessment. Conclusion: strengthening the support networks of older caregivers and encouraging satisfactory exchanges of social support can assist in improving cognitive performance, which can have a positive impact on caregivers’ health.

RESUMEN Objetivo: identificar la asociación entre las características de la red de apoyo y el desempeño cognitivo de los cuidadores mayores y comparar características entre cuidadores y no cuidadores. Métodos: se evaluaron 85 cuidadores mayores y 84 no cuidadores mayores registrados en unidades de atención primaria en cuanto a características sociodemográficas, cognitivas y de apoyo social. Se realizó un análisis de regresión lineal múltiple. Resultados: entre los no cuidadores, se encontraron asociaciones significativas entre un mejor desempeño cognitivo y recibir apoyo emocional/afectivo; cada aumento de un punto en la puntuación de apoyo emocional y la puntuación de apoyo afectivo se relacionó con un aumento de 0,43 puntos y 0,39 puntos en la evaluación cognitiva, respectivamente. Entre los cuidadores mayores, cada aumento de un punto en la puntuación de apoyo emocional se relacionó con un aumento de 0,55 puntos en la evaluación cognitiva. Conclusión: el fortalecimiento de las redes de apoyo para los cuidadores mayores y el fomento de intercambios satisfactorios de apoyo social puede ayudar a mejorar el rendimiento cognitivo, lo que puede tener un impacto positivo en la salud de los cuidadores.

Abstract INTRODUCTION: Neck circumference (NC) and anthropometric data of people living with HIV (PLWH) are correlated. METHODS: Socioeconomic, NC, body mass index (BMI), tricipital skinfold thickness (TSF), mid-arm circumference (MAC), mid-arm muscle circumference (MAMC), waist-hip ratio (WHR), waist-to-height ratio (WHtR), waist circumference (WC), and hip circumference (HC) data of 72 PLWH were correlated. RESULTS Higher adiposity was observed in NC (40.3% [n=29]) and WC (31.9% [n=23]). Correlations between NC/BMI, NC/WC, NC/HC, NC/MAC, NC/MAMC, and NC/WHtR were significant. Increased NC (40.3%[n=29]) and WC (31.9 [n=23]) were associated with higher cardiometabolic risk. CONCLUSIONS: NC correlations are adequate for estimating cardiometabolic risk.

RESUMO. Dados os benefícios da função familiar adequada para a saúde e o bem-estar de pessoas idosas, é importante compreender quais fatores predizem o funcionamento familiar adequado em pessoas idosas que cuidam de seus cônjuges. Objetivo: Analisar preditores de funcionamento familiar em idosos cuidadores de cônjuges idosos. Métodos: Um estudo transversal foi usado para investigar uma amostra não probabilística de 298 cuidadores cônjuges idosos. Foram realizadas entrevistas domiciliares para avaliar variáveis sociodemográficas e situação de cuidado, funcionamento familiar (APGAR familiar), função cognitiva, percepção de estresse e sintomas depressivos. Os dados coletados foram analisados por meio de regressão logística múltipla com método stepwise forward para seleção das variáveis. Resultados: Idosos cuidadores com algum grau de comprometimento cognitivo (OR=-0,160, IC95% 0,444–0,579), sintomas depressivos (OR=-0,848, IC95% 0,726–0,992) ou altos níveis de estresse (OR=-0,955, IC95% 0,914-0,999) tiveram menores níveis de funcionamento familiar. Ter mais filhos esteve relacionado a maiores níveis de funcionamento familiar em aproximadamente 1,3 vez (IC95% 1,080–1,057). Conclusão: A presença de estresse, depressão, declínio cognitivo e número de filhos são preditores do funcionamento familiar e devem ser considerados como parte de estratégias sociais e de saúde no contexto de cuidado familiar.

ABSTRACT. Given the benefits of adequate family function for the health and well-being of older adults, it is important to understand what factors predict adequate family function in older people who care for their spouses. Objective: Analyse predictors of family function in older spousal caregivers. Methods: A cross-sectional study design was used to investigate a non-probabilistic sample of 298 older spousal caregivers. Home-based face-to-face interviews were used to evaluate sociodemographic variables and care context, family function (Family APGAR), cognitive function, perceived stress, and depressive symptoms. Data were analysed using multiple logistic regression with stepwise forward method for variable section. Results: Older caregivers having some degree of cognitive impairment (OR=-0.160, 95%CI 0.444–0.579), depressive symptoms (OR=-0.848, 95%CI 0.726–0.992) or high levels of stress (OR=-0.955, 95%CI 0.914-0.999) had overall lower levels of family function. Having more children was linked to approximately 1.3 times higher family function (95%CI 1.080–1.057). Conclusion: Stress, depression, cognitive decline, and number of children are predictors of family function and should be considered in social and health care strategies within the family caregiving context.

Objetivo: Comparar o desempenho cognitivo e os aspectos do contexto do cuidado em idosos cuidadores de idosos em uma de acompanhamento de três anos. Métodos: Trata-se de um estudo longitudinal com medidas em 2014 e 2017, realizado com 61 cuidadores idosos cadastrados na atenção primária de um município do interior do Estado de São Paulo. Características sociodemográficas, contexto do cuidado, cognição, desempenho nas atividades da vida diária, sobrecarga e sintomas depressivos foram coletados em entrevistas individuais. A análise dos dados foi realizada para comparar variáveis categóricas e contínuas. Resultados: Foram encontrados aumentos significativos entre a linha de base e o acompanhamento do escore no domínio da linguagem da avaliação cognitiva (p=0,024), receber ajuda material (p=0,020), tempo de cuidado (p=0,045) e dependência do idoso em relação às atividades básicas da vida diária (p=0,042). Conclusão: Os cuidadores de idosos tiveram melhor desempenho no campo da linguagem, passaram a receber mais apoio material / financeiro, experimentaram aumentos no tempo gasto no cuidado e na dependência do idoso.

ABSTRACT. Cognitive health is important for the quality of life and well-being of elderly caregivers. Cognitive impairments can negatively affect the ability to care for oneself and others. Objective: To compare cognitive performance and aspects of the care context in elderly caregivers of older adults in a three-year follow-up investigation Methods: A longitudinal study was conducted of 61 elderly caregivers treated in primary care in a city in the interior of the state of São Paulo with data collected in 2014 and 2017. Sociodemographic characteristics, the care context, cognition, performance for activities of daily living, burden and depressive symptoms were collected in individual interviews. Data analysis was performed to compare categorical and continuous variables. Results: Significant increases were found between baseline and follow-up for the language domain score of the cognitive evaluation (p=0.024), receipt of material support (p=0.020), time providing care (p=0.045) and dependence of elderly care recipient for basic activities of daily living (p=0.042). Conclusion: Elderly caregivers performed better on the domain of language, received more material/financial support, spent more time on care and reported greater dependence of the elderly care recipient.

Resumo O objetivo deste artigo é comparar o nível de sobrecarga, otimismo e as estratégias de enfrentamento de idosos cuidadores de idosos que residem com e sem crianças. Estudo transversal realizado com amostra de 301 idosos cuidadores cadastrados nas Unidades de Saúde da Família. Foram entrevistados 301 idosos cuidadores, sendo que 257 não residiam com crianças e 44 residiam. Os instrumentos utilizados foram Escala de Sobrecarga de Zarit, Escala de Espiritualidade de Pinto e Pais Ribeiro e Inventário para Avaliação das Estratégias de Enfrentamento. Foram usados o teste T de Student e teste U Mann-Whitney para a comparação entre os dois grupos. O nível de sobrecarga foi significativamente maior nos idosos cuidadores que residiam com crianças (p = 0,01) e não houve diferença significativa para o otimismo. Os idosos cuidadores que não residiam com crianças reportaram o uso significativamente superior de estratégias de enfrentamento focadas em emoções negativas (p < 0,01), religiosidade (p < 0,01) e inibição de emoções negativas (p = 0,01). O nível de sobrecarga foi maior entre os idosos cuidadores que residiam com crianças. Os idosos cuidadores que não tinham crianças no domicílio reportaram maior uso de estratégias focadas na emoção e na religião para enfrentar as demandas do cuidado ao idoso.

Abstract This article aims to compare level of burden, optimism and coping strategies among elderly caregivers of other elderly individuals who reside with and without children. A cross-sectional study was with a sample of 301 elderly caregivers of elderly Brazilians seen in primary health care. Among the 301 caregivers interviewed, 44 resided with children and 257 did not. Evaluations involved the administration of the Zarit Burden Inventory, Spirituality Scale (optimism) and Coping Strategies Inventory. Either the Student’s t-test or the Mann-Whitney U-test was used for the comparisons between the two groups. Burden level was significantly higher among elderly caregivers who resided with children (p = 0.01). No significant differences were found with regard to optimism. Elderly caregivers who did not reside with children reported significantly greater use of coping strategies focused on the expression of negative emotions (p < 0.01), religiosity (p < 0.01) and the inhibition of negative emotions (p = 0.01). The elderly caregivers of other elderly individuals who resided with children had a higher level of burden and used coping strategies less than those who did not reside with children.

Resumo A associação entre comprometimento cognitivo e fragilidade tem sida estudada em idosos e os critérios de fragilidade e níveis de fragilidade podem também apresentar influência na função cognitiva. Objetivo: Analisar a associação entre comprometimento cognitivo e os critérios de fragilidade em idosos. Métodos: O estudo analisou transversalmente dados de 667 idosos (≥60 anos) do estudo ‘Variables associated to cognition in elderly caregivers’ conduzido com usuários da atenção primária a saúde de áreas urbanas e rurais. Comprometimento cognitivo foi definido em função dos escores no Miniexame do Estado Mental baseado na escolaridade e a fragilidade e pré-fragilidade foi operacionalizada segundo os critérios do Cardiovascular Health Study. Foram realizados modelos de regressão multinominal para analisar a associação entre níveis de fragilidade e comprometimento cognitivo. Resultados: Proporções semelhantes de mulheres (54,8%) e homens (45,2%) participaram do estudo (média de idade: 71 anos). Comprometimento cognitivo, pré-fragilidade e fragilidade foram encontrados em 34, 54 e 24% dos participantes, respectivamente. Concomitante comprometimento cognitivo e fragilidade foi evidenciado em 13%. As chances de apresentar comprometimento cognitivo aumentaram em 330% (Odds Ratio [OR]: 4.3; intervalo de confiança de 95% [IC95%] 2.4‒7.7; p<0.001) entre indivíduos frágeis e 70% (OR: 1.7; IC95% 1.0‒2.8; p=0.033) entre indivíduos pré-frágeis em comparação com indivíduos robustos/não-frágeis. Após controle da idade, escolaridade, local de residência e dependência funcional, os critérios de lentidão e fadiga foram significativamente associados ao comprometimento cognitivo. Conclusão: Os idosos mais frágeis têm maior probabilidade de apresentar comprometimento cognitivo comparados a adultos idosos pré-frágeis ou robustos. A prevalência de comprometimento cognitivo e fragilidade é consistente com os dados relatados na literatura e fornece suporte para futuras investigações sobre a fragilidade cognitiva.

Abstract The association between cognitive impairment and physical frailty has been studied in older adults. The criteria degree of frailty may be keys to associated cognitive impairment. Objective: To analyze the association between cognitive impairment and the criteria for frailty. Methods: We cross-sectionally examined data from 667 older adults (≥60 years of age) from a study entitled ‘Variables associated to cognition in elderly caregivers’ involving patients in an urban and rural primary healthcare center. We defined cognitive impairment based on different groups of scores on the Mini Mental State Examination, and defined frailty and prefrailty using the criteria by the Cardiovascular Health Study. We performed multinomial regression models to analyze the association between levels of frailty and cognitive impairment. Results: Similar proportions of women (54.8%) and men (45.2%) participated in the study (mean age: 71 years old). We found cognitive impairment, prefrailty and frailty in 34, 54, and 24% of the participants, respectively. Concomitant cognitive impairment and frailty was found in 13% of them. The chances of cognitive impairment increased up to 330% (Odds Ratio [OR]: 4.3; 95% confidence interval [95%CI] 2.4‒7.7; p<0.001) among frail individuals, and 70% (OR: 1.7; 95%CI 1.0‒2.8; p=0.033) among prefrail individuals compared to robust/non-frail individuals. After controlling for age, education, place of residence and functional dependence, slowness and fatigue criteria were significantly associated with cognitive impairment. Conclusion: Older adults with frailty have a greater likelihood of concomitant cognitive impairment than prefrail and robust older adults. The prevalence of cognitive impairment and frailty is consistent with data reported in literature. The present findings contribute to the investigation of cognitive frailty.

Abstract Background: South American rattlesnakes are represented in Brazil by a single species, Crotalus durissus, which has public health importance due to the severity of its envenomation and to its wide geographical distribution. The species is subdivided into several subspecies, but the current classification is controversial. In Brazil, the venoms of C. d. terrificus and C. d. collilineatus are used for hyperimmunization of horses for antivenom production, even though the distinction of these two subspecies are mostly by their geographical distribution. In this context, we described a comparative compositional and functional characterization of individual C. d. collilineatus and C. d. terrificus venoms from three Brazilian states. Methods: We compared the compositional patterns of C. d. terrificus and C. d. collilineatus individual venoms by 1-DE and RP-HPLC. For functional analyzes, the enzymatic activities of PLA2, LAAO, and coagulant activity were evaluated. Finally, the immunorecognition of venom toxins by the crotalic antivenom produced at Butantan Institute was evaluated using Western blotting. Results: The protein profile of individual venoms from C. d. collilineatus and C. d. terrificus showed a comparable overall composition, despite some intraspecific variation, especially regarding crotamine and LAAO. Interestingly, HPLC analysis showed a geographic pattern concerning PLA2. In addition, a remarkable intraspecific variation was also observed in PLA2, LAAO and coagulant activities. The immunorecognition pattern of individual venoms from C. d. collilineatus and C. d. terrificus by crotalic antivenom produced at Butantan Institute was similar. Conclusions: The results highlighted the individual variability among the venoms of C. durissus ssp. specimens. Importantly, our data point to a geographical variation of C. durissus ssp. venom profile, regardless of the subspecies, as evidenced by PLA2 isoforms complexity, which may explain the increase in venom neurotoxicity from Northeastern through Southern Brazil reported for the species.
