RESUMO A relação entre poesia e guerra é longeva e aponta para os arcanos do gênero, da gesta clássica à falência moderna do herói. Contrapondo A rosa do povo, de Carlos Drummond de Andrade, aos poemas de España, aparta de mí este cáliz, de César Vallejo, pretende-se compreender como, enquanto intelectual, o poeta se situa entre a saída melancólica e a sustentação viril da voz, entre o retraimento do ser pelo horror e pela impotência, e o êxtase que preside à ação. ABSTRACT The relationship between poetry and war is a long-standing one and betokens the arcana of the genre, from the classical poetic diction to the hero’s failure in modernity. By contrasting Carlos Drummond de Andrade’s A Rosa do Povo to César Vallejo’s España, aparta de mí este cáliz, my aim is to understand how, as intellectuals, poets position themselves between melancholy and a virile voice, between the being’s waning from horror and impotence, and the ecstasy that presides over action. ABSTRACT Introduction: Brazil has many inequities in the healthcare provided nationwide. Therefore, in order to access challenges in treatment, available resources and current practices, to identify barriers in delivering a good quality of care among Brazilian centers treating children and adolescents with Non-Hodgkin Lymphoma (NHL) and to generate a future prospective guideline, a customized online survey was distributed to pediatric hematologists and oncologists across the country. Results: A total of 97 surveys were completed (35% response rate), from 47 cities in all Brazilian regions and 79 units of care, with a median of 1 answer by the center (range 1 - 5). Most respondents work at an institution supported exclusively by public/philanthropic resources (58%), with an average of 5 to 9 new cases/year (49%), and 41% have 4 to 6 oncologists/centers. Additionally, 22% have no easy access to the intensive care unit, 26% have no access to Rasburicase, 28% have no access to Rituximabe as front-line therapy and 41% have unreliable methotrexate monitoring levels. Those differences cannot be explained thoroughly by regional wealth variances, nor by the financing model. Regarding the pathology service, 70% consider having reasonable quality assistance, but the timeframe to deliver diagnosis is satisfactory to 46%. There is no uniform management of care, with the current guideline from the Sociedade Brasileira de Oncologia Pediátrica being adopted by 54 to 59%, depending on the NHL subtype. Conclusion: This study provides insights into the heterogeneity of care among Brazilian centers. Recognizing those diversities will support the design of effective strategies and collaboration nationwide.

ABSTRACT This article describes progress in tackling noncommunicable diseases (NCDs) in the Americas since the Pan American Health Organization (PAHO) started its NCD program 25 years ago. Changes in the epidemiology of NCDs, NCD policies, health service capacity, and surveillance are discussed. PAHO’s NCD program is guided by regional plans of action on specific NCDs and risk factors, as well as a comprehensive NCD plan. Its work involves implementing evidence-based World Health Organization technical packages on NCDs and their risk factors with the aim of achieving the Sustainable Development Goal target of a one third reduction in premature mortality caused by NCDs by 2030. Important advances have been made in the past 25 years in implementation of: policies on NCD risk factors; interventions to improve NCD diagnosis and treatment; and NCD surveillance. Premature mortality from NCDs decreased by 1.7% a year between 2000 and 2011 and 0.77% a year between 2011 and 2019. However, policies on risk factor prevention and health promotion need to be strengthened to ensure more countries are on track to achieving the NCD-related health goals of the Sustainable Development Goals by 2030. Actions are recommended for governments to raise the priority of NCDs by: making NCDs a core pillar of primary care services, using revenues from health taxes to invest more in NCD prevention and control; and implementing policies, laws, and regulations to reduce the demand for and availability of tobacco, alcohol, and ultra-processed food products.

RESUMO Este artigo descreve o progresso no combate às doenças não transmissíveis (DNTs) nas Américas desde que a Organização Pan-Americana da Saúde (OPAS) iniciou seu programa para essas doenças há 25 anos. Discute-se como evoluíram a epidemiologia das DNTs, as políticas contra essas doenças, a capacidade dos serviços de saúde e a vigilância. O programa da OPAS para as DNTs é orientado por planos de ação regionais sobre DNTs específicas e fatores de risco, bem como por um plano integral contra essas doenças. O trabalho envolve a implementação de pacotes técnicos da Organização Mundial da Saúde baseados em evidências sobre as DNTs e seus fatores de risco, no intuito de alcançar a meta do Objetivo de Desenvolvimento Sustentável de reduzir em um terço a mortalidade prematura causada pelas DNTs até 2030. Avanços importantes foram obtidos nos últimos 25 anos na implementação de políticas sobre fatores de risco das DNTs, intervenções para melhorar o diagnóstico e o tratamento das DNTs, e vigilância das DNTs. A mortalidade prematura causada pelas DNTs diminuiu 1,7% ao ano entre 2000 e 2011 e 0,77% ao ano entre 2011 e 2019. Contudo, as políticas sobre a prevenção dos fatores de risco e a promoção da saúde precisam ser fortalecidas para que mais países estejam no rumo certo para alcançar as metas de saúde relacionadas a essas doenças, no âmbito dos Objetivos de Desenvolvimento Sustentável até 2030. São recomendadas medidas para que os governos elevem a prioridade das DNTs ao torná-las um pilar central dos serviços de atenção primária, usando a receita dos tributos saudáveis para investir mais na prevenção e no controle das DNTs, e ao implementar políticas, leis e regulamentos para reduzir a demanda e a disponibilidade de álcool, tabaco e produtos alimentícios ultraprocessados.

RESUMEN En este artículo se describe el progreso en la lucha contra las enfermedades no transmisibles (ENT) en la Región de las Américas desde que la Organización Panamericana de la Salud (OPS) iniciara su programa contra las ENT hace 25 años. Se abordan los cambios en las características epidemiológicas, las políticas, la capacidad de los servicios de salud y la vigilancia de estas enfermedades. Este programa de la OPS se rige por planes regionales de acción sobre enfermedades y factores de riesgo específicos, así como por un plan integral de ENT. Su labor consiste en poner en práctica paquetes técnicos de la Organización Mundial de la Salud basados en la evidencia sobre las ENT y sus factores de riesgo con el objetivo de alcanzar la meta de los Objetivos de Desarrollo Sostenible (ODS) de reducir en un tercio la mortalidad prematura causada por las ENT para el 2030. En los últimos 25 años se han logrado importantes avances en la ejecución de políticas sobre los factores de riesgo de estas enfermedades, en las intervenciones para mejorar su diagnóstico y tratamiento, y en la vigilancia. La mortalidad prematura por ENT disminuyó 1,7% anual entre el 2000 y el 2011 y 0,77% anual entre los años 2011 y 2019. Sin embargo, es necesario fortalecer las políticas de prevención de factores de riesgo y promoción de la salud para garantizar que más países estén bien encaminados para lograr las metas de salud de los ODS relacionadas con las ENT para el 2030. Se recomiendan medidas para que los gobiernos prioricen más las ENT y las conviertan en un pilar central de los servicios de atención primaria, al usar los ingresos generados por los impuestos en el sector de la salud para incrementar las inversiones en la prevención y control de las ENT, y ejecutar políticas, leyes y regulaciones para reducir la demanda y la disponibilidad de tabaco, alcohol y alimentos ultraprocesados.

Abstract Objective To identify the race/color self-reported by women 60 days postpartum; to identify the prevalence of signs of depressive disorder among these women and to verify the association between signs of depressive disorder and the self-reported race/color. Methods Cross-sectional study carried out in a noncapital city in the state of São Paulo. An instrument with sociodemographic data and the Edinburgh Postnatal Depression Scale were used for data collection. Data was analyzed using the Statistical Package for Social Sciences, SPSS, version 17.0. Results A total of 186 women participated in this study, 60.8% reported being brown, 24.2% had signs of depressive disorder and, among these, 81.7% were black. Signs of depressive disorder were associated with the variables: number of children (p=0.006), planned pregnancy (p=0.04) and type of delivery (p< 0.001). Conclusion The results of this study contribute to greater visibility of the issue of women’s mental health, specifically of black women, because even though no significant association was identified among those who showed signs of depressive disorder, most were black.

Resumen Objetivo Identificar la raza/color autodeclarado por mujeres con 60 días de posparto, identificar la prevalencia de indicios de trastorno depresivo en esas mujeres y verificar la relación entre los indicios de trastorno depresivo y el ítem raza/color autodeclarado. Métodos Estudio transversal realizado en un municipio del interior paulista. Para la recopilación de datos, se utilizó un instrumento con datos sociodemográficos y la Escala de Depresión Posnatal de Edimburgo. Para analizar los datos se utilizó el Statistical Package for Social Sciences, SPSS, versión 17.0. Resultados Participaron en el estudio 186 mujeres, el 60,8 % declararon ser de color pardo, el 24,2 % presentó indicios de trastorno depresivo y, entre ellas, el 81,7 % era de raza negra. Los indicios de trastorno depresivo se relacionaron a las variables: número de hijos (p=0,006), embarazo planeado (p=0,04) y tipo de parto (p< 0,001). Conclusión Los resultados de este estudio contribuyen para una mayor visibilidad del tema de salud mental de las mujeres, específicamente de mujeres negras, ya que, aunque no se haya identificado una relación significativa entre las que presentaron indicios de trastorno depresivo, la mayoría era de raza negra.

Foram utilizados, para a coleta de dados, um instrumento com dados sociodemográficos e a Escala de Depressão Pós-Natal de Edimburgo. Os dados foram analisados utilizando-se o Statistical Package for Social Sciences, SPSS, versão 17.0. Resultados Participaram deste estudo 186 mulheres, 60,8% referiram ser da cor parda, 24,2% apresentaram indicativo de transtorno depressivo e, entre estas, 81,7% eram da raça negra. O indicativo de transtorno depressivo associou-se às variáveis: número de filhos (p=0,006), gestação planejada (p=0,04) e tipo de parto (p< 0,001). Conclusão Os resultados deste estudo contribuem para maior visibilidade da temática da saúde mental das mulheres, especificamente das mulheres negras, pois, mesmo que não tenha sido identificada associação significativa dentre aquelas que apresentaram indicativo de transtorno depressivo, a maioria era da raça negra.

ABSTRACT Objective: to trace the sociodemographic and clinical profile, causal factors, and therapeutic management provided to children with pressure ulcers during hospitalization. Method: cross-sectional study; retrospective. Sample of 64 medical records of children with PU, admitted to a hospital in southern Brazil, from January/2016 to July/2021. Data analyzed by descriptive and inferential statistics. Results: Profile of children in intensive care (62.5%); stage 1 pressure ulcers (35.9%); and use of simple cover (37.5%). Of the total cases, 25% by medical device use. Consultations were related to stage 3 injury (p=0.027). Nursing diagnosis risk of pressure ulcer was identified in 48.4% of cases, while the Braden/Braden Q scale was identified in 78.1%. Patients classified as high risk (46%) had limited mobility (p=0.000). Conclusions: Pressure ulcers in children in intensive care with limited mobility require everything from simple intervention to consulting according to the classification of the injury.

RESUMEN Objetivo: trazar el perfil sociodemográfico y clínico, los factores causales y la conducta terapéutica seguida por los niños con UPP durante la hospitalización. Método: estudio transversal, retrospectivo. Muestra de 64 historias clínicas de niños con lesión por presión, ingresados en un hospital del sur de Brasil, de enero/2016 a julio/2021. Datos analizados por estadística descriptiva e inferencial. Resultados: Perfil de niños en terapia intensiva (62,5%); lesión por presión en estadio 1 (35,9%); y uso de cobertura simple (37,5%). Del total de casos, el 25% se debieron al uso de un dispositivo médico. Las consultas se relacionaron con el estadio 3 de la lesión (p=0,027). El diagnóstico de enfermería de riesgo de UP se identificó en el 48,4% de los casos, mientras que la escala Q de Braden/ Braden se identificó en el 78,1%. Los pacientes clasificados como de alto riesgo (46%) tenían movilidad limitada (p=0,000). Conclusiones: Las úlceras por presión en niños en cuidados intensivos con movilidad limitada requieren desde la simple intervención hasta el uso de consultoría según la clasificación de la lesión.

Diagnóstico de enfermagem risco de lesão por pressão foi identificado em 48,4% dos casos, enquanto a escala de Braden/Braden Q, em 78,1%. Pacientes classificados como risco elevado (46%) apresentavam mobilidade limitada (p=0,000). Conclusões: Lesão por pressão em crianças sob cuidados intensivos com mobilidade limitada requerem desde intervenção simples até o uso de consultoria conforme classificação da lesão.

ABSTRACT Polycystic liver disease, a hereditary pathology, usually manifests as autosomal dominant polycystic kidney disease. The many cysts in the liver cause massive hepatomegaly, majorly affecting the patient’s quality of life. In cases of refractory symptoms, liver transplantation is the only treatment choice. A 43-year-old woman was followed up as a hepatology outpatient in August 2020, with a progressive increase in abdominal volume, lower limb edema, and cachexia. The patient was diagnosed with polycystic renal and liver disease with massive hepatomegaly in March 2021, a combined kidney-liver transplant. Liver size represented 13% of the patient’s corporal composition, weighing 8.6kg. The patient was discharged on the 7th postoperative day with no complications. Only 10-20% of patients with polycystic liver disease have clinical manifestations, most of which result from hepatomegaly. An increase in liver volume deteriorates liver function until the condition becomes end-stage liver disease, as kidney function is already compromised; liver-kidney transplantation remains the only treatment choice. The case described drew significant attention to the massive hepatomegaly presented in the patient, with the liver representing over 10% of the patient’s body weight, approximately five to six times larger than a normal-sized liver.

Abstract Essential oils have been widely studied because they are natural sources of substances that have antimicrobial properties. In fruit growing, especially in mango crop, anthracnose (Colletotrichum gloeosporioides) is considered the main disease because it causes series of damages in the fruit production chain and, in this sense, essential oils can be an option in this disease control. The objective of this study was to evaluate the effect of essential oils obtained from the leaves of Chá-de-moça (Pectis brevipedunculata) and Melosa (Dizygostemon riparius) in the in vitro and in vivo control of the C. gloeosporioides fungus, in seedlings and mango fruits. The in vitro bioassays were performed by testing the effect of different concentrations of essential oils (0, 1, 2, 3 and 4 µl/mL) on the reduction of mycelial growth of the C. gloeosporioides fungus. Additionally, the anthracnose control in mango seedlings (Tommy Atkins, Constantina, Comum and Rosa cultivars) was evaluated by foliar application of essential oils at 4 µl/mL concentration. Disease control in fruits of the same mango cultivars was evaluated at 3 µl/mL concentration of essential oils. The evaluation was carried out by measuring the average diameter of the colonies, for the in vitro treatment, and the lesions for the in vivo treatments, in two diametrically opposite directions. There was a decrease in the fungus mycelial growth in all tested concentrations. There was a decrease in the disease severity from the sixth day after the oils application in Tommy Atkins cultivar seedlings, when treated with both essential oils at 4 µl/mL concentration. As for preventive treatments with fruits, it was observed that all cultivars achieved a reduction in severity from 54.83% at 3 µl/mL concentration with the use of both essential oils. Given the results obtained, it was observed that the essential oils P. brevipedunculata and D. riparius can be a viable alternative in the anthracnose control in mango culture.

Resumo Os óleos essenciais têm sido largamente estudados por serem fontes naturais de substâncias que possuem propriedades antimicrobianas. Na fruticultura, especialmente na cultura de manga, a antracnose (Colletotrichum gloeosporioides) é consideradaa principal doença por causar uma série de prejuízos na cadeia produtiva dos frutos, e,nesse sentido,os óleos essenciais podem ser uma opção no controle desta doença. O objetivodeste trabalho foi avaliar o efeito dos óleos essenciais obtidos das folhas de Chá-de-moça (Pectis brevipedunculata) e Melosa (Dizygostemon riparius) no controle in vitro e in vivo do fungo C. gloeosporioides, em mudas e frutos de mangueira. Os bioensaios in vitro foram realizados testando-se o efeito de diferentes concentrações dos óleos essenciais (0; 1;2; 3 e 4 µl/mL) sob a redução do crescimento micelial do fungo C. gloeosporioides.Adicionalmente, avaliou-se o controle da antracnose em mudas de manga (cultivares Tommy Atkins,Constantina, Comum e Rosa), pela aplicação foliar dos óleos essenciais, na concentração de 4 µl/mL. O controle da doença em frutos das mesmas cultivares de manga foi avaliado naconcentração de 3 µl/mL dos óleos essenciais. A avaliação foi feita pela mensuração dodiâmetro médio das colônias, para o tratamento in vitro, e das lesões, para ostratamentos in vivo, em dois sentidos diametralmente opostos. Houve diminuição do crescimento micelial do fungo em todas as concentrações testadas. Observou-se diminuição na severidade dadoença a partir do sexto dia após aplicação dos óleos nas mudas de cultivar Tommy, quandotratada com ambos os óleos essenciais, na concentração de 4 µl/mL. Já para os tratamentospreventivos com frutos, foi observado que todas as cultivares obtiveram redução daseveridade a partir de 54,83%, na concentração de 3 µl/mL, com uso de ambos os óleos essenciais.Diante dos resultados obtidos, foi observado que os óleos essenciais P. brevipedunculata e D. ripariuspodem ser uma alternativa viável no controle de antracnose na cultura de manga.

Abstract Objective: The authors investigated the functional status at ICU admission and at hospital discharge, and the impact of dysfunctions on survivors’ lifespan. Method: Single-center retrospective cohort. The FSS (Functional Status Scale) was calculated at ICU admission and at hospital discharge. A new morbidity was defined as an increase in FSS ≥ 3. Results: Among 1002 patients, there were 855 survivors. Of these, 194 (22.6%) had died by the end of the study; 45 (5.3%) had a new morbidity. Means in the motor domain at admission and discharge were 1.37 (SD: 0.82) and 1.53 (SD 0.95, p = 0.002). In the feeding domain, the means were 1.19 (SD 0.63) and 1.30 (SD 0.76), p = 0.002; global means were 6.93 (SD 2.45) and 7.2 (SD 2.94), p = 0.007. Acute respiratory failure requiring mechanical ventilation, the score PRISM IV, age < 5 years, and central nervous system tumors were independent predictors of new morbidity. New morbidity correlated with lower odds of survival after hospital discharge, considering all causes of death (p = 0.014), and was independently predictive of death (Cox hazard ratio = 1.98). In Weibull models, shortening in the life span of 14.2% (p = 0.014) was estimated as a new morbidity. Conclusions: New morbidities are related to age, disease severity at admission, and SNC tumors. New morbidities, in turn, correlate with lower probabilities of survival and shortening of the remaining life span. Physical rehabilitation interventions in this population of children may have the potential to provide an increase in lifespan.

ABSTRACT Objective: To analyze and compare COVID-19 patient characteristics, clinical management and outcomes between the peak and plateau periods of the first pandemic wave in Portugal. Methods: This was a multicentric ambispective cohort study including consecutive severe COVID-19 patients between March and August 2020 from 16 Portuguese intensive care units. The peak and plateau periods, respectively, weeks 10 - 16 and 17 - 34, were defined. Results: Five hundred forty-one adult patients with a median age of 65 [57 - 74] years, mostly male (71.2%), were included. There were no significant differences in median age (p = 0.3), Simplified Acute Physiology Score II (40 versus 39; p = 0.8), partial arterial oxygen pressure/fraction of inspired oxygen ratio (139 versus 136; p = 0.6), antibiotic therapy (57% versus 64%; p = 0.2) at admission, or 28-day mortality (24.4% versus 22.8%; p = 0.7) between the peak and plateau periods. During the peak period, patients had fewer comorbidities (1 [0 - 3] versus 2 [0 - 5]; p = 0.002) and presented a higher use of vasopressors (47% versus 36%; p < 0.001) and invasive mechanical ventilation (58.1 versus 49.2%; p < 0.001) at admission, prone positioning (45% versus 36%; p = 0.04), and hydroxychloroquine (59% versus 10%; p < 0.001) and lopinavir/ritonavir (41% versus 10%; p < 0.001) prescriptions. However, a greater use of high-flow nasal cannulas (5% versus 16%, p < 0.001) on admission, remdesivir (0.3% versus 15%; p < 0.001) and corticosteroid (29% versus 52%, p < 0.001) therapy, and a shorter ICU length of stay (12 days versus 8, p < 0.001) were observed during the plateau. Conclusion: There were significant changes in patient comorbidities, intensive care unit therapies and length of stay between the peak and plateau periods of the first COVID-19 wave.

RESUMO Objetivo: Analisar e comparar as características de pacientes críticos com a COVID-19, a abordagem clínica e os resultados entre os períodos de pico e de platô na primeira onda pandêmica em Portugal. Métodos: Este foi um estudo de coorte multicêntrico ambispectivo, que incluiu pacientes consecutivos com a forma grave da COVID-19 entre março e agosto de 2020 de 16 unidades de terapia intensiva portuguesas. Definiram-se as semanas 10 - 16 e 17 - 34 como os períodos de pico e platô. Resultados: Incluíram-se 541 pacientes adultos com mediana de idade de 65 [57 - 74] anos, a maioria do sexo masculino (71,2%). Não houve diferenças significativas na mediana de idade (p = 0,3), no Simplified Acute Physiology Score II (40 versus 39; p = 0,8), na pressão parcial de oxigênio/fração inspirada de oxigênio (139 versus 136; p = 0,6), na terapia com antibióticos na admissão (57% versus 64%; p = 0,2) ou na mortalidade aos 28 dias (24,4% versus 22,8%; p = 0,7) entre o período de pico e platô. Durante o período de pico, os pacientes tiveram menos comorbidades (1 [0 - 3] versus 2 [0 - 5]; p = 0,002); fizeram mais uso de vasopressores (47% versus 36%; p < 0,001) e ventilação mecânica invasiva na admissão (58,1% versus 49,2%; p < 0,001), e tiveram mais prescrição de hidroxicloroquina (59% versus 10%; p < 0,001), lopinavir/ritonavir (41% versus 10%; p < 0,001) e posição prona (45% versus 36%; p = 0,04). Entretanto, durante o platô, observou-se maior uso de cânulas nasais de alto fluxo (5% versus 16%; p < 0,001) na admissão, remdesivir (0,3% versus 15%; p < 0,001) e corticosteroides (29% versus 52%; p < 0,001), além de menor tempo de internação na unidade de terapia intensiva (12 versus 8 dias; p < 0,001). Conclusão: Houve mudanças significativas nas comorbidades dos pacientes, nos tratamentos da unidade de terapia intensiva e no tempo de internação entre os períodos de pico e platô na primeira onda da COVID-19.

Resumo: A pandemia da COVID-19 impactou diversas esferas da sociedade e alterou dinâmicas na gestão das empresas. O presente estudo tem o objetivo de compreender de que modo a pandemia da COVID-19 afetou a gestão das empresas de chocolate artesanal no Brasil, especificamente as empresas de chocolates bean to bar - ou seja, feitos pelos mesmos produtores desde a amêndoa até a barra de chocolate. Para tanto, desenvolveu-se uma revisão de literatura sobre o mercado de chocolates artesanais no Brasil e as dificuldades enfrentadas por empresas a partir da crise mundial causada pela pandemia da COVID19, que baseou a pesquisa qualitativa. Foram feitas, então, entrevistas semiestruturadas com quatorze membros de quatro associações de chocolates artesanais no Brasil. Os dados coletados foram submetidos à técnica de análise de vídeos de Denzin (2004). A partir das análises, observou-se que as empresas foram impactadas de diferentes maneiras a partir das vivências dos proprietários, da localização de suas fábricas, dos recursos disponíveis e das iniciativas tomadas a partir do conhecimento da crise. A mais significativa mudança na gestão foi a necessidade de adaptação às redes sociais digitais, bem como a profissionalização da presença online e de e-commerce para viabilização da continuidade de operação.

Abstract: The COVID-19 pandemic impacted different spheres of society and changed dynamics in the management of companies. This study aims to understand how the COVID-19 pandemic affected artisanal chocolate businesses in Brazil, specifically bean to bar chocolate companies - that is, made by the same producer from almond to bar of chocolate. Therefore, a literature review was developed on the artisanal chocolate market in Brazil and on the difficulties faced by companies during the global crisis caused by the COVID19 pandemic. Qualitative research was developed with semi-structured interviews with fourteen members from four artisan chocolate associations in Brazil. The data collected were analyzed using the video analysis technique by Denzin (2004). From the analyses, it was observed that companies were impacted in different ways based on the experiences of the owners, the location of their companies, available resources and initiatives based on knowledge of the crisis. The biggest change in management was the need to adapt to digital social networks, as well as the professionalization of the online and e-commerce presence to enable continuity of operation.

Por lo tanto, se descubrió una revisión de la literatura sobre el mercado del chocolate artesanal en Brasil y las dificultades que enfrentan las empresas ante la crisis global provocada por la pandemia COVID19, en la que se basó la investigación cualitativa. Luego se realizaron selecciones semiestructuradas con catorce miembros de cuatro asociaciones artesanales de chocolate en Brasil. Los datos recolectados fueron recolectados utilizando la técnica de análisis de video de Denzin (2004). A partir de los análisis, se observó que las empresas fueron impactadas de diferentes formas en función de las experiencias de los propietarios, la ubicación de sus empresas, los recursos disponibles y las iniciativas basadas en el conocimiento de la crisis. El mayor cambio en la gestión fue la necesidad de adaptarse a las redes sociales digitales, así como la profesionalización de la presencia online y del comercio electrónico para permitir la continuidad operativa.

ABSTRACT Objective: To evaluate the precipitating factors of diabetic ketoacidosis (DKA) in patients with type 1 diabetes hospitalized through the emergency department of a tertiary hospital. Materials and methods: Individuals with type 1 diabetes hospitalized for DKA from January 2005 to March 2010 (first period [P1], n = 75) and from April 2010 to January 2017 (second period [P2], n = 97) were identified through a query of electronic medical records. Data were collected by reviewing medical records. Only the first hospitalization of each participant in each period was included. Results: In P2, 44 patients (45.4%) were women, mean age was 26.2 ± 14.5 years, and 74 patients (76.3%) had a previous diagnosis of type 1 diabetes. Only 1 patient had glycated haemoglobin (HbA1c) below 64 mmol/mol (8.0%). Most patients (62.2%) had had a previous episode of DKA. In P1, non-adherence was the main cause of DKA (38.7%), followed by infection (24.0%). In P2, these rates were 34.0% and 24.7%, respectively; no statistical difference was observed between the two study periods (p = 0.790). Conclusion: Over time, non-adherence remained the main precipitating factor of DKA, followed by infection, and no significant difference was observed between the two study periods. Elevated HbA1c, outside the therapeutic range, indicates suboptimal diabetes care and may explain, at least in part, poor adherence as a precipitating factor of decompensation. Health strategies, such as improved self-management of type 1 diabetes, may contribute to a future reduction in DKA episodes.

Resumo A osteotomia Le Fort I é amplamente utilizada em cirurgia ortognática para corrigir as deformidades maxilares. No entanto, esse tipo de osteotomia pode estar relacionado ao aumento da largura da base alar. Os objetivos do presente estudo foram comparar duas técnicas de sutura da base alar após a realização da osteotomia Le Fort I, bem como observar qual tipo apresenta melhor resultado no controle do alargamento da base alar após o reposicionamento maxilar em cirurgia ortognática. Foi realizado um ensaio clínico randomizado com 40 pacientes alocados aleatoriamente em dois grupos de intervenção: grupo 1 - pacientes submetidos à técnica de sutura interna e grupo 2 - pacientes submetidos à técnica de externa. Dos 40 pacientes, 65% eram do sexo feminino e 35% do masculino. A média de idade dos pacientes foi de 30,25 no grupo I e 28,6 no grupo II. Houve aumento da largura da base alar em ambos os grupos, com diferença significativa entre as médias (P <0,001). Foi possível comparar a evolução das médias da largura da base alar entre o grupo I e o grupo II. Assim, observou-se que a técnica externa (grupo II) controlou melhor a largura da base alar após a osteotomia Le Fort I. Não foi possível relacionar o alargamento da base alar com o tipo de movimentação maxilar realizado (P> 0,05). No geral, a sutura da base alar é um componente essencial das osteotomias Le Fort para controlar o alargamento da base alar. Nesse estudo, a técnica externa foi mais eficaz quando comparada à técnica interna no controle do alargamento da largura da base alar.

Abstract Le Fort I osteotomy is widely used in orthognathic surgery to correct maxillary deformities. However, this osteotomy may be related with the increase of alar base width. The aims of the present study were to compare two alar cinch suture after Le Fort I osteotomy and observe which type presents a better result in controlling the enlargement of the alar base after maxillary repositioning in orthognathic surgery. A randomized clinical trial was carried out with 40 patients randomly assigned in two intervention groups: group 1 - patients submitted to internal suture and group 2 - patients submitted to external suture. Of the 40 patients, 65% were female and 35% were male. The mean age of the patients was 30,25 in group I and 28,6 in group II. There was an increase in the alar base width in both groups, with significant difference between the means (P < 0,001). It was possible to compare the evolution of the means of the alar base width between group I and group II. Thus, it was observed that the external technique (group II) better controlled alar base width after Le Fort I osteotomy. It was not possible to relate the enlargement of the alar cinch with maxillary movement performed (P > 0,05). Overall, alar base cinch suture is an essential component of Le Fort osteotomies to control the alar base width. In this study, the external technique was more effective when compared to the internal technique in controlling the enlargement of the alar base width.

ABSTRACT Objectives: To describe the oral health profile and evaluate the impact of tooth loss on diet quality and glycemic control among 66 patients with type 2 diabetes (T2DM) treated in an endocrinology outpatient clinic at a teaching hospital. Materials and methods: Questionnaires about diabetes self-care (SDSCA), masticatory ability, diet quality, anxiety level about dental treatment, and oral health were applied. Laboratory tests were retrieved from medical records or newly collected samples. Results: The presence of fewer than 21 teeth was associated with an unsatisfactory self-perceived masticatory ability (r = 0.44; p = 0.007). Most participants reported not having received guidance on oral health from their endocrinologists (81.8%) and having had the last visit to the dentist 2 years or more before the study (36.8%). The mean HbA1c level in the group with fewer than 21 teeth was comparable to that in the group with functional dentition (8.9 ± 1.5 and 8.7 ± 1.6%, respectively; p = 0.60). Conclusion: Adults with T2DM have a high prevalence of tooth loss and lack of information about oral hygiene care. Our results reinforce the need for more effective communication between medical and dental care teams.

Abstract: This study aimed to evaluate the spatial distribution of the air circulation speed and the final moisture content of pine lumber after kiln drying by using geostatistics. Two kilns acting on boards of 27 mm and 42 mm were evaluated. Air circulation speed and final moisture content were collected in different regions of the kilns. There was no significant difference for air circulation speed and final moisture content between the front and rear regions of the equipment. In the horizontal axis (Y-axis), air circulation speed averages did not differ statistically in both situations, but higher values for this variable were obtained from the spaces between the piles. Final moisture content tended to increase when closer to the door opening. In the vertical axis (Z-axis), the lower part tended to reach higher air circulation speed values. Thus, the spatial behavior influences the air circulation speed and final moisture content during lumber drying. Besides, the variograms presented the same tendency in comparison to the data obtained in a conventional manner, which indicates that geostatistics can be used to represent variables in kilns during the drying process.
