ABSTRACT Patients undergoing hematopoietic stem cell transplantation (HSCT) might present acute and late toxicities and the oral tissues are frequently affected. With the survival increasing, patients show late and long-term morbidities, and there is an important association between the general and the oral health. The first and second parts of this Consensus have showed the importance of the adequacy of oral health in the pre-HSCT, and the main alterations and oral care during the period of admission for HSCT. This third part aims to review specific themes of post-HSCT dental care, such as graft-versus-host disease (GVHD) and the pediatric patient. It also aims to review pertinent subjects, both during the HSCT period and post-HSCT, concerning quality of life, pain, cost-effectiveness, and remote care. Based on this review, it is evident the importance of the work of the dental surgeon (DS) in the follow-up and treatment of the HSCT patient, always collaborating with the whole multidisciplinary team.

ABSTRACT During the state of immune vulnerability in hematopoietic stem cell transplantation (HSCT), the patient has an increased risk of developing a vast number of complications, including severe problems in the oral cavity. These situations require professional oral care to act in the diagnosis and treatment of these conditions, as well as to develop prevention protocols to minimize patient's complications. Oral mucositis, opportunistic infections, bleeding, specific microbiota, taste, and salivary alterations are complications that can occur during HSCT and interfere with various aspects, such as pain control, oral intake, nutrition, bacteremia and sepsis, days of hospitalization and morbidity. Several guidelines have been published to address the role of professional oral care during the HSCT, we describe a consensus regarding these recommendations.

ABSTRACT The oral involvement in the Hematopoietic Stem Cell Transplantation is well described in the literature. The goal of the dental treatment and management of the oral lesions related to the HSCT is to reduce the harm caused by preexisting oral infection or even the worsening of oral acute/chronic GVHD and late effects. The aim of this guideline was to discuss the dental management of patients subjected to HSCT, considering three phases of the HSCT: pre-HSCT, acute phase, and late phase. The literature published from 2010 to 2020 was reviewed in order to identify dental interventions in this patient population. The selected papers were divided into three groups: pre-HSCT, acute and late, and were reviewed by the SBTMO Dental Committee's members. When necessary, an expertise opinion was considered for better translating the guideline recommendations to our population dental characteristics. This manuscript focused on the pre-HSCT dental management. The objective of the pre-HSCT dental management is to identify possible dental situations that On behalf of the Dental Committee of the Brazilian Society of Gene Therapy and Bone Marrow Transplantation (SBTMO) can worsening during the acute phase after the HSCT. Each guideline recommendations were made considering the Dentistry Specialties. The clinical consensus on dental management prior to HSCT provides professional health caregivers with clinical setting-specific information to help with the management of dental problems in patients to be subjected to HSCT.

Abstract Background Evaluation and treatment of primary and secondary headaches is a global public health challenge. Recognizing the epidemiological impact of headaches, a group of researchers linked to the Brazilian Headache Society proposed the Brazilian Headache Registry and drew up its initial protocol. Objective Here we describe the methods and preliminary data obtained from the pilot study. Methods This was a multicenter longitudinal observational study conducted between September 2020 and August 2021. Prospective data were collected in three specialist centers for headache care in states in southern and southeastern Brazil. Patients aged 18 years or older who sought care for headache in tertiary centers and who agreed to participate in the study, were considered eligible. Results Sixty-six patients were included in the pilot study: 43 (65%) from Rio Grande do Sul state and 23 (35%) from Minas Gerais state. Overall, 90% were female, and the subjects' mean age was 38.2 ± 11.2 years. Primary headaches accounted for 85.3% of the diagnoses made. Among secondary headaches, medication overuse headache was the most frequent type (7.1%). Conclusions The pilot study showed the feasibility of the research protocol developed for tertiary centers. The Brazilian Headache Registry will form a source of longitudinal data with the aim of contributing to better characterization of the various phenotypes of patients with primary and secondary headaches, and to detailing the use of health resources and identifying predictors of better clinical outcomes.

Resumo Antecedentes A avaliação e o tratamento das cefaleias primárias e secundárias são um desafio global de saúde pública. Reconhecendo o impacto epidemiológico das cefaleias, um grupo de pesquisadores vinculados à Sociedade Brasileira de Cefaleia propôs a criação de um Registro Brasileiro de Cefaleia e elaborou seu protocolo inicial. Objetivo Nesta publicação descrevemos os métodos e dados preliminares obtidos a partir do estudo piloto. Métodos Trata-se de um estudo prospectivo observacional longitudinal multicêntrico, realizado entre setembro de 2020 e agosto de 2021. Foram coletados dados em três centros especializados no atendimento de cefaleia, em estados da região sul e sudeste do Brasil. Pacientes com idade igual ou superior a 18 anos que procuraram os centros terciários por queixa de cefaleia e concordaram em participar do estudo foram considerados elegíveis. Resultados Sessenta e seis pacientes foram incluídos no estudo piloto, 43 (65%) do Rio Grande do Sul e 23 (35%) de Minas Gerais. Da amostra total, 90% eram do sexo feminino e a idade média dos sujeitos foi de 38,2 ± 11,2 anos. As cefaleias primárias representaram 85,3% dos diagnósticos realizados. Entre as cefaleias secundárias, a cefaleia por uso excessivo de medicamentos foi a mais frequente (7,1%). Conclusões O estudo piloto evidenciou a viabilidade do protocolo de pesquisa desenvolvido para centros terciários. O Registro Brasileiro de Cefaleia constituirá uma fonte de dados longitudinais com o objetivo de contribuir para melhor caracterização dos diversos fenótipos de pacientes com cefaleias primárias e secundárias, detalhar o uso de recursos de saúde e identificar preditores de melhores desfechos clínicos.

Resumen Los desastres tienen el poder de causar profundos impactos socioeconómicos en la sociedad y aumentar el riesgo social. El beneficio eventual es una medida de protección social para prevenir y promover el enfrentamiento de situaciones ajenas a la vida cotidiana, como un desastre. En este sentido, ¿cuál es el papel del beneficio eventual en un contexto de desastre? Este artículo tiene como principal objetivo verificar el papel del beneficio eventual en situaciones de desastre en Brasil. Además, está estructurado en tres objetivos específicos: a) discutir cómo se vinculan los principios de seguridad jurídica y de la prohibición del retroceso social a la provisión del beneficio eventual; b) describir cómo se estructura el beneficio eventual en el sistema legal brasileño; c) analizar cómo es la provisión del beneficio eventual en una situación de desastre. Para responder a esta pregunta, la investigación utiliza el método de investigación documental de artículos y libros científicos en el ámbito del derecho constitucional y la seguridad social. Los resultados indican que el beneficio eventual no se resume en un monto económico, sino en un conjunto de ayudas destinadas a cubrir las necesidades existentes de una población en un escenario de desastre.

Abstract Disasters can lead to social and economic losses and a sharp increase in social risks. The ‘benefício eventual’ is a social protection measure whose aim is to prevent vulnerability and to respond to social risks in unpredictable situations, such as disasters. In this sense, what is the role of the ‘benefício eventual’ in the context of disasters? The main objective of this study is to verify the role of ‘benefício eventual’ in disaster situations in Brazil. To provide a more detailed approach on the research process, this investigation has three specific objectives: a) discuss the convergences between the principles of legal security and the prohibition of retrogression related to the provision of the ‘benefício eventual’; b) describe the legislation of the ‘benefício eventual’; c) analyze how the ‘benefício eventual’ is provided during a disaster. This research uses the documentary analysis of constitutional and social security legislation, academic papers, and books. The results indicate that the ‘benefício eventual’ is not merely an amount of money, but a set of policies to fulfill population’s needs after a disaster.

Résumé L’impact socioéconomique des catastrophes et autres désastres est énorme sur la société et aggrave les risques sociaux. Afin d’en atténuer les effets, a été conçu ce que l’on désigne sous les termes de bénéfice éventuel, défini comme une mesure de protection sociale visant à préparer et à former la population si elle est confrontée à des situations sans aucun rapport avec la vie quotidienne, comme les désastres. Dans ce contexte, quel rôle le bénéfice éventuel occupe-t-il lorsque l’on a affaire à un désastre? Cette étude a pour principal objectif celui de vérifier le rôle joué par le bénéfice éventuel lorsque des catastrophes ont eu lieu au Brésil. Divisée en trois parties, on y aborde plus spécifiquement les thèmes suivants: a) une interrogation sur les liens associant les principes de sécurité juridique et le rejet de toute régression sociale qui sont sous-entendus par ce que l’on nomme les bénéfices éventuels ; b) une description de la structure desdits bénéfices dans le système légal au Brésil; c) une analyse sur l’apport du bénéfice éventuel lors de désastres. Pour parvenir à ce but, ce travail a eu recours à la méthode de recherche documentaire à partir d’articles et d’ouvrages scientifiques, plus précisément ceux dont la réflexion utilise des outils propres au droit constitutionnel et à la sécurité sociale. Les résultats montrent que le bénéfice éventuel ne se résume pas simplement à une aide économique mais se caractérise aussi par un ensemble de mesures visant à faire face aux besoins réels de la population victime d’un désastre.

RESUMO A disseminação metastática ocular de tumores sistêmicos é incomum, ocorrendo principalmente na coroide e em pacientes idosos. O câncer de pulmão é considerado o principal tumor metastático ocular em homens, contudo, outras doenças oculares, como as uveítes e lesões retinianas, podem mimetizar os implantes secundários tumorais nos tecidos oculares. O aspecto fundoscópico das neoplasias da coroide pode apresentar similaridade com outros processos infecciosos, especialmente o tuberculoma de coroide. Dessa forma, a investigação clínica detalhada é de grande importância no diagnóstico de pacientes com massas coroideanas, especialmente quando configuram a primeira manifestação de uma doença sistêmica e grave. ABSTRACT Ocular metastases from systemic tumors are uncommon. The choroid is the most frequent target, with a preference for elderly individuals. Lung cancer is the predominant primary tumor that metastasizes to the eyes in males, although other ocular conditions such as uveitis and retinal lesions can mimic secondary tumor implants in ocular tissues. On fundoscopy, choroidal metastasis resembles other infectious processes, especially choroidal tuberculoma. Therefore, patients presenting with choroidal masses should undergo detailed clinical examinations, especially if the mass is the first manifestation of a systemic and severe disease. Abstract Introduction Malignant Hyperthermia (MH) is a pharmacogenetic, hereditary and autosomal dominant syndrome triggered by halogenates/succinylcholine. The In Vitro Contracture Test (IVCT) is the gold standard diagnostic test for MH, and it evaluates abnormal skeletal muscle reactions of susceptible individuals (earlier/greater contracture) when exposed to caffeine/halothane. MH susceptibility episodes and IVCT seem to be related to individual features. Objective To assess variables that correlate with IVCT in Brazilian patients referred for MH investigation due to a history of personal/family MH. Methods We examined IVCTs of 80 patients investigated for MH between 2004‒2019. We recorded clinical data (age, sex, presence of muscle weakness or myopathy with muscle biopsy showing cores, genetic evaluation, IVCT result) and IVCT features (initial and final maximum contraction, caffeine/halothane concentration triggering contracture of 0.2g, contracture at caffeine concentration of 2 and 32 mmoL and at 2% halothane, and contraction after 100 Hz stimulation). Results Mean age of the sample was 35±13.3 years, and most of the subjects were female (n=43 or 54%) and MH susceptible (60%). Of the 20 subjects undergoing genetic investigation, 65% showed variants in RYR1/CACNA1S genes. We found no difference between the positive and negative IVCT groups regarding age, sex, number of probands, presence of muscle weakness or myopathy with muscle biopsy showing cores. Regression analysis revealed that the best predictors of positive IVCT were male sex (+12%), absence of muscle weakness (+20%), and personal MH background (+17%). Conclusions Positive IVCT results have been correlated to male probands, in accordance with early publications. Furthermore, normal muscle strength has been confirmed as a significant predictor of positive IVCT while investigating suspected MH cases.

Resumen Introducción: el incremento del consumo de alimentos procesados y ultraprocesados puede predisponer a alteraciones metabólicas. Objetivo: verificar la asociación del consumo de alimentos con el fenotipo metabólico en trabajadores de un hospital cuaternario de Rio de Janeiro, Brasil. Métodos: fueron elegibles trabajadores de ambos sexos de edad > 18 años. El cuestionario de frecuencia de alimentos y la clasificación NOVA se utilizaron en el análisis del consumo de alimentos. El fenotipo metabólico consideró la presencia de al menos una alteración metabólica (glucemia, lípidos séricos y presión arterial) combinada con el IMC (eutrófico o exceso de peso) de la siguiente manera: 1) eutrófico metabólicamente saludable (EHM); 2) eutróficos metabólicamente no saludables (MUE); 3) exceso de peso metabólicamente saludable (MHEW); 4) exceso de peso metabólicamente no saludable (MUEW). Resultados: de los 160 participantes incluidos (edad media: 45,2 ± 1,1 años, 59,5 % de mujeres), el 21,9 % refirieron hipertensión arterial y el 4,4 % diabetes. La mayoría presentaron exceso de peso (74,6 %), siendo aproximadamente un 40 % obesos. El fenotipo MUEW presentó mayor porcentaje de grasa corporal y adiposidad central representada por mayor CC y VFA en comparación con los otros fenotipos. La masa corporal magra fue similar entre los grupos. La mediana de alimentos ultraprocesados fue de 32,4 % para los eutróficos, 32,7 % para el sobrepeso y 34,3 % para los obesos. No se observaron asociaciones significativas entre el consumo de ultraprocesados y el fenotipo eutrófico metabólicamente no saludable (OR: 1,01; IC 95 %: 0,96-1,06), exceso de peso metabólicamente saludable (OR: 1,03; IC 95 %: 0,98-1,08) y exceso de peso metabólicamente no saludable (OR: 1,00; IC 95 %: 0,96-1,05) en comparación con los eutróficos metabólicamente sanos. Conclusión: el consumo de alimentos ultraprocesados fue elevado. En este análisis transversal, no se observó asociación de fenotipos metabólicos y consumo de grupos de alimentos según el grado de procesamiento de los alimentos.

Abstract Introduction: an increase in the consumption of processed and ultra-processed foods may predispose to metabolic abnormalities. Objective: to verify the association of food consumption with metabolic phenotype in workers from a quaternary hospital in Rio de Janeiro, Brazil. Methods: workers of both sexes aged > 18 years were eligible. A food frequency questionnaire and the NOVA classification were used in the food consumption analysis. Metabolic phenotype considered the presence of at least one metabolic alteration (blood glucose, serum lipids, and blood pressure) combined with BMI (eutrophic or excess weight) as follows: 1) metabolically healthy eutrophic (MHE); 2) metabolically unhealthy eutrophic (MUE); 3) metabolically healthy excess weight (MHEW); 4) metabolically unhealthy excess weight (MUEW). Results: from the included 160 participants (mean age, 45.2 ± 1.1 years; 59.4 %, women), 21.9 % self-reported arterial hypertension and 4.4 % diabetes. Most presented excess weight (74.6 %), with approximately 40 % being obese. The MUEW phenotype had higher body fat percentage and central adiposity represented by higher WC and VFA in comparison to the other phenotypes. The lean body mass was similar between the groups. The median of ultra-processed foods was 32.4 % for eutrophic, 32.7 % for overweight, and 34.3 % for obese subjects. No significant associations were observed between ultra-processed food consumption and metabolically unhealthy eutrophic (OR: 1.01; 95 % CI: 0.96-1.06), metabolically healthy excess weight (OR: 1.03; 95 % CI: 0.98-1.08), and metabolically unhealthy excess weight (OR: 1.00; 95 % CI: 0.96-1.05) in comparison to metabolically healthy eutrophic. Conclusion: consumption of ultra-processed food was high. In this cross-sectional analysis, no association of metabolic phenotypes with consumption of food groups according to degree of food processing were observed.

Abstract Objective: To assess the predictive value of selected growth phenotypes for neonatal morbidity and mortality in preterm infants < 30 weeks and to compare them with INTERGROWTH-21st (IG21). Methods: Retrospective analysis of data from the Brazilian Neonatal Research Network (BNRN) database for very low birth weight (VLBW) at 20 public tertiary-care university hospitals. Outcome: the composite neonatal morbidity and mortality (CNMM) consisted of in-hospital death, oxygen use at 36 weeks, intraventricular hemorrhage grade 3 or 4, and Bell stage 2 or 3 necrotizing enterocolitis. Selected growth phenotypes: small-for-gestational-age (SGA) defined as being < 3rd (SGA3) or 10th (SGA10) percentiles of BW, and large-for-gestational-age (LGA) as being > 97th percentile of BW. Stunting as being < 3rd percentile of the length and wasting as being < 3rd percentile of BMI. Single and multiple log-binomial regression models were fitted to estimate the relative risks of CNMM, comparing them to IG21. Results: 4,072 infants were included. The adjusted relative risks of CNMM associated with selected growth phenotypes were (BNRN/IG21): 1.45 (0.92–2.31)/1.60 (1.27–2.02) for SGA; 0.90 (0.55–1.47)/1.05 (0.55–1.99) for LGA; 1.65 (1.08–2.51)/1.58 (1.28–1.96) for stunting; and 1.48 (1.02–2.17) for wasting. Agreement between the two references was variable. The growth phenotypes had good specificity (>95%) and positive predictive value (70-90%), with poor sensitivity and low negative predictive value. Conclusion: The BNRN phenotypes at birth differed markedly from the IG21 standard and showed poor accuracy in predicting adverse neonatal outcomes.

ABSTRACT In the last decade, other quality attributes have emerged as the main aspects that impact pork consumers acceptance, such as environmentally friendly production, nutritional value, and animal welfare. This study applies a Check-All-That-Apply (CATA) questionnaire for sensory characterization of loins from genetically lean immunocastrated male pigs fed diets containing either a standard commercial diet with 1.5 % soybean oil (control) or 3 % oil from either soybean oil (SO), canola oil (CO), or fish oil (FO) during the growing-finishing phases. Twenty CATA attributes for loin samples were generated. We interviewed 101 consumers to select all the CATA terms that they considered suitable to characterize sensory attributes of longissimus lumborum muscle samples of pigs from each dietary treatment. The CATA results indicated differences among pork loin samples depending on the source and inclusion level of dietary oil. Loins from pigs fed 3 % SO were characterized by a higher frequency of the “juicy texture” attribute (p < 0.05) and a lower frequency of the “dry texture” attribute (p < 0.05) compared to loins from 3 % FO-fed pigs. Moreover, loins from pigs fed 3 % SO had a lower citation frequency of the “tasteless” attribute (p < 0.05) compared to loins from 3 % FO-fed pigs. The attributes “cooked pork meat-like taste”, “juicy texture”, “tender texture”, and “brightness” were placed close to the overall liking. “Tender texture”, “juicy texture”, “pork meat-like” (odor and taste), and “cooked pork meat-like taste” attributes were considered drivers of liking. Feeding genetically lean immunocastrated male pigs with diets containing 3 % SO may be advisable from a pork sensory quality perspective.

ABSTRACT Objective: To identify factors related to parental stress of women during pregnancy and the child’s first month of life. Method: Prospective longitudinal study in two stages. Analysis of home interviews with 121 participants, Gestational Stress Scale, and Parental Stress Scale. Fisher’s exact test, Spearman’s correlation, and linear and logistic multivariate regression were applied, with p < 0.05. Results: Most of the participants were between 18 and 35 years old, had 11 to 13 years of education, had no paid work, had a partner, usually the child’s father, planned pregnancy, were multiparous, and underwent prenatal care. During pregnancy, 67.8% had stress. In the first month after the child’s birth, most had low parental stress (52.1%). High parental stress correlated with some gestational stress. Planning pregnancy decreased parental stress. Conclusion: Gestational and parental stress in the child’s first month of life were correlated and pregnancy planning was a factor that reduced stress levels. Timely actions to reduce parental stress are essential for parenting and the child’s overall health.

RESUMEN Objetivo: Identificar factores relacionados con el estrés parental de la mujer durante el embarazo y el primer mes de vida del niño. Método: Estudio longitudinal prospectivo en dos etapas. Análisis de entrevistas domiciliarias con 121 participantes, Escala de Estrés Gestacional y Estrés Parental. Se aplicó la prueba exacta de Fisher, correlación de Spearman y regresión multivariada lineal y logística, con p < 0,05. Resultados: La mayoría de las participantes tenía entre 18 y 35 años, tenía entre 11 y 13 años de escolaridad, no tenía trabajo remunerado, tenía pareja, generalmente el padre del niño, embarazo planificado, multípara y realizó control prenatal. Durante el embarazo, el 67,8% tuvo estrés. En el primer mes después del nacimiento del hijo, la mayoría presentó bajo estrés parental (52,1%). El estrés parental alto se correlacionó con algo de estrés gestacional. La planificación del embarazo disminuyó el estrés parental. Conclusión: El estrés gestacional y parental en el primer mes de vida del niño se correlacionaron y la planificación del embarazo fue un factor en la reducción de los niveles de estrés. Las acciones oportunas para reducir el estrés parental son esenciales para la responsabilidad parental y la salud general del niño.

RESUMO Objetivo: Identificar fatores relacionados ao estresse parental de mulheres na gestação e primeiro mês de vida da criança. Método: Estudo longitudinal prospectivo em duas etapas. Análise de entrevistas em domicílio com 121 participantes, Escala de Estresse Gestacional e de Estresse Parental. Aplicados teste exato de Fisher, correlação de Spearman e regressão multivariada linear e logística, com p < 0,05. Resultados: A maior parcela das participantes tinha entre 18 e 35 anos, 11 a 13 anos de estudo, sem trabalho remunerado, com companheiro, geralmente o pai da criança, gestação planejada, multípara e realização do pré-natal. Na gestação, 67,8% apresentaram estresse. No primeiro mês após o nascimento do filho, a maioria teve baixo estresse parental (52,1%). Estresse parental alto apresentou correlação com algum estresse gestacional. Planejar a gestação diminuiu o estresse parental. Conclusão: Estresse gestacional e parental no primeiro mês de vida da criança foram correlacionados e o planejamento da gestação constituiu fator de diminuição dos níveis de estresse. Ações oportunas para reduzir o estresse parental são fundamentais para a parentalidade e saúde integral da criança.

ABSTRACT Objective: The Mallampati classification system has been used to predict obstructive sleep apnea (OSA). Upper airway soft tissue structures are prone to fat deposition, and the tongue is the largest of these structures. Given that a higher Mallampati score is associated with a crowded oropharynx, we hypothesized that the Mallampati score is associated with tongue volume and an imbalance between tongue and mandible volumes. Methods: Adult males underwent clinical evaluation, polysomnography, and upper airway CT scans. Tongue and mandible volumes were calculated and compared by Mallampati class. Results: Eighty patients were included (mean age, 46.8 years). On average, the study participants were overweight (BMI, 29.3 ± 4.0 kg/m2) and had moderate OSA (an apnea-hypopnea index of 26.2 ± 26.7 events/h). Mallampati class IV patients were older than Mallampati class II patients (53 ± 9 years vs. 40 ± 12 years; p < 0.01), had a larger neck circumference (43 ± 3 cm vs. 40 ± 3 cm; p < 0.05), had more severe OSA (51 ± 27 events/h vs. 24 ± 23 events/h; p < 0.01), and had a larger tongue volume (152 ± 19 cm3 vs. 135 ± 18 cm3; p < 0.01). Mallampati class IV patients also had a larger tongue volume than did Mallampati class III patients (152 ± 19 cm3 vs. 135 ± 13 cm3; p < 0.05), as well as having a higher tongue to mandible volume ratio (2.5 ± 0.5 cm3 vs. 2.1 ± 0.4 cm3; p < 0.05). The Mallampati score was associated with the apnea-hypopnea index (r = 0.431, p < 0.001), BMI (r = 0.405, p < 0.001), neck and waist circumference (r = 0.393, p < 0.001), tongue volume (r = 0.283, p < 0.001), and tongue/mandible volume (r = 0.280, p = 0.012). Conclusions: The Mallampati score appears to be influenced by obesity, tongue enlargement, and upper airway crowding.

RESUMO Objetivo: A classificação de Mallampati tem sido usada para prever a apneia obstrutiva do sono (AOS). As estruturas de tecidos moles das vias aéreas superiores são propensas a deposição de gordura, sendo a língua a maior dessas estruturas. Como existe uma relação entre um grau mais elevado na classificação de Mallampati e maior obstrução da orofaringe, aventamos a hipótese de que a classificação de Mallampati está relacionada com o volume da língua e com um desequilíbrio entre o volume da língua e o da mandíbula. Métodos: Homens adultos foram submetidos a avaliação clínica, polissonografia e TC das vias aéreas superiores. O volume da língua e o volume da mandíbula foram calculados e comparados conforme a classificação de Mallampati. Resultados: Foram incluídos 80 pacientes (média de idade: 46,8 anos). Em média, os participantes do estudo apresentavam sobrepeso (IMC = 29,3 ± 4,0 kg/m2) e AOS moderada (índice de apneias e hipopneias = 26,2 ± 26,7 eventos/h). Os pacientes da classe IV de Mallampati eram mais velhos que os da classe II (53 ± 9 anos vs. 40 ± 12 anos; p < 0,01) e apresentavam maior circunferência do pescoço (43 ± 3 cm vs. 40 ± 3 cm; p < 0,05), AOS mais grave (51 ± 27 eventos/h vs. 24 ± 23 eventos/h; p < 0,01) e maior volume da língua (152 ± 19 cm3 vs. 135 ± 18 cm3; p < 0,01). Os pacientes da classe IV de Mallampati também apresentavam maior volume da língua que os da classe III (152 ± 19 cm3 vs. 135 ± 13 cm3; p < 0,05), bem como maior relação entre o volume da língua e o da mandíbula (2,5 ± 0,5 cm3 vs. 2,1 ± 0,4 cm3; p < 0,05). A classificação de Mallampati apresentou relação com o índice de apneias e hipopneias (r = 0,431, p < 0,001), o IMC (r = 0,405, p < 0,001), a circunferência do pescoço e da cintura (r = 0,393, p < 0,001), o volume da língua (r = 0,283, p < 0,001) e o volume da língua/volume da mandíbula (r = 0,280, p = 0,012). Conclusões: A classificação de Mallampati aparentemente é influenciada pela obesidade, aumento da língua e maior obstrução das vias aéreas superiores.

ABSTRACT Introduction: Risk factors and postoperative complications can worsen the condition of patients undergoing coronary artery bypass grafting; some of these factors and complications are closely related to mortality rate. Objective: To describe clinical factors and outcomes related to mortality of patients undergoing coronary artery bypass grafting and on invasive mechanical ventilation. Methods: This is a single-center retrospective data analysis of patients who underwent coronary artery bypass grafting on invasive mechanical ventilation between 2013 and 2019. Data regarding clinical characteristics, postoperative complications, intensive care unit and mechanical ventilation time, and their relationship with mortality were analyzed. Results: Four hundred seventy-two patients who underwent coronary artery bypass grafting entered the study. Their mean age was 62.3 years, and mean body mass index was 27.3. The mortality rate was 4%. Fifty percent of the patients who had ventilator-associated pneumonia died. Considering the patients who underwent hemotherapy and hemodialysis, 20% and 33% died, respectively. Days of intensive care unit stay and high Acute Physiology and Chronic Health Evaluation score and Simplified Acute Physiology Score were significantly related to death. Conclusion: Factors and clinical conditions such as the patients’ age, associated comorbidities, the occurrence of ventilator-associated pneumonia, length of stay in the intensive care unit, and mechanical ventilation time are related to higher mortality in patients undergoing coronary artery bypass grafting.

ABSTRACT Objective: to analyze associations between moral distress and ethical issues experienced by nurses during COVID-19. Method: this is a quantitative, cross-sectional study, carried out with 101 nurses working in university hospitals, between March and May 2022, through socio-occupational issues, ethical implications that lead to moral distress, and the Ethical Issues Experienced by Nurses in Emergency Questionnaire, adapted and validated for Brazilians. Descriptive statistics, ANOVA, Pearson’s chi-square and multiple linear regression were used, and p<0.05 was adopted. Results: the 9 elements attributed to ethical implications were classified with some degree of importance for moral distress, with the highest average in the following items: I feel a commitment, responsibility and moral obligation to provide care to the infected population (4.26) and I have the knowledge or experience to work on the frontline of COVID-19 (3.44). The association of moral distress with ethical issues showed that individuals who attributed high importance to ethical implications also had ethical issues at a moderate/high level, emphasizing a higher mean in questions of “patient care” (4.07). It was evident that participants’ moral distress was more affected by the following constructs: “perception of hospital measures against COVID-19” (p=.000), “ethical issues in patient care” (p=.000) and “perception of social stigmatization” (p=.000). Conclusion: when relating ethical issues to moral distress, it was possible to show that COVID-19 generated an abrupt change in nurses’ work routine, which hinders adequate decision-making in situations involving issues beyond care, but also at a professional and organizational level, leading to the experience of moral distress.

RESUMEN Objetivo: analizar asociaciones entre sufrimiento moral y problemas éticos vividos por enfermeros durante la COVID-19. Método: estudio cuantitativo, transversal, realizado con 101 enfermeros que actúan en hospitales universitarios, entre marzo y mayo de 2022, a través de cuestiones sociolaborales, implicaciones éticas que conducen al sufrimiento moral y del Ethical Issues Experienced by Nurses in Emergency Questionnaire, adaptado y validado para brasileños. Se utilizó estadística descriptiva, ANOVA, chi-cuadrado de Pearson y regresión lineal múltiple, y se adoptó p<0,05. Resultados: los 9 elementos atribuidos a implicaciones éticas fueron clasificados con algún grado de importancia para el sufrimiento moral, con mayor promedio en los siguientes ítems: Siento compromiso, responsabilidad y obligación moral de brindar atención a la población infectada (4,26) y tengo conocimiento o experiencia para actuar en la primera línea de COVID-19 (3,44). La asociación del sufrimiento moral con los problemas éticos mostró que los individuos que atribuían alta importancia a las implicaciones éticas también presentaban problemas éticos en un nivel moderado/alto, destacando un promedio más alto en cuestiones de “atención al paciente” (4,07). Se evidenció que el malestar moral de los participantes se vio más afectado por los siguientes constructos: “percepción de las medidas hospitalarias frente al COVID-19” (p=,000), “problemas éticos en la atención al paciente” (p=.000) y “percepción de estigmatización social” (p=,000). Conclusión: al relacionar los problemas éticos con el sufrimiento moral, se pudo evidenciar que el COVID-19 generó un cambio abrupto en la rutina de trabajo de los enfermeros, lo que dificulta la toma de decisiones adecuadas en situaciones que involucran cuestiones más allá del cuidado, pero también a nivel profesional y organizacional que lleva a la experiencia del sufrimiento moral.

Empregou-se estatística descritiva, ANOVA, Qui-quadrado de Pearson e regressão linear múltipla, e adotou-se p<0,05. Resultados: os 09 elementos atribuídos às implicações éticas foram classificados com algum grau de importância para o sofrimento moral, apresentando maior média nos seguintes itens: sinto compromisso, responsabilidade e obrigação moral em prestar cuidados à população infectada (4,26) e possuo conhecimento ou experiência para atuar na linha de frente à COVID-19 (3,44). A associação do sofrimento moral aos problemas éticos demonstrou que os indivíduos que atribuíram alta importância às implicações éticas também apresentavam problemas éticos em nível moderado/alto, ressaltando maior média nas questões de “cuidado ao paciente” (4,07). Evidenciou-se que o sofrimento moral dos participantes foi mais afetado pelos seguintes construtos: “percepção das medidas hospitalares contra a COVID-19” (p=,000), “problemas éticos no atendimento a pacientes” (p=,000) e “percepção da estigmatização social” (p=,000). Conclusão: ao relacionar os problemas éticos ao sofrimento moral, foi possível evidenciar que a COVID-19 gerou uma mudança abrupta na rotina de trabalho dos enfermeiros, o que dificulta a tomada de decisão adequada diante situações que envolve questões além do cuidado, mas também em nível profissional e organizacional levando a vivência de sofrimento moral.
