Abstract Background The relationship between Multidrug Resistant-Gram Negative Bacteria (MDR-GNB) infection and colonization in critically ill COVID-19 patients has been observed, however, it is still poorly understood. This study evaluated the risk factors for acquiring MDR-GNB in patients with severe COVID-19 in Intensive Care Units (ICU). Methods This is a nested case-control study in a cohort of 400 adult patients (≥ 18 years old) with COVID-19, hospitalized in the ICU of 4 hospitals in the city of Curitiba, Brazil. Cases were critical COVID-19 patients with one or more MDR GNB from any surveillance and/or clinical cultures were taken during their ICU stay. Controls were patients from the same units with negative cultures for MDR-GNB. Bivariate and multivariate analyses were done. Results Sixty-seven cases and 143 controls were included. Independent risk factors for MDR bacteria were: male gender (OR = 2.6; 95% CI 1.28‒5.33; p = 0.008); the hospital of admission (OR = 3.24; 95% CI 1.39‒7.57; p = 0.006); mechanical ventilation (OR = 25.7; 95% CI 7.26‒91; p < 0.0001); and desaturation on admission (OR = 2.6; 95% CI 1.27‒5.74; p = 0.009). Conclusions Male gender, desaturation, mechanical ventilation, and the hospital of admission were the independent factors associated with MDR-GNB in patients in the ICU with COVID-19. The only modifiable factor was the hospital of admission, where a newly opened hospital posed a higher risk. Therefore, coordinated actions toward a better quality of care for critically ill COVID-19 patients are essential.

Resumo: Objetivo As amebas testáceas são protistas de vida livre reconhecidas como bioindicadores, capazes de revelar alterações ambientais apresentando variação de riqueza, diversidade e abundância de espécies. Para avaliar a produção científica relativa ao uso de amebas testáceas como bioindicadores, foi realizada uma análise cienciométrica no período de 1960 a 2020. Métodos A seleção de artigos científicos foi feita nas plataformas Google Acadêmico, Scielo, Science Direct e Online Library. Todos os artigos selecionados foram categorizados por cronologia, periódico de publicação, país, afiliação dos autores, se o estudo era empírico ou experimental, se incluía apenas amebas testáceas e aspectos do meio ambiente. Resultados Um total de 215 artigos de noventa e dois periódicos revelou um aumento notável no número de publicações nas últimas décadas. Os dois periódicos que mais publicaram dados sobre o tema foram Microbial Ecology e Ecological Indicators. Embora o maior número de trabalhos tenha sido publicado no continente europeu, os países que mais contribuíram para o tema foram o Canadá na região temperada e o Brasil nos trópicos. Edward A.D. Mitchell publicou o maior número de estudos e em termos de instituição, a Universidade Franche-Comté. A maioria dos trabalhos que associaram amebas testáceas como bioindicadores foram empíricos, realizados tecamebas habitantes de musgos e, como tal, dissociadas de outros grupos. Conclusões Apesar do aumento verificado nas publicações com testáceas como bioindicadores, nossos resultados indicam uma concentração de estudos em regiões limitadas do globo. Sob outro aspecto, um grande número de estudos avaliou a comunidade de testáceas como um todo, mostrando uma escassez de conhecimento aprofundado sobre espécies e grupos funcionais. Com isso, nosso levantamento aponta para uma grande variedade de ecossistemas aquáticos desprovidos de estudos sobre o assunto, indicando a necessidade de fomentar a pesquisa sobre o papel das amebas testáceas como bioindicadoras, em especial nas regiões tropicais.

Abstract: Aim The free-living protists testate amoebae are recognized as being bioindicators, able to reveal environmental alteration via remodifying richness, diversity and abundance of species. To assess the scientific production on the use of testate amoebae as bioindicators, a scientometric analysis was undertaken from 1960 to 2020. Methods The sourcing of scientific articles derived from Google Academic, Scielo, Science Direct and Online Library research platforms. All selected papers were factored according to chronology, journal of publication, country, authors’ affiliation, whether the study was empirical or experimentation, if it included solely testate amoebae or other groups of organisms, and aspects of the environment. Results A total of 215 papers from ninety-two journals revealed a notable increase in publication numbers over the last decades. The two journals that most published data on this theme were Microbial Ecology and Ecological Indicators. Whilst the largest number of papers was published in the European Continent, the countries that most contributed to the subject were Canada in the temperate region and Brazil in the tropics. Edward A.D. Mitchell published the largest number of studies and regarding institutions the Franche-Comté University. The majority of papers that associated testaceans as bioindicators were empirical, conducted with Thecamoebian inhabiting moss and as such, dissociated from other groups. Conclusions Despite a suggested uptick in research of testaceans as bioindicators, our results indicate a concentration of studies to limited regions of the globe. In another aspect, a great number of studies assess testaceans’ community as-a-whole, showing a shortage of in-depth knowledge into species and functional groups. Therefore, our survey points to a wide variety of aquatic ecosystems devoid of study matter, indicating the need to foster the research on testate amoeba's role as bioindicators especially in the tropical regions.

The increasing prevalence of obesity is occurring in all ages, including in developing countries. The objective was to evaluate the achieved growth and nutriti nal status of two groups of schoolchildren from public schools: one at the beginning of school age and another in the transition to adolescence. The analysis of sample with 1082 children, divided into two groups according to the age, a group of 6 and 7 years (G1) and the other with 9 and 10 years (G2), from the public elementary schools of Floriano polis. For nutritional status classification of schoolchildren used the parameter z score of BMI (zBMI,) for children with five years and older (WHO 2007). Measures of central ten dency, dispersion and proportions based data analysis. The results were: the average age of G1 and G2 was respectively of 6.5 years (SD= 0.31), and 9.6 years (SD= 0.26); average of attainted growth (z score of height to age - H/A) of the sample (H/A: 0.25, SD= 1.09) was above the benchmark; the prevalence of overweight (overweight, obesity and severe obesity together) was 42.5%. The z score of BMI di tribution showed a deviation to the right, with an average of 0.78 and positive z score a SD = 1.32 (95% CI 0.69 to 0.85), without statistical difference between G1 and G2. In conclusion, the schoolchildren showed in 2012 appropriate height growth concomitantly to the absence of malnutrition and a very high prevalence of excess weight, which can re present a late stage of Nutritional Transition.

O aumento da prevalência do excesso de peso é observado em todas as idades, inclusive nos países em desenvolvimento. O objetivo foi avaliar o crescimento al cançado e o estado nutricional de crianças de duas gerações de escolares: uma no início da idade escolar e outra na tran sição para a adolescência. A análise de amostra, com 1082 crianças, divididas em dois grupos de idade, um grupo de 6 e 7 anos (G1) e o outro de 9 e 10 anos (G2), do Ensino Fundamental da Rede Pública de Florianópolis. Para clas sificação do estado nutricional dos escolares utilizou-se o parâmetro escore z de IMC (zIMC), para crianças com cinco anos e mais (OMS 2007). Foram realizadas análises de tendência central, dispersão e proporções. Os resultados foram: idade média de G1 e G2 foi respectivamente de 6,5 anos, desvio padrão (dp) de 0,31, e de 9,6 anos (dp= 0,26); média de estatura (E) da amostra (escore z de E: 0,25, dp= 1,09) foi superior à do referencial; prevalência de excesso de peso (sobrepeso, obesidade e obesidade grave, juntos) foi de 42,5%. A distribuição dos escolares segundo escore z de IMC, mostrou desvio para a direita, com uma média de zIMC de 0,78 e dp=1,32 (IC95% 0,69 a 0,85), sem diferença estatística entre G1 e G2. Em conclusão, os escolares apresentavam em 2012, crescimento adequado de estatura concomitantemente à ausência de desnutrição e a uma prevalência bastante elevada de excesso de peso, o que pode ser atribuído a uma etapa avançada de Transição Nutricional.

Resumo Objetivo: Investigar relações existentes entre excesso de peso em pré-escolares, duração do aleitamento materno e a idade de introdução de leite não materno. Métodos: Estudo transversal de amostra representativa de 817 pré-escolares, 2-4 anos de idade, de creches municipais de Taubaté. O peso e a altura das crianças foram mensurados nas creches em 2009, 2010 e 2011. Calculado o escore z de Índice de Massa Corporal (zIMC), as crianças foram classificadas como risco de sobrepeso (zIMC≥1 a<2) ou como excesso de peso (zIMC≥2). A análise dos dados foi feita por comparação de proporções, coeficiente de correlação e regressão linear multivariada. Resultados: A prevalência de risco de sobrepeso foi 18,9% e de excesso de peso (sobrepeso ou obesidade) de 9,3%. A mediana de duração do aleitamento materno e a idade de introdução do leite não materno foi de 6 meses. O zIMC da criança evidenciou correlação direta com o peso ao nascer (r=0,154; p<0,001) e com o Índice de Massa Corporal (IMC) materno (r=0,113; p=0,002). A correlação foi inversa com a duração total do aleitamento materno (r=−0,099; p=0,006) e a idade de introdução de leite não materno (r=−0,112; p=0,002). Não houve correlação entre o zIMC da criança com o comprimento ao nascer, duração do aleitamento exclusivo e idade da mãe. Conclusões: Quanto mais precoce a introdução de leite não materno, maior a correlação com excesso de peso na idade pré-escolar.

Abstract Objective: Investigate associations between excess weight in preschool children, breastfeeding duration and age of non-breast milk introduction. Methods: Cross-sectional study of a representative sample of 817 preschool children, aged 2-4 years, attending municipal day care centers in the city of Taubaté. The weight and height of children were measured in the day care centers in 2009, 2010 and 2011. The body mass index z-score (BMIz) was calculated and children were classified as risk of overweight (BMIz≥1 to<2) or excess weight (BMIz≥2). Data analysis was carried out by comparison of proportions, coefficient of correlation and multivariate linear regression. Results: The prevalence of risk of overweight was 18.9% and of excess weight (overweight or obesity) was 9.3%. The median duration of breastfeeding and age of introduction of non-breast milk was 6 months. The child's BMIz showed direct correlation with birth weight (r=0.154; p<0.001) and maternal body mass index (BMI) (r=0.113; p=0.002). The correlation was inverse with the total duration of breastfeeding (r=−0.099; p=0.006) and age at non-breast milk introduction (r=−0.112; p=0.002). There was no correlation between the child's BMIz with birth length, duration of exclusive breastfeeding and mother's age. Conclusions: The earlier the introduction of non-breast milk, the higher the correlation with excess weight at preschool age.

Growing phenomenon, which involves high morbidity and consequently high costs for health systems, obesity has been found also among the pediatric population and is currently considered a public health problem. The aim of this study was to verify if in children in the early preschool age we can see the prevalence of overweight and if introducing complementary feeding as well as the type of food introduced, are associated with this condition in this age group. It is an observational analytic study with children born in 2011-2012 that attended public schools in Taubaté -SP during 2014. In addition to the weight and height of children, information about the history of feeding and birth were collectedusing a standardized questionnaire.The nutritional status was defined as having overweight children with z-scores for body mass index (zIMC) ≥ 1.We conducted bivariate analysis and then linear regression analysis of multiple variables.The prevalence of overweight was elevated (27.5%). Only birth weight showed significant correlation with respect to zIMC (r = 0.22, p < 0.0001). The multivariable analysis showed no relationship with the various foods, but showed birth weightas a high risk factor, the male and the total duration of breastfeeding as protective factors. As a result, we conclude that the early introduction of new foods is not a risk factor for the development of overweight at the beginning of preschool age.

Fenômeno crescente, que envolve alta morbidade e altos custos para os sistemas de saúde, a obesidade está sendo encontrada também entre a população pediátrica, sendo considerada atualmente um problema de saúde pública. O objetivo do presente estudo foi verificar se em crianças no início da idade préescolar já se observa prevalência de excesso de peso e se a introdução de novos alimentos, assim como o tipo de alimento, está associada à essa condição nutricional. Trata-se de um estudo observacional, analítico, de uma coorte histórica com crianças nascidas em 2011/12, frequentadoras de creches públicas em Taubaté - SPem 2014. Além do peso e estatura das crianças, coletou-se informações quanto aos antecedentes de alimentação e de nascimento utilizando um questionário padronizado. Quanto aoestado nutricional definiu-se como portadoras de excesso de peso (EP) crianças com escores z do índice de massa corpórea (zIMC) ≥ 1. Realizou-se a análise bivariada e posteriormente análise de regressão linear de múltiplas variáveis. A prevalência de EP encontrada foi elevada (27,5%). Apenas o peso ao nascer (PN) mostrou correlação significante em relação ao zIMC (r= 0,22, p<0,0001). Aanálise de múltiplas variáveis não mostrou relação com os diversos alimentos, mas evidenciou o PN elevado como fator de risco, o sexo masculino e a duração total do aleitamento materno como fatores de proteção. Como consequência, é possível concluir que a introdução precoce de novos alimentos não representa fator de risco para o desenvolvimento do excesso de peso no início da idade pré-escolar.

OBJECTIVE: To analyze maternal conceptions about excess weight in infancy and the nutritional status of their preschool-aged children. METHODS: A mixed, exploratory study was performed using semi-structured interviews. Two study groups were defined: a group of 16 mothers of children with excess weight and a group of 15 mothers of eutrophic children. The interviews were submitted to content analysis using CHIC software (Classification Hiérarchique Implicative et Cohésitive¯). RESULTS: The mothers of children with excess weight tended to conceive thin children as malnourished, while those of normal weight children emphasized the influence of family and genetics as determinants of a child’s nutritional status. Although there was a certain consensus among the mothers that an unhealthy diet contributes to the risk of a child developing excess weight, the concept of genetics as a determinant of a child’s nutritional status was also present in the dialogue from the mothers of both groups. This result indicates a lack of clarity regarding the influence of eating behavior and family lifestyle on weight gain and the formation of a child’s eating habits. Both groups indicated that the mother has a decisive role in the eating habits of her child; however, the mothers of children with excess weight did not seem to take ownership of this concept when addressing the care of their own children. CONCLUSION: Differences in conceptions, including taking ownership of care, may contribute to the development of excess weight in preschool-aged children.

OBJETIVO: Os objetivos do presente estudo foram determinar equações de referência para pressão inspiratória nasal (PIN) e propor equações de referência para a população brasileira. MÉTODOS: Foram avaliados 243 indivíduos saudáveis (111 homens e 132 mulheres), entre 20 e 80 anos, com CVF > 80% e razão VEF1/CVF > 85% do predito. Todos os indivíduos realizaram testes de força muscular respiratória para a determinação de PIN, PImáx e PEmáx. RESULTADOS: Os valores da PIN foram maiores no sexo masculino que no feminino (p < 0,05), e a PIN apresentou correlação negativa com a idade para homens (r = -0,29; p < 0,001) e mulheres (r = -0,33; p < 0,0001). A análise de regressão linear múltipla também revelou que a idade permaneceu exercendo influência na predição da PIN em homens (R² = 0,09) e mulheres (R² = 0,10). Os valores preditos da PIN encontrados foram superiores àqueles obtidos em outras populações. CONCLUSÕES: Nesse contexto, sugerem-se equações preditivas para PIN em indivíduos brasileiros saudáveis na faixa etária entre 20 e 80 anos, com o intuito de minimizar discrepâncias diagnósticas ao comparar indivíduos.

OBJECTIVE: The objectives of this study were to determine reference values for sniff nasal inspiratory pressure (SNIP) and to propose reference equations for the population of Brazil. METHODS: We evaluated 243 healthy individuals (111 males and 132 females), between 20 and 80 years of age, with an FVC and FEV1/FVC ratio > 80% and > 85% of the predicted value, respectively. All of the subjects underwent respiratory muscle strength tests to determine MIP, MEP, and SNIP. RESULTS: We found that SNIP values were higher in males than in females (p < 0.05) and that SNIP correlated negatively with age, for males (r = -0.29; p < 0.001) and for females (r = -0.33; p < 0.0001). Linear regression also revealed that age influenced the predicted SNIP, for males (R² = 0.09) and females (R² = 0.10). We obtained predicted SNIP values that were higher than those obtained for other populations. CONCLUSIONS: We have devised predictive equations for SNIP to be used in adults (20-80 years of age) in Brazil. These equations could help minimize diagnostic discrepancies among individuals.

CONTEXTO E OBJETIVO: O Brasil atravessa um período de transição epidemiológica com mudanças demográfica e nutricional. A prevalência de obesidade está aumentando também entre crianças, causando inúmeros problemas de saúde, que estão se tornando questões de saúde pública. Objetivou-se avaliar a prevalência de excesso de peso em crianças de dois e três anos. TIPO DE ESTUDO E LOCAL: Estudo transversal em creches municipais de Taubaté, São Paulo, Brasil. MÉTODOS: Em amostra probabilística e randomizada de 447 pré-escolares, foram mensurados peso e altura, e calculado o índice de massa corporal (IMC). O estado nutricional foi classificado segundo pontos de corte do referencial da Organização Mundial de Saúde (2006). Compararam-se os valores médios de peso, estatura e IMC das crianças segundo a idade e sexo. RESULTADOS: A amostra final (n = 447) apresentou as seguintes médias. Idade: 38,6 meses (± 3,5), escore Z de peso para estatura (P/E): 0,50 (± 1,22), estatura para idade (E/I): -0,03 (± 1,07), peso para idade (P/I): 0,51 (± 1,23) e IMC: 0,51(± 1,23). A prevalência de excesso de peso (IMC > 1 z) foi de 28,86% e de crianças abaixo do peso ideal (IMC < -2 z) foi de 0,89%. Não houve diferença das médias de IMC entre as idades de dois e três anos (P = 0,66). CONCLUSÃO: Na população de crianças de dois e três anos de idade, já se evidencia elevada prevalência de excesso de peso, praticamente sem desnutrição, indicando que já pode estar ocorrendo uma transição nutricional importante inclusive em cidades médias de países em desenvolvimento.

CONTEXT AND OBJECTIVE: Brazil is undergoing a period of epidemiological transition associated with demographic and nutritional changes. The prevalence of obesity is also increasing in children and is causing numerous health problems that are becoming public health issues. The aim here was to evaluate the prevalence of overweight among children of two and three years of age. DESIGN AND SETTING: Cross-sectional study in municipal day care centers in Taubaté, state of São Paulo, Brazil. METHODS: Weight and height measurements were made on 447 preschool children forming a probabilistic randomized sample. Their body mass index (BMI) was calculated. Their nutritional status was classified using the World Health Organization reference cutoff points (2006). Their mean weight, height and BMI were compared according to their age and sex. RESULTS: The mean values for the final sample (n = 447) were as follows: mean age: 38.6 months (± 3.5) and Z scores for: weight/height (W/H): 0.50 (± 1.22); height/age: -0.03 (± 1.07); weight/age (W/A): 0.51 (± 1.23); and BMI: 0.51(± 1.23). The prevalence of overweight children (BMI > 1 z) was 28.86%, while the prevalence of underweight children (BMI < -2 z) was 0.89%. There were no differences in mean BMI among the two and three-year age groups (P = 0.66). CONCLUSION: A high prevalence of overweight was observed in the sample of two and three-year-old children, with practically no malnutrition, thus showing that a significant nutritional transition may already be occurring, even in medium-sized cities of developing countries.

OBJETIVO: Analisar a prevalência de risco de sobrepeso, e sobrepeso e obesidade em crianças de pré-escolas privadas e filantrópicas da região metropolitana de São Paulo. MÉTODOS: Comparação de dois estudos transversais, com crianças matriculadas em pré-escolas privadas (PEP) ou filantrópicas (PEF), nos quais o universo de crianças foi avaliado. Para classificar o risco de sobrepeso, e sobrepeso e obesidade, e os valores de índice de massa corpórea (IMC) foram transformados em escores z (Organização Mundial de Saúde - 2006 e 2007). RESULTADOS: A prevalência de risco de sobrepeso (z IMC > 1 a < 2) nas PEP foi de 21,9% e nas PEF, de 24,6%, com RP= 1,12 (IC 95%: 0,96-1,32), sem diferença estatística. Considerando as crianças com sobrepeso e obesidade (z IMC > 2), a prevalência nas PEP foi de 14,3% e nas PEF foi de 9,0%, RP = 1,54 (IC 95%: 1,23-1,93), com p = 0,0002. No sexo masculino, a prevalência de sobrepeso e obesidade nas PEP foi de 16,4% (n = 409) e nas PEF, de 11,1% (n = 829), RP = 1,48 (IC 95%: 1,10-1,98), e no sexo feminino foi de 12,5% (n = 400) nas PEP e 6,6% (n = 698) nas PEF, RP = 1,90 (IC 95%: 1,30-2,78), ambas as diferenças significantes. CONCLUSÃO: A prevalência de risco de sobrepeso foi semelhante e muito elevada nos dois grupos de crianças. O sobrepeso e a obesidade ainda apresentaram maior frequência nas crianças de pré-escolas privadas. Assim, apesar de uma melhor condição socioeconómica ainda ser fator de risco para sobrepeso e obesidade em pré-escolares, o mesmo parece que já não ocorre quando se analisa o risco de sobrepeso.

OBJECTIVE: To assess the risk prevalence of overweight and obesity in children enrolled in private and philanthropic preschools in the State of São Paulo. METHODS: Comparison of two cross sectional studies with children enrolled in private preschools (PPS) or philanthropic (PHP) of the São Paulo Metropolitan Region. Both surveys evaluated the children's environment. To determine the risk of overweight, excess weight and obesity, body mass index (BMI) values were transformed into z scores (according to the World Health Organization - 2006 and 2007). RESULTS: The risk prevalence of overweight (≥ 1 BMIz < 2) in PPS was 21.9% and 24.6% in PHP, with PR = 1.12 (95% CI: 0.96-1.32), without statistical difference. Considering the children with overweight or obesity, (BMIz ≥ 2) the prevalence in PPS was 14.3% and in PHP was 9.0%, with PR = 1.54 (95% CI: 1.23-1.93), p = 0.0002. Overweight and obesity prevalence in males in PPS was 16.4% (n = 409) and in PHP, 11.1% (n = 829), PR = 1.48 (95% CI: 1.10-1.98) and in females it was 12.5% (n = 400) in the PPS and 6.6% (n = 698) in PHP, corresponding to PR = 1.90 (95% CI: 1.30-2.78), both significant differences. CONCLUSION: Both groups showed a similar and very high prevalence of weight excess. However, overweight and obesity showed a higher prevalence in children from private preschools. This indicates that even though a better socioeconomic level is still a risk factor for overweight and obesity in preschoolers, the same does not seem to occur when analyzing the risk of overweight.

OBJETIVO: Analisar a associação do sobrepeso e da obesidade com o aleitamento materno e a alimentação complementar em pré-escolares. MÉTODOS: Estudo transversal envolvendo 566 crianças matriculadas em escolas particulares no município de São Paulo, SP, 2004-2005. A variável dependente foi sobrepeso e obesidade. Para a classificação do estado nutricional das crianças foram utilizadas as curvas de percentis do Índice de Massa Corporal para idade, classificando como sobrepeso valores e"P85 e <P95, e como obesidade valores e"P95. As variáveis explanatórias analisadas foram: características sociodemográficas da criança e sua família peso ao nascer; estado nutricional dos pais; aleitamento materno; alimentação complementar e alimentação atual. A análise de associação das variáveis explanatórias com o desfecho foi feita por meio de regressão logística simples e regressão logística múltipla com modelo hierarquizado. RESULTADOS: A prevalência de sobrepeso e obesidade da população estudada foi de 34,4%. Foram fatores de proteção contra sobrepeso e obesidade o aleitamento materno exclusivo por seis meses ou mais (IC 95% [0,38;0,86]; OR=0,57; p=0,02) e o aleitamento materno por mais de 24 meses (IC 95% [0,05;0,37]; OR=0,13; p=0,00). CONCLUSÕES: Os resultados sugerem que o aleitamento materno pode proteger as crianças contra o sobrepeso e a obesidade, agregando mais uma vantagem ao leite materno.

OBJECTIVE: To analyze the association of overweight and obesity with breastfeeding and complementary feeding in pre-school children. METHODS: Cross-sectional study with 566 children, enrolled in private schools of the city of São Paulo, Southeastern Brazil, in 2004-2005. The dependent variable was overweight and obesity. Body Mass Index percentile curves were employed to classify children's nutritional status, considering values e"P85 and <P95 as overweight, and values e"P95 as obesity. The following explanatory variables were analyzed: child and family socio-demographic characteristics; birth weight; parents' nutritional status; breastfeeding; complementary feeding; and current feeding. Analysis of association between explanatory variables and outcome was performed with simple logistic regression and multiple logistic regression with hierarchical model. RESULTS: Prevalence of overweight and obesity in the population studied was 34.4%. The following were protective factors against overweight and obesity: exclusive breastfeeding for six months or more (95% CI [0.38;0.86]; OR=0.57; p=0.02) and breastfeeding for more than 24 months (95% CI [0.05;0.37]; OR=0.13; p=0.00). CONCLUSIONS: Results suggest that breastfeeding can protect children against overweight and obesity, thus representing yet another advantage of maternal milk.

OBJETIVO: Analizar la asociación del sobrepeso y de la obesidad con el amamantamiento materno y la alimentación complementaria en preescolares. MÉTODOS: Estudio transversal envolviendo 566 niños matriculados en escuelas privadas en el municipio de Sao Paulo (Sureste de Brasil), 2004-2005. La variable dependiente fue sobrepeso y obesidad. Para la clasificación del estado nutricional de los niños fueron utilizadas las curvas de percentiles del índice de masa corporal para edad, clasificando como sobrepeso valores ?P85 y <P95, y como obesidad valores ?P95. Las variables explicativas analizadas fueron: características sociodemográficas del niño y su familia; peso al nacer; estado nutricional de los padres; amamantamiento materno; alimentación complementaria y alimentación actual. El análisis de asociación de las variables explicativas con el hecho, fue realizado por medio de regresión logística simple y regresión logística múltiple con modelo jerarquizado. RESULTADOS: La prevalencia de sobrepeso y obesidad de la población estudiada fue de 34,4%. Fueron factores de protección contra sobrepeso y obesidad, el amamantamiento materno exclusivo por seis meses o más (IC 95% [0,38;0,86]; OR=0,57; p=0,02) y el amamantamiento materno por más de 24 meses (IC 95% [0,05;0,37]; OR=0,13; p=0,00). CONCLUSIONES: Los resultados sugieren que el amamantamiento materno puede proteger los niños contra sobrepeso y la obesidad, agregando otra ventaja a la leche materna.

OBJETIVO: Analisar a associação do sobrepeso e da obesidade com o aleitamento materno e a alimentação complementar em pré-escolares. MÉTODOS: Estudo transversal envolvendo 566 crianças matriculadas em escolas particulares no município de São Paulo, SP, 2004-2005. A variável dependente foi sobrepeso e obesidade. Para a classificação do estado nutricional das crianças foram utilizadas as curvas de percentis do Índice de Massa Corporal para idade, classificando como sobrepeso valores e"P85 e <P95, e como obesidade valores e"P95. As variáveis explanatórias analisadas foram: características sociodemográficas da criança e sua família peso ao nascer; estado nutricional dos pais; aleitamento materno; alimentação complementar e alimentação atual. A análise de associação das variáveis explanatórias com o desfecho foi feita por meio de regressão logística simples e regressão logística múltipla com modelo hierarquizado. RESULTADOS: A prevalência de sobrepeso e obesidade da população estudada foi de 34,4%. Foram fatores de proteção contra sobrepeso e obesidade o aleitamento materno exclusivo por seis meses ou mais (IC 95% [0,38;0,86]; OR=0,57; p=0,02) e o aleitamento materno por mais de 24 meses (IC 95% [0,05;0,37]; OR=0,13; p=0,00). CONCLUSÕES: Os resultados sugerem que o aleitamento materno pode proteger as crianças contra o sobrepeso e a obesidade, agregando mais uma vantagem ao leite materno.

OBJECTIVE: To analyze the association of overweight and obesity with breastfeeding and complementary feeding in pre-school children. METHODS: Cross-sectional study with 566 children, enrolled in private schools of the city of São Paulo, Southeastern Brazil, in 2004-2005. The dependent variable was overweight and obesity. Body Mass Index percentile curves were employed to classify children's nutritional status, considering values e"P85 and <P95 as overweight, and values e"P95 as obesity. The following explanatory variables were analyzed: child and family socio-demographic characteristics; birth weight; parents' nutritional status; breastfeeding; complementary feeding; and current feeding. Analysis of association between explanatory variables and outcome was performed with simple logistic regression and multiple logistic regression with hierarchical model. RESULTS: Prevalence of overweight and obesity in the population studied was 34.4%. A alimentação complementar é influenciada por complexos fatores sociais, econômicos e culturais. OBJETIVO: Verificar a idade de introdução de alimentos complementares no primeiro ano de vida, e sua relação com variáveis demográficas e socioeconômicas. MÉTODOS: Estudou-se uma coorte de crianças nascidas em Hospital Universitário do Município de São Paulo. A verificação da idade de introdução de alimentos foi feita a partir de fichas preenchidas pela mãe, diariamente, onde constavam os alimentos oferecidos à criança a cada dia, até o final do seguimento. A introdução dos alimentos foi descrita utilizando-se a técnica de análise de sobrevida de Kaplan & Meier, e a análise de associação entre idade de introdução dos alimentos e as variáveis independentes foi feita pelo modelo de análise multivariável de Cox. RESULTADOS: As idades medianas de introdução de água ou chá, leite não materno, frutas, verduras e legumes, cereais e tubérculos foram menores de 6 meses. Foram verificadas associações estatisticamente significativas entre escolaridade materna, renda familiar, trabalho materno e introdução de alguns alimentos. CONCLUSÃO: Houve introdução precoce de água ou chá e de leite não materno. Além da renda e do trabalho materno, a escolaridade materna parece ser a variável que mais influencia a introdução de alimentos complementares.

Complementary feeding is a complex process that is influenced by social, economic and cultural factors. OBJECTIVES: To verify the age of introduction of complementary food in the first year of life, and its relation with demographic and socioeconomic variables. METHODS: A cohort of children born in a University Hospital in the Municipality of Sao Paulo was studied. The age of introduction of foods to the infants was verified through a form that mothers filled out every day with the foods offered to their children, until the end of the follow-up. The introduction of foods was described using the Kaplan & Meier life table analysis. The association between age of introduction of foods and independents variables was analyzed by using Cox's multivariate analysis model. RESULTS: The median age of introduction of water or tea, milk, fruits, greens and vegetables, cereals and tubercles was less than six months. Statistically significant associations between mother's schooling, family income, working conditions of the mother and supplementary foods introduction were observed. CONCLUSION: There was an early introduction of the water or tea and non-breast milk. In addition to family income and mother's working conditions, mother's schooling seems to be the variable that most influenced the introduction of supplementary foods.
