A quimioterapia para a leishmaniose não é satisfatória e existem hoje, várias drogas e esquemas terapêuticos em teste. Além do tratamento ideal, busca-se um critério de cura efetivo, onde a análise da histopatologia da cicatriz poderá ser de grande valia. Este trabalho propõe caracterizar o padrão histopatológico de casos humanos de leishmaniose tegumentar americana, em 32 pacientes do município de Caratinga-MG, antes e após o tratamento com os seguintes métodos terapêuticos: 1) leishvacin + glucantime; 2) leishvacin + BCG associado ao glucantime; 3) glucantime; 4) leishvacin + BCG. Foram colhidos fragmentos das lesões de todos os pacientes, através de biópsias, antes e após o tratamento, com diagnóstico de cura. Após análise das lâminas, as preparações foram descritas, do ponto de vista histopatológico, e agrupadas levando em conta a prevalência e a significância do elemento característico. Tal processo resultou na classificação: 1. reação exsudativa; 2. reação exsudativa giganto-celular; 3. reação exsudativa produtiva; 4. reação exsudativa produtiva giganto-celular; 5. reação exsudativa produtiva necrótica; 6. reação necrótica exsudativa; 7. reação produtiva exsudativa; 8. reação produtiva giganto-celular; 9. reação produtiva exsudativa giganto-celular; 10. reação produtiva exsudativa giganto-celular granulomatosa; 11. reação produtiva e 12. reação produtiva cicatricial (cura histopatológica). Observamos após tal análise, que nem sempre a cura clínica coincide com a cura histopatológica.

Chemical therapy for the treatment of leishmaniasis is still inadequate, and a number of drugs and therapeutic programs are being tested. Besides treatment, the ultimate goal is an effective cure, and histopathological analyses of the lesion cicatrices constitute an important measure of treatment success, or otherwise, in this respect. In this paper, we describe histopathological patterns in cases of American cutaneous leishmaniasis in 32 patients from the municipality of Caratinga, Minas Gerais, Brazil, before and after treatment with the following therapeutic methodos: l) leishvacin + glucantime; 2) leishvacin + BCG associated with glucantime; 3) glucantime; 4) leishvacin + BCG. Lesion fragments were collected from all patients by biopsy prior to, and approximately 30 days after, each treatment which resulted in a clinical diagnosis of cure. Following the analysis of slides, the preparations were described from a histopathological point of view and grouped taking into account the prevalence or significance of the characteristic elements. This process resulted in the following classification: 1. exsudative reaction (ER); 2. exsudative giant cell reaction (EGCR); 3. exsudative productive reaction (EPR); 4. exsudative productive giant cell reaction (EPGCR); 5. exsudative productive necrotic reaction (EPNR); 6. necrotic exsudative reaction (NER); 7. productive exsudative reaction (PER), 8. productive giant cell reaction (PGCR); 9. productive exsudative giant cell reaction (PEGCR); 10. productive exsudative giant cell granulomatous reaction (PEGCGR); 11. productive reaction (PR) and 12. productive cicatricial (cure) reaction (PCR). After this analysis, it was noted that clinical cure did not always coincide with histopathological cure.

O método daperoxidase-antiperoxidase foi utilizado para estudar as propriedades imunocitoquimicas de Leishmanias e de amastigotas do Trypanosoma cruzi, in situ, após os tecidos terem sido submetidos a diferentes tipos de fixação. Anti-soros foram obtidos de coelhos cronicamente infectados com três cepas de T. cruzi ou imunizados com L. mexicana ámazonensis e L. braziliensis guyanensis e aplicados nos cortes histológicos de 5 µm de espessura. Os antígenos de T. cruzi foram corados muito bem pelos três soros anti-T. cruzi e pelos dois soros anti-Leishmania com diluições entre 1:1.000 e 1:2.000. Diferentemente, os antígenos dç Leishmania foram revelados pelos soros anti- Leishmania somente em baixas diluições, ou seja, entre 1:60 e 1:160 enquanto que os soros anti-T. cruzi, mesmo nestas diluições baixas, proporcionaram colorações fracas e irregulares auando usados para revelar Leishmania. Embora não haja explicação clara para esta reação imunocitoquimica cruzada "reversa-monodirecional" entre Leishmania e amastigotas de T. cruzi os resultados do presente trabalho mostram que anticorpos policlonais contra diferentes espécies de Leishmania, quando usados para detecção imunocitoquímica de Leishmania e T. cruzi in situ, reagem mais fortemente com amastigotas de T. cruzi do que com espécies homólogas.

The unlabelled antibody peroxidase-antiperoxidase method was used to study the immunocytochemical properties of Leishmania and Trypanosoma cruzi amastigotes in situ after tissues had been submitted to different fixation procedures. Antisera were obtained from rabbits chronically infected with different strains of T. cruzi or immunized with L. mexicana amazonensis and L. braziliensis guyanensis, and were applied on 5 µm thick sections. T. cruzi antigens were well stained by the three anti-T. cruzi sera and the two anti-heis.hmama.sera at optimum dilution between 1:1,000 and 1:2,000, regardless the parasite strain. Differently, the leishmanial antigens were revealed by Leishmania sera only at low dilutions (between 1:60 -1:160), whereas the anti-T. cruzi sera, at these low dilutions, gave rather weak stainings. Although there is no clear explanation for this immunocytochemical "reverse-monodirectional" cross-reactivity between Leishmania and T. cruzi, the present results show that polyclonal antibodies agains Leishmania species, when used for immunocytochemical detection of these parasites in situ, react more strongly with T. cruzi amastigotes than with the homologous amastigotes.

Duas cepas de Leishmania originalmente isoladas in vitro de casos humanos de leishmaniose cutânea e que ab initio não infectaram animais de laboratório, tornaram-se infectantes para hamnsters após serem mantidos por vários anos em meio de cultura quimicamente definido. Foram realizadas observações sobre o crescimento de promastigotas in vitro, curso da infecção em hamsters, morfologia das amastigotas, mobilidade eletroforética de oito enzimas solúveis. Foram obtidas informações sobre a densidade de flutuação do n-DNA e do k-DNA e uma das cepas foi testada contra anticorpos monoclonais. Ambas as cepas permanecem sem identificação precisa.

Attempts have been made to characterize two strains of Leishmania that became infective to golden hamsters only after they had been maintained for several years in a chemically defined culture medium. Observations were made on the growth rates of promastigotes in vitro, course of infection in hamsters, morphology of amastigotes, and electrophoretic mobility patterns of eight isoenzymes. Information was obtained about the buoyant densities of n-DNA and k-DNA, and one strain was tested against monoclonal antibodies. The identity of both strains remains obscure.

Os Autores avaliaram os antígenos de Cysticercus cellulosae para o imunodiagnóstico da cistieercose humana pela reação de hemaglutinação indireta. Observaram que os antígenos de escólex (ES) e ES pico II são mais específicos que os antígenos de C. cellulosae total (CC), líquido vesicular e de membrana (ME). Usando soros de pacientes com cistieercose comprovada parasitologicamente, obtiveram uma sensibilidade de 0,80 para o ES, 0,84 para o ES II e especificidade 1,0, quando os soros foram usados na diluição 1:8 para o antígeno ES e 1:16 para o antígeno ES II.

The Authors evaluated the use of antigens from Cysticercus cellulosae in the immunodiagnosis of human cysticercosis by haemagglutination test. The extract prepared from the es-colex and its chromatographic peak II were more specific than those from whole larvae, vesicular liquid and membrane antigens. Using sera from patients with parasitologically proven cysticercosis, positive reactions were obtained with a sensitivity index of 0.80 for escolex antigen and 0.84 escolex peak II antigen. Specificity of 1.0 was obtained for sera diluted at 1:8 for ES and 1:16 for ES II antigens.

Reações de fixação de complemento feitas em três laboratórios, usando basicamente a mesma técnica, apresentaram resultados discrepantes com soros chagásicos. Somente 16 soros de um lote de 26 pacientes em fase aguda de infecção tiveram resultados positivos nos três laboratórios, embora nenhum deles fosse unanimente negativo. Quando 224 soros de casos parasitologicamente comprovados de doença de Chagas foram examinados, resultados positivos foram obtidos em 95% deles em um laboratório, 89% em outro e apenas 64% no terceiro. Os resultados mostraram que as discrepãncias observadas dependiam da qualidade dos antígenos usados, dando ênfase à necessidade não só da padronização dos preparados antigênicos, como também da metodologia do teste, a fim de se poder ter resultados comparáveis quanto à sensibilidade e especificidade da reação de Guerreiro-Machado.

Complement-fixation tests performed in three laborarories, using basically the same tecnique, presented discrepant results with Chagasic sera. Only 16 sera from a lot of 26 in the acute phase of the infection had positive tests in the three laboratories, although none of them presented a consensus of negative tests. When 224 sera from parasitologically proven cases of Chagas disease were tested, positive results were obtained in 95% of them in one laboratory, 89% in the second one and only 64% in the other. The results showed that the discrepancies observeddependedof the quality of the antigens used, stressing the necessity of the standardization not only of the antigen preparations but also of the methodology of the tests, in order to have comparable results in the Guerreiro-Machado tests, in sensitivity and specificity.
