Resumo Os fatores de risco para o desenvolvimento da obesidade na adolescência constituem o objeto deste estudo. Por meio de uma revisão integrativa da literatura, buscou-se identificar os fatores de risco da obesidade nos adolescentes. Utilizou-se a estratégia PICO para se formular a seguinte pergunta norteadora: quais são os fatores de risco para o desenvolvimento da obesidade na adolescência? Os portais de pesquisa consultados foram: BVS e EBSCOhost, com o corpus do estudo ficando com 25 artigos. O fenômeno da obesidade foi entendido como resultante de fatores biológicos, sociais, psicológicos e nutricionais. Constatou-se que não existe um consenso sobre riscos e benefícios, o que dificulta recomendações de evidência. Foram identificadas, ainda, propostas passíveis de serem implementadas: a modificação de hábitos alimentares, o controle de peso e a prática de exercícios físicos. Tais modificações comportamentais podem ser recomendadas para os contextos familiares, escolares e dos serviços de saúde. A revisão recomenda estratégias de prevenção e pensar a escola como um espaço rico para promoção da saúde por intermédio da educação. Do ponto de vista político e social, é preciso enfrentar as propagandas da indústria alimentícia, que seduzem os adolescentes a consumirem alimentos processados e ricos em gordura e açúcar.

Abstract The scope of this study was to examine the risk factors for the development of obesity in adolescence. The objective was to identify risk factors for adolescent obesity by means of an Integrative Review of the Literature. The PICO strategy was used to formulate the following central line of inquiry: What are the risk factors for the development of obesity during adolescence? The VHL and EBSCOhost research databases were consulted, resulting in a selection of 25 articles for in-depth study. The phenomenon of obesity was understood as the result of biological, social, psychological, and nutritional factors. The research revealed a lack of consensus on the risks and benefits, which makes evidence-based recommendations difficult. The study identified proposals that can be implemented, such as a change in eating habits, weight control and the practice of physical exercise. Such behavioral changes can be recommended within the context of the family, schools, and health services. The review recommends prevention strategies and the recognition of school as the ideal medium for health promotion through education. From a political and social standpoint, it is necessary to challenge the prevalence of publicity of the food industry that entices adolescents to consume processed foods with high fat and sugar content.

RESUMO Objetivo Identificar a frequência de afasia em usuários internados em um hospital público municipal de referência e caracterizar o perfil sociodemográfico e clínico da amostra estudada. Métodos Foi realizado um estudo transversal, descritivo, baseado em análise de prontuários e avaliação fonoaudiológica breve, à beira do leito. Foram incluídos no estudo adultos ou idosos, de ambos os sexos, falantes nativos do português brasileiro, com AVC, em fase aguda ou subaguda. Resultados Em três meses, 13 usuários se enquadraram nos critérios de inclusão, nove consentiram em participar e sete foram avaliados. Dentre os participantes avaliados, três apresentaram quadro afásico, com proporção de 42,8% de casos. Quanto ao perfil, a maioria dos nove participantes era de mulheres, idosas e com escolaridade média. No subgrupo com diagnóstico inicial de afasia, os três usuários apresentaram quadros afásicos de grau moderado à grave, decorrente de lesões córtico-subcorticais. Conclusão Considerando a proporção de casos de afasia encontrados em usuários internados em fase aguda e subaguda do primeiro AVC e as características desse grupo, é indispensável a atenção à saúde dessa população, bem como o mapeamento de casos em outras regiões do estado e do país.

ABSTRACT Purpose To identify the rate of aphasia in users admitted to a public referral hospital and to characterize the sociodemographic and clinical profile of the sample studied. Methods A descriptive cross-sectional study was carried out based on analysis of medical records and a brief bedside speech-language assessment. The study included younger or older adults, of both sexes, native speakers of Brazilian Portuguese, with stroke in the acute or subacute phase. Results In a 3-month period, 13 users met the inclusion criteria, of whom 9 agreed to participate and 7 were evaluated. Three participants had aphasia, representing 42.8% of cases. Regarding the profile of the 9 participants, the majority were women, elderly and had medium educational level. The three users in the subgroup with initial diagnosis of aphasia had moderate-to-severe aphasia secondary to cortical-subcortical lesions. Conclusion Given the rate of aphasia cases found among users hospitalized for primary acute and subacute strokes and the characteristics of this group, efforts toward providing health care for this population and mapping cases in other regions of the state and country are fundamental.

RESUMO Objetivo: Avaliar os resultados dos exames utilizados para identificar a sensibilização IgE-mediada ao Aspergillus fumigatus em pacientes com fibrose cística. Métodos: Estudo transversal descritivo com amostra de conveniência de 86 pacientes com fibrose cística, acompanhados em Serviço de Referência de Fibrose Cística de hospital universitário terciário. Realizaram-se exames para avaliar sensibilização ao A. fumigatus em pacientes com fibrose cística: IgE sérica total, contagem de eosinófilos sanguíneos, identificação do fungo por swab de orofaringe ou por cultura de escarro, IgE sérica específica e testes cutâneos de hipersensibilidade imediata. Foram comparados os resultados dos diferentes exames realizados. Resultados: Em 33 (38,4%) pacientes com fibrose cística, com faixa etária de 1 a 33 anos (mediana de 8 anos), os resultados dos exames sobre sensibilização IgE mediada ao A. fumigatus foram: em 16 pacientes, aumento de IgE sérica específica (>0,35 kU/L); em 23, positividade aos testes cutâneos; e seis mostraram sensibilização a partir dos dois exames. Foram observados dois pacientes com eosinofilia (>1.000 eosinófilos/mm3) e sete com aumento de IgE sérica total (>1.000 UI/mL), sem que esses apresentassem positividade aos testes cutâneos ou aumento de IgE específica ao A. fumigatus. Em nenhum paciente foi isolado A. fumigatus no swab de orofaringe e/ou na cultura de escarro. Conclusões: Concluímos que, entre os exames para avaliar a sensibilização ao A. fumigatus na fibrose cística, são necessários os teste cutâneos de hipersensibilidade imediata e a dosagem de IgE sérica específica ao A. fumigatus. A eosinofilia sérica e a cultura de secreções respiratórias não foram essenciais neste estudo.

ABSTRACT Objective: To evaluate the results of the tests used to identify the IgE mediated sensitization to Aspergillus fumigatus in patients with cystic fibrosis. Methods: This is a cross-sectional descriptive study with a convenience sample of 86 patients diagnosed with cystic fibrosis in the Reference Service in Cystic Fibrosis at a tertiary teaching hospital. The following tests were performed to assess the sensitization to A. fumigatus in patients with cystic fibrosis: Total serum IgE, eosinophil count, fungus detection through oropharyngeal swab or sputum culture, serum-specific IgE, and immediate-type hypersensitivity (IgE) skin tests. We compared the results of the different tests performed. Results: In 33 (38.4%) patients with cystic fibrosis, with ages ranging from 1 to 33 years (median of 8 years), the IgE-mediated A. fumigatus sensitization test results were: in 16 patients, there was an increase in serum-specific IgE (>0.35 kU/L); in 23, skin tests were positive; and six had sensitization in both tests. We observed two patients with eosinophilia (>1,000 eosinophils/mm3) and seven with increasing total serum IgE (>1,000 IU/mL), all of whom obtained negative results in skin tests and had no IgE increase specific to A. fumigatus. A. fumigatus was not detected in oropharyngeal swabs and/or sputum culture of any patients. Conclusions: We conclude that, among the tests used to assess sensitization to A. fumigatus in cystic fibrosis patients, both serum-specific IgE and immediate-type hypersensitivity (IgE) skin tests are required. Serum eosinophilia and respiratory secretion culture were not essential in this study.

Abstract Chagas disease is a neglected chronic condition with a high burden of morbidity and mortality. It has considerable psychological, social, and economic impacts. The disease represents a significant public health issue in Brazil, with different regional patterns. This document presents the evidence that resulted in the Brazilian Consensus on Chagas Disease. The objective was to review and standardize strategies for diagnosis, treatment, prevention, and control of Chagas disease in the country, based on the available scientific evidence. The consensus is based on the articulation and strategic contribution of renowned Brazilian experts with knowledge and experience on various aspects of the disease. It is the result of a close collaboration between the Brazilian Society of Tropical Medicine and the Ministry of Health. It is hoped that this document will strengthen the development of integrated actions against Chagas disease in the country, focusing on epidemiology, management, comprehensive care (including families and communities), communication, information, education, and research .

RESUMO A doença de Chagas é uma condição crônica negligenciada com elevada carga de morbimortalidade e impacto dos pontos de vista psicológico, social e econômico. Representa um importante problema de saúde pública no Brasil, com diferentes cenários regionais. Este documento traduz a sistematização das evidências que compõe o Consenso Brasileiro de Doença de Chagas. O objetivo foi sistematizar estratégias de diagnóstico, tratamento, prevenção e controle da doença de Chagas no país, de modo a refletir as evidências científicas disponíveis. Sua construção fundamentou-se na articulação e contribuição estratégica de especialistas brasileiros com conhecimento, experiência e atualização sobre diferentes aspectos da doença. Representa o resultado da estreita colaboração entre a Sociedade Brasileira de Medicina Tropical e o Ministério da Saúde. Espera-se com este documento fortalecer o desenvolvimento de ações integradas para enfrentamento da doença no país com foco em epidemiologia, gestão, atenção integral (incluindo famílias e comunidades), comunicação, informação, educação e pesquisas.

Objetivo Analisar a treinabilidade e a reversibilidade de variáveis da aptidão física em idosas praticantes de ginástica funcional. Método Estudo ex postfacto com amostra composta por 115 idosas de seis grupos de ginástica funcional do Programa Viver Ativo de Florianópolis-SC. Para avaliação da aptidão física, utilizou-se a Bateria de Rikli & Jones ("levantar e sentar", "flexão do antebraço", "sentado e alcançar", "alcançar atrás das costas" "levantar e sentar na cadeira" e "andar seis minutos"). A intervenção foi de oito meses, e o destreino, de três. Foram usados resultados de três testagens: pré-teste, pós-teste 1 e pós-teste 2. Utilizou-se a estatística descritiva e inferencial com o teste t Student pareado, com post hoc de Scheffé. Resultados O desempenho diferiu na agilidade e capacidade aeróbia; houve treinabilidade na força e resistência de membros inferiores e superiores e flexibilidade de membros inferiores; verificou-se destreino nas variáveis força e resistência de membros superiores e capacidade aeróbia. Conclusão O programa produz efeitos positivos na força e resistência de membros inferiores e superiores e na flexibilidade de membros inferiores em idosas. Um período de interrupção de três meses resulta em destreino de força e resistência de membros inferiores e da capacidade aeróbia.

Objective To assess the trainability and reversibility of variables of physical fitness in elderly participants in Active Living Functional Gymnastics. Method This ex post facto study was composed of 115 elderly women from six functional fitness groups in the Active Living Program in Florianopolis. The Rikli and Jones battery of tests (Chair Stand Test, Arm Curl Test, Chair Sit and Reach Test, Back Scratch Test, 8-Foot Up and Go Test, 6 Minute Walk Test) was used. The intervention period lasted for eight months, and the detraining period took three months. Descriptive and inferential statistics with paired Student t-test and Scheffé post hoc was used. Results The performance of the age groups differed in agility and aerobic capacity; Trainability was identified in the strength and resistance variables of the lower and upper limbs, and the flexibility of the lower limbs; Detraining was perceived in the strength and resistance of upper limbs, and aerobic capacity. Conclusion A Functional Gymnastics program produces positive effects on the strength and resistance of the lower and upper limbs, and flexibility of the lower limbs in elderly women. An interruption period lasting three months results in detraining in strength and resistance of the lower limbs and aerobic capacity.

The Laboratory of Physiology provides support for the diagnosis of functional disorders associated with cleft lip and palate and also conducts studies to assess, objectively, the institutional outcomes, as recommended by the World Health Organization. The Laboratory is conceptually divided into three units, namely the Unit for Upper Airway Studies, Unit for Stomatognathic System Studies and the Unit for Sleep Studies, which aims at analyzing the impact of different surgical and dental procedures on the upper airways, stomatognathic system and the quality of sleep of individuals with cleft lip and palate. This paper describes the main goals of the Laboratory in the assessment of procedures which constitute the basis of the rehabilitation of cleft lip and palate, i.e., Plastic Surgery, Orthodontics and Maxillofacial Surgery and Speech Pathology.

Treatment of patients with cleft lip and palate is completed with fixed prostheses, removable, total, implants and aims to restore aesthetics, phonetics and function and should be guided by the basic principles of oral rehabilitation, such as physiology, stability, aesthetics, hygiene and the expectations of the patient. In order to obtain longevity of a prosthetic rehabilitation, the periodontal and dental tissue as well as the biomechanics of the prosthesis are to be respected. The purpose of this article is to describe the types of prosthetics treatment, which are performed at HRAC/USP for the rehabilitation of cleft area in adult patients.

This paper presents the treatment protocol of maxillofacial surgery in the rehabilitation process of cleft lip and palate patients adopted at HRAC-USP. Maxillofacial surgeons are responsible for the accomplishment of two main procedures, alveolar bone graft surgery and orthognathic surgery. The primary objective of alveolar bone graft is to provide bone tissue for the cleft site and then allow orthodontic movements for the establishment of an an adequate occlusion. When performed before the eruption of the maxillary permanent canine, it presents high rates of success. Orthognathic surgery aims at correcting maxillomandibular discrepancies, especially anteroposterior maxillary deficiencies, commonly observed in cleft lip and palate patients, for the achievement of a functional occlusion combined with a balanced face.

The aim of this article is to present the pediatric dentistry and orthodontic treatment protocol of rehabilitation of cleft lip and palate patients performed at the Hospital for Rehabilitation of Craniofacial Anomalies - University of São Paulo (HRAC-USP). Pediatric dentistry provides oral health information and should be able to follow the child with cleft lip and palate since the first months of life until establishment of the mixed dentition, craniofacial growth and dentition development. Orthodontic intervention starts in the mixed dentition, at 8-9 years of age, for preparing the maxillary arch for secondary bone graft procedure (SBGP). At this stage, rapid maxillary expansion is performed and a fixed palatal retainer is delivered before SBGP. When the permanent dentition is completed, comprehensive orthodontic treatment is initiated aiming tooth alignment and space closure. Maxillary permanent canines are commonly moved mesially in order to substitute absent maxillary lateral incisors. Patients with complete cleft lip and palate and poor midface growth will require orthognatic surgery for reaching adequate anteroposterior interarch relationship and good facial esthetics.

OBJETIVO: Avaliar a presença de imunocomplexos e a atividade fagocitária de polimorfonucleares neutrofílicos em pacientes com Lúpus Eritematoso Sistêmico (LES) com e sem sinais e sintomas de atividade da doença. MÉTODOS: Analisou-se o sangue periférico de 55 indivíduos, sendo 10 pacientes com forma ativa da doença, 15 fora de atividade e 30 indivíduos sadios. Foi utilizada imunodifusão radial para identificação de imunocomplexos e estudada a etapa de ingestão da fagocitose por neutrófilos com partículas de zimosan. RESULTADOS: Observou-se a presença de crioprecipitado constituído por IgM, IgG, IgA, componentes C3 e C4 do complemento nos pacientes com LES. As médias aritméticas da ingestão por neutrófilos de partículas de zimosan incubado com soro homólogo e zimosan incubado com soro autólogo mostraram uma diminuição significativa (p<0.05) para os pacientes com LES em atividade quando comparadas as de pacientes com LES fora atividade e de indivíduos sadios. CONCLUSÃO: Concluímos haver presença de imunocomplexos nos pacientes com LES em atividade e fora de atividade e uma diminuição da fagocitose por neutrófilos no grupo de pacientes com LES em atividade. As conclusões do presente estudo são coerentes com a patogênese da doença e com a alta mortalidade por infecções nesses pacientes.

OBJECTIVE: To evaluate the presence of immune complexes and the phagocytes by polymorphonuclear neutrophils in patients with systemic lupus erythematosus, with and without disease activity. METHODS: The peripheral blood of 55 subjects was analyzed. Ten of those subjects had disease activity, 15 had not disease activity, and 30 were healthy. We used radial immune diffusion to detect immune complexes. The phagocytic function was estimated by the ingestion of zymosan by polymorphonuclear neutrophils. RESULTS: In this study we found the presence of immune complexes formatted of IgM, IgG, IgA, and complement component C3 and C4 in LES patients. The arithmetic average of zymosan particles ingested by the neutrophils incubated with homologous human serum and autologous human serum was significantly decreased (p<0.05) in the LES activity patients when we compare with the group without activity, and the control group. CONCLUSION: We conclude that there are immune complexes in the LES patients with and without disease activity, and there is a reduction in the digestive step of the phagocytes by polymorphonuclear neutrophils in patients with disease activity. The conclusions of the present study are according with the pathogenesis of the disease and with the high mortality in these patients.

OBJECTIVE: To study the prevalence of systemic hypertension and its control in the population of Catanduva, in the state of São Paulo, Brazil. METHODS: We carried out a randomized cross-sectional population-based study of the urban population of Catanduva with individuals above 18 years of age (688 individuals accounting for 0.9% of the referred population). We interviewed study participants to analyze the major qualitative and quantitative variables that could influence the hypertensive scenario and the risk for systemic hypertension. Blood pressure was measured through the indirect method according to the III Consenso Brasileiro de Hipertensão (III Brazilian Consensus on Hypertension), which established blood pressure levels > or = 140/90 mm Hg as hypertensive. RESULTS: The prevalence of systemic hypertension was higher in individuals with: (1) history of hypertension (p<0.0001); (2) diabetes mellitus (p=0.05); (3) body mass index (B. M. I) > or = 25 kg/m² (p<0.001); (4) low educational level (p<0.0001); (5) familial income ranging from 1 to 5 minimum wages (p<0.05); (6) unmarried status (divorced/separated and widow(er)s) (p<0.0001). Of the interviewed individuals, 27.6% (p=0.05) had blood pressure levels under control. CONCLUSION: Our study showed that the prevalence of systemic hypertension was 31.5%, and that 27.6% of the individuals interviewed had blood pressure levels under control at the time of the interview.
