ABSTRACT Background: A severe traumatic brain injury is a frequent pathology, it implies a non-negligible morbidity and mortality. Hypernatremia could be considered a factor associated with poor prognosis in order to optimize treatment in the group of high-risk patients. We aim to determine if hypernatremia is a prognostic factor of mortality in patients with severe traumatic brain injury in a public hospital which is a local reference center in trauma. Material and Methods: We performed a non-probabilistic sampling of adult patients of both sexes with severe traumatic brain injury from the Intensive Care Unit of the Hospital Regional Docente de Trujillo, Perú during the 2015-2018 period. Results: Mortality in the group with and without hypernatremia was 38% and 6% respectively (p=0.001). The difference in natremia mean values between the group with and without mortality was 21,86 (95% CI 18.2-25.5) (p=0.000). In the multivariate analysis, we found hypernatremia with adjusted OR of 16.73 (95% CI 1.96-142.82) (p=0.01). The ROC curve showed adequate performance to predict mortality, with an AUC 0.878 (95% CI 0.77-0.97) (p=0.00). Conclusion: Hypernatremia is a prognostic factor of mortality in patients with severe traumatic brain injury.

RESUMEN Introducción: El trauma craneoencefálico severo es una patología frecuente, conlleva una morbimortalidad no despreciable. La hipernatremia podría considerarse un factor asociado a mal pronóstico con el fin de optimizar la terapéutica en el grupo de pacientes de alto riesgo. Se determinó si la hipernatremia es un factor pronóstico de mortalidad en pacientes con trauma craneoencefálico en un hospital público que es centro de referencia local en trauma. Material y Métodos: Se realizó muestreo no probabilístico de pacientes con trauma encefalocraneano severo, adultos, ambos sexos de la Unidad de Cuidados Intensivos del Hospital Regional Docente de Trujillo, Perú en el periodo del 2015-2018. Resultados: La mortalidad en el grupo con y sin hipernatremia fue de 38% y 6% respectivamente (p=0,001). La diferencia de medias de la natremia entre el grupo con y sin mortalidad fue de 21,86 (IC 95% 18,2-25,5) (p=0,000). En el análisis multivariado encontramos a la hipernatremia con OR ajustado de 16,73 (IC 95% 1,96-142,82) (p=0,01). La curva ROC mostró un rendimiento adecuado para predecir la mortalidad, con un AUC de 0,878 (IC 95% 0,77-0,97) (p=0,000). Conclusión: La hipernatremia es un factor pronóstico de mortalidad en pacientes con trauma encefalocraneano severo.

ABSTRACT Introduction: The aim of this study was to synthesize the evidence on the causes of sudden cardiac death (SCD) associated with hypokalemia in individuals ≥15 to <65 years of age without structural heart disease. Case report: We reported a case and collected SCD cases following the PECO strategy. We conducted a bibliographic search of the literature published up to November 30, 2021. We included 25 cases. The mean age was 33.8 ± 2.4 years. Of the total cases, 60% were male and 60% corresponded to acquired etiologies. Thyrotoxicosis was the etiology of 8% of cases. The QTc interval was prolonged in 90% of the cases in which it could be recorded. Sixty percent of cases occurred out of hospital. Only 24% of episodes resulted in death. Conclusions; Thyrotoxicosis is a rare cause of hypokalemic sudden cardiac arrest (SCA). Hypokalemic SCA seems to be associated with better clinical results compared to SCA associated with hyperkalemia or normokalemia.

RESUMEN Introducción: El objetivo de este estudio fue sintetizar la evidencia sobre las causas de muerte súbita cardiaca (MSC) asociada a hipokalemia en individuos de ≥15 a <65 años de edad sin cardiopatía estructural. Reporte de caso: Reportamos un caso y recopilamos casos de MSC siguiendo la estrategia PECO. Realizamos una búsqueda bibliográfica hasta el 30 de noviembre del 2021. Incluimos 25 casos. La edad media fue 33,8 ± 2,4 años. Del total de casos, 60% fueron varones y 60% correspondieron a etiologías adquiridas. La tirotoxicosis fue la etiología del 8% de casos. El intervalo QTc estuvo prolongado en 90% de casos en los que se pudo registrar. El 60% de casos fueron extrahospitalarios. Solo el 24% de los episodios resultaron en muerte. Conclusiones: La tirotoxicosis es una causa rara de parada cardiaca súbita (PCS) hipokalémica. La PCS parece asociarse a mejores clínicos comparada con la PCS asociada a kiperkalemia o normokalemia.

RESUMEN Introducción: Los linfomas gástricos primarios (LGP) representan menos del 5% de los linfomas no Hodgkin (LNH), siendo la gran mayoría de estas neoplasias de estirpe de células B de alto grado. Los LGP de células T son muy raros y usualmente se asocian a infección por HTLV-1 en regiones endémicas. Material y método: Describimos el caso de una paciente mujer de mediana edad que se presentó con neumoperitoneo debido a úlcera gástrica perforada, síndrome consuntivo y lesiones cutáneas y orales. Resultados: La histopatología e inmunohistoquímica confirmaron LGP de células T con metástasis cutánea. La serología para HTLV-1, virus del Epstein-Barr y VIH fueron negativas. Conclusión: La finalidad de este reporte es dar a conocer esta forma de presentación sumamente rara de LGP.

ABSTRACT Background: Primary gastric lymphomas (PGL) account for less than 5% of non-Hodgkin lymphomas (NHL). The vast majority of PGL are high-grade B-cell lymphomas. PGL are very rare and are usually associated with HTLV-1 infection in endemic regions. Methods: We describe the case of a middle-aged female patient who presented pneumoperitoneum due to a perforated gastric ulcer, wasting syndrome, and skin and oral lesions. Results: Histopathology and immunohistochemistry confirmed primary gastric T-cell lymphoma with skin metastasis. The serology for HTLV-1, Epstein-Barr virus and HIV were negative. Conclusion: The aim of this report is to present this extremely rare presentation of PGL.

ABSTRACT Preoperative tests are very common in daily clinical practice. However, ordering preoperative tests is not generally based on the best scientific evidence. When preoperative tests are ordered without justification, they may generate false-positive results, cause to repeat or order new tests and consultations, prolong hospital stay, result in problems to the patient (more risks, confusion and anxiety), and increase costs. A systematic review and cost-effectiveness analysis of preoperative tests for the most frequent low- and intermediate-risk non-cardiac elective surgeries performed at the Hospital Regional Lambayeque were conducted. According to the cost-effectiveness analysis, the annual cost of unnecessary preoperative tests amounts to S/ 62,778.00. However, if preoperative tests were adequately ordered and NICE guidelines were followed, their cost would decrease in 38.34 % (i.e., S/ 138.40 for each patient). The results show that preoperative tests should not be ordered for low- and intermediate-risk non-cardiac elective surgeries on a routinely basis. A thorough preoperative medical evaluation would safely decrease the need of other tests for this type of surgery.

RESUMEN Los exámenes preoperatorios son parte habitual en la práctica clínica. Sin embargo, su requerimiento no se sustenta siempre en la mejor evidencia científica. Cuando se solicitan sin justificación pueden generar resultados falsos positivos, la necesidad de repetir o solicitar nuevos exámenes e interconsultas, prolongar la estancia hospitalaria, ocasionar problemas al paciente (mayor riesgo, confusión y ansiedad) e incremento de los costos. Se realizó una revisión sistemática y análisis costo-efectividad de los exámenes preoperatorios en las cirugías electivas no cardíacas de riesgo bajo e intermedio más frecuentes realizadas en el Hospital Regional Lambayeque. Según el análisis costo-efectividad, el monto anual de los exámenes preoperatorios innecesarios sería de S/ 62 778, pero si se solicitaran de manera adecuada y se siguieran las pautas establecidas en la Guía NICE, este costo se reduciría en 38,34 % (es decir S/ 138,40 por cada paciente). Los resultados muestran que no se debe solicitar exámenes preoperatorios de manera rutinaria para las cirugías no cardíacas electivas de riesgo bajo e intermedio y que una minuciosa evaluación médica preoperatoria reduciría, de manera segura, la necesidad otras pruebas para este tipo de cirugías.

ABSTRACT SARS-CoV-2 infection mainly affects the respiratory system; but, multiple extrapulmonary manifestations have also been observed in various systems and organs. The epithelial cells of the digestive tract and hepatobiliary tree express ECA2 and TMPRSS2 necessary for viral entry, on the other hand, viral RNA has been detected in stool samples, even after having become negative in respiratory samples. The presence of clinical digestive manifestations is associated with a higher viral load and can sometimes be the only manifestations of COVID-19, even in the absence of respiratory symptoms. The most common symptoms are anorexia and diarrhea, nausea, vomiting, and abdominal pain, the latter being characteristic of seriously ill patients. Gastrointestinal bleeding, hemorrhagic colitis, or acute hepatitis are very rare. In a patient with gastrointestinal manifestations, COVID-19 should be considered within the differential diagnosis.

RESUMEN La infección por SARS-CoV-2 afecta principalmente el sistema respiratorio; pero, también se han observado múltiples manifestaciones extrapulmonares en diversos sistemas y órganos. Las células epiteliales del tubo digestivo y árbol hepatobiliar expresan ECA2 y TMPRSS2 necesarios para el ingreso viral, por otro lado, el ARN viral ha sido detectado en muestras de heces, incluso después de haberse negativizado en muestras respiratorias. La presencia de manifestaciones clínicas digestivas se asocia a mayor carga viral y en ocasiones pueden ser las únicas manifestaciones de COVID-19, incluso en ausencia de síntomas respiratorios. Los síntomas más comunes son anorexia y diarrea, náuseas, vómitos y dolor abdominal, siendo este último característico de pacientes gravemente enfermos. Es muy poco frecuente la presencia de hemorragia gastrointestinal, colitis hemorrágica o hepatitis aguda. Ante un paciente con manifestaciones gastrointestinales debe considerarse a COVID-19 dentro del diagnóstico diferencial.

ABSTRACT Introduction . Infections caused by SARS-CoV-2 may produce a severe inflammatory response, sepsis, respiratory failure, organ dysfunction, and death. From a pathophysiological point of view, it is plausible that glucocorticoids could be a reasonable therapeutic option for Covid-19. However, the evidence supporting the benefit of corticosteroids on mortality and other clinical outcomes is not conclusive. Objective : determinate if there exist evidence supporting corticosteroids therapy in patients with severe Covid- 19 and Acute Respiratory Distress Syndrome (ARDS). Material and method : This study was performed in concordance with PRISMA guidelines. According to the PICO strategy, Population were patients with severe Covid-19 and/or ARDS; Intervention, parenteral corticosteroid administration; Comparator, patients with severe Covid-19 and/or ARDS with standard treatment (without corticosteroids); and Outcome, mortality and other clinical outcomes. Search strategy was performed in the data bases PubMed, Medline, Cochrane, Embase, and LILACS, using terms related with the PICO question; from January 1 to august 31 (2020). A total of 287 articles were initially retrieved. Of these, we finally selected only 9 article which fulfilled with PICO and selection criteria. Results : According to our study, parenteral corticosteroids reduce mortality in patients with Severe Covid-19 and/or ARDS [OR 0.83 (IC 95% 0.63-1.08)]. Conclusion : the systematic review shows that parenteral corticosteroids could reduce the mortality of patients with severe Covid-19 and ARDS.

RESUMEN Introducción . La infección causada por el SARS-CoV-2 puede producir un cuadro inflamatorio severo, sepsis, falla respiratoria, disfunción orgánica, y muerte. Es plausible fisiopatológicamente, que los corticoides sean una opción terapéutica razonable para Covid-19. Sin embargo, la evidencia del efecto benéfico de los corticoides sobre la mortalidad no es categórica. Objetivo : determinar si existe evidencia que avale el uso de corticoides parenterales en Covid-19 grave y Síndrome de Distrés Respiratorio Agudo (SDRA). Material y método : Este estudio se desarrolló en concordancia con la guía PRIMA. Según la estrategia PICO, nuestra Población fueron los pacientes con Covid-19 grave y/o SRDA; la Intervención, el uso de corticoides parenterales; el Comparador, los pacientes con Covid-19 severo con tratamiento estándar (sin corticoides); y el Outcome, la mortalidad y otros resultados clínicos. La búsqueda se realizó en las bases de datos PubMed, Medline, Cochrane, Embase, y LILACS usando los términos relacionados con nuestra pregunta PICO, entre el de 01 enero al 31 de agosto del 2020. Se logró recopilar un total de 287 artículos, de los que finalmente se seleccionaron 9 que cumplieron con los criterios PICO y de selección. Resultados : Según nuestro estudio, los corticoides parenterales reducen la mortalidad en pacientes con Covid-19 severo y SRDA [OR 0.83 (IC 95% 0.63-1.08)]. Conclusión : la revisión sistemática muestra que los corticoides parenterales podrían disminuir la mortalidad de pacientes con Covid-19 severo y ARDS.

ABSTRACT Yellow fever (YF) and leptospirosis are under-diagnosed endemic zoonoses of the tropical regions of Africa and South America. Both may be clinically indistinguishable and present as an acute icterohemorrhagic febrile syndrome. We report the case of a 20-year-old male from the department of Amazonas who presented with nine days of disease characterized by multiorgan failure (neurological, renal, hepatic, respiratory, and hematological involvement). He received antibiotic treatment, as well as, transfusion, dialysis, hemodynamic, and ventilatory support. Despite the severity of the clinical condition, he evolved favorably. YF was confirmed by Rt-PCR and positive serology was obtained for leptospira by ELISA and microagglutination. However, from a laboratory point of view, real co-infection by yellow fever and leptospira could not be demonstrated. This case of severe YF with non-fatal outcome emphasizes the importance of adequate syndromic diagnosis, and early and aggressive supportive treatment that can save a patient's life.

RESUMEN Fiebre amarilla (FA) y leptospirosis son zoonosis endémicas subdiagnosticadas de las regiones tropicales de África y Sudamérica. Ambas, pueden ser clínicamente indistinguibles y presentarse como síndrome febril icterohemorrágico agudo. Reportamos el caso de un varón de 20 años, procedente del departamento de Amazonas que se presentó con nueve días de enfermedad caracterizada por falla multiorgánica (compromiso neurológico, renal, hepático, respiratorio, y hematológico). Recibió tratamiento antibiótico, soporte transfusional, dialítico, hemodinámico, y ventilatorio; y a pesar de la gravedad del cuadro clínico, evolucionó favorablemente. Se confirmó FA por Rt-PCR y se obtuvo serología positiva para leptospira por ELISA y microaglutinación. Sin embargo, no se pudo demostrar, desde el punto de vista laboratorial, coinfección real por FA y leptospira. Este caso de FA severa con desenlace no fatal enfatiza la importancia del diagnóstico sindrómico adecuado, y un tratamiento de soporte precoz y agresivo que puede salvar la vida del paciente.

ABSTRACT Plague is a re-emerging disease caused by the bacteria Yersinia pestis. Humans usually get the disease through the bite of an infected flea. Plague is a fulminant systemic disease, with pneumonic plague being the most lethal form. Late diagnosis is one of the main causes of mortality and spread of the disease, as it limits the effectiveness of control measures. We present the case of a 42-year-old male, who had previously traveled to an endemic plague area and then presented hyperpyrexia, hypotension, and inflammatory inguinal adenopathy. Despite the very characteristic clinical picture, nobody (before admission to our hospital) suspected plague. An effective combination of antibiotics and intensive treatment was initiated only on the fifth day of illness. The patient went into septic shock, respiratory failure, and death. Plague was confirmed by polymerase chain reaction (PCR). This case emphasizes the importance of having a high suspicion rate for plague.

RESUMEN La peste es una enfermedad reemergente causada por Yersinia pestis. Los humanos generalmente adquieren la enfermedad por picaduras de pulgas. La peste es una enfermedad sistémica fulminante, siendo la peste neumónica la forma más letal. El diagnóstico tardío es una de las principales causas de mortalidad y diseminación de la enfermedad, dado que limita la efectividad de las medidas de control. Presentamos el caso de un varón de 42 años, que previamente había viajado a una zona endémica de peste, y luego presentó hiperpirexia, hipotensión, y adenopatía inguinal inflamatoria. A pesar del cuadro clínico muy característico, nadie (antes del ingreso a nuestro hospital) sospechó peste. Se inició una combinación antibiótica efectiva y tratamiento intensivo recién al quinto día de enfermedad. El paciente evolucionó con shock séptico, falla respiratoria, y muerte. Se confirmó peste por reacción en cadena de polimerasa (PCR). Este caso enfatiza la importancia de tener un alto índice de sospecha para peste.
