Abstract Objetives: To assess if patients with Chagasic heart disease (CHD) received effective automatic implantable defibrillator (AID) shocks earlier than patients with ischemic heart disease (IHD). Methods: Retrospective cohort of patients with CHD and IHD who received an implantable cardioverter defibrillator (ICD) between 2009 and 2018, in a tertiary hospital. We evaluated the time between the implant of ICD and the first effective shock in patients with CHD and compared it with the IHD control population. Results: We included a total of 64 patients, 20 with CHD and 44 with IHD. CHD patients presented earlier an effective shock than patients with IHD during the first year (hazard ratio [HR]: 8.4; 95% confidence interval [95% CI]: 2.09-34.02; p = 0.0027), and at three years (HR: 4.61; 95% CI: 1.51-14.07; p = 0.0072). 100% of CHD patients who received the ICD as secondary prevention of sudden cardiac death presented an effective shock during the first 26 months of follow-up. Conclusions: Patients with CHD received effective ICD shocks earlier than the IHD patients. All patients with CHD and ICD as secondary prevention had an appropriate ICD shock at short term, representing the highest risk population, and supporting the indication of the device in a setting where randomized clinical trials are lacking.

Resumen Objetivos: Determinar si los pacientes con cardiopatía chagásica (CCh) presentaron choques apropiados del desfibrilador automático implantable (DAI) de manera más precoz que los pacientes con cardiopatía isquémica (CI). Métodos: Estudio de cohorte retrospectivo que incluyó los pacientes con CCh y CI en quienes se implantó un DAI entre los años 2009 y 2018 en un hospital de alta complejidad. El seguimiento se realizó hasta los 36 meses, evaluándose el momento del primer choque apropiado del dispositivo. Resultados: Se incluyeron 64 pacientes, 20 con CCh y 44 con CI. Se observó que una mayor proporción de pacientes con CCh presentaron choques apropiados durante el primer año (hazard ratio [HR]: 8.4; intervalo de confianza del 95% [IC95%]: 2.09-34.02; p = 0.0027) y a 3 años (HR: 4.61; IC95%: 1.51-14.07; p = 0.0072). El 100% de la población con CCh e implante del DAI como prevención secundaria de muerte súbita presentaron choques apropiados durante los primeros 26 meses de seguimiento. Conclusiones: Los pacientes con CCh presentaron choques apropiados del DAI de manera más precoz que los pacientes con CI. Todos los pacientes con CCh y DAI como prevención secundaria presentaron choques apropiados, representando una población de mayor riesgo. Esta información apoya la indicación del DAI en estos pacientes a pesar de la escasa evidencia en ensayos aleatorizados.

ABSTRACT Artificial cultivation increases clams' availability and is an alternative to the extraction from natural banks. The culture of clams requires species-specific research in the different growth stages, and studies on the effects and interactions of culture parameters are essential to obtain and control the proper development of larvae. This paper aims to compare methods to induce spawning, describe the embryonic development, and compare the effect of different culture densities on the yield of "D" larvae of the "taca" clam Ameghinomya antiqua. Breeders were collected on the southwest coast of Quinchao Island, Chiloé, Chile. Spawning induction assays were performed comparing different combinations of biological and physical factors. Experiments on the effect of embryonic density in the obtention of "D" larvae were performed, and the embryonic development was described at 11 ± 1°C. The spawning inductions were successfully achieved with the addition of food combined with temperature changes, resulting in the liberation of oocytes with a jelly coat with a diameter of 140 μm. Trochophore larvae were observed at 40 h post-fertilization. The percentage of embryos developed showed significant differences when testing cultures with densities of 20, 40, and 60 embryos mL-1. Experiments with 20 embryos mL-1 density were the ones that obtained a greater number of developed embryos (50%). These results suggest spawning induction with the addition of food and temperature changes with a density of 20 embryos mL-1. This paper describes the embryonic development and technology development for spawning induction for the first time.

ABSTRACT Software testing focuses on detecting defects or failures during code execution. Testing is a challenging creative task, requiring automation. The adoption of good practices and testing strategies contributes to increasing the efficiency of software development companies. MTest.search model for automatic unit test generation has defined domain model extension, test, and execution mechanisms. In this work, mechanisms to extend the search-based reduction model are presented. The proposed extensions take into account the objects and sets involved in the source code, and enhance the detection of defects or failures based on the significance of the values and paths / scenarios involved in the test. To validate the proposal, three case studies were defined using classical methods and real projects.

RESUMEN Las pruebas de software se centran en la detección de defectos o fallos durante la ejecución del código. La prueba constituye una tarea creativa desafiante, por lo que se hace necesaria su automatización. La adopción de buenas prácticas y estrategias de pruebas, contribuye a la elevar la eficiencia de las empresas desarrolladoras de softwares. El modelo MTest.search para la generación automática de pruebas unitarias tiene definidos mecanismos de extensión del modelo de dominio, de prueba y de ejecución. En este trabajo se presentan mecanismos para extender el modelo de reducción basado en búsquedas. Las extensiones propuestas tienen en cuenta los objetos y conjuntos involucrados en el código fuente, y potencia la detección de defectos o fallos a partir de la significación de los valores y caminos/escenarios involucrados en la prueba. Para validar la propuesta se definieron dos casos de estudios empleando métodos clásicos y de proyectos reales.

ABSTRACT Introduction: fractures of the distal end of the radius are very frequent and most of them require surgical reduction. Objectives: to characterize patients with fractures of the distal third of the radius treated surgically with percutaneous fixation with K-wires according to epidemiological and clinical variables of interest. Methods: a cross-sectional, descriptive and prospective study with a quantitative-qualitative approach was carried out at the “Arnaldo Milián Castro” Hospital of Villa Clara Province, in the period from September 1, 2018 to August 31, 2019. The universe was integrated by all patients with confirmed diagnosis of fractures of the distal end of the radius treated surgically with percutaneous fixation with K-wires and the sample by 22. Results: fractures of the distal third of the radius were more frequent in female patients, A2 and B1 fractures were fixed more regularly, radiography played a decisive role in the evaluation and follow-up and the few complications showed the validity of this simple and inexpensive treatment. Conclusions: the results of surgical treatment of fractures of the distal third of the radius treated with percutaneous fixation with K-wires were evaluated, most of the patients presented satisfactory results (excellent or good)

RESUMEN Introducción: las fracturas del extremo distal del radio son muy frecuentes y en su gran mayoría requieren de reducción quirúrgica. Objetivos: caracterizar los pacientes con fracturas del tercio distal del radio tratados quirúrgicamente con fijación percutánea con agujas de Kirschner según variables epidemiológicas y clínicas de interés. Métodos: se realizó un estudio transversal, descriptivo y prospectivo con enfoque cuanti-cualitativo en el Hospital “Arnaldo Milián Castro” de la Provincia de Villa Clara, en el período comprendido desde el primero de septiembre de 2018 hasta el 31 de agosto de 2019. El universo estuvo integrado por todos los pacientes con diagnóstico confirmado de fracturas del extremo distal del radio tratados quirúrgicamente con fijación percutánea con agujas de Kirschner y la muestra por 22. Resultados: las fracturas del tercio distal del radio fueron más frecuentes en las pacientes femeninas, las fracturas A2 Y B1 fueron fijadas con mayor regularidad, la radiografía jugó un papel determinante en la evaluación y el seguimiento y las pocas complicaciones evidenciaron la vigencia de este sencillo y barato tratamiento. Conclusiones: al evaluar los resultados del tratamiento quirúrgico de las fracturas del tercio distal del radio tratadas con fijación percutánea con agujas de Kirschner la mayoría de los pacientes presentaron resultados satisfactorios (excelente o bien).

ABSTRACT INTRODUCTION The progress report on provincial areas for public health research was updated through the Ministerial Network for Health Research (REMINSA) METHODS Virtual meetings were held for discussion with REMINSA referents, and a survey in Google Forms® on regulatory, organizational and budgetary behavior was conducted, including task scope and needs for improvement RESULTS A total of 21 responses were received from 21 provinces with referents. There is a broad framework of regulations for human health research and, to a lesser extent, for the promotion of public health research. The results showed that 71% have some institutional structure, 20% offer research grants, and 20% have medical residencies with a research profile. The main activity is regulation or participation in ethics committees. The promotion, dissemination of results and development of evidence-based research projects were relegated (20%). The research areas are well-established in most of the provinces DISCUSSION The challenge lies in promoting the development of these systems in provinces without ministerial areas and strengthening the jurisdictions that do have such areas. REMINSA has identified opportunities to improve communication between jurisdictions, regulations for the promotion of research, and networking for research, management and decision-making.

RESUMEN INTRODUCCIÓN A través de la Red Ministerial de Investigación en Salud (REMINSA) se actualizó el estado de situación de las áreas provinciales de investigación en salud pública MÉTODOS Se realizaron reuniones virtuales de debate con los referentes REMINSA y una encuesta en Google Forms® sobre funcionamiento normativo, organizacional, presupuestario, alcance de tareas y necesidades de mejoras RESULTADOS Se recibieron 21 respuestas de un total de 21 provincias con referentes. Se verificó la existencia de un amplio marco de normativas regulatorias de la investigación en salud humana y, en menor medida, para impulsar la investigación en el marco de la salud pública. El 71% tiene cierta estructura institucional, el 20% ofrece becas de investigación, y el 20% posee residencias con perfil en investigación. La principal actividad es la regulación o participación en comités de ética. La promoción, difusión de resultados y elaboración de proyectos de investigación basada en evidencia quedaron relegadas (20%). Las áreas de investigación están consolidadas en la mayoría de las provincias DISCUSIÓN El desafío consiste en promover el desarrollo de estos sistemas en las provincias que no cuentan con áreas ministeriales y fortalecer las provincias que sí las poseen. REMINSA ha identificado oportunidades de mejorar la comunicación entre jurisdicciones, las normativas de promoción de la investigación y el trabajo en red para la investigación, la gestión y la toma de decisiones. ^s+

RESUMEN Objetivo. La mortalidad por cáncer en la región centro de Argentina está dentro de las más elevadas del país. Dos posibles escenarios ambientales podrían explicar esta situación, su actividad agrícola y la calidad del agua de consumo. El objetivo del estudio es evaluar la interacción existente entre estas variables. Métodos. Estudio retrospectivo de diseño ecológico. Se analizaron decenios de superficies sembradas totales (SST) y el porcentaje de población expuesta a niveles elevados de arsénico y nitratos en aguas de consumo en relación con las tasas de mortalidad total (TMT) y órgano-específicas (TME); se calculó el coeficiente de correlación de Spearman. Resultados. Se hallaron correlaciones positivas y significativas entre los dos primeros decenios de SST y el primer decenio de TMT, así como correlaciones con cáncer de pulmón, páncreas y colon en hombres; y con cáncer de colon en mujeres. Los niveles elevados de arsénico se asocian con las TMT en ambos sexos y en todos los decenios, aunque de manera específica con cáncer de pulmón en hombres. No se hallaron correlaciones significativas con nitratos. Conclusiones. La asociación entre las SST y las TMT y TME presentan mayor intensidad cuanto más cerca están entre ellas temporalmente, y expresan un mayor impacto de los cultivos más antiguos. La asociación entre las TMT y el consumo de aguas arsenicales, así como la hallada en TME de pulmón en hombres y colon en mujeres, muestran que esta exposición histórica es central para entender las condiciones epidemiológicas regionales. Se deben explorar fuerzas sinérgicas entre estas variables.

ABSTRACT Objective. Cancer mortality in the central region of Argentina is among the highest in the country. Two possible environmental factors could explain this situation: agricultural activity and drinking water quality. The objective of the study is to evaluate the interaction between these variables. Methods. This is a retrospective ecological study. Total cropland over 10-year periods as well as the percentage of the population exposed to high levels of arsenic and nitrates in drinking water were analyzed and compared to total mortality rates (TMR) and organ-specific mortality rates (SMR); the Spearman's rank correlation coefficient was then calculated. Results. Positive and significant correlations were found between the first two 10-year periods of total cropland and the first 10-year period of TMR, as well as correlations with lung, pancreatic, and colon cancers in men and colon cancer in women. Elevated arsenic levels are associated with TMR in both sexes across all 10-year periods but are specifically associated with lung cancer in men. No significant correlation was found with nitrates. Conclusions. The association between total cropland and TMR/SMR is stronger with proximity over time, with greater impact from the oldest crops. The association between TMR and consumption of water containing arsenic, as well as the association with lung SMR in men and colon SMR in women, show that exposure over time is essential to understanding regional epidemiological conditions. The synergy between these variables should be explored.

RESUMO Objetivo. A mortalidade por câncer na região central da Argentina está entre as mais elevadas do país. Dois possíveis cenários ambientais poderiam explicar esta situação: a atividade agrícola e a qualidade da água para consumo humano. O objetivo do estudo é avaliar a interação entre essas variáveis. Métodos. Estudo ecológico retrospectivo. Foram analisadas a área semeada total (AST) em decênios e a porcentagem da população exposta a níveis elevados de arsênio e nitrato na água para consumo humano em relação às taxas de mortalidade total (TMT) e específica por órgão (TME). A correlação foi analisada mediante cálculo de coeficientes de Spearman. Resultados. Foram constatadas correlações positivas e significativas entre os dois primeiros decênios de AST e o primeiro decênio de TMT, bem como correlações com câncer de pulmão, de pâncreas e colorretal nos homens e com câncer colorretal nas mulheres. Níveis elevados de arsênio apresentaram associação com a TMT em ambos os sexos e em todos os decênios e, mais especificamente, com a mortalidade por câncer de pulmão em homens. Não foram constatadas correlações significativas com nitratos. Conclusões. A associação entre AST e as taxas de mortalidade total e específica por órgão é tanto mais forte quanto mais próximas estão uma da outra no tempo, e expressa um maior impacto dos cultivos mais antigos. A associação entre TMT e o consumo de águas arsenicais, bem como a associação encontrada na TME por câncer de pulmão em homens e na TME por câncer colorretal em mulheres, mostra que esta exposição histórica é fundamental para compreender as condições epidemiológicas regionais. Forças sinérgicas entre estas variáveis devem ser exploradas.

Resumen La incidencia de fractura de cadera cada año es muy alta, lo que genera una importante carga asistencial y socioeconómica. Estas fracturas pueden producirse a cualquier edad, pero la gran mayoría ocurren en personas mayores de 65 años y con predominancia del sexo femenino, debido al mayor riesgo de osteoporosis tras la menopausia. La diabetes mellitus tipo 2 (DM2), aparte de alterar el metabolismo glucídico, lipídico y proteico, también causa una desregulación del calcio, fósforo y magnesio, y alteraciones del metabolismo óseo. La prevalencia de desnutrición en pacientes con fractura de cadera es también elevada, por la edad avanzada, y la misma lesión aguda generar respuestas catabólicas e inflamatorias que resultan en desnutrición relacionada con la enfermedad y sarcopenia, lo que agrava el estado clínico del paciente. El presente artículo describe los resultados del consenso de expertos y las respuestas de los panelistas sobre el manejo nutricional en la práctica clínica habitual de los pacientes con diabetes/hiperglucemia hospitalizados en planta (no críticos) con fractura de cadera.

Abstract The yearly incidence of hip fracture is very high, which generates significant healthcare and socioeconomic burden. These fractures can occur at any age, but the vast majority occur in people over 65 years of age and predominantly in women, due to the increased risk of menopause-associated osteoporosis. Type 2 diabetes mellitus (DM2), apart from altering glucose, lipid and protein metabolisms, also causes a deregulation of calcium, phosphorus and magnesium and dysfunction in bone metabolism. The prevalence of malnutrition in patients with hip fracture is also high, due to their advanced age, and the acute injury itself provokes catabolic and inflammatory responses that result in disease-related malnutrition and sarcopenia, which aggravates the patient's clinical condition. This article describes the results of the expert consensus and the responses of the panelists on the nutritional management in routine clinical practice of patients with diabetes/hyperglycemia hospitalized (non-critically ill) with hip fracture.

Resumen La insuficiencia cardíaca (IC) es una de las principales causas de morbilidad y mortalidad entre las personas mayores, lo que la convierte en un importante problema de salud pública. Las enfermedades cardiovasculares en general, y la IC en particular, son comorbilidades frecuentes en personas con diabetes tipo 2 (DM2). La presencia de DM2 e IC se asocia con síntomas y signos clínicos más graves, y peor calidad de vida y pronóstico. Además, debido al estado hipercatabólico y los trastornos de la absorción de nutrientes, la desnutrición está presente en muchos casos de IC. El presente artículo describe los resultados del consenso de expertos y las respuestas de los panelistas sobre el manejo nutricional en la práctica clínica habitual de los pacientes con diabetes/hiperglucemia hospitalizados en planta (no críticos) con IC.

Abstract Heart failure (HF) is one of the leading causes of morbidity and mortality among older people, making it a major public health problem. Cardiovascular diseases in general, and HF in particular, are common comorbidities in people with type 2 diabetes (DM2). The concurrence of DM2 and HF is associated with more severe clinical symptoms and signs, and poorer quality of life and prognosis. Furthermore, due to the hypercatabolic state and nutrient absorption disorders, malnutrition is present in many HF cases. This article describes the results of the expert consensus and the responses of the panelists on the nutritional management in routine clinical practice of patients with diabetes/hyperglycemia hospitalized (non-critically ill) with HF.

Resumen La diabetes es una comorbilidad frecuente en pacientes con cáncer, ya que comparten factores de riesgo comunes. En la enfermedad oncológica, la presencia de caquexia tumoral representa un factor de mal pronóstico, que se ve agravado por un estado nutricional deficiente. Clínicamente, la caquexia se manifiesta como una reducción significativa del peso corporal, acompañado de cambios en la composición corporal y alteraciones en el equilibrio del sistema biológico, y causa una disfunción progresiva. El presente artículo describe los resultados del consenso de expertos y las respuestas de los panelistas sobre el manejo nutricional en la práctica clínica habitual de los pacientes con diabetes/hiperglucemia hospitalizados en planta (no críticos) con caquexia tumoral concurrente.

Abstract Diabetes is a frequent comorbidity in cancer patients, since they share common risk factors. In cancer, the concurrence of cachexia represents a poor prognostic factor, which is aggravated by poor nutritional status. Clinically, cancer cachexia manifests as a significant reduction in body weight, accompanied by changes in body composition and alterations in the balance of the biological system, and causes progressive dysfunction. This article describes the results of the expert consensus and the responses of the panelists on the nutritional management in routine clinical practice of patients with diabetes/hyperglycemia hospitalized (non-critically ill) with cancer cachexia.

Resumen La sarcopenia se describe como una afección multidimensional que afecta negativamente a la masa muscular, la fuerza muscular y el rendimiento físico. La prevalencia de sarcopenia en personas con diabetes es muy superior a la de la población general, especialmente en individuos que presentan un estado nutricional deficiente. Tanto la sarcopenia como la desnutrición son condiciones susceptibles de intervención para mejorar el pronóstico clínico. El presente artículo describe los resultados del consenso de expertos y las respuestas de los panelistas sobre el manejo nutricional en la práctica clínica habitual de los pacientes con diabetes/hiperglucemia hospitalizados en planta (no críticos) con sarcopenia concurrente.

Abstract Sarcopenia is a multidimensional condition that negatively affects muscle mass, muscle strength, and physical performance. The prevalence of sarcopenia in people with diabetes is much higher than that of the general population, especially in individuals with poor nutritional status. Both sarcopenia and malnutrition are conditions amenable to intervention to improve clinical prognosis. This article describes the results of the expert consensus and the responses of the panelists on the nutritional management in routine clinical practice of patients with diabetes/hyperglycemia hospitalized (non-critically ill) with sarcopenia.

Resumen La diabetes mellitus tipo 2 es una enfermedad muy prevalente en todo el mundo y genera una carga clínica y económica creciente debido a sus complicaciones micro- y macrovasculares. Con frecuencia, las personas con diabetes son hospitalizadas por diversas patologías. Estos pacientes tienen, por lo general, un mayor riesgo de complicaciones, de estancias prolongadas y de mortalidad. Un factor adicional que empeora el pronóstico en estos pacientes es la presencia de desnutrición, sobre todo en personas de edad avanzada. Todo ello hace que el manejo de estos pacientes sea complejo y requiera un abordaje nutricional específico, cuya finalidad sea cubrir los requerimientos nutricionales manteniendo siempre el control glucémico. La finalidad de este trabajo es generar, en base a los datos disponibles en la bibliografía y la experiencia clínica, recomendaciones consensuadas por parte de dieciocho expertos en Endocrinología y Nutrición sobre el abordaje nutricional de pacientes hospitalizados con diabetes/hiperglucemia y comparar el manejo óptimo basado en estas recomendaciones con la atención habitual a pie de cama, según un panel de médicos españoles encuestados sobre su práctica clínica diaria. En este primer artículo de este número extraordinario de la revista Nutrición Hospitalaria, se describe la metodología seguida y los resultados obtenidos sobre las cuestiones comunes para todas las patologías.

Abstract Type 2 diabetes mellitus is a highly prevalent disease worldwide, generating an increasing clinical and economic burden due to its micro- and macrovascular complications. Frequently, people with diabetes are hospitalized for various pathologies. These patients generally have higher risk of complications, prolonged hospitalizations and mortality. An additional factor that worsens the prognosis in these patients is the concurrence of malnutrition, especially in elderly people. All this makes the management of these patients challenging and requires a specific nutritional approach, whose purpose is to cover the nutritional requirements while always maintaining glycemic control. The purpose of this work is to provide, based on the evidence available in the literature and clinical experience, consensus recommendations by eighteen experts in Endocrinology and Nutrition on the nutritional approach of hospitalized patients with diabetes/hyperglycemia and compare the optimal management, based on these recommendations with bedside usual care according to a panel of Spanish doctors surveyed about their daily clinical practice. This first article of this extraordinary issue of the journal Nutrición Hospitalaria describes the methodology of the study and the results obtained regarding common issues for all pathologies.

Resumen Las personas con diabetes tienen un riesgo elevado de requerir una intervención quirúrgica a lo largo de su vida y de tener complicaciones perioperatorias en caso de un control metabólico deficiente. La hospitalización representa un evento estresante que, unido a otros factores asociados a procedimientos diagnósticos y terapéuticos, conlleva un deterioro del estado nutricional de los pacientes. Se ha observado una asociación entre un estado nutricional deficiente y resultados adversos en pacientes quirúrgicos. El presente artículo describe los resultados del consenso de expertos y las respuestas de los panelistas sobre el manejo nutricional en la práctica clínica habitual de los pacientes con diabetes/hiperglucemia hospitalizados en planta (no críticos) en el periodo perioperatorio.

Abstract People with diabetes are at high risk of requiring surgical intervention throughout their lives, and of perioperative complications in case of poor metabolic control. Hospitalization represents a stressful event that, together with other factors associated with diagnostic and therapeutic procedures, leads to a deterioration in the nutritional status of the patients. An association between poor nutritional status and adverse outcomes in surgical patients has been observed. This article describes the results of the expert consensus and the responses of the panelists on the nutritional management in routine clinical practice of patients with diabetes/hyperglycemia hospitalized (non-critically ill) in the perioperative setting.

ABSTRACT INTRODUCTION : A participatory National Public Health Research Agenda (ANISP) with thematic prioritization is a strategic element to generóte evidence-based recommendations and public policies that have a positive impact on the health of populations and enable to achieve health objectives. The Directorate of Health Research (DIS) ofthe Argentine Ministry of Health (MSAL), through the Ministerial NetWork of Health Research (REMINSA), along with adors from the provincial and national government levels belonging to public, privóte, health, academic and research sectors participated in the update of the ANISP. They adapted the original tooI proposed by the Pan American Health Organizatlon and used in the process in 2019. The update included different stages. The selection ofthe topics had the legitimacy, recognition and participation ofthe actors involved, related to health, to government and privóte management and to scientific research; the work was conducted in a federal and transversal manner by consensus with the provincial networks and a Central Advisory Committee in the MSAL. Based on the preliminary guidelines obtained, a semi-structured online survey was developed and distributed to all federal actors, receiving 431 responses. The process resulted in 55 priorilized guidelines, divided into 6 thematic oreas and 33 sub-themes, selected by voting according to importance, impact and feasibility.

RESUMEN INTRODUCCIÓN : Una Agenda Nacional de Investigación en Salud Pública (ANISP) participativa y con priorización temática constituye un elemento estratégico para generar recomendaciones y políticas públicas basadas en evidencia, que imparten positivamente en la salud de las poblaciones y permitan lograr los objetivos sanitarios. En la actualización de la ANISP participaron la Dirección de Investigación en Salud (DIS) del Ministerio de Salud de la Nación (MSAL), a través de la Red Ministerial de Investigación en Salud (REMINSA), y actores de los niveles gubernamentales provinciales y nacionales pertenecientes a los sectores público, privado, de la salud, académico y de investigación. Se adaptó la herramienta original propuesta por la Organización Panamericana de la Salud, utilizada en el proceso en 2019. La actualización abarcó diferentes etapas. La selección de los temas contó con la legitimidad, reconocimiento y participación de los actores vinculados a la salud, a la gestión gubernamental y privada y a la investigación científica; se trabajó de manera federal y transversal, por consenso con las redes provinciales y un Comité Central Asesor en el MSAL. A partir de los lineamientos preliminares obtenidos, se elaboró una encuesta en línea semiestructurada, que fue distribuida a todos los actores federales y recibió 431 respuestas. El proceso resultó en 55 lineamientos priorizados, divididos en 6 áreas temáticas y 33 subtemas, seleccionados por votación según importancia, impacto y factibilidad.

ABSTRACT INTRODUCTION : Cáncer is an important cause of mortality around the world; the Central Región (CR) of Argentina concentrates the highest rates in the country. The objective was to describe cancer mortality rates in the CR and their behavior o ver a long period of time. METHODS : A descriptive study was conducted. Mortality rates for malignant neoplasms were calculated by gender and adjusted by age for each five-year period, expressed per 100,000 population during 1992-2016. The Estimated Annual Percent Change was applied for trends. Spatial distribution was expressed by quintiles of joint distribution of periods. RESULTS : Both provincial and departmental rates were above the national rates for the same five-year periods (men from Entre Ríos province in 1992-1996 showed the highest rate, while women from Córdoba province in 2007-2011 showed the lowest one). Provincial rates showed a downward trend for both men and women. Departmental rates showed prevailing decreasing trends. DISCUSSION : High mortality rates in the CR had already been observed in the 1980s and still persist. Considerable differences in mortality between provinces, departments, and males and females were found. Findings were summarized through graphics, which showed a movement in the concentration of region mortality, located on the northern border of Entre Ríos and Santa Fe by the end of the period.

RESUMEN INTRODUCCIÓN : El cáncer es una causa importante de mortalidad en el mundo; la Región Centro (RC) de Argentina concentra las mayores tasas del país. El objetivo del estudio fue describir las tasas de mortalidad por cáncer en la RC y su comportamiento en un período de tiempo prolongado. MÉTODOS : Se efectuó un estudio descriptivo. Se calcularon tasas de mortalidad por neoplasias malignas de cada quinquenio por sexo y ajustadas por edad, expresadas por 100 000 habitantes durante 1992-2016. Para las tendencias se aplicó el Porcentaje de Cambio Anual Estimado. La distribución espacial se realizó según quintiles de la distribución conjunta de períodos. RESULTADOS : Tanto las tasas provinciales como departamentales se encontraron por encima de las nacionales para su respectivo quinquenio (la más alta, en hombres de Entre Ríos en 1992-1996; la más baja, en mujeres de Córdoba en 2007-2011). Se observó una tendencia en descenso de las tasas provinciales para ambos sexos. Los departamentos mostraron un predominio de tendencias decrecientes. DISCUSIÓN : Las altas tasas de mortalidad encontradas en la RC ya habían sido observadas en la década del 80 y aún persisten. Se hallaron marcadas diferencias de la mortalidad entre las provincias, entre los departamentos y entre sexos. La graficación permitió resumir los hallazgos y observar un movimiento de la concentración de la mortalidad en la región, con localización en la frontera norte de Entre Ríos y Santa Fe al final del período.

Resumo Microplásticos são partículas plásticas menores que 5 mm encontradas no meio ambiente, que podem causar problemas à saúde dos organismos aquáticos e humanos, sendo classificados como poluentes emergentes. Na Guatemala, o Lago Amatitlán é um dos lagos mais poluídos, pois recebe descargas de águas residuais, tratadas e não tratadas, bem como outros resíduos da Cidade da Guatemala e de outras populações urbanas. Em um estudo recente, foram encontrados microplásticos em sedimentos do Lago Amatitlán. Então era necessário saber se os peixes do lago estão ingerindo microplásticos, que podem ser prejudiciais. Assím, o objetivo desta pesquisa foi determinar os microplásticos em peixes do Lago Amatitlán, para o qual foram coletados 65 exemplares de tilápia do Nilo (Oreochromis niloticus) nos meses de outubro e dezembro de 2020 e janeiro de 2021. Os peixes foram dissecados no laboratório, onde o número e tipo de microplásticos foram analisados no trato digestivo, após sua separação por digestão e filtração. Linhas ou fibras foram o tipo de microplástico mais comum, sendo encontradas em 63 espécimes (96,9%), variando de 0 a 27 linhas / fibras por indivíduo. A frequência de outros tipos de microplásticos encontrados foi de 0-3 fragmentos / indivíduo, 0-2 filmes / indivíduo e 0-4 espumas / indivíduo, enquanto microesferas não foram encontradas em nenhum espécime. A presença de microplásticos na tilápia do Lago Amatitlán é um indicador de contaminação do lago por poluentes que podem afetar a rede trófica e representa um risco para os consumidores de peixe, exigindo atenção das autoridades ambientais e sanitárias.

Abstract Microplastics are plastic particles smaller than 5 mm found in the environment, which can cause health problems for aquatic organisms and humans, being classified as emerging pollutants. In Guatemala, Lake Amatitlán is one of the most polluted lakes as it receives discharges of wastewater, treated and untreated, as well as other waste from Guatemala City and other major urban populations. In a recent study, microplastics were found in sediments in Lake Amatitlán, so it was necessary to determine whether the lake's fish are affected by these pollutants, which can be harmful to their health. This research aimed to determine the microplastics in fish from Lake Amatitlán, for which 65 specimens of Nile tilapia (Oreochromis niloticus) were collected in October and December 2020, and January 2021. The collected fish were dissected in the laboratory, where the number and type of microplastics were analyzed in the digestive tract, after their separation by digestion and filtration. Lines or fibers were the most common type of microplastics, found in 63 specimens (96.9% of the analyzed specimens), ranging from 0 to 27 lines/fibers per individual. The frequency of other types of microplastics found were 0-3 fragments/individual, 0-2 films/individual, and 0-4 foams/individual, while microspheres were not found in any specimen. The presence of microplastics in tilapia from Lake Amatitlán is an indicator of contamination in the lake by pollutants that could affect the trophic network and represents a risk for the fish consumers, requiring the attention of environmental and health authorities.
