ABSTRACT Introduction: The human immunodeficiency virus (HIV) is the pathogen that causes human immunodeficiency syndrome (AIDS). Its importance lies in the fact that during the year 2020 in the world 37.6 million people lived with HIV, 1.5 million contracted the disease, and there is no vaccine to prevent their infection. Objective: To study the publications on the development of vaccines worldwide using bibliometric statistics on the subject of HIV and its progression in the last 20 years, emphasizing the period of the SARS-CoV-2 pandemic. Methods: A bibliometric statistical analysis was performed using the PubMed (Medline) database. For the qualitative analysis, the SCImago database was used. Results: An increase in publications was observed since 2001, reaching a maximum in 2016. Publications in first quartile journals were considered, with authors with a high H index, with important interactions between them. From 2017 to date, there has been a decrease in the number of articles published. With this method, it is not feasible, for now, to determine that the installation of the pandemic had a negative impact on the publication of articles on this topic. Conclusion: HIV should be established as a priority item in the scientific/medical area, this methodology offers ample possibilities to deepen and understand how science and its development are being approached, particularly in crucial issues such as vaccine research and the HIV.

RESUMEN Introducción: el virus de inmunodeficiencia humana (VIH) es el patógeno causante del síndrome de inmunodeficiencia humana (SIDA). Su importancia radica en que durante el año 2020 en el mundo 37,6 millones de personas vivían con VIH, y 1,5 millones contrajeron la enfermedad, no existe una vacuna para prevenir su infección. Objetivo: estudiar las publicaciones sobre el desarrollo de las vacunas a nivel mundial usando estadística bibliométrica en el tema VIH y su progresión en los últimos 20 años, haciendo énfasis en el periodo de la pandemia por SARS-CoV-2. Métodos: se realizó un análisis estadístico bibliométrico, usando la base de datos PubMed (Medline). Para el análisis cualitativo se utilizó la base SCImago. Resultados: se observó un aumento en las publicaciones desde el año 2001, alcanzando un máximo el año 2016. Se consideraron las publicaciones en revistas del primer cuartil, con autores(as) con elevado índice H, con importantes interacciones entre ellos. Desde el año 2017 a la fecha se observa una disminución en el número de artículos publicados. Con este método no es factible, por ahora, determinar que la instalación de la pandemia impacto negativamente en la publicación de artículos de dicho tema. Conclusión: el VIH debería instaurarse como un ítem prioritario en el área científica/médica, esta metodología ofrece amplias posibilidades para profundizar y comprender cómo se está abordando la ciencia y el desarrollo de esta, en particular en cuestión cruciales como es la investigación en vacunas y el VIH.

Abstract: The effect of the variables temperature, time and amount of enzyme on the degree of hydrolysis and the level of bitterness of the hydrolysates obtained from anchovy (Engraulis ringens) was examined. Anchovy without head, skin, tail, and viscera, is treated in a two-stage enzymatic hydrolysis process by using the response surface method. In the first stage, the Corolase 8000 enzyme is assessed by optimizing the process (60.1°C, 93.4 minutes, and 1.68% enzyme), maximizing the degree of hydrolysis up to a value of 12.8%. In the second stage, the enzyme Corolase 7089 is applied, maximizing the degree of hydrolysis (14.8%) while minimizing bitterness (3.8) by conducting the process at 49.1°C, for 54.3 min, with 0.472% Corolase 7089 enzyme. Mathematical models obtained can be applied in the standardization of a process if the degree of hydrolysis and the level of bitterness that best suits a specific purpose are established.

Resumen: Se examinó el efecto de las variables temperatura, tiempo y cantidad de enzima sobre el grado de hidrólisis y el nivel de amargor de los hidrolizados obtenidos de anchoveta (Engraulis ringens). La anchoveta sin cabeza, piel, cola, ni vísceras, fue sometida a dos etapas de hidrólisis enzimática empleando el método de superficie de respuesta. Primero, se evaluó la enzima Corolasa 8000, el proceso fue optimizado (60.1°C, 93.4 min y 1.68 % de enzima), permitiendo maximizar el grado de hidrólisis hasta 12.8 %. Posteriormente, se empleó la enzima Corolasa 7089, se maximizó el grado de hidrólisis (14.8 %) y minimizó el nivel de amargor (3.8) al trabajar a 49.1°C, durante 54.3 min y utilizando 0.472 % de enzima. Los modelos matemáticos obtenidos pueden ser aplicados en la estandarización de un proceso si se establece el grado de hidrólisis y el nivel de amargor que más se adecúa a un propósito específico.

RESUMO: A mangabeira é uma espécie frutífera cujas populações naturais se encontram fragmentadas por ações antrópicas. Desse modo, são necessários estudos sobre a avaliação do impacto da fragmentação sobre a diversidade e estrutura genética dessas populações para o estabelecimento de estratégias de conservação adequadas. No presente estudo, foram utilizados dados de análises com marcadores microssatélites em simulações computacionais para estimar as taxas de migração e autofecundação de seis populações de mangaba. As populações estudadas estão localizadas nos estados nordestinos do Ceará, Pernambuco e Sergipe. Foram testadas diferentes taxas de autofecundação e migração, e selecionada a combinação que apresentou valores de heterozigosidade observada e esperada mais próximos dos obtidos com marcadores microssatélites. Com base nas simulações, a autofecundação foi de 0,3 e a taxa de migração variou de 0,5 a 0,6, valores que podem ter conduzido ao aumento da endogamia e deriva genética, resultando em baixa diversidade genética. Recomenda-se a expansão da área e a redução de perturbações para promover a ocorrência de polinizadores, que desempenham um papel importante no aumento da diversidade genética.

ABSTRACT: Mangaba tree is a fruit tree species whose natural populations are fragmented by anthropic actions. For this reason, studies assessing the impact of fragmentation on the diversity and genetic structure of these populations are required in order to establish suitable conservation strategies. In our study, we used data from analyzes through microsatellite markers in computer simulations to estimate the rates of migration and selfing of six mangaba populations. The studied populations are located in the northeastern states of Ceará, Pernambuco and Sergipe. We tested different selfing and migration rates and selected the combination that showed values of observed and expected heterozygosity closest to those previously obtained with microsatellite markers. According to our simulations, selfing and migration were moderate. This may have led to an increase in inbreeding and genetic drift, resulting in low genetic diversity. We recommend expanding the area and reducing disturbance to promote the occurrence of pollinators, which play an important role in increasing genetic diversity.

Abstract In the last 18 years, Colombia has seen a modest reduction in income inequality compared to other Latin American countries as measured by the Gini index. However, when the scale is reduced and cities are observed, remarkably diverse trajectories are evident: some cities have had a notable reduction, while others have stagnated. This article focuses on four Colombian cities with divergent trajectories in the evolution of inequality. Through an analysis of income distribution using data from the Continuous Household Survey (2002-2005) and the Extensive Integrated Household Survey - GEIH by its Spanish acronym (2008-2018), and following the decomposition methodology suggested by Azevedo et al. (2013), this article focuses on the role of income from pensions and transfers in the variation of the Gini. Administrative records are used to mark the proposed analyses more robust. The results highlight the differentiated role of transfers in reducing inequality and that of pensions in increasing it.

Resumen En los últimos 18 años, Colombia ha reducido modestamente la desigualdad de ingresos medida con el índice de Gini frente a otros países latinoamericanos. Pero cuando se observa el conjunto de sus ciudades saltan a la vista trayectorias diversas: unas la han reducido notablemente, mientras que en otras hay estancamiento. Este artículo se enfoca en 4 ciudades colombianas con trayectorias divergentes en la evolución de la desigualdad. A través de un análisis de la distribución del ingreso, a partir de datos de la Encuesta Continua de Hogares (2002-2005) y la Gran Encuesta Integrada de Hogares (2008-2018), este artículo se centra en el papel de los ingresos por pensiones y por ayudas en la variación del Gini, y utiliza registros administrativos para robustecer los análisis propuestos. Dentro de los resultados se destaca el diferenciado rol de las ayudas en la reducción de la desigualdad, y el de las pensiones en su aumento.

ABSTRACT This paper explores the Argentinean psychologists' perceptions about the objective and exclusive tasks of psychological intervention. In addition, those activities that can be shared with other professionals who are not psychologists are also investigated. This analysis aims to reveal whether psychologists share a clear position on this issue. The development of their field as a profession demands an introspective attitude towards current psychological practice. This will enable to establish precise guidelines and sustain the psychology professional identity. The research is based on the importance of building a strong identity that promotes the general public's trust in psychologists. To this end, a questionnaire with three open-ended questions was used, followed by a thematic categorical content analysis. The results show inconsistencies in the psychologists' answers to the three questions under study. In the end, the lack of consensus in defining an exclusive objective for psychological intervention is concluded.

RESUMEN Este artículo explora las percepciones de psicólogos argentinos sobre el objetivo y las tareas exclusivas de la intervención psicológica, sumado a las actividades compartidas con profesionales que no son psicólogos. El objetivo de este análisis es revelar si los psicólogos comparten una posición clara respecto a este tema. El desarrollo profesional de este campo demanda una actitud introspectiva hacia las actuales prácticas psicológicas, lo que permitirá establecer directrices precisas y sostener una identidad profesional de la Psicología. La investigación se justifica en la importancia de construir una identidad fuerte que promueva la confianza del público en general en los psicólogos. Con este fin, se utilizó un cuestionario con tres preguntas abiertas, seguido por un análisis temático de contenido. Los resultados evidencian respuestas variables por parte de los psicólogos ante las preguntas bajo estudio. Finalmente, se concluye que existe una falta de consenso en la definición de un objetivo exclusivo para la intervención psicológica.

Abstract: In the literature Informed consent (IC) assumptions is well established. However, the different stages and the conditions under which the IC for anesthetic practices is obtained, is scarce. The aim of the present study is to explore the phases and conditions of IC in anesthesiology. Anonymized clinical records of 325 patients submitted to anesthetic procedures at the Institute of Oncology of Porto were analyzed. A total agreement between the anesthetic techniques established in the IC and those performed, was reach with 270 patients. The importance of IC in clinical practice is discussed and an ideal process for IC is argued.

Resumen: El consentimiento informado (CI) está bien establecido en la literatura. Sin embargo, la información sobre las diferentes fases y condiciones en las que se obtiene el CI para las prácticas anestésicas es escasa. El objetivo del presente estudio es explorar las fases y condiciones de obtención de la CI en anestesiología. Se analizaron las historias clínicas anónimas de 325 pacientes sometidos a procedimientos anestésicos en el Instituto de Oncología de Oporto. Se alcanzó una concordancia total entre las técnicas de anestesia establecidas en el CI y las realizadas con 270 pacientes. Se defiende la importancia del CI en la práctica clínica y se discute un proceso ideal para obtenerlo.

Resumo: Na literatura o Consentimento Informado (CI) é bem estabelecido. Contudo, a informação sobre as diferentes fases e as condições em que o CI para práticas anestésicas é obtido, é escassa. O objetivo do presente estudo é explorar as fases e condições da obtenção do CI em anestesiologia. Foram analisados os registos clínicos anónimos de 325 pacientes submetidos a procedimentos anestésicos no Instituto de Oncologia do Porto. Foi alcançado um acordo total entre as técnicas anestésicas estabelecidas no CI e as realizadas, com 270 pacientes. A importância do CI na prática clínica é defendida e discute-se um processo ideal para a obtenção do CI.

Resumen La infección por SARS-CoV-2 se presenta generalmente como un síndrome respiratorio agudo, caracterizado por fiebre, tos, odinofagia y disnea. Sin embargo, desde el comienzo de la pandemia, a fines del año 2019, fueron reportados otros síntomas menos frecuentes, como manifestación inicial de la en fermedad, entre ellos la aparición de hipo (singulto). Se describe el caso de un varón de 62 años de edad con antecedentes de hipertensión arterial, diabetes e insuficiencia cardiaca, que sufrió hipo persistente como primer síntoma de COVD-19, seguido de síntomas respiratorios. Luego de efectuado el diagnóstico de infección por SARS-CoV-2, el paciente fue hospitalizado y recibió el tratamiento correspondiente. El singulto mejoró parcial mente con el uso de fármacos anti-dopaminérgicos (metoclopramida) y desapareció al sexto día de internación. Se requirió la corrección de la glucemia con insulina corriente. Evolucionó favorablemente y fue externado luego de 14 días de hospitalización.

Abstract SARS-CoV-2 infection usually manifests as an acute respiratory syndrome, characterized by fever, cough, sore throat and dyspnea. Nonetheless, since the beginning of the pandemic in December 2019, less frequent initial symptoms were reported, as the sudden appearance of hiccups (singultus). We describe a clinical case of a 62-year-old male with a medical history of arterial hypertension, diabetes and chronic cardiac insufficiency, who complained of persistent hiccups as initial manifestation of COVID-19, followed by respiratory symptoms. After the SARS-CoV-2 infection diagnosis was made, the patient was hospitalized, receiving the corresponding treat ment. The singultus partially improved with dopaminergic antagonists and it disappeared on the sixth day of hospitalization. Glycemic correction with regular insulin was required. He presented a favorable outcome, being discharged after 14 days of hospitalization.

Resumen Introducción: la telemedicina puede mejorar la calidad asistencial y el uso de recursos. La pandemia de COVID-19 ha hecho necesaria su implementación en la práctica habitual. Por ello, un grupo de endocrinólogos de la Comunidad Valenciana, Murcia y Baleares creó un comité para su desarrollo. Objetivos: establecer unas recomendaciones para mejorar la calidad de la consulta de patología nutricional y diseñar unos indicadores para su gestión. Metodología: se siguió la metodología Delphi con participación de 13 profesionales con el fin de alcanzar el mayor consenso. Para ello se circuló un cuestionario de 16 puntos en 3 rondas: en la primera se estableció el grado de consenso; en la segunda, los participantes tuvieron acceso a los resultados de la primera y respondieron a las cuestiones planteadas. Se consideró que había acuerdo si el consenso era ≥ 75 % de los participantes, y que existía acuerdo fuerte si este era ≥ 90 %. Además, se estableció la temática de los indicadores de calidad. En la tercera se desarrollaron dichos indicadores. Resultados: tras 3 rondas y una reunión de revisión se establecieron los sobre aspectos organizativos 5 recomendaciones con acuerdo fuerte; sobre los aspectos administrativos, 6 recomendaciones con acuerdo fuerte y 4 con acuerdo. Se seleccionaron 8 indicadores de calidad que se desarrollaron en formato de fichas. Conclusiones: este documento recopila una serie de recomendaciones sobre cuestiones, necesidades y requisitos a tener en cuenta para una consulta telemática de calidad al paciente con patología nutricional. Así mismo, se han desarrollado unos indicadores para mejorar la calidad asistencial.

Abstract Introduction: telemedicine can improve the standards of clinical care and use of resources. The COVID-19 pandemic has required its implementation in routine practice. For this reason, a group of endocrinologists from Valencia, Murcia and the Balearic Islands created a committee for its development. Objectives: to establish recommendations in order to improve the quality of consultation in nutritional disorders, and to create indicators for its management. Methodology: the Delphi methodology was followed with the participation of 13 professionals in order to reach the widest consensus. A 16-item questionnaire was distributed within 3 rounds: in the first round, the degree of consensus was established; in the second round, the participants responded to the posed questions after having access to the first-round results. Agreement was considered if ≥ 75 % participants reached consensus, while strong agreement was considered if consensus was reached by ≥ 90 %. In addition, quality indicators were established. In a third round, these indicators were developed. Results: after 3 rounds and a revision 5 recommendations with strong agreement were established based on organizational aspects. Regarding administrative aspects, 6 recommendations with strong agreement were established while 4 recommendations reached the level of agreement. Eight quality indicators were selected and developed. Conclusions: this document compiles a list of recommendations about needs and requirements to be taken into account for a quality telematic consultation in patients with nutritional disorders. In addition, health care quality indicators have been created and developed.

Abstract Introduction: Due to lack of data, the management of air leaks in children and adolescents is based on protocols for adults. In this study, the authors review and report their institutional experience in the area. Methods: Retrospective and descriptive study of spontaneous air leak cases diagnosed in a Portuguese Pediatric Emergency Department (PED) between January 2007 and December 2018. Results: Twenty-one episodes of spontaneous air leak were diagnosed in 16 patients (87.5% male), with a mean (± standard deviation) age of 14.3 (± 5.1) years. Eighteen cases of spontaneous pneumothorax were reported, fifteen of which primary (eleven first-time episodes and four recurrences) and three secondary (two first-time episodes and one recurrence) to asthma. Three cases of pneumomediastinum with subcutaneous emphysema were identified, two of which related to infection. Smoking habits and/or recent physical activity were major triggers. Pleuritic chest pain was the most frequent symptom on admission. Conservative treatment was the only approach used in six cases. Most cases required thoracic drainage, five of which required surgical intervention. Discussion: This study shows similar demographic features, triggers, and clinical presentation to those reported in the literature for these cases. The management of the condition was based on recommendations established for adults. Conclusion: Although spontaneous air leak is an uncommon condition, it is a reality in PED. Prospective studies in pediatric age are required to develop adequate recommendations for children and adolescents.

Resumo Introdução: Devido à escassez de dados em idade pediátrica, a abordagem da síndrome de fuga de ar em crianças e adolescentes baseia-se em protocolos estabelecidos para adultos. Neste estudo, os autores descrevem a sua experiência institucional na área. Métodos: Estudo retrospetivo descritivo de episódios espontâneos de síndroma de fuga de ar diagnosticados num Serviço de Urgência Pediátrico (SUP) em Portugal entre 1 de janeiro de 2007 e 31 de dezembro de 2018. Resultados: Foram diagnosticados vinte e um episódios de síndroma de fuga de ar em dezasseis crianças, 87.5% das quais do género masculino, com uma idade média (± desvio padrão) de 14,3 (± 5,1) anos. Foram reportados 18 casos de pneumotórax espontâneo, quinze dos quais primários (onze primeiros episódios e quatro recorrências) e três secundários (dois primeiros episódios e uma recorrência) a asma. Foram contabilizados três episódios de pneumomediastino com enfisema subcutâneo, dois dos quais secundários a infeção. A exposição ativa a fumo de tabaco e/ou esforço físico recente foram fatores etiopatogénicos preponderantes na maioria dos casos. Dor torácica pleurítica foi o sintoma mais frequente na apresentação clínica. A terapêutica conservadora foi a única abordagem utilizada em seis casos, com os restantes a necessitar de drenagem transtorácica. Cinco episódios exigiram intervenção cirúrgica. Discussão: Este estudo evidencia fatores demográficos e etiopatogénicos e apresentação clínica semelhantes aos descritos na literatura para estes casos. Relativamente à abordagem, foram seguidas as recomendações existentes para a idade adulta. Conclusão: Apesar de pouco frequente, a síndrome de fuga de ar é uma realidade no SUP. São necessários estudos prospetivos especificamente em idade pediátrica para permitir o desenvolvimento de recomendações adequadas às particularidades desta faixa etária.

RESUMEN Después de la cirugía de cáncer de mama, las mujeres pueden desarrollar deficiencias musculoesqueléticas que afectan los movimientos de las extremidades superiores y reducen la calidad de vida. El objetivo de esta investigación fue analizar el efecto de la cirugía unilateral del cáncer de mama sobre la cinemática del hombro y la actividad eléctrica de los músculos trapecio superior, deltoides medio y pectoral mayor. Se incluyeron ocho participantes diestras, edad promedio 46.5 ± 5.45 años y masa promedio 71.21 ± 13.33 kg, con cirugía de cáncer de mama unilateral, sin reconstrucción mamaria y sin síntomas de linfedema. Los movimientos de flexión-extensión y abducción-aducción del hombro se evaluaron con cámaras infrarrojas y se midió la actividad eléctrica mediante electromiografía de superficie. El análisis estadístico de los ángulos de dirección mostró una reducción significativa del movimiento de flexión-extensión y abducción-aducción en el lado afectado en la mayoría de las participantes (p<0.05). La actividad eléctrica muscular no presentó diferencia significativa entre los dos lados para los movimientos de flexión-extensión y abducción-aducción (p>0.05). Los resultados sugieren que el procedimiento quirúrgico podría comprometer el rango de movimiento del lado afectado. Además, esta investigación contribuye a esclarecer el efecto del procedimiento quirúrgico en el rango de movimiento de los miembros superiores.

ABSTRACT After breast cancer surgery, women might develop musculoskeletal impairments that affect movements of the upper limbs and reduce the quality of life. The objective of this research was to analyze the effect of the unilateral breast cancer surgery on the kinematics of the shoulder and the electrical activity of the upper trapezius, middle deltoid and pectoralis major muscles. Eight right-handed female participants, mean age 46.5 ± 5.45 years and mean body mass 71.21 ± 13.33 kg, with unilateral breast cancer surgery, without breast reconstruction and without lymphedema symptoms were included in the research. Flexion-extension and abduction-adduction movements of the shoulder were evaluated with infrared cameras and the electrical activity was measured using surface electromyography. The statistical analysis of the direction angles showed a significant reduction of the flexion-extension and abduction-adduction movements in the affected side in most of the participants (p<0.05). The muscle electrical activity did not present a significant difference between the two sides for the flexion-extension and abduction-adduction movements (p>0.05). The results suggest that the surgical procedure could compromise the range of motion of the affected side. Furthermore, this research contributes to clarify the effect of the surgical procedure in the range of motion of the upper limbs.

Abstract The clinical evaluation of the patient with COVID-19 allows better care, application of safety criteria and preventive measures. The disease progresses from mild to severe and critical. In this work, is evaluated in patients with COVID-19 clinical format to identify moderate to severe stages of the disease. Following a cohort of male and female patients over 18 years of age admitted to the Infectology Service of the General Hospital of Mexico. Each patient is studied using the “COVID-19 Infectology” clinical format and in the first 24 hours of admission, a real-time RT-PCR molecular test is performed for SARS-CoV-2 infection. 65 patients classified as severe COVID-19 were studied, the RT-PCR was positive in 60 patients and negative in 5, clinical data did not differ from the positive ones and the 5 negative were considered false negative cases of the molecular test. There were no differences between positives and negatives with Fisher’s test, and no difference in age, comorbidities, or prognostic evaluation with Student’s t test. The conclusion is that the clinical format “COVID-19 Infectology” allows to recognize the cases and identify those that are in a severe evolution.

ABSTRACT Background: Left ventricular (LV) stroke volume (SV) is ejected by a combination of longitudinal shortening (LS), circumferential shortening (CS) and ventricular twist. Longitudinal shortening is caused by the motion of the mitral annulus towards the apex, causing wall thickening (CS and radial thickening), reduction of the ventricular cavity and LV ejection. The role of LS in aortic stenosis (AS) is not defined. Objective: The aim of this study was to analyze the contribution of LS and CS to SV and left ventricular ejection fraction (LVEF) in severe AS. Methods: The study included 152 patients (70 ± 12 years, 64 women) with severe AS (valve area <1 cm2) studied by cardiac Doppler echocardiography. The contribution of LS to SV was evaluated by considering the base of the heart as a cylinder, with volume = base x height; the base was assumed as a circle with radius equal to (systolic septal thickness + systolic posterior wall thickness + end-systolic diameter)/2; and height, as an average of the mitral lateral, septal, anterior and inferior annulus systolic excursion. The contribution of CS to SV was estimated as: SV - contribution of LS to SV. Both contributions were expressed in absolute form and as SV percentages (%SV-LS and %SV-CS). Results: Longitudinal shortening contributed with approximately 2 thirds of SV (68 ± 18 %) and CS with the rest. SV-LS correlated inversely with SV (r = - 0.45 p <0.001) and SV-CS had direct correlation (r = 0.45 p <0.001). The contribution of %SV-LS was greater in patients with LVEF <50%. Percent SV-LS correlated inversely with relative wall thickness (RWT) (r = 0.32, p <0.01). Conclusion: The contribution of LS to SV is greater than that of CS, and correlates inversely with SV. Percent SV-LS is higher in patients with EF <50%. The aforementioned findings could have implications when considering cut-off points for longitudinal function indices (strain) without considering LVEF and/or ventricular geometry.

RESUMEN Introducción: El volumen sistólico (VS) del ventrículo izquierdo (VI) es eyectado por una combinación de acortamiento longitudinal (AL), circunferencial (AC) y giro ventricular. El AL se produce por el movimiento del anillo mitral hacia el ápex, lo que ocasiona engrosamiento de la pared (AC y radial), reducción de la cavidad ventricular y eyección del VS. No está definido el rol del AL en la estenosis aórtica (EAo). Objetivo: Analizar la contribución del AL y AC al VS y la fracción de eyección ventricular izquierda (FEVI) en la EAo grave. Materiales y métodos: Se estudiaron 152 pacientes (70 ± 12 años, 64 mujeres) con EAo grave (área valvular <1 cm2) con ecoDoppler cardíaco. La contribución del AL al VS se evalúo considerando a la base del corazón como un cilindro., con volumen = base × altura; la base, un círculo con radio igual a (espesor septal en sístole + espesor pared posterior en sístole + diámetro de fin de sístole)/2; y la altura, el promedio de la excursión sistólica del anillo lateral, septal, anterior e inferior. La contribución del AC al VS se estimó como: VS - contribución del AL al VS. Ambas contribuciones se expresaron en forma absoluta y como porcentajes del VS (% VS-AL y % VS-AC). Resultados: El AL contribuyó con 2 tercios aproximadamente al VS (68 ± 18 %) y el AC con el resto. El VS-AL correlacionó inversamente con el VS (r = - 0,45 p <0,001) y el VS-AC tuvo correlación directa (r = 0,45 p <0,001). La contribución del % VS-AL fue mayor en los pacientes con FEVI <50%. El % VS-AL correlacionó inversamente con el EPR (r = - 0,32 , p <0,01). Conclusión: La contribución del AL al VS es mayor que la del AC, y correlaciona inversamente con el VS. Es mayor el % VS-AL en los pacientes con FEVI <50%. Los hallazgos mencionados podrían tener implicancias al considerar puntos de corte para los índices de función longitudinal (strain) sin considerar la FEVI y/o geometría ventricular.

Abstract Introduction: In patients with severe aortic stenosis, low-gradient, low-flow and preserved ejection fraction (≥ 50%) (LG/LFp) the decrease in stroke volume index (Svi) could be due to diminished contractility. The aim of this study was to analyze if low Svi in patients with LG/LFp is related to a decrease in contractility. Methods: Thirty patients with LF/LGp and 63 normal subjects were studied with Doppler echocardiography. The level of contractility (LC) was assessed by mean of midwall shortening fraction (mSF) - end-systolic stress (ESS) relationship. Relative wall thickness (RWT) and aortic valve area (AVA) were calculated. Results: LC was diminished in 40% of LF/LGp patients. By bivariate analysis LC correlated with RWT and mSF. Svi correlated with AVA but not with LC. In multiple linear regression RWT and mSF were predictors independents of LC. About Svi, AVA was the only predictor independent variable. Conclusion: Decreased contractility was present in 40% of LF/LGp patients despite preserved ejection fraction. LC had correlation with mSF and EPR. Low Svi appears to be related to aortic stenosis severity rather than low contractility.

Resumen Introducción: En la estenosis aórtica grave con gradiente bajo, fracción de expulsión preservada (≥ 50%) y flujo bajo (GB/FBp), la disminución del índice de volumen sistólico (IVS) podría deberse a una reducción de la contractilidad. El objetivo de este estudio fue analizar si la disminución del IVS se relaciona con la de la contractilidad. Métodos: Se estudió a 30 pacientes con GB/FBp y 63 individuos normales con Doppler cardíaco. El nivel de contractilidad (NC) se valoró mediante la relación entre la fracción de acortamiento mesoparietal (FAm) y el estrés de fin de sístole (EFS). Se calculó el espesor parietal relativo (EPR) y el área valvular aórtica (AVA). Resultados: El NC estuvo disminuido en el 40% de los pacientes con GB/FBp. En el análisis bivariado, el NC se correlacionó con el EPR y la FAm, pero no con el IVS, el cual mantuvo correlación con el AVA. En el análisis de regresión lineal múltiple, el EPR y la FAm fueron variables predictoras independientes del NC y el AVA del IVS. Conclusión: La contractilidad estuvo deprimida en el 40% de los pacientes con GB/FBp a pesar de tener fracción de expulsión preservada. El NC se correlacionó con el acortamiento circunferencial (FAm) y la geometría ventricular (EPR). La disminución del IVS que caracteriza a los pacientes con GB/FBp parece vincularse más con la gravedad de la estenosis aórtica y menos con la disminución de la contractilidad.

ABSTRACT Chitosan and its derivatives are natural compounds that have potential in agriculture with respect to controlling one of rice diseases; blast (Pyriculariaoryzae) of great importance worldwide. In general, this disease is controlled with synthetic fungicides belonging to the benzimidazole group; however, their use has generated adverse results to the environment together with the low sensitivity of the fungus to them. This article provides a review of published research on chitosan, its physicochemical characteristics, general information on P. oryzaefungus, the chitosan fungicidal action and its derivatives in research carried out in vitro and in situ on this fungus and the possible mechanisms of action of this compound.

RESUMEN El quitosano y sus derivados son compuestos naturales que tienen potencial en la agricultura con respecto a controlar una de las enfermedades del arroz; la piriculariosis (Pyricularia oryzae) de gran importancia a nivel mundial. En general, esta enfermedad se controla con fungicidas sintéticos pertenecientes al grupo de los benzimidazoles; sin embargo, su uso ha generado resultados adversos al medio ambiente aunado a la poca sensibilidad del hongo hacia ellos. En este artículo, se proporciona una revisión de investigaciones publicadas acerca del quitosano, sus características fisicoquímicas, generalidades del hongo P. oryzae, la acción fungicida del quitosano y sus derivados en investigaciones llevadas a cabo in vitro e in situ sobre este hongo y, en general, los posibles mecanismos de acción de este compuesto.

ABSTRACT Kidney transplantation of organs from adult donors implanted into a narrow anatomical cavity in underweight pediatric patients offers significant medical and surgical challenges to be considered. In this publication we report the first case in Paraguay of a kidney with two renal arteries, grafted to the aorta and inferior vena cava within the abdominal cavity, on a 12 kilogram pediatric patient, evaluating the medical, anatomical and surgical conditions faced, as well as the treatment options instituted to successfully carry out this procedure.

RESUMEN El trasplante renal de órganos provenientes de donantes adultos implantados en una cavidad anatómica estrecha en pacientes pediátricos de bajo peso, ofrece importantes desafíos médicos y quirúrgicos a ser considerados. En esta publicación reportamos el primer caso en el Paraguay de un riñón con dos arterias renales injertado a la aorta y vena cava inferior, dentro de la cavidad abdominal de un paciente pediátrico de 12 kilogramos de peso, evaluando las dificultades médicas, anatómicas y quirúrgicas enfrentadas, así como las opciones de tratamiento instituidas para llevar a cabo este procedimiento de manera exitosa.
