ABSTRACT Background: The objective of this study was to evaluate whether low magnesium concentration is related to increased cardiovascular risk in obese women. Methods: This case-control study included women, who were divided into two groups: case (BMI>35 kg/m2) and control group (18.5>BMI>24.9 kg/m2). Body weight, height, waist, neck and hip circumference were measured according to the methodology described by the Ministry of Health. The analyses of plasma, erythrocyte, and urinary magnesium concentrations were performed using the inductively coupled plasma optical emission spectrometry method. The lipid fractions were analyzed according to the enzymatic colorimetric method using a Cobas Integra automatic biochemical analyzer. Results: Mean plasma and erythrocyte magnesium concentrations were lower in the obese women compared to the control group (p<0.05). The urinary excretion of this mineral showed a significant difference between the two groups (p<0.05), and the obese women excreted a higher amount of magnesium in the urine. With regard to the cardiovascular risk parameters, obese women demonstrated higher values compared to the control group (p<0.05). The study showed a negative correlation between erythrocyte magnesium and systolic and diastolic blood pressure in the control group (p<0.05). Conclusion: These results indicate that obese women exhibit changes in nutritional status with regard to magnesium, with reduced concentrations in plasma and erythrocytes, and increased concentration in the urine. However, the results do not suggest a role of magnesium in protection against cardiovascular risk factors in the patients evaluated in this study.

RESUMEN El objetivo de este estudio fue evaluar si una concentración baja de magnesio está relacionada con un mayor riesgo cardiovascular en mujeres obesas. Métodos: Este estudio de casos y controles incluyó mujeres, que se dividieron en dos grupos: grupo de casos (IMC>35 kg/m2) y control (18,5>IMC>24,9 kg/m2). El peso corporal, la altura, la cintura, la circunferencia del cuello y la cadera se midieron según la metodología descrita por el Ministerio de Salud. Los análisis de las concentraciones de magnesio en plasma, eritrocitos y orina se realizaron utilizando el método de espectrometría de emisión óptica de plasma acoplado inductivamente. Las fracciones lipídicas se analizaron según el método colorimétrico enzimático utilizando un analizador bioquímico automático Cobas Integra. Resultados: Las concentraciones medias de magnesio en plasma y eritrocitos fueron menores en las mujeres obesas en comparación con el grupo de control (p<0.05). La excreción urinaria de este mineral mostró una diferencia significativa entre los dos grupos (p<0.05), y las mujeres obesas excretaron una mayor cantidad de magnesio en la orina. En cuanto a los parámetros de riesgo cardiovascular, las mujeres obesas mostraron valores más altos en comparación con el grupo control (p<0,05). El estudio mostró una correlación negativa entre el magnesio de los eritrocitos y la presión arterial sistólica y diastólica en el grupo de control (p<0,05). Conclusión: Estos resultados indican que las mujeres obesas presentan cambios en el estado nutricional con respecto al magnesio, con concentraciones reducidas en el plasma y eritrocitos, y concentración aumentada en la orina. Sin embargo, los resultados no sugieren un papel del magnesio en la protección contra los factores de riesgo cardiovascular en los pacientes evaluados en este estudio.

Resumo Contexto Os cateteres venosos centrais para hemodiálise (CVCH) de curta permanência (CCP) e cateteres tunelizáveis de longa permanência (CTLP) são fundamentais para a terapia hemodialítica. Entretanto, há escassa casuística nacional acerca da incidência de complicações desses dois tipos de cateteres. Objetivos Analisar as complicações e tempo de duração de CVCHs em centro de hemodiálise de hospital de ensino. Métodos Foi feito um estudo unicêntrico, longitudinal e retrospectivo de 115 pacientes consecutivos submetidos a implante de cateteres para hemodiálise (67 CCP e 48 CTLP) em um período de 2 anos, com análise de sobrevida geral, perviedade, perda do acesso e incidência de complicações. Resultados Sessenta por cento eram do sexo masculino e a média de idade foi de 62 anos. O principal sítio de punção foi a veia jugular interna direita. Hipertensão arterial sistêmica estava presente em 95% dos casos. A mediana de permanência do cateter foi de 50 dias (CCP) versus 112 dias (CTLP; p < 0,0001). Não houve diferença na sobrevida global. Infecção relacionada ao cateter apresentou maior incidência nos CCP, sendo Staphylococcus sp. o microrganismo mais encontrado. A taxa de infecção por 1.000 dias foi maior nos CCP em relação aos CTLP (16,7 eventos/1.000 dias versus 7,0 eventos/1.000 dias, respectivamente). Baixa renda foi o único fator relacionado a maior incidência de infecção. Conclusões O tempo de permanência dos CTLP foi significativamente maior que os CCP, porém ainda assim abaixo dos valores relatados na literatura e sem impacto na sobrevida global. Baixa renda foi um fator associado a infecção de cateter.

Abstract Background Short-term (ST) and long-term tunneled (LTT) central venous catheters for hemodialysis (CVCH) are critical for hemodialysis therapy. However, few studies have been conducted in Brazil to investigate the incidence of complications with these two types of catheters. Objectives To analyze complications and duration of CVCH in a hemodialysis center at a teaching hospital. Methods Single-center, longitudinal, and retrospective study of 115 consecutive patients undergoing hemodialysis catheter placement (67 ST and 48 LTT) over a 2-year period, analyzing overall survival, patency, loss of access, and incidence of complications. Results Sixty percent of the patients were male and mean age was 62 years. The most common puncture site was the right internal jugular vein. Systemic arterial hypertension was present in 95% of cases. Median catheter in-place duration was 50 days (ST) vs. 112 days (LTT; p < 0.0001). There was no difference in overall survival. Incidence of catheter-related infection was higher in ST CVCH, with Staphylococcus sp. the microorganism most often found. The infection rate per 1000 days was higher in ST than in LTT catheters (16.7 events/1000 days vs. 7.0 events/1000 days). Low income was the only factor related to higher incidence of infection. Conclusions The in-place duration of long-term catheters was significantly longer compared to short-term CVCH, but still below the values reported in the literature and without impact on overall survival. Low income was a factor associated with catheter infection.

Abstract The dentine hypersensitivity (DH) is able to impair the oral health related quality of life (OHRQoL). However, there isn't any specific validated questionnaire to be used in Brazil. The objective was to adapt and to validate the English version of the Dentine Hypersensitivity Experience Questionnaire (DHEQ-15) for use in Brazil. DHEQ-15 was cross-culturally adapted into the Brazilian-Portuguese language and then validated in a cross-sectional study with 100 participants recruited at a University clinic. Study sample comprised 2 groups: 100 individuals with DH, and 100 individuals without. The instrument was self-administered twice 7 to 10 days apart. The participants answered a global rating of oral health. The psychometric properties of the Brazilian version of DHEQ-15 were verified through internal consistency (Cronbach's α) and test-retest reliability (intraclass correlation coefficient - ICC), convergent (Spearman correlation) and discriminant (Mann-Whitney test) validity. The significance threshold was set at p<0.05. Sample comprised 69 men and 131 women, of mean age 30.4y. The Brazilian DHEQ-15 demonstrated very good internal consistency (α = 0.945). Test-retest reliability revealed excellent reproducibility (ICC = 0.959, p < 0.001). There was statistically significant correlation between the scores obtained on all DHEQ-15 domains and the global rating of oral health (p<0.001). Participants with DH scored significantly higher than those without DH (p<0.001). This study provides evidence supporting the cross-cultural validity of the Brazilian version of DHEQ-15 for use in Brazil.
