Abstract Over the last three decades, stroke care has undergone significant transformations mainly driven by the introduction of reperfusion therapy and the organization of systems of care. Patients receiving treatment through a well-structured stroke service have a much higher chance of favorable outcomes, thereby decreasing both disability and mortality. In this article, we reviewed the scientific evidence for stroke reperfusion therapy, including thrombolysis and thrombectomy, and its implementation in the public health system in Brazil.

Resumo Nas últimas três décadas, o tratamento do AVC sofreu transformações significativas, impulsionadas principalmente pela introdução das terapias de reperfusão e pela organização dos serviços de AVC. Os pacientes que recebem tratamento em um serviço de AVC bem estruturado têm uma probabilidade muito maior de resultados favoráveis, diminuindo assim a incapacidade funcional e a mortalidade. Neste artigo, revisamos as evidências científicas para as terapias de reperfusão do AVC, incluindo trombólise e trombectomia e sua implementação no sistema público de saúde no Brasil.

Abstract Introduction: Chimney EVAR (ChEVAR) for the treatment of complex abdominal aortic aneurysm (AAA) has been largely relegated for when fenestrated endografts are unavailable, especially due to durability concerns. However, the off-the-shelf nature of ChEVAR makes it a good option for emergent AAA repair. We report our institutional experience in ChEVAR in an urgent setting. Methods: ChEVAR procedures were collected f rom 2019 to 2023 in a tertiary hospital, and a retrospective analysis was performed. This includes gathered information f rom electronic medical records, surgical reports and the picture archiving system. Descriptive statistics were applied. The main endpoints were technical success,30-day and 1-year survival. Results: Five patients were submitted to urgent aneurysm repair using the chEVAR technique. Average age was 73,4 years and all patients were male. Arterial hypertension was present in 100% patients and all patients were ASA 4. Indications for surgery were post EVAR type 1a endoleak with associated rupture or abdominal pain in two patients; symptomatic/contained rupture of pararenal AAA in two patients; and contained rupture of a thoracoabdominal aneurysm in one patient. A total of eight target vessels were catheterized: two patients required single-vessel chimney and the remainder two-vessel chimney. Target vessels were two superior mesenteric arteries and six renal arteries. Technical success rate was 100% and 30-day mortality was 0%. There were no major complications. Follow-up time is 20.2 months (4.7-38). Target vessel patency during the follow-up period was 87.5%. Three patients (60%) died due to non-aortic related pathologies, on average 18 months after surgery (4.7-38), with a 1-year survival of 80%. Conclusion: Our experience with ChEVAR for emergent AAA repair is satisfactory, with high technical success rates and low short-term mortality. Sac regression, low rates of target vessel occlusion and type 1a endoleaks reveal a favourable profile for aneurysm exclusion. ChEVAR is a viable option in emergent setting for patients unfit for open repair.

ABSTRACT Purpose: This study aimed to use computational models for simulating the movement of respiratory droplets when assessing the efficacy of standard slit-lamp shield versus a new shield designed for increased clinician comfort as well as adequate protection. Methods: Simulations were performed using the commercial software Star-CCM+. Respiratory droplets were assumed to be 100% water in volume fraction with particle diameter distribution represented by a geometric mean of 74.4 (±1.5 standard deviation) μm over a 4-min duration. The total mass of respiratory droplets expelled from patients’ mouths and droplet accumulation on the manikin were measured under the following three conditions: with no slit-lamp shield, using the standard slit-lamp shield, and using our new proposed shield. Results: The total accumulated water droplet mass (kilogram) and percentage of expelled mass accumulated on the shield under the three aforementioned conditions were as follows: 5.84e-10 kg (28% of the total weight of particle emitted that settled on the manikin), 9.14e-13 kg (0.045%), and 3.19e-13 (0.015%), respectively. The standard shield could shield off 99.83% of the particles that would otherwise be deposited on the manikin, which is comparable to 99.95% for the proposed design. Conclusion: Slit-lamp shields are effective infection control tools against respiratory droplets. The proposed shield showed comparable effectiveness compared with conventional slit-lamp shields, but with potentially enhanced ergonomics for ophthalmologists during slit-lamp examinations.

RESUMO Introdução: Os oftalmologistas têm alto risco de contrair a doença do Coronavírus-19 devido à proximidade com os pacientes durante os exames com lâmpada de fenda. Usamos um modelo de computação para avaliar a eficácia das proteções para lâmpadas de fenda e propusemos uma nova proteção ergonomicamente projetada. Métodos: As simulações foram realizadas no software comercial Star-CCM +. Os aerossóis de gotículas foram considerados 100% de água em fração de volume com distribuição de diâmetro de partícula representada por uma média geométrica de 74,4 ± 1,5 (desvio padrão) μm ao longo de uma duração de quatro minutos. A massa total de gotículas de água acumulada no manequim e a massa expelida pela boca do paciente foram medidas em três condições diferentes: 1) Sem protetor de lâmpada de fenda, 2) com protetor padrão, 3) Com o novo protetor proposto. Resultados: A massa total acumulada das gotas de água (kg) e a porcentagem da massa expelida acumulada no escudo para cada uma das respectivas condições foram; 1) 5,84e-10 kg (28% do peso total da partícula emitida que assentou no manequim), 2) 9,14e-13 kg (0,045%), 3,19e-13 (0,015%). O escudo padrão foi capaz de proteger 99,83% das partículas que, de outra forma, teriam se depositado no manequim, o que é semelhante a 99,95% para o projeto proposto. Conclusão: Protetores com lâmpada de fenda são ferramentas eficazes de controle de infecção contra gotículas respiratórias. O protetor proposto mostrou eficácia comparável em comparação com os protetores de lâmpada de fenda convencionais, mas potencialmente oferece uma melhor ergonomia para oftalmologistas durante o exame de lâmpada de fenda.

Abstract Background Phenylketonuria (PKU) is an inborn error of metabolism caused by deficient activity of phenylalanine hydroxylase. In Brazil, the National Neonatal Screening Program enables early treatment of patients with PKU, which prevents them from developing severe neurological damage and mental disabilities. However, between 20 and 30% of early-treated patients with PKU present focal cognitive deficits, including deficits in working memory, processing speed, and psychiatric symptoms such as anxiety, depression, and attention deficit hyperactivity disorder (ADHD). Therefore, age-specific neuropsychiatric and cognitive tests are important components of PKU patient care. To date, there are no officially approved guidelines or recommendations of tools in Portuguese validated for use in Brazil that could be applied to assess these parameters in patients with PKU. Objective To recommend tools validated for use in Brazil that can be used in daily clinical practice to assess quality of life and neuropsychological outcomes in patients with PKU. Methods Six Brazilian experts discussed about eligible tools based on their clinical experience, the feasibility of their use in clinical routines, and their availability in public health services. Before the meeting, an independent review of the literature was conducted to identify the currently validated tools in Brazil, using the MEDLINE and SciELO databases. Results The experts recommended nine tools to assess quality of life (Peds-QL, SF-36 or WHOQOL-bref), executive function (BRIEF or Bayley-III), IQ (SONR 2½-7[a] or WASI) and ADHD (MTA-SNAP-IV and ASRS). Conclusions These instruments may be easily incorporated into clinical practice and improve the quality of multidisciplinary care of patients with PKU.

Resumo Antecedentes A fenilcetonúria (PKU) é um erro inato do metabolismo causado pela atividade deficiente da fenilalanina hidroxilase. No Brasil, o Programa Nacional de Triagem Neonatal permite o tratamento precoce de pacientes com PKU, o que os impede de desenvolver danos neurológicos e deficiências intelectuais graves. No entanto, já foi descrito que de 20 a 30% dos pacientes tratados precocemente com PKU apresentam déficits cognitivos focais, incluindo déficits na memória de trabalho, velocidade de processamento e sintomas psiquiátricos como ansiedade, depressão e Transtorno de Déficit de Atenção e Hiperatividade (TDAH). Neste sentido, testes neuropsiquiátricos e cognitivos são componentes importantes no cuidado destes pacientes. Atualmente, não existe um compêndio científico ou recomendações de ferramentas validadas em português para avaliar a saúde mental em pacientes brasileiros com PKU. Objetivo Recomendar ferramentas validadas localmente para avaliar a qualidade de vida e aspectos neuropsicológicos de pacientes com PKU. Métodos Seis especialistas brasileiros discutiram as ferramentas mais apropriadas com base em suas experiências clínicas, a viabilidade de realizar as avaliações na rotina clínica, e o acesso às ferramentas na saúde pública. Antes da reunião, foi realizada uma revisão independente da literatura para identificar as ferramentas validadas no Brasil, utilizando as bases de dados MEDLINE e Scielo. Resultados Os especialistas recomendaram nove ferramentas para avaliar a qualidade de vida (Peds-QL, SF-36 ou WHOQOL-bref), função executiva (BRIEF ou Bayley-III), QI (SONR 2½-7[a] ou WASI) e TDAH (MTA-SNAP-IV e ASRS). Conclusões Estes instrumentos podem ser facilmente incorporados na prática clínica e melhorar a qualidade dos cuidados multidisciplinares dos pacientes com PKU.

ABSTRACT BACKGROUND AND OBJECTIVES: Conventional electrodiagnostic studies (EDX) are frequently used to support the diagnosis of peripheral neuropathic pain. However, routine EDX has poor diagnostic yield for identifying small fiber neuropathy, which may be cause of neuropathic pain in some patients. This study aimed to assess the gain in diagnostic yield brought by adding pain-related evoked potentials with concentric electrode (CN-PREP) and nociceptive withdrawal reflex (NWR) assessments to EDX. METHODS: Transversal observational accuracy study which included patients referred to routine EDX in a tertiary-care hospital who reported chronic neuropathic pain in their lower limbs. Besides routine EDX, subjects underwent CN-PREP and NWR assessments. Diagnostic yield and tolerability were examined and compared between test studies. RESULTS: The study enrolled 100 patients (54% female), with 57 ± 12 years. EDX was altered in 47% of all patients. The addition of CN-PREP alone, and NWR combined with CN-PREP increased diagnostic yield to 69% and 72%, respectively. CN-PREP proved to be well tolerable, while NWR was associated with higher test-related pain intensity and discontinuation rate (9% vs. 0%). Considering EDX as the reference test, CN-PREP sensitivity was 85.1% and specificity 58.5%. CONCLUSION: Combining CN-PREP with the routine EDX for patients with neuropathic pain is feasible and results in increased diagnostic yield. Conversely, the addition of NWR to the aforementioned tests provides little improvement to this yield and is less tolerable to the patient. Further studies are needed to determine the actual sensitivity and specificity of CN-PREP when compared to the gold-standard for small fiber neuropathy diagnosis, i.e. intraepidermal nerve fiber density assessment.

RESUMO JUSTIFICATIVA E OBJETIVOS: Estudos convencionais de eletrodiagnóstico (EDX) são frequentemente usados para apoiar o diagnóstico de dor neuropática periférica. No entanto, o EDX de rotina tem baixo rendimento diagnóstico para identificar neuropatia de pequenas fibras. O objetivo deste estudo foi avaliar o ganho no rendimento diagnóstico pela adição de avaliações de potenciais evocados relacionados à dor com eletrodo concêntrico (CN-PREP) e reflexo de retirada nociceptiva (NWR) ao EDX. MÉTODOS: Estudo de precisão observacional transversal que incluiu pacientes encaminhados para EDX de rotina com dor neuropática crônica em membros inferiores. Além do EDX de rotina, os indivíduos foram submetidos às avaliações CN-PREP e NWR. O rendimento diagnóstico e a tolerabilidade foram examinados e comparados entre os estudos de teste. RESULTADOS: O estudo envolveu 100 pacientes (54% mulheres), com 57 ± 12 anos. O EDX estava alterado em 47%. A adição de CN-PREP sozinho e NWR combinado com CN-PREP aumentou o rendimento diagnóstico para 69% e 72%, respectivamente. O CN-PREP provou ser bem tolerável, enquanto o NWR foi associado a maior intensidade de dor relacionada ao teste e taxa de descontinuação (9% vs. 0%). Considerando o EDX como teste de referência, a sensibilidade do CN-PREP foi de 85,1% e a especificidade de 58,5%. CONCLUSÃO: A combinação do CN-PREP com o EDX de rotina para pacientes com dor neuropática é viável e resulta em maior rendimento diagnóstico. Já a adição de NWR aos testes mencionados fornece pouca melhora nesse rendimento e é menos tolerável para o paciente. Mais estudos são necessários para determinar a real sensibilidade e especificidade do CN-PREP quando comparado ao padrão-ouro para diagnóstico de neuropatia de pequenas fibras, ou seja, a avaliação da densidade de fibras nervosas intraepidérmicas.

Abstract This article aims to demonstrate how research in communication design can contribute to disseminating community practices in economically disadvantaged contexts, such as in social housing neighbourhoods. This study stems from the ECHO research project, where the main mission is to understand and document two self-initiated community practices in the social housing neighbourhood of Balteiro, in Vila Nova de Gaia, Portugal, to disseminate them and foster their replication in similar social contexts. It begins with a literature review on the role of design on behalf of communities, focusing, in particular, on the concepts of social design and universal design, which substantiate the relevance and pertinence of the issue under study. Then, it describes the case study of Balteiro. It presents the methodologies adopted in this research, namely the interviews conducted to learn about the history of the two community practices. This provided the basis for developing a preliminary study for an online platform and a series of documentary films. This study proved the importance of involving designers in this dissemination process through multidisciplinary teams. The research work in the field should be close to the community, the institutions and the different social actors involved to understand the complexity of the study context and define how design should operate. The empirical experience from this study has shown that work in the social area should be developed locally, focused on small communities or informal organisations, and aware that results are achieved slowly and gradually.

Resumo No presente artigo pretende-se demonstrar de que modo a investigação em design de comunicação pode contribuir para a disseminação de práticas comunitárias em contextos economicamente desfavorecidos, como é o caso dos bairros sociais. Este estudo surge no âmbito do projeto de investigação ECHO, onde a principal missão é conhecer e documentar duas práticas comunitárias auto-iniciadas no bairro social do Balteiro, em Vila Nova de Gaia, Portugal, com o objetivo de as divulgar e estimular a sua replicação em contextos sociais análogos. Começa-se por apresentar uma revisão de literatura sobre o papel do design ao serviço das comunidades, focada, em particular, nos conceitos de design social e do design universal, que fundamenta a relevância e pertinência da problemática estudada. De seguida, descreve-se o caso de estudo do Balteiro. Apresentam-se as metodologias adotadas nesta investigação, nomeadamente, o conjunto de entrevistas realizadas com o objetivo de conhecer a história das duas práticas comunitárias, e que serviram de base ao desenvolvimento de um estudo prévio para uma plataforma online e um conjunto de documentários. Este estudo comprovou a importância do envolvimento dos designers neste processo de disseminação, em equipas que deverão ser multidisciplinares. O trabalho de investigação no terreno deve ser próximo da comunidade, das instituições e dos diferentes atores sociais envolvidos, de forma a compreender a complexidade do contexto de estudo e definir o modo de atuação do design. A experiência empírica adquirida com este estudo demonstrou que o trabalho na área social deve ser desenvolvido a nível local, focado em pequenas comunidades ou organizações informais, com a consciência de que os resultados se atingem de forma lenta e gradual.

Abstract Introduction: The incidence of rectal neuroendocrine tumors (r-NETs) is increasing, and most small r-NETs can be treated endoscopically. The optimal endoscopic approach is still debatable. Conventional endoscopic mucosal resection (EMR) leads to frequent incomplete resection. Endoscopic submucosal dissection (ESD) allows higher complete resection rates but is also associated with higher complication rates. According to some studies, capassisted EMR (EMR-C) is an effective and safe alternative for endoscopic resection of r-NETs. Aims: This study aimed to evaluate the efficacy and safety of EMR-C for r-NETs ≤10 mm without muscularis propria invasion or lymphovascular infiltration. Methods: Single-center prospective study including consecutive patients with r-NETs ≤10 mm without muscularis propria invasion or lymphovascular invasion confirmed by endoscopic ultrasound (EUS), submitted to EMR-C between January 2017 and September 2021. Demographic, endoscopic, histopathologic, and follow-up data were retrieved from medical records. Results: A total of 13 patients (male: 54%; n = 7) with a median age of 64 (interquartile range: 54-76) years were included. Most lesions were located at the lower rectum (69.2%, n = 9), and median lesion size was 6 (interquartile range: 4.5-7.5) mm. On EUS evaluation, 69.2% (n = 9) of tumors were limited to muscularis mucosa. EUS accuracy for the depth of invasion was 84.6%. We found a strong correlation between size measurements by histology and EUS (r = 0.83, p < 0.01). Overall, 15.4% (n = 2) were recurrent r-NETs and had been pretreated by conventional EMR. Resection was histologically complete in 92% (n = 12) of cases. Histologic analysis revealed grade 1 tumor in 76.9% (n = 10) of cases. Ki-67 index was inferior to 3% in 84.6% (n = 11) of cases. The median procedure time was 5 (interquartile range: 4-8) min. Only 1 case of intraprocedural bleeding was reported and was successfully controlled endoscopically. Follow-up was available in 92% (n = 12) of cases with a median follow-up of 6 (interquartile range: 12-24) months with no evidence of residual or recurrent lesion on endoscopic or EUS evaluation. Conclusion: EMR-C is fast, safe, and effective for resection of small r-NETs without high-risk features. EUS accurately assesses risk factors. Prospective comparative trials are needed to define the best endoscopic approach.

Resumo Introdução: Os tumores neuroendócrinos do reto (r-NETs) apresentam incidência crescente. A maioria dos tumores de pequenas dimensões pode ser excisada endoscopicamente, no entanto, a abordagem ótima é controversa. A mucosectomia convencional associa-se, frequentemente, a resseção endoscópica incompleta. A disseção endoscópica submucosa (ESD) permite elevadas taxas de resseção completa, mas é tecnicamente complexa e associa-se a maior número de complicações. Alguns estudos sugerem a mucosectomia assistida por cap (EMR-C) como uma alternativa eficaz e segura. Objetivo: Este estudo pretendeu avaliar a eficácia e segurança da mucosectomia com cap na resseção de r-NETs com dimensões ≤10 mm, sem invasão da muscularis própria nem infiltração linfovascular. Material e Métodos: Estudo prospetivo unicêntrico incluindo consecutivamente r-NETs com ≤10 mm, sem invasão da muscularis própria ou linfovascular confirmada em ultrassonografia endoscópica (EUS), submetidos a mucosectomia assistida cap entre janeiro de 2017 e setembro de 2021. Colheita de dados demográficos, clínicos e histopatológicos através de registos médicos eletrónicos. Resultados: Incluídos 13 doentes (género masculino: 54%; n = 7) com idade mediana de 64 (intervalo interquartil [IIQ]: 54-76) anos. A maioria das lesões localizava-se no reto inferior (69.2%; n = 9) e apresentava tamanho mediano de 6 (IIQ: 4.5-7.5) mm. Na avaliação por EUS, 69.2% (n = 9) encontravam-se limitados à muscularis mucosa. A acuidade da EUS na avaliação do envolvimento das camadas da parede retal foi de 84.6% e o tamanho avaliado por EUS correlacionou-se fortemente com o medido na histologia (r = 0.83, p < 0.01). Dois casos (15.4%) corresponderam a recorrências de mucosectomias convencionais prévias. A resseção foi macroscópica e histologicamente completa em 92% (n = 12) dos casos. A análise histológica revelou 76.9% (n = 10) tumores de grau 1. O índice Ki-67 foi inferior a 3% em 84.6% (n = 11) dos casos. O tempo mediano de procedimento foi 5 (IIQ: 4-8) minutos. Verificou-se apenas um caso de hemorragia intraprocedimento resolvida endoscopicamente. O seguimento de 92% dos casos (n = 12) com mediana de 6 (IIQ:12-24) meses não revelou lesão residual ou recorrên-cia em avaliações endoscópica e ultrassonográfica. Discussão/Conclusão: A EMR-C é uma técnica endoscópica segura, rápida e efetiva para a resseção de r-TNEs pequenos sem fatores de alto risco. A EUS apresenta elevada acuidade na avaliação dos fatores de risco. Estudos comparativos prospetivos são necessários para estabelecimento da abordagem endoscópica mais profícua.

Abstract In this work, measuring capabilities of a Sagnac Interferometer-based Distributed Optical Fiber Sensor (DOFS) system are demonstrated by comparing its results to a conventional amplitude-based Distributed Acoustic Sensing (DAS) system. Through a set of three different experiments, it was demonstrated that both sensing systems can detect and locate dynamical deformation along the fiber, in addition to determining its frequency components. The coherent amplitude-based DAS presented a lower background noise. In contrast, even though the Sagnac interferometry-based system presented low-frequency noise in the measurements, it is less susceptible to harmonic distortion for higher amplitude deformations since its sensing principles rely on the measurement of the accumulated phase rather than the amplitude of the backscattered signal purely. Abstract This work describes the circulation over the continental slope and the São Paulo Plateau in the Santos Basin during the SANSED winter 2019 survey. The cruise consisted of four legs in the period between June, 11 and August, 03 2019. The observed circulation is dominated by the Atlantic southwestern boundary current regime and remotely-generated anticyclones and cyclones. The former is composed by the Brazil Current, the Intermediate Western Boundary Current and their mesoscale meanders; the latter are 300km vortical rings with origin in the eastern side of the South Atlantic Basin. A Lagrangian scheme applied over satellite altimeter maps indicate that the origin of these rings is primarily the Cape Basin of South Africa. The interaction between the boundary currents, their cyclonic meanders, and the anticyclonic rings is complex, and varies widely. During the SANSED winter 2019 survey period, three anticyclones interacted with the Brazil Current, instabilizing it, forming dipoles with the current cyclonic meanders, leading to their downstream propagation. Ancienter cyclonic eddies within Santos Basin may interfere with the propagation of the large anticyclones further south. In addition, the continuous arrival of remotely-originated anticyclones, the larger portion over the São Paulo Plateau presented a tendency of counter-clockwise circulation during the whole cruise period.

ABSTRACT The expression ‘you need to know to conserve’ is a well-known cliche among biologists. Documenting the richness of a group of organisms is the first step towards understanding biodiversity and preparing efficient conservation plans. In this context, many efforts have been made to quantify the number of species on Earth and estimate the number of species still unknown to science. A few countries have complete and integrated databases estimating the approximate number of species recorded for their territory, particularly in the Global South. In Brazil, a country of continental dimensions, revealing the richness of the second most diverse clade of invertebrates (=Mollusca) has been a goal of taxonomists. Recently, in an unprecedented, collective, and integrated effort among Brazilian malacologists, it was possible to estimate how many valid species of molluscs are there in Brazil. In this effort, more than 30 mollusc experts joined together to update the Taxonomic Catalogue of the Brazilian Fauna (TCBF), a governmental website that allows a quick and real-time updating of all Metazoan. So far, more than 5,000 updates have been made in TCBF, indicating the presence of 3,552 valid species of molluscs in Brazil, distributed among the main clades as follows: Caudofoveata (10 spp.), Solenogastres (6 spp.), Polyplacophora (35 spp.), Scaphopoda (43 spp.), Cephalopoda (92 spp.), Bivalvia (629 spp.) and Gastropoda (2,737 spp.). The present study, in addition to demonstrating for the first time the richness of Brazilian molluscs, also presents the state of the art of this important phylum of invertebrates highlighting its most representative and neglected groups.

Abstract: The aim of this paper is to investigate trade unionists leaders' perspective on Industry 4.0. Questionnaires and interviews were carried out. Two conclusions were pointed out: a fragmented understanding of the concept and the low participation of workers in the implementation process. Looking forward to a just transition, the improvement of either the conceptual grasp and the workers' participation are necessary.

Resumo: Este artigo apresenta a perspectiva dos dirigentes sindicais sobre o atual cenário da I4.0 no setor metalúrgico brasileiro. Foram aplicados questionários e realizada entrevista em uma amostra representativa. Evidencia-se i. compreensão fragmentada do conceito ii. baixo envolvimento dos tralhadores no processo de implementação. Conclui-se que ampliar a participação e o entendimento conceitual dos dirigentes faz-se necessário para que uma transição justa ocorra.

ABSTRACT Objective: the aim of this study was to compare the results of open and videolaparoscopic transvesical prostatectomy techniques in the treatment of benign prostatic hyperplasia (BPH) in a tertiary hospital. Methods: we reviewed medical records of patients who underwent transvesical adenectomy due to BPH between March 2019 and March 2021 at the urology service of Hospital de Clínicas do Paraná (HCPR), 42 patients were included in the open transvesical prostatectomy group and 22 in the videolaparoscopic group. Then, a comparison was made between the techniques in terms of surgical time, bleeding, length of stay, need for intensive care, among others, in addition to postoperative outcome. Results: the mean surgical time was shorter in the open technique compared to the laparoscopic technique (141 min vs 274 min). The videolaparoscopic group had a shorter mean hospital stay (3.5 days vs 6.36 days). There was no statistical significance in the comparison regarding the need for an intensive care unit, as well as in the assessment of postoperative bleeding. Conclusion: comparatively, the techniques demonstrated a similar outcome, with a low rate of complications and satisfactory results for the treatment of BPH. The laparoscopic technique is a surgery with a shorter hospital stay, but at the expense of a longer surgical time.

RESUMO Objetivo: o objetivo deste estudo foi comparar o resultado das técnicas de prostatectomia transvesical aberta e videolaparoscópica no tratamento de hiperplasia prostática benigna (HPB) em um hospital terciário. Métodos: foram revisados prontuários de pacientes submetidos a adenectomia transvesical devido a HPB entre março de 2019 a março de 2021 no serviço de urologia do Hospital de Clínicas do Paraná (HCPR), sendo incluídos 42 pacientes no grupo prostatectomia transvesical aberta e 22 no grupo videolaparoscópico. Em seguida foi feita a comparação entre as técnicas nos quesitos tempo cirúrgico, sangramento, tempo de internamento, necessidade de terapia intensiva, entre outras, além de desfecho pós-operatório. Resultados: o tempo cirúrgico médio foi menor na técnica aberta em comparação com a técnica videolaparoscópica (141 min vs 274 min). O grupo videolaparoscópico apresentou um tempo médio de internamento menor (3,5 dias vs 6,36 dias). Não houve significância estatística na comparação quanto a necessidade de unidade de terapia intensiva, assim como na avaliação do sangramento pós-operatório. Conclusão: comparativamente, as técnicas demonstraram um desfecho semelhante, com baixa taxa de complicações e resultados satisfatórios para o tratamento da HPB. Sendo a técnica videolaparoscópica uma cirurgia com menor tempo de internamento, porém às custas de um maior tempo cirúrgico. .

Abstract This study describes the hydrography and water masses of the Santos Basin Continental Shelf (SBCS) during two hydrographic campaigns (SANAGU, SANSED) in 2019. Coastal Water (CW) is the dominant water mass in the southern portion of the SBCS, with relatively low salinity values (S<35.5 g kg–1), and satellite data show that local precipitation and river discharge could not account for the low salinity observed during the cruises in the southern region of the domain. The low salinity observed is explained by the transport from the south influenced by Subtropical Shelf Water (STSW), which was influenced by the La Plata River discharge. In the northern region of the SBCS, the South Atlantic Central Water (SACW) dominates the bottom layers of the water column, with the wind playing a major role in the uplift of this water mass, as evidenced by a wind impulse analysis. In this part of the shelf, Tropical Water (TW) was the second water mass in volume and occupied the surface layers and offshore the shelf. CW is restricted to a thin surface layer, which reaches distances of a few kilometers from the coast. Our analysis show the differences in the hydrographic structure of the SBCS and suggests that the SBCS can be divided in two regions with distinct characteristics: 1) the area southwest of São Sebastião, where the hydrographic parameters were modulated by the presence of the Subtropical Shelf Water (STSW); 2) the area northeast of São Sebastião, where the uplifting of SACW was the dominant process.

ABSTRACT: This study investigated the association between neurological signs as well as plausible risk factors and the seroprevalence of Toxoplasma gondii and Neospora caninum infection in dogs of the Campo Grande region of Mato Grosso do Sul, Brazil. In this study, the dogs were divided into two groups based on the presence and the absence of neurological signs (n=30 in each group). Serological diagnosis was performed using the indirect fluorescent antibody test. In the group with neurological disorders, 23.3% and 30% of the dogs had anti-T. gondii and anti-N. caninum antibodies, respectively. Moreover, three dogs from this group showed co-infection with both protozoa. In the group without neurological signs, 16.7% and 13.3% of the dogs were seropositive for T. gondii and N. caninum, respectively. Although presence of neurological signs was not associated with T. gondii and N. caninum infections (P = 0.747 and P = 0.21, respectively), there was a statistical association between T. gondii seropositivity and peripheral neurological alteration (P = 0.016) among dogs with neurological signs. Raw meat ingestion was the only risk factor associated with the presence of anti-N. caninum antibodies (P = 0.041). Results revealed evidence of exposure to N. caninum and T. gondii in dogs irrespective of the presence of neurological signs. Moreover, this study highlighted the need for serological investigation of T. gondii in dogs with disturbances in peripheral nervous systems and not offering raw meat to animals to avoid the risk of N. caninum infection.

RESUMO: Este estudo teve como objetivo investigar a associação entre sinais neurológicos, assim como possíveis fatores de risco e soroprevalência de Toxoplasma gondii e Neospora caninum em cães de Campo Grande, região de Mato Grosso do Sul, Brasil. Nesse estudo, os cães foram divididos em dois grupos, baseado na presença ou ausência de sinais neurológicos (n = 30 em cada grupo). O diagnóstico sorológico foi realizado por meio do teste imunofluorescência indireta (RIFI). No grupo com distúrbios neurológicos, 23.3% e 30% dos cães tinham anticorpos anti-T. gondii e anti-N. caninum, respectivamente. Além disso, três cães deste grupo estavam coinfectados com ambos os protozoários. No grupo sem sinais neurológicos, 16.7% e 13.3% dos cães foram soropositivos para T. gondii e N. caninum, respectivamente. Apesar da presença de sinais neurológicos não ter sido associado a infecção por T. gondii ou N. caninum ( P = 0.747 e P = 0.21, respectivamente), houve associação estatística entre soropositividade para T. gondii e alteração neurológica periférica (P = 0.016) entre os cães com sinais neurológicos. A ingestão de carne crua foi o único fator de risco que apresentou associação com a presença de anticorpos para N. caninum (P = 0.041). Estes resultados mostraram evidência de que a exposição a N. caninum e T. gondii em cães independe da presença de sinais neurológicos. Além disso, este estudo destaca a necessidade de investigação sorológica para T. gondii em cães com distúrbios neurológicos periféricos, e o oferecimento de carne crua aos animais deve ser evitada devido ao risco de infecção por neosporose.
