ABSTRACT Background: Thyroid dysfunction has been reported in association with several chronic diseases, including advanced liver disease. This disease and its management are often neglected in clinical practice. The bundle discussed here is aimed at proposing systematic assistance according to the best evidence-based practices available. Objective: To construct and validate a bundle to evaluate thyroid function in patients with liver cirrhosis. Methods: The process of constructing and validating the bundle was carried out in the following stages: a) bibliographic survey; b) bundle elaboration; and c) content validation. The bibliographic survey was carried out in an integrative review about evidence related with the thyroid function of patients with liver cirrhosis. The findings from the integrative review were considered as supporting evidence for the elaboration of the bundle. The tool then created used accessible language and was evidence-based, ensuring that information was based on current literature. Results: The bundle was restructured to provide guidance on the management of patients with liver dysfunctions, including: cirrhosis due to general causes, cirrhosis due to hepatitis C, non-alcoholic fatty liver disease, primary biliary cholangitis, and hepatocellular carcinoma. The orientations in the bundle included: exams to be requested to screen for thyroid disorders, and guidance about the treatment of these dysfunctions and their associated complications. We analyzed specialist evaluation of the bundle using the Content Validity Index (CVI). We carried out a binomial test to evaluate consistency and specialist agreement regarding the items in the bundle, considering values >0.61 as a good level. The items in the bundle were considered to be valid (CVI >0.80). The general CVI of the instrument was 0.95 (CI95%: 0.91-0.98). Conclusion: The bundle was considered valid to facilitate medical decision making, aiding physicians to manage, in a practical and effective approach, the thyroid function of patients with liver cirrhosis. This tool should not be used as a replacement for individual, evaluation of the physician providing assistance. We recommend the structured bundle to be added to medical practice, considering its simple application, low cost, and potential to contribute for the management of these patients.

RESUMO Contexto: A disfunção tireoidiana tem sido relatada em associação com várias doenças crônicas, incluindo a doença hepática avançada. O seu reconhecimento e manejo são, muitas vezes, negligenciados na prática clínica. O bundle consiste em um pacote de cuidados, com a finalidade de promover a assistência de forma sistematizada, a partir da melhor prática baseada em evidências. Objetivo: Construir e validar um bundle para avaliação da função tireoidiana em pacientes com cirrose hepática. Métodos: O processo de construção e validação do bundle foi realizado a partir das seguintes etapas: a) levantamento bibliográfico; b) elaboração do bundle; e c) validação do conteúdo. O levantamento bibliográfico foi realizado a partir de revisão integrativa sobre as evidências relacionadas à função tireoidiana em pacientes com cirrose hepática. Os achados encontrados na revisão integrativa foram considerados como subsídios para construção do bundle. A ferramenta construída se baseou em evidências científicas, de forma a garantir informação pautada em literatura atual e com linguagem acessível. Resultados: O bundle foi estruturado com base nas seguintes finalidades: fornecer orientações sobre o manejo de pacientes com disfunção hepática, categorizado por: cirrose por causas gerais, cirrose por vírus da hepatite C, doença hepática gordurosa não alcoólica, colangite biliar primária e carcinoma hepatocelular. As orientações do bundle incluíram: exames a serem solicitados no rastreio dos distúrbios tireoidianos; orientações sobre indicação de tratamento destas disfunções e possíveis complicações associadas. A análise da avaliação do bundle pelos especialistas foi realizada a partir do cálculo do Índice de Validade do Conteúdo (IVC). Foi feito o cálculo do teste binomial para avaliar a concordância e consistência dos especialistas em relação aos itens do bundle, aceitando-se como bom nível o valor >0,61. Os itens do bundle foram considerados validados (IVC >0,80). O instrumento apresentou IVC geral de 0,95 (IC95%: 0,91-0,98). Conclusão: O bundle foi considerado válido para facilitar a tomada de decisão para o médico conduzir, de maneira prática e efetiva, o manejo da função tireoidiana em pacientes com cirrose hepática. Defende-se que a ferramenta não deve ser utilizada em substituição à avaliação individualizada e autonomia do médico assistente. Recomenda-se que a forma estruturada seja incorporada ao atendimento médico tendo em vista a fácil aplicabilidade, o baixo custo e o potencial para contribuir com o manejo desses pacientes.

The neoliberal economic model propagated since the 1990s in Brazil and the economic, educational and evaluation policies of public universities impacted the meaning of work, the organization of work and the mental health of university professors. The mercantilist view under the aegis of capitalism has transfigured the organization of teachers' work, making it precarious, based on individualism, competition, work overload and the subtraction of leisure time by working time. Due to Covid-19 and the need for health measures to protect life and reduce the spread of the virus, emergency work and remote teaching were imposed in higher education institutions, enhancing the reality of the precarious work of the teacher. Given this context, this theoretical essay points out some possible ways to deal with the challenges of public HEIs and aims to problematize the reality of public university teaching work before and during the Covid-19 pandemic.

El modelo económico neoliberal propagado desde la década de 1990 en Brasil y las políticas económicas, educativas y de evaluación de las universidades públicas impactaron el sentido del trabajo, la organización del trabajo y la salud mental de las(os) profesores universitarios. La visión mercantilista bajo la égida del capitalismo ha transfigurado la organización del trabajo docente, haciéndola precaria, basada en el individualismo, la competencia, la sobrecarga de trabajo y la sustracción del tiempo de ocio por el tiempo de trabajo. Debido al Covid-19 y la necesidad de medidas sanitarias para proteger la vida y reducir la propagación del virus, se impuso el trabajo de emergencia y la enseñanza a distancia en las instituciones de educación superior, realzando la realidad del trabajo precario de las(os) docentes. Ante este contexto, este ensayo teórico apunta algunas vías posibles para enfrentar los desafíos de las IES públicas y pretende problematizar la realidad del trabajo docente universitario público antes y durante la pandemia de la Covid-19.

Diante desse contexto, este ensaio teórico aponta alguns caminhos possíveis para lidarmos com os desafios da IES pública e tem como objetivo problematizar a realidade do trabalho docente antes e durante a pandemia da Covid-10.

Resumo O Ministério da Saúde orienta que a declaração de óbito não seja emitida em casos de óbito fetal com gestação inferior a 20 semanas ou feto com peso inferior a 500 g ou estatura menor que 25 cm, acrescentando que a legislação permite a emissão da declaração em casos em que a família deseje fazer o sepultamento do feto. Nesse contexto, são poucos os casos de aborto em que a declaração é feita. Este artigo realizou revisão integrativa que responde à pergunta: os rituais de fechamento, particularmente o sepultamento (possibilitado pela emissão da declaração de óbito) em caso de morte fetal inferior a 20 semanas de idade gestacional, ajudariam no processo de luto dos pais? A literatura consultada trouxe informações favoráveis à emissão da declaração de óbito e possibilitou discussão médica, jurídica e antropológica do tema.

Abstract The Ministry of Health advises that death certificates should not be issued in cases of fetal death for a pregnancy of less than 20 weeks or fetus weighing less than 500 g or shorter than 25 cm in height; however, the legislation allows the issuance of the certificate in cases where the family wishes to bury the fetus. Given this context, abortion cases in which the certificate is issued are few. This article presents an integrative review that answers the question: would the death ceremonies, particularly the burial (made possible by the issuance of the death certificate), in case of fetal death under 20 weeks of gestational age help in the parents’ mourning process? The literature consulted presented favorable information for the issuance of the death certificate and enabled a medical, legal and anthropological discussion of the theme.

Resumen El Ministerio de Salud brasileño recomienda que no se debe emitir el certificado de defunción en los casos de muerte fetal de menos de 20 semanas de gestación, feto con peso inferior a 500 g o estatura inferior a 25 cm, pero agrega que se puede permitirlo cuando la familia opta por el entierro del feto. En este contexto, el certificado se emite en pocos casos de aborto. Este artículo realizó una revisión integradora a partir de la pregunta: ¿Ayudarían en el proceso de duelo de los padres los rituales de inhumación, sobre todo el entierro (habilitado mediante la emisión de un certificado de defunción) en caso de muerte fetal con menos de 20 semanas de edad gestacional? La literatura consultada aportó con informaciones favorables a la emisión del certificado de defunción y permitió fomentar la discusión médica, jurídica y antropológica del tema.

RESUMO Objetivo Avaliar a concordância entre instrumentos de avaliação quantitativa do palato duro. Método Este estudo transversal foi realizado com uma amostra de 30 crianças de 6 a 11 anos de Santa Maria, sul do Brasil. Os instrumentos de medidas do palato avaliados foram: paquímetro digital, utilizado diretamente na cavidade oral e em modelos de gesso, arco tridimensional Korkhaus, usado diretamente na cavidade oral e em modelos de gesso, e Dolphin Imaging Software utilizado para medições em tomografia computadorizada de feixe cônico (CBCT). A concordância entre os diferentes instrumentos foi avaliada por meio do Coeficiente de Correlação Intraclasse (ICC). Resultados As médias de todas as dimensões transversais obtidas pela tomografia computadorizada de feixe cônico foram menores do que as dos outros instrumentos - os valores de concordância na largura entre os caninos e na largura entre os primeiros molares foram baixos na comparação entre a tomografia computadorizada de feixe cônico e os demais instrumentos. Na largura entre o primeiro e o segundo pré-molar, todas as comparações apresentaram valores de concordância aceitáveis. Valores de concordância aceitáveis também foram obtidos ao comparar a profundidade do palato na região do segundo pré-molar com o uso de um divisor de arco dentro da cavidade oral e no gesso. Conclusão A maioria dos instrumentos apresentou concordância satisfatória nas medidas relacionadas ao plano transverso do palato duro. Porém, quando avaliado o plano vertical, apenas o divisor de arco aplicado tanto no gesso quanto na cavidade oral apresentou concordância ideal.

ABSTRACT Purpose To evaluate the agreement among instruments of the quantitative evaluation of hard palate. Methods This cross-sectional study was performed with a sample of 30 children aged 6 to 11 from Santa Maria, Southern Brazil. The instruments for palate measurements evaluated were: digital caliper, used directly in the oral cavity and in plaster casts, Korkhaus tridimensional bow, used directly in the oral cavity and in plaster casts, and Dolphin Imaging Software used for measurements in cone-beam computed tomography (CBCT). The agreement among different instruments was evaluated using the Intraclass Correlation Coefficient (ICC). Results The means of all transversal dimensions obtained by cone-beam computed tomography were lower than those of the other instruments - the agreement values in the width between the canines and in the width between the first molars were lower when comparing the cone-beam computed tomography and the other instruments. In the width between the first and second premolars, all comparisons showed acceptable agreement values. Good concordance values were obtained when comparing the palate depth at the second premolar region when using a bow divider inside the oral cavity and in the cast. Conclusion Most instruments presented satisfactory agreement in the measurements related to the transverse plane of the hard palate. However, when the vertical plane was evaluated, only the bow divider applied to both cast and oral cavity presented ideal agreement.

RESUMO O sono é essencial para o adequado funcionamento de todos os indivíduos. Os distúrbios respiratórios do sono ocorrem em todas as faixas etárias, constituindo motivo frequente de consulta médica. O objetivo deste consenso foi atualizar os conhecimentos sobre os principais distúrbios respiratórios do sono tanto na população adulta quanto na pediátrica, com ênfase na apneia obstrutiva do sono. A apneia obstrutiva do sono é uma doença extremamente prevalente, porém frequentemente subdiagnosticada. Associa-se frequentemente a uma série de comorbidades, notadamente cardiovasculares, metabólicas e neurocognitivas, que impactam significativamente na qualidade de vida e na mortalidade. Por conta disso, o Departamento de Distúrbios Respiratórios do Sono da Sociedade Brasileira de Pneumologia e Tisiologia reuniu 14 especialistas com reconhecida e comprovada experiência em distúrbios respiratórios do sono para a elaboração deste documento.

ABSTRACT Sleep is essential for the proper functioning of all individuals. Sleep-disordered breathing can occur at any age and is a common reason for medical visits. The objective of this consensus is to update knowledge about the main causes of sleep-disordered breathing in adult and pediatric populations, with an emphasis on obstructive sleep apnea. Obstructive sleep apnea is an extremely prevalent but often underdiagnosed disease. It is often accompanied by comorbidities, notably cardiovascular, metabolic, and neurocognitive disorders, which have a significant impact on quality of life and mortality rates. Therefore, to create this consensus, the Sleep-Disordered Breathing Department of the Brazilian Thoracic Association brought together 14 experts with recognized, proven experience in sleep-disordered breathing.

Abstract Introduction: Primary immune thrombocytopenia (ITP) is the most common cause of thrombocytopenia in children, with a reported incidence of 1.1-12.5 cases per 100 000 children. However, currently, there are several definitions of ITP, as well as diagnostic and therapeutic approaches. Objective: To develop an evidence-based clinical practice guideline (CPG) to standardize the definition of ITP and, in this way, reduce the variability of its diagnosis, and to provide indications for the treatment of acute, persistent, and chronic ITP in patients under 18 years of age. Materials and methods: The CPG was prepared by a multidisciplinary group that followed the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) guidelines for developing CPGs, formulated PICO clinical questions, and conducted systematic reviews. GRADE evidence profiles were created and recommendations, with their corresponding level of evidence and strength, were made after a panel of experts assessed the benefit-risk balance, the quality of evidence, the patients' values and preferences, and the context in which they should be implemented. Results: A total of 23 recommendations were made to pediatricians, hematologists, and health professionals working in emergency services for treating acute, persistent, and chronic ITP. Overall, the CPG has low quality of evidence, and the recommendations were made in order to improve the success rate of ITP treatment and the prognosis of children with this condition. Conclusions: Although ITP is the main cause of thrombocytopenia in pediatrics, to date there is not enough high-quality evidence that supports the recommendations presented here for its proper classification and treatment in children. Thus, further studies providing high-quality evidence on this issue are required.

Resumen Introducción. La púrpura trombocitopénica inmunológica (PTI) es la causa más frecuente de trombocitopenia en población pediátrica, con una incidencia de 1.1 a 12.5 casos por cada 100 000 niños. Sin embargo, en la actualidad hay diferentes definiciones de PTI, así como enfoques diagnósticos y terapéuticos. Objetivo. Desarrollar una guía de práctica clínica (GPC) basada en la evidencia para unificar las definiciones de PTI, y de esta forma reducir la variabilidad de su diagnóstico, y para proporcionar indicaciones para el tratamiento de la PTI aguda, persistente y crónica en pacientes menores de 18 años. Materiales y métodos. La GPC fue desarrollada por un grupo multidisciplinario, el cual siguió las guías GRADE para la realización de GPC, formuló preguntas clínicas PICO y realizó revisiones sistemáticas. Se crearon perfiles de evidencia GRADE y se realizaron las recomendaciones, con su respectivo nivel de evidencia y fortaleza, luego de que un panel de expertos evaluara el equilibrio beneficio-riesgo, la calidad de la evidencia, las preferencias y valoraciones de los pacientes y el contexto en el que debieran implementarse. Resultados. Se formularon 23 recomendaciones para el tratamiento de la PTI aguda, persistente y crónica dirigidas a pediatras, hematólogos y profesionales de la salud que trabajan en servicios de urgencias. En general, la evidencia de la guía es de baja calidad y las recomendaciones fueron formuladas para mejorar la tasa de éxito del tratamiento de la PTI y el pronóstico de estos pacientes. Conclusiones. A pesar de que la PTI es la principal causa de trombocitopenia en población pediátrica, actualmente no hay suficiente evidencia de alta calidad que respalde las recomendaciones aquí presentadas para su adecuada clasificación y tratamiento en niños. Por lo anterior, se requiere realizar nuevos estudios que brinden evidencia de alta calidad en el tema.

ABSTRACT Purpose: to investigate the relation between forward masking and cognitive-language skills in children as a function of literacy stage. Methods: twenty-seven children registered in literacy stage at public schools, aged from 6 to 9 years old, with no learning difficulties or audiological problems reported, participated in this study. Frequency Following Responses were registered in two test conditions: 1) /da/ alone; 2) /da/ presented 4 milliseconds after a speech noise. Two language protocols were applied: a cognitive-linguistic skills protocol and a phonological awareness one. Results: forward masking was evident in Frequency Following Responses of all children regardless of literacy stage. Frequency Following Responses latencies in both testing conditions showed no difference among participants’ age. Cognitive-language skills scores were below those expected for all children, with significant improvement noticed as a function of age. No correlation between language performance and forward masking was found. Conclusion: there was no relation between forward masking and cognitive-language skills in children, as a function of literacy stage.

ABSTRACT This study has estimated the risk of Leishmania transmission via blood transfusion in one of the largest blood banks in Northeastern Brazil, where visceral leishmaniasis is endemic. Five hundred blood samples from donors were tested for circulating Leishmania spp. DNA by real-time PCR. Positive samples were tested by a species-specific conventional PCR targeting Leishmania infantum . Overall, 6.2% (95% CI: 4.1–8.3%) of the samples carried Leishmania DNA and in one sample the species was confirmed as L. infantum . No statistically significant differences were found in relation to gender, sex, education level, incomeas well as the place of residence between positive and negative blood donors. Our results confirm the presence of asymptomatic Leishmania carriers among blood donors in a large blood bank in Northeastern Brazil. Considering the studied population, we estimate that for every 1,000 blood donors screened, 41 to 83 will be positive for Leishmania DNA. This finding reinforces the urgent need for elaborating specific Blood bank guidelines to allow the early detection of asymptomatic Leishmania carriers among blood donors before their blood products are transfused to uninfected individuals.

Abstract INTRODUCTION: Laboratory and clinical features of childhood tuberculosis (TB) are non-specific and establishing an accurate diagnosis remains a challenge. This study evaluated a Single tube nested-PCR (STNPCR) to detect genomic DNA of Mycobacterium tuberculosis complex in blood and urine. METHODS: Biological samples were obtained from children (<15 years old) with clinical suspicion of pulmonary and extrapulmonary TB at public hospitals in Recife-Pernambuco, Brazil. Cultures yielded negative results in a majority of childhood TB cases, which are generally paucibacillary. A set of clinical, epidemiological, radiological, and laboratory criteria with evident clinical improvement after anti-TB treatment were frequently used to define childhood TB cases. RESULTS: Ninety children with clinical suspicion were enrolled in this study (44 with TB and 46 without TB). The pulmonary TB group had 20 confirmed cases and 46 negative controls, while the extrapulmonary TB group had 24 confirmed cases. The STNPCR showed sensitivities to pulmonary and extrapulmonary TB of 47.4% and 52.2% (blood) and 38.8% and 20% (urine), respectively. Considering the low performance of STNPCR on separate samples, we decided to perform a combined analysis (parallel sensitivity analysis) of the results from blood and urine samples. The parallel sensitivity increased to 65% in blood and 62.5% in urine. The specificity in both samples ranged from 93.5-97.8%. CONCLUSIONS: Although STNPCR showed moderate sensitivity, the specificity is high; therefore, the test can be used as an auxiliary tool to diagnose TB in children. It is a rapid test that demonstrated better performance than other diagnostic tests in paucibacillary samples as it does in childhood tuberculosis.

SUMMARY: The use of halophytes such as Salicornia bigelovii represents an alternative with economic potential for desert agriculture and coastal areas. However, the success in the establishment of commercial plots has been limited by the low germination percentages of this species. Propagation using plant cuttings represents a viable and rapid alternative to this problem. The formation of functional roots is fundamental in the propagation of plant cuttings and has been achieved through the application of chemical growth regulators. The application of chitosan in some crops has evidenced to be an alternative for the germination process and an enhancer of harvest performance; however, there are no studies that evaluate its effect on the halophyte S. bigelovii. The objective of this research was to evaluate the effect of marine-derived chitosan and indole butyric acid as root-promoting agents in cuttings of Salicornia bigelovii. The concentrations (individually and combined) of chitosan at 100 and 50% and indole butyric acid were applied at a rate of 0.937 and 1.25 g kg‑1. A completely randomized design of nine treatments and five repetitions was used and 225 experimental units were evaluated. The following variables were measured: root length, fresh weight and total dry weight of the root, and number and length of new shoots. The results showed that the most efficient treatment was 100% chitosan + 0.937 g kg-1 indole butyric acid, promoting the production of photosynthetic shoots up to 800%. These results indicate that it is feasible to achieve the propagation of Salicornia by means of cuttings using marine-derived chitosan as a growth promoter.

RESUMEN: El aprovechamiento de halófitas como Salicornia bigelovii representa una alternativa con potencial económico para la agricultura del desierto y zonas costeras. No obstante, el éxito en el establecimiento de parcelas comerciales se ha visto limitado por los bajos porcentajes de germinación de la especie. La propagación por esquejes representa una opción viable y rápida a esta problemática. La formación de raíces funcionales es fundamental en la propagación por esquejes y se ha logrado mediante la aplicación de productos químicos reguladores de crecimiento. La aplicación de quitosano en algunos cultivos ha mostrado ser una alternativa en el proceso de germinación y mejores rendimientos de cosecha; sin embargo, con relación a la halófita S. bigelovii, son nulos los estudios sobre el efecto del quitosano. El objetivo de la presente investigación fue evaluar el efecto del quitosano de origen marino y el ácido indol butírico como agentes promotores de raíces en esquejes de Salicornia bigelovii. Se aplicaron las concentraciones (individual y en conjunto) de quitosano al 100 y 50% y ácido indol butírico a razón de 0.937 y 1.25 g kg-1. Se utilizó un diseño completamente al azar con nueve tratamientos con cinco repeticiones, evaluándose 225 unidades experimentales. Como variables respuesta se midieron: longitud radicular, peso fresco y peso seco total de la raíz, número y longitud de nuevos brotes. Los resultados indican que el tratamiento más eficiente fue 100% de quitosano + 0.937 g kg-1 de ácido indol butírico, el cual promovió la producción de brotes fotosintéticos hasta en un 800%. Estos resultados demuestran que es factible lograr la propagación de Salicornia mediante esquejes utilizando quitosano de origen marino como promotor de crecimiento.

Resumo Contexto O linfedema de membros inferiores é uma doença crônica decorrente de dano no sistema linfático que influencia a mobilidade, a funcionalidade e a qualidade de vida dos indivíduos. Questionários e o teste físico são métodos bastante práticos, de fácil aplicação e baixo custo, que fornecem dados importantes para a avaliação desses pacientes. Objetivos Avaliar a influência do linfedema unilateral de membro inferior na funcionalidade e na qualidade de vida, correlacionando três ferramentas de avaliação. Métodos Estudo descritivo com 25 indivíduos com linfedema unilateral em membro inferior, de ambos os sexos. Foi avaliada a perimetria e foram aplicados The Medical Outcome Study Short Form-36 Health Survey (SF-36) para avaliação da qualidade de vida, Lymphoedema Functioning, Disability and Health Questionnaire for Lower Limb Lymphoedema (Lymph-ICF-LL) para estudo das habilidades físicas, mentais e sociais relacionadas ao linfedema e o Timed Up and Go (TUG) para avaliação da funcionalidade. Resultados Houve a presença de linfedema em todo o membro inferior dos participantes. Os domínios mais prejudicados pelo linfedema foram os aspectos físicos (25,0 ± 31,4) e emocionais (36,0 ± 42,9) no SF-36 e o domínio mobilidade (6,0 ± 2,6) no Lymph-ICF-LL. O TUG foi realizado em 9,88 ± 1,98 s. Houve correlação entre o TUG e os questionários e entre os dois questionários utilizados. Conclusões Indivíduos com linfedema unilateral em membro inferior apresentam um impacto negativo na qualidade de vida e na funcionalidade avaliadas através de questionários, que correlacionam entre si. Não foi encontrada alteração no TUG, mas houve correlação entre ele e os questionários utilizados.

Abstract Background Lymphedema of the lower limbs is a chronic disease caused by damage to the lymphatic system that influences people’s mobility, functionality, and quality of life. Questionnaires and physical test are very practical, easy to apply, and low cost methods that provide important data for evaluation of these patients. Objectives To evaluate the influence of unilateral lower limb lymphedema on functionality and quality of life, correlating 3 assessment tools. Methods This was a descriptive study investigating 25 patients of both sexes with unilateral lymphedema in a lower limb. Limb volume was assessed using circumferential tape measurements, the Medical Outcomes Study Short Form-36 Health Survey (SF-36) was used to assess quality of life, the Lymphoedema Functioning, Disability and Health Questionnaire for Lower Limb Lymphoedema (Lymph-ICF-LL) was used to assess physical, mental, and social skills related to lymphedema, and the Timed Up and Go (TUG) test was used for functional assessment. Results Lymphedema was present throughout the affected lower limb of participants. The domains most affected by lymphedema were physical aspects (25.0 ± 31.4) and emotional aspects (36.0 ± 42.9) from the SF-36 and the mobility domain (6.0 ± 2.6) from the Lymph -ICF-LL. Patients performed the TUG in 9.88 ± 1.98 seconds. The TUG was correlated with the questionnaires and the questionnaires were correlated with each other. Conclusions People with unilateral lower limb lymphedema exhibited negative impacts on quality of life and functionality, as evaluated by questionnaires, which were correlated with each other. TUG performance was within normal limits, but results correlated with the questionnaires used.

Abstract INTRODUCTION: This study evaluated the performance of the IS6110-TaqMan® assay in different types of biological samples and tissues for laboratory diagnosis of extrapulmonary tuberculosis. METHODS: 143 biological samples and tissues from patients with suspected extrapulmonary tuberculosis from the health services of Recife/Pernambuco/Brazil were evaluated with the IS6110-TaqMan® assay. RESULTS: The sensitivities of the IS6110-TaqMan® assay calculated for blood, urine, both blood and urine samples, tissue biopsies, extrapulmonary body fluid samples, and all samples from patients calculated together were 55.9%, 33.3%, 68.8%, 43.8%, 29.6%, and 73.7%, respectively, and the specificities were 80%, 100%, 78.6%, 100%, 100%, and 84.2%, respectively. CONCLUSIONS The accuracy of qPCR was high in various clinical sample types. The analysis of more than one type of clinical sample collected from the same patient with extrapulmonary tuberculosis enhances the diagnostic power of the IS6110-TaqMan® assay when compared with the use of only one clinical sample.

RESUMO Objetivo Analisar a influência do gênero e da idade nas dimensões do palato duro, bem como verificar os parâmetros de referência disponíveis na literatura. Estratégia de pesquisa Dois examinadores realizaram a pesquisa de forma independente nas bases de dados Cochrane Library, PubMed-Medline e Web of Knowledge utilizando os descritores de acordo com as regras de sintaxe de cada banco de dados. Critérios de seleção Estudos em humanos observacionais ou experimentais, que avaliaram as dimensões do palato duro ou do arco dentário maxilar com pelo menos uma mensuração no plano transversal, vertical ou sagital em oclusões normais ou más oclusões classe I e que realizaram comparações das dimensões entre os gêneros e/ou idades. Análise de dados Análise descritiva, seguindo subdivisões: delineamento, amostra, instrumentos de avaliação, medidas em milímetros e análise estatística. A qualidade dos estudos incluídos foi verificada através da escala “Newcastle - Ottawa Quality”. Resultados Foram selecionados 18 estudos. Destes, 11 apresentaram resultados das dimensões do palato duro ou do arco dentário maxilar conforme o gênero, seis em idade e gênero e um somente em idade. Conclusão As medidas foram maiores no gênero masculino e houve um aumento progressivo nas dimensões do nascimento até o período de dentição permanente.

ABSTRACT Purpose Analyze the influence of gender and age on hard palate dimensions and verify the reference parameters available in the literature. Research strategies Two reviewers independently performed a search at the Cochrane Library, PubMed-Medline and Web of Knowledge databases using descriptors according to the syntax rules of each database. Selection criteria Observational or experimental human studies evaluating the dimensions of the hard palate or maxillary dental arch, with at least one transverse, vertical or sagittal plane measurement, in normal occlusions or class I malocclusions, and comparisons of the dimensions between genders and/or ages. Data analysis Descriptive analysis with the following subdivisions: design, sample, evaluation instruments, measurements in millimeters, and statistical analysis. Quality of the included studies was verified by the Newcastle - Ottawa Quality scale. Results Eighteen studies were selected and 11 presented results for hard palate or maxillary dental arch dimensions according to gender, six in age and gender and one in age only. Conclusion The dimensions were larger in males and progressive increase in the measurements was observed from birth to the permanent dentition period.

Objective: analyzing the strategies adopted by mental health nurses for professional qualification in a Psychosocial Care Center. Methods: a historical-social study with written documents and interviews with ten health professionals linked to the Psychosocial Care Center. Data analysis followed chronological order of the facts, and the emerging themes were triangulated and based on concepts that support Brazilian psychiatric reforms. Results: for professional qualifications, nurses invested in participating in congresses, seminars and symposiums; conducting study groups in order to develop new practical skills for mental health some nurses participated more frequently in workshops and followed other professionals for better insertion in this new context. Conclusion: the nurses approached interdisciplinary care to reconfigure their practices, investing in intellectual empowerment and faced the challenge of transforming mental health care in a deinstitutionalized practice.

Objetivo: analisar as estratégias adotadas pelos enfermeiros em saúde mental para qualificação profissional no Centro de Atenção Psicossocial. Métodos: estudo histórico-social com documentos escritos e entrevistas com dez profissionais de saúde vinculados ao Centro de Atenção Psicossocial. A análise dos dados seguiu uma ordem cronológica dos fatos, e os temas emergidos foram triangulados e fundamentados em conceitos que sustentam a reforma psiquiátrica brasileira. Resultados: para qualificação profissional, investiu-se na participação em congressos, seminários e simpósios; na realização de grupos de estudos de modo a desenvolver novas habilidades práticas voltadas à saúde mental, alguns enfermeiros participaram com mais frequência das oficinas e acompanharam outros profissionais para melhor inserção naquele novo contexto. Conclusão: os enfermeiros se aproximaram do cuidado interdisciplinar para reconfigurarem suas práticas, investiram na capacitação intelectual e enfrentaram o desafio de transformar a atenção em saúde mental em prática desinstitucionalizada.
