Resumo Distantes temporalmente da declaração da emergência em saúde pública de importância internacional (ESPII) e emergência em saúde pública de importância nacional (ESPIN) provocada pela epidemia de zika, no ano de 2015, enunciamos a herança da emergência humanitária. Com base em uma pesquisa qualitativa, por meio de grupos focais realizados com profissionais de saúde e familiares das crianças afetadas epidemia de zika em Natal e Feira de Santana, buscamos discutir esse fenômeno de saúde pública pelas lentes da Antropologia do Estado. Concluímos que o não reconhecimento do Estado como uma instância encarnada no cotidiano das práticas por parte dos seus agentes locais leva à reprodução de práticas discriminatórias esvaziadas de sentido político e do reconhecimento de moralidades que permeiam as ausências nas ações de promoção de saúde e estratégias de reconhecimento e busca por estratégias para a garantia do direito à saúde.

Abstract Temporarily distant from the declaration of the Public Health Emergency of International Importance (ESPII) and Public Health Emergency of National Importance (ESPIN) caused by the Zika epidemic, in 2015, we enunciate the legacy of the humanitarian emergency. Based on qualitative research, through focus groups with health professionals and families of children affected by the Zika epidemic in Natal and Feira de Santana, we seek to discuss this public health phenomenon through the lens of State Anthropology. We conclude that the non-recognition of the State as an instance embodied in the daily practices of its local agents leads to the reproduction of discriminatory practices emptied of political sense and the recognition of moralities that permeate the absences in health promotion actions and recognition strategies, and search for methods to guarantee the right to health.

Resumo O artigo trata das impressões compartilhadas de pesquisadores e sua experiência no trabalho de campo no Nordeste do Brasil, no âmbito do projeto Promoção da saúde no contexto da epidemia de zika: atores e cenários nos processos de tomada de decisão (FIOCRUZ/ZIKAlliance). Aqui procuramos compartilhar impressões de campo sobre uma pesquisa acerca da epidemia de Zika vírus, em dois municípios situados em diferentes localidades do Nordeste. Evidenciamos a importância do caderno de campo na pesquisa qualitativa e tecemos reflexões acerca dos resultados das entrevistas realizadas com mulheres em idade reprodutiva, gestantes, profissionais de saúde, mães e pais de crianças acometidas por microcefalia e trabalhadores autônomos. A partir dos depoimentos, foi possível observar aproximações, identificações e estranhamentos dos sujeitos de pesquisa com os pesquisadores, constituindo importante termômetro para nossa percepção do campo e das possibilidades de produção de conhecimento científico sobre a epidemia do Zika no Nordeste do Brasil.

ABSTRACT: This trial evaluated the production performance, gastrointestinal nematode infection (GIN), and selected blood analytes, in Corriedale lambs during the finishing phase in an integrated crop-livestock system (ICLS) with 2 stocking rates. Fifty-two Corriedale lambs were divided into 4 groups: a semi confinement system with a low stocking rate (T1) for 70 days; a semi confinement system with a high stocking rate (T2) for 70 days; confinement for 28 days, followed by semi confinement with a low stocking rate (T3) for 42 days; and confinement for 28 days, followed by semi confinement with a high stocking rate (T4) for 42 days. Body weight, dry matter intake, blood collection, and faecal analysis of nematode eggs were performed for each animal. The stocking rates did not influence body weight gain for the semiconfined lambs (T1 and T2) or for the confined/semiconfined lambs (T3 and T4). All treatments had lower EPG counts at the end of the experimental period. The nematode genera identified during the experiment trial were Haemonchus and Trichostrongylus. There were no significant differences (P > 0.05) for packed cell volume, total plasma protein, and eosinophil means between treatments. These results suggested that the confinement followed by semi confinement in an ICLS, was an option for lamb production during the finishing phase which could be useful in reducing GIN without affecting the production performance. RESUMO: O objetivo deste experimento foi avaliar o desempenho produtivo, infecção por nematódeos gastrointestinais (GIN) e variáveis sanguíneas selecionadas, em cordeiras Corriedale durante a fase de terminação em sistema integrado lavoura-pecuária (ICLS) com duas taxas de lotação. O experimento utilizou 52 cordeiras Corriedale divididas em quatro grupos: sistema de semiconfinamento com baixa taxa de lotação por área (T1) por 70 dias; sistema de semiconfinamento com alta lotação por área (T2) por 70 dias; confinamento por 28 dias, seguido de semiconfinamento com baixa lotação por área (T3) por 42 dias; e confinamento por 28 dias, seguido de semiconfinamento com alta lotação por área (T4) por 42 dias. Peso corporal, ingestão de matéria seca, coleta de sangue e análise fecal para ovos de nematódeos foram avaliados para cada animal. Abstract Objectives The COVID-19 pandemic has had a significant global impact since its declaration in March 2020. The COVID-19 pandemic has disproportionately impacted cancer patients, particularly those with breast cancer. This study aims to analyze the effects of the pandemic on women diagnosed with breast cancer recurrence. Methods A cohort study was conducted at a tertiary public hospital in São Paulo State, Brazil. Data were collected from electronic records. Patients diagnosed with breast cancer and experiencing recurrence between January 2011 and March 2022 were included. Survival analysis was performed using the Kaplan-Meier estimator and Cox regression. Results The study included 187 patients, 45 in the pandemic group (recurrence after March 23, 2020) and 142 in the pre-pandemic group. Distant recurrences were more frequent in both groups (pre-pandemic: 62.7 %, pandemic: 75.5 %). Compared to the pre-pandemic group (1.8 years), the pandemic group experienced a longer mean time to recurrence detection (2.9 years) and significantly decreased median survival (9 months vs. 22 months). The Cox regression analysis confirmed an increased risk of death for women diagnosed with breast cancer recurrence during the pandemic period (HR = 1.92, 95 % CI 1.19‒3.12). Conclusion The present study is among the first to investigate the pandemic's specific effects on breast cancer recurrence, revealing concerning delays in detection and a decrease in survival rates. Prompt diagnosis, timely treatment initiation, and comprehensive support are crucial during public health crises. These findings urge healthcare systems to prioritize tailored care for breast cancer patients during pandemics. Abstract Toxoplasma gondii and Neospora caninum are two closely related protozoans that infect a wide range of animals, including birds. However, the occurrence of N. caninum and T. gondii in seabirds is unknown. Therefore, this study aimed to determine the presence of T. gondii and N. caninum DNA in tissue samples of seabirds. Tissue samples of the pectoral muscles, heart, and brain were collected from 47 birds along the coastline of Santa Catarina State, SC, Brazil. The DNA was extracted from the tissues and screened using nested-PCR (nPCR) targeting internal transcribed spacer 1 (ITS1). T. gondii DNA was detected in tissues from seven seabirds (7/47, 14.8%), kelp gull (Larus dominicanus) (5/21), and Manx shearwater (Puffinus puffinus) (2/8). N. caninum DNA was detected in tissues of nine seabirds (9/47, 19.1%), the kelp gull (L. dominicanus) (4/21), Manx shearwater (P. puffinus) (2/8), neotropic cormorant (Phalacrocorax brasilianus) (1/4), brown booby (Sula leucogaster) (1/5), and white-chinned petrel (Procellaria aequinoctialis) (1/1); however, no co-infection was observed. Resumo Toxoplasma gondii e Neospora caninum são dois protozoários intimamente relacionados que infectam uma ampla gama de animais, incluindo-se aves. Entretanto, a ocorrência de N. caninum e T. gondii em aves marinhas é desconhecida. Portanto, este estudo objetivou determinar a presença de DNA de T. gondii e N. caninum em amostras de tecidos, coletadas de 47 aves marinhas. Amostras de tecidos dos músculos peitorais, coração e cérebro foram coletadas de 47 aves marinhas mortas ao longo da costa do estado de Santa Catarina, SC, Brasil. O DNA foi extraído dos tecidos e testado, usando-se a reação em cadeia da polimerase nested (nPCR), tendo em vista o espaçador interno transcrito 1 (ITS1). ABSTRACT Objective To investigate the relationship between the levels of adipokines and other endocrine biomarkers and patient outcomes in hospitalized patients with COVID-19. Methods In a prospective study that included 213 subjects with COVID-19 admitted to the intensive care unit, we measured the levels of cortisol, C-peptide, glucagon-like peptide-1, insulin, peptide YY, ghrelin, leptin, and resistin.; their contributions to patient clustering, disease severity, and predicting in-hospital mortality were analyzed. Results Cortisol, resistin, leptin, insulin, and ghrelin levels significantly differed between severity groups, as defined by the World Health Organization severity scale. Additionally, lower ghrelin and higher cortisol levels were associated with mortality. Adding biomarkers to the clinical predictors of mortality significantly improved accuracy in determining prognosis. Phenotyping of subjects based on plasma biomarker levels yielded two different phenotypes that were associated with disease severity, but not mortality. Conclusion As a single biomarker, only cortisol was independently associated with mortality; however, metabolic biomarkers could improve mortality prediction when added to clinical parameters. RESUMO Objetivo Investigar a relação entre os níveis de adipocinas e outros biomarcadores endócrinos e os desfechos em pacientes hospitalizados com COVID-19. Métodos Estudo prospectivo que incluiu 213 indivíduos com COVID-19 internados na unidade de terapia intensiva, em que foram medidos os níveis de cortisol, peptídeo-C, peptídeo-1 semelhante ao glucagon, insulina, peptídeo YY, grelina, leptina e resistina; analisamos suas contribuições para o agrupamento de pacientes, gravidade da doença e previsão de mortalidade hospitalar. Resultados Os níveis de cortisol, resistina, leptina, insulina e grelina diferiram significativamente entre os grupos por gravidade, conforme definido pela escala de gravidade da Organização Mundial da Saúde. Além disso, níveis mais baixos de grelina e mais altos de cortisol foram associados à mortalidade. A adição de biomarcadores aos preditores clínicos de mortalidade melhorou significativamente a precisão na determinação do prognóstico. Abstract Objective Neoadjuvant chemotherapy (NACT) has become the standard of care for patients with triple-negative breast cancer (TNBC) with tumors > 1 cm or positive axillary nodes. Pathologic complete response (pCR) has been used as an endpoint to select patients for treatment scaling. This study aimed to examine the benefit of adding adjuvant capecitabine for TNBC patients who did not achieve pCR after standard NACT in a real-world scenario. Methods This retrospective cohort study included all patients with TNBC who underwent NACT between 2010 and 2020. Clinicopathological data were obtained from the patient records. Univariate and multivariate analyses were conducted at the 5 years follow-up period. Results We included 153 patients, more than half of whom had stage III (58.2%) and high-grade tumors (60.8%). The overall pCR rate was 34.6%, and 41% of the patients with residual disease received adjuvant capecitabine. Disease-specific survival (DSS) among the patients who achieved pCR was significantly higher (p<0.0001). Residual disease after NACT was associated with detrimental effects on DSS. In this cohort, we did not observe any survival benefit of adding adjuvant capecitabine for patients with TNBC subjected to NACT who did not achieve pCR (p=0.52). Conclusion Our study failed to demonstrate a survival benefit of extended capecitabine therapy in patients with TNBC with residual disease after NACT. More studies are warranted to better understand the indication of systemic treatment escalation in this scenario. Aim This study aimed to perform an in vitro comparative analysis of the antifungal activity of different calcium silicate-based endodontic sealers against three fungal species. Methods The antifungal properties of three calcium silicate-based sealers were tested: Bio-C Sealer, Cambiar a Sealer Plus BC, and MTA-Fillapex. Two commonly used sealers were used as controls: AH Plus and Endomethasone. An agar diffusion test was performed to analyze the antifungal activity of the sealers against Candida albicans, Candida glabrata, Candida tropicalis, and a mixed microbial culture medium. The results were analyzed using ANOVA (p <0.05). Results Endomethasone exhibited the highest inhibition against all strains examined, maintaining a consistent level of inhibition throughout 7 days. MTA-Fillapex demonstrated the best performance among the calcium silicate-based sealers for the three fungal species (p < 0.05), maintaining stable values over the 7 days, surpassing that of Endomethasone. Nevertheless, MTA-Fillapex only exhibited antimicrobial effect against the mixed culture for the first 24 hours, and no antimicrobial activity was observed at 48 hours, being surpassed by all tested sealers (p < 0.05). Here, we seek to share field impressions of research on the Zika Virus in two municipalities located in different locations in the Northeast. We highlight the importance of the field notebook in qualitative research and reflect on the results of interviews conducted with women of reproductive age, pregnant women, health professionals, mothers and fathers of children affected by microcephaly, and self-employed workers. From the testimonies, it was possible to observe similarities, identifications, and estrangements between the research subjects and the researchers, constituting a vital thermometer for our perception of the field and the possibilities of producing scientific knowledge about the Zika epidemic in Northeastern Brazil. Resumo Distantes temporalmente da declaração da emergência em saúde pública de importância internacional (ESPII) e emergência em saúde pública de importância nacional (ESPIN) provocada pela epidemia de zika, no ano de 2015, enunciamos a herança da emergência humanitária. Com base em uma pesquisa qualitativa, por meio de grupos focais realizados com profissionais de saúde e familiares das crianças afetadas epidemia de zika em Natal e Feira de Santana, buscamos discutir esse fenômeno de saúde pública pelas lentes da Antropologia do Estado. Abstract Temporarily distant from the declaration of the Public Health Emergency of International Importance (ESPII) and Public Health Emergency of National Importance (ESPIN) caused by the Zika epidemic, in 2015, we enunciate the legacy of the humanitarian emergency. Based on qualitative research, through focus groups with health professionals and families of children affected by the Zika epidemic in Natal and Feira de Santana, we seek to discuss this public health phenomenon through the lens of State Anthropology. We conclude that the non-recognition of the State as an instance embodied in the daily practices of its local agents leads to the reproduction of discriminatory practices emptied of political sense and the recognition of moralities that permeate the absences in health promotion actions and recognition strategies, and search for methods to guarantee the right to health. ABSTRACT Background: The established use of non-selective beta-blockers (NSBB) in the primary and secondary prevention of esophageal varices has recently been questioned in the subgroup of patients with diuretic-refractory ascites. Objective: Critically analyze the body of evidence on the topic in order to assist clinical decisions. Methods: A literature review was carried out in the Pubmed® and Scielo® databases. In total, 20 articles between 2010 and 2023 were read by independent researchers. Conclusion: It remains doubtful whether the use of NSBB is deleterious in cirrhotic patients with refractory ascites, however our literature review allows us to conclude that these drugs should not be proscribed in these patients. On the contrary, a doctor-patient decision based on tolerability and hemodynamic parameters certainly seems to be a safe conduct. RESUMO Contexto: O uso consagrado de betabloqueadores não seletivos (BBNS) na prevenção primária e secundária de varizes esofágicas foi recentemente questionado no subgrupo de pacientes com ascite refratária a diurético. Objetivo: Analisar criticamente o corpo de evidências sobre a temática a fim de auxiliar decisões clínicas. Métodos: Foi realizada uma revisão da literatura nos bancos de dados Pubmed® e Scielo®. No total, 20 artigos entre os anos 2010 e 2023 foram lidos por pesquisadores independentes. Conclusão: Ainda permanece duvidoso se o uso de BBNS é deletério nos cirróticos com ascite refratária, no entanto nossa revisão de literatura permite concluir que essas drogas não devem ser proscritas nesses pacientes. Abstract: Seed treatments with chemical phytosanitary products can compromise mobilization of seed reserves for the seedlings under water deficit conditions. The aim of this study was to evaluate the physiological quality, the initial seedling development, and the mobilization of reserves in soybean seeds treated with phytosanitary products under water deficit. The trial was set up in a completely randomized design in a 4 × 4 factorial arrangement: chemical treatments (control, Thiamethoxam, Fludioxonil + Metalaxyl-M, and Fipronil + Pyraclostrobin + Tiophanate-methyl) × osmotic potentials (0, -0.1, -0.2, and -0.3 MPa). We analyzed the following variables: germination; hypocotyl length, root length, and total seedling length; seedling length vigor index; cotyledon dry matter weight; seedling dry matter yield; seed reserve reduction; relative dry matter yield; seed reserve reduction rate; and conversion efficiency of seed reserves into seedling dry matter. Seed quality and seedling development decline under water deficit conditions. The treatment with Fipronil + Pyraclostrobin + Thiophanate-methyl results in lower phytotoxicity. The estimated cotyledon dry matter (CDM) weight is an indicator of phytotoxicity and water deficit. Mobilization of reserves is compromised in seeds treated with insecticides, reducing the capacity of conversion of reserves into dry matter. Resumo: O tratamento químico de sementes com produtos fitossanitários sob condições de déficit hídrico, podem comprometer a mobilização das reservas das sementes para as plântulas. Objetivou-se avaliar a qualidade fisiológica, o desenvolvimento inicial de plântulas e a mobilização de reservas em sementes de soja tratadas com produtos fitossanitários sob déficit hídrico. O ensaio foi instalado em delineamento inteiramente casualizado em esquema fatorial 4 x 4: tratamentos químicos (Controle; Thiametoxam; Fludioxonil + Metalaxil-M e Fipronil + Piraclostrobina + Tiofanato-metílico) x potenciais osmóticos (0; -0,1; -0,2 e -0,3 MPa). Abstract Background Time in therapeutic range (TTR) plays an important role in the effectiveness of anticoagulant therapy with vitamin K antagonists. Objective To identify factors associated with variation in TTR. Methods The study sites were anticoagulation clinics at two university hospitals in Minas Gerais, with a total of 1357 patients studied. TTR was calculated using the Rosendaal method. Binary logistic regressions were carried out with the study variables, followed by multiple regression with those that were significant for the model, adopting a significance level of 5%. Results The outpatient clinics presented different values in mean TTR; one corresponded to the expected percentage, while the other did not. Multiple regression identified the variable male sex as a protective factor in relation to TTR < 60% (odds ratio [OR]: 0.42; p = 0). RESUMO Objetivo: Estimar as prevalências e fatores associados com as hepatites A, B e C em mulheres trans e travestis em cinco regiões do Brasil. Métodos: Estudo transversal com mulheres trans e travestis em cinco capitais brasileiras (Campo Grande, Manaus, Porto Alegre, Salvador e São Paulo), entre dezembro/2019 e julho/2021. As amostras foram submetidas à detecção de marcadores das infecções pelos vírus das hepatites A (HAV), B (HBV) e C (HCV), utilizando-se testes rápidos e quimioluminescência. Amostras positivas foram submetidas à detecção de HBV-DNA e HCV-RNA por PCR em tempo real e genotipadas por sequenciamento de Sanger. Resultados: As análises de 1.317 amostras indicaram taxas de prevalências nas mulheres trans e travestis recrutadas de 69,1%, 24,4% e 1,5% para exposição ao HAV, HBV e HCV, respectivamente. Elevada taxa de suscetibilidade ao HBV (35,7%) e baixa prevalência do marcador vacinal (40,0%) foram observadas. Mostraram-se associadas à presença de anti-HAV: idade maior que 26 anos, autodeclarar-se preta-parda, ter apenas educação básica, história de encarceramento e uso de preservativo na última relação sexual com parceiro casual. Quanto à exposição ao HBV, foi associada a idade maior que 26 anos, cor da pele preto-parda, ter sido profissional do sexo e história de encarceramento. Idade maior de 37 anos, história de abuso sexual e consumo frequente de álcool foram associadas ao HCV. Conclusão: As maiores prevalências de HAV nessa população encontram-se nas regiões Norte e Nordeste. ABSTRACT Objective: To estimate the prevalence and factors associated with hepatitis A, B, and C in transgender women and travestis's networks, in 5 regions of Brazil. Methods: This cross-sectional study includedtransgender women and travestis in five Brazilian capitals (Campo Grande, Manaus, Porto Alegre, Salvador, and São Paulo), between December/2019 and July/2021. All samples were subjected to detection of serological markers of hepatitis virus A (HAV), B (HBV), and C (HCV) infections through rapid tests and chemiluminescent microparticle immunoassays. Positive samples in the screening tests were submitted to detect HBV DNA and HCV-RNA by real-time PCR and genotyped by Sanger sequencing. Results: Analysis of 1,317 samples showed network prevalence rates of 69.1%, 25.1%, and 1.5% for HAV, HBV, and HCV exposure, respectively. A high susceptibility rate to HBV infection (35.7%) and low prevalence of vaccine response markers (40%) were also observed. Age greater than 26 years, self-declared black/brown skin color, having only primary education, history of incarceration, and use of a condom in the last sexual intercourse with a casual partner were associated with total anti-HAV. Exposure to HBV was associated with age greater than 26 years, self-declared black/brown, history of being a sex worker, and incarceration. Age > 37 years, history of sexual abuse, and frequent alcohol consumption were associated with hepatitis C infection. Conclusion: The highest prevalence of HAV in this population was found in the North and Northeast regions, and the prevalence found was higher than that in the general population, suggesting greater vulnerability. The prevalence of HCV infection in our study was similar to that observed in the general population.
