Resumo O objetivo deste artigo é analisar a implantação do Programa de Promoção da Alimentação Adequada e Saudável na Atenção Primária à Saúde em municípios de Minas Gerais. Pesquisa de Métodos Mistos com Estratégia Incorporada Concomitante de dados. Na abordagem quantitativa utilizou-se pesquisa avaliativa na perspectiva da análise de implantação. Na abordagem qualitativa realizou-se estudo exploratório-descritivo utilizando a análise de conteúdo. Os dados qualitativos e quantitativos foram incorporados para análise a partir de sua imbricação. O Grau de Implantação do programa foi 92,5% representando uma implantação adequada. Na imbricação dos dados, a metodologia das ações propostas no programa e a realização da atividade de Educação Permanente foram pontos fortes convergentes que emergiram. Recursos Humanos insuficientes para planejamento e execução do programa e falta de cozinha para oficina culinária foram apontados como desafios. A implantação do programa foi adequada e poderá ser estendida para outros municípios brasileiros, de forma a apoiar os profissionais a estruturar intervenções de promoção da alimentação adequada e saudável na rotina de trabalho na Atenção Primária.

Abstract This article aims to analyze the implementation of an Adequate and Healthy Food Promotion Program in Primary Health Care in the municipalities of Minas Gerais. Mixed Methods Research with Data Concomitant Embedded Strategy. Evaluative research from the perspective of the implementation analysis was employed in the quantitative approach. An exploratory-descriptive study was conducted using content analysis in the qualitative approach. Qualitative and quantitative data were incorporated for analysis from their imbrication. The Program Implementation Level (IL) was 92.5%, representing an adequate implementation. The methodology of the actions proposed in the program and the implementation of Continuing Education were convergent strengths that emerged from data imbrication. The need for more human resources for planning and implementing the program and the lack of a kitchen for cooking workshops were identified as challenges. The Program’s implementation was adequate and can be extended to other Brazilian municipalities to help professionals structure interventions to promote adequate and healthy food in the Primary Care work routine.

ABSTRACT Sweet corn is susceptible to the attack of various pathogens that affect its metabolism and compromise its quality and production. This study aimed to identify the causal agent of stem rot in sweet corn plants under greenhouse conditions. The identity of the pathogenic isolate was confirmed by sequencing of internal transcribed spacer (ITS1/ITS4), beta-tubulin (BT2A/BT2B), calmodulin (CL1/CL2) and translation elongation factor 1α (TEF1). The morphological and sequencing characteristics showed that Fusarium sacchari, which belongs to the Fusarium fujikuroi complex, is directly associated with the symptoms observed in the field. This fungal isolate has never been associated with diseases in Zea mays (saccharata group) in Brazil; therefore, this is the first report of the fungus infecting sweet corn in a cultivated area.

RESUMO O milho doce está sujeito ao ataque de vários patógenos que afetam o seu metabolismo e comprometem a sua qualidade e produção. Objetivou-se identificar o agente causal da podridão do colmo em plantas de milho doce, em casa-de-vegetação. A identidade do isolado patogênico foi confirmada pelo sequenciamento do espaço interno transcrito (ITS1/ITS4), beta-tubulina (BT2A/BT2B), calmodulina (CL1/CL2) e fator de elongação da tradução 1α (TEF1). As características morfológicas e de sequenciamento revelaram que Fusarium sacchari, pertencente ao complexo Fusarium fujikuroi, está diretamente associado aos sintomas observados em campo. Esse isolado fúngico nunca foi associado a doenças em Zea mays (grupo saccharata) no Brasil; portanto, este é o primeiro relato do fungo infectando milho doce em área de cultivo.

Abstract Objective To evaluate clinical characteristics and outcomes of COVID-19 patients infected with HIV, and to compare with a paired sample without HIV infection. Methods This is a substudy of a Brazilian multicentric cohort that comprised two periods (2020 and 2021). Data was obtained through the retrospective review of medical records. Primary outcomes were admission to the intensive care unit, invasive mechanical ventilation, and death. Patients with HIV and controls were matched for age, sex, number of comorbidities, and hospital of origin using the technique of propensity score matching (up to 4:1). They were compared using the Chi-Square or Fisher's Exact tests for categorical variables and the Wilcoxon for numerical variables. Results Throughout the study, 17,101 COVID-19 patients were hospitalized, and 130 (0.76%) of those were infected with HIV. The median age was 54 (IQR: 43.0;64.0) years in 2020 and 53 (IQR: 46.0;63.5) years in 2021, with a predominance of females in both periods. People Living with HIV (PLHIV) and their controls showed similar prevalence for admission to the ICU and invasive mechanical ventilation requirement in the two periods, with no significant differences. In 2020, in-hospital mortality was higher in the PLHIV compared to the controls (27.9% vs. 17.7%; p = 0.049), but there was no difference in mortality between groups in 2021 (25.0% vs. 25.1%; p > 0.999). Conclusions Our results reiterate that PLHIV were at higher risk of COVID-19 mortality in the early stages of the pandemic, however, this finding did not sustain in 2021, when the mortality rate is similar to the control group.

ABSTRACT Despite its rarity, symptomatic micronutrient deficiency remains a public health problem. Scurvy is the differential diagnosis for bleeding disorders and hematological and rheumatological diseases, especially in patients with eating disorders. However, it is unrelated to autism spectrum disorders or other neurodevelopmental disorders. A previously healthy 10-year-old boy living in São Paulo, Brazil, had a history of significant food selectivity unrelated to autism spectrum disorder, resulting in symptomatic ascorbic acid deficiency (scurvy). This resulted in pain and purpuric lesions on the lower limbs, gingival edema, bleeding during tooth brushing, asthenia, weakness, malaise, and sadness. Therefore, dietary anamnesis is important for routine monitoring of child growth and development. This process helps prevent nutritional deficiencies, facilitates early diagnosis of eating disorders, and enables multidisciplinary follow-up for these patients.

Abstract INTRODUCTION: Excessive sodium consumption is associated with increased blood pressure, which is an important risk factor for non-communicable diseases (NCDs). This study therefore aimed to describe the burden of NCDs attributable to excessive sodium consumption among Brazilians. METHODS: This observational study used mortality and Disability Adjusted Life Years (DALY) rates, and their respective uncertainty intervals (UI), from the Global Burden of Disease Study 2019 (GBD 2019). The burden was obtained by the population attributable fraction of each NCD, considering the minimum theoretical value of risk (intake of 0-3g of sodium/day); the excessive consumption proportion in the population, obtained through population inquiries; and the relative risks obtained through meta-analyses. RESULTS: Excessive sodium consumption was the third highest dietary risk contributing to deaths (30,814; 95% UI = 2,034 - 84,130) and DALYs (699,119; 95% UI= 43,130 - 1,914,066) in 2019. States from the Northeast region had the highest age-standardized rates of deaths and DALYs, and the male population was more affected by NCDs caused by excessive sodium consumption. Cardiovascular diseases were the main contributing factors in the burden attributable to excessive sodium consumption. CONCLUSIONS: Regardless of the progress in addressing NCDs related to this risk factor, the impact remains high, especially among men and in the Northeast region. More effective measures are needed to reduce sodium in industrialized products, such as health promotion actions to combat sodium consumption, in order to prevent and control NCDs in Brazil.

Abstract INTRODUCTION: An unhealthy diet is a modifiable risk factor for non-communicable diseases (NCDs), one of the most important public health problems in Brazil. This study aimed to analyze the burden of NCDs attributable to dietary risks in Brazil between 1990-2019. METHODS: Secondary data from the Global Burden of Disease Study were used to estimate the burden attributable to fifteen dietary risks in Brazil. The main sources of data for Brazil were national surveys and international databases. A comparative risk assessment was used to obtain the population attributable fraction. We described the intake of each dietary risk and the distribution of number and rates of deaths and Disability-adjusted life years (DALYs) attributable to diet by sex, age, state, and year from 1990-2019. RESULTS: Cardiovascular diseases, diabetes mellitus, and neoplasms were the main NCDs attributable to an unhealthy diet. Age-standardized mortality and DALYs rates attributable to unhealthy diet decreased between 1990-2019 (-51.5% and -48.8, respectively). Diet high in red meat and sodium, and low in whole grains were the three main risk factors contributing to the burden of NCDs both in 1990 and 2019. The burden of NCDs was higher among males in the middle-aged population (around 50 years), as well as in the states of Maranhão, Rio de Janeiro, and Alagoas. CONCLUSIONS: The present study found a suboptimum diet among the Brazilian population. The major contributors to this burden were diet high in red meat and sodium and low in whole grains. This study supports priorities in public policies on food and nutrition to reduce the burden of NCDs.

Resumo: Introdução: Empatia é definida como a capacidade de ouvir e compreender o outro, e, portanto, é um componente essencial na relação médico-paciente. Estudos apontam que proporcionar aos alunos a oportunidade de contato com o paciente logo no início do curso desperta a consciência da importância da empatia nas relações. Dessa forma, tal contato, nos primeiros semestres da Faculdade de Medicina, permite que os estudantes potencializem suas habilidades empáticas, oportunizando a construção de uma identidade profissional mais ampla e completa. Objetivo: O estudo tem como objetivo avaliar a influência do contato com o paciente na disciplina de Semiologia I, durante o segundo ano da graduação, sobre a empatia de estudantes de Medicina. Método: Trata-se de um estudo observacional e prospectivo, realizado com estudantes de Medicina de uma faculdade privada em Vitória, no Espírito Santo, matriculados na disciplina teórico-prática Semiologia I em 2019/2 e 2020/1, por meio de aplicação de questionários antes da primeira atividade prática em enfermaria e após a última. Analisaram-se as variáveis sociodemográficas e o escore de empatia. Diferenças entre os semestres foram avaliadas pelos testes qui-quadrado, exato de Fisher e de Mann-Whitney. Com os testes de Mann-Whitney e Kruskal-Wallis, investigou-se a relação entre as variáveis sociodemográficas e a empatia. A influência do contato com o paciente na empatia foi averiguada pelo teste de Wilcoxon, todos com 95% de significância. Resultado: Participaram do estudo 38 alunos em 2019/2 e 60 em 2020/1. Foram significativas somente as associações entre o contato com o paciente nas enfermarias e o escore de empatia (p = 0,008), e entre o sexo e o escore de empatia (p = 0,000), sendo esta maior para mulheres e ao final da experiência da disciplina. Conclusão: A disciplina Semiologia I se mostrou capaz de afetar positivamente a empatia, que se apresentou maior no sexo feminino, corroborando a literatura. Os resultados obtidos refletem somente um semestre específico do curso, não o perfil global de empatia dos estudantes, os níveis de empatia em momentos distintos da graduação ou o comportamento dos discentes.

Abstract: Introduction: Empathy is defined as the ability to listen and understand the other, thus becoming an essential component in the doctor-patient relationship. Studies indicate that the opportunity to make contact with the patient early in the course raises awareness of the importance of empathy in the relationships. Thus, such contact, in the first semesters of medical school, enables students to enhance their empathic skills, permitting the construction of a broader and more complete professional identity. Objective: The study aims to evaluate the influence on the empathy of medical students through the interaction with patients in the discipline of Semiology I, during the second year of undergraduate school. Method: Observational and prospective study with medical students from a private medical school in Vitória (ES), enrolled in the theoretical-practical discipline of Semiology I in 2019/2 and 2020/1, through the application of questionnaires, before the first practical activity in the ward and after the last one. Sociodemographic variables and empathy score were analyzed. The differences between the semesters were evaluated by Chi-square, Fisher’s exact or Mann-Whitney tests. The relationship between sociodemographic variables and empathy was assessed by the Mann-Whitney and Kruskal-Wallis tests. The influence of contact with patients on students’ empathy was analyzed by the Wilcoxon test, all with 95% significance. Results: The sample consisted of 38 students in 2019/2 and 60 in 2020/1. Only the associations between contact with the patients in the wards and the empathy score (p=0.008) and gender and the empathy score (p=0.000) were significant; empathy was greater among women and at the end of the discipline experience. Conclusion: The interaction between medical students and patients during the discipline of Semiology I was able to positively affect empathy, corroborating the literature. The higher levels of empathy among women also corroborate the literature. The obtained results reflect only a specific semester of the course, not the students’ overall empathy profile, levels of empathy at different moments of undergraduate school, or their behavior.

Background: A population study is an important tool that can be used to understand the actual epidemiological scenario of the Covid-19 in different territories, identify its magnitude, understand its transmission dynamics, and its demographic, geographical, and social distribution. Objective: The aim of this study was to determine the prevalence and dynamics of SARS-CoV-2 infection in the population of two Brazilian cities during the pandemic first wave and subsequent socioeconomic and health effects. Materials & methods: This paper described the methodological procedures adopted and the prevalence of the SARS-CoV-2 infection in the population. A household survey was conducted between October and December 2020, in two historic cities of Brazil's mining region. Anti-SARS-CoV-2 antibody was detected using the Wondfo® rapid test. The face-to-face interview consisted of administration of a questionnaire containing registration data, sociodemographic and economic variables, living habits, general health condition, mental health, sleep habits, and eating and nutrition. Results: We evaluated 1,762 residents, of which 764 (43.4%) were in Mariana and 998 (56.6%) in Ouro Preto. For both cities, 51.9% of the interviewees were female, with a predominance of the age range 35 to 59 years old (47.2%). The prevalence of the SARS-CoV-2 infection was 5.5% in all cities, 6.2% in Ouro Preto, and 4.7% in Mariana. The prevalence was similar between cities (P>0.05). Conclusion: The study was effective in verifying the seroprevalence of infection by the virus and its findings will enable further analyses of the health conditions of the population related to social isolation and the risk of infection with SARS-CoV-2.

Resumo: O estudo tem como objetivo descrever a adequação das unidades básicas de saúde (UBS) em relação à estrutura e ao processo de trabalho relacionados ao manejo da obesidade, além de avaliar a satisfação dos/as usuários/as com os serviços de saúde. Este estudo transversal foi realizado utilizando dados do Programa Nacional de Melhoria do Acesso e da Qualidade (PMAQ) 2013-2014, uma iniciativa nacional para avaliar o desempenho das equipes de saúde. A coleta de dados ocorreu em 2013 e 2014 através de entrevistas com equipes e usuários/as dos serviços saúde. Todos os indicadores de adequação para o cuidado da obesidade foram criados no âmbito do artigo, com base nos dados do PMAQ. Foram avaliadas 24.055 UBS em 4.845 municípios, além de dados de 114.615 usuários/as. Apenas 7,6% das UBS analisadas tinham estrutura adequada para o tratamento da obesidade. O acesso adequado foi observado em 26,6%, e a organização adequada dos serviços foi encontrada para 27,8% das UBS. Os cuidados de saúde foram classificados como “bons” ou “muito bons” por 82,4% dos usuários/as. Esses indicadores variaram por região geográfica, com melhores resultados para as regiões Sul e Sudeste. Os resultados sugerem que, possivelmente, o país ainda está na fase inicial de sistematização dos cuidados com a obesidade, com significativas disparidades entre as regiões.

Abstract: This study aims to describe the adequacy of basic health units (UBS) in Brazil regarding structure and work process for obesity management and to evaluate user satisfaction with primary health care services. This cross-sectional study was conducted with data from the 2013-2014 National Program for Improving Primary Care Access and Quality (PMAQ) - an initiative to assess primary health care teams’ performance. Data were collected between 2013 and 2014 through interviews with primary health care teams and users. All indicators of adequate care for obesity were defined within the article scope, based on data from the PMAQ. Of the 24,055 UBS analyzed, located in 4,845 different cities, only 7.6% had adequate structure for obesity management. Likewise, only 26.6% of the 114,615 users interviewed reported adequate access, and 27.8% of the UBS showed adequate service organization. Healthcare was considered as “good” or “very good” by 82.4% of users. These indicators varied according to geographic region, showing better results for the South and Southeast. Our results suggest that the country may still be at the initial stage of systematizing care with obesity, presenting significant disparities among regions.

Resumen: El objetivo de este estudio es describir la adecuación de las unidades básicas de atención (UBS) en Brasil, respecto a su estructura y proceso de trabajo para la gestión de la obesidad, así como evaluar la satisfacción de los usuarios con los servicios de las UBS. Este estudio transversal se realizó con datos del 2013-2014 del Programa Nacional de Mejora del Acceso y de la Calidad de la Atención Básica (PMAQ) - una iniciativa para evaluar el desempeño de los equipos de las UBS. Se recogieron datos entre 2013 y 2014 a través de entrevistas a equipos y usuarios de las UBS. Todos los indicadores del cuidado adecuado para la obesidad fueron definidos en el ámbito del artículo, basados en datos del PMAQ. De las 24.055 UBS analizadas, localizadas en 4.845 ciudades diferentes, solamente un 7,6% tenía una estructura adecuada para la gestión de la obesidad. Asimismo, solo un 26,6% de los 114.615 usuarios entrevistados de UBS informaron de un acceso adecuado, y un 27,8% de las UBS presentaron una organización adecuada del servicio. La atención en salud fue considerada como “buena” o “muy buena” por parte de un 82,4% de los usuarios. Estos indicadores variaron de acuerdo con la región geográfica, mostrando mejores resultados en el Sur y Sureste. Nuestros resultados sugieren que el país tal vez se encuentre todavía en un estadio inicial de sistematización del cuidado con la obesidad, presentando significantes disparidades entre regiones.

RESUMO OBJETIVO Analisar a quantidade e a diversidade do consumo de frutas e hortaliças, assim como sua relação com características da aquisição e do ambiente alimentar do consumidor. MÉTODOS Estudo da linha de base oriundo de ensaio comunitário controlado e aleatorizado que investigou amostra representativa de serviços da atenção primária à Saúde (Programa Academia da Saúde) de Belo Horizonte, MG. Analisou-se o consumo de frutas e hortaliças em porções/dia, e sua diversidade por Questionário de Frequência Alimentar. Os usuários também foram questionados sobre frequência, local de compra e disponibilidade dos alimentos no domicílio. Para aferir o ambiente alimentar do consumidor, realizou-se auditoria dos estabelecimentos comerciais dentro de um raio de 1,6 km ao redor das unidades amostradas do programa. RESULTADOS Investigaram-se 3.414 adultos e idosos (88,1% mulheres) e 336 estabelecimentos comerciais em 18 unidades do Programa Academia da Saúde. O consumo médio de frutas e hortaliças foi adequado [5,4 (DP = 2,1) porções/dia], porém monótono, com ingestão média diária de dois tipos. Nos estabelecimentos auditados, observou-se boa diversidade (77,7% e 85,0%) e variedade (74,5% e 81,4%) de frutas e hortaliças, mas com qualidade inferior de hortaliças (60,4%). Após ajuste por variáveis sociodemográficas, identificou-se que o conhecimento sobre safras de alimentos (p = 0,006), aumento da disponibilidade mensal de frutas nos domicílios (p < 0,001), maior variedade de frutas (p = 0,03) e qualidade das hortaliças (p = 0,05) nos estabelecimentos comerciais poderiam melhorar o consumo quantitativo de frutas e hortaliças, enquanto a maior variedade de frutas (p = 0,008) poderia ampliar a diversidade do consumo. CONCLUSÕES O consumo de frutas e hortaliças foi quantitativamente adequado, porém monótono, sendo influenciado pelo ambiente do consumidor. Tais resultados evidenciam a necessidade de aperfeiçoar as ações educativas nos serviços e programas de saúde, além de atuar sobre o ambiente do consumidor, visando promover e manter o consumo adequado e diversificado, conforme preconizado pelas diretrizes brasileiras para a alimentação adequada e saudável.

ABSTRACT OBJECTIVE To analyze the quantity and diversity in the consumption fruits and vegetables, as well as its relationship with the consumer’s purchase characteristics and food environment. METHODS Baseline study stemming from a controlled and randomized community trial investigating a sample representative of Primary Health Care services (Health Academy Program) of Belo Horizonte, state of Minas Gerais. The intake of fruits and vegetables was analyzed in servings/day, whereas diversity was assessed by the Food Frequency Questionnaire. Users were also questioned on the frequency, purchase location and availability of these foods at their households. To assess the consumer’s food environment, commercial establishments within a 1.6 km radius around the program unit sampled were audited. RESULTS 3,414 adults and older adults (88.1% women) were investigated, as well as 336 commercial establishments, in 18 units of the Health Academy Program. The average consumption of fruits and vegetables was adequate [5.4 (SD = 2.1) servings/day] but monotonous, with average daily intake of two different types. In the establishments audited, a good diversity (77.7% and 85.0%) and variety (74.5% and 81.4%) of fruits and vegetables was observed, although with lower quality of vegetables (60.4%). After adjusting for sociodemographic variables, we identified that knowledge on food crops (p = 0.006), increased monthly availability of fruits at households (p < 0.001), and greater variety of fruits (p = 0.03) and quality of vegetables (p = 0.05) in commercial establishments could improve the quantitative intake of fruits and vegetables, whereas a greater variety of fruits (p = 0.008) would increase consumption diversity. CONCLUSIONS The intake of fruits and vegetables was quantitatively adequate but monotonous, being influences by the consumer environment. Such results highlight the need for improving educational actions in health services and programs, in addition to acting on the consumer environment, aiming to promote and maintain the adequate and diversified consumption, as recommended by Brazilian guidelines for proper and healthy eating.
