Abstract Objective: To compare information on highly complex radiological procedures—computed tomography (CT) and magnetic resonance imaging (MRI)—between the public and private health care systems, across the five regions of Brazil, in terms of the numbers of radiological devices and examinations performed, between 2015 and 2021. Materials and Methods: This was a descriptive time series analysis of secondary data in the public domain, available from the Information Technology Department of the Brazilian Unified Health Care System, an entity of the Brazilian National Ministry of Health (NMH) that is responsible for collecting and storing health-related information in Brazil. The analysis included the numbers of CT and MRI scanners; the volumes and types of examinations; the type of institution (public or private); the regions of the country; and the years (2015 to 2021). Results: Progressive increases in the numbers of CT and MRI devices, as well as in the volumes of examinations, were observed over the years in all regions of the country. The private sector showed higher rates of equipment acquisition and of growth in the number of examinations. However, the public health care system did not reach the equipment targets set by the NMH, whereas the private health care system surpassed those targets. A greater number of examinations were performed in the private sector than in the public sector. Conclusion: During the period evaluated, the public health care system did not meet the equipment or examination targets recommended by the NMH, in any of the regions of the country, unlike the private health care system, which exceeded both in all of the regions. ABSTRACT Wild birds have great prominence in the transmission of diseases to humans, mainly due to their ease of access to human populations, raising concerns about the potential impact of that proximity on public health. The present study reports ARV and PBV occurrence in wild birds from an environmental protection area in the Amazon biome, in Pará state, Brazil. We obtained 155 fecal specimens from 47 species of wild birds for RNA extraction, ARV and PBV detection utilizing molecular methods, nucleotide sequencing, and phylogenetic analysis. ARV prevalence was 0.6% (1/155), being positive in an individual of Myrmotherula longipennis, and PBV prevalence was 1.29% (2/155), affecting two individuals of Guira guira. The PBV strains were assigned to genogroup I based on phylogenetic analysis, and they shared a strong phylogenetic link with strains isolated from different geographic locations. The ARV strain was more closely related to strains that had previously circulated in the same region. The presence of ARV and PBV in this habitat suggests that infection cycles of these agents occur naturally in the wild ecosystem, potentially triggering transmission events between bird species and humans. RESUMO As aves silvestres têm grande destaque na transmissão de doenças ao homem, principalmente pela facilidade de acesso à população humana, levantando preocupações sobre o potencial impacto dessa proximidade no contexto da saúde pública. O presente estudo relata a ocorrência de ARV e PBV em aves silvestres de uma área de proteção ambiental no bioma Amazônico. SUMMARY OBJECTIVE: The aim of this study was to evaluate the prevalence and risk factors related to metabolic dysfunction-associated steatotic liver disease in inflammatory bowel disease patients. METHODS: This is a cross-sectional study conducted on adults with inflammatory bowel disease from 2019 to 2021. Metabolic dysfunction-associated steatotic liver disease encompasses patients with steatosis and at least one cardiometabolic risk factor. Patients with alcohol consumption ≥20 g/day, chronic liver diseases, or methotrexate use were excluded. RESULTS: Almost 140 patients were included: 67.1% were female, with a mean age of 49.7±13.7 years, and 63.6% had Crohn's disease. The mean duration of inflammatory bowel disease was 9.7±7.9 years. Metabolic dysfunction-associated steatotic liver disease was observed in 44.3% and advanced liver fibrosis was excluded in 63.5% by Fibrosis-4. Patients with metabolic dysfunction-associated steatotic liver disease were older (p = 0.003) and had a higher number of metabolic syndrome components (2.9±1.1 versus 1.6±1.0; p<0.001), greater abdominal circumference (p<0.001), and body mass index (p<0.001). The only factor related to inflammatory bowel disease associated with metabolic dysfunction-associated steatotic liver disease was disease duration (11.6±9.5 versus 8.3±6.2; p = 0.017). A higher number of metabolic syndrome components and obesity increase by 2.2 times and an altered waist circumference by 2.6 times the occurrence of metabolic dysfunction-associated steatotic liver disease. CONCLUSION: A high prevalence of metabolic dysfunction-associated steatotic liver disease was observed in patients with inflammatory bowel disease, with the main risk factors being associated with metabolic syndrome predicting it, but not with inflammatory bowel disease features and/or its treatment. ABSTRACT. This study aimed to evaluate the feeding time and reactivity of male and female foals exposed to novel objects (seven plastic balls measuring 8 cm in diameter) placed inside the feeder. Eleven mixed-breed foals (males = 6; 316 ± 42 kg of BW, females = 5; 290 ± 35 kg of BW) were used. The amount of concentrate feed was calculated using 2000 g kg-1 BW for males and 1500 g kg-1 BW for females. Behavioral variables were assessed using a scoring system. Data on time spent feeding were subjected to analysis of variance and regression analysis (p < 0.05) and compared using the PROC REG procedure of SAS (with object = without object). A line of equality (y = x) was used to test correlations. The behavioral model was fitted by a generalized linear model using the GLIMMIX procedure of SAS, and means were compared (p < 0.05) by t-tests. Feeding time differed between sexes (males = 23.7 ± 4.8 min., females = 19.5 ± 3.1 min.), attributed to the longer time spent by males in attentive behavior when plastic balls were placed in the feeders. There was no effect on composite behavioral score. Both males (18.37%) and females (14.29%) were classified as calm while feeding in feeders with unfamiliar objects. ABSTRACT Objective: This study aimed at investigating the extent to which COVID-19-induced fatalities affect the duration of palliative care among patients with advanced cancer. Methods: A retrospective cohort study was conducted at the Palliative Care Unit of the Brazilian Instituto Nacional de Câncer in Rio de Janeiro, Brazil, on 1,104 advanced cancer patients who died under exclusive palliative care between March 11, 2020, and March 31, 2021. Wilcoxon rank-sum (Mann-Whitney U) and log-rank tests were performed to examine statistical differences between the medians of time, and the Kaplan-Meier estimator was used to graphically illustrate survival over time under exclusive palliative care contingent upon the underlying causes of death of the two experimental groups (cancer versus COVID-19). Results: A total of 133 (12.05%) patients succumbed to COVID-19. In both groups, the median time under exclusive palliative care was less than one month. The exclusive palliative care survival curves did not exhibit any statistically significant difference between the groups. Conclusion: Death due to COVID-19 did not modify the duration of exclusive palliative care among patients with advanced cancer. Resumen Introducción: Al realizar y promover prácticas que tienen como objetivo la mejora de la salud, el equipo de enfermería asumió el protagonismo en los tiempos de la pandemia por COVID-19. Por tanto, es evidente la exposición personal-profesional-familiar, la falta de personal y la inseguridad laboral-emocional, además de las consecuencias que recaerán sobre la salud y los contextos laborales de este grupo profesional. Objetivo: Analizar el contexto sanitario y laboral de profesionales de enfermería durante la pandemia por COVID-19. Metodología: Estudio de método mixto, combinando los abordajes a través de la estrategia de incorporación concomitante - QUAN(qual), realizado en Minas Gerais, Brasil, entre agosto y diciembre de 2020. Se aplicó un guion de preguntas, mediante llamada telefónica, a 58 profesionales, personal técnico y auxiliares de enfermería. El guion contenía preguntas abiertas y cerradas relacionadas con cuestiones sociodemográficas y los contextos y condiciones de salud, trabajo y COVID-19. Se realizó un análisis cuantitativo descriptivo y un análisis de contenido temático. Resultados: La mayor parte de la muestra estuvo compuesta por mujeres (93.1 %) y personal técnico de enfermería (69 %). El 39.7 % trabajaba en turno de noche, el 58.6 % declaró haber sufrido violencia en el trabajo y el 48.3 % informó de diagnósticos de trastorno mental. Los contextos de trabajo en la pandemia mostraron la necesidad de estímulo y valoración profesional y las relaciones con los factores institucionales, la dinámica y la organización del trabajo, las condiciones adecuadas de trabajo y la relación interpersonal favorable. Los contextos de salud, mientras tanto, mostraron una relación con los intereses, las oportunidades, la rutina, la normalidad, la esperanza y la tenacidad que se produciría al final de la COVID-19. Conclusión: La pandemia de la COVID-19 fortaleció un contexto de salud y trabajo crítico, atemorizado e inseguro del equipo de enfermería brasileño, exacerbando la necesidad de providencias, acciones y políticas que consideren la salud de la persona trabajadora como estructurante para la organización de los servicios.

Metodologia: Estudo de método misto, combinando as abordagens por meio da estratégia de incorporação concomitante - QUAN(qual), realizado em Minas Gerais, Brasil, entre agosto e dezembro de 2020. Aplicou-se, via ligação telefônica, um roteiro de perguntas a 58 profissionais enfermeiros, técnicos e auxiliares de enfermagem. Abstract Introduction: By performing and promoting practices that aim to improve health, the nursing team has assumed itself as a protagonist during the COVID-19 pandemic. Therefore, the personal, professional, and family exposure is evident; the lack of personnel and occupational security, in addition to the consequences on the health and work contexts of these professionals. Objective: To analyze the health and work contexts of nursing professionals during the pandemic of COVID-19. Methodology: This is a mixed-method study that combines approaches through the concurrent incorporation strategy - QUAN(qual), conducted in Minas Gerais, Brazil, between August and December 2020. A script of questions was applied over the phone to 58 professional nurses, nursing technicians, and nursing assistants. The script included open and closed questions related to sociodemographic issues and health, work, and COVID-19 contexts and conditions. A descriptive quantitative analysis and thematic content analysis were performed. Results: The majority were women (93.1%) nursing technicians (69%). The 39.7% of the participants worked the night shift; the 58.6% reported experiencing violence at work and the 48.3% was diagnosed with mental disorders. The work contexts during the pandemic showed that there was a need for encouragement and professional evaluation; they also portrayed the relationships with the institutional factors, the work dynamics and the organization, the appropriate working conditions and the favorable interpersonal relationships. On the other hand, health contexts showed the relationship between interests, opportunities, routine, normality, hope and tenacity that would occur at the end of COVID-19. Conclusion: The pandemic of COVID-19 strengthened a critical, fearful, and insecure health and work context of the Brazilian nursing team, exacerbating the need for provisions, actions and policies that consider worker's health crucial for the organization of services.

ABSTRACT Objective To verify whether the presence of related nutritional risk indicators prior to COVID-19 diagnosis is associated with poor survival in patients with cancer. Methods We retrospectively analyzed the data of hospitalized cancer patients who tested positive for COVID-19 between March 2020 and February 2021. Nutritional risk was defined as the presence of one of the following characteristics: body mass index <20kg/m 2 , scored Patient-generated Subjective Global Assessment ≥9 points or classification B, albumin level <3.5g/dL, and C-reactive protein level ≥10mg/L, evaluated between 7 and 60 days prior to the date of patient inclusion. The endpoint measure was all-cause mortality within 30 days of COVID-19 diagnosis. Results A total of 253 patients were included, most of whom were elderly (62.4%) and female (63.6%). Overall, 45.4% of the patients were at nutritional risk. Survival was significantly lower in patients at nutritional risk (8 days; interquartile range [IQR]: 3-29) than in patients not at nutritional risk (16 days; IQR: 6-30) (p<0.001). The presence of prior nutritional risk was associated with increased 30-day mortality (HR: 1.42; 95%CI: 1.03-1.94), regardless of age, gender, tumor site or stage, and other risk factors, and the model had good discrimination accuracy (concordance statistic: 0.744). Conclusion The presence of prior nutritional risk indicators is related to poor prognosis in patients with cancer and COVID-19, emphasizing the importance of nutritional care, notably during this pandemic. RESUMEN Objetivo: Construir y validar escenarios de simulación clínica para la atención de emergencia de pacientes con dolor torácico. Métodos: Estudio metodológico realizado en dos etapas: construcción y validación. La construcción se dio a través del levantamiento de evidencias de la literatura nacional e internacional. La etapa de validación se dio a través de la evaluación de los instrumentos por parte de los jueces, según el Índice de Validación de Contenido y aplicación de la prueba piloto con el público objetivo. ABSTRACT Objective: To construct and validate clinical simulation scenarios for emergency care for patients with chest pain. Methods: A methodological study carried out in two stages: construction and validity. The construction took place through the survey of evidence from national and international literature. The validity stage took place through instrument assessment by judges, according to the Content Validity Index and application of a pilot test with the target audience. Fifteen judges with expertise in simulation, teaching and/or care participated in the research, in addition to 18 nursing students, in the pilot test. Results: Two scenarios of clinical simulation were constructed, and all the assessed items obtained a value above 0.80, showing evidence of validity, being considered instruments suitable for application. Conclusion: The research contributed to the development and validity of instruments that can be applied for teaching, assessment and training in clinical simulation in emergency care for patients with chest pain. ABSTRACT Background: The rate of tuberculosis (TB) infection among the prison population (PP) in Brazil is 28 times higher than that in the general population, and prison environment favors the spread of TB. Objective: To describe TB transmission dynamics and drug resistance profiles in PP using whole-genome sequencing (WGS). Methods: This was a retrospective study of Mycobacterium tuberculosis cultivated from people incarcerated in 55 prisons between 2016 and 2019; only one isolate per prisoner was included. Information about movement from one prison to another was tracked. Clinical information was collected, and WGS was performed on isolates obtained at the time of TB diagnosis. Results: Among 134 prisoners included in the study, we detected 16 clusters with a total of 58 (43%) cases of M. tuberculosis. Clusters ranged from two to seven isolates with five or fewer single nucleotide polymorphism (SNP) differences, suggesting a recent transmission. Six (4.4%) isolates were resistant to at least one anti-TB drug. Two of these clustered together and showed resistance to rifampicin, isoniazid, and fluoroquinolones, with 100% concordance between WGS and phenotypic drug-susceptibility testing. Prisoners with clustered isolates had a high amount of movement between prisons (two to eight moves) during the study period. Conclusions: WGS demonstrated the recent transmission of TB within prisons in Brazil. The high movement among prisoners seems to be related to the transmission of the same M. tuberculosis strain within the prison system. Resumo O Brasil é caracterizado por fortes desigualdades sociais e diferenças no acesso a alimentos de qualidade e em quantidade suficiente, o que representa uma violação do direito humano à alimentação adequada. O objetivo foi avaliar os gastos com alimentação de acordo com o perfil social do responsável pelo domicílio. Dados da Pesquisa de Orçamentos Familiares Brasileiros de corte transversal (2017/2018) foram usados com uma amostra nacionalmente representativa de participantes da pesquisa domiciliar (n=52.917). A regressão de Poisson foi utilizada para estimar razões de prevalência (RP) para avaliar a associação de diferentes perfis sociais com a aquisição de alimentos. O perfil caracterizado pela mulher auto classificada como branca, com maior escolaridade, cujas características estiveram positiva e significativamente associadas a maior aquisição de frutas (RP=1,22; IC95% 1,09-1,36), verduras e legumes (PR=1,24; IC95% 1,09-1,41) e queijo (RP=1,32; IC95% 1,09-1,59). Mulheres negras com baixa escolaridade apresentaram associação negativa com o consumo de refrigerantes (RP=0,53; IC95% 0,45-0,62) e alimentos preparados (RP=0,52; IC95% 0,37-0,74). The aim was to assess food expenditures according to the social profiles of the head of the households. Data from the cross-sectional Brazilian Household Budget Survey (2017/2018) were used with a nationally representative sample of household survey participants (n=52,917). Poisson regression was used to estimate prevalence ratios (PR) to assess the association of different social profiles with the acquisition of food. The profile characterized by woman self-classified as white, with a higher education, which characteristics were positively and significantly associated with more acquisition of fruits (PR=1.22; CI95% 1.09-1.36) and vegetables and greens (PR=1.24; CI95% 1.09-1.41). Black women with low education levels showed a negative association with the consumption of soda (PR=0.53; CI95% 0.45-0.62), and prepared food (PR=0.52; CI95% 0.37-0.74). The results reveal great inequalities in the purchase of food between the social profiles of the heads of the family.

RESUMO Pacientes com câncer avançado demandam cuidados paliativos. Nosso objetivo foi o de avaliar as barreiras ao encaminhamento ao cuidado paliativo na percepção de oncologistas. Desenvolvemos um estudo com oncologistas de uma instituição de referência nacional, questionando-os quanto a aspectos da sua formação acadêmica, à compreensão dos significados de cuidados paliativos, bem como limites e benefícios do encaminhamento de pacientes a uma unidade de cuidados paliativos exclusivos. A análise qualitativa foi realizada por meio da técnica de análise de conteúdo de Bardin. Participaram 19 oncologistas, que, apesar de definirem o cuidado paliativo com características multidisciplinares, voltado para doença avançada, com o objetivo de melhorar a qualidade de vida, relataram dificuldades no encaminhamento relacionadas ao próprio profissional, expectativas dos pacientes ou familiares, obstinação terapêutica e características institucionais. A criação de um ‘time consultor’ nas unidades de cuidados usuais foi a principal estratégia relatada como potencial facilitadora para essa transição de cuidados. Concluímos que a deliberação do cuidado paliativo exclusivo para pacientes com câncer avançado é uma tarefa difícil, que perpassa diferentes barreiras. A dicotomia existente entre ‘tratamento’ e ‘paliação’ na modalidade do cuidado paliativo exclusivo deve ser repensada, contrapondo a ideia do cuidado paliativo ofertado a partir do diagnóstico.

ABSTRACT Patients with advanced cancer demand palliative care. Our objective was to assess the barriers for referral to the palliative care in the perception of oncologists. In a study with the oncologists from a national reference institution who were asked about aspects related to their academic background, the understanding of the meanings of palliative care, as well as limits and benefits the referral of patients to an exclusive palliative care unit. Qualitative analysis was performed using Bardin’s content analysis. Nineteen oncologists participated. Despite defining the palliative care with multidisciplinary characteristics, aimed at advanced disease, with the objective of improving quality of life, they reported difficulties in the referral, related to the professional, expectations of patients/relatives, therapeutic obstinacy and institutional characteristics. The creation of a ‘Consulting Team’ in the usual care units was the main strategy reported as a facilitator potential for this care transition. We concluded that the deliberation of the exclusive palliative care for patients with advanced cancer is a difficult task, which goes through different barriers, which can result in a late referral. The dichotomy that exists between ‘treatment’ and ‘palliative care’ in the exclusive palliative care must be rethought, opposing the idea of palliative care offered from diagnosis.

RESUMEN Objetivo: Evaluar la eficacia del vídeo educativo sobre los conocimientos acerca de la reanimación cardiopulmonar de legos en la sala de espera de Unidad Básica de Salud. Metodología: Estudio cuasi-experimental del tipo pre y post test realizado de mayo a noviembre de 2019. La muestra consistió en 66 personas que estaban en la sala de espera, aguardando la consulta en la Unidad Básica de Salud.Para evaluar los conocimientos, se utilizó uncuestionario antes y después de la intervención educativa mediada por el vídeo educativo "Parada cardiorrespiratoria: cómo actuar para salvar". En el análisis se utilizaron estadísticas descriptivas y las pruebas de McNemar y Wilcoxon. Resultados: En el pre-test hubo una mediana de respuestas correctas de 5 (rango intercuartil = 7), mientras que en el post-test la mediana de respuestas correctas fue de 8 (rango intercuartil = 7), p<0,001. Hubo una mejora en los conocimientos, especialmente en las preguntas sobre el reconocimiento de la parada cardiopulmonar (p<0,001), el número de Servicio de atención móvil de emergencia (p<0,001), la posición de la víctima (p<0,001) y el nivel de fuerza de compresión (p<0,001). Conclusión: El vídeo educativo es eficaz para mejorar los conocimientos de los legos en materia de reanimación cardiopulmonar y puede utilizarse en la educación sanitaria de las personas en la sala de espera.

RESUMO Objetivo: Avaliar a efetividade de vídeo educativo no conhecimento de leigos em sala de espera de uma unidade básica de saúde acerca da reanimação cardiopulmonar. Metodologia: Estudo quase-experimental do tipo antes e depois realizado no período de maio a novembro de 2019. A amostra foi constituída por 66 pessoas que estavam em sala de espera, aguardando consulta na Unidade Básica de saúde. Para avaliar o conhecimento, foi utilizado teste teórico antes e após a intervenção educativa mediada pelo vídeo educativo “Parada cardiorrespiratória: como agir para salvar”. Na análise, utilizou-se a estatística descritiva e os testes de McNemar e Wilcoxon. Resultados: No pré-teste, houve mediana de acertos de 5 (Intervalo interquartílico=7), enquanto no pós-teste a mediana de acertos foi de 8 (Intervalo interquartílico=7), p<0,001. Houve melhora no conhecimento, principalmente nas questões sobre reconhecimento da PCR (p<0,001), número do Serviço de Atendimento Móvel de Urgência (SAMU) (p<0,001), posição da vítima (p<0,001) e nível de força nas compressões (p<0,001). Conclusão: O vídeo educativo é efetivo para a melhora no conhecimento de leigos sobre ressuscitação cardiopulmonar e pode ser realizado na educação em saúde de pessoas em sala de espera.

ABSTRACT Aim: To evaluate the effectiveness of an educational video about cardiopulmonary resuscitation on lay people in the waiting room of a basic health unit. Methods: This was a quasi-experimental study of the before-and-after type conducted from May to November 2019. The sample consisted of 66 people who were in the waiting room waiting for consultation in the Basic Health Unit. To evaluate the knowledge, a theoretical test was used before and after the educational intervention mediated by the educational video "Cardiorespiratory arrest: how to act to save". Descriptive statistics and the McNemar and Wilcoxon tests were used in the analysis. Results: In the pre-test, there was a median of correct answers of 5 (interquartile range = 7), while in the post-test, the median was 8 (interquartile range = 7), p<0.001. There was an increase in knowledge, especially on the questions about CRA recognition (p<0.001), Emergency mobile care service (SAMU) number (p<0.001), victim position (p<0.001), and compression force level (p<0.001). Conclusion: The educational video is effective in improving laypersons' knowledge about cardiopulmonary resuscitation and can be used in the health education of patients in the waiting room.

Resumo O objetivo deste artigo é avaliar o risco nutricional e cardiovascular segundo medidas antropométricas em idosos quilombolas do estado do Maranhão. Trata-se de estudo transversal realizado em 11 comunidades remanescentes de quilombolas do município de Bequimão, Maranhão, Brasil. Realizou-se censo da população idosa que representou 205 pessoas. Foram estimados os riscos nutricional e cardiovascular por meio dos indicadores antropométricos segundo sexo e idade. Realizou-se Testes de Qui-quadrado de Pearson ou Exacto de Fisher e análises de variância. Diferenças foram consideradas estatisticamente significantes quando p<0,05. Idosos quilombolas vivem em precárias condições de moradia e de infraestrutura sanitária, com elevado risco nutricional e cardiovascular, mas com diferenças entre sexo e idade. O excesso de peso foi mais prevalente em mulheres e idosos mais jovens, enquanto os homens e idosos com 80 ou mais anos apresentaram-se mais desnutridos e com maior perda de massa corporal. O risco cardiovascular foi maior entre as mulheres e em todas as faixas etárias. Idosos quilombolas vivem em vulnerabilidade socioeconômica e apresentaram alta prevalência de baixo peso, perda de massa muscular e alto risco cardiovascular, sendo maior risco entre mulheres e idosos do grupo de maior idade.

Abstract This article aims to assess nutritional and cardiovascular disease (CVD) risk based on anthropometric measures among older persons living in Quilombola communities in the state of Maranhão, Brazil. We conducted a cross-sectional study with 205 older persons living in 11 Quilombola communities in Bequimão, Maranhão. Nutritional and CVD risk were estimated according to sex and age group based on anthropometric indicators using Pearson’s chi-square or Fisher’s exact tests and analysis of variance, adopting a significance level of p<0.05. The study participants suffer precarious housing, basic sanitation and social conditions. Prevalence of nutritional and CDV risk was high across the sample, showing differences between sexes and age groups. Prevalence of excess weight was higher in women and the youngest age group, while prevalence of malnourishment and loss of muscle mass was higher in men and individuals aged 80 years and over. Prevalence of CVD risk was high across all age groups and higher in women than men. The older persons living in the Quilombola communities investigated by this study are socially vulnerable and showed high prevalence of low weight, loss of muscle mass and CDV risk. The prevalence of CVD risk was higher among women and the oldest age group.
