ABSTRACT Introduction: given the growing complexity of the health problems of the population, it is essential to improve the research component in future doctors to take on this challenge and act in correspondence with the context. Objective: to refer to the trends that has characterized the training process of the research component in the Medicine career. Methods: a qualitative research with a historical approach was carried out, based on the documentary review, the chronological sub-method and the use of key informants, as predominant methods to develop the objective of this study. Results: the theoretical systematization carried out made it possible to determine the historical-pedagogical milestones to describe the assumed indicators, their progress in each of the stages and elucidate the trends that have characterized said process. Discussion: the formation process of the investigative component in the Medicine career has been on the rise with the improvement of the study plans and programs. However, insufficiencies still persist, given, among other factors, by the unsuccessful projection from the basic biomedical sciences, to communicate the results of the student's research activity, with the use of available technologies in correspondence with the context.

RESUMEN Introducción: dada la complejidad creciente de los problemas de salud de la población, se hace imprescindible perfeccionar el componente investigativo en los futuros médicos para asumir este reto y actuar en correspondencia con el contexto. Objetivo: describir las tendencias que han caracterizado el proceso formativo del componente investigativo en la carrera de Medicina. Métodos: se realizó una investigación cualitativa con enfoque histórico, a partir de la revisión documental, el método cronológico y la utilización de informantes clave, como métodos predominantes para desarrollar el objetivo de este estudio. Resultados: la sistematización teórica realizada posibilitó la determinación de los hitos histórico-pedagógicos para describir los indicadores asumidos, su progreso en cada una de las etapas y dilucidar las tendencias que han caracterizado dicho proceso. Discusión: el proceso de formación del componente investigativo en la carrera de Medicina ha ido en ascenso con el perfeccionamiento de los planes y programas de estudio. Sin embargo aún persisten insuficiencias, dada entre otros factores por la no acertada proyección desde las ciencias básicas biomédicas, para comunicar los resultados de la actividad investigativa del estudiante, con el uso de las tecnologías disponibles en correspondencia con el contexto.

ABSTRACT Introduction: The use of the media in the teaching and learning processes has evolved since its inception with an increasing trend in the different types of teaching, mainly focused on programmed instruction, was the control of learning is in function of renewing and predicts behaviors and expected behaviors in students. Objective: To present the main results of the implementation of the educational software epithelial tissue, in the subject Cell, tissues and integumentary system. Method: A quasi-experimental study was carried out in the Department of Basic Biomedical Sciences of the University of Medical Sciences of Camagüey, in the first semester of the 2019-2020 academic year. The application of epithelial tissue software, as an educational technology, facilitates the learning of subjects, helps to solve problems and contributes to developing cognitive skills in students, favoring developer learning. Conclusions: The educational software: Epithelial Tissue, constitutes a useful tool for learning the subject that makes it possible to improve the evaluation as a component of the teaching-learning process of the subject Cell, tissues and integumentary system and requires the commitment and permanent updating of the teacher to achieve relevant use.

RESUMEN Introducción: El uso de los medios en los procesos de enseñanza y aprendizaje ha evolucionado desde sus inicios con una tendencia al aumento en los diferentes tipos de enseñanzas, enfocado principalmente a la instrucción programada, donde el control de los aprendizajes está en función de renovar y predecir los comportamientos y las conductas esperadas en los alumnos. Objetivo: Exponer los principales resultados de la implementación del software educativo sobre Tejido Epitelial en la asignatura Célula, Tejidos y Sistema tegumentario. Métodos: Se realizó un estudio cuasi experimental, en el Departamento de Ciencias Básicas Biomédicas de la Universidad de Ciencias Médicas de Camagüey, en el primer semestre del curso 2019-2020. La aplicación del software de Tejido Epitelial, como tecnología educativa facilita el aprendizaje de materias, ayuda a resolver problemas y contribuye a desarrollar habilidades cognitivasen los estudiantes favoreciendo un aprendizaje desarrollador. Conclusiones: El software educativo: Tejido Epitelial, constituye una herramienta útil para el aprendizaje del tema que posibilita mejorar la evaluación como componente del proceso de enseñanza aprendizaje de la asignatura Célula, tejidos y sistema tegumentario y requiere del compromiso y la actualización permanente del docente para lograr un uso pertinente.

ABSTRACT Gastroenteritis caused by rotavirus is a major global health problem, therefore it is recommended that vaccination against rotavirus be included in immunization programs. To evaluate the impact of a future introduction in Cuba of a vaccine against this pathogen, it is necessary to have a pre-vaccination baseline of the burden of gastroenteritis caused by rotavirus. Between November 2017 and April 2018, a surveillance system for gastroenteritis caused by rotavirus was implemented in the Paediatric Hospital of Centro Habana. Definitions were established for the categories of suspected, probable and confirmed cases. For each captured child, stool samples were collected, analyzed with rapid strips and confirmated by ELISA. To determine the severity of the disease, the Vesikari score was used. The results were expressed in absolute and relative figures; the analysis was performed through chi-square. Of the total admissions for acute diarrheal disease, 26% met the inclusion criteria and 46% were confirmed for rotavirus. Overcrowding at home and attending a day care center were risk factors. The gastroenterology service absorbed the greatest burden of hospital admissions for this cause. The results shown validate the role of the implemented surveillance system and provide new evidence on the burden of disease and use of services for rotavirus gastroenteritis in a cuban pediatric hospital.

RESUMEN La gastroenteritis causada por rotavirus constituye un importante problema de salud mundial, por lo que se recomienda incluir la vacunación contra el rotavirus en los programas de inmunización. Para evaluar el impacto de una futura introducción en Cuba de una vacuna contra este patógeno, resulta necesario crear una línea de base pre-vacunación de la carga de la gastroenteritis causada por este virus. Entre noviembre 2017 a abril 2018 se implementó en el Hospital Pediátrico de Centro Habana un sistema de vigilancia para la gastroenteritis causada por rotavirus. Se establecieron las definiciones para las categorías de caso sospechoso, probable y confirmado. Por cada niño captado se recogió una muestra de heces que se analizó con tiras rápidas y se confirmó la presencia de rotavirus por ELISA. Para determinar la severidad de la enfermedad se utilizó la escala de Vesikari. Los resultados fueron expresados en cifras absolutas y relativas, el análisis se realizó a través de la prueba de chi-cuadrado. Del total de ingresos por enfermedad diarreica aguda, el 26% cumplió los criterios de inclusión y el 46% resultó confirmado como rotavirus. El hacinamiento en el hogar y asistir al círculo infantil se comportaron como factores de riesgo. El servicio de gastroenterología absorbió la mayor carga de ingresos hospitalarios por esta causa. Los resultados mostrados validan la funcionalidad del sistema de vigilancia implementado y brindan nuevas evidencias sobre la carga de la enfermedad y la utilización de los servicios de un hospital pediátrico cubano, debido a la gastroenteritis provocada por rotavirus, lo que justifica la introducción de la vacuna.

ABSTRACT Within the genus Kalanchoe Adans, the K. blossfeldiana Poelln species is the most popular for the beauty of its flowering. This constitutes one of the best known ornamental pot plants in the world, occupying one of the highest sales rates in Dutch auctions. Propagation in this species is carried out through traditional routes (seeds and cuttings) and tissue culture. In cut macropropagation it is recommended to use apical cuttings. However, to satisfy the high demand for this plant on the market, a macropropagation system is necessary that allows the use of various types of cuttings in unison in order to increase the number of explants to be used in each propagation cycle. In the present work, a protocol for the macropropagation of this species was developed from cuttings.

RESUMEN Dentro del género Kalanchoe Adans, la especie K. blossfeldiana Poelln es la más popular por la belleza de su floración. Esta constituye una de las plantas ornamentales de macetas más conocidas en el mundo y ocupa una de las tasas de venta más alta en subastas holandesas. La propagación en esta especie se realiza a través de vías tradicionales (semillas y esquejes) y cultivo de tejidos. En la macropropagación por esquejes se recomienda utilizar esquejes apicales. No obstante, para satisfacer la alta demanda de esta planta en el mercado se hace necesario un sistema de macropropagación que permita utilizar varios tipos de esquejes al unísono para así incrementar el número de explantes a utilizar en cada ciclo de propagación. En el presente trabajo se desarrolló un protocolo para la macropropagación de esta especie a partir de esquejes.

ABSTRACT Introduction: Medical education is based on the principles of on-the-job training. As a form of organization of the teaching process, on-the-job training plays a leading role in fulfilling such a mission. Primary health care settings should therefore permanently expand their involvement in the training of these professionals. Objective: Determine medical student and teacher satisfaction with on-the-job training in primary health care settings. Methods: A longitudinal descriptive study was conducted in Santo Domingo, Villa Clara province, during the school year 2016-2017. The sample was 26 third-year medical students from Manuel Piti Fajardo University Polyclinic, 24 sixth-year students rotating in Comprehensive General Medicine in this municipality, and 20 teachers actively involved in the training of these students. Data were obtained from document review and surveys applied to students and teachers. Results: Good performance prevailed. Students acquired their skills in the two years assigned to them, and all passed their end-of-term exams in the ordinary call. Most teachers were Assistant Professors methodologically trained to lead on-the-job training tasks. Conclusions: Most of the students and teachers in the study reported being satisfied with participating in on-the-job training activities in these environments.

RESUMEN Introducción: La formación del estudiante de Medicina se basa en los principios de la vinculación del estudio con el trabajo. La educación en el trabajo, como forma organizativa del proceso docente, lleva el protagonismo para cumplir con esta misión, por lo que los escenarios de la Atención Primaria de Salud deben ganar cada vez mayor espacio en la formación de estos profesionales. Objetivo: Valorar la satisfacción de estudiantes y profesores de la carrera de Medicina con la educación en el trabajo desde los escenarios de la Atención Primaria de Salud. Métodos: Se realizó un estudio descriptivo de corte longitudinal durante el curso 2016-2017 en Santo Domingo, provincia Villa Clara. La muestra estuvo constituida por 26 estudiantes de Medicina de tercer año del Policlínico Universitario “Manuel Piti Fajardo”, 24 alumnos de sexto año que hicieron su rotación de Medicina General Integral en este municipio, y 20 profesores que participaron activamente en la formación de estos educandos. La información se obtuvo mediante revisión documental y encuestas realizadas a estudiantes y profesores. Resultados: Predominó el buen desempeño de los estudiantes, al adquirir sus habilidades en los dos años de la carrera, así como la promoción en la convocatoria ordinaria. Los profesores, en su mayoría, eran asistentes y estaban preparados metodológicamente para desarrollar la educación el trabajo. Conclusiones: La mayoría de los educandos y profesores del estudio se consideraron satisfechos con desarrollar la educación en el trabajo desde estos escenarios.

ABSTRACT INTRODUCTION Mortality analysis based on a single cause of death is not, in most cases, fully informative. There are several more illuminating procedures using a multiple cause of death approach; these are little known and rarely used in Cuba. The simplest of these methods, while methodologically limited, consists of summing all deaths from a specific cause mentioned on death certificates, regardless of whether the cause is listed as underlying or contributing. OBJECTIVE Using Cuban data, critically assess and implement two of the most recognized approaches to analyzing multiple causes of death. METHODS Multiple causes of death in Cuba were assessed for the years 2005, 2010 and 2015, employing death records from the National Medical Records and Health Statistics Bureau of Cuba’s Ministry of Public Health. With the example of diabetes mellitus as underlying cause, we explored connections between underlying and associated (antecedent and contributing) causes on death certificates using two approaches from the international literature: the simple method and the cause-of-death association indicator. RESULTS The study identified main trends in multiple causes of death identified in 2005, 2010 and 2015, overall and by age group and sex. We observed a trend to increasing mean number of causes of death per death certificate between 2005 and 2015. The number of causes reported showed no substantial differences by age group or sex. Diseases of the arteries, arterioles and capillaries were by far the most frequently associated with diabetes mellitus as underlying cause. CONCLUSIONS The multiple causes of death approach affords more nuanced understanding of patterns of disease, comorbidity and death in the Cuban population. The indicators used fulfill different roles: the simple method brings to light the full range of ways in which a given cause contributes to mortality, and the cause-of-death association indicator enables exploration of links between different causes of death, not possible with the simple method.

INTRODUCTION Pneumococcal infections are a major cause of morbidity and mortality and are associated with considerable economic burden on health systems. To prevent pneumococcal infections, 7-valent conjugate vaccines have been available for over a decade; more recently, 10- and 13-valent conjugate vaccines have been formulated, which are more immunogenic than vaccines with capsular polysaccharides only. In Cuba, a new vaccine candidate has been developed, PCV7-TT, a conjugate of tetanus toxoid with antigens of seven of the serotypes of Streptococcus pneumoniae with highest circulation in Cuba and in the world: 1, 5, 6B, 14, 18C, 19F and 23F. OBJECTIVE Assess the safety of the vaccine candidate PCV7-TT in healthy adults and conduct a preliminary assessment of its immunogenicity. METHODS A phase I, double-blind clinical trial was performed at the National Toxicology Center in Havana, Cuba. Healthy male volunteers aged 18–35 years were randomly assigned to two groups: 20 received the vaccine candidate PCV7-TT and 20 the polyvalent antipneumococcal vaccine PNEUMO-23 used as control, each in a single intramuscular dose. To assess safety, the occurrence of adverse events was monitored for 30 days following inoculation. To explore immunogenicity, concentrations of serotype-specific antibodies was quantified before and 30 days after inoculation, as well titers of opsonophagocytic antibodies. (National Clinical Trial Registry RPCEC00000133) RESULTS Local adverse events were pain, redness, induration, increased sensitivity to touch, and warmth in the injection area. Pain was registered in 70% of individuals who received PCV7-TT and in 75% of those vaccinated with PNEUMO-23. Reported systemic adverse events were general malaise, headache and drowsiness. All adverse events appeared in the first 72 hours post inoculation and lasted no longer than 3 days. One event was reported that was classified as severe in intensity and serious in consequences, but it was unrelated to vaccination—acute appendicitis in one individual inoculated with the control vaccine. Before vaccination, all participants but one had antibody concentrations ≥0.20 μg/ml against the vaccine strains; after vaccination 100% of individuals were positive and the concentrations of antibodies increased in previously positive volunteers. Some individuals had opsonophagocytic antibodies against serotypes 1, 14, 19F and 23F before vaccination, with highest concentrations against serotypes 14 and 19F. After vaccination, the percent of individuals with opsonophagocytic titers ≥1:8 for all serotypes in the vaccine was >50% in both groups. CONCLUSIONS A single dose of candidate vaccine PCV7-TT was safe when used in healthy adults. Preliminary results showed that it was able to activate an immune response against the serotypes of Streptococcus pneumoniae used.

A placebo-controlled randomized, double-blind, clinical trial was carried out to assess the safety, reactogenicity, and immunogenicity of the lyophilized vaccine candidate against cholera derived from the live attenuated 638 Vibrio cholerae O1 El Tor Ogawa strain. One hundred and twenty presumably healthy female and male adult volunteers aged between 18 and 50 years were included. They were from Maputo, Mozambique a cholera endemic area, where, in addition, human immunodeficiency virus (HIV) seroprevalence is from 20 to 30%. A dose of 2 x 10 9 colony forming units (CFU) was given to 80 subjects and other 40 received only vaccine lyoprotectors as a placebo control. Out-patient follow-up of adverse events was carried out during the following 30 days after vaccination. The immune response was evaluated by the estimation of seroconversion rate and the geometric mean titer (GMT) of vibriocidal antibodies in the sera from volunteers that was collected previously, and at days 14 and 21 after immunization. No serious adverse events were reported. The adverse events found in the vaccine group were similar to those of the placebo groups. They were independent from the detection of antibodies against HIV-1, HIV-2, hepatitis (H) A; HC and hepatitis B surface antigen. The presence of helminthes did not modify the incidence of adverse events. The 638 vaccine strain was isolated in 37 (46.25%) vaccinated volunteer's feces. The peak of the GMT of vibriocidal antibodies in the vaccine group was 9056 versus 39 in the placebo group at 14 days with a total seroconversion of 97.4% at 21 days. The 638 vaccine candidate is safe and immunogenic in a cholera endemic region.

Se realizó un ensayo clínico controlado con placebo, aleatorizado y a doble ciego, para evaluar la seguridad, reactogenicidad e inmunogenicidad del candidato vacunal liofilizado contra el cólera de la cepa viva atenuada 638 de V. cholerae O1 El Tor Ogawa. Se incluyeron 120 voluntarios de ambos sexos, aparentemente sanos, entre 18 y 50 años de edad, en Maputo, Mozambique, un área endémica de cólera, donde la seroprevalencia del virus de inmunodeficiencia humana (VIH) es de 20% a 30%. Ochenta sujetos recibieron como tratamiento una dosis oral de 2 x 10 9 UFC del candidato vacunal 638 y otros 40 una dosis del placebo que contenía solo los lioprotectores. Se realizó seguimiento ambulatorio de los eventos adversos durante 30 días. La respuesta inmune se evaluó por medio de la estimación de la tasa de seroconversión y la media geométrica del título (MGT) de anticuerpos vibriocidas en los sueros de los voluntaries, antes y a los 14 y 21 días posteriores a la inmunización. No se reportaron eventos adversos graves. La incidencia de eventos adversos reportados en el grupo que recibió la vacuna fue similar al del grupo placebo. Los eventos adversos encontrados fueron independientes de la detección de anticuerpos contra el VIH-1, VIH-2, hepatitis (H) A, HC y el antígeno de superficie del virus de la hepatitis B. La presencia de helmintos no modificó la incidencia de eventos adversos. La cepa vacunal fue aislada en 37 (46,25%) voluntarios que recibieron la vacuna. A los 14 días el pico de la MGT de anticuerpos vibriocidas en el grupo de la vacuna fue de 9056 frente a 39 en el grupo placebo, con una seroconversión total de 97,4% a los 21 días. Se concluye que el candidato vacunal 638 es seguro e inmunogénico en una región endémica de cólera.

Immunoprophylaxis is the choice strategy for the prevention and control of meningococcal meningitis epidemics caused by Neisseria meningitidis serogroup A in the so-called meningitis belt of Africa, a disease that reaches high rates of morbidity and mortality. The lyophilized purified polysaccharide vaccine from Neisseria meningitidis serogroups A and C was produced as a result of a collaboration agreement between Finlay Institute, Center for Research, Development and Production of Vaccines, Cuba and the Institute of Technology and Bio-Immunobiologicals Manguinhos of Brazil, at the request of the World Health Organization to meet the needs of the vaccine in African countries in health emergencies. In the vaccination campaign carried out during an outbreak in Niger in 2009, a study of pharmacovigilance was conducted to describe the adverse events following the administration of this product to 303 037 people. Two methods were used to collect adverse events (spontaneous reporting and intensive search in hospitals). A total of 81 adverse events were reported, only 11% were under the category «induced by the vaccine» and considered «very probably related» to this. The results show that the vaccine used in the immunization campaign in Niger is safe, has low reactogenicity and their safety profile corresponds to that described in the sanitary medical registry in Brazil.

La inmunoprofilaxis constituye la estrategia por excelencia para la prevención y control de las epidemias de meningitis meningocócica provocadas por el serogrupo A de Neisseria meningitidis en el llamado cinturón de la meningitis en África, donde alcanza altas tasas de morbimortalidad. La vacuna liofilizada de polisacáridos purificados de Neisseria meningitidis de los serogrupos A y C fue producida como resultado de un convenio de colaboración entre el Instituto Finlay, Centro de Investigación, Desarrollo y Producción de Vacunas de Cuba y el Instituto de Tecnología e Inmunobiológicos de Bio-Manguinhos de Brasil, a solicitud de la Organización Mundial de la Salud, para cubrir las necesidades de la vacuna en los países africanos en emergencia sanitaria. Durante la campaña de vacunación desarrollada contra un brote epidémico en Níger, en el 2009, se realizó un estudio de vigilancia poslicenciamiento, con el objetivo de describir los eventos adversos consecutivos a la administración de este producto a 303.037 personas. Se utilizaron dos métodos para la recolección de los eventos adversos: notificación espontánea y vigilancia intensiva en los hospitales. Se reportaron un total de 81 eventos adversos, de los cuales solo el 11% fueron categorizados "inducidos por la vacuna" y considerados "muy probablemente relacionados" con esta. Los resultados obtenidos muestran que la vacuna utilizada en la campaña de vacunación de Níger es segura y poco reactogénica y el perfil de seguridad se corresponde con el descrito en el registro sanitario de Brasil.

To encourage the use of R in the clinical research and from the experience with this software for data processing and analysis, some of its features and the major advantages offering its use in the clinical trials are showed. R is free, flexible and dynamic software with a wide variety of statistical techniques of fast incorporation and that make possible its application. It allow in a conveniently way to present the corresponding information to users since make easy to register the statistical process making that documentation be available to potential audits supporting the results obtained from study.

Con el fin de estimular el uso de R en la investigación clínica y a partir de la experiencia con este software para el procesamiento y análisis de datos, se muestran algunas de sus características y las principales ventajas que ofrece su utilización en el campo de los ensayos clínicos. R es un software libre, flexible y dinámico, con una amplia variedad de técnicas estadísticas que se incorporan rápidamente y hacen posible su aplicación. Permite cómodamente presentar la información correspondiente a los usuarios, a la vez que hace posible registrar el proceso estadístico de forma que la documentación quede disponible a posibles auditorias que avalen los resultados obtenidos del estudio.

Neisseria meningitidis carriers are the main infection and transmission source of the meningococcal disease. To know their prevalence, the characteristics of isolated strains and the risk factors associated to carrier status, provide important information for epidemiological surveillance and control. A descriptive-transversal study on N. meningitidis carriers was performed. It involved 112 workers from a biopharmaceutical production center in Havana, from 18 to 60 years old. Bioethical requirements were complied before starting the study. A nasopharyngeal swab was performed to all subjects and they were surveyed to find out on risk factors (age, sex, overcrowding, smoking and drinking habit, amygdalectomy and background of respiratory infection) that may favor the condition of the carrier. The identification of N. meningitidis strains was carried out using conventional methods, the classification of the serogroups by slide agglutination with commercial antisera and the identification of serotypes and subtypes by an immunoenzymatic assay (ELISA) of whole cells with monoclonal antibodies. Eight percent of N. meningitidis carriers was detected, serogroup B (77.8%) was predominant and B:4:P1.4 (33.3%) was the most frequently observed phenotype. When analyzing the carrier status and its association to risk factors, statistically significant difference was only observed in age (p=0.05) and sex (p= 0.013). The possibility of occupational risk was demonstrated in those subjects, who due to their profession are involved with pathogenic microorganisms.

Los portadores de Neisseria meningitidis constituyen la principal fuente de infección y transmisión de la enfermedad meningocócica. Conocer su prevalencia, las características de las cepas aisladas y los factores de riesgos asociados con el estado de portador, aportan datos valiosos al control y vigilancia epidemiológica de esta entidad clínica. Para cumplimentar los objetivos propuestos se realizó un estudio transversal descriptivo de portadores de N. meningitidis en 112 trabajadores de un centro de producción de biofarmacéuticos de La Habana, con edades comprendidas entre 18_60 años. Previo a su realización se cumplió con las exigencias bioéticas requeridas para este tipo de estudio. A todos se les realizó un exudado nasofaríngeo y una encuesta, donde se indagó sobre factores de riesgo (edad, sexo, hacinamiento, hábito de fumar, consumo de bebidas alcohólicas, amigdalectomía y antecedentes de infección respiratoria) que favorecen la condición del portador. La identificación de las cepas de N. meningitidis se realizó según métodos convencionales, la clasificación de los serogrupos se hizo por aglutinación en láminas portaobjetos con antisueros comerciales y para la identificación de los serotipos y subtipos se empleó un ensayo inmunoenzimático (ELISA) de células enteras con anticuerpos monoclonales. Se detectó un 8% de portadores de N. meningitidis con predominio del serogrupo B (77,8%) y el fenotipo más frecuente fue el B:4:P1.4 (33,3%). Al analizar el estado de portador y su asociación con los factores de riesgo, la edad (p = 0,05) y el sexo (p = 0,013) mostraron diferencias significativas. Se demostró la posibilidad del riesgo ocupacional en aquellos individuos que por su profesión están en contacto con microorganismos patógenos.

OBJECTIVE: to serologically confirm the occurrence of human leptospirosis in presumptive cases from the department of Valle del Cauca in Colombia, to study clinical-epidemiological data associated to the disease and to find out the most prevalent leptospiral serogroups. METHOD: a total of 150 serum samples collected during the acute phase from clinically suspected patients of having leptospirosis were analyzed. Samples were evaluated by indirect hemagglutination and microagglutination tests, taking a microagglutination titer equal to or greater than 1:800 as definition of confirmed case. Epidemiological data as well as symptoms and signs of interest were analyzed. RESULTS: these serological techniques allowed defining 31 confirmed cases (20.6%) and 16 probable cases (10.6 %). Icterohaemorrhagiae was the serogroup with the higher serological reactivity in both confirmed and probable cases. Most common symptoms were fever (89.3 %), headaches (86.7 %) and myalgias (77.3 %). Symptoms of severe leptospirosis like jaundice and renal failure were observed in 36 % and 6.7 % of cases, respectively. About 70 % of confirmed or probable cases came from urban areas and 85 % of them were men aged 40 years as average. The epidemiological risk factors were exposure to rats and domestic animal (95.7 %) and potentially contaminated water (36.2 %). CONCLUSIONS: the results of the present study suggest that leptospirosis is a public health problem in Valle del Cauca, a region located on the south west of the country.

OBJETIVO: confirmar serológicamente la ocurrencia de leptospirosis humana en casos presuntivos procedentes del departamento del Valle del Cauca, estudiar variables clínico-epidemiológicas asociadas a la enfermedad e identificar los serogrupos de Leptospira más prevalentes en la región. MÉTODOS: se analizaron 150 muestras séricas colectadas en fase aguda, de casos con sospecha clínica y con riesgo epidemiológico de leptospirosis. Fueron evaluadas mediante hemoaglutinación indirecta y microaglutinación tomando como criterio de confirmación de la enfermedad la presencia de un título por microaglutinación mayor o igual que 1:800. Se analizaron datos epidemiológicos de interés así como síntomas y signos presentados en cada caso. RESULTADOS: las técnicas serológicas empleadas permitieron definir 31 casos confirmados (20,6 %) y 16 casos probables (10,6 %). Icterohaemorrhagie fue el serogrupo de mayor reactividad serológica tanto en casos confirmados como probables. Los síntomas más comunes fueron fiebre (89,3 %), cefalea (86,7 %) y mialgias (77,3 %). Síntomas de leptospirosis severa como ictericia e insuficiencia renal fueron observados en 36 % y 6,7 % de los casos, respectivamente. Cerca de 70 % de los casos confirmados o probables procedía de zonas urbanas y 85 % resultó de hombres con un promedio de edad de 40 años. Los factores de riesgo más frecuentes fueron la exposición a roedores o animales domésticos (95,7 %) y aguas potencialmente contaminadas (36,2 %). CONCLUSIONES: los resultados del presente estudio sugieren que la leptospirosis constituye un problema de salud pública en el Valle del Cauca, región suroccidental de Colombia.

During clinical trials it is common to appoint a commission which is in charged of assessing the causality of adverse events which are reported and transcribed to the collection data logbooks. To be able to answer the question: "Does the vaccine under study cause an adverse event.?" different pathways could be taken by decision makers, so it is obvious that the final conclusion could vary too much and a poorly assessment can generate mistaken conclusions. That confirms the need of having a procedure to define which categories must be used to analyze and to classify the relation of causality. To create an algorithm is one answer to the question. We reviewed the literature and did not find any algorithm which could be considered a "master key" to set or to reject causality, so this paper aims at designing an algorithm beginning with the international methodology used in post commercialization. A first proposal was designed which was individually and collectively analyzed by the multi-disciplinary group of the Medical Management from Finlay Institute. Finally, the proposed algorithm was approved by consensus, which could be useful to apply in clinical trials inside and outside of the institution.

En el transcurso de los ensayos clínicos usualmente se designa a una comisión, la cual se encarga de evaluar la causalidad de los eventos adversos que se reportan y transcriben en los cuadernos de recogida de datos. Para llegar a contestar la pregunta de: ¿si una vacuna que se está estudiando ha causado un evento adverso?, los decisores pueden tomar diferentes caminos, por lo que obviamente la conclusión final podría variar mucho y la evaluación deficiente puede generar conclusiones erróneas, lo que confirma la necesidad de disponer de un procedimiento para definir las categorías que se utilizarán para analizar y clasificar la relación de causalidad. Una forma de contestar a la pregunta es mediante la construcción de un algoritmo. En relación con los ensayos clínicos no encontramos en la literatura un algoritmo que sea el "patrón clave" para establecer o descartar la causalidad, sin embargo es factible diseñarlo partiendo de la metodología internacional utilizada poscomercialización, lo cual fue el propósito de este trabajo. Se diseñó una primera propuesta que fue analizada individual y colectivamente por el grupo multidisciplinario de la Gerencia Médica del Instituto Finlay. Finalmente se aprobó por consenso el algoritmo que puede ser útil para aplicar en los ensayos clínicos dentro y fuera de la institución.
