RESUMEN Objetivo. Conocer la opinión del profesorado sobre la calidad e influencia del Programa Recreo Saludable en la mejora de los refrigerios que lleva el alumnado. Método. Estudio observacional, descriptivo y transversal, mediante encuesta en línea autoadministrada al profesorado de los centros adheridos al Programa Recreo Saludable de Extremadura. Se preguntó por el seguimiento de las recomendaciones sobre consumo de refrigerios saludables, por parte de los alumnos, con respuestas tipo Likert de tres niveles de adherencia (alta: 75-100%, media: 50% y baja: 0-25%) y por la calidad de los recursos materiales y formativos. Se consideró el nivel de significación estadística para un p-valor menor de 0,05. Resultados. El 82,8%, 52,3% y 8,5% del profesorado observó una alta adherencia a la recomendación de incluir agua, fruta y pan integral, respectivamente, y el 38,1%, 33,0% y 62,7% a eliminar zumos envasados, lácteos azucarados y bollería, respectivamente. La satisfacción general y los recursos del programa se evaluaron en 4,3 y 4 puntos, sobre 5, respectivamente. Conclusiones. Para el profesorado encuestado, el programa tuvo un impacto alto en la inclusión de agua y la eliminación de bollería industrial en los refrigerios; medio en la inclusión de fruta y eliminación de zumos envasados y lácteos azucarados, y bajo en la inclusión de pan integral. Otorga una alta valoración a los recursos facilitados y una alta satisfacción general.

ABSTRACT Objective. To ascertain the teachers' opinion about the quality and impact of the healthy break programme on the improvement of snacks brought in by students. Methods. Observational, descriptive and cross-sectional study by means of a self-administered online survey to teachers from the centres taking part in the healthy break programme in Extremadura. We asked about the follow-up of the recommendations on the consumption of healthy snacks by students, with three levels of adherence Likert-type responses (high: 75%-100%, medium: 50% and low: 0%-25%) and quality of material and training resources. A P-value less than 0.05 was deemed statistically significant. Results. A total of 82.8%, 52.3% and 8.5% of teachers observed high adherence with the recommendation to include water, fruit and whole-grain bread, respectively. Moreover, 38.1%, 33.0% and 62.7% to eliminate packaged juices, sweetened dairy products and pastries, respectively. Overall satisfaction and programme resources were rated at 4.3 and 4 points, out of 5, respectively. Conclusions. For the teachers surveyed, the programme had a high impact on the inclusion of water and elimination of industrial pastries in snacks; a medium impact on the inclusion of fruit and elimination of packaged juices and sweetened dairy products; and a low impact on the inclusion of whole-grain bread. The teaching staff offered a high evaluation of the resources provided and general satisfaction.

Resumen Objetivo: Evaluar el coste-efectividad de diferentes estrategias de vacunación universal infantil frente a la hepatitis A en España. Método: A partir de un modelo dinámico y un árbol de decisión, se realizó un análisis de coste-efectividad para comparar tres estrategias de vacunación frente a la hepatitis A: no vacunación y vacunación universal infantil con una y dos dosis. La perspectiva del estudio escogida fue la del Sistema Nacional de Salud (SNS) y se consideró como horizonte temporal toda la vida del paciente. Tanto los costes como los efectos se descontaron al 3% anual. Los resultados en salud se midieron en años de vida ajustados por calidad (AVAC) y la medida de coste-efectividad utilizada es la razón de coste-efectividad incremental (RCEI). Además, se llevaron a cabo análisis de sensibilidad determinísticos por escenarios. Resultados: En el caso particular de España, con baja endemicidad de hepatitis A, las diferencias en resultados en salud entre las distintas estrategias de vacunación (con una o dos dosis) y la no vacunación son prácticamente inexistentes, en términos de AVAC. Además, las RCEI obtenidas son elevadas, superando los límites establecidos de disposición a pagar obtenidos en España (22.000-25.000 €/AVAC). El análisis de sensibilidad determinístico muestra que los resultados son sensibles a las variaciones de los parámetros clave, aunque en ningún caso resultan coste-efectivos. Conclusiones: La vacunación universal infantil frente a la hepatitis A no sería una opción coste-efectiva desde la perspectiva del SNS en España en la actualidad.

Abstract Objective: To evaluate the cost-effectiveness of infant universal vaccination against hepatitis A in Spain. Method: Using a dynamic model and decision tree model, a cost-effectiveness analysis was performed to compare three vaccination strategies against hepatitis A: non-vaccination strategy versus universal childhood vaccination of hepatitis A with one or two doses. The perspective of the study was that of the National Health System (NHS) and a lifetime horizon was considered. Both costs and effects were discounted at 3% per year. Health outcomes were measured in terms of quality adjusted life years (QALY) and the cost-effectiveness measure used was the incremental cost-effectiveness ratio (ICER). In addition, deterministic sensitivity analysis by scenarios was performed. Results: In the particular case of Spain, with low endemicity for hepatitis A, the difference in health outcomes between vaccination strategies (with 1 or 2 doses) and non-vaccination are practically non-existent, terms of QALY. In addition, the ICER obtained is high, exceeding the limits of willingness to pay from Spain (€22,000-25,000/QALY). The deterministic sensitivity analysis showed that the results are sensitive to the variations of the key parameters, although in no case the vaccination strategies are cost-effective. Conclusions: Universal infant vaccination strategy against hepatitis A would not be a cost-effective option from the NHS perspective in Spain.

Abstract Objective: Inflammatory joint diseases cause pain and disability. The objective of this study was to measure the quality of life of patients with rheumatoid arthritis, psoriatic arthritis, and ankylosing spondylitis treated with certolizumab pegol and compare the results with those of the general population. Method: Using a cross-sectional design and sociodemographic and clinical variables, adherence to treatment and quality of life data were collected using the Euroqol-5d-5L (EQ-5D) questionnaire. The quality of life of the general population was obtained from the Spanish National Health Survey. Answers to the EQ-5D questionnaire were analyzed in both groups using two-part models, which measure the probability of having perfect health as well as the disutility caused by the disease. Results: The sample included 59 patients with high adherence (92.3%). The mean utility value was 0.78 and pain was the most affected dimension. The reduction in utility (EQ-5D index) of patients with inflammatory joint disease as compared to the general population was 0.127. Conclusions: The subjects of the study showed a significantly lower quality of life than the general population despite effective control of the disease. Two-part models facilitate the interpretation of quality-of-life studies using the EQ-5D.

Resumen Objetivo: Las enfermedades articulares inflamatorias causan dolor y discapacidad. El objetivo fue medir la calidad de vida de los pacientes con artritis reumatoide, artritis psoriásica, espondilitis anquilosante en tratamiento con certolizumab pegol y compararla con la de la población general. Método: Con un diseño transversal se recogieron variables sociodemográficas y clínicas, adherencia al tratamiento y calidad de vida mediante el cuestionario Euroqol-5d-5L (EQ-5D). La calidad de vida de la población general se obtuvo de la Encuesta Nacional de Salud. El EQ-5D se analizó en ambos grupos mediante modelos de dos partes que miden la probabilidad de tener una salud perfecta y la disutilidad causada por la enfermedad. Resultados: La muestra incluyó 59 pacientes con una adherencia alta (92,3%). La utilidad media fue de 0,78 y el dolor resultó la dimensión más afectada. La reducción de utilidad (índice EQ-5D) de los pacientes con enfermedades articulares inflamatorias respecto a la población general fue de 0,127. Conclusiones: Los pacientes muestran una calidad de vida significativamente menor que la población general a pesar del buen control de la enfermedad. Los modelos de dos partes facilitan la interpretación de los estudios de calidad de vida mediante EQ-5D.

Resumo: Strongyloides stercoralis é um nematódo intestinal que coloniza e se reproduz na mucosa do intestino delgado proximal. A infeção em hospedeiros imunocompetentes é auto-limitada mas em doentes imunocomprometidos pode ter um curso complicado e causar hiperinfeção. Apresentamos um caso de uma mulher de 60 anos que é admitida devido a uma exacerbação de uma púrpura trombocitopénica trombótica adquirida com necessidade de altas doses de corticoides. A doente inicia quadro de pirose persistente, náuseas, vómitos e intolerância alimentar. Faz-se o diagnóstico de síndrome de secreção inapropriada de hormona antidiurética (SIADH). A endoscopia digestiva alta evidencia mucosa gástrica e duodenal com edema e eritema, para além de erosões e pregas espessadas duodenais. O TC e a enteroRMN mostram dilatação duodenal e alterações inflamatórias. A histologia mostra S. stercoralis a colonizar a mucosa do antro e duodeno. O S. stercolaris é um parasita humano, endémico em regiões tropicais e subtropicais. Tem um ciclo de vida complexo já que completa o seu ciclo todo dentro do organismo humano: penetra pela pele, migra para os pulmões e atinge o trato gastrointestinal. Os sítios mais afetados são o duodeno e o jejuno proximal. O ciclo de vida envolve autoinfeção na mucosa intestinal ou pele perianal, especialmente em doentes imunocomprometidos, com a imunodepressão podendo levar a síndrome de hiperinfeção e doença disseminada. Contudo, o envolvi mento gástrico é raramente descrito. O SIADH tem sido relacionado com o síndrome de hiperinfeção, contudo, o seu mecanismo não é claro. O relativo inespecífico quadro clínico e alterações imagiológicas, assim como a baixa sensibilidade dos testes de parasitas de rotina atrasam e fazem o diagnóstico desafiante.

Abstract: Strongyloides stercoralis is an intestinal nematode that colonizes and reproduces in the upper small intestinal mucosa. Infection in immunocompetent hosts is self-limited but in immunocompromised patients it can be complicated and cause hyperinfection. We present a 60-year-old female who was admitted due to an exacerbation of acquired thrombotic thrombocytopenic purpura requiring high doses of corticosteroids. The patient began to experience persistent pyrosis, nausea, vomiting, and oral intolerance. She was di agnosed with syndrome of inappropriate secretion of antidiuretic hormone (SIADH). Upper endoscopy was performed and showed esophageal, gastric, and duodenal mucosa with edema and erythema. Moreover, there were superficial erosions and thickened folds in duodenum. Gastric and duodenal biopsies were taken. Abdominal computed tomography and magnetic enteroresonance displayed duodenal dilation and inflammatory changes. The histological study of biopsies showed colonization by S. stercolaris in the antrum and duodenum. S. stercolaris is a human parasite that is endemic in tropical, subtropical, and temperate regions. Its lifecycle is complex because it completes its entire cycle within the human host; it penetrates the skin, migrates to the lungs, and reach the gastrointestinal tract. The most affected site is the duodenum and upper jejunum. The lifecycle includes autoinfection through the intestinal mucosa or perianal skin, especially in immunocompromised hosts. Immunossuppression can lead to hyperinfection syndrome and disseminated disease. However, involvement of the stomach has relatively rarely been reported. SIADH has been related to systemic hyperinfection, although the mechanism is not clear. The relatively nonspecific clinical and imaging features and the low sensitivity of routine parasite tests make the diagnosis challenging and delayed.

Abstract Introduction: Selection of plants not adapted to the environment and low water availability are factors that limit the success of reforestation. Objective: To determine drought resistance in plants from three provenances of Pinus cembroides Zucc. and three of P. orizabensis D. K. Bailey and Hawksworth. Materials and methods: An irrigation experiment (38 to 45 % moisture) and a drought experiment (30 to 36 % moisture) were established for 11 months, starting with 16-month-old plants. A split-plot experimental design consisting of two moisture environments (irrigation and drought) with replicates (four blocks) nested within them was used; in each block six populations (three per species) with 10 plants per experimental unit were evaluated. Results and discussion: The biomass in different parts of the plants was 24 to 51 % lower in drought. Pinus cembroides had greater growth in height, stem base diameter and stem and root biomass in irrigation, and greater growth in diameter and biomass of branches and root in drought than P. orizabensis. The aerial/root biomass ratio was higher in P. orizabensis, which increased by 34 % in drought. In P. orizabensis, the provenance with the best performance in both environments was the one from the highest elevation. Conclusion: Pinus cembroides showed greater growth and drought resistance than P. orizabensis. There are differences among provenances within each species in restrictive and non-limited moisture conditions.

Resumen Introducción: La selección de plantas no adaptadas al ambiente y la baja disponibilidad de agua son factores que limitan el éxito de las reforestaciones. Objetivo: Determinar la resistencia a la sequía en plantas de tres procedencias de Pinus cembroides Zucc. y tres de P. orizabensis D. K. Bailey y Hawksworth. Materiales y métodos: Se estableció un experimento de riego (38 a 45 % de humedad) y sequía (30 a 36 % de humedad) por 11 meses, iniciando con plantas de 16 meses de edad. El diseño experimental fue parcelas divididas: dos ambientes de humedad (riego y sequía) con repeticiones (cuatro bloques) anidadas dentro de ellos; en cada bloque se evaluaron seis poblaciones (tres por especie) con 10 plantas por unidad experimental. Resultados y discusión: La biomasa de diversas partes de las plantas fue 24 a 51 % menor en la sequía. Pinus cembroides presentó mayor crecimiento en altura, diámetro de la base del tallo y biomasa de tallo y raíz en riego, y mayor crecimiento en diámetro y biomasa de ramas y raíz en sequía que P. orizabensis. La relación biomasa aérea/radical fue mayor en P. orizabensis, la cual aumentó 34 % en sequía. En P. cembroides, el sistema radical y el crecimiento fueron mayores en plántulas procedentes de la población más árida. En P. orizabensis, la procedencia con mejor desempeño en ambos ambientes fue la de mayor elevación. Conclusión: Pinus cembroides mostró mayor crecimiento y resistencia a la sequía que P. orizabensis. Hay diferencias entre procedencias dentro de cada especie en condiciones de humedad restrictivas y en las no limitadas.

Abstract Background and objective In 1995, the first notification relating anaplastic large cell lymphoma (BIA-ALCL) and breast implants was established. Twenty four years later, hundreds of articles have been published about this topic. The aim of this study is to review the published cases and summarize the current knowledge about this entity bringing it closer to Hispanic readers. Methods A systematic review was performed in PubMed, ScienceDirect, SciELO and Google Scholar databases since 1995 to October 2019. Results A total number of 122 case reports were analyzed. The information collected was heterogeneous. The shortage of Ibero-Latinoamerican published cases was evidenced. Data elements abstracted included information about patient demographics, medical history, implant characteristics, presenting symptoms, diagnosis and staging, treatment, and patient outcomes. Conclusions Despite diagnosis and current treatment to BIA-ALCL are fairly standardized, more rigorous studies are required to establish actual incidence and etiology. The lack of common criteria when collecting or reporting clinical cases makes difficult a truthful and uniform data collection. Communication of any incident related to breast implants, both to the national implant registries and to the scientific community, is necessary in order to gather quality information as a basis for evidence-based decision making.

Resumen Introducción y objetivo Desde 1995 en que se publicó la primera notificación que relacionaba el linfoma anaplásico de células grandes al hecho de portar implantes mamarios (LACG-AIM) han transcurrido más de 25 años y han sido publicados cientos de artículos. El presente trabajo tiene como objetivo realizar una revisión sistemática y analítica de los casos publicados, así como sintetizar el conocimiento actual sobre esta entidad y acercarlo al lector de habla hispana. Material y método Realizamos una búsqueda sistemática en las bases de datos PubMed, ScienceDirect y SciELO así como en el buscador de Google Académico entre 1995 y octubre de 2019, que pretende revisar las características de los casos recogidos en la literatura en dicho periodo de tiempo. Resultados El número total de casos recogidos en la bibliografía analizada fue de 122. La información resultó heterogénea y mayoritariamente basada en notificaciones de casos. Cabe destacar la escasez de casos publicados desde países íbero-latinoamericanos. Exponemos los principales datos recogidos relativos a características del linfoma, sintomatología, diagnóstico, patogenia, estudios genéticos, mutaciones, tratamiento, pronóstico y supervivencia. Conclusiones Aunque el diagnóstico y tratamiento actual del LACG-AIM se encuentran bastante estandarizados, la incidencia real y la etiología de esta entidad necesitan de estudios más rigurosos. La falta de criterios comunes a la hora de recoger o notificar los casos hace difícil una recogida veraz y uniforme. Es necesaria la comunicación de cualquier incidente relacionado con las prótesis mamarias, tanto a los registros nacionales de implantes como a la comunidad científica, a fin de recopilar información de calidad como base para la toma de decisiones basadas en evidencia.
