■ ABSTRACT Introduction: The aesthetic procedure market is growing exponentially in Brazil. This growth has aroused the interest of several professional categories. The decision to practice in the sector must consider the market opportunities in the location in which you intend to operate. However, the area lacks comparative analyses documenting probable regional differences in the country. The objective of the study is to describe market differences in aesthetic procedures between Brazilian states and regions. An index of potential cosmetic consumption (IPCC) is calculated for such a comparative analysis. Method: Cross-sectional study involving providers of nonsurgical aesthetic procedures in Brazil. Searches on Google Maps® were conducted using key terms, and telephone interviews were conducted to obtain information on professional categories, types of providers, and services offered. Positive predictive values were obtained for all search strategies and used to estimate the total number of providers. Population size and per capita income were considered to calculate the IPCCs for Brazilian states. Results: São Paulo, Minas Gerais, and Rio de Janeiro presented the highest IPCCs, being 524, 210, and 180, respectively. Roraima had an IPCC of 14, the lowest in the country. The Southeast Region presented, on average, the highest IPCC (242) among all Brazilian regions. Conclusion: Considering population size and income, the Southeast Region presents the greatest market opportunities for nonsurgical aesthetic procedures in Brazil. Our findings may be of interest to healthcare professionals and investors who work or intend to work in the sector.

■ RESUMO Introdução: O mercado de procedimento estéticos cresce exponencialmente no Brasil. Tal crescimento tem despertado o interesse de várias categorias profissionais. A decisão de praticar no setor deve considerar as oportunidades de mercado da localidade na qual se pretende atuar. Entretanto, a área carece de análises comparativas documentando prováveis diferenças regionais no país. O objetivo do estudo é descrever as diferenças de mercado em procedimentos estéticos entre os estados e regiões brasileiras. Um índice de potencial consumo de cosmiatria (IPCC) é calculado para tal análise comparativa. Método: Estudo transversal envolvendo prestadores de procedimentos estéticos não cirúrgicos no Brasil. Buscas no Google®-Google Maps® foram conduzidas usando termos-chave e entrevistas telefônicas realizadas para obter informações sobre categorias profissionais, tipo de provedores e serviços oferecidos. Valores preditivos positivos foram obtidos para todas as estratégias de busca e usados para estimar o número total de provedores. O tamanho da população e a renda per capita foram considerados para o cálculo dos IPCCs para os estados brasileiros. Resultados: São Paulo, Minas Gerais e Rio de Janeiro apresentaram os maiores IPCCs, sendo 524, 210 e 180, respectivamente. Roraima teve um IPCC de 14, o mais baixo do país. A Região Sudeste apresentou, em média, o maior IPCC (242) entre todas as regiões brasileiras. Conclusão: Considerando o tamanho da população e a renda, a Região Sudeste apresenta as maiores oportunidades de mercado em procedimentos estéticos não cirúrgicos no Brasil. Nossos achados podem ser de interesse para profissionais de saúde e investidores que atuam ou pretendem atuar no setor.

RESUMO Introdução: A redução da carga pelo método das repetições máximas (RM) pode ser necessária para promover maior número de repetições e, consequentemente, maior volume total, tempo sob tensão e classificações do esforço percebido. Objetivo: Comparar os efeitos de diferentes cargas do exercício leg press no número de repetições, volume total, tempo sob tensão e percepção de esforço. Métodos: Dezoito universitárias (23,9 ± 3,8 anos) realizaram duas sessões experimentais com 90% e 100% de 10-12 RM em desenho transversal balanceado. Resultados: O número de repetições da segunda e terceira séries, volume total e tempo sob tensão a 90% de 10-12 RM foi estatisticamente maior do que a 100% de 10-12 RM (p < 0,05). A percepção do esforço da primeira e segunda séries e a carga de treinamento (percepção do esforço x duração das sessões) foram maiores na sessão realizada com 100% de 10-12 RM (p < 0,05). Conclusão: Uma pequena redução da carga resulta em maior número de repetições, volume total e tempo sob tensão. A sessão com maior carga induziu maior percepção do esforço e carga de treinamento. Assim, cientistas e treinadores podem considerar cargas menores para maximizar o número de repetições, o volume total e o tempo sob tensão, o que pode causar maiores adaptações musculares a longo prazo. Nível de evidência II; Estudo prospectivo comparativo.

ABSTRACT Introduction: Load reduction using the repetition maximum (RM) method may be necessary to promote higher numbers of repetitions, and consequently, higher total volume, time under tension, and perceived exertion ratings. Objective: To compare the effects of different leg press exercise loads on number of repetitions, total volume, time under tension, and perceived exertion. Methods: Eighteen women university students (23.9 ± 3.8 years) performed two experimental sessions with 90% and 100% of 10-12 RM in a balanced crossover design. Results: The number of repetitions of the second and third sets, the total volume, and time under tension at 90% of 10-12 RM was statistically higher than at 100% of 10-12 RM ( p < 0.05). The perceived exertion of the first and second sets and the training load (perceived exertion x duration of sessions) were higher at 100% of the 10-12 RM session ( p < 0.05). Conclusion: A small reduction in load results in a greater number of repetitions, total volume, and time under tension. The session with the higher load appeared to induce higher perceived exertion and training load. Thus, scientists and coaches might consider lower loads to maximize the number of repetitions, total volume, and time under tension, which may cause greater long-term muscular adaptations. Level of evidence II; Comparative prospective study.

RESUMEN Introducción: La reducción de la carga por el método de las repeticiones máximas (RM) puede ser necesaria para promover un mayor número de repeticiones y, consecuentemente, un mayor volumen total, tiempo bajo tensión y calificaciones del esfuerzo percibido. Objetivo: Comparar los efectos de diferentes cargas del ejercicio de prensa de piernas sobre el número de repeticiones, volumen, tiempo bajo tensión y esfuerzo percibido. Métodos: Dieciocho estudiantes universitarios (23,9 ± 3,8 años) realizaron dos sesiones experimentales con el 90% y el 100% de 10-12 RM en un diseño trasversal equilibrado. Resultados: El número de repeticiones de la segunda y tercera serie, el volumen total y el tiempo bajo tensión al 90% de 10-12 RM fue estadísticamente mayor que al 100% de 10-12 RM (p < 0,05). El esfuerzo percibido de la primera y segunda serie y la carga de entrenamiento (esfuerzo percibido x duración de las sesiones) fueron superiores en la sesión realizada al 100% de 10-12 RM (p < 0,05). Conclusión: Una pequeña reducción de la carga da lugar a un mayor número de repeticiones, volumen total y tiempo bajo tensión. La sesión con mayor carga indujo un mayor esfuerzo percibido y carga de entrenamiento. Por lo tanto, los científicos y entrenadores pueden considerar cargas más bajas para maximizar el número de repeticiones, el volumen total y el tiempo bajo tensión, lo que puede causar mayores adaptaciones musculares a largo plazo. Nivel de evidencia II; Estudio prospectivo comparativo.

Abstract Objective To evaluate clinical characteristics and outcomes of COVID-19 patients infected with HIV, and to compare with a paired sample without HIV infection. Methods This is a substudy of a Brazilian multicentric cohort that comprised two periods (2020 and 2021). Data was obtained through the retrospective review of medical records. Primary outcomes were admission to the intensive care unit, invasive mechanical ventilation, and death. Patients with HIV and controls were matched for age, sex, number of comorbidities, and hospital of origin using the technique of propensity score matching (up to 4:1). They were compared using the Chi-Square or Fisher's Exact tests for categorical variables and the Wilcoxon for numerical variables. Results Throughout the study, 17,101 COVID-19 patients were hospitalized, and 130 (0.76%) of those were infected with HIV. The median age was 54 (IQR: 43.0;64.0) years in 2020 and 53 (IQR: 46.0;63.5) years in 2021, with a predominance of females in both periods. People Living with HIV (PLHIV) and their controls showed similar prevalence for admission to the ICU and invasive mechanical ventilation requirement in the two periods, with no significant differences. In 2020, in-hospital mortality was higher in the PLHIV compared to the controls (27.9% vs. 17.7%; p = 0.049), but there was no difference in mortality between groups in 2021 (25.0% vs. 25.1%; p > 0.999). Conclusions Our results reiterate that PLHIV were at higher risk of COVID-19 mortality in the early stages of the pandemic, however, this finding did not sustain in 2021, when the mortality rate is similar to the control group.

ABSTRACT Objective To stratify ultrasound samples in a pediatric population undergoing evaluation for acute appendicitis to examine the variability in cecal appendix diameter, in different age groups, and to determine whether there is a prevalent value for each age group. Methods A retrospective cross-sectional study with 196 children aged 0 to 15 years. Data were extracted from reports of ultrasound examinations carried out between 2008 and 2015. Children with sonographic diagnosis of appendicitis or other signs of periappendiceal inflammation were excluded. Results The evaluation of the anteroposterior measurement of the cecal appendix revealed a mean diameter of 4.14mm (standard deviation: 0.93mm; 95%CI: 3.86-4.14). Cecal appendix diameter did not differ significant between age groups. Conclusion Evaluation of the anteroposterior diameter of the cecal appendix in centimeters in a sample of 196 children aged 0 to15 years revealed a mean diameter of 4.14mm (standard deviation, 0.93mm. There were no significant differences in cecal appendix diameter following stratification by age. Results indicate a single value can be adopted for mean cecal appendix diameter in pediatric populations.

Resumo Na erva-mate (Ilex paraguariensis) o principal produto colhido é composto por folhas (FO) e galhos finos (GF), resultando em elevada exportação de nutrientes, dentre eles o N. As informações técnico-científicas disponíveis para subsidiar uma recomendação da adubação nitrogenada racional são insuficientes, o que põe em risco a sustentabilidade da produção, especialmente se as colheitas são mais intensivas. Por isso, objetivou-se avaliar a influência da adubação nitrogenada e do intervalo de colheita no teor de N e carbono orgânico (CO) do solo, na produtividade e no estado nutricional da erva-mate. O experimento foi instalado em São Mateus do Sul - PR, em erval com sete anos e meio de idade, estabelecido em Latossolo Vermelho Distrófico, no qual se avaliaram doses de 0, 20, 40, 80, 160 e 320 kg ha-1 de N, para intervalos de colheita de 12, 18 e 24 meses. Os teores de N e CO do solo, nas formas lábil (NL e COL) e total (NT e COT), foram determinados nas profundidades de 0-10, 10-20 e 20-40 cm. A colheita consistiu na remoção de aproximadamente 95 % da copa que brotou da colheita anterior. A massa verde colhida foi determinada individualmente para os componentes FO, GF e galho grosso (GG), sendo FO+GF correspondente à erva-mate comercial (ECOM). Avaliou-se, também, a relação entre massa verde/seca (MV/MS) e o teor de N na FO, GF e GG. O efeito positivo da adubação nitrogenada no teor de NL e COL do solo foi mais expressivo no menor intervalo de colheita e negativo ou nulo para o intervalo de 24 meses. As doses de N aumentaram o teor de NT nos maiores intervalos de colheita e reduziram para colheita de 12 meses e o COT não foi influenciado pela adubação. Houve aumento da produtividade com as doses de N, e as respostas mais expressivas ocorreram nos maiores intervalos entre colheitas. Intervalo de 12 meses entre colheitas é insuficiente para a planta recuperar-se do impacto causado pela colheita. O intervalo de 18 meses é o mais recomendável, desde que a planta esteja bem nutrida em N. A adubação nitrogenada não influencia o teor de água na erva-mate comercial verde, sendo esta dependente do intervalo de tempo entre colheitas. Teor foliar de N entre 33,0 e 37,0 g kg-1 pode indicar plantas bem nutridas, independentemente do intervalo de colheita.

Abstact Leaves (FO) and thin branches (GF) are the main plant components harvested from Ilex paraguariensis (mate) plantations, leading to high exportation of nutrients, among them, the nitrogen (N). Scientific and technical information are scarce to support fertilizer recommendation aiming to replenish N exportation and to sustain mate productivity. Thus, this study aimed to evaluate the influence of N fertilization and harvest intervals on N and organic carbon (CO) content in the soil and on mate productivity and N content. The experiment was conducted in São Mateus do Sul, Paraná state, Brazil, in a seven-year old mate plantation established on an Oxisol, where we evaluated the effect of 0, 20, 40, 80, 160, and 320 kg ha-1 of N, for harvest intervals of 12, 18, and 24 months. Nitrogen (NL) and CO (COL) in the labile and total (NT and COT) forms were determined in the soil at depths of 0-10, 10-20 and 20-40 cm. Harvest consisted in the removal of 95 % of the tree crown sprouted from the previous harvesting. The green material collected was evaluated individually for the components FO, GF and thick branches (GG), being FO+GF the commercial mate product (ECOM). We also evaluated the plant component relationships between green/dry weights (MV/MS), as well as the contents of N in FO, GF and GG. The positive effect of N fertilization on NL and COL contents in the soil was more expressive for the shortest harvesting interval, and was negative or null for the 24-month interval. Nitrogen doses increased the content of NT in the largest harvest interval and reduced the yield for 12 months; COT was not influenced by fertilization. We observed increased productivity with N doses, and the most significant responses occurred in longer harvest intervals. The interval of 12 months between harvests is insufficient for the plant recovery from the previous harvesting impacts. The 18-month interval is more recommendable because the plant is well nourished in N. Nitrogen does not affect the water content of the commercial green yerba mate, which is more affected by the time between harvests. The leaf N content between 33.0 and 37.0 g kg-1 may indicate well-nourished plants, regardless the harvest interval.

ABSTRACT: Spatial and temporal patterns of soil water content (SWC) can not only improve the understanding of soil water processes but also the water management in the field. The spatial distribution of SWC depends on the spatial variability of soil attributes, vegetation and landscape features. The aim of this study was to evaluate: i) the spatial and temporal variability pattern in an agroecological system; ii) understand the factors affecting the spatial variations of SWC; iii) determine if wet and dry zones conserve their spatial position; iv) evaluate the possibility of using this information to reduce the number of SWC measurements. The experiment was carried out in an area of 2,502 m2, where a regular grid with spacing of 10 m was laid out. At each point, time domain reflectometer sensors were installed at depths of 0.05, 0.15, 0.30 m to monitor the SWC for 18 days in 2014 (Jan, Feb and Mar) and 9 days in 2014/2015 (Dec and Jan). The SWC, at the three soil depths, followed a similar and systematic pattern, being highest in the deepest layers, and exhibited temporal stability. The correlation between SWC and clay content varied both with the depth and the magnitude of SWC. During the wet season it is necessary to intensify the sampling density to estimate the SWC, while during the dry season the Spearman rank correlation remained high indicating the need for a small sampling effort only. The driest zones tend to conserve their spatial position more for a longer period than compared to wettest zones.

Resumo Fundamento: A cirurgia de revascularização miocárdica (CRM) e a intervenção coronária percutânea (ICP) são estratégias amplamente utilizadas no manejo da doença arterial coronariana (DAC) estável. Objetivo: Avaliar o prognóstico de pacientes com DAC estável inicialmente tratada com terapia médica (TM), em comparação com os pacientes submetidos a procedimentos de revascularização. Métodos: Estudo prospectivo de coorte com 560 pacientes ambulatoriais de um hospital terciário com seguimento médio de 5 anos. Os pacientes foram classificados nos grupos TM (n = 288), ICP (n = 159) e CRM (n = 113) de acordo com sua estratégia inicial de tratamento. Os desfechos primários foram mortalidade global e eventos combinados de morte, síndrome coronária aguda e AVC. Resultados: Durante o seguimento, as taxas de mortalidade foram de 11,1% em TM, 11,9% em ICP e 15,9% em pacientes submetidos à CRM, sem diferença estatística (Hazard Ratio [HR] para ICP, 1,05; Intervalo de Confiança de 95% [IC95%], 0,59 a 1,84; e HR para CRM, 1,20; IC95%, 0,68 a 2,15). Os desfechos combinados ocorreram com maior frequência entre os pacientes inicialmente submetidos à ICP em relação à TM (HR 1,50, IC 95% 1,05 a 2,14) e não diferiram entre TM e CRM (HR 1,24, IC95% 0,84 a 1,83). Entre os pacientes com diabetes (n = 198), a ICP foi a única estratégia terapêutica preditiva de desfechos combinados (HR 2,14; IC 95%: 1,25 a 3,63). Conclusão: Neste estudo observacional de doença arterial coronariana estável, não houve diferença na mortalidade global entre as estratégias iniciais de terapia médico ou de cirurgia de revascularização. Os pacientes inicialmente tratados com ICP tiveram maior chance de desenvolver eventos cardiovasculares maiores combinados.

Abstract Background: Coronary artery bypass grafting surgery (CABG) and percutaneous coronary intervention (PCI) are widely-used strategies in the management of stable coronary artery disease (CAD). Objective: To evaluate the prognosis of patients with stable CAD initially treated by medical therapy (MT), compared to the patients who were submitted to revascularization procedures. Methods: We conducted a prospective cohort study of 560 patients from an outpatient clinic in a tertiary hospital, with a mean follow-up of 5 years. Patients were classified into MT (n = 288), PCI (n = 159) and CABG (n=113) groups according to their initial treatment strategy. Primary endpoints were overall mortality and combined events of death, acute coronary syndrome, and stroke. Results: During follow-up, death rates were 11.1% in MT, 11.9% in PCI and 15.9% in CABG patients, with no statistical difference (hazard ratio [HR] for PCI, 1.05; 95% confidence interval [95%CI], 0.59 to 1.84; and HR for CABG, 1.20; 95% CI: 0.68 to 2.15). Combined outcomes occurred more often among patients initially submitted to PCI compared to MT (HR 1.50, 95% CI 1.05 to 2.14), and did not differ between MT and CABG patients (HR 1.24, 95% CI 0.84 to 1.83). Among patients with diabetes (n=198), PCI was the only therapeutic strategy predictive of combined outcomes (HR 2.14; 95% CI 1.25 to 3.63). Conclusion: In this observational study of stable coronary artery disease, there was no difference in overall mortality between initial medical therapy or revascularization surgery strategies. Patients initially treated with PCI had greater chance to develop combined major cardiovascular events.

Resumo Atualizamos a lista de mamíferos do estado de Mato Grosso do Sul, Brasil com base em registros primários. Cento e sessenta e seis espécies são listadas como ocorrentes no estado, sendo 47 de mamíferos de médio e grande porte, 46 de pequenos mamíferos e 73 de morcegos. As espécies listadas estão distribuídas em 31 famílias: Didelphidae (17 spp.), Dasypodidae (7 spp.), Myrmecophagidae (2 spp.), Cebidae (1 sp.), Callithrichidae (2 spp.), Aotidae (1 sp.), Pitheciidae (1 sp.), Atelidae (1 sp.), Leporidae (1 sp.), Felidae (7 spp.), Canidae (4 spp.), Mustelidae (5 spp.), Mephitidae (2 sp.), Procyonidae (2 spp.), Tapiridae (1 sp.), Tayassuidae (2 spp.), Cervidae (4 spp.), Sciuridae (1 sp.), Cricetidae (22 spp.), Erethizontidae (1 sp.), Caviidae (3 spp.), Dasyproctidae (1 sp.), Cuniculidae (1 sp.), Echimyidae (4 spp.), Phyllostomidae (41 spp.), Emballonuridae (2 spp.), Molossidae (16 spp.), Vespertilionidae (9 spp.), Mormoopidae (1 sp.), Noctilionidae (2 spp) e Natalidade (1 sp.). Estes números representam um aumento de quatorze espécies com registro primário para o estado em comparação com a listagem publicada anteriormente. Ainda assim, é evidente a escassez de informações em diversas regiões do estado, e a necessidade de implementação de coleções zoológicas regionais. O estado de Mato Grosso do Sul representa apenas 4,19% do território brasileiro, mas o número de mamíferos atinge 24,13% do total de espécies que ocorrem no país.

Abstract We updated the checklist of mammals from Mato Grosso do Sul, Brazil based on primary records only. One hundred and sixty-six mammal species were listed as occurring in the state, 47 of them being medium to large, 47 small mammal and 73 bat species. The listed species are distributed in 31 families: Didelphidae (17 spp.), Dasypodidae (7 spp.), Myrmecophagidae (2 spp.), Cebidae (1 sp.), Callithrichidae (2 spp.), Aotidae (1 sp.), Pitheciidae (1 sp.), Atelidae (1 sp.), Leporidae (1 sp.), Felidae (7 spp.), Canidae (4 spp.), Mustelidae (5 spp.), Mephitidae (2 spp.), Procyonidae (2 spp.), Tapiridae (1 sp.), Tayassuidae (2 spp.), Cervidae (4 spp.), Sciuridae (1 sp.), Cricetidae (22 spp.), Erethizontidae (1 sp.), Caviidae (3 spp.), Dasyproctidae (1 sp.), Cuniculidae (1 sp.), Echimyidae (4 spp.), Phyllostomidae (41 spp.), Emballonuridae (2 spp.), Molossidae (16 spp.), Vespertilionidae (9 spp.), Mormoopidae (1 sp.), Noctilionidae (2 spp.), and Natalidade (1 sp.). These numbers represent an increase of fourteen species with primary records for the state in comparison with the previously published checklist. However, it is evident the scarcity of information at several regions of the state, and the need of implementation of regional zoological collections. The state of Mato Grosso do Sul represent only 4.19% of the Brazilian territory, but the number of mammal species reach 24.13% of the known species occurring in the country.

ABSTRACT Bacillary angiomatosis (BA) is an angioproliferative disease of immunocompromised patients that usually presents as vascular tumors in the skin and subcutaneous tissues. It is caused by chronic infections with either Bartonella henselae or B. quintana. Oral cavity BA is exceedingly rare and even rarer without simultaneous cutaneous disease. We report herein the case of a 51-year-old HIV-infected man who presented severe odynophagia and an eroded lesion on the hard palate that progressed to an oronasal fistula. No cutaneous lesions were recorded. Doxycycline led to complete resolution. To the best of our knowledge, only six previous cases of oral BA without tegumentary disease have been previously reported and none of them progressed to fistula.

O transplante de intestino, ao redor do mundo, tem crescido de maneira sólida e consistente nos últimos 10 anos. No final da década de 1990, passou de um modelo experimental para uma prática clínica rotineira no tratamento dos pacientes com complicação severa da nutrição parenteral total com falência intestinal. Nos últimos anos, vários centros têm relatado uma crescente melhora nos resultados de sobrevida do transplante no primeiro ano (ao redor de 80%), porém, a longo prazo, ainda é desafiador. Diversos avanços permitiram sua aplicação clínica. O surgimento de novas drogas imunossupressoras, como o tacrolimus, além das drogas indutoras, os anticorpos antilinfocíticos mono e policlonal, nos últimos 10 anos, foi de suma importância para a melhora da sobrevida do transplante de intestino/multivisceral, mas, apesar dos protocolos bastante rígidos de imunossupressão, a rejeição é bastante frequente, podendo levar a altas taxas de perdas de enxerto a longo prazo. O futuro do transplante de intestino e multivisceral parece promissor. O grande desafio é reconhecer precocemente os casos de rejeição, prevenindo a perda do enxerto e melhorando os resultados a longo prazo, além das complicações causadas por infecções oportunistas, doenças linfoproliferativas pós-transplante e a doença do enxerto contra hospedeiro.

Intestinal transplantation has shown exceptional growth over the past 10 years. At the end of the 1990’s, intestinal transplantation moved out of the experimental realm to become a routine practice in treating patients with severe complications related to total parenteral nutrition and intestinal failure. In the last years, several centers reported an increasing improvement in survival outcomes (about 80%), during the first 12 months after surgery, but long-term survival is still a challenge. Several advances led to clinical application of transplants. Immunosuppression involved in intestinal and multivisceral transplantation was the biggest gain for this procedure in the past decade due to tacrolimus, and new inducing drugs, mono- and polyclonal anti-lymphocyte antibodies. Despite the advancement of rigid immunosuppression protocols, rejection is still very frequent in the first 12 months, and can result in long-term graft loss. The future of intestinal transplantation and multivisceral transplantation appears promising. The major challenge is early recognition of acute rejection in order to prevent graft loss, opportunistic infections associated to complications, post-transplant lymphoproliferative disease and graft versus host disease; and consequently, improve results in the long run.

Em 1958, Francis Moore descreveu a técnica do transplante de fígado em cães. Em 1963, Starzl e sua equipe realizaram o primeiro transplante de fígado. Nos primeiros cinco transplante de fígado, nenhum paciente sobreviveu mais que 23 dias. Até 1977, aproximadamente 200 transplante de fígado tinham sido realizados no mundo. Neste período, foi estabelecida a solução de problemas técnicos do transplante de fígado. Calne, em 1979, utilizou, pela primeira vez, a ciclosporina em dois pacientes submetidos ao transplante de fígado. Starzl e seus colaboradores relataram, já em 1989, que a sobrevida de 1.179 pacientes submetidos ao transplante de fígado em 1 e 5 anos foi, respectivamente, de 73 e 64%. Finalmente, em 1990, Starzl relatou o primeiro uso do novo imunossupressor tacrolimo em pacientes de transplante de fígado que apresentavam rejeição mesmo com o tratamento imunossupressor convencional. O transplante de fígado iniciou-se no Hospital Israelita Albert Einstein em 1990 e já foram realizados mais de 1.400 transplantes. Em 2013, foram realizados 102 transplantes de fígado de doadores falecidos. As principais indicações para o transplante foram carcinoma hepatocelular (38%), cirrose hepática secundária ao vírus C (33,3%) e cirrose alcoólica (19,6%). Destes, 36% dos transplantes apresentavam MELD biológico superior a 30. As sobrevidas do paciente e do enxerto no primeiro ano foram, respectivamente, 82,4 e 74,8%. Um dos maiores desafios da área do transplante de fígado é o número insuficiente de doadores para uma demanda crescente de candidatos ao procedimento. Dessa forma, destacamos que tópicos relacionados à seleção de doadores/receptores, alocação e preservação de órgãos devem contribuir para o aumento e a melhora dos resultados do transplante de fígado.

In 1958 Francis Moore described the orthotopic liver transplantation technique in dogs. In 1963, Starzl et al. performed the first liver transplantation. In the first five liver transplantations no patient survived more than 23 days. In 1967, stimulated by Calne who used antilymphocytic serum, Starzl began a successful series of liver transplantation. Until 1977, 200 liver transplantations were performed in the world. In that period, technical problems were overcome. Roy Calne, in 1979, used the first time cyclosporine in two patients who had undergone liver transplantation. In 1989, Starzl et al. reported a series of 1,179 consecutives patients who underwent liver transplantation and reported a survival rate between one and five years of 73% and 64%, respectively. Finally, in 1990, Starzl et al. reported successful use of tacrolimus in patents undergoing liver transplantation and who had rejection despite receiving conventional immunosuppressive treatment. Liver Transplantation Program was initiated at Hospital Israelita Albert Einstein in 1990 and so far over 1,400 transplants have been done. In 2013, 102 deceased donors liver transplantations were performed. The main indications for transplantation were hepatocellular carcinoma (38%), hepatitis C virus (33.3%) and alcohol liver cirrhosis (19.6%). Of these, 36% of patients who underwent transplantation showed biological MELD score > 30. Patient and graft survival in the first year was, 82.4% and 74.8%, respectively. A major challenge in liver transplantation field is the insufficient number of donors compared with the growing demand of transplant candidates. Thus, we emphasize that appropriated donor/receptor selection, allocation and organ preservation topics should contribute to improve the number and outcomes in liver transplantation.

Este trabalho tem como objetivo avaliar dois aspectos importantes em um pedido de patente de formas cristalinas de fármacos: (i) caracterização físico-química das formas cristalinas e (ii)o procedimento de preparo da forma II do fármaco clopidogrel, um blockbuster de vendas. Realizaram-se buscas em bancos de dados patentários on line. Os resultados mostraram que (i) a maioria dos pedidos de patente de formas cristalinas do clopidogrel não se adequam com proposta do INPI devido ao número insuficiente de técnicas analíticas utilizadas na caracterização da fase cristalina. Ainda, em alguns pedidos de patente não há a presença da avaliação da pureza química/cristalográfica; (ii) a importância de se utilizar mais de duas técnicas de avaliação e (iii) que não foi possível a reprodução da cristalização com o procedimento apresentado no pedido de patente.

The aim of this study was to evaluate two important aspects of patent applications of crystalline forms of drugs: (i) the physicochemical characterization of the crystalline forms; and (ii) the procedure for preparing crystals of the blockbuster drug clopidogrel. To this end, searches were conducted using online patent databases. The results showed that: (i) the majority of patent applications for clopidogrel crystalline forms failed to comply with proposed Brazilian Patent Office guidelines. This was primarily due to insufficient number of analytical techniques evaluating the crystalline phase. In addition, some patent applications lacked assessment of chemical/crystallography purity; (ii) use of more than two analytical techniques is important; and (iii) the crystallization procedure for clopidogrel bisulfate form II were irreproducible based on the procedure given in the patent application.
