Resumen Introducción. Ante la amenaza de la pandemia por COVID-19, la mayoría de los países han establecido diversas medidas de control y prevención para disminuir la rápida propagación de esta enfermedad. Objetivos. Comparar las estrategias de control de la pandemia por COVID-19 implementadas en Perú (de confinamiento y distanciamiento social de la Organización Mundial de la Salud (OMS)) y Reino Unido (de inmunidad de rebaño (IR)) en términos de morbimortalidad, y simular la implementación de la IR en Perú durante la etapa inicial de la pandemia. Materiales y métodos. Estudio exploratorio con un modelo de simulación basado en datos oficiales de ambos países registrados al inicio de la pandemia. Se evaluaron las tasas de mortalidad, letalidad e infección en Reino Unido (IR) y Perú (confinamiento y distanciamiento social) dentro de los 55 días posteriores al reporte del primer caso de COVID-19 en Reino Unido y al inicio de la implementación de la estrategia de control y prevención recomendada por la OMS en Perú. Además, se simuló el impacto de haber aplicado la IR, según pautas de la OMS, como estrategia inicial en Perú. Se utilizó la prueba t-Student para muestras relacionadas para determinar las diferencias entre ambas estrategias en las dos etapas del estudio. Resultados. En el periodo de seguimiento se registraron 15 034 y 33 931 casos de COVID-19 en Reino Unido y Perú, respectivamente. La tasa de letalidad fue mayor para Reino Unido (7.82% vs. 2.74%), y la tasa de mortalidad acumulada fue mayor en Perú (2.89 vs. 1.74x100 000 habitantes; p=0.0001). Respecto a la simulación, se estableció una población crítica mínima de 60% (>19 millones de casos positivos) para que Perú logre la IR, con 1 223 473.1 muertes y una tasa de hospitalización de 44 770x100 000 pacientes. Conclusiones. Durante el periodo de seguimiento (55 días), la estrategia de Reino Unido resultó en una mayor letalidad y la peruana, en más del doble de casos de COVID-19. La simulación de la IR en Perú mostró un dramático incremento de todos los indicadores desfavorables de la pandemia.

Abstract Introduction: In light of the threat posed by the COVID-19 pandemic, most countries have implemented several containment and prevention measures to slow down the rapid spread of the disease. Objectives: To compare the COVID-19 pandemic containment strategies implemented in Peru [World Health Organization (WHO) - confinement and social distancing] and the United Kingdom [herd immunity (HI)] in terms of morbidity and mortality, and to simulate the implementation of HI in Peru during the initial stage of the pandemic. Materials and methods: Exploratory study with a simulation model based on official data obtained from both countries at the beginning of the pandemic. Mortality, case fatality, and infection rates documented within the first 55 days after the first COVID-19 case report in the United Kingdom and the start of the WHO-recommended containment and prevention strategy implementation in Peru were evaluated. Additionally, the impact of applying HI, according to WHO guidelines, as the initial strategy in Peru was simulated. The Paired-samples t-test was used to determine the differences between the two strategies at both stages of the study. Results: During the follow-up period, 15 034 and 33 931 COVID-19 cases were reported in the United Kingdom and Peru, respectively. The case fatality rate was higher in the United Kingdom (7.82% vs. 2.74%), while the cumulative mortality rate was higher in Peru (2.89 vs. 1.74x100 000 inhabitants p= 0.0001). Regarding the simulation, a minimum critical population of 60% (>19 million positive cases) was established for Peru to achieve HI, with 1 223 473.1 deaths and a hospitalization rate of 44 770x100 000 patients. Conclusions: During the follow-up period (55 days), the United Kingdom's strategy resulted in a higher case fatality rate, while the Peruvian strategy in over twice as many COVID-19 cases. The HI simulation strategy in Peru showed a sharp increase in all unfavorable indicators of the pandemic.

Abstract Introduction: Infertility is a multicausal disease and the genetic component represents one of its main events. Although the distribution of infertility may vary between populations, couples in low-and-middle-income countries may be more affected by infertility with a proportion of cytogenetic alterations still unclear. Objective: To evaluate the frequency of cytogenetic alterations and their correlation with the number of abortions in Peruvian patients with a diagnosis of infertility. Method: A cross-sectional study was carried out in 400 patients between 18 and 60 years-old, of both genders with a diagnosis of infertility. The clinical characteristics available during the genetic examination were recorded and the conventional cytogenetic analysis was with GTG banding in peripheral blood samples. The correlation analysis was performed with the Spearman test. Results: Of the total 389 patients who met the inclusion criteria, of these 169 (43.44%) patients had reports of abortions (mean: 2.25, range: 1-7). We found a significant correlation between the number of abortions and cytogenetic alterations (p < 0.000). We report 25/289 (6.43%) chromosomal alterations, where 11/25 (44%) were constitutive heterochromatin, and 6/25 (24%) were reciprocal translocations. The most frequent cytogenetic alterations were 16qh + and 9qh + (both 16%), and affected 17 (68%) men. Conclusions: There is a moderate frequency of cytogenetic alterations in Peruvian patients diagnosed with infertility, where the most frequent alterations were constitutive heterochromatin. Furthermore, we evidenced a significant correlation between the number of abortions and cytogenetic alterations.

Resumen Introducción: La infertilidad es una enfermedad multicausal y el componente genético representa uno de sus principales eventos. Si bien la distribución de la infertilidad puede variar entre poblaciones, las parejas de los países con bajos y medianos ingresos pueden verse más afectadas por la infertilidad, con una proporción de alteraciones citogenéticas aún no esclarecidas. Objetivo: Evaluar la frecuencia de alteraciones citogenéticas y su correlación con el número de abortos en pacientes peruanas con diagnóstico de infertilidad. Método: Se realizó un estudio de corte transversal en 400 pacientes de 18 a 60 años, de ambos sexos, con diagnóstico de infertilidad. Se registraron las características clínicas disponibles durante el examen genético y el análisis citogenético convencional fue con bandeo GTG en muestras de sangre periférica. El análisis de correlación se realizó con la prueba de Spearman. Resultados: Del total, 389 pacientes cumplieron los criterios de inclusión, y de estos, 169 (43,44%) tuvieron reportes de abortos (promedio: 2,25, rango: 1-7). Hallamos una correlación significativa ente el número de abortos y las alteraciones citogenéticas (p < 0,000). Reportamos 25/289 (6,43%) alteraciones cromosómicas, de las que 11/25 (44%) fueron heterocromatinas constitutivas y 6/25 (24%) fueron translocaciones reciprocas. Las alteraciones citogenéticas más frecuentes fueron 16qh+ y 9qh+ (ambas con un 16%), y afectaron a 17 (68%) varones. Conclusiones: Existe una moderada frecuencia de alteraciones citogenéticas en pacientes peruanos con diagnóstico de infertilidad, y las alteraciones más frecuentes fueron heterocromatina constitutivas. Además, evidenciamos una correlación significativa ente el número de abortos y las alteraciones citogenéticas.

Resumen Introducción. Para prevenir el desarrollo de enfermedad hepática crónica en pacientes con VIH, durante la terapia antirretroviral de gran actividad (TARGA) se deben estimar y monitorear cambios en el perfil hepático. Objetivo. Determinar la variación de las concentraciones del perfil hepático en pacientes con VIH durante la TARGA. Materiales y métodos. Estudio retrospectivo longitudinal realizado en 100 pacientes con VIH atendidos en el Hospital Nacional Hipólito Unanue, Lima, Perú, entre 2015 y 2017. Se incluyeron pacientes en todos los estadios de infección clínica que estuvieran recibiendo TARGA y en los que se contara con resultados del perfil hepático para alanina aminotransferasa (ALT), aspartato aminotransferasa (AST), fosfatasa alcalina (FA), gammaglutamiltranspeptidasa (GGT) y proteínas totales (PT). Se realizaron tres análisis de control de la función hepática durante la TARGA (1 cada 3 meses) y los participantes se agruparon en niveles normales y elevados para todos los marcadores hepáticos. Las diferencias entre las muestras analizadas fueron determinadas mediante la prueba t-Student para muestras relacionadas, con un intervalo de confianza de 95% y un nivel de significancia de p<0.05. Resultados. La edad promedio fue de 33±9.56 años y el 67% fueron varones. Los valores séricos promedio de AST, ALT y FA disminuyeron entre la primera y la tercera medición (p=0.021, p=0.076 y p=0.002, respectivamente). No se observaron diferencias significativas en los niveles de GGT y PT entre las tres mediciones, ni entre los pacientes con valores normales y elevados para AST, ALT, FA y PT, pero sí para GGT (p=0.010). Conclusiones. Se observaron variaciones en los niveles de los marcadores hepáticos de todos los participantes, con una tendencia a la reducción en AST, ALT y FA entre las etapas iniciales y finales de la terapia, lo que implica que la TARGA podría ejercer un rol en el daño tisular hepático.

Abstract Introduction: Estimating and monitoring changes in liver function tests is necessary to prevent the occurrence of chronic liver disease in HIV patients undergoing highly active antiretroviral therapy (HAART). Objective: To determine the variation liver profile test levels in HIV patients undergoing HAART. Materials and methods: Retrospective longitudinal study conducted in 100 HIV patients treated at the Hospital Nacional Hipólito Unanue, Lima, Peru, between 2015 and 2017. Patients in all stages of clinical infection under HAART and with liver function panel results for alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (ALP), gamma-glutamyl transferase (GGT), and total protein (TP) were included. Three follow-up liver function tests (every 3 months) were performed while undergoing HAART and participants were categorized as having normal or elevated levels for all liver markers. Differences between the samples analyzed were determined using the paired-samples T test, with a 95% confidence interval and a significance level of p<0.05. Results: Participants' mean age was 33±9.56 years and 67% were male. Mean serum AST, ALT and ALP values decreased between the first and the third measurement (p=0.021, p=0.076 and p=0.002, respectively). No significant differences in GGT and TP levels were observed between the three measurements, nor between patients with normal and elevated AST, ALT, ALP and TP values, but significant differences were observed for GGT (p=0.010). Conclusions: Variations in liver marker levels were observed in all participants, with a decreasing trend in AST, ALT and ALP between the early and late stages of HAART, implying that this therapy could play a role in liver tissue damage.

ABSTRACT Objective Depression is a mental problem that affects the well-being of healthcare workers, impacting the quality of care and even leading to commit suicide. We aim to the levels of depression in frontline healthcare workers during the first severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) outbreak in Peru. Methods A prospective cohort study was designed in the coronavirus disease 2019 (COVID-19) Care and Isolation Center – Villa Panamericana in eastern Lima. Care and Isolation Center-Villa Panamericana houses about 150 healthcare workers and COVID-19 patients. The Montgomery-Asberg Depression Rating scale was used for depression assessment. Results A total of 96 participants (30±5.6 years) were analyzed: 15 (15.6%) physicians, 39 (40.6%) nurses, 14 (14.6%) medical technologists, and 28 (29.2%) nurse technicians. Mild, moderate, and severe depression were present in 35 (36.5%), 44 (45.8%), and 9 (9.4%) of the cases, respectively. The physicians and nurses reported more severe levels of depression: 8 (53.3%) physicians and 18 (46.2%) nurses presented moderate depression; and 2 (13.3%) physicians and 3 (7.7%) nurses presented severe depression (p=0.005). This study determined greater symptoms of depression according to years of work (p=0.001). Thirty-two healthcare workers had COVID-19, 4 (12.5%) physicians, 9 (28.1%) nurses, 7 (21.9%) medical technologists, and 12 (37.5%) nurse technicians. Twenty-four (75%) participants showed symptoms of COVID-19 and developed moderate (12 [37.5%]) and severe (3 [9.4%]) symptoms of depression (p=0.041). Conclusion This study clearly demonstrated a high prevalence of depression in the Care and Isolation Center-Villa Panamericana frontline healthcare workers during the COVID-19 pandemic in Peru.

Resumen Objetivo: Determinar los factores de riesgo en adultos con COVID-19 en población rural andina durante 2020. Métodos: En este estudio de cohorte retrospectivo multicéntrico, incluimos a 184 pacientes adultos (≥18 años) con pruebas serológicas y moleculares para CO VID-19 de tres hospitales de la sierra peruana (Ancash y Apurímac) incluidos con sospecha clínica entre abril y junio. Se utilizó análisis descriptivos y regresión logística univariable para explorar los factores de riesgo asociados a los pacientes con COVID-19. Resultados: Del total de pacientes, 14 (7.6%) tuvieron SARS-CoV-2. En los pacientes infectados 12 (85.7%) fueron varones con promedio de edad de 47.3±21 años. Las comorbilidades estuvieron presentes en cerca de la tercera parte de pacientes, siendo la hipertensión y diabetes las más frecuentes (ambas 14.3%), y la sinto matología más frecuentes fueron fiebre y cefalea (57.2%). La regresión univariable mostró mayores probabilidades de infección con SARS-CoV-2 en la población rural andina asociada con la edad avanzada (OR: 1.1 IC95% 0.7-1.8; p=0,019), comorbilidades previas (OR: 1.7, IC95% 0.32-9.39; p=0,006), y sintomatología previa (OR: 49.8, IC95% 5.6-436.9; p=0,0011). Conclusiones: Los posibles factores de riesgo como la edad avanzada, las comorbilidades y sintomatología previas están relacionados con el desarrollo de CO VID-19 en población rural andina de Perú.

Abstract Objective: To determine the risk factors in adults with COVID-19 in the rural Andean population during 2020. Methods: This multicenter retrospective cohort study included 184 adult patients (≥18 years) with COVID-19’s serological and molecular tests from three Hospitals in the Peruvian mountains (Ancash and Apurímac) included with clinical suspicion between April and June. Descriptive analysis and univariate logistic regression were used to explore the risk factors associated with patients with COVID-19. Results: Of total of patients, 14 (7.6%) had a SARS-CoV-2. In infected patients 12 (85.7%) were men with an average age of 47.3±21 years. Comorbidities were present in about a third of patients, with hypertension and diabetes being the most frequent (both 14.3%), and the most frequent symptoms were fever and hea dache (57.2%). Univariate regression showed higher probabilities of infection with SARS-CoV-2 in the rural Andean population associated with advanced age (OR: 1.1 95% CI 0.7 - 1.8; p = 0.019), previous comorbidities (OR: 1.7, 95%CI 0.32 - 9.39; p = 0.006), and previous symptoms (OR: 49.8, 95%CI 5.6 - 436.9; p = 0.0011). Conclusions: Possible risk factors such as advanced age, comorbidities and previous symptoms are related to the development of COVID-19 in the rural Andean population of Peru.

ABSTRACT Introduction: The characteristics of new medical students are important to estimate the relevance of the courses they will attend during their professional training. Objective: Determine the profile of students and the impact of undergraduate courses on medical students from Norbert Wiener University in Lima, Peru. Methods: A prospective study was conducted of 148 second- and third-year undergraduate students during the year 2019. These students had already attended basic, specific and specialty courses. A total 34 courses were selected for impact evaluation, using a questionnaire containing 53 multiple choice closed-ended questions divided into three sections: 1) demographic data, 2) frequent activities and 3) course impact. Results: Seventy-seven (55.8%) students were aged 20-30 years, 69.6% were female and 26.8% were workers. The students spend a fair amount of time reading scientific articles (61%) and little time learning languages (69.3%) or about sports, art and politics (≥ 72%). The highest-impact specific courses were Normal Morphophysiology I (74%) and II (71.5%), Damage and Responses I (72.1%) and Nutrition for Health Sciences (44%), whereas the specialty courses obtaining the highest percentages were Semiology, Physiopathology and Clinical Reasoning (72.4%), Internal Medicine I (73.5%), Damage and Responses II (65%) and Internal Medicine II (76.5%). Conclusions: Determination was made of a heterogeneous profile and a high impact of specific and specialty undergraduate courses in Peruvian medical students.

RESUMEN Introducción: Las características de los nuevos estudiantes de medicina son importantes para estimar la trascendencia de los cursos durante su formación profesional. Objetivo: Determinar el perfil de los estudiantes y el impacto de los cursos de pregrado en estudiantes de Medicina de la Universidad Norbert Wiener en Lima, Perú. Métodos: Se realizó un estudio prospectivo en 148 estudiantes de pregrado de segundo y tercer año durante el 2019, que ya habían cursado los cursos básicos, cursos específicos y de especialidad. Para evaluar el impacto se seleccionaron 34 cursos y se utilizó cuestionario de 53 preguntas cerradas de opción múltiple dividida en tres partes: 1) datos demográficos, 2) actividades frecuentes, 3) impacto de cursos. Resultados: Del total, 77 (55,8 %) estudiantes tuvieron entre 20-30 años, 69,6 % fueron mujeres y 26,8 % trabajaban. Los estudiantes dedican un tiempo regular a la lectura de artículos científicos (61 %), y poco tiempo a aprender idiomas (69,3 %), deporte, arte o política (≥ 72 %). Los cursos con alto impacto fueron: Morfofisiología Normal I (74 %) y II (71,5 %), Noxas y Respuestas I (72,1 %) y Nutrición para las Ciencias de la Salud (44 %), en los cursos específicos. En los cursos de especialidad los de mayores porcentajes fueron Semiología, Fisiopatología y Raciocinio Clínico (72,4 %), Medicina Interna I (73,5 %), Noxas y Respuestas II (65 %), Medicina Interna II (76,5 %). Conclusiones: Se determinó un perfil heterogéneo y alto impacto en los cursos de especialidad y específicos de pregrado en estudiantes peruanos de Medicina.

Abstract On the gradual loss of John Hull's vision and memories, recounted in Notes on blindness (2016), we propose an informational explanation of the neurological changes that blindness leads to. The brain, as a reorganization system, can adapt to the loss of functions, such as sight, and on this base the development of new adaptive capacities. In John Hull, the epicconscious activity is maintained on the basis of a perceptual activity that is oriented essentially from the auditory and the tactile, and no longer from the visual modality. The visual memory is fading, while the conscious-psychic activity is based on the auditory memory and the tactile memory. The occipital primary areas were wrapped, from above (kinetically) in the paleocortical areas, achieving non-typical processing: such as activation in the perception of sound and touch. Epicconscious activity is never altered. John Hull always imagines and thinks, always acts and perceives. The essential thing that happens to him is that he must learn to perceive not visually but, and essentially: auditory and tactile. John Hull, then, has learned to see with his auditory and tactile sensations.

Resumen Sobre la gradual desaparición de la visión y los recuerdos de John Hull, contada en Notas sobre la ceguera (2016), planteamos una explicación informacional de los cambios neurológicos a los que les conlleva la ceguera. El cerebro, en tanto un sistema de reorganización, puede adaptarse a la pérdida de funciones, como la vista, y sobre esta asentar el desarrollo de nuevas capacidades de adaptación. En John Hull, la actividad epiconsciente se mantiene sobre la base de una actividad perceptual que se orienta esencialmente desde lo auditivo y lo táctil, y ya no desde la modalidad visual. La memoria visual se va desvaneciendo, mientras que la actividad psíquica consciente se sostiene sobre la memoria auditiva y la memoria táctil. Las áreas primarias occipitales, fueron envueltas, desde arriba (cinéticamente) en las áreas paleocorticales, logrando un procesamiento no típico: como la activación en la percepción del sonido y el tacto. La actividad epiconsciente nunca se ve alterada. John Hull siempre imagina y piensa, siempre actúa y percibe. Lo esencial que le ocurre es que debe aprender a percibir no visualmente sino, y de modo esencial: auditiva y táctilmente. John Hull, entonces, ha aprendido a ver con sus sensaciones auditivas y táctiles.

Abstract There is a notable activity of the cinema within the medical training processes promoting the development of multiple inherent skills. In this study, we present a proposal for film therapy in the complementary context of current medical care. We review series of aforetime, broadcast around the 70`s, consisting of short chapters (~25 minutes). Series like Candy Candy, Marco, or Heidi, have marked differences from modern series, and the potential to be therapeutic instruments in themselves due to: i) presenting specific characters (a boy or a girl in particular, in a specific context) , ii) the existence of an argumentative story (which is followed chapter by chapter, which forces the child to recreate a story longitudinally), iii) the approach to existential problems (the lives of the characters force them to live limit situations, usually linked to the historical conditions of their time); and iv) the experience of experiences of a moral nature (involving a discussion of good and bad). These could link the child and the father, since each chapter represents an activity of daily life, exempting them from the mass transmission of series without educational (therapeutic) content.

Resumen Existe una remarcada actividad del cine dentro de los procesos de formación médica promoviendo el desarrollo de múltiples habilidades inherentes. En este estudio, presentamos una propuesta de terapéutica fílmica en el contexto complementario de la asistencia médica actual. Revisamos series de antaño, emitidas alrededor de los 70`s, que constan de capítulos cortos (~25 minutos). Series como Candy Candy, Marco, o Heidi, tienen marcadas diferencias de las series contemporáneas, y el potencial de ser instrumentos terapéuticos en sí mismos debido a: i) presentar personajes concretos (un niño o una niña en particular, en un contexto específico), ii) la existencia de un relato argumentativo (que se sigue capítulo a capítulo, lo cual obliga a que el niño o niña recree longitudinalmente una historia), iii) el abordaje de problemas existenciales (la vida de los personajes los obligan a vivir situaciones límite, usualmente ligados a las condiciones históricas de su tiempo); y iv) la vivencia de experiencias de orden moral (que suponen una discusión sobre lo bueno y lo malo). Estos podrían vincular al niño y el padre, ya que cada capítulo representa una actividad de la vida diaria, eximiendo a estos de la transmisión masiva de series sin contenidos educativos (terapéuticos).

Abstract We propose the answer to a question that until today is very questionable, is there life after death? In this document we will give an answer from the perspective of the Sociobiological Informational Theory. For this we need exact moments of the movie Coco (2017), in which this theory can be applied, thanks to the cinema. It should be noted first that in the film there are two aspects of the same side. A life as social information and a life as a conscious-psy- chic system, then the key moment that serves as an example to explain this phenomenon is when the character «Chicharrón» disappears from the world of the dead when no one in the real world remembers. However, when the old «Coco» remembers her deceased father «Hector», he manages to revive informationally. At the same time, we also indicate and congratulate the upright graphic quality in the details that characterize the animation. In short, we are going to say that, indeed, yes, there is life (informationally speaking) after death. And this because when we appear in the world we begin to live as social information, then we are born, and our life as social information grows in increasing complexity.

Resumen Se plantea la respuesta a una pregunta que hasta el día de hoy es muy discutible, ¿hay vida luego de la muerte? En este documento daremos una respuesta desde la perspectiva de la Teoría Sociobiológica Informacional. Para esto precisamos momentos exactos de la película Coco (2017), en el cual gracias al cine se puede dar aplicación a esta teoría, cabe indicar primero que en la película hay dos aspectos de un mismo lado. Una vida como información social y una vida como sistema psíquico consciente, entonces el momento clave que nos sirve como ejemplo para explicar este fenómeno es cuando el personaje «Chicharrón» desaparece del mundo de los muertos cuando nadie en el mundo real lo recuerda, sin embargo cuando la anciana «Coco» recuerda a su padre fallecido «Héctor», este logra revivir informacionalmente. A su vez también señalamos y felicitamos la buena calidad gráfica en los detalles que caracterizan la animación. En síntesis, vamos a decir que, en efecto, sí, existe la vida (informacionalmente hablando) después de la muerte. Y esto porque cuando aparecemos en el mundo empezamos a vivir como información social, entonces nacemos, y nuestra vida como información social crece en complejidad creciente.

ABSTRACT Introduction: Dyslipidemia is a risk factor for the development of cardiovascular diseases, which are considered to be the main cause of death at the global level. Objectives: To assess lipid disorders in Hispanic Americans patients in the primary health care level in Lima. Methods: In the period 2015-2016 a prospective research was made in 117 patients of the Service of Clinical Laboratory, La Fe Policlinic in Lima. There were used the guides named National Cholesterol Education Program Adult Treatment Panel III (NCEP ATP III), US Preventive Services Task Force, and the clinical practice guidelines of the Mexican Institute of Social Security. The lipid variables were established in accordance with each guide: High Triglycerides (TG) (TG ≥150 mg/dl) and high total cholesterol (CT ≥ 200 mg/dl), High LDL-C cholesterol (LDL-C ≥ 130 mg/dl) and decreased HDL-C (HDL-C ≤ 40 mg/dl). Results: 71 % of the patients presented decreased HDL-C, 41 % hypertriacilglyceridemia, 35.9 % hypercholesterolemia and 33.3 % high LDL-C. According to the US Preventive Services Task Force guide 9 patients with dyslipidemia grade B were identified by the National Cholesterol Education Program Adult Treatment Panel III (NCEP ATP III) guide, and 99 patients in 4 groups of dyslipidemia and 5 patients without lipid disorders were identified according to the clinical practice guidelines of the Mexican Institute of Social Security. Conclusions: It is observed a high frequency of patients with lipid disorders mainly by decreased HDL-C and there is evidence of the advantages of the clinical practice guidelines in the primary level of health care.

RESUMEN Introducción: Las dislipidemias constituyen un factor de riesgo para el desarrollo de las enfermedades cardiovasculares, las que se consideran la principal causa de muerte a nivel mundial. Objetivos: Evaluar los trastornos lipídicos en pacientes hispanoamericanos en el primer nivel de atención sanitaria de Lima. Métodos: En el periodo de 2015-2016 se realizó una investigación prospectiva en 117 pacientes del Servicio de Laboratorio Clínico, Policlínico La Fe en Lima. Se tuvieron en cuenta las guías National Cholesterol Education Program Adult Treatment Panel III (NCEP ATP III), US Preventive Services Task Force y la guía de práctica clínica del Instituto Mexicano del Seguro Social. De acuerdo con cada guía se establecieron las variables lipídicas: triglicéridos (TG) elevados (TG ≥ 150 mg/dl) y colesterol total elevado (CT ≥ 200 mg/dl), colesterol LDL-C elevado (LDL-C ≥ 130 mg/dl) y HDL-C disminuido (HDL-C ≤ 40 mg/dl). Resultados: El 71 % de los pacientes presentaron HDL-C disminuido, el 41 % hipertriacilgliceridemia, el 35,9 % hipercolesterolemia y el 33,3 % LDL-C elevado. Según la guía US Preventive Services Task Force se determinaron 9 pacientes con dislipidemia grado B, por la guía National Cholesterol Education Program Adult Treatment Panel III (NCEP ATP III), 99 pacientes en 4 grupos de dislipidemia y 5 pacientes sin trastornos lipídicos según la guía de práctica clínica del Instituto Mexicano del Seguro Social. Conclusiones: Se observa una alta frecuencia de pacientes con trastornos lípidos principalmente por HDL-C disminuido y se evidencia las ventajas de la guía de práctica clínica en el primer nivel de atención sanitaria.

ABSTRACT Introduction: Dyslipidemia is a risk factor for the development of cardiovascular diseases, which are considered to be the main cause of death at the global level. Objectives: To assess lipid disorders in Hispanic Americans patients in the primary health care level in Lima. Methods: In the period 2015-2016 a prospective research was made in 117 patients of the Service of Clinical Laboratory, La Fe Policlinic in Lima. There were used the guides named National Cholesterol Education Program Adult Treatment Panel III (NCEP ATP III), US Preventive Services Task Force, and the clinical practice guidelines of the Mexican Institute of Social Security. The lipid variables were established in accordance with each guide: High Triglycerides (TG) (TG ≥150 mg/dl) and high total cholesterol (CT ≥ 200 mg/dl), High LDL-C cholesterol (LDL-C ≥ 130 mg/dl) and decreased HDL-C (HDL-C ≤ 40 mg/dl). Results: 71 % of the patients presented decreased HDL-C, 41 % hypertriacilglyceridemia, 35.9 % hypercholesterolemia and 33.3 % high LDL-C. ABSTRACT The waste used cooking oil (WUCO) is one of the most serious factors of water pollution because it is able to create a layer that hinders the passage of oxygen and is difficult to remove. In order to demonstrate the capacity and efficiency of in vitro biodegradation of WUCO by lipolytic fungi, a study was conducted with three genera of fungi from industrial effluents of palm oil (Penicilium, Aspergillus and Amorphoteca). The efficiency of experimental biodegradation was evaluated in a container with equal parts (300 mL) of WUCO and tap water, where they were inoculated: ± 104 conidia/mL for individual experiments, ± 503 conidia/mL for a dual evaluation (two genera) and ± 33 300 conidia/mL for an evaluation of the three fungi. For each experiment three replicates were performed and all were incubated at 27 and 30 ºC for up to eight days. Lipolytic activity levels among genera of fungi were significant during the first days of the experiment (p < 0.05), normalizing to eighth day (p = 0.573). The combination of the three fungi improved the efficiency of biodegradation (p < 0.001), comparable with the eight-day evaluation of individual experiments (p = 0.012). All experiments at 27 ºC had better efficiency (p < 0.001). The three lipolytic fungi have high capacity and efficiency of biodegradation of WUCO in vitro after eight days of incubation at 27 ºC.

RESUMEN Los residuos de aceites usados de cocina (RAUC) son uno de los más graves factores de contaminación del agua porque son capaces de crear una capa que dificulta el paso de oxígeno y es difícilmente eliminable. Con el objetivo de demostrar la capacidad y eficiencia de biodegradación in vitro de RAUC mediante hongos lipolíticos se realizó un estudio con tres géneros hongos provenientes de efluentes industriales de aceite de palma (Penicilium, Aspergillus y Amorphoteca). Se evaluó la eficiencia de biodegradación en un contenedor con partes iguales (300 mL) de RAUC y agua de grifo donde se inocularon ± 104 conidios/mL en los experimentos con un género, ± 503 conidios/mL con dos géneros y ± 33 300 conidios/mL con los tres géneros. Para cada experimento se realizaron tres repeticiones y todos fueron incubados a 27 y 30 ºC hasta por ocho días. Los niveles de actividad lipolítica entre géneros de hongos difirieron durante los primeros días del experimento (p < 0.05) normalizándose hacia el octavo día (p = 0.573). La combinación de tres géneros mejoró la eficiencia de biodegradación (p < 0.001), comparable con la evaluación a los 8 días de los experimentos individuales (p = 0.012). Todos los experimentos a 27 ºC tuvieron mejor eficiencia que los de 30 ºC (p < 0.001). Los tres hongos lipolíticos tienen alta capacidad y eficiencia de biodegradación de RAUC in vitro luego de ocho días de incubación a 27 ºC.
