ABSTRACT Objective: To determine the association between type 2 diabetes mellitus and cataracts in Peruvian adults over 50 years of age. Methods: Cross-sectional analytical observational study corresponding to the secondary analysis of the 2019 Encuesta Demográfica y de Salud Familiar database. Type 2 diabetes mellitus was considered an independent variable and cataract as a dependent variable. A descriptive analysis was performed using absolute frequencies and weighted proportions, as well as a multivariate analysis using generalized linear models of the Poisson family with logarithmic link function to estimate prevalence ratios on the association between diabetes and cataracts. Results: Data from a total of 8,775 Peruvian adults over 50 years of age were analyzed, of whom 1,322 self-reported a diagnosis of cataract. Of the total respondents, 9.2% reported having been diagnosed with diabetes mellitus and 16.7% with cataracts. The crude model (p<0.001) and the adjusted models 1 (p=0.007) and 2 (p=0.029) found an association between having diabetes mellitus and having cataracts. Conclusion: Two out of ten adults aged 50 years old and older in Peru had cataracts. There was an association between having diabetes and the presence of cataracts in the Peruvian population.

RESUMO Objetivo: Determinar a associação entre o diabetes mellitus tipo 2 e catarata em adultos peruanos com mais de 50 anos de idade. Métodos: Estudo analítico observacional transversal correspondente à análise secundária da base de dados do Encuesta Demográfica y de Salud Familiar de 2019. O diabetes mellitus tipo 2 foi considerado variável independente e a catarata, variável dependente. Foi realizada uma análise descritiva utilizando frequências absolutas e proporções ponderadas, bem como uma análise multivariada, usando modelos lineares generalizados da família Poisson com função de ligação logarítmica para estimar razões de prevalência sobre a associação entre o diabetes e a catarata. Resultados: Foram analisados dados de 8.775 adultos peruanos com mais de 50 anos de idade, dos quais 1.322 autorrelataram diagnóstico de catarata. Do total de inquiridos, 9,2% relataram ter sido diagnosticados com diabetes mellitus e 16,7%, com cataratas. O modelo bruto (p<0,001), o modelo ajustado 1 (p=0,007) e o modelo ajustado 2 (p=0,029) encontraram associação entre ter diabetes mellitus e ter catarata. Conclusão: Dois em cada dez adultos com 50 anos de idade ou mais no Peru tinham catarata. Houve associação entre ter diabetes e a presença de catarata na população peruana.

ABSTRACT Objective: to map diverse evidence on perspectives and challenges for implementing the Expert Patient Program. Method: this is a scoping review, guided by the method proposed by the JBI and which followed the recommendations set forth in the Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews, with its research protocol registered in the Open Science Framework, with DOI:10.17605/OSF.IO/D7K6A. The search for studies was carried out in July 2022 in databases and in national and international portals of theses and dissertations. Results: the final sample consisted of six scientific articles, all written in English. Health professionals' qualification, to identify and qualify expert patients, management's commitment to the program, use of mobile technologies and Evidence-Based Practice are among the perspectives for implementation. The challenges are related to lack of understanding of the term "self-management" and to the deficit of public policies and financial investment. Conclusion: the results found emphasize the importance of implementing the Patient Expert Program for the public health context, mainly in relation to the population with chronic non-communicable diseases.

RESUMEN Objetivo: mapear diversa evidencia sobre las perspectivas y los desafíos para la implementación del Programa “Paciente Experto”. Método: revisión de alcance, orientada a partir del método propuesto por el JBI y en la que se siguieron las recomendaciones de Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews, con registro del protocolo de investigación en Open Science Framework (DOI:10.17605/OSF.IO/D7K6A). La búsqueda de estudio se realizó en julio de 2022, tanto en las bases de datos como en portales de tesis y disertaciones nacionales e internacionales. Resultados: la muestra final estuvo compuesta por seis artículos científicos, todos redactados en inglés. Entre las perspectivas para la implementación se incluyen las siguientes: calificación de los profesionales de la salud, para identificar y calificar pacientes expertos; compromiso de la gerencia con el programa; uso de las tecnologías móviles y Práctica Basada en Evidencia. Por el otro lado, los desafíos están relacionados con no comprender el término "autogestión" y con el déficit de políticas públicas e inversiones financieras. Conclusión: los resultados encontrados enfatizan la importancia de implementar el Programa “Paciente Experto” para el contexto de la salud pública, principalmente en relación con la población que padece enfermedades crónicas no transmisibles.

A busca pelos estudos foi realizada no mês de julho de 2022, e ocorreu nas bases de dados e em portais de teses e dissertações nacionais e internacionais. Resultados: A amostra final foi composta por seis artigos científicos, todos redigidos na língua inglesa. Entre as perspectivas para implementação, estão a qualificação dos profissionais de saúde, para identificar e qualificar pacientes expertos, o comprometimento da gestão com o programa, o uso das tecnologias móveis e a Prática Baseada em Evidência. Já os desafios são relacionados à falta de compreensão do termo "autogestão” e ao déficit de políticas públicas e investimento financeiro. Conclusão: Os resultados encontrados enfatizam a importância da implementação do Programa Paciente Experto para o contexto de saúde pública, principalmente, em relação à população portadora de doenças crônicas não transmissíveis.

Abstract Introduction After the advent of low-temperature thermoplastics, their prevalence in the manufacture of orthoses for upper limbs has been identified by several authors. The understanding of their properties by occupational therapists and other professionals working in this field is important in the process of selecting the thermoplastic, which follows a logic to match the material’s characteristics to the desired function for each orthosis. Objectives Systematize the characteristics of low-temperature thermoplastics and, from that, develop an instrument for carrying out practical empirical tests with the materials, to establish criteria for their handling and evaluation. Method Exploratory study consisting of the creation of testing procedures and, consequently, the development of a qualitative assessment instrument that values the practical experience of the professional who handles the material and evaluates each requirement. Results The created instrument includes 14 material characteristics, accompanied by a definition, a procedure with recommendations for the practical testing, and a field for filling in response alternatives. Conclusions The instrument considers important characteristics to be verified during the evaluation of materials and can direct the professional’s observations and records, aiding in clinical decision-making. This will be important to improve the quality of orthoses and other assistive technology devices made with these thermoplastics. In addition, the systematization of the practical evaluation of thermoplastic materials can assist in the development of health studies and research involving materials for orthoses.

Resumo Introdução Após o advento dos termoplásticos de baixa temperatura, sua prevalência na confecção de órteses para membros superiores é identificada por vários autores. A compreensão de suas propriedades, por parte do terapeuta ocupacional e de outros profissionais que atuam na área, é importante no processo de seleção do termoplástico, que segue uma lógica para combinar as características do material à função desejável para cada órtese. Objetivos Sistematizar as características dos termoplásticos de baixa temperatura e, a partir disso, elaborar um instrumento para realizar testes práticos empíricos com os materiais a fim de estabelecer critérios para o seu manuseio e avaliação. Método Pesquisa de caráter exploratório, constituindo a criação de procedimentos de teste e, consequentemente, a elaboração de um instrumento de avaliação de abordagem qualitativa e que valoriza a experiência prática do profissional, que manipula o material e avalia cada requisito. Resultados O instrumento criado contempla 14 caraterísticas do material, acompanhadas de uma definição, um procedimento com recomendações para o teste prático e um campo destinado ao preenchimento das alternativas de resposta. Conclusão O instrumento considera características importantes a serem verificadas durante a avaliação dos materiais e poderá direcionar o olhar e os registros do profissional, auxiliando o nas decisões clínicas. Isso será importante para melhorar a qualidade das órteses e de outros dispositivos de Tecnologia Assistiva confeccionados com esse grupo de materiais. Além disso, a sistematização da avaliação prática dos materiais termoplásticos poderá auxiliar no desenvolvimento de estudos da área da saúde e de pesquisas envolvendo materiais para órteses.

Abstract Alcohol has been widely consumed for centuries and is linked to the aggravation of diseases. Several studies have shown that excessive consumption of ethanol results in morphophysiological changes in the male reproductive system. One of the effects of ethanol is the decrease in testosterone concentration and hormonal therapies are an alternative to minimize the changes resulting from chronic alcoholism. Qualitative studies were commonly carried out to evaluate the male histopathological alterations resulting from ethanol consumption, being necessary quantitative and non-subjective techniques. This study analyzes the importance of fractal analysis as a useful tool to identify and quantify tissue remodeling in rats submitted to ethanol consumption and hormone therapy with testosterone. Prostate of animals submitted to chronic ethanol consumption showed tissue disorganization, which was confirmed by an increasing of fractal dimension. Regarding the prostatic stroma, collagen fractal dimension and quantification revealed lower values in animals that were only submitted to androgen therapy. ABSTRACT Introduction: Telomere length (TL) is a biomarker of cellular proliferative history. In healthy individuals, leukocyte TL shortens with age and associates with the lifespan of men and women. However, most of studies had used linear regression models to address the association of the TL attrition, aging and sex. Methods: We evaluated the association between the TL, aging and sex in a cohort of 180 healthy subjects by quantile regression. The TL of nucleated blood cells was measured by fluorescent in situ hypridization (flow-FISH) in a cohort of 89 men, 81 women, and 10 umbilical cord samples. The results were validated by quantitative polymerase chain reaction (qPCR) and compared to a linear regression analysis. Results: By quantile regression, telomere dynamics slightly differed between sexes with aging: women had longer telomeres at birth and slower attrition rate than men until the sixth decade of life; after that, TL eroded faster and became shorter than that in men. These differences were not observed by linear regression analysis, as the overall telomere attrition rates in women and men were similar (42 pb per year, p < 0.0001 vs. 45 pb kb per year, p < 0.0001). Also, qPCR did not recapitulate flow-FISH findings, as the telomere dynamics by qPCR followed a linear model. Conclusion: The quantile regression analysis accurately reproduced a third-orderpolynomial TL attrition rate in both women and men, but it depended on the technique applied to measure TL. The Flow-FISH reproduced the expected telomere dynamics through life and, differently from the qPCR, was able to detect the subtle TL variations associated with sex and aging.

Resumen: Este artículo tiene como objetivo analizar tanto los conceptos y relaciones del Modelo de la Triple Hélice (MTH) que propone Etzkowitz y Leydesdorff aplicado a la seguridad y la defensa internacionales como la contribución del MTH al desarrollo de economías basadas en el conocimiento. El método utilizado fue un análisis cualitativo y descriptivo mediante la revisión de fuentes científicas confiables y páginas web oficiales de países y organizaciones como la Organización de las Naciones Unidas para la Educación, la Ciencia y la Cultura (UNESCO), la Organización del Tratado del Atlántico Norte (OTAN) y la Unión Europea (UE). Los resultados mostraron que la generación de capacidades en Ciencia, Tecnología e Innovación produce activos en seguridad y defensa y conduce al desarrollo sistémico de productos e iniciativas innovadoras. Este círculo virtuoso les permite a universidades, estados y empresas evolucionar fundamentando su crecimiento en el desarrollo del conocimiento. En conclusión, se encontró que la inversión en investigación y desarrollo para la defensa y la seguridad, en articulación con los actores de la Triple Hélice, es un motor de cambio tecnológico que fortalece el concepto de economías basadas en el conocimiento, como se evidencia en los casos analizados.

Resumo: Este artigo tem como objetivo analisar os conceitos e relações do Modelo de Hélice Tríplice (THM) proposto por Etzkowitz & Leydesdorff aplicado à segurança e defesa internacional e a contribuição do THM para o desenvolvimento das economias baseadas no conhecimento. O método utilizado foi uma análise qualitativa, descritiva, revisando fontes científicas confiáveis e páginas da web oficiais de países e organizações como a Organização das Nações Unidas para a Educação, a Ciência e a Cultura (UNESCO), a Organização do Tratado do Atlântico Norte (OTAN) e a União Europeia (EU). Os resultados mostraram que a geração de capacidades em Ciência, Tecnologia e Inovação levam a ativos em segurança e defesa e ao desenvolvimento sistêmico de produtos e iniciativas inovadoras, criando um círculo virtuoso que permite que universidades, estados e empresas, evoluir baseando seu crescimento no desenvolvimento do conhecimento. Por fim, como conclusão constatou-se que o investimento em pesquisa e desenvolvimento em defesa e segurança, articulado com os atores da Tríplice Hélice, é um propulsor de mudanças tecnológicas que fortalecem o conceito de economias baseadas no conhecimento nos países que realizam essa articulação, conforme evidenciado nos casos analisados.

Abstract: This article aims to analyze the concepts and relationships of the Triple Helix Model (THM) proposed by Etzkowitz & Leydesdorff as applied to international security and defense and the contribution of THM to the development of knowledge-based economies. The method used was a qualitative, descriptive analysis, reviewing reliable scientific sources and official web pages of countries and organizations such as the United Nations Educational, Scientific and Cultural Organization (UNESCO), the North Atlantic Treaty Organization (NATO) and the European Union (EU). The results showed that the generation of capacities in Science, Technology, and Innovation lead to assets in security and defense and the systemic development of products and innovative initiatives, creating a virtuous circle that allows universities, states and firms, to evolve basing their growth on the development of knowledge. Finally, as a conclusion it was found that investment in research and development in defense and security, articulated with the actors of the Triple Helix, is a driver of technological change that strengthen the concept of knowledge-based economies in the countries that achieve this articulation, as evidenced in the cases analyzed.

Abstract Objective: To provide guidelines on the coronavirus disease 2019 (COVID-19) vaccination in patients with immune-mediated rheumatic diseases (IMRD) to rheumatologists considering specific scenarios of the daily practice based on the shared-making decision (SMD) process. Methods: A task force was constituted by 24 rheumatologists (panel members), with clinical and research expertise in immunizations and infectious diseases in immunocompromised patients, endorsed by the Brazilian Society of Rheumatology (BSR), to develop guidelines for COVID-19 vaccination in patients with IMRD. A consensus was built through the Delphi method and involved four rounds of anonymous voting, where five options were used to determine the level of agreement (LOA), based on the Likert Scale: (1) strongly disagree; (2) disagree, (3) neither agree nor disagree (neutral); (4) agree; and (5) strongly agree. Nineteen questions were addressed and discussed via teleconference to formulate the answers. In order to identify the relevant data on COVID-19 vaccines, a search with standardized descriptors and synonyms was performed on September 10th, 2021, of the MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, ClinicalTrials.gov, and LILACS to identify studies of interest. We used the Newcastle–Ottawa Scale to assess the quality of nonrandomized studies. Results: All the nineteen questions-answers (Q&A) were approved by the BSR Task Force with more than 80% of panelists voting options 4—agree—and 5—strongly agree—, and a consensus was reached. These Guidelines were focused in SMD on the most appropriate timing for IMRD patients to get vaccinated to reach the adequate covid-19 vaccination response. Conclusion: These guidelines were developed by a BSR Task Force with a high LOA among panelists, based on the literature review of published studies and expert opinion for COVID-19 vaccination in IMRD patients. Noteworthy, in the pandemic period, up to the time of the review and the consensus process for this document, high-quality evidence was scarce. Thus, it is not a substitute for clinical judgment.

Abstract Agro-industrial processing activities generate by-products that are discarded at high economic and environmental costs; however, many of them still contain high levels of bioactive compounds (BCs) such as dietary fiber (DF) and phenolic compounds (PCs), which, when combined, are known as antioxidant dietary fiber (ADF). These BCs have properties that are beneficial to our health, among them anti-inflammatory, antioxidant and cardioprotective. Moreover, DF can regulate glucose and cholesterol levels. The mechanism of ADF action depends on its chemical structure and the interactions between the DF and PCs in it. Recent studies have reported the incorporation of several of these by-products on a diet through food products consumed worldwide, such as bread. This review describes the chemical characteristics of the ADF complex and the effect of its molecular interactions on sensory, nutritional and health-promoting properties of functional bakery goods.

ABSTRACT Background: Coronavirus disease (COVID-19) serology testing evaluates the prevalence of COVID-19 cases. Methods: A seroepidemiological survey of COVID-19 among healthcare workers was performed (June 2020 to November 2020) in Ribeirão Preto, São Paulo, Brazil. Overall, 10,172 and 2,129 workers participated in the first and second phases, respectively. Results: First phase: 12.7% tested positive for COVID-19 (73.5% females and 35.2% aged 30-39 years), and 29.6% were nursing technicians. Second phase: 12.1% tested positive for COVID-19 (65.5% females and 33.3% aged 40-49 years), and 24.8% were nursing assistants. Conclusions: In 2020, healthcare workers in Ribeirão Preto had COVID-19 in a similar way.

Abstract This study investigates the biomechanics of the spine after insertion of vertebral body tethering (VBT) with different cord pretensions. For that purpose, a Finite Element model of the average thoracolumbar spine was stepwise calibrated and validated. The VBT instrumentation was inserted in the left side of the L1-L2 segment with different cord pretensions. As a second test, the L1-L2 segment was submitted to an external pure moment of 6 Nm in left and right lateral bending. The range of motion (ROM) for the spine with VBT was determined with respect to its initial post VBT position. Pretension forces of 100 N and 300 N resulted in a change of scoliotic angle of 2.7° and 5.3° to the left side of the spine, respectively. The ROM of the native spine was 4.5° in right lateral bending and reduced to 1.8° and 1.4° for the cases of the spine with a cord pretension of 100 N and 300 N, respectively. In left lateral bending, the absolute ROM of the native spine was 4.6°. For the cases of a cord pretension of 100 N and 300 N, the spine bent 1.9° and 0.8° to the left side from its initial post VBT position, respectively.

Resumo A massificação do esporte tornou visível a interação de várias dimensões do desempenho que favorecem ou dificultam o desenvolvimento de um atleta de alto rendimento. Algumas delas são o contexto sociocultural em que os atletas vivem, a influência das políticas públicas no desenvolvimento a longo prazo da carreira esportiva, o ambiente familiar, amigos, treinadores, além de aspectos nutricionais e psicológicos. Este estudo qualitativo busca compreender a partir da voz dos atletas quais são os aspectos considerados como obstáculos ao alto rendimento no Chile. Os resultados mostram que os obstáculos são a falta de apoio, dupla carreira, implementação de formação e discriminação de gênero. Conclui propondo novos horizontes para as políticas públicas no âmbito do apoio aos atletas que se encarregam de resolver esses obstáculos.

Abstract The massification of sports has made visible the interaction of various dimensions of performance that favor or hinder the development of high-performance athletes. Some of these are the social and cultural context in which athletes live, the influence of public policies on the long-term development of sports careers, the family environment, friends, coaches, as well as nutritional and psychological aspects. This qualitative study seeks to understand from the voice of athletes what are the aspects considered as obstacles to high performance in Chile. The results show that the obstacles are the lack of support, dual career, implementation to train and gender discrimination. It concludes by proposing new horizons for public policies in the framework of supporting athletes who take charge of solving these obstacles.

Resumen La masificación de los deportes ha hecho visible la interacción de varias dimensiones del rendimiento que favorecen o dificultan el desarrollo de un deportista de alto rendimiento. Algunas de estas dimensiones son el contexto social/cultural en el que viven los deportistas, la influencia de las políticas públicas en el desarrollo a largo plazo de las carreras deportivas, el entorno familiar, de amigos y entrenadores, así como también aspectos nutricionales y psicológicos. Este estudio de tipo cualitativo busca comprender desde la voz de los y las deportistas cuáles son los aspectos considerados como obstaculizadores para el alto rendimiento en Chile. Los resultados demuestran que son obstaculizadores la falta de apoyo, la carrera dual, implementación para entrenar y discriminación de género. Se concluye proponiendo nuevos horizontes para las políticas públicas en el marco del apoyo a deportistas que se hagan cargo de resolver estos obstaculizadores.

Abstract Grape pomace (GP) and pecan shell (PS) are agro-industrial by-products rich in healthy bioactives, mainly phenolic compounds (PC) and dietary fiber (DF). Raw GP and PS were chemically characterized, and the adsorption isotherms of their PC-insoluble DF (IDF) complexes were evaluated. GP and PS statistically differed (p < 0.05) in chemical and physicochemical properties. Total PC, total flavonoids, and IDF were higher in GP. Thirteen and eight PC were identified in GP and PS by HPLC-MS/MS chromatography, finding isoquercetin and type B dimer and trimer procyanidins as the most abundant compounds. PC-IDF (phenolic compounds-insoluble dietary fiber) adsorption isotherms were determined by both spectroscopic (Freundlich) and HPLC-MS/MS (Freundlich/Langmuir) techniques, observing that PS IDF presented higher PC adsorption at all tested concentrations. Epicatechin, isoquercetin, and quercetin were the main identified PC in both by-products, and were able to fit to both Langmuir and Freundlich isotherm by HPLC-MS/MS.

SUMMARY: This study aimed to determine and compare the body composition, bioelectric parameters, and handgrip strength in federated and recreational Chilean climbers. Thirteen Chilean climbers voluntarily participated, being 4 federated (25.75 ± 2.87 years) and 9 recreational (22.33 ± 1.41 years). Body composition was measured using a multifrequency octopolar bioelectrical impedance meter, while handgrip strength was determined with a dynamometer. Although there were no statistical differences in the body composition variables between groups, the trunk phase angle was statistically higher in the federated compared to the recreational climbers (p = 0,011 [95 % CI = 1,10; 5,20]). Regarding handgrip strength, the relative strength was higher for federated (p = 0,025 [95 % CI = 0,10; 0,22]), while the difference in strength between dominant and non-dominant hand was higher for recreational climbers (p = 0,012 [95 % CI = 1,60; 10,05]). This study is one of the first that explore the differences between federated and recreational Chilean climbers. These results suggest a differentiation at the level of phase angle and handgrip strength, which should be corroborated in future studies.

RESUMEN: El objetivo del siguiente estudio fue determinar y comparar los valores de composición corporal, parámetros bioeléctricos y fuerza de prensión manual de escaladores chilenos federados y recreativos. Participaron voluntariamente 13 escaladores chilenos, de los cuales 4 eran federados (25,75 ± 2,87 años) y 9 recreativos (22,33 ± 1,41 años). La composición corporal se evaluó por medio de un impedanciómetro bioeléctrico octopolar multifrecuencia, mientras que la fuerza de prensión manual se determinó con un dinamómetro manual. Si bien no hubo diferencias en las variables de composición corporal entre ambos grupos, el ángulo de fase del tronco fue superior en los federados en comparación a los recreativos (p = 0,011 [95 % IC = 1,10; 5,20]). Respecto a la fuerza de prensión manual, la fuerza relativa fue superior para los federados (p = 0,025 [95 % IC = 0,10; 0,22]), mientras que la diferencia de la fuerza entre la mano dominante y no dominante fue mayor para los recreativos (p = 0,012 [95 % IC = 1,60; 10,05]). Este es uno de los primeros estudios que explora las diferencias entre escaladores chilenos federados y recreativos. Los resultados sugieren una diferenciación a nivel de ángulo de fase y fuerza de prensión manual, lo cual debería corroborarse con futuros estudios.
