Abstract Congenital knee dislocation (CKD), a rare yet impactful condition, presents challenges in diagnosis and treatment. We report a clinical case of a prenatally diagnosed CKD at 31+4 weeks of gestation, emphasizing multidisciplinary management’s pivotal role. An early orthopedic intervention, with conservative measures, was fundamental to achieve a favorable infant outcome, underlining the importance of timely diagnosis and expert care. Prenatal CKD ultrasound screening therefore emerges as a crucial tool in guiding therapeutic decisions, optimizing postnatal outcomes, and reducing the need for surgical intervention.

Resumo A luxação congênita do joelho (CKD), uma condição rara porém impactante, apresenta desafios no diagnóstico e tratamento. Apresentamos um caso clínico de CKD com diagnóstico pré-natal, às 31+4 semanas de gestação, enfatizando o papel fundamental da abordagem multidisciplinar. A intervenção ortopédica precoce, com medidas conservadoras, foi fundamental para alcançar um desfecho favorável para a criança, destacando a importância do diagnóstico precoce e do cuidado especializado. O rastreio pré-natal por ecografia da CKD surge, portanto, como uma ferramenta crucial na orientação das decisões terapêuticas, otimizando os resultados pós-natais e reduzindo a necessidade de intervenção cirúrgica.

Abstract We announce the mitochondrial genomes of seven species of the genus Sporophila (S. bouvreuil, S. iberaensis, S. melanogaster, S. minuta, S. nigrorufa, S. pileata, and S. ruficollis) which were validated by comparative genomic and phylogenetic analysis with related species. The mitochondrial genomes of seven passerines of the genus Sporophila were assembled (three complete and four nearly complete genomes) and were validated by reconstructing phylogenetic relations within Thraupidae. The complete mitogenomes ranged from 16,781 bp in S. ruficollis to 16,791 bp in S. minuta. We identified a conserved genome composition within all mitogenomes with 13 protein-coding genes, 22 tRNAs and two rRNAs. We observed a bias in the nucleotide composition and six mutational hotspots in Sporophila mitogenomes. Our mitogenome-based phylogenetic tree has S. minuta, S. maximiliani and S. nigricollis as sister species of the remaining species in the genus. ABSTRACT This study aimed to determine the antibiotic profile of microorganisms isolated from urine samples of patients with community urine tract infections (UTI) admitted to the University Hospital of the Federal University of Sao Carlos to support an appropriate local empirical treatment. A retrospective cross-sectional study was conducted from October 2018 to October 2020. Data from 1,528 positive urine cultures for bacterial pathogens and antibiograms were tabulated. Bacterial species prevalence and their resistance profile were analyzed and compared by sex and age. For Gram-negative fermenting bacteria, resistance rates were compared between patients with previous hospitalization and the total of infections caused by this group. For comparisons, the Chi-square test was performed, using Fisher’s exact test when necessary (BioEstat program, adopting p ≤ 0.05). A multivariate analysis was applied to assess the effect of the studied variables in predicting multidrug resistance. Infections were more prevalent in women and older adults. Gram-negative bacteria represented 90.44% of total cultures. In both sexes, E. coli prevalence was significantly higher in adults compared with older adults (p < 0.0001). For several antibiotics, resistance rates were higher in the older adults compared with other ages and in patients with Gram-negative fermenting infections and previous hospitalization compared with the total of infections by this group of bacteria. The closer to the hospitalization, the higher the number of antibiotics with superior resistance rates. Resistance rates for aminoglycosides, carbapenems, ceftazidime, nitrofurantoin, piperacillin+tazobactam, and fosfomycin were less than 20%, considered adequate for empirical treatment. ABSTRACT This study aimed to investigate the effects of different levels of vitamin D3 in broiler diets on performance and bone health. A total of 360 one-day old male Cobb500® broiler chicks were subjected to five treatments of different levels of vitamin D3 in diets during two rearing phases: 0 IU/kg in both phases; 625 IU/kg in starter and 500 IU/kg in grower phase (25% of commercial inclusion); 1,250 IU/kg in starter and 1,000 IU/kg in grower phase (50% of commercial inclusion); 1,875 IU/kg in starter and 1,500 IU/kg in grower phase (75% of commercial inclusion); and 2,500 IU/kg in starter and 2,000 IU/kg in grower phase (100% of commercial inclusion). The traits studied weekly were feed intake, body weight, feed conversion, and viability. At 21 and 35 days of age, tibiae and femurs were removed, dissected, and evaluated for dry matter (DM), ash (%MM), calcium (%Ca), phosphorus (%P) and breaking strength. Abstract The Psychology of Working Theory has achieved notable prominence in career studies for its focus on how various - both individual and contextual - influence careers and job outcomes. “Volition” stands out as one of the key elements of this theory, representing the individuals’ ability to make career choices despite obstacles. This study aimed to adapt and validate both internal and external structures of the Work Volition Scale - Student Version (WVS-SV) to Brazil. A total of 590 university students from public and private institutions of varied courses and semesters, mostly women (59%), took part in the study. Exploratory and confirmatory factor analyses indicated that the scale effectively measured two dimensions: volition and constraints. External evidence, including satisfaction with life and career, supported the theoretical aspects of this construct. ABSTRACT Backgrounds: Fortunately, much has been studied about COVID-19 in patients with inflammatory bowel diseases (IBD). Evidence suggests that these patients do not appear to be at increased risk of severe COVID-19. However, there are still some uncertainties regarding the clinical manifestations of COVID-19 in patients with immune-mediated diseases. Objective: This study aimed to describe the main symptoms of COVID-19 and their frequency in IBD patients and evaluate the impact of the IBD therapeutic drugs on clinical presentation of COVID-19 and to determine factors associated with COVID-19 in this population. Methods: Adult patients with IBD from three tertiary-care public, teaching hospitals in Ceará, Northeastern Brazil, were evaluated during one scheduled appointment from March to December 2020. Patients with possible or confirmed COVID-19 were compared with patients without COVID-19. Furthermore, incidences of each symptom were evaluated based on the use of IBD therapeutic drugs. Results: A total of 515 patients with IBD were included in the study: 234 with CD, and 281 with UC. Of these, 174 patients (34%) had possible/confirmed COVID-19 of whom 156 (90%) were symptomatic. Main symptoms were fever (65%) and headache (65%); gastrointestinal symptoms occurred in one third of patients and were higher than COVID-19 in general population. RESUMO Contexto: Felizmente, muito se tem estudado sobre a COVID-19 em pacientes com doenças inflamatórias intestinais (DII). As evidências sugerem que esses pacientes não parecem ter risco aumentado de COVID-19 grave. Mas ainda se tem algumas incertezas com relação às manifestações clínicas da COVID-19 em portadores de doenças imunomediadas. Objetivo: Este estudo teve como objetivo descrever os principais sintomas da COVID-19 e sua frequência em pacientes com DII e avaliar o impacto dos medicamentos utilizados no tratamento das DII na apresentação clínica da COVID-19. Métodos: Pacientes adultos com DII de três hospitais públicos terciários de ensino do Ceará, Nordeste do Brasil, foram avaliados em consulta ambulatorial no período de março a dezembro de 2020. Pacientes com COVID-19 possível ou confirmada foram comparados com pacientes sem COVID-19. Além disso, as incidências de cada sintoma foram avaliadas com base no uso de medicamentos utilizados para tratamento da DII. Resultados: Foram incluídos no estudo 515 pacientes com DII: 234 com DC e 281 com RCU. Destes, 174 pacientes (34%) tinham COVID-19 possível/confirmado, dos quais 156 (90%) eram sintomáticos. ABSTRACT: Leishmaniasis is a zoonotic disease caused by parasites of the Leishmania genus, resulting in various clinical forms, including a highly lethal visceral form. This study aimed to identify Leishmania spp. in sandflies and dogs in a small Brazilian town. DPP® Visceral Canine Leishmaniasis (CVL) was used for screening, and ELISA and conventional PCR were used for confirmatory testing, while sandflies were captured using CDC light traps and conventional PCR targeting ITS1. The Hill series was used to identify the diversity profile of species in the sampled area using R software. The study identified a Leishmania spp. prevalence of 4.02% in dogs, exceeding the 2% limit required by the Brazilian Ministry of Health. A total of 443 sandflies belonging to 14 different species were identified, with Lutzomyia longipalpis being the most abundant (73.81%). Negligence regarding leishmaniasis in small towns can lead to late diagnosis, hence the need to implement effective strategies, including early diagnosis and treatment of human and canine cases, vector control programs, and awareness campaigns to educate the public about risks and preventive measures. These measures can help prevent the spread of leishmaniasis and improve health outcomes for affected individuals and animals. RESUMO: A leishmaniose é uma doença zoonótica causada por parasitas do gênero Leishmania, resultando em várias formas clínicas, incluindo uma forma visceral altamente letal. Este estudo teve como objetivo identificar Leishmania spp. DNA em flebotomíneos e cães em uma pequena cidade brasileira. A investigação usou DPP® Visceral Canine Leishmaniasis (CVL) para triagem, e ELISA e PCR convencional para teste confirmatório, enquanto flebotomíneos foram capturados usando armadilhas de luz CDC e PCR convencional visando ITS1. A série Hill foi utilizada para identificar o perfil de diversidade das espécies na área amostrada, utilizando o software R. O estudo identificou uma prevalência de Leishmania spp. em 4,02% dos cães, ultrapassando o limite de 2% exigido pelo Ministério da Saúde do Brasil. Foi identificado um total de 443 flebotomíneos, pertencentes a 14 espécies diferentes, com Lutzomyia longipalpis sendo o mais abundante (73,81%). Resumo O artigo explora a teleassistência a partir dos desenvolvimentos da telessaúde e da aceleração da transformação digital na saúde provocada pela pandemia de COVID-19, com foco no Sistema Único de Saúde (SUS). Aborda questões terminológicas, escopo de ações, potencialidades do uso para atenção à saúde e condicionantes e contingências para a utilização da teleassistência no Brasil, concentrando-se nas teleconsultas e nas interações entre profissionais de saúde e pacientes. Por fim, apresenta um conjunto de proposições para o desenvolvimento das políticas e práticas de teleassistência no Brasil, tendo em vista os princípios do SUS, organizados em dois eixos estratégicos centrais: diretrizes político organizacionais e proposições operacionais e de organização dos serviços e do cuidado. Destaca-se a importância de ponderar e elucidar os alcances e os limites das novas tecnologias para evitar idealizações e deslumbramentos com suas propostas de solução para os complexos problemas de saúde. Abstract This article explores telecare from telehealth developments and the recent acceleration of the digital health transformation caused by the COVID-19 pandemic, focusing on the Brazilian Unified Health System (SUS). It addresses terminological issues, the scope of actions, the potential use for healthcare, and constraints and contingencies for telecare in Brazil, focusing on teleconsultations and interactions between health professionals and patients. Finally, it presents a set of propositions for the development of telecare policies and practices in Brazil, considering SUS principles, in two central themes: organizational political guidelines and operational propositions to organise services and healthcare delivery. The importance of clarifying the scope and limits of new technologies is highlighted in the attempt to avoid idealizations with proposed solutions to complex health problems. Telecare solutions should be compatible with SUS principles and with the recommended model of care, with the healthcare network coordinated and organised by primary care, ensuring access to health services and integrated and quality healthcare for the Brazilian society. Resumo A literatura científica sobre anomalias em anfíbios é vasta, mas casos registrados de heterocromia ocular em anfíbios estão quase ausentes, mesmo considerando registros anedóticos. No território brasileiro, apenas dois casos de heterocromia ocular foram registrados anteriormente para anfíbios, ambos para anuros, o primeiro para a Mata Atlântica do nordeste do Brasil e o segundo para a região amazônica do norte do Brasil. No presente relato, descrevemos dois casos de heterocromia ocular no hilídeo Boana albomarginata, ambos no bioma Mata Atlântica, estado da Bahia, nordeste do Brasil, representando o primeiro registro de heterocromia ocular para esta espécie e o terceiro caso registrado para uma espécie de anfíbio no Brasil. Um olho de cada exemplar observado apresentava atipicamente íris de cor azul chamativa e homogênea, preenchendo toda sua superfície ou apenas a metade inferior, representando casos de heterocromia completa e setorial, respectivamente. Além disso, a não-usual íris azul só esteve detectável sob a luz do dia, quando a cor de fundo da íris era branca. À noite, a cor de fundo padrão da íris cobreada ou dourada permaneceu inalterada e a cor azul não pôde ser detectada. Nenhuma outra anomalia detectável ou comportamento incomum foi observada para ambos os exemplares. Abstract The scientific literature on amphibian anomalies is large, but registered cases of amphibians’ ocular heterochromia are almost absent, even considering anecdotal records. In the Brazilian territory, only two cases of ocular heterochromia were formerly registered for amphibians, both for anurans, the first from the Atlantic Forest of northeastern Brazil and the second from the Amazonian region of northern Brazil. In the present account, we report two cases of ocular heterochromia in the hylid treefrog Boana albomarginata, both from the Atlantic Forest biome, Bahia state, northeastern Brazil, representing the first record of ocular heterochromia for this species and the third case registered for an amphibian’s species in Brazil. One eye of each observed specimen had an unusually flashy and homogeneous blue-colored iris, filling its entire surface or only its lower half, representing cases of complete and sectoral heterochromia, respectively. Furthermore, the unusual flashy blue iris was only detectable during daylight, when the iris background color was white. At night, the standard copper to golden iris background color remains unchanged, and the blue color cannot be detected. No other detectable anomalies or unusual behaviors were observed for both specimens. This is the first report of amphibian ocular heterochromia represented by a flashy blue color iris and limited to a daylight iris color pattern. RESUMEN Objetivos: Analizar percepción de enfermeros sobre métodos de trabajo en enfermería en contexto hospitalario. Métodos: Estudio descriptivo con abordaje cualitativa, realizado en un hospital del norte de Portugal, con participación de 17 enfermeros. Como instrumento de recolecta de datos, recorrido a la entrevista semiestructurada. Datos recolectados entre mayo e junio de 2023, pasaron por análisis de contenido, con suporte del software Atlas.ti. Resultados: Emergieron tres áreas temáticas: “Métodos de trabajo de enfermeros en contexto hospitalario”, destacándose la concepción y componentes de métodos de trabajo y en uso; “Implementación de métodos de trabajo”, señalándose factores influenciadores y desafíos a la implementación; e “Impacto de métodos de trabajo de enfermeros para los pacientes, enfermeros e instituciones”. ABSTRACT Objectives: To analyze nurses’ perspectives on nurses’ work methods in the hospital context. Methods: A descriptive study with a qualitative approach was conducted in a hospital in northern Portugal, involving 17 nurses. Semi-structured interviews were used for data collection. Data collected between May and June 2023 underwent content analysis, supported by Atlas.ti software. El estudio buscó comprender la influencia de la estrategia Pay for Performance (P4P), por medio del Programa de Mejora del Acceso y Calidad de la Atención Básica (Pmaq-AB), en la actuación de los trabajadores y las trabajadoras. This study sought to understand the influence of pay-for-performance (P4P) on worker performance using data from the National Program for Improving Primary Care Access and Quality (PMAQ-AB). We conducted a qualitative case study with professionals working in family health strategy teams. The data were analyzed using the narrative construction technique. The findings show that P4P contributed to the improvement of work processes, while at the same time stimulating competition and conflict between the teams, causing feelings of guilt and injustice. However, the original purpose of the financial incentive is defeated due to poor working terms and conditions, becoming akin to a salary supplement. Método: Estudio metodológico desarrollado en dos etapas: (1) identificación de los conceptos relevantes para la prioridad sanitaria elegida a partir de la literatura; (2) mapeo cruzado de los conceptos identificados con los conceptos contenidos en la CIPN® versión 2019/2020. Resultados: 9460 términos fueron extraídos de la literatura, de los cuales 4065 términos fueron excluidos por no estar relacionados con el objeto de estudio y 5395 fueron sometidos a la técnica de mapeo, resultando 290 términos constantes en la ICNP® y 5134 términos no constantes. Los términos constantes se clasificaron en los siguientes ejes: 120 en el eje Enfoque, 13 en Juicio, 48 en Acción, 23 en Localización, 38 en Medios, ocho en Tiempo y uno en Cliente. ABSTRACT Objectives: To build a specialized nursing terminology for the care of people with respiratory diseases and Covid-19 or who have respiratory diseases after Covid-19, based on ICNP®. Method: Methodological study developed in two stages: (1) identification of the relevant concepts for the health priority chosen from the literature; (2) cross-mapping of the identified concepts with the concepts contained in ICNP® version 2019/2020. Results: 9460 terms were extracted from the literature, of which 4065 terms were excluded because they were not related to the object of study and 5395 were submitted to the mapping technique, resulting in 290 constant terms in the ICNP® and 5134 non-constant terms. The constant terms were classified into the following axes: 120 in the Focus axis, 13 in Judgment, 48 in Action, 23 in Location, 38 in Means, eight in Time and one in Client. In addition, 36 nursing diagnoses/outcomes and three nursing interventions were mapped. Conclusion: The terminology will support the quality of care provided by the nursing team and the manual and electronic recording of patient data. Resumo O artigo explora a teleassistência a partir dos desenvolvimentos da telessaúde e da aceleração da transformação digital na saúde provocada pela pandemia de COVID-19, com foco no Sistema Único de Saúde (SUS). Aborda questões terminológicas, escopo de ações, potencialidades do uso para atenção à saúde e condicionantes e contingências para a utilização da teleassistência no Brasil, concentrando-se nas teleconsultas e nas interações entre profissionais de saúde e pacientes. Por fim, apresenta um conjunto de proposições para o desenvolvimento das políticas e práticas de teleassistência no Brasil, tendo em vista os princípios do SUS, organizados em dois eixos estratégicos centrais: diretrizes político organizacionais e proposições operacionais e de organização dos serviços e do cuidado. Destaca-se a importância de ponderar e elucidar os alcances e os limites das novas tecnologias para evitar idealizações e deslumbramentos com suas propostas de solução para os complexos problemas de saúde. As soluções de teleassistência devem ser compatíveis com princípios e diretrizes do SUS e com o modelo de atenção preconizado, que prevê a organização da rede a partir da atenção primária, para garantir acesso, integralidade e qualidade da atenção à saúde para a sociedade brasileira.
