Abstract One of the most widespread applications of poly(lactic acid) (PLA) is as a raw material in the form of filaments for 3D printing. To improve final disposal alternatives and minimize their effects on the environment, the aim of this study is to determine the biodegradability of 3D-printed PLA waste composted in a landfill with leachate soil and garden soil for 90 days and 180 days. The soil characteristics and material properties were evaluated by laboratory analyses. Changes in soil chemical composition and the loss of microorganisms were recorded. The thermal and mechanical properties of PLA did not change significantly, but fungal colonies, encrustation, and changes in the original colour were found, indicating the onset of surface biodegradation of the samples. Abstract This paper presents a novel nature-inspired tri-band frequency selective surface (FSS) characterized by closely spaced bands, angular stability, and polarization independence. The FSS element adopts a segmented Fibonacci spiral geometry. While a single-segmented Fibonacci spiral does not inherently possess polarization independence, the proposed proof of concept achieves this by cascading the same design geometry rotated by 90° between them. Resonant frequencies of 1.63 GHz, 2.46 GHz, and 3.43 GHz are achieved, with minimal separation ratios of 1.5 from the second to the first frequency and 1.4 from the third to the second. Additionally, angular stability is confirmed up to 45°. Abstract Background: Ultra-low-dose Spinal Anesthesia (SA) is the practice of employing minimal doses of intrathecal agents so that only the roots that supply a specific area are anesthetized. The aim of this study was to compare the effectiveness and safety of ultra-low-dose spinal anesthesia with that of Perineal Blocks (PB). Methods: A two-arm, parallel, double-blind randomized controlled trial comparing two anesthetic techniques (SA and PB) for hemorrhoidectomy and anal fistula surgery was performed. The primary outcomes were postoperative pain, complementation and/or conversion of anesthesia, and hemodynamic changes. Results: Fifty-nine patients were included in the final analysis. The mean pain values were similar in the first 48 h in both groups (p > 0.05). The individuals allocated to the SA group did not need anesthetic complementation; however, those in the PB group required it considerably (SA group, 0% vs. PB group, 25%; p = 0.005). Hemodynamic changes were more pronounced after PB: during all surgical times, the PB group showed lower MAP values and higher HR values (p < 0.05). Postoperative urinary retention rates were similar between both groups (SA group 0% vs. PB group 3.1%, p = 0.354). Conclusion: SA and PB are similarly effective in pain control during the first 48 h after hemorrhoidec-tomy and anal fistula surgery. Although surgical time was shorter among patients in the PB group, the SA technique may be preferable as it avoids the need for additional anesthesia. Resumo O modelo de Acesso Avançado ou Aberto (Advanced/Open Access) vem sendo estimulado por gestores e valorizado pela medicina de família e comunidade brasileira como modelo de gestão da clínica na Equipe de Saúde da Família. Este artigo de revisão integrativa discute como essa tecnologia pode qualificar ou prejudicar a Atenção Primária à Saúde. Embora ajude a promover mudanças necessárias em agendas tradicionalmente voltadas para ações programáticas, o modelo tem um forte viés gerencialista. Ao desconsiderar premissas básicas, sua implantação pode resultar em sofrimento do profissional e em sua alienação perante o território e o cuidado integral em saúde, além de reforçar o modelo biomédico e a medicalização social. Apontamos caminhos para que ‘avançado’ não signifique ‘precipitado’, destacando que uma implantação com base no diálogo entre trabalhadores, gestores e usuários parece mais coerente com a própria literatura sobre Acesso Avançado e com a produção nacional sobre Acolhimento no Sistema Único de Saúde.

Abstract The Advanced/Open Access model has been encouraged by managers and valued by Brazilian family and community medicine as a model of clinical management in the Family Health Team. This integrative review article discusses how this technology can qualify or hinder Primary Health Care. Although it helps to promote necessary changes in agendas traditionally focused on programmatic actions, the model has a strong managerialist bias. By disregarding basic premises, its implementation may result in professional suffering and alienation from the territory and integral health care, besides reinforcing the biomedical model and social medicalization. We point out ways for ‘advanced’ not to mean ‘precipitate’, highlighting that an implementation based on dialogue among workers, managers, and users seems more coherent with the literature on Advanced Access and with the national production on Welcoming in the Unified Health System.

Resumen El modelo de Acceso Avanzado o Abierto (Advanced/Open Access) ha sido estimulado por los gestores y valorado por la medicina de familia y por la comunidad brasileña como modelo de gestión de la clínica en el Equipo de Salud de la Familia. Este artículo de revisión integradora analiza cómo esta tecnología puede calificar o perjudicar la Atención Primaria de Salud. Si bien ayuda a promover cambios necesarios en agendas tradicionalmente enfocadas en acciones programáticas, el modelo tiene un fuerte sesgo administrativista. Al desconocer premisas básicas, su implementación puede resultar en sufrimiento del profesional y en su alienación ante el territorio y del cuidado integral en salud, además de reforzar el modelo biomédico y la medicalización social. Señalamos caminos para que ‘avanzado’ no signifique ‘apresurado’, destacando que una implementación basada en el diálogo entre trabajadores, gestores y usuarios parece más coherente con la literatura sobre Acceso Avanzado y con la producción nacional sobre Acogida en el Sistema Único de Salud.

Abstract The Euterpe oleracea Mart. has great importance in the neotropical forestry economy. Its berry is a product of great commercial value used extensively for human consumption. Most E. oleracea researches evaluate its food features, however, its potential use for phytoremediation and stipe use remains unknown. This research aimed to assess the seedling’s phytoextraction potential and the structural chemical composition in the seedlings and mature palm trees stipes. We used the Energy-dispersive X-ray fluorescence analysis to determine the concentration of the chemical elements. E. oleracea seedlings showed a great phytoextraction potential for aluminum and iron. The aluminum seedlings concentration was four times higher than preconized as a hyperaccumulator species. Calcium concentration was lower than considered normal, which may represent an antagonism effect caused by the strong presence of aluminum and iron. The fast uptake and accumulation of the seedlings highlight the potential to use this species in phytoremediation programs.

RESUMO Objetivo: Avaliar o uso do ácido tranexâmico (ATX) e aminocapróico (AEAC) na redução da perda sanguínea em cirurgias para trauma do quadril e femur proximal. Métodos: Estudo prospectivo com 49 pacientes operados em hospital de trauma entre nov./15 e fev./17. Pacientes divididos em dois grupos: ATX (n = 24) e AEAC (n = 25). Comparações feitas de acordo com o sexo, idade na cirurgia, ASA, tipo de fratura e cirurgia, perda sanguínea estimada durante a cirurgia, níveis de hemoglobina e hematócrito pré e pós-operatório e o custo das medicações. Dados processados no SPSS 22.0 com nível de significância de p < 0,05. Resultados: Não foram encontradas diferenças significativas entre as seguintes variáveis: idade, sexo, ASA e perda sanguínea estimada durante a cirurgia. Nenhum paciente necessitou de transfusão sanguínea nos dois grupos. Na avaliação pós-operatória, não houve diferença significativa entre os grupos nos valores de queda da hemoglobina e hematócrito (p > 0,05). Analisando os custos de ambos as medicações, observou-se um custo mais elevado do AEAC em relação ao ATX (R$ 90,00 - R$ 15), resultando em R$ 75, 00 a mais por paciente. Conclusão: O uso dos antifibrinolíticos foi eficiente na redução da perda sanguínea, sem a necessidade de hemotransfusões. Nível de evidência II, Estudo Prospectivo Comparativo.

ABSTRACT Objective: To evaluate the use of tranexamic acid (TXA) and ε-aminocaproic acid (EACA) in reducing blood loss in hip and proximal femur trauma surgery. Methods: Prospective study with 49 patients surgically treated in a trauma hospital between Nov/2015 and Feb/2017. The patients were divided in two groups: TXA (n = 24) and EACA (n = 25). The comparison was made according to gender, age at the time of surgery, ASA, fracture and surgery type, estimated blood loss during surgical approach, hemoglobin and hematocrit levels pre and post-operative, and pharmacological cost. The data was processed using SPSS 22.0 with significance level of p < 0,05. Results: No significant difference was found in the variables age, gender, ASA and estimated blood loss during surgical approach. No patient needed blood transfusion. When evaluated post-operatively, the hemoglobin and hematocrit values decrease had no significant difference between the antifibrinolytics (p > 0.05). When analyzing total cost for both pharmacological agents, higher cost was observed in EACA than in TXA (US$ 16.09 - US$ 2.73), resulting in a US$ 13.36 addition per patient. Conclusion: Antifibrinolytic use was efficient on lowering the total blood loss, without the need of blood transfusion. Level of evidence II, Prospective Comparative Study.

RESUMO O DC de Neuroimunologia da ABN e o BCTRIMS trazem, nesse documento, as recomendações sobre vacinação da população com doenças desmielinizantes do sistema nervoso central (SNC) contra infecções em geral e contra o coronavírus da síndrome respiratória aguda grave 2 (SARS-CoV-2), causador da COVID-19. Destaca-se a gravidade do atual momento frente ao avanço da COVID-19 em nosso País, o que torna mais evidente e importante a criação de guia de referência para orientação aos médicos, pacientes e autoridades de saúde pública quanto à vacinação, meio efetivo e seguro no controle de determinadas doenças infecciosa. O DCNI/ABN e o BCTRIMS recomendam que os pacientes com doenças desmielinizantes do SNC (ex., EM e NMOSD) sejam constantemente monitorados, quanto a atualização do seu calendário vacinal, especialmente, no início ou antes da mudança do tratamento com uma droga modificadora de doença (DMD). É importante também salientar que as vacinas são seguras e os médicos devem estimular o seu uso em todos os pacientes. Evidentemente, deve ser dada especial atenção às vacinas com vírus vivos atenuados. Por fim, é importante que os médicos verifiquem qual DMD o paciente está em uso e quando foi feita a sua última dose, pois cada fármaco pode interagir de forma diferente com a indução da resposta imune.

ABSTRACT The Scientific Department of Neuroimmunology of the Brazilian Academy of Neurology (DCNI/ABN) and Brazilian Committee for Treatment and Research in Multiple Sclerosis and Neuroimmunological Diseases (BCTRIMS) provide recommendations in this document for vaccination of the population with demyelinating diseases of the central nervous system (CNS) against infections in general and against the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which causes COVID-19. We emphasize the seriousness of the current situation in view of the spread of COVID-19 in our country. Therefore, reference guides on vaccination for clinicians, patients, and public health authorities are particularly important to prevent some infectious diseases. The DCNI/ABN and BCTRIMS recommend that patients with CNS demyelinating diseases (e.g., MS and NMOSD) be continually monitored for updates to their vaccination schedule, especially at the beginning or before a change in treatment with a disease modifying drug (DMD). It is also important to note that vaccines are safe, and physicians should encourage their use in all patients. Clearly, special care should be taken when live attenuated viruses are involved. Finally, it is important for physicians to verify which DMD the patient is receiving and when the last dose was taken, as each drug may affect the induction of immune response differently.

Abstract Optical thermometry is a growing technological field which exploits the ability of certain materials to change their optical properties with temperature. In this work, poly(glycerol dendrimer) microspheres (PGLyD) encapsulating cobalt-doped hopeite (CoHo) was synthesized for its potential use as optical thermometer. The structure of the CoHo:PGLyD was studied using scanning electron microscopy (SEM), infrared spectroscopy (ATR-FTIR), X-ray diffraction (XRD) and thermogravimetric analysis (TGA). On the basis of SEM images, the CoHo:PGLyD exhibited a spherical shape in which around 80% of the microspheres were within 0.82 µm. A multilayer feed-forward artificial neural network (MLP-ANN) was used to extract the chromaticity profile dependence of the CoHo:PGLyD with temperature. A color change was observed for the CoHo:PGLyD, going from dark blue (490-550 nm) to pale pink (650 nm) when the temperature changed from 20-200 °C, respectively. Abstract A modeling of the resonant behavior of a continuously reconfigurable frequency selective surface, FSS, is described in this paper. The FSS is based on four arms star geometry and its reconfigurability is achieved by the use of varactors. FSS, four arms star geometry and varactor principles are explained. Considering available a set of measured or numerical data, with frequency responses for different varactor capacitance values, the FSS equivalent circuit is stablished. Then, the varactor capacitance effect is included and the resonant behavior can be easily determined. After stablished, the FSS equivalent circuit can be applied even for a different varactor. In order to validate the proposed modeling, the equivalent circuit of a reconfigurable FSS using the varactor SMV1231 (0.466 pF ≤ Cv ≤ 2.35 pF is obtained. The equivalent circuit results are compared to numerical (ANSYS) and measured results, verifying a good agreement. Following, for the same geometry, the equivalent circuit is applied to a reconfigurable FSS using the SMV1234 varactor (1.32 pF ≤ Cv ≤ 9.63 pF) and once more a good agreement between the results is observed, indicating the applicability of the proposed modeling, which is especially attractive for optimization process.

Abstract In this paper, a double layer stopband Frequency Selective Surface (FSS) for Ultra-Wide Band (UWB) applications is presented. The proposed FSS consists of two cascaded conventional FSS, one with double square loops and other with square loops. The − 10dB frequency band from 3.00 GHz to 11.64 GHz is achieved, with a fractional bandwidth of 149 %. Effects of angular stability and polarization independence over the frequency response are investigated and conclusions are presented. In order to show transmission performance, the FSS was simulated by ANSYS HFSS. The FSS shows angular stability and polarization independence in entire UWB band (3.10 – 10.60 GHz). A prototype was built and measurements were performed. A good agreement between simulated and measured results was observed.

O estudo identificou e analisou algumas interações na internet no cotidiano de pessoas vivendo com HIV/AIDS (PVHA) no Brasil. Como método, realizamos entrevistas com PVHA utilizando análise de conteúdo e uma etnografia virtual de um grupo fechado de PVHA no Facebook. Chegamos aos seguintes resultados: a sociabilidade produzida na internet ajuda a reduzir o sofrimento em relação ao preconceito; não há muitos espaços de acolhimento para PVHA na internet; PVHA vinculadas às redes sociais têm mais incentivo para não desistir da medicação; as negociações sobre medicação e sintomas ocorrem nas redes sociais. Conclui-se que há necessidade de ter espaços de acolhimento às PVHA na internet garantidas por políticas públicas; educação médica precisa abordar questões relacionadas à internet e saúde.

This paper identified and analyzed some interactions on the internet in the daily of people living with HIV and AIDS (PLHA) in Brazil. As methods we made interviews with PLHA analyzed by content analysis and a virtual ethnography of a secret group of PLHA on Facebook. These are the following results: sociability produced in the internet helps to reduce suffering in relation to prejudice; there are not many welcoming zones for PLHA on the internet; PLHA linked to social networks have more encouragement to not give up the medication; negotiations about medication and symptoms take place in social networks. We conclude that there is a need to have welcoming zones to PLHA on the internet guaranteed by public policy; medical education needs to cover issues related to the internet and health.

El estudio identificó y analizó algunas interacciones en internet en el cotidiano de personas que viven con VIH/SIDA (PVHA) en Brasil. Como método, realizamos entrevistas con PVHA utilizando análisis de contenido y una etnografía virtual de un grupo cerrado de PVHA en Facebook. Llegamos a los resultados siguientes: la sociabilidad producida en internet ayuda a reducir el sufrimiento con relación al prejuicio; no hay muchos espacios de acogida para PVHA en internet; PVHA vinculadas a las redes sociales tienen mayor incentivo para no desistir de la medicación; las negociaciones sobre medicación y síntomas ocurren en las redes sociales. Se concluye que existe la necesidad de tener espacios de acogida a PVHA en internet aseguradas por políticas públicas; la educación médica precisa abordar cuestiones relacionadas a internet y a la salud.

ABSTRACT Objective: To show a video of a complete supine Percutaneous Nephrolithotomy (csPCNL) performed for the treatment of a staghorn calculus, from the surgeon's point of view. The procedure was recorded with a GoPro® camera, demonstrating the ten essential steps for a successful procedure. Materials and methods: The patient was a 38 years-old woman with 2.4cm of left kidney lower pole stone burden who presented with 3 months of lumbar pain and recurrent urinary tract infections. She had a previous diagnosis of polycystic kidney disease and chronic renal failure stage 2. CT scan showed two 1.2cm stones in the lower pole (Guy's Stone Score 2). She had a previous ipsilateral double J insertion due to an obstructive pyelonephritis. The csPCNL was uneventful with a single access in the lower pole. The surgeon had a Full HD GoPro Hero 4 Session® camera mounted on his head, controlled by the surgical team with a remote control. All of the mains steps were recorded. Informed consent was obtained prior to the procedure. Results: The surgical time was 90 minutes. Hemoglobin drop was 0.5g/dL. A post-operative CT scan was stone-free. The patient was discharged 36 hours after surgery. The camera worked properly and didn't cause pain or muscle discomfort to the surgeon. The quality of the recorded movie was excellent. Conclusion: GoPro® camera proved to be a very interesting tool to document surgeries without interfering with the procedure and with great educational potential. More studies should be conducted to evaluate the role of this equipment.
