Abstract: This research study assessed the use of Pueraria phaseoloides crops as cover in oil palm (Elaeis guineensis) plantations to improve the ecological function of soils under tropical conditions. The plots examined (90 palms·ha-1) were established 5, 7, 11, and 30 years ago. The results showed a high and stable specific coverage index that ranged between 52 and 87%. This enhanced soil moisture by reducing the incidence of solar radiation. It also increased competitive capacity with the complex of weeds present and improved soil nutrient averages (1.60 mg/L N and 0.42 mg/L P). Fruit bunch average yields also improved in the short term, ranging from 10.6 to 22.4 t·ha-1, with more frequent harvest events when compared to the control. It is concluded that Pueraria phaseoloides is a sustainable alternative to improve the establishment of plantations under tropical conditions.

Resumen: Esta investigación evaluó el uso de los cultivos de Pueraria phaseoloides como cobertura en plantaciones de palma aceitera (Elaeis guineensis) para mejorar el funcionamiento ecológico de los suelos en condiciones tropicales. Se examinaron parcelas de densidades de siembra de 90 palmas·ha-1 establecidos hace 5, 7, 11 y 30 años. Los resultados mostraron un índice de cobertura específico elevado y estable que osciló entre 52 y 87 %. Se observó mejor humedad del suelo con una menor incidencia de radiación solar, mayor capacidad competitiva con el complejo de malezas presentes, y mejores promedios de nutrientes (1.60 mg/L N y 0.42 mg/L P). Además, se incrementaron los rendimientos promedios de los racimos de fruta a corto plazo entre 10.6 y 22.4 t·ha-1 con eventos de cosechas más frecuentes en comparación con la estándar. Se concluye que Pueraria phaseoloides es una alternativa sostenible para mejorar el establecimiento de las plantaciones en condiciones tropicales.

ABSTRACT: Introduction: The consumption of psychoactive substances has become a public health problem, having great consequences on the health, economy, academic performance, family, and social coexistence of the consumer. Objective: To determine the risk factors influencing the consumption of psychoactive substances in university students during COVID-19. Materials and Methods: Research with quantitative correlational, cross-sectional descriptive type, with a sample of 272 university students of a University of Barranquilla of the Nursing program of fourth, fifth, sixth, seventh, and eighth semesters. The instrument used was a virtual survey divided into five sections, comprising identification and sociodemographic data, individual, sociocultural, family, and psychological factors. Results: In the statistical analysis, the predominant sex was female with 69%, and 74% of them aged 17-20 years. Seventy-nine percent stated that they had consumed psychoactive substances, and 58% indicated that the type of psychoactive substance they had consumed for the first time was alcohol. Discussion: In contrast to a study conducted by Fernández et al, in 2021, it was found that the frequency of consumption of psychoactive substances in the students of the Simón Bolívar University was higher with a percentage of 74%, on the other hand the study in question only 41.8% consumed some type of psychoactive substance (alcohol, tobacco, etc.).Through the research carried out, it was possible to evaluate each of the factors that significantly influence the consumption of psychoactive substances, taking into account each of the variables evaluated (sociodemographic, consumption, individual, family, psychological, sociocultural) in times of COVID-19.

RESUMEN: Introducción: El consumo de sustancias psicoactivas se ha convertido en un problema de salud pública, teniendo grandes consecuencias en la salud, economía, rendimiento académico, la convivencia familiar y social del individuo consumidor. Objetivo: Determinar los factores de riesgo que influyen en el consumo de sustancias psicoactivas en estudiantes universitarios en tiempos de COVID-19. Materiales y Métodos: Investigación con enfoque cuantitativo, de tipo descriptivo correlacional, con una muestra de 272 estudiantes universitarios de una Universidad de Barranquilla del programa de Enfermería de cuarto, quinto, sexto, séptimo y octavo semestre. El instrumento utilizado fue una encuesta virtual estuvo dividida en 5 secciones, que comprenden la parte de datos de identificación y sociodemograficos, factores individuales, socioculturales, familiares y psicologicos. Resultados: Realizando el análisis estadístico, predominó el sexo femenino con 69%, y las edades 17-20 años con 74%. El 79% afirmaban que sí habían consumido sustancias psicoactivas y 58% indicaban que el tipo de sustancia psicoactiva que habían consumido por primera vez era el alcohol. Discusión: En contraste con un estudio realizado por Fernández et al, en el 2021, se encontró que la frecuencia de consumo de sustancias psicoactivas en los estudiantes de la Universidad Simon Bolivar es mayor, con un porcentaje del 74%, por otro lado, el estudio en mención solo el 41.8% consume algun tipo de sustancia psicoactiva (alchohol, tabaco, etc). A través de la investigación realizada permitió evaluar cada uno de los factores que influyen de manera significativa en el consumo de sustancias psicoactivas teniendo en cuenta cada una de las variables evaluadas (sociodemográficas, de consumo, individuales, familiares, psicológicas, socioculturales) en tiempos de Covid 19.

UNCONVENTIONAL POLYAROMATICS COMPOUNDS AND MERCURY IN THE TODOS OS SANTOS BAY, BRAZIL: RECENT ADVANCES AND CONTRIBUTIONS TO BETTER UNDERSTAND OF THEIR ROLE IN THE MARINE ENVIRONMENT. Baía de Todos os Santos (BTS) is the second-largest Brazilian bay; it was the first oil producing region in Brazil, with three large industrial parks in its north-northeast portion. In addition, the BTS has two large ports and a population of around 3 million inhabitants distributed among 14 cities. In the present study, recent studies on contamination by polycyclic aromatic compounds (PACs) and Hg in sediments, water, and biota from the BTS region are presented and critically discussed. Among the PACs, studies evaluating the presence of polycyclic aromatic hydrocarbons (PAHs) and their nitrated (nitro-PAHs), oxygenated derivatives (oxy-PAHs), and those containing atoms of S as a heteroatom (PASHs) are considered, as well as the element Hg distributed in seawater, sediments, and biota. Regarding biomonitoring organisms, recent results on polychaetes, ascidians, and corals are presented. Trends for future studies are presented at the end of the text.

Abstract Objective: Determining the appropriate approach for delivery after previous cesarean is a very controversial issue. Our objective was to establish whether pregnant women with a previous cesarean have an increased maternal and fetal morbidity and mortality after attempting vaginal delivery as well as to determine which factors may influence the achievement of a vaginal birth after cesarean. Materials and methods: A retrospective observational cohort study including 390 patients (196 cesarean group and 194 nulliparous group) was carried out. We compared neonatal and maternal outcomes between groups. Afterward, a multivariate logistic regression was applied for our second objective. Results: There were higher rates of uterine rupture (2% vs. 0%, p: 0.045) and puerperal hemorrhage (9.7% vs. 3.1%, p: 0.008) in the cesarean group and lower vaginal delivery rate (58.2% vs. 77.8%, p < 0.0005). We found that the induced onset of labor (OR = 2.9) and new born weight (OR = 1.0001) were associated with an increased risk of cesarean section. Conclusions: Our findings stress the need for further investigations in this field, which might provide a basis for a better management of patients with a previous cesarean.

RESUMEN INTRODUCCIÓN: La fibrilación auricular aumenta 5 veces el riesgo de un ataque cerebrovascular de origen cardioembólico. La anticoagulación está indicada para prevención primaria y secundaria de embolismo cerebral. En pacientes con alto riesgo de sangrado no modificable se ha propuesto el cierre de apéndice auricular izquierdo como alternativa a la anticoagulación. MÉTODOS: Serie de casos retrospectiva de pacientes incluidos en el registro ReACTIVE entre los años 2019 y 2020 con diagnóstico de fibrilación auricular, puntaje de CHA2DS2-VASc ≥ 4, HAS BLED ≥ 3 y contraindicación de terapia anticoagulante a largo plazo. Se incluyeron variables demográficas, factores de riesgo, desenlace clínico usando la escala de Rankin modificado y se hizo seguimiento clínico a 3 meses después del procedimiento. RESULTADOS: Se incluyeron 6 pacientes, el 50 % de ellos mujeres, con mediana de edad de 80,8 ± 4,8 años. El 83,3 % presentó antecedente de sangrado mayor bajo consumo de anticoagulantes. En el seguimiento a 90 días, ningún paciente experimentó sangrado ni ataque cerebrovascular, un paciente murió por causas no cardiovasculares, en tanto que el 83,3 % de los pacientes tuvieron un Rankin modificado favorable (0-2). CONCLUSIONES: El cierre de apéndice auricular es una opción terapéutica en pacientes con fibrilación auricular y contraindicación para anticoagulación por alto riesgo de sangrado. Nuestra serie obtuvo resultados similares a los publicados a escala nacional a pesar de incluir pacientes con edad más avanzada. La conformación de equipos interdisciplinarios de corazón y cerebro es útil en la selección de pacientes para esta terapia.

ABSTRACT INTRODUCTION: Atrial fibrillation increases the risk of a cerebrovascular attack of cardioembolic source by 5 times. Anticoagulation is indicated for primary and secondary prevention of cerebral embolism. In patients at high risk of non-modifiable bleeding, closure of the left atrial appendage has been proposed as an alternative to anticoagulation. METHODS: Retrospective case series of patients included in the ReACTIVE registry between 2019 and 2020 with a diagnosis of atrial fibrillation, CHA2DS2-VASc score ≥ 4, HAS-BLED ≥ 3, and long-term anticoagulant therapy contraindication. Demographic variables, risk factors, and clinical outcomes were included using the modified Rankin scale, and clinical follow-up was done three months after the procedure. RESULTS: 6 patients were included, 50 % women, median age 80.8 ± 4.8 years. 83.3 % history of major bleeding under anticoagulant treatment. At the 90-day follow-up, no patient had a bleeding or cerebrovascular attack, one patient died from non-cardiovascular causes, and 83.3 % of the patients had a favorable modified Rankin (0-2). CONCLUSIONS: Atrial appendix closure is a therapeutic option in patients with atrial fibrillation and a contraindication for anticoagulation due to a high risk of bleeding. Our series obtained results like those published at the national level despite including older patients. The formation of interdisciplinary heart and brain teams is helpful in the selection of patients for this therapy.

Abstract Objectives: The aim of this study was to evaluate the role of the C-reactive protein (CRP) and white cell count (WC) in the prediction of anastomotic leakage (AL) in major abdominal surgery. Methods: Multicenter, prospective, and observational study of adult patients who underwent major abdominal surgery. CRP and hemogram were measured after post-operative day (POD) 3 and POD 5. Complications were classified according to the Clavien-Dindo classification. Diagnostic accuracy was evaluated by the area under the receiver operating characteristic curve (AUC). Results: A total of 97 patients were included in the study. The mean age was 63 ± 12 years and 47 (48%) were male. Colorectal (56%) and gastric cancer (36%) were the most frequent diagnoses. About 23% had post-operative complications, of which 5% had AL. The most significant predictive factor was the increase in CRP ≥ 2.84 mg/L among POD 3 and 5 (AUC, 0.99, sensitivity, 95.6%, specificity, 100%, positive likelihood ratio, 23.0). The accuracy of the other biomarkers was lower, CRP on POD 3 (AUC, 0.55), on POD 5 (AUC, 0.93), WC on POD 3 (AUC, 0.33), and POD 5 (AUC, 0.35). Conclusion: The increase of CRP among POD 3 and 5 was an early predictor of AL in adult patients with major abdominal surgery.

Resumen Objetivo: El objetivo de este estudio fue evaluar el papel de la proteína C-reactiva (PCR) y el recuento de glóbulos blancos (RGB) en la predicción de la Fuga Anastomotica (FA) en la cirugía abdominal mayor. Método: Estudio multicéntrico, prospectivo y observacional de pacientes adultos sometidos a cirugía abdominal mayor. Se midieron la PCR y el hemograma después del día postoperatorio (DPO) 3 y DPO 5. Las complicaciones se categorizaron según la clasificación de Clavien-Dindo y la precisión diagnóstica se evaluó mediante el área bajo la curva (AUC). Resultados: Se incluyeron un total de 97 pacientes. La edad media era de 63 ± 12 años y 47 (48%) eran hombres. El factor predictivo más significativo fue el aumento de la PCR ≥ 2,84 mg/L entre los DPO 3 y 5 (AUC, 0,99, sensibilidad, 95,6%, especificidad, 100%, ratio de probabilidad positiva, 23,0). La precisión de los demás biomarcadores fue menor, la PCR en el DPO 3 (AUC, 0,55), en el DPO5 (AUC, 0,93), el RGB en el DPO 3 (AUC, 0,33) y en el DPO 5 (AUC, 0,35). Conclusiones: El aumento de la PCR entre los DPO 3 y 5 fue un predictor temprano de FA en pacientes adultos con cirugía abdominal mayor.

ABSTRACT The objective of the study was to identify the resistance of Mycobacterium tuberculosis to drugs in Paraguay, 2014 to 2017. A retrospective observational study was carried out. The data from the National Tuberculosis Program of Paraguay between the years 2014 to 2017 were used. All patients with a diagnosis of Tuberculosis who underwent a resistance test were included. Data were extracted in Excel and analyzed with Stata 17.0. 3429 tuberculosis patients who had a result of at least one sensitivity test were included. Resistance was found in 2.1% of patients. Resistance to Rifampicin was present in 0.3% of cases while to Izionazide in 0.6% of cases. The prevalence of resistance was higher in men 3.4 (95% CI 2.2 - 4.8) p = 0.003, who resided in the Chaco 6.0 (95% CI 3.4 - 9.7) p = 0.000, previously treated 2.7 (95% CI 1.1 - 5.1) p = 0.010. In the model, it was observed that a previously treated patient has a greater chance of having resistance OR 2.62 (95% CI 1.1 - 6.24). The prevalence of resistance of Mycobacterium tuberculosis to drugs was related to having been previously treated.

RESUMEN El objetivo del estudio fue identificar la resistencia del Mycobacterium tuberculosis a los fármacos en Paraguay, 2014 a 2017. Se realizó un estudio observacional retrospectivo. Se utilizaron los datos del Programa Nacional de Tuberculosis del Paraguay comprendidos entre los años 2014 a 2017. Se incluyeron todos los pacientes con diagnóstico de Tuberculosis que se realizaron un test de resistencia. Se extrajeron los datos en Excel y fueron analizados con Stata 17.0. Se incluyeron 3429 pacientes con tuberculosis que contaban con resultado de al menos una prueba de sensibilidad. La resistencia se encontró en 2.1% de los pacientes. La resistencia a la Rifampicina estuvo presente en el 0.3% de los casos mientras que a la Izionazida en el 0.6% de los casos. La prevalencia de resistencia fue más alta en hombres 3.4 (IC 95% 2.2 - 4.8) p=0.003, que residían en el chaco 6.0 (IC 95% 3.4 - 9.7) p=0.000, previamente tratados 2.7 (IC 95% 1.1 - 5.1) p=0.010. En el modelo se pudo observar que un paciente previamente tratado tiene mayores posibilidades de tener resistencia OR 2.62 (IC 95% 1.1 - 6.24). La prevalencia de resistencia del Mycobacterium tuberculosis a fármacos estuvo relacionada con haber sido previamente tratado.

ABSTRACT Introduction: Telomere length (TL) is a biomarker of cellular proliferative history. In healthy individuals, leukocyte TL shortens with age and associates with the lifespan of men and women. However, most of studies had used linear regression models to address the association of the TL attrition, aging and sex. Methods: We evaluated the association between the TL, aging and sex in a cohort of 180 healthy subjects by quantile regression. The TL of nucleated blood cells was measured by fluorescent in situ hypridization (flow-FISH) in a cohort of 89 men, 81 women, and 10 umbilical cord samples. The results were validated by quantitative polymerase chain reaction (qPCR) and compared to a linear regression analysis. Results: By quantile regression, telomere dynamics slightly differed between sexes with aging: women had longer telomeres at birth and slower attrition rate than men until the sixth decade of life; after that, TL eroded faster and became shorter than that in men. These differences were not observed by linear regression analysis, as the overall telomere attrition rates in women and men were similar (42 pb per year, p < 0.0001 vs. 45 pb kb per year, p < 0.0001). Also, qPCR did not recapitulate flow-FISH findings, as the telomere dynamics by qPCR followed a linear model. Conclusion: The quantile regression analysis accurately reproduced a third-orderpolynomial TL attrition rate in both women and men, but it depended on the technique applied to measure TL. The Flow-FISH reproduced the expected telomere dynamics through life and, differently from the qPCR, was able to detect the subtle TL variations associated with sex and aging.

ABSTRACT INTRODUCTION: The development of monoclonal antibodies (mAbs) against Calcitonin Gene Related Peptide (CGRP) has determined a new therapeutic era in migraine prophylaxis, demonstrating its effectiveness in patients with episodic migraine (EM) and chronic migraine (CM), obtaining a response in naive patients and in those who are refractory to multiple medications. A 50% decrease in migraine attacks per month during the first 3 months of use is the approximate outcome in 50% of patients receiving this therapy. OBJECTIVE: This consensus from the Colombian Association of Neurology (ACN) has the objective of serving as a guide for the rational use of antiCGRP mAbs in patients with EM and CM. METHODS AND MATERIALS: The headache committee through the application of the Delphi methodology and discussions in subsequent meetings, develops this consensus, supported in the published literature and expert recommendations. RESULTS: Fourteen answers from headache experts were received regarding the use of drugs for migraine prophylaxis, analyzing their applicability in frequent clinical situations. DISCUSSION: AntiCGRP mAbs have proved their effectiveness with adequate pathophysiological support, but with a high price in a highly prevalent disease, there is then a need to select the patient who best benefits from this therapy. CONCLUSIONS: AntiCGRP mAbs are recommended in patients with EM and CM that have previously failed to other prophylactic drugs.

RESUMEN INTRODUCCIÓN: El desarrollo de anticuerpos monoclonales (mAbs) contra el péptido relacionado con el gen de la calcitonina (CGRP) ha determinado una nueva era terapéutica en la profilaxis de migraña, demostrando su efectividad en pacientes con migraña episódica (ME) y migraña crónica (MC), con respuesta desde pacientes naïve hasta refractarios a múltiples medicamentos. La disminución del 50% de los ataques de migraña al mes (DMM) durante los primeros 3 meses de uso es el desenlace aproximado en el 50% de los pacientes que reciben esta terapia. OBJETIVO: Este consenso de la Asociación Colombiana de Neurología (ACN) tiene el objetivo de guiar la selección y uso racional de los mAbs antiCGRP en pacientes con ME y MC. MATERIALES Y MÉTODOS: El comité de cefalea de la ACN mediante la aplicación de la metodología Delphi y discusiones en reuniones posteriores desarrolló un documento en formato de consenso soportado en literatura y recomendaciones de expertos. RESULTADOS: Se obtuvieron respuestas de 14 expertos en cefalea sobre moléculas utilizadas en profilaxis de migraña, analizando su aplicabilidad en situaciones clínicas frecuentes. DISCUSIÓN: Los mAbs antiCGRP han demostrado efectividad con adecuado soporte fisiopatológico, considerando que son moléculas de alto precio en una enfermedad de alta prevalencia, existe la necesidad de guíar la selección del paciente que mejor puede beneficiarse de su administración CONCLUSIONES: Los mAbs antiCGRP están recomendados en pacientes con ME y MC que presentan falla terapéutica a otras moléculas profilácticas.

Abstract Introduction The average life expectancy, as well as the prevalence of Type 2 diabetes (T2D), is increasing worldwide. Population-based studies have demonstrated that the duration of T2D has been associated with cognitive impairment. However, despite the high prevalence of T2D and cognitive impairment in Chile, the association between years with T2D and suspicion of cognitive impairment has not yet been investigated. The objective of this study was to investigate the association between duration of T2D and suspicion of cognitive impairment in Chilean older adults. Material and Methods 1,040 older adults aged ≥60 years from the Chilean National Health Survey (2009-2010) were included. Suspicion of cognitive impairment was assessed by the abbreviated Mini-Mental State Examination (MMSE). The number of years with T2D was self-reported and categorised into four groups. Poisson Regression analysis was used to assess the association between altered MMSE and the number of years with DM2, adjusted by potential confounders including socio-demographic, lifestyle, adiposity and health-related factors. Results When the analyses were adjusted for socio-demographic factors, people who had T2D for 15 to 24 and ≥25 years had 2.2-times (95%CI: 1.07; 3.33) and 5.8-times (95%CI: 3.81; 11.0) higher relative risk (RR) of cognitive impairment, compared to those without T2D. When the analyses were additionally adjusted for lifestyle and health-related covariates, the RR for cognitive impairment was 1.76-times (95%CI: 1.02; 2.50) and 4.54-times (95%CI: 2.70; 6.38) higher for those who had T2D for 14-24 years and ≥25 years, respectively. Conclusions Number of years with T2D was associated with suspicion of cognitive impairment. A longer duration of T2D was associated with a higher likelihood of cognitive impairment in the Chilean older population, independently of confounder factors included in the study.

RESUMEN Introducción: La esperanza de vida está aumentando en todo el mundo, así como la diabetes tipo 2 (DM2). Estudios poblacionales han demostrado que la duración de la DM2 se ha asociado con el deterioro cognitivo. Sin embargo, a pesar de la alta prevalencia de DM2 y deterioro cognitivo en Chile, aún no se ha investigado la asociación entre años con DM2 y la sospecha de deterioro cognitivo. El objetivo del estudio fue investigar la asociación entre la duración de la DM2 y la sospecha de deterioro cognitivo en personas mayores chilenas. Material y Métodos Participaron 1.040 personas ≥60 años de la Encuesta Nacional de Salud de Chile (2009-2010). El deterioro cognitivo se evaluó mediante el Mini Examen del Estado Mental abreviado (MMSE). El número de años con DM2 fue categorizado en cuatro grupos. Para valorar la asociación entre MMSE alterado y el número de años con DM2, se utilizó una regresión de Poisson, ajustados a posibles factores de confusión sociodemograficos, de estilos de vida, adiposidad y salud. Resultados Cuando se ajustaron los análisis por factores sociodemográficos, las personas con 15 a 24 y ≥25 años con DM2 presentaron 2,2 veces (IC95%: 1,07; 3,33) y 5,8 veces (IC95%: 3,81; 11,0) riesgo relativo (RR) de deterioro cognitivo, en comparación con aquellas sin DM2. Luego de ajustar adicionalmente los análisis para las covariables relacionadas con el estilo de vida y la salud, el RR para deterioro cognitivo fue 1,76 veces (IC95%: 1,02; 2,50) y 4,54 veces (IC95%: 2,70; 6,38) más alto para aquellas personas con 14-24 y ≥25 años de DM2. Conclusiones Se asoció el número de años con DM2 con la sospecha de deterioro cognitivo. Una mayor duración de la DM2 se asoció con una mayor probabilidad de deterioro cognitivo en la población mayor chilena.

Abstract Individuals with malignancies and COVID-19 have a lower survival compared with the general population. However, the information about the impact of COVID-19 on the whole hematological population is scarce. We aimed to describe the 30th day overall survival (OS) after COVID-19 infection in pa tients with a hematological disease in Argentina. A completely anonymous survey from the Argentine Society of Hematology was delivered to all the hematologists in Argentina; it started in April 2020. A cut-off to analyze the data was performed in December 2020 and, finally, 419 patients were reported and suitable for the analysis (average age: 58 years, 90% with malignant diseases). After the COVID-19 diagnosis, the 30-day OS for the whole population was 80.2%. From the entire group (419), 101 (24.1%) individuals required intensive care unit admission, where the 30-day OS was 46.6%. Among allogeneic stem cell transplant recipients, the 30-day OS was 70.3%. Factors associated with a low OS were two or more comorbidities, an active hematological disease and history of chemotherapy. In individuals with the three factors, the 30-day OS was 49.6% while the 30-day OS in those without those factors was 100%. Patients with hematological diseases have a higher mortality than the general population. This group represents a challenge and requires careful decision-making of the treatment in order not to compromise the chances of cure.

Resumen El presente estudio tuvo por objetivo primario conocer la mortalidad de pacientes con enfermedad hematológica que presentaron infección por COVID-19 en Argentina. Para ello se difundió una encuesta desde la Sociedad Argentina de Hematología (SAH) entre los hematológos para informar sobre los pacientes con enfermedades hematológicas y diagnóstico de infección por SARS- CoV-2, entre el 19/4/2020, y el 7/12/2020. Se incluyeron individuos de todas las edades con diagnóstico de enfermedad hematológica benigna o maligna e infección por SARS-CoV-2 confirmada por técnica de RT-PCR. Se analizaron 419 pacientes (mediana 58 años; 90% enfermedades malignas). La supervivencia al día 30 fue de 80.2%. La supervivencia fue menor en aquellos que requirieron internación (74.2%), cuidados intensivos (46.6%) y asistencia respiratoria mecánica (36.8%). Entre los trasplantados alogénicos la supervivencia fue 70.3%. Los factores vinculados a la supervivencia global fueron las comorbilidades, el estado de la enfermedad al momento de la infección y el antecedente de quimioterapia. Se pudo establecer un score en el que aquellos que tuvieron un puntaje de 4 alcanzaron una supervivencia del 49.6% al día 30, mientras que la de los pacientes con score 0 fue del 100% a 30 días. En comparación con la población general, los pacientes con enfermedades hematológicas presentan una mayor mortalidad vinculada al COVID-19, motivo por el cual es primordial definir pautas destinadas a disminuir la exposición de los mismos sin comprometer las posibilidades de beneficiarse del tratamiento de la enfermedad de base.

ABSTRACT Objective: To study the association of SLC16A11 gene variants with obesity and metabolic markers in nondiabetic Chilean adults. Materials and methods: This cross-sectional study included 263 non-diabetic adults. The genotype of the rs75493593 polymorphism of SLC16A11 gene was performed by real-time PCR. It's association with adiposity markers (body weight, BMI, waist circumference and fat mass percentage), metabolic markers (glucose, insulin, HOMAIR, leptin, total cholesterol, LDLc, HDLc, triglycerides, ALT, GGT and hsCRP) and blood pressure was analyzed by linear regression. Results: The minor allele (T) of the SLC16A11 gene (rs75493593) has a frequency of 29.7% among Chileans. Risk genotypes (GT and TT) were associated with a significant 1.49 mU/l increase in plasmatic insulin for each copy of the minor allele (95% CI: 0.12, 2.87, p < 0.05). This association remained significant after adjusting for socio-demographic variables, physical activity and smoking (1.36 mU/l, 95% CI: 0.16, 2.58 p < 0.05), but was lost when BMI was included as a confounding factor. Higher BMI was also significantly associated with polymorphic genotypes in SLC16A11, independent of socio-demographic variables. Conclusion: The minor allele of the SLC16A11 gene (T) is highly prevalent among Chileans and is associated with increased insulin and BMI in nondiabetic individuals. These findings suggest that the genetic variant in SLC16A11 is not only associated with type 2 diabetes as previously shown in Mexicans, but is also related to early metabolic alterations in healthy subjects that may lead to type 2 diabetes.

OBJECTIVE: To assess the possible factors that influence sleep quality in adolescents with and without chronic immunosuppressive conditions quarantined during the coronavirus disease 2019 (COVID-19) pandemic. METHODS: This cross-sectional study included 305 adolescents with chronic immunocompromised conditions and 82 healthy adolescents. Online surveys were completed, which included questions on socio-demographic data and self-rated healthcare routine during COVID-19 quarantine and the following validated questionnaires: the Pittsburgh Sleep Quality Index (PSQI), Pediatric Quality of Life Inventory 4.0 (PedsQL4.0), and Pediatric Outcome Data Collection Instrument (PODCI). RESULTS: The median current age [14 (10-18) vs. 15 (10-18) years, p=0.847] and frequency of female sex (62% vs. 58%, p=0.571) were similar in adolescents with chronic conditions compared with healthy adolescents. The frequency of poor sleep quality was similar in both groups (38% vs. 48%, p=0.118). Logistic regression analysis, including both healthy adolescents and adolescents with chronic conditions (n=387), demonstrated that self-reported increase in screen time (odds ratio [OR] 3.0; 95% confidence interval [CI] 1.3-6.8; p=0.008) and intrafamilial violence report (OR 2.1; 95% CI 1.2-3.5; p=0.008) were independently associated with poor sleep quality in these adolescents. However, the PODCI global function score was associated with a lower OR for poor sleep quality (OR 0.97; 95% CI 0.94-0.99; p=0.001). Further logistic regression, including only adolescents with chronic conditions (n=305), demonstrated that self-reported increase in screen time (OR 3.1; 95% CI 1.4-6.8; p=0.006) and intrafamilial violence report (OR 2.0; 95% CI 1.2-3.4; p=0.011) remained independently associated with poor quality of sleep, whereas a lower PODCI global function score was associated with a lower OR for sleep quality (OR 0.96; 95% CI 0.94-0.98; p<0.001). CONCLUSION: Self-reported increases in screen time and intrafamilial violence report impacted sleep quality in both healthy adolescents and those with chronic conditions. Decreased health-related quality of life was observed in adolescents with poor sleep quality.

Resumen Objetivo: Establecer la prevalencia de mujeres mayores de 50 años con mamas densas estudiadas por mamografía en el área de Bucaramanga. Métodos: Se realizó un estudio descriptivo retrospectivo durante el periodo comprendido entre enero de 2014 y agosto de 2015, en el que se revisaron 10.110 mamografías de pacientes mayores de 50 años. Posteriormente, se hizo una revisión de la literatura en las principales bases de datos. Resultados: De las 10.110 mamografías realizadas, 4.448 fueron encontradas como positivas para mama densa. De estas, el 39,69% correspondió a tejido mamario heterogéneamente denso y el 4,29%, a tejido mamario extremadamente denso, para un total de 43,9% de los casos estudiados. Conclusión: La densidad mamaria es un factor de riesgo independiente de cáncer de mama, considerado de mediana importancia frente a otros factores. A medida que incrementa la densidad mamaria, se genera la superposición del tejido mamario radio-opaco que puede ocultar y llegar a retardar el diagnóstico de un cáncer subyacente. Dada la alta prevalencia de tejido mamario denso encontrado en la población estudiada, se sugiere complementar el tamizaje con otros métodos de imágenes.

Abstract Objective: To establish the prevalence of dense breast tissue in a group of women aged over 50 years, who were studied with mammography in the city of Bucaramanga (Colombia). Methods: A retrospective descriptive study was conducted from January 2014 to August 2015; 10.110 mammograms of patients aged over 50 years were reviewed. Subsequently, a review of the literature in the main databases was made. Results: From 10.110 mammograms that were performed, 4.448 were found to be positive for dense breast tissue. 39,69% had heterogeneously dense breast tissue, and 4,29% extremely dense breast tissue, for a total result of 43,9% patients with positive dense breast tissue. Conclusion: Breast density is an independent risk factor for breast cancer, being of medium importance compared with other factors. As the breast density increases, the overlap of the radiopaque breast tissue that can hide and delay the diagnosis of an underlying cancer is generated. The high prevalence of dense breast tissue that we found in this population suggests the need of complementing screening with other imaging methods.
