Abstract Objective: To assess the predictive value of selected growth phenotypes for neonatal morbidity and mortality in preterm infants < 30 weeks and to compare them with INTERGROWTH-21st (IG21). Methods: Retrospective analysis of data from the Brazilian Neonatal Research Network (BNRN) database for very low birth weight (VLBW) at 20 public tertiary-care university hospitals. Outcome: the composite neonatal morbidity and mortality (CNMM) consisted of in-hospital death, oxygen use at 36 weeks, intraventricular hemorrhage grade 3 or 4, and Bell stage 2 or 3 necrotizing enterocolitis. Selected growth phenotypes: small-for-gestational-age (SGA) defined as being < 3rd (SGA3) or 10th (SGA10) percentiles of BW, and large-for-gestational-age (LGA) as being > 97th percentile of BW. Stunting as being < 3rd percentile of the length and wasting as being < 3rd percentile of BMI. Single and multiple log-binomial regression models were fitted to estimate the relative risks of CNMM, comparing them to IG21. Results: 4,072 infants were included. The adjusted relative risks of CNMM associated with selected growth phenotypes were (BNRN/IG21): 1.45 (0.92–2.31)/1.60 (1.27–2.02) for SGA; 0.90 (0.55–1.47)/1.05 (0.55–1.99) for LGA; 1.65 (1.08–2.51)/1.58 (1.28–1.96) for stunting; and 1.48 (1.02–2.17) for wasting. Agreement between the two references was variable. The growth phenotypes had good specificity (>95%) and positive predictive value (70-90%), with poor sensitivity and low negative predictive value. Conclusion: The BNRN phenotypes at birth differed markedly from the IG21 standard and showed poor accuracy in predicting adverse neonatal outcomes.

Abstract The objective of This article was to identify the main challenges of promoting health surveillance in a triple border region of the Brazilian legal Amazon. A single explanatory case study was carried out, with a qualitative approach, which used documentary data and interviews. The Results demonstrate that health surveillance is essential for disease control in the studied region. In addition, the differences between the health systems of the three countries that make up the triple border (Brazil, Colombia, and Peru) showed to be the main challenge for establishing health policies.

Resumo O objetivo deste artigo foi identificar os principais desafios da promoção da vigilância em saúde em uma região de tríplice fronteira da Amazônia Legal brasileira. Foi realizado um estudo de caso único, explicativo, com abordagem qualitativa, que utilizou dados documentais e entrevistas. Os resultados demonstram que a vigilância em saúde é fundamental para o controle de doenças na região. Além disso, as diferenças dos sistemas de saúde dos três países que compõem a tríplice fronteira (Brasil, Colômbia e Peru) se mostraram o principal desafio para o estabelecimento de políticas sanitárias.

SUMMARY OBJECTIVE: The aim of this study was to assess the rate of repeated pregnancy in adolescence and its association with early marriage and education level. METHODS: This is a cross-sectional study conducted by searching the Live Births Data System. The study included all adolescents in the age group 10–19 years with live births from 2015 to 2019 (n=2,405,248), divided into three groups: G1: primiparas; G2: with 1 previous pregnancy; and G3: with two or more previous pregnancies. RESULTS: Total repeated pregnancies remained stable, along the years. In the age group 10–14 years, the decrease in the period was from 5.0 to 4.7%, whereas in the age group 15–19 years, it was from 27.8 to 27.3%. Being married or in a stable union increases by 96% the chance of repeated pregnancy in the age group 10–14 years (p<0.001; OR=1.96; 95% confidence interval [CI] 1.85–2.09). In the age group 15–19 years, the chance of repeated pregnancy among the married or in stable union increased 40% (p<0.001; OR=1.40; 95%CI 1.39–1.41)). Girls aged 10–14 years with an education level of<8 years had a 64% higher chance of repeated pregnancy (p<0.001; OR=1.64; 95%CI 1.53–1.75), and among those aged 15–19 years, there was a 137% higher chance of repeated pregnancy (p<0.001; OR=2.37; 95%CI 2.35–2.38). CONCLUSION: Repeated pregnancy in adolescence in Brazil remains very high over the years. There is an association between low education level and early marriage with repeated pregnancies in adolescence.

Abstract Hydrogels are used for wound treatment, as they may contain one or more active components and protect the wound bed. Papain is one of the active substances that have been used with this purpose, alongside urea. In this paper, carboxypolymethylene hydrogels containing papain (2% and 10% concentrations) and urea (5% concentration) were produced. Physical-chemical stability was performed at 0, 7, 15 and 30 days at 2-8ºC, 25ºC and 40ºC, as well as the rheological aspects and proteolytic activity of papain by gel electrophoresis. Clinical efficacy of the formulations in patients with lower limb ulcers was also evaluated in a prospective, single-center, randomized, double-blind and comparative clinical trial. The results showed 7-day stability for the formulations under 25ºC, in addition to approximately 100% and 15% of protein activity for 10% and 2% papain hydrogel, respectively. The rheological profile was non-Newtonian for the 10% papain hydrogel tested. There were no significant differences regarding the mean time for healing of the lesions, although 10% papain presented a better approach to be used in all types of tissue present in the wound bed.

ABSTRACT Purpose: to map health determinants associated with exclusive breastfeeding. Methods: a scoping review conducted according to the JBI methodology and recommendations in PRISMA-Extension for Scoping Reviews. Data were collected in the MEDLINE/PubMed, LILACS, CINAHL, Scopus, and EMBASE databases and in the Brazilian Digital Library of Theses and Dissertations for the grey literature. The results were synthesized and organized into three theoretical models: distal, intermediate, and proximal. Literature Review altogether, 3,998 titles were found, of which 145 were included in the review, after all selection stages. They had various methodological designs and were published between 2002 and 2022. The following health determinants associated with exclusive breastfeeding were mapped: maternal educational attainment, family income, attendance to and frequency of prenatal care, breastfeeding guidance and practices, time until first breastfeeding, newborn’s sex, maternal employment and maternity leave, pacifier use, and exclusive breastfeeding guidance at health services. Conclusion: determinants that can facilitate or hinder exclusive breastfeeding are extrinsic and intrinsic to the mother and infant. Public policies are needed to protect every person’s right to breastfeeding.

RESUMO Objetivo: mapear os determinantes em saúde associados ao aleitamento materno exclusivo. Métodos: revisão de escopo realizada seguindo a metodologia do JBI, concomitantemente com as recomendações do checklist PRISMA-Extension for Scoping Reviews. A coleta de dados foi realizada nas bases de dados Medline/PubMed, Lilacs, Cinahl, Scopus e Embase e, para identificação da literatura cinzenta, utilizou-se a Biblioteca Digital Brasileira de Teses e Dissertações. A síntese dos resultados foi organizada em três modelos teóricos: distal, intermediário e proximal. Revisão da Literatura: foram encontrados 3.998 títulos e, após todas as etapas de seleção, foram incluídos 145 estudos, de diferentes desenhos metodológicos, publicados entre os anos de 2002 e 2022. Foram mapeados como determinantes em saúde associados ao aleitamento materno exclusivo: escolaridade materna, renda familiar, realização e número de consultas no pré-natal, orientação e práticas do aleitamento materno, tempo até a primeira mamada, sexo do recém-nascido, trabalho materno e licença-maternidade, uso de chupeta e orientações sobre aleitamento materno exclusivo nos serviços de saúde. Conclusão: os determinantes que podem facilitar ou dificultar o aleitamento materno exclusivo são extrínsecos e intrínsecos ao binômio mãe e bebê, e fazem-se necessárias políticas públicas que protejam o direito de amamentação de qualquer ser humano.

Abstract Objective: The nutritional status resultant from dietary habits along with socioeconomic conditions and the school environment are directly related to the individual's health condition not only in their childhood but also throughout adulthood. The aim of this study was to evaluate the effects of socioeconomic factors on the anthropometric profile and to analyze a probable association between this profile and biochemical markers in children attending public daycare centers. Methods: It is a transversal study developed in a probability sample of clusters of children from 6 months to 5 years old. Anthropometric and socioeconomic data were gathered at the CMEIs, questionnaires on the nutritional status were applied and blood was collected at the Family Health Units (USFs). Results: Female children are three times more likely to be underweight; in families with five members, it is 1/3 more likely that children of higher-educated parents are overweight. Among the results of the biochemical tests, hypervitaminosis A was a relevant aspect, positively correlating with copper (p=0.005) and zinc (p=0.008). Conclusion: Therefore, since the influence of the family is an important predictor of overweight and its future outcomes related to nutritional deficiencies and inadequate dietary intake, educational interventions are vital as a way to pave the path to prevention.

Abstract Objective: To verify the association between impacts produced by tooth loss and sociodemographic variables in the oral health-related quality of life (OHRQoL) in the elderly. Material and Methods: This is an analytical cross-sectional study carried out in the municipality of Vitória/ES, Brazil, which sample was composed of 402 older adults. The Oral Health Impact Profile (OHIP-14) was used to measure the individuals' OHRQoL. For data analysis, descriptive and bivariate analyses were performed using the Fisher's exact test and the Mantel-Haenzsel test to assess the effects of combined dimensions. The significance level adopted was 5%. To assess the strength of association between independent variables and dimensions, Odds-Ratio (OR) with 95% confidence interval (CI) was calculated. Results: The prevalence of negative impact on OHRQoL in the elderly was 32.6%. The greatest impact perception was found in individuals with up to 10 years of schooling (p=0.021 and OR=1.602), with need for removable partial dentures (p=0.000 and OR=2.873) and those who did not accept the loss of dental elements (p=0.000 and OR=3.064). Conclusion: Older female adults with socioeconomic class C/D-E, with up to 10 years of schooling, who lost 11 or more teeth, who declared the need for removable partial dentures, suffered greater impacts caused by tooth loss on OHRQoL.

ABSTRACT Background: Snakebites are a major problem in developing countries owing to their high morbidity rates, severity, and sequelae. In Brazil, most cases of envenomation are caused by Bothrops and Lachesis snakes. The present study aimed to evaluate variables associated with death, systemic complications, and amputations in victims of envenomation due to Bothrops or Lachesis snake. Methods: An analytical epidemiological study was performed with data from the Notifiable Diseases Information System (Sistema de Informação de Agravos de Notificação [SINAN]) relating to the Brazilian Amazon for the period 2010-2015. A hierarchical Poisson regression analysis was performed with three aspects, namely, individual characteristics, sociodemographic characteristics, and clinical conditions. Results: The following associations were observed: i) advanced age (>65 years), sepsis, renal failure, and hemorrhagic manifestations were related to two outcomes - death due to snakebite and death from all causes; ii) more advanced age (≥46 years), time to health care longer than 6 hours, renal and hemorrhagic manifestations, and region of occurrence being rural areas were associated with systemic complications; and iii) children (up to 12 years old) were associated with amputations. Conclusions: Knowledge of the characteristics associated with severe outcomes in snakebites may help identify patients who will require more intensive care or longer follow-up and may provide the physician with counseling rationale for their possible prognosis.

Abstract The growth, sexual maturity and fertility-related parameters related of young Nellore bulls with divergent residual feed intake (RFI) raised on pasture were evaluated. After classification of 48 young males as low and high RFI (more and less efficient, respectively), the animals were evaluated for growth and reproductive parameters at 28-day intervals from 14.3 to 24.6 months of age. The semen was cryopreserved in the last sampling and fresh and post-thaw semen samples were evaluated. Low RFI bulls exhibited higher initial and final body weight (P < 0.05), but feed intake, body condition score and growth measures evaluated by carcass ultrasound were unaffected by RFI (P > 0.05). The scrotal circumference, sperm concentration, defects, and quality of fresh semen, and ultrasonographic testicular characteristics were unaffected by RFI (P > 0.05). However, velocity parameters such as average path and curvilinear velocities determined by computer-assisted sperm analysis of thawed semen submitted to the rapid thermoresistance test were improved (P < 0.05) in low RFI bulls, but this improvement in quality did not enhance in vitro sperm fertilizing ability. Our results demonstrated significant differences in metabolism and growth performance between bulls of divergent RFI. In addition, there was slight improvement in the semen quality of bulls with low RFI bulls, but this did not enhance in vitro fertilizing ability. Selection of beef bulls for RFI can be performed, which will result in economic benefits by improving the growth performance of the animals without affecting reproductive parameters.

SUMMARY OBJECTIVE: This study aimed to assess live birth frequency and age-specific fertility rates (ASFR) in the period 1996–2018 and the number of pregnancies at <14 years old in the period 2012–2018. METHODS: This was a cross-sectional study conducted by search on Live Births Data System (SINASC/DATASUS) database. RESULTS: There was a variation in ASFR in Brazil of 0.78‰ in 1996 to 0.87‰ in 2018 (+11.5%). In the north region, it increased from 1.28‰ to 1.66‰ in 2018. In the northeast region, it increased from 0.72‰ to 1.66‰ (+131%) in 1996–2011, but decreased to 1.31‰ in 2018 (-21% in relation to 2011). When comparing 1996 and 2018, in the southeast region, there was a 22% decrease; in the south region, it was 48.2%; and in the Center-West region, it was 34%; but in the north region, there was a 29.7% increase, and in the northeast region, it was 81.9%. When adding girls who became pregnant aged 13 years and gave birth at 14, there was a threefold increase in the rate. CONCLUSIONS: The increase of pregnancies in <14 years old in less developed regions of Brazil shows an association with socioeconomic factors and reveals the severe problem of rape of vulnerable persons in the country.

The purpose of this article is to compare the evolution of COVID-19 in Manaus and Fortaleza, two epicenters of the pandemic in 2020, analyzing legal measures by local governments and levels of social isolation. An algorithm was defined to calculate the Homestay Index (HSI), using data from the Google Mobility Report. The Decree's timeline, the HSI evolution, the incidence of COVID-19 and the number of deaths from March/2020 to January/2021 were analyzed. The population of Fortaleza was exposed to more consistent measures of social distance, than those of Manaus. Longer homestay was observed from March to May 2020 and Fortaleza achieved higher and more lasting levels. As of June 2020, the HSI fell, notably in Manaus, reaching levels below zero in late December. As an aggravating factor, the government decreed ample isolation in Manaus on December 23/2020, but after protests, it repeals it on December 26/2020. A Judicial Decision determines the complete closure in Manaus on January 02/2021, but it was too late: SUS collapses with an exponential increase in deaths. In Fortaleza, the demand for health services is high, but under control. We consider that only the strict application of non-pharmacological measures and mass immunization can prevent further deaths.

O objetivo desse artigo é comparar o comportamento do COVID-19 em Manaus e Fortaleza, dois epicentros da pandemia em 2020, analisando medidas legais dos governos locais e níveis de isolamento social. Definiu-se um algoritmo para calcular o Índice de Permanência Domiciliar (IPD), com dados do Google Mobility Report. Analisou-se a linha do tempo dos Decretos, a evolução do IPD, da incidência de COVID-19 e do número de óbitos de março / 2020 a janeiro / 2021. A população de Fortaleza esteve exposta a medidas de distanciamento social mais consistentes que as de Manaus. Maior permanência domiciliar foi observada de março a maio de 2020 e Fortaleza atingiu níveis mais elevados e duradouros. A partir de junho o IPD caiu, sobretudo em Manaus, atingindo níveis abaixo de zero no final de dezembro. Como agravante, o governo decreta amplo isolamento em Manaus em 23/12/2020, mas após protestos, revoga-o em 26/12/2020. Uma Decisão Judicial determina o fechamento completo em Manaus em 01/02/2021, mas foi tarde demais: o SUS entra em colapso com aumento exponencial dos óbitos. Em Fortaleza a demanda por serviços de saúde está elevada, mas sob controle.

Resumo Fator de crescimento endothelial (VEGF) e fator de crescimento de fibroblasto (FGF-2) tem a capacidade de aumentar a proliferação e permeabilidade vascular. O objetivo deste estudo foi quantificar a liberação dos dois fatores de crescimento (VEGF e FGF-2) após expansão rápida da maxilla (ERM). Trinta animais foram divididos aleatoriamente em dois grupos. Grupo Controle (5 ratos - sutura intacta) e grupos Experimentais (25 ratos submetidos a ERM) que foram avaliados em períodos diferentes de tratamento. Cinco animais foram eutanaziados em diferentes períodos de avaliação aos 0, 2, 3, 5 e 7 dias após ERM. RT-PCR foi usado para avaliar a expressão gênica dos fatores de crescimento liberados nos diferentes períodos de estudo. Os dados foram submetidos à análise estatística usando ANOVA seguido do pós-teste de Tukey com nível de significância de a=0.05. RT-PCR mostrou que os RNAm de VEGF e FGF-2 estavam expressos na sutura palatina mediana intacta. Os RNAm de VEGF e FGF-2 foram estimulados nos períodos iniciais (24h) após ERM (p<0.001 e p<0.01, respectivamente). Os nívies moleculares de VEGF nunca retornaram aos valores originais, e a expressão de FGF-2 reduziu até o dia 5 (p<0.001) e de repente aumentou até o dia 7, retornando aos níveis originais. ERM aumentou a secreção de VEGF, mas diminuiu a secreção de FGF-2 quando comparado ao tecido intacto. Os resultados mostraram que estes fatores de crescimento são liberados e regulados na sutura palatina mediana após ERM e podem ser de importante contribuição para o entendimento da resposta reparadora geral do tecido da sutura durante o processo de remodelação óssea.

Abstract Vascular endothelial growth factor (VEGF) and basic fibroblast growth factor (FGF-2) have the ability to increase vascular proliferation and permeability. The aim of this study was to quantify the release of two diffusible angiogenic growth factors (VEGF and FGF-2) after rapid maxillary expansion (RME). Thirty animals were randomly assigned to two groups. Control group (5 rats - intact suture) and Experimental groups (25 rats with RME) which were evaluated in different periods of treatment. Five animals were euthanized in different periods of healing at 0, 1, 2, 3, 5 and 7 days after RME. RT-PCR was used to evaluate the gene expression of angiogenic growth factors released on different periods of study. Data were submitted to statistical analysis using ANOVA followed by Tukey test and significance was assumed at a=0.05. RT-PCR showed that mRNAs of VEGF and FGF-2 were expressed in intact palatal suture tissue. mRNAs of VEGF and FGF-2 was upregulated in early periods (24 h) after RME (p<0.001 and p<0.01, respectively). The molecular levels of VEGF never returned to its original baseline values, and FGF-2 expression decreased up to day 5 (p<0.001) and suddenly increased at day 7, returning to its original level. RME increased VEGF secretion, but decreased FGF-2 secretion when compared to intact tissue. The results showed that these angiogenic growth factors are released and regulated in the palatal suture tissue after RME and could make an important contribution to the knowledge of overall reparative response of the suture tissue during the bone remodeling process.

Abstract This report describes a case of multisystem inflammatory syndrome in a child that evolved with a pattern of toxic shock syndrome with coronary artery ectasia and neurological involvement, documented by magnetic resonance imaging, with changes in the corpus callosum and myopathy in the pelvic girdle and paravertebral musculature.

Objetivo: analisar o efeito no conhecimento sobre a comunicação terapêutica pelos acadêmicos de Enfermagem com o uso de aplicativos. Método: ensaio clínico randomizado e controlado realizado junto a 60 estudantes de Enfermagem. No tópico Grupamentos e técnicas de comunicação terapêutica, o grupo intervenção-GI (n=30) utilizou o aplicativo e o grupo controle-GC (n=30) foi submetido à aula tradicional. Foi avaliado o conhecimento pré (Teste zero -0) e pós-teste (imediato-Teste 1 e após 30 dias-Teste 2) dos envolvidos. Resultados: o teste 1, realizado imediatamente após a intervenção, mostrou uma média de 11 acertos no grupo controle e 13 no intervenção, com significância estatística (p=0,036). O teste 2 mostrou uma diminuição dos acertos nos dois grupos (GI=10,87 e GC=9,3), mas manteve a diferença entre GI e GC no pós-teste (p<0,01). Conclusão: a utilização do aplicativo sobre a comunicação terapêutica favoreceu o conhecimento dos estudantes, quando comparado ao método tradicional de ensino. Registro REBEC RBR-4TF6MR.

Objective: to analyze the effect on the knowledge of therapeutic communication by Nursing students through the use of applications. Method: a randomized and controlled clinical trial conducted with 60 nursing students. In the topic Groups and therapeutic communication techniques, the intervention-IG group (n=30) used the application and the control-CG group (n=30) was submitted to the traditional class. The pre- (Zero Test -0) and post-test knowledge (immediate-Test 1 and after 30 day-Test 2) of those involved were evaluated. Results: Test 1, performed immediately after the intervention, showed a mean of 11 hits in the control group and 13 in the intervention, with statistical significance (p=0.036). Test 2 showed a decrease in hits in both groups (IG=10.87 and CG=9.3), but maintained the difference between IG and CG in the post-test (p<0.01). Método: ensayo clínico aleatorio controlado, realizado en 60 estudiantes de enfermería. En el tópico, agrupamientos y técnicas de comunicación terapéutica, el grupo intervención-GI (n=30) utilizó el aplicativo y el grupo control-GC (n=30) fue sometido a una clase tradicional. Fue evaluado el conocimiento pretest (Test cero -0) y postest (inmediato-Test 1 y después de 30 días-Test 2) de los participantes. Resultados: el test 1, realizado inmediatamente después de la intervención, mostró media de 11 aciertos en el grupo control y 13 en la intervención, con significación estadística de p=0,036. El test 2 mostró diminución de los aciertos en los dos grupos (GI=10,87 y GC=9,3), pero mantuvo diferencia entre GI y GC en el postest (p<0,01).
