ABSTRACT Background: Physical violence during pregnancy is a poorly evaluated sensitive public health problem that, additionally, is related to various perinatal complications. The objective is to estimate the proportion, geographic distribution and sociodemographic factors associated with physical violence during pregnancy between 2016 and 2018. Material and methods: Secondary baseline analysis of the Demographic and Family Health Survey (ENDES), which included respondents who answered whether or not they had experienced physical violence during pregnancy in the last 12 months. Results: The proportion of physical violence was 9,9% [95%CI:9,6-10,4%] during 2016, 9,2% [95%CI:8,8-9,6%] during 2017 and 8,6% [95%CI:8,3-8,9%] during 2018, The regions with the highest proportion were Puno, Arequipa, and Apurímac during the 3 years. Among the associated factors, the residue in rural areas (PR:0,49; p=0,011) and be "very rich" (PR:0,63; p=0,029) was protective; while they were at risk of not presenting studies (PR:1,87; p=0,014), the cohabiting marital status (PR:1,51; p=0,001), separated (PR:3,56; p<0,001), showing an age between 40 a 49 years (PR:1,79; p=0,012) and that partner drinks alcohol (RP:1,61; p<0,001). Conclusions: The proportion of violence in Peru has been decreasing in recent years, presenting higher indicators in the south of the country. The factors that predispose this phenomenon are the wealth index, educational level, marital status, and the age of the pregnant woman.

RESUMEN Introducción: La violencia física durante el embarazo es una problemática de salud pública sensible poco evaluada que, adicionalmente, se relaciona con diversas complicaciones perinatales. El objetivo es estimar la proporción, distribución geográfica y factores sociodemográficos asociados a la violencia física durante el embarazo entre el 2016 y 2018. Material y métodos: Análisis de base secundaria de la Encuesta Demográfica y de Salud Familiar (ENDES), donde se incluyeron a quienes respondieron si presentaron o no violencia física durante el embarazo en los último 12 meses. Resultados: La proporción de violencia física fue de 9,9% [IC95%:9,6-10,4%] durante el 2016, 9,2% [IC95%:8,8-9,6%] durante el 2017 y 8,6% [IC95%:8,3-8,9%] durante el 2018. Las regiones con mayor proporción fueron Puno, Apurímac y Cusco durante los 3 años. Entre los factores asociados, fueron protectores el residir en área rural (RP:0,49; p=0,011) y ser "muy rico" (RP:0,63; p=0,029); mientras que fueron de riesgo el no presentar estudios (RP:1,87; p=0,014), el estado civil conviviente (RP:1,51; p=0,001), separada (RP:3,56; p<0,001), mostrar una edad entre 40 a 49 años (RP:1,79; p=0,012) y que pareja consuma alcohol (RP:1,61; p<0,001). Conclusiones: La proporción de violencia en el Perú se ha ido reduciendo en los últimos años, presentando mayores indicadores en el sur del país. Los factores que predisponen este fenómeno son el índice de riqueza, grado de instrucción, estado civil y la edad de la gestante.

ABSTRACT Introduction: Domestic violence during pregnancy is associated with various maternal complications, abortion or stillbirth. However, in Latin America, its characterization and predisposing factors have not been widely addressed. Objective: To determine the prevalence and associated factors of domestic violence during pregnancy in a Peruvian specialized hospital. Material and methods: Cross-sectional analytical study involving 180 puerperal women without complications attended at the Instituto Nacional Materno Perinatal. Prevalence were estimated by intervals and associations using the Poisson regression test. Results: The prevalence of violence during pregnancy was 29.44% [95%CI: 22.72-36.17%]; the psychological type reached 27.78% [95%CI: 21.17-34.38%], the physical 8.33% [95%CI: 4.26-12.41%] and the sexual 3.33% [95%CI: 0.69-5.98%]. The main aggressor is his current partner, who violently throughout the pregnancy and eventual frequency, except for sexual violence that occurred weekly (83.33%). The factors that predispose the appearance of violence during pregnancy were the feeling of sadness (p=0.042; aRP:1.60) or failure in personal development (p=0.004; RP:1.98), being accused of infidelity (p=0.044; aPR:1.74), not receiving the expected affection (p=0.028, aPR:1.62) and considering that no one should be involved or aware of their problems (p<0.001; aPR:2.40). Conclusions: About a third of pregnant women suffer from some type of violence, the most prevalent being psychological and frequently carried out by their partner. Factors that promote its appearance have been identified.

RESUMEN Introducción: La violencia doméstica durante el embarazo se asocia a diversas complicaciones maternas, aborto o muerte fetal. Sin embargo, en Latinoamérica, su caracterización y factores predisponentes no ha sido muy abordado. Objetivo: Determinar la prevalencia y factores asociados de la violencia doméstica durante el embarazo en un hospital especializado peruano. Material y métodos: Estudio analítico transversal donde participaron 180 puérperas sin complicaciones atendidas en el Instituto Nacional Materno Perinatal. Las prevalencias fueron estimadas por intervalos y las asociaciones mediante la prueba de regresión de Poisson. Resultados: La prevalencia de violencia durante el embarazo fue del 29,44% [IC95%:22,72-36,17%]; la de tipo psicológica alcanzó el 27,78% [IC95%:21,17-34,38%], la física de 8,33% [IC95%:4,26-12,41%] y la sexual de 3,33% [IC95%:0,69-5,98%]. El principal agresor es su actual pareja, quien violenta durante todo el embarazo y de frecuencia eventual, con excepción de la violencia sexual que se presentó semanalmente (83,33%). Los factores que predisponen la aparición de violencia durante el embarazo fueron el presentar sentimiento de tristeza (p=0,042; RPa:1,60) o fracaso en su desarrollo personal (p=0,004; RPa:1,98), ser acusada de infidelidad (p=0,044; RPa:1,74), no recibir el afecto esperado (p=0,028, RPa:1,62) y considerar que nadie debe involucrarse ni enterarse de sus problemas (p<0,001; RPa:2,40). Conclusiones: Cerca de la tercera parte de las gestantes sufren de algún tipo de violencia, siendo la más prevalente la de tipo psicológica y realizada frecuentemente por su pareja. Se han identificado factores que propician su aparición.

Abstract OBJECTIVE: Evaluate the relationship between the pregestational body mass index and the gestational weight gain with the birth weight. MATERIALS AND METHODS: Cross-sectional analytical study. Through probabilistic sampling, 197 pregnant women and their neonates participated, whose delivery was attended at the San Juan de Lurigancho Hospital, 2018.The pregestational body mass index was classified as low weight, normal, overweight and obese; the gestational weight gain in insufficient, adequate and excessive; and the birth weight in grams. The variation of birth weight according to pregestational body mass index and weight gain were evaluated using the Kruskal-Wallis and ANOVA H tests, respectively. Similarly, a multiple linear regression model is required, a 95% confidence level is considered. RESULTS: The patients who started the pregnancy with obesity and low weight presented neonates with greater birth weight (3516.9 ± 480.7 g and 3564 ± 148.5 g, respectively), it was determined that there is not association between the pregestational body mass index and the birth weight (p = 0.753). However, the birth weight was higher in postpartum women with excessive weight gain (3582.9 ± 442.1 g) and lower in those with insufficient weight gain (3278.9 ± 447.9 g), a significant association was demonstrated between weight gain and birth weight (p < 0.001). CONCLUSION: The pregestational body mass index, independently, was not related to birth weight. However, together with the gestational weight gain, they were positively related to the weight of the newborn.

Resumen OBJETIVO: Evaluar la relación entre el índice de masa corporal previo al embarazo, la ganancia de peso y el peso del recién nacido. MATERIALES Y MÉTODOS: Estudio analítico y transversal mediante muestreo probabilístico efectuado en pacientes que concluyeron su embarazo en el Hospital San Juan de Lurigancho, Perú, durante el año 2018. Parámetros de estudio: índice de masa corporal previo al embarazo: bajo peso, normal, sobrepeso y obesidad; ganancia de peso: insuficiente, adecuada y excesiva; peso del recién nacido en gramos. La variación del peso al nacer según el índice de masa corporal previo al embarazo y la ganancia de peso se evaluaron con pruebas H de Kruskal-Wallis y ANOVA, respectivamente. También se utilizó un modelo de regresión lineal múltiple con IC95%. RESULTADOS: Se estudiaron 197 pacientes que iniciaron el embarazo siendo obesas o con bajo peso que tuvieron neonatos con mayor peso al nacer (3516.9 ± 480.7 g y 3564 ± 148.5 g, respectivamente). Se determinó que no existe asociación entre el índice de masa corporal previo al embarazo y el peso del recién nacido (p = 0.753). Sin embargo, el peso al nacer fue mayor en los hijos de madres con excesiva ganancia de peso (3582.9 ± 442.1 g) y menor en quienes tuvieron insuficiente ganancia de peso (3278.9 ± 447.9 g). Esto demuestra asociación significativa entre la ganancia de peso durante el embarazo y el peso al nacer (p < 0.001). CONCLUSIONES: El índice de masa corporal previo al embarazo, de forma independiente, no se relacionó con el peso al nacer. Sin embargo, junto con la ganancia de peso durante el embarazo sí se relacionó positivamente con el peso del recién nacido.

ABSTRACT The characteristics and trends of telehealth research in Latin America, especially clinical trials, have been little explored. The objective was to characterize the studies registered in clinicaltrials.gov about telehealth in Latin America. We performed an observational and descriptive study and evaluated the telehealth research in Latin America registered until 2018 in clinicaltrials.gov. The type of study, disease, country, initiative and type of institution were evaluated. We found 37 studies registered. The studies on telehealth were mainly clinical trials of parallel assignment type (81.1%). The most discussed diseases were: arterial hypertension (23.9%), diabetes (11.3%) and cardiovascular diseases (9.9%). The 64.9% of the works were local initiatives, being Brazil (29.3%) and Argentina (14.6%) the countries with the highest number of investigations. In conclusion, there is a low number of Latin American studies registered in clinicaltrials.gov, the studies focused mainly on chronic diseases and were developed by local initiative. We need more clinical trials on telehealth in the Latin American context to help to consolidate its development.

RESUMEN Las características y las tendencias de la investigación en telesalud en Latinoamérica, en especial los ensayos clínicos controlados aleatorizados, han sido poco explorados. El objetivo de este trabajo fue caracterizar los estudios registrados en clinicaltrials.gov sobre telesalud en Latinoamérica. Se hizo un estudio descriptivo donde se evaluaron las investigaciones sobre telesalud en Latinoamérica registradas hasta el año 2018 en clinicaltrials.gov. Se evaluó el tipo de estudio relacionado con enfermedad o tema, país, iniciativa y tipo de institución. Se localizaron 37 trabajos registrados. Los referidos a telesalud fueron principalmente ensayos controlados de tipo asignación paralela (81,1 %). Las enfermedades más abordadas fueron: hipertensión arterial (23,9 %), diabetes (11,3 %) y enfermedades cardiovasculares (9,9 %). El 64,9 % de los trabajos fueron de iniciativa local, en los que resultaron Brasil (29,3 %) y Argentina (14,6 %) los países con mayor número de investigaciones. En conclusión, existe una baja cantidad de estudios latinoamericanos registrados en clinicaltrials.gov, los cuales se enfocaron principalmente en enfermedades crónicas y fueron desarrollados por iniciativa local. Se requieren más ensayos clínicos controlados aleatorizados sobre telesalud en el contexto latinoamericano para ayudar a consolidar su desarrollo.

Introduction. The scientific production of Peru is low in relation to other countries; the impact of investment in such production at the national level is unknown. Objective. To evaluate the association between spending and investment in research and scientific production on health issues in peruvian universities during 2015. Methods. Observational study of secondary sources analysis of the I National Research and Development Census to Research Centers of the National Council of Science, Technology and Technological Innovation (Concytec), from where Peruvian universities and publications in the area of science were selected. health. The incidence of crude and adjusted relative risk (IRR) was evaluated to determine the degree of association. Results. There is a significant association between the scientific production and the remuneration of the research teachers (IRR: 0,998, p<0,01), remuneration to the support staff (IRR: 1,023, p<0,01), fees to external consultants (IRR : 0,991, p<0,05), purchase of other services (IRR: 1,003; p<0,01) and investment in buildings (IRR: 0,998; p<0,01). Conclusions. The remuneration of research faculty, support staff, external consultants, purchase of other services and investment in buildings, is associated with the highest scientific production in health issues in Peruvian universities.

Introducción. La producción científica del Perú es baja en relación a otros países; se desconoce el impacto de la inversión en dicha producción a nivel nacional. Objetivo. Evaluar la asociación entre el gasto e inversión en investigación y la producción científica en temas de salud en universidades peruanas durante el año 2015. Métodos. Estudio observacional de análisis de fuentes secundarias del I Censo Nacional de Investigación y Desarrollo a Centros de Investigación del Consejo Nacional de Ciencia, Tecnología e Innovación Tecnológica (Concytec), de donde se seleccionó a las universidades peruanas y las publicaciones en el área de ciencias de la salud. Se evaluó la incidencia de riesgo relativo (IRR) crudo y ajustado para determinar el grado de asociación. Resultados. Existe una asociación significativa entre la producción científica y la remuneración a los docentes investigadores (IRR: 0,998; p<0,01), remuneración al personal de apoyo (IRR: 1,023; p<0,01), honorarios a consultores externos (IRR: 0,991; p<0,05), compra de otros servicios (IRR: 1,003; p<0,01) y la inversión en edificios (IRR: 0,998; p<0,01). Conclusiones. La remuneración a docentes investigadores, al personal de apoyo, a consultores externos, compra de otros servicios e inversión en edificios, se asocia a la mayor producción científica en temas de salud en las universidades peruanas.

Abstract OBJECTIVE: Identify the predisposing factors of severe preeclampsia. MATERIALS AND METHODS: Observational, analytical study of cases and controls, carried out in patients attended, from January to December 2017, at the Hermilio Valdizan and Tingo María Contingency Hospitals of the Huánuco region. Factors associated with preeclampsia were considered: pre-conception, maternal and environmental factors. For the association between variables, a multivariate analysis was performed using the logistic regression model, calculating the odds ratio and 95% confidence intervals. RESULTS: We included 136 cases (severe preeclampsia) and 272 controls. Among the predisposing factors of severe preeclampsia were: preeclampsia (p <0.001, OR = 13.28), age of the mother over 35 years (p <0.001, OR = 3.93), obesity (p = 0.001, OR= 3.66), pregnancy with a new sexual partner (p <0.001; OR = 7.15) and twin pregnancy (p = 0.01; OR = 9.57). CONCLUSION: The history of preeclampsia, maternal age over 35 years, obesity, pregnancy of a new sexual partner and twin pregnancy are risk factors for severe preeclampsia.

Resumen OBJETIVO: Identificar los factores predisponentes de preeclampsia severa. MATERIALES Y MÉTODOS: Estudio retrospectivo, observacional, analítico, de casos y controles, efectuado en pacientes atendidas, de enero a diciembre de 2017, en los Hospitales de Contingencia Hermilio Valdizan y Tingo María de la región de Huánuco, Perú. Se consideraron factores asociados con preeclampsia: los previos a la concepción, maternos y ambientales. Para la asociación entre variables se realizó un análisis multivariado mediante el modelo de regresión logística, se calcularon la razón de momios y el IC95%. RESULTADOS: Se incluyeron 136 casos (preeclampsia severa) y 272 controles. Entre los factores predisponentes de preeclampsia severa se encontraron: antecedentes de preeclampsia (p < 0.001; RM = 13.27), edad de la madre mayor de 35 años (p < 0.001; RM = 3.93), obesidad (p = 0.001; RM = 3.66), embarazo con un nuevo compañero sexual (p < 0.001; RM = 7.15) y el embarazo gemelar (p = 0.01; RM = 9.57). CONCLUSIÓN: Los antecedentes de preeclampsia, la edad materna mayor de 35 años, la obesidad, el embarazo con un compañero sexual distinto y el embarazo gemelar fueron los factores de riesgo de preeclampsia severa en el grupo de estudio.

The university scientific production at the Latin-American level is still far below the international level. In spite of this, there are no publications describing this situation in Peru. Therefore, it was sought to determine the scientific production on health topics in indexed and non-indexed journals during the years 2014 and 2015 through an analysis of the secondary database of the National Council for Science, Technology and Technological Innovation (CONCYTEC). It was found that the median ranged between 1 and 2 articles published by university per year, with the production increasing between 10 and 98,16 % in national indexed and non-indexed journals, and decreasing in 42,86 % in international non-indexed journals

La producción científica universitaria a nivel latinoamericano aún se encuentra muy por debajo del nivel internacional; sin embargo, no existen artículos publicados que describan esta situación en el Perú. Por ello, mediante un análisis de la base secundaria del Consejo Nacional de Ciencia, Tecnología e Innovación Tecnológica (Concytec), se buscó determinar la producción científica en temas de salud en revistas indizadas y no indizadas durante los años 2014 y 2015. Se encontró que la mediana oscila entre 1 y 2 artículos publicados por universidad al año, con un un incremento de la producción entre el 10 al 98,16 % en las revistas indizadas y las no indizadas nacionales, mientras que la producción en revistas no indizadas internacionales tuvo un decremento del 42,86 %

Introduction. Prenatal care still does not cover the total of pregnant women, a situation that is repeated in Latin America and the few that evaluate the barriers that explain this phenomenon. The objective of study was to determine the factors associated with the access of prenatal care in its opportunity and coverage dimensions. Methods. Study of quantitative, observational, analytical approach carried out in 110 schools of the Hospitals of Huaral and Chancay, Peru. The factors (personal and sociodemographic) associated with access to prenatal care were evaluated according to their coverage (minimum 6 controls) and timely care (first control <14 weeks). A multivariate logistic regression analysis was performed considering a confidence level of 95%. Results. It was found that forgetfulness of appointments is shown as a factor associated with inadequate coverage of antenatal care (p=0,048), while the presence of a safe state of prenatal care was associated with timely prenatal care (p=0,013). Conclusions. The absence of forgetfulness of appointments and the presence of a safe state of health during pregnancy is associated with access to prenatal care.

Introducción. La atención prenatal aún no cubre al total de gestantes, situación que se repite en Latinoamérica y son pocos los estudios que evalúan las barreras que explican este fenómeno. El objetivo del presente estudio fue determinar los factores asociados al acceso de la atención prenatal en sus dimensiones de atención oportuna y cobertura. Métodos. Estudio de enfoque cuantitativo, observacional y analítico, realizado en 110 puérperas de los hospitales de Huaral y Chancay, Perú. Se evaluaron los factores (personales y sociodemográficos) asociados al acceso a la atención prenatal de acuerdo a su cobertura (mínimo 6 controles) y atención oportuna (primer control <14 semanas). Se realizó un análisis de regresión logística multivariada considerando un nivel de confianza del 95%. Resultados. Se halló que el olvido de las citas se muestra como un factor asociado a la cobertura inadecuada de la atención prenatal (p=0,048), mientras que la presencia de un seguro de salud, previo al control prenatal, se asoció a la atención prenatal oportuna (p=0,013). Conclusiones. La ausencia de olvido de las citas y la presencia de un seguro de salud previo al embarazo se asocian con el acceso a la atención prenatal.
