Changes in the rest-activity rhythm in migraine patients are associated with anxiety symptoms

Objective: To characterize rest-activity rhythm in chronic migraine (CM) and to investigate the relationship between this rhythm and depressive and anxiety symptoms in patients with CM. Methods: This was a study of adults aged 20 to 40 years. The rest-activity rhythm of patients with CM (n=23) and non-headache controls (NH, n=23) was assessed by actigraphy for 15 days, and they completed the following assessments: Visual Analogue Scale for pain intensity; Headache Diary; Headache Impact Test-6; Morningness-Eveningness Questionnaire; Pittsburgh Sleep Quality Index; Epworth Sleepiness Scale; Beck Depression Inventory; and State-Trait Anxiety Inventory. Results: Patients with CM showed less activity over 24 hours and more fragmented sleep. Reduced interdaily stability of the rest-activity rhythm was observed, with less robustness of this rhythm in the CM group. Multiple linear regressions revealed a significant association between the rest-activity rhythm and trait anxiety variables in patients with CM, specifically regarding the relative amplitude of the cycle, activity throughout 24 hours and during sleep, and robustness of the rest-activity rhythm. Conclusions: Our findings provide evidence that the robustness of the rest-activity rhythm, activity throughout 24 hours, and sleep fragmentation are associated with trait anxiety in patients with CM. Valores ecobiométricos e índice de resistividade da artéria oftálmica externa em catetos (Tayassu tajacu, Linnaeus, 1758)

RESUMO Foram avaliados ultrassonograficamente, pela via transpalpebral, 28 bulbos oculares de 14 catetos adultos, através de técnica padronizada pelo operador. Adicionalmente foi realizado o estudo hemodinâmico da artéria oftálmica externa pela técnica de Doppler colorido. Os dados coletados foram analisados estatisticamente pelo programa Bioestat 5.0 for Windows, adotando-se 5% de significância. Com a metodologia empregada, obtiveram-se os seguintes valores para os globos oculares direito e esquerdo, respectivamente D1: 1,72 ± 0,29mm e 1,76 ± 0,40mm; D2: 9,95 ± 1,08mm e 10,6 ± 0,99mm; D3: 7,42 ± 0,93mm e 7,45 ± 0,72mm e D4: 17,6 ± 0,78mm e 17,8 ± 0,59mm. Os valores médios do índice de resistividade da artéria oftálmica externa foram 0,435 ± 0,02 e 0,448 ± 0,02 (globos oculares direito e esquerdo, respectivamente). Não houve diferença estatística quanto aos antímeros oculares em nenhum dos parâmetros estudados. Conclui-se que a ecobiometria ocular e a Dopplerfluxometria da artéria oftálmica na espécie Tayassu tajacu é executável e reprodutível, desde que haja domínio do examinador em relação à anatomia e à técnica adequada. Os valores inferidos neste estudo servem de referência para médicos veterinários no diagnóstico de doenças oculares.
ABSTRACT Twenty-eight ocular bulbs of fourteen adult catheters were evaluated through ultrasound with the transpalpebral approach, using a standardized technique by the operator. Additionally, the hemodynamic study of the external ophthalmic artery was performed using the color Doppler technique. The collected data were statistically analyzed by the Bioestat 5.0 for Windows program, adopting 5% of significance. With the methodology employed, the following values were obtained for the right and left eyeballs, respectively D1: 1.72 ± 0.29mm and 1.76 ± 0.40mm; D2: 9.95 ± 1.08mm and 10.6 ± 0.99mm; D3: 7.42 ± 0.93mm and 7.45 ± 0.72mm and D4: 17.6 ± 0.78mm and 17.8 ± 0.59mm. The mean resistivity index values of the external ophthalmic artery were 0.435 ± 0.02 and 0.448 ± 0.02 (right and left eyeball respectively). There was no statistical difference regarding ocular antimers in any of the studied parameters. We concluded that ocular echobiometry and ophthalmic artery Doppler flowmetry in Tayassu tajacu species is executable and reproducible, provided there is an examiner's domain regarding the anatomy and proper technique. The values inferred in this study serve as a reference for veterinarians in the diagnosis of eye diseases.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Estudo radiográfico do posicionamento da falange distal e suas possíveis correlações com obesidade em equinos de patrulhamento da Polícia Militar do Estado de Minas Gerais

Com o objetivo de se verificarem possíveis correlações entre a falange distal, o estojo córneo e a obesidade, 55 equinos de patrulhamento provenientes do Regimento de Cavalaria Alferes Tiradentes do Estado de Minas Gerais - RCAT tiveram seus dígitos torácicos radiografados. Posteriormente, os animais foram classificados quanto ao acúmulo de tecido adiposo regional por métodos subjetivos (escore corporal e escore de pescoço) e objetivos (índice de massa corporal, circunferência do pescoço e determinação de espessura do tecido adiposo por meio de ultrassonografia na inserção da cauda e do abdômen). Na população estudada, o índice de massa corporal evidenciou que 52,6% dos equinos se encontravam acima do peso. A circunferência do pescoço a 50% de seu comprimento dorsal e a medida ultrassonográfica na inserção da cauda se mostraram os melhores métodos objetivos para quantificar o acúmulo de tecido adiposo. Foram identificadas particularidades radiográficas características da população estudada que diferem de parâmetros internacionais. Observaram-se correlações significantes entre sinais de obesidade e medidas radiográficas indicadoras de laminite, sugerindo associação entre essas duas condições na população estudada.
With the aim to verify possible correlations between the spatial relationship between the distal phalanx and the hoof and signs of obesity, 55 horses from the Cavalry Regiment Alferes Tiradentes of Minas Gerais State Police - RCAT had x-rays taken from their thoracic hoofs. After that, horses were classified based on their subjective (body condition and neck scores) and objective (body mass index, neck circumferences, ultrasound measurement of fat tissue of the tail and on abdomen) methods for adiposity. Within the population, the body mass index showed that 52.6% of horses were overweight. Neck circumference at 50% of its dorsal length and ultrasound measurement of fat thickness on the insertion of the tail were the best objective methods. We identified specific radiographic characteristics of the population that were different from international parameters. Significant correlations among signs of obesity and measurements that indicate lamintis were found, suggesting association between these conditions in this horse population.

Fetal hemoglobin (HbF), encoded by the HBG2 and HBG1 genes, is the best-known genetic modulator of sickle cell anemia, varying dramatically in concentration in the blood of these patients. This variation is partially associated with polymorphisms located in the promoter region of the HBG2 and HBG1 genes. In order to explore known and unknown polymorphisms in these genes, the sequences of their promoter regions were screened in sickle cell anemia patients and correlated with both their HbF levels and their βS-globin haplotypes. Additionally, the sequences were compared with genes from 2 healthy groups, a reference one (N = 104) and an Afro-descendant one (N = 98), to identify polymorphisms linked to the ethnic background.The reference group was composed by healthy individuals from the general population. Four polymorphisms were identified in the promoter region of HBG2 and 8 in the promoter region of HBG1 among the studied groups. Four novel single nucleotide polymorphisms (SNP) located at positions -324, -317, -309 and -307 were identified in the reference group. A deletion located between -396 and -391 in the HBG2 promoter region and the SNP -271 C→T in the HBG1 promoter region were associated with the Central African Republic βS-globin haplotype. In contrast, the -369 C→G and 309 A→G SNPs in the HBG2 promoter region were correlated to the Benin haplotype. The polymorphisms -396_-391 del HBG2, -369 SNP HBG2 and -271 SNP HBG1 correlated with HbF levels. Hence, we suggest an important role of HBG2 and HBG1 gene polymorphisms on the HbF synthesis.

Although Helicobacter heilmannii infection is less common than H. pylori infection in humans, it is considered to be of medical importance because of its association with gastritis, gastric ulcer, carcinoma, and mucosa-associated lymphoid tissue lymphoma of the stomach. However, there have been no studies evaluating the role of the Th cell response in H. heilmannii gastric infection. We evaluated the participation of pro-inflammatory and anti-inflammatory cytokines, IFN-gamma and IL-4, in H. heilmannii gastric infection in genetically IFN-gamma- or IL-4-deficient mice. The serum IFN-gamma and IL-4 concentrations were determined by ELISA. The gastric polymorphonuclear infiltrate was higher (P = 0.007) in H. heilmannii-positive than in H. heilmannii-negative wild-type (WT) C57BL/6 mice, whereas no significant inflammation was demonstrable in the stomach of H. heilmannii-positive IFN-gamma-/- C57BL/6 mice. The degree of gastric inflammatory cells, especially in oxyntic mucosa, was also higher (P = 0.007) in infected IL-4-/- than in WT BALB/c mice. Serum IFN-gamma levels were significantly higher in IL-4-/- than in WT BALB/c mice, independently of H. heilmannii-positive or -negative status. Although no difference in serum IFN-gamma levels was seen between H. heilmannii-positive (11.3 ± 3.07 pg/mL, mean ± SD) and -negative (11.07 ± 3.5 pg/mL) WT BALB/c mice, in the group of IL-4-/- animals, the serum concentration of IFN-g was significantly higher in the infected ones (38.16 ± 10.5 pg/mL, P = 0.04). In contrast, serum IL-4 levels were significantly decreased in H. heilmannii-positive (N = 10) WT BALB/c animals compared to the negative (N = 10) animals. In conclusion, H. heilmannii infection induces a predominantly Th1 immune response, with IFN-gamma playing a central role in gastric inflammation.

Hepatic changes in mice chronically infected with Helicobacter trogontum

We infected NIH germ-free female mice with Helicobacter trogontum, a recently described intestinal bacterium of rats, in order to study the lesions it induced in the liver of this host. Fifteen mice were challenged with a single dose of H. trogontum (test group) and killed 6, 12 and 18 months after inoculation (5 animals/group). Nine animals were challenged with 0.85% saline alone (control group) and killed at the same times. Fragments from the liver, cecum and colon were obtained for microbiologic and histologic examination. Stool samples were also collected. H. trogontum was detected in the cecum, colon and/or stool samples of all test mice. As expected, the bacterium was not isolated from any specimen obtained from the control animals. On the other hand, although we could not cultivate the bacterium from the liver, 13 test animals (86.7%) presented histological changes in this organ. The 6-month group presented infiltration of mononuclear and polymorphonuclear cells in the hepatic parenchyma and the two other groups presented foci of mononuclear cells. The results suggest that H. trogontum can elicit a hepatic inflammatory response in mice since the only difference between control and test animals was the presence of H. trogontum in the latter. This result, together with the growing number of related reports in the literature, reinforces the possible role of Helicobacter infection in the pathogenesis of hepatobiliary diseases.

We evaluated the accuracy of a 2nd generation ELISA to detect Helicobacter pylori infection in adults from a developing country in view of variations in sensitivity and specificity reported for different populations. We studied 97 non-consecutive patients who underwent endoscopy for evaluation of dispeptic symptoms. The presence of H. pylori was determined in antral biopsy specimens by culture, by the preformed urease test and in carbolfuchsin-stained smears. Patients were considered to be H. pylori positive if at least two of the three tests presented a positive result or if the culture was positive, and negative if the three tests were negative. Sixty-five adults (31 with peptic ulcer) were H. pylori positive and 32 adults were H. pylori negative. Antibodies were detected by Cobas Core anti-H. pylori EIA in 62 of 65 H. pylori-positive adults and in none of the negative adults. The sensitivity, specificity and positive and negative predictive values of the test were 95.4, 100, 100 and 91.4%, respectively. The Cobas Core anti-H. pylori EIA presented high sensitivity and specificity when employed for a population in Brazil, permitting the use of the test both to confirm the clinical diagnosis and to perform epidemiologic surveys.

Polialveolose: patogênese do enfisema lobar congênito?

INTRODUÇÃO: O enfisema lobar congênito é doença rara, cuja patogênese permanece desconhecida. O interesse acadêmico reside na falta de uniformidade dos achados anatomopatológicos. Em cerca de 50%, não se consegue definir a lesão anatomopatológica. A teoria mais recente relaciona esta doença com um número aumentado de alvéolos dentro de cada ácino pulmonar. OBJETIVO: O objetivo deste trabalho é efetuar uma revisão do estudo anatomopatológico original de 12 crianças com diagnóstico de enfisema lobar congênito, procurando, mediante método quantitativo, pesquisar a presença do aumento do número de alvéolos no ácino pulmonar. MÉTODO: Os autores realizaram um estudo caso-controle pareado pela idade. O número de alvéolos em cada ácino pulmonar foi pesquisado pela técnica da contagem radial de alvéolos. Para comparar as medidas obtidas nos casos e seus respectivos controles, utilizaram a prova U de Mann-Whitney. Regressão linear avaliou a influência da idade sobre as medidas. RESULTADOS: Encontraram um número elevado de alvéolos nos portadores de enfisema lobar congênito operados com idade inferior a 3 anos, um número semelhante entre as idades de 3 e 7 anos e um número diminuído a partir dessa idade. Na criança normal, ocorre um aumento no número de alvéolos com o aumento da idade. No enfisema lobar congênito, esse número se mantém praticamente constante, independente da idade. CONCLUSÃO: Os achados permitiram concluir que: o lobo polialveolar pode ser diagnosticado por um método simples e prático como a contagem radial de alvéolos, colaborando para diminuir a freqüência de casos de etiologia desconhecida, um número de alvéolos maior que o esperado para idade encontra-se associado ao enfisema lobar congênito no paciente (operado) antes dos 3 anos de idade e que, ao contrário do que ocorre no pulmão normal, no lobo acometido por enfisema lobar congênito não ocorre aumento no número de alvéolos com o aumento na idade.
BACKGROUND: Congenital lobar emphysema is an unusual condition and its pathogenesis remains unknown. The variety of findings in pathology studies of the ressected specimens led to increasing academic interest. About 50 per cent of the cases have no definitive diagnosis in pathology. The most recent theory proposes an increased number of alveoli within each acinus (polyalveolar lobe). PURPOSE: The aim of this paper is to report the morphometric measures of surgical specimens of 12 patients with congenital lobar emphysema, using the Emery and Mithal technique (radial alveolar count). METHODS: We made a case-control study, classifying the cases by age. Mann-Whitney´s U test and linear regression techniques were used in data analysis: Mann-Whitney´s U in comparing the cases and respective controls and linear regression to evaluate the influence of age in the measures found. RESULTS: The results revealed a significantly higher radial alveolar count than expected for the age group under 3 years; no difference was observed in the age group between 3 and 7 years and in children older than 7, the radial alveolar count was lower than expected. The normal development of the lung consists in an increasing number of alveoli increase from birth until adulthood, but this number remains constant, independent of age in congenital lobar emphysema. CONCLUSIONS: Such findings allow us to conclude that polyalveolar lobe can and must be diagnosed by a simple and practical method, such as the radial alveolar count, which decreases the incidence of the unknown etiology. The findings of an increased number of alveoli on patients younger than 3 is related to congenital lobar emphysema, since the number of alveoli does not increase in congenital lobar emphysema, just the opposite to what one would expect in the normal development of the lung.

In the present study we compared the inoculation of swine gastric mucus into the stomach of mice, the urease test and carbolfuchsin-stained smears for the diagnosis of the infection with "Gastrospirillum suis" ("Helicobacter heilmannii" type 1), an uncultivated tightly spiralled gastric bacterium. Fragments obtained from the antral and oxyntic mucosa of the stomach of 50 slaughtered pigs were used for urease test, for carbolfuchsin-stained smears and for obtaining scrapings of mucus for mouse inoculation. The mice were killed by spinal dislocation 10 days after inoculation and fragments of the antral and oxyntic mucosa were used for spiral bacterium identification (urease test and carbolfuchsin-stained smears). Among the methods employed for the diagnosis of "H. heilmannii" infection, the inoculation of gastric mucus into the stomach of mice was the most sensitive and demonstrated bacterial positivity in 31 (62.0%) swine. Direct examination showed tightly spiralled bacteria in the gastric mucosa of only 4 (8.0%) of the 50 pigs studied. Among them, 3 (6.0%) presented a positive preformed urease test. Spiral bacteria were not seen in the gastric mucosa of any control mice. These results show that the use of the mouse inoculation method improved the detection of "H. heilmannii" in swine
