ABSTRACT Purpose To identify the risk of dysphagia and its association with signs suggestive of sarcopenia, nutritional status and frequency of oral hygiene in the hospitalized elderly. Methods This is an analytical cross-sectional study with the participation of 52 elderly patients admitted to a medical clinic at a public hospital in the Federal District, Brazil. The Eating Assessment Tool, Strength, Assistance with walking, Rise from a chair, Climb stairs and Falls + Calf Circumference and the Mini Nutritional Assessment shortform were applied, in addition to the collection of sociodemographic data and health conditions. Results Among the elderly participants, 30.8% were at risk of self-reported dysphagia. The factors associated with the risk of dysphagia were: signs suggestive of sarcopenia (p=0.04), nutritional status (p<0.001) and oral hygiene frequency (p=0.03). Conclusion In the geriatric population of the present study, with the majority of the participants having tested positive for Covid-19, the risk of dysphagia was associated with signs suggestive of sarcopenia, nutritional status and frequency of oral hygiene.

RESUMO Objetivo Identificar o risco de disfagia e sua associação com os sinais sugestivos de sarcopenia, estado nutricional e frequência da higiene oral em idosos hospitalizados. Método Trata-se de um estudo transversal analítico com participação de 52 idosos internados em clínica médica de um hospital público no Distrito Federal. Foram aplicados os instrumentos Eating Assessment Tool, Strength, Assistance with walking, Rise from a chair, Climb stairs and Falls + Circunferência da Panturrilha e o Mini Nutritional Assessment shortform além de coleta de dados sociodemográficos e de condições de saúde. Resultados Dos idosos participantes 30,8% apresentaram risco de disfagia autorrelatada. Os fatores associados ao risco de disfagia foram sinais sugestivos sarcopenia (p=0,04), estado nutricional (p<0,001) e frequência da higiene oral (p=0,03). Conclusão Na população geriátrica deste estudo, em sua maioria com Covid-19, o risco de disfagia esteve associado aos sinais sugestivos de sarcopenia, estado nutricional e frequência da higiene oral.

ABSTRACT Purpose: to analyze the telehealth speech therapy approach in patients with oropharyngeal dysphagia and determine the applications and effects of this practice. Methods: the following descriptors were used for selection: Telerehabilitation, Telemedicine, "Remote Consultation", "Healthcare Delivery", "Distance Counseling", "Therapy, Computer-Assisted", "Speech-Language Pathology", "Speech Therapy", "Swallowing Disorders", Dysphagia, “Bottle Feeding" and "Enteral Nutrition” in PubMed, MedLine, Scopus and Web of Science databases and in the gray literature, by Google Scholar and ProQuest. Studies were selected without time limits, in Portuguese, English and Spanish, that described the applicability and/or effects of telehealth in speech therapy practice in patients with swallowing disorders/dysphagia, regardless of gender and age. For analysis, the following were considered: level of scientific evidence and recommendation, telehealth modality, objectives, methods and results/conclusion. Literature Review: 490 articles were found and, after exclusion of duplicates, analysis of titles, abstracts and reading of full articles, 22 studies were selected. The articles were classified into telehealth modalities: tele-education, telediagnosis, teleconsulting, teleregulation and telemonitoring. Conclusion: the telehealth modalities described had a great potential to promote significant improvements in patients presented with swallowing disorders/dysphagia, suggesting them as viable for speech therapy services. Among them, teleconsultation was the least addressed.

RESUMO Objetivo: analisar a abordagem fonoaudiológica por meio da telessaúde em pacientes com disfagia e determinar as aplicações e efeitos desta prática. Métodos: para seleção foram utilizados os descritores: Telerehabilitation, Telemedicine, "Remote Consultation", "Delivery of Health Care", "Distance Counseling", "Therapy, Computer-Assisted", “Speech-Language Pathology”, "Speech Therapy", "Deglutition Disorders", Dysphagia, “Bottle Feeding" e "Enteral Nutrition” nas bases de dados PubMed, MedLine, Scopus e Web of Science e na literatura cinzenta, por meio do Google Acadêmico e ProQuest. Foram selecionados estudos sem delimitação de tempo, nas línguas português, inglês e espanhol, que descrevessem a aplicabilidade e/ou efeitos da telessaúde na prática fonoaudiológica junto à pacientes com alteração da deglutição/disfagia, sem restrição de sexo e idade. Para análise considerou-se: nível de evidência científica e recomendação, modalidade de telessaúde, objetivos, métodos e resultados/conclusão. Revisão da Literatura: foram encontrados 490 artigos e, após exclusão de duplicatas, análise dos títulos, resumos e leitura completa dos artigos, 22 estudos foram selecionados. Os artigos foram classificados nas modalidades da telessaúde: tele-educação, telediagnóstico, teleconsultoria, telerregulação e telemonitoramento. Conclusão: as modalidades de telessaúde descritas tiveram grande potencial para promover melhoras significativas em pacientes com alteração de deglutição/disfagia, sugerindo-as como viáveis para serviços fonoaudiológicos. Entre elas, a teleconsulta foi a menos explorada.

Abstract Objective To evaluate swallowing-related quality of life in hospitalized elderly patients. Methods Cross-sectional analytical and observational approach was used in our study. Fifty-two elderly patients in a medical clinic of a public hospital in Distrito Federal [Federal District] participated. In addition to collecting sociodemographic and health condition data, the Eating Assessment Tool and the swallowing-related quality of life Questionnaire were applied. Results Of the elderly participants, 30.8% had a self-reported risk of dysphagia. The elderly at risk for dysphagia had lower scores in the “eating duration” domain and higher scores in the “mental health” domain. The only domain in which there was no statistical difference in the response pattern of the participants who were, or were not at risk for dysphagia was the “sleep domain.” Among the variables of the 11 domains of the Swallowing Quality of Life Questionnaire, mostly positive correlations were found, with different degrees. Conclusion The swallowing-related quality of life of hospitalized elderly patients is directly manifested as decreased social interaction, increased eating time, fear to eat, and swallowing as a burden.

Resumen Objetivo Evaluar la calidad de vida relacionada con la deglución en adultos mayores hospitalizados. Métodos Estudio transversal analítico y observacional. Participaron 52 adultos mayores internados en clínica médica de un hospital público en el Distrito Federal. Se aplicaron los instrumentos Eating Assessment Tool y Quality of Life in Swallowing Disordens, además de la recopilación de datos sociodemográficos y condiciones de salud. Resultados De los adultos mayores que participaron, el 30,8 % presentó riesgo de disfagia autoinformada. Los adultos mayores con riesgo de disfagia presentaron un menor puntaje en el dominio “tiempo para alimentarse” y un puntaje más alto en el dominio “salud mental”. El único dominio en el que no hubo una diferencia estadística en el tipo de respuesta de los participantes que presentaron o no riesgo de disfagia fue el del dominio sueño. Entre las variables de los 11 dominios del Quality of Life in Swallowing Disordens se observaron mayormente correlaciones positivas en diferentes niveles. Conclusión La calidad de vida relacionada con la deglución de adultos mayores hospitalizados está directamente manifestada con la reducción de la convivencia social, el aumento de tiempo para alimentarse, el miedo y la carga.

Resumo Objetivo Avaliar a qualidade de vida relacionada à deglutição em idosos hospitalizados. Métodos Estudo transversal analítico e observacional. Participaram 52 idosos internados em clínica médica de um hospital público no Distrito Federal. Foram aplicados os instrumentos Eating Assessment Tool e o Quality of Life in Swallowing Disordens, além de coleta de dados sociodemográficos e condições de saúde. Resultados Dos idosos participantes 30,8% apresentaram risco de disfagia autorrelatada. Os idosos com risco de disfagia apresentaram menor pontuação no domínio “tempo de se alimentar” e maior pontuação no domínio de “saúde mental”. O único domínio que não houve diferença estatística no padrão de resposta dos participantes que apresentaram ou não risco de disfagia foi o domínio sono. Entre as variáveis dos 11 domínios do Quality of Life in Swallowing Disordens foi possível observar correlações positivas em sua maioria com diferentes graus. Conclusão A qualidade de vida relacionada à deglutição de idosos hospitalizados está diretamente manifestada com a diminuição do convívio social, aumento do tempo para se alimentar, medo e fardo.

Abstract Objective to investigate and demonstrate the association between the self-perceived swallowing assessment carried out by nurses and the dysphagia risk classification carried out by speech therapists in hospitalized elderly patients. Method an analytical cross-sectional study using the Eating Assessment Tool and the Speech and Hearing Therapy Protocol for Dysphagia Risk Assessment in 52 elderly patients in a medical clinic, as well as collecting sociodemographic data and health conditions. Fisher's exact test and logistic regression for odds ratios were used for statistical analysis. Results there was an association (p=0.01) between the nurse's assessment and that of the speech therapist, with an odds ratio of a hospitalized elderly person exposed to the risk of dysphagia by the Eating Assessment Tool presenting a change in the Speech Therapy Protocol for Dysphagia Risk Assessment (OR 3.89 95% CI: 1.10-13.68). Conclusion and implications for practice the findings indicate that there is an association between the nurse's assessment and that of the speech therapist in swallowing disorders and that Nursing can act to identify risks, prevent and rehabilitate dysphagia.

Resumen Objetivo investigar y demostrar la asociación entre la evaluación de la autopercepción de la deglución realizada por enfermeros y la clasificación del riesgo de disfagia realizada por el audiólogo en ancianos hospitalizados. Método estudio analítico transversal realizado con la aplicación de los Eating Assessment Tool y del Protocolo de Evaluación del Riesgo de Disfagia por Logopedas en 52 pacientes ancianos de una clínica médica, además de la recogida de datos sociodemográficos y condiciones de salud. Para el análisis estadístico se utilizó la prueba exacta de Fisher y la regresión logística para el odds ratio. Resultados hubo asociación (p=0,01) entre la evaluación del enfermero y del logopeda, con odds ratio de un anciano hospitalizado expuesto al riesgo de disfagia por la Eating Assessment Tool presentar una alteración en el Protocolo Logopédico de Evaluación del Riesgo de Disfagia (OR 3,89 IC 95%: 1,10-13,68). Conclusión e implicaciones para la práctica los hallazgos indican que existe una asociación entre la valoración de enfermería y logopedia en los trastornos de la deglución y que la Enfermería puede actuar en la identificación de riesgos, prevención y rehabilitación en la disfagia.

Resumo Objetivo investigar e demonstrar a associação entre a avaliação de autopercepção da deglutição realizada pelo enfermeiro e a classificação de risco de disfagia realizada pelo fonoaudiólogo em idosos hospitalizados. Método estudo transversal analítico realizado com a aplicação dos instrumentos Eating Assessment Tool e do Protocolo Fonoaudiológico de Avaliação de Risco para Disfagia em 52 idosos em clínica médica, além da coleta de dados sociodemográficos e de condições de saúde. Para a análise estatística foi utilizado o teste exato de Fisher e a regressão logística para a razão de chances. Resultados houve a associação (p=0,01) entre a avaliação do enfermeiro e a do fonoaudiólogo, com razão de chances de um idoso hospitalizado exposto ao risco de disfagia pelo Eating Assessment Tool apresentar a alteração no Protocolo Fonoaudiológico de Avaliação do Risco para Disfagia (OR 3,89 IC 95%: 1,10-13,68). Conclusão e implicações para a prática os achados apontam que há uma associação entre a avaliação do enfermeiro e a do fonoaudiólogo nas alterações da deglutição e que a Enfermagem poderá atuar na identificação de riscos, prevenção e reabilitação em disfagia.

RESUMO Objetivo Apresentar um protocolo clínico e um sistema de medição de indicadores que auxilie no monitoramento de qualidade da assistência fonoaudiológica ao aleitamento materno em alojamento conjunto. Métodos Trata-se de um estudo descritivo, realizado em um hospital-escola do Distrito Federal. Até o momento inicial deste estudo, não havia um fonoaudiólogo no alojamento conjunto responsável pelo atendimento direcionado à alimentação. Foi identificada a possibilidade de atividades práticas e de estágio para os alunos do curso de Fonoaudiologia, mas para que isso fosse realizado, ações gerenciais eram necessárias. Iniciou-se a elaboração de um protocolo clínico assistencial, desenvolvido em duas etapas: 1) proposição do protocolo – utilizando-se a prática baseada em evidência; 2) avaliação do conteúdo e aparência do protocolo por profissionais envolvidos no cuidado materno-infantil. Resultados O Protocolo Fonoaudiológico de Assistência à Saúde Materno-Infantil (PASMI) foi composto por: verificação dos dados de internação no prontuário; anamnese; triagem fonoaudiológica; avaliação fonoaudiológica; intervenção fonoaudiológica; orientação fonoaudiológica e geração de indicadores de qualidade. As ações foram descritas citando-se as referências de base e definições operacionais. A fase de avaliação por especialistas apresentou escore médio de concordância de 0,889. As considerações emitidas pelos juízes foram acatadas quando mostraram embasamento científico. Conclusão A utilização de métodos padronizados poderá permitir a melhor assistência às condições de saúde fonoaudiológica do binômio mãe-bebê, assim como melhor formação de fonoaudiólogos. Novos estudos devem ser realizados, buscando verificar a efetividade do PASMI frente à população envolvida.

ABSTRACT Purpose Present a clinical protocol and indicator measurement system that contribute to monitoring the quality of speech therapy during breastfeeding in rooming-in care. Methods A descriptive study carried out in a University Hospital in the Federal District, Brazil. Until the start of this study, there was no speech therapist responsible for feeding-related care in the rooming-in department. The possibility of practical activities and internships for speech therapy students was identified, but to that end, management action was needed. Thus, a clinical care protocol was designed in two stages: 1) compiling the proposed protocol in line with evidence-based practice; 2) assessment of its content and appearance by professionals involved in mother and child care. Results The Mother and Child Speech Therapy Protocol (MCSTP; PASMI in Portuguese) consisted of the following: checking admission data on the medical chart; anamnesis; screening speech-language; assessing speech-language ; providing speech-language guidance and generating quality indicators. These actions were described based on references and operational definitions. Average agreement in the expert assessment phase was 0.889. The judges’ recommendations were accepted when supported by scientific evidence. Conclusion The use of standardized care methods could ensure better speech-language care for mothers and babies and improve speech therapy training. Further research is needed to assess the effectiveness of the MCSTP in the target population.

RESUMO Objetivo: descrever o padrão de mastigação e deglutição de indivíduos com obesidade, indicados à realização de cirurgia bariátrica/gastroplastia, antes do procedimento, por meio de protocolos clínicos padronizados. Métodos: trata-se de um estudo descritivo transversal, do tipo caso controle, que apresenta dados preliminares de um estudo maior. Oito participantes, divididos em quatro casos controle e quatro casos pesquisa, pareados por idade e gênero, foram submetidos a avaliação clínica fonoaudiológica por meio do “Questionário sobre comportamento alimentar e as condições anatomofuncionais do sistema estomatognático”, do protocolo AMIOFE-E e do teste de pressão máxima de língua. Resultados: os participantes dos casos pesquisa apresentaram em comparação aos casos controle: predominância de mastigação unilateral preferencial; referiram que apenas “engole o alimento”; repetição de deglutição em alimentos líquidos e sólidos, com presença de tensão dos músculos faciais; bochechas com volume aumentado e presença de flacidez; tempo de refeição e mastigação menores; tensão aumentada de lábios, músculos mentual e faciais no repouso e na deglutição; predominância de habilidade insuficiente com movimentos associados e/ou tremor nas tarefas de mobilidade de lábios, língua, mandíbula e bochechas; e desconhecimento da atuação fonoaudiológica. Conclusão: os pacientes do grupo pesquisa, apresentaram maior ocorrência de alterações miofuncionais orofaciais quando comparados ao grupo controle. Identifica-se a necessidade do aumento do número de participantes nesta série de casos, assim como a possibilidade de reavaliação no momento pós-cirúrgico visando uma descrição longitudinal do padrão de mastigação e deglutição.

ABSTRACT Objective: to describe the chewing and swallowing pattern of obese individuals indicated for bariatric surgery/gastroplasty before the procedure, using standardized clinical protocols. Methods: a cross-sectional descriptive case-control study that presents preliminary data for a larger investigation. Eight participants matched for age and sex were divided into two groups (four in the research group and four as controls) and submitted to clinical speech therapy assessment using the “Questionnaire on eating behavior and the anatomofunctional conditions of the stomatognathic system” from the Expanded Protocol of Orofacial Myofunctional Evaluation with Scores (OMES-E) and the maximum tongue pressure test. Results: when compared to controls, participants from the research group exhibited a predominantly unilateral chewing preference; reported that they just “swallowed food”; repeated swallowing of liquid and solid foods, with facial muscle tension; increased cheek volume and the presence of flaccidity; shorter meal and chewing times; increased tension of the lips, mentalis and facial muscles at rest and during swallowing; predominance of insufficient ability with associated movements and/or tremors during lip, tongue, jaw and cheek movements; and lack of knowledge regarding speech therapy. Conclusion: orofacial myofunctional dysfunctions were more frequent in patients from the research group when compared to controls. A need to increase the number of participants in this case series and conduct a post-surgery reassessment, in order to provide a longitudinal description of the chewing and swallowing pattern, was verified.

ABSTRACT Introduction: Dentofacial deformities are associated with changes in dental occlusion, which may cause changes in the myofunctional system according to the type of disproportion. These deformities can cause changes and/or adaptations in the stomatognathic system. The objective of this study was to identify the relationship between speech therapy and orthognathic surgery based on refereed publications. Method: A bibliographic survey was conducted without considering a specific publication period in the databases, PubMed, SciELO and BVS. Results: Fifteen articles were critically evaluated with respect to the type of study, objectives, number and sex of participants, methodology, results and conclusions. Based on their aims, the articles were grouped into 1) speech-therapy group (STG); 2) changes group (CG); 3) techniques group (GT) and 4) others group (OG). Conclusion: There is little information regarding direct speech therapy intervention, and the majority of the studies were directed to the aesthetic and functional changes in the hard and soft tissue in patients who underwent orthognathic surgery. Some studies addressed methods and/or specific examination procedures to verify the changes in the soft and hard tissues of the facial profile of patients who underwent orthognathic surgery, and a study detailed the profile of individuals who underwent treatment for dentofacial deformity and apnea-hypopnea syndrome.

RESUMO Introdução: As deformidades dentofaciais estão associadas às alterações de oclusões dentárias, que podem causar modificações no sistema miofuncional de acordo com o tipo de desproporção. Essas deformidades, podem provocar alterações e/ou adaptações no sistema estomatognático. O objetivo desse estudo foi identificar, com base na literatura arbitrada, a relação entre a Fonoaudiologia e a Cirurgia Ortognática. Método: Foi realizado levantamento bibliográfico sem período específico, nas bases de dados PubMed, SciELO e BVS. Resultados: Foram avaliados 15 artigos de forma crítica, quanto ao tipo de estudo, seus objetivos, número e gênero dos participantes, metodologia, resultados e conclusões. Os artigos foram agrupados com base nos seus objetivos em 1) Grupo Fonoaudiologia (GF); 2) Grupo Mudanças (GM); 3) Grupo Técnicas (GT) e 4) Grupo Outros (GO). Conclusão: Pouco se encontrou sobre a intervenção fonoaudiológica direta, sendo a maioria dos estudos direcionada às mudanças estéticas e funcionais do tecido duro e mole de pacientes submetidos à cirurgia ortognática. Houve também estudos que abordaram métodos e/ou exames específicos para verificar as mudanças nos tecidos moles e duros do perfil facial dos pacientes submetidos à cirurgia ortognática e estudo que detalhou o perfil dos indivíduos que realizaram tratamento para deformidade dentofacial e síndrome da Apneia e Hipopneia do Sono.

RESUMO Objetivo Descrever o processo de implantação de Protocolo Assistencial para os Distúrbios da Deglutição em unidade hospitalar pública de alta densidade tecnológica no Distrito Federal. Método Trata-se de um estudo descritivo do tipo Relato de Experiência com descrição do processo de implantação de Protocolo Assistencial para os Distúrbios da Deglutição em unidade hospitalar pública de alta densidade tecnológica no Distrito Federal. Resultados A implantação do Protocolo consistiu em três etapas de trabalho: Etapa 1 - busca na literatura sobre os procedimentos que caracterizariam as melhores práticas propedêuticas e terapêuticas; Etapa 2 - discussão com equipe multidisciplinar para ajustes necessários à execução da proposta de implantação de um Protocolo Assistencial para os Distúrbios da Deglutição; e Etapa 3 - proposta final do Protocolo Assistencial para os Distúrbios da Deglutição considerando as particularidades do serviço. Conclusão A proposta final do Protocolo Assistencial para os Distúrbios da Deglutição (PADD) apresenta um método sistematizado pré-definido para acompanhar os pacientes com alteração da deglutição de unidades de saúde de alta densidade tecnológica. O PADD propõe-se a favorecer a detecção precoce do distúrbio, a tomada de decisões terapêuticas uniformes, a utilização de métodos propedêuticos e terapêuticos de baixo custo e a auxiliar gestores no processo de avaliar a qualidade do serviço ofertado através da mensuração de indicadores.

ABSTRACT Purpose To describe the implementation process of a Clinical Practice Protocol for Patients with Swallowing Disorders (CPSD) in a public hospital of high technological density in the Federal District of Brazil. Methods This is an Experience Report study which describes the implementation process of a Clinical Practice Protocol for Patients with Swallowing Disorders in a public hospital of high technological density in the Federal District. Results The protocol was implemented in three stages: Stage 1 - literature search on the procedures that characterize the best propaedeutic and therapeutic practices; Stage 2 - discussion with a multidisciplinary team about the adjustments needed to implement the CPSD; and Stage 3 - Final proposal for the protocol implementation considering the particularities of the service. Conclusion The final proposal of the Clinical Practice Protocol for Patients with Swallowing Disorders (CPSD) provides a predefined, systematic method to monitor patients with swallowing disorders in high technological density health facilities. In addition to being inexpensive, the CPSD fosters decision-making, favors early detection of disorders, and assists managers with evaluating the quality of the service offered.

RESUMO Objetivo caracterizar o perfil e a fala de pacientes submetidos à palatoplastia primária em um hospital escola de São Paulo, levando-se em consideração a idade do paciente no momento da cirurgia (precoce até os 2 anos de idade e tardio após 2 anos). Método 97 indivíduos, de ambos os gêneros, com diagnóstico de fissura de palato associada ou não à de lábio, divididos em dois grupos: 1) grupo precoce (GP), composto por 43 indivíduos operados até o segundo ano de vida; 2) grupo tardio (GT), composto por 54 indivíduos operados após o segundo ano. Os participantes foram submetidos à avaliação clínica fonoaudiológica. Os parâmetros avaliados e considerados para o estudo foram: classificação da ressonância, presença de ronco nasal audível, ocorrência de fraca pressão intraoral, ocorrência de emissão nasal, classificação da inteligibilidade de fala e presença de distúrbios articulatórios compensatórios (DACs). Uma porcentagem randomicamente selecionada de participantes (30%) foi reavaliada por mais duas fonoaudiólogas e a comparação entre os juízes indicou alta concordância. Resultados (nível de significância de 5%): os grupos não se diferenciaram em relação à classificação da ressonância (p=0,067), grau de hipernasalidade (p=0,113), presença de ronco nasal (p=0,179), ocorrência de fraca pressão intraoral (p=0,152), ocorrência de emissão nasal (p=0,369) e classificação da inteligibilidade de fala (p=0,113). Em relação à presença de DACs, os grupos se diferenciaram (p=0,020), com maior ocorrência de fonemas alterados no GT. Conclusão foi possível caracterizar o perfil geral e de fala dos pacientes submetidos à palatoplastia primária do referido hospital escola. Concluiu-se que a realização da cirurgia precocemente traz melhores resultados em relação à fala.

ABSTRACT Purpose To characterize the profile and speech characteristics of patients undergoing primary palatoplasty in a Brazilian university hospital, considering the time of intervention (early, before two years of age; late, after two years of age). Methods Participants were 97 patients of both genders with cleft palate and/or cleft and lip palate, assigned to the Speech-language Pathology Department, who had been submitted to primary palatoplasty and presented no prior history of speech-language therapy. Patients were divided into two groups: early intervention group (EIG) – 43 patients undergoing primary palatoplasty before 2 years of age and late intervention group (LIG) – 54 patients undergoing primary palatoplasty after 2 years of age. All patients underwent speech-language pathology assessment. The following parameters were assessed: resonance classification, presence of nasal turbulence, presence of weak intraoral air pressure, presence of audible nasal air emission, speech understandability, and compensatory articulation disorder (CAD). Results At statistical significance level of 5% (p≤0.05), no significant difference was observed between the groups in the following parameters: resonance classification (p=0.067); level of hypernasality (p=0.113), presence of nasal turbulence (p=0.179); presence of weak intraoral air pressure (p=0.152); presence of nasal air emission (p=0.369), and speech understandability (p=0.113). The groups differed with respect to presence of compensatory articulation disorders (p=0.020), with the LIG presenting higher occurrence of altered phonemes. Conclusion It was possible to assess the general profile and speech characteristics of the study participants. Patients submitted to early primary palatoplasty present better speech profile.

RESUMO Objetivo Verificar e caracterizar a deglutição de pacientes internados, diagnosticados com Síndrome da Imunodeficiência Adquirida (SIDA), com queixas de deglutição. Métodos Análise retrospectiva de 17 prontuários de pacientes com SIDA, atendidos em um hospital escola, entre 2011 e 2012, submetidos a protocolos de avaliação e tratamento da deglutição em beira de leito. Os dados coletados foram referentes à avaliação da deglutição em beira de leito, por meio de protocolos específicos, no momento inicial e na alta fonoaudiológica ou hospitalar. Os pacientes foram divididos em dois grupos: Grupo de alta fonoaudiológica (GAF): pacientes que receberam alta fonoaudiológica antes da alta hospitalar; Grupo de alta hospitalar (GAH): pacientes que receberam alta hospitalar sem terem recebido alta fonoaudiológica. Resultados Os grupos não se diferenciaram em relação à idade e gênero. Quanto à classificação da disfagia: 1) ambos os grupos apresentaram escores significativamente melhores no momento da alta, em relação ao momento inicial (GAH - p=0,024; GAF - p=0,011); 2) os grupos não se diferenciaram no momento inicial (p=0,349); 3) no momento de alta, os grupos apresentaram diferença significativa (p=0,002), com melhores resultados para o GAF. A análise dos sinais clínicos sugestivos de disfagia e de penetração/aspiração apontou que, na comparação intragrupos (ambos os grupos), houve diferença significativa somente para a presença de deglutições múltiplas, com redução do número de deglutições após o tratamento. Conclusão Os grupos obtiveram melhora na escala de classificação da disfagia e na remissão de sinais clínicos sugestivos de disfagia orofaríngea e/ou penetração/aspiração, sendo que o GAF alcançou melhores resultados.

ABSTRACT Purpose To verify and characterize the function of swallowing in Acquired Immunodeficiency Syndrome (AIDS) patients with a complaint of dysphagia. Methods We performed a retrospective analysis of the medical records of 17 AIDS patients treated in a school hospital between 2011 and 2012, who underwent a bedside evaluation and treatment of swallowing. The data were related to the results of the bedside evaluation that followed specific protocols, both at baseline and discharge. Patients were divided into two groups: 1) patients who completed the swallowing intervention during the admission (GCSI); and 2) patients who did not complete the swallowing intervention during the admission (GNCSI). Results The groups did not differ in age and gender. For the classification of swallowing: 1) both groups had significantly better scores at discharge compared to those at baseline (GNCSI: p=0.024; GCSI: p=0.011); 2) the scores did not differ between groups at baseline (p=0.349); 3) at discharge, the scores were different between groups (p=0.002), with better results in the GCSI group. The analysis of the clinical signs and symptoms of oropharyngeal dysphagia or penetration/aspiration indicated that in both groups, there was a statistically significant difference between baseline and discharge only in the presence of multiple swallows (a decrease in the number of swallows after intervention). Conclusion Swallowing ability improved after intervention. Additionally, clinical signs and symptoms suggestive of oropharyngeal dysphagia or penetration/aspiration resolved. The GCSI group showed better results than GNCSI after intervention.

RESUMO INTRODUÇÃO: Aspectos da representação neuroanatômica do funcionamento cortical que controla a deglutição têm sido investigados e identificados em humanos, utilizando-se técnicas de mapeamento cerebral, como a Ressonância Magnética funcional (RNMf). OBJETIVO: Esta revisão crítica qualitativa da literatura levantou publicações científicas sobre a funcionalidade do sistema nervoso central durante tarefas de deglutição, por meio da base de dados PubMed. MÉTODO: O levantamento realizado limitou-se a seres humanos, com idade superior a 18 anos, nos idiomas inglês e português, entre janeiro de 2002 e dezembro de 2013. Publicações sem acesso completo, repetidas por sobreposição das palavras chave, estudos de caso, revisões de literatura, cartas ao editor e não relacionadas diretamente ao tema foram excluídas. RESULTADO: Identificou-se 649 estudos, sendo que somente 21 se enquadraram aos critérios estabelecidos. CONCLUSÃO: Foram encontrados artigos que objetivaram, por meio da RNMf, estudar a função de deglutição em patologias; comparar a deglutição em diferentes idades; descrever a ativação cerebral em diferentes situações de estimulação. Estes estudos apontam múltiplas regiões corticais que participam no controle da deglutição. Conclui-se que a RNMf é um método não invasivo, quantitativo, que demonstra respostas, muitas vezes, não visíveis clinicamente.

ABSTRACT INTRODUCTION: Aspects of the neuroanatomical representation of swallowing have been investigated in humans through brain mapping techniques, such as functional magnetic resonance imaging (fMRI). OBJECTIVE: This critical qualitative review of the literature analyzed international scientific publications in the PubMed database that investigated the activation of the central nervous system in humans during the act of swallowing. METHODS: This investigation was limited to articles that investigated adults older than 18 years, published in English or Portuguese, between January 2002 and December 2013. Publications that did not have access to the full text, that were repeated by overlapping keywords, case studies, literature reviews, letters to the editor, and those not directly related to the topic of the investigation were excluded. RESULTS: A total of 649 articles were identified, of which 21 matched the inclusion criteria. CONCLUSION: The main purpose of the manuscripts that investigate the swallowing process through fMRI were: to characterize swallowing in different pathologies; to compare swallowing in different age groups; to describe brain activation in different stimulation conditions. These studies indicate multiple cortical regions involved in swallowing control. Overall, the studies indicate that fMRI is a non-invasive and quantitative method that allows the investigation of characteristics that are quite often not clinically visible.

Objetivo Caracterizar os aspectos miofuncionais orofaciais de pacientes acometidos pela síndrome de Parry-Romberg, por meio de protocolos clínicos padronizados e da Eletromiografia de Superfície (EMGs) dos músculos mastigatórios. Métodos A amostra foi composta por quatro pacientes com síndrome de Parry-Romberg e quatro indivíduos normais, separados em Grupo Pesquisa (GP) e Grupo Controle (GC), respectivamente, pareados por gênero e idade. Os grupos foram comparados em relação ao desempenho na avaliação clínica fonoaudiológica - Protocolo de Avaliação Miofuncional Orofacial com Escores Expandido (AMIOFE-E), registro das medidas de antropometria facial e de movimentos mandibulares e na avaliação instrumental - Eletromiografia de Superfície (EMGs) dos músculos mastigatórios. Para todas as comparações, foi utilizado o nível de significância de 5%. Resultados Observou-se diferença significativa entre os grupos nas variáveis postura/posição e escore total do AMIOFE-E. Além disso, a análise das variáveis categóricas do AMIOFE-E indicou diferença significativa entre os grupos para palato - altura e largura - e comportamento da língua na deglutição e função mastigatória. A análise das medidas antropométricas indicou diferença significativa entre os grupos somente para e lateralidade mandibular à direita. Não foram observadas diferenças para os dados eletromiográficos. Conclusão O presente estudo é o primeiro a avaliar o Sistema Miofuncional Orofacial de indivíduos acometidos pela síndrome de Parry-Romberg, por meio de protocolos padronizados. Os resultados indicam que esses indivíduos apresentam alterações, principalmente quanto à mobilidade mandibular e mobilidade de língua, na deglutição e na função de mastigação.

Purpose To characterize the orofacial miofunctional aspects of patients with Parry-Romberg syndrome, using standardized clinical protocols and Surface Electromyography (sEMG) of the masticatory muscles. Methods Participants were four patients with Parry-Romberg syndrome and four healthy individuals, paired by age and gender, divided in two groups: Study Group (SG) and Control Group (CG), respectively. The groups were compared regarding performance during clinical examination - Orofacial Myofunctional Evaluation with Expanded Scores (OMES-E) and record of facial anthropometric measures and of jaw movements, and during an instrumental evaluation - Surface Electromyography (sEMG) of the masticatory muscles. Results Statistically significant differences between the groups were observed for the following variables: 1) numerical: mandibular lateral movement to the right, OMES-E posture/position and total score, 2) categorical: palate, behavior of the tongue during swallowing and chewing function. At sEMG no statistically significant differences were observed. Conclusion The present study is the first to evaluate the orofacial myofunctional system of individuals with Parry-Romberg syndrome using standardized protocols. Results indicate that these individuals present alterations specially when considering mandibular movements, tongue mobility during swallowing and chewing function.

ABSTRACT Introduction: This qualitative literature review analyzed international scientific publications on possible orofacial myofunctional alterations in patients with Parry-Romberg syndrome by using PubMed. Methods: The survey was conducted in English, between 2002 and 2012, and was limited to human beings of any age. Publications without full access, duplicated by overlapping keywords, literature reviews, letters to the editor, and those not directly related to the research topic were excluded. Results: We identified 719 studies, of which 21 were within the established criteria. Based on the selected studies, patients with Parry-Romberg syndrome may show changes in soft and hard tissues such as atrophy of the sternocleidomastoid, masseter, and pterygoid muscles; atrophy in the cheek region and depression of the nasolabial fold; deviation of the lips and nose; unilateral tongue atrophy; atrophy of the mouth angle; progressive resorption of the maxilla and mandible bone; atrophy of the zygomatic arch and frontal bone, and facial asymmetry; atrophic root development or pathological resorption of permanent tooth numbers; and jaw reduction and delayed eruption of the upper and lower teeth. Conclusion: Despite the growing interest in the diagnosis and symptomatic description of individuals with Parry-Romberg syndrome, publications that address functional and interdisciplinary treatments are scarce. Therefore, specific studies aimed at improving the quality of life of these patients are needed.

RESUMO Introdução: Esta revisão qualitativa da literatura analisou publicações científicas internacionais sobre possíveis alterações miofuncionais orofaciais em pacientes acometidos pela Síndrome de Parry-Romberg, por meio da base de dados PubMed. Métodos: O levantamento realizado limitou-se a seres humanos, de qualquer faixa etária, no idioma inglês, entre os anos 2002 e 2012. As publicações sem acesso completo, repetidas por sobreposição das palavras-chave, revisões de literatura, cartas ao editor e as não relacionadas diretamente ao tema foram excluídas. Resultados: Foram identificados 719 estudos, sendo 21 dentro dos critérios estabelecidos. Com base nos estudos selecionados, pacientes acometidos pela Síndrome de Parry-Romberg podem apresentar alterações dos tecidos mole e duro, tais como atrofia dos músculos esternocleidomastoideo, masseter e pterigoideos; atrofia na região da bochecha e depressão da prega nasolabial; desvio dos lábios e nariz; atrofia unilateral da língua; atrofia do ângulo da boca; reabsorção progressiva do osso da maxila e da mandíbula; atrofia do arco zigomático, do osso frontal e assimetria facial; desenvolvimento atrófico das raízes ou reabsorção patológica dos números de dentes permanentes; redução da mandíbula e erupção atrasada dos dentes superiores e inferiores. Conclusão: Apesar do crescente interesse pelo diagnóstico e pela descrição sintomatológica de indivíduos com Síndrome de Parry-Romberg, a escassez de publicações que abordem tratamentos funcionais e interdisciplinares é evidente. Verifica-se a necessidade da realização de estudos mais específicos que visem à melhoria da qualidade de vida desses pacientes.

Objetivo Identificar como a função da deglutição tem sido avaliada por meio da ultrassonografia (USG). Estratégia de pesquisa Esta revisão da literatura levantou publicações científicas internacionais sobre a USG e seu uso na avaliação da deglutição, por meio da base de dados PubMed. Foi realizada a localização e seleção dos estudos através de levantamento de textos publicados sobre o assunto, no período de janeiro de 2002 a agosto de 2013, limitando-se a estudos em seres humanos, no idioma inglês. Critérios de seleção Foram excluídos aqueles repetidos por sobreposição das palavras-chave, estudos de caso, revisões de literatura, cartas ao editor e os não relacionados diretamente à temática. Resultados Foram identificados 17 estudos que corresponderam aos critérios de inclusão. Observou-se que mais da metade dos estudos avaliou a deglutição de indivíduos adultos saudáveis, sem preferência por nenhum dos gêneros. Os parâmetros adotados para a análise das imagens não foram unânimes, havendo variação considerável entre os estudos. Conclusão A USG da deglutição demonstrou ser um método rápido, não invasivo, de baixo custo, que fornece parâmetros objetivos sobre a deglutição e que pode ser realizado em beira de leito, uma vez que o equipamento costuma ser de fácil manuseio e transporte.

Purpose To identify how the deglutition function has been evaluated using ultrasound (US). Research strategy This literature review used the PubMed database to survey international scientific publications about US and its use to evaluate deglutition. Studies were located and selected by surveying relevant articles published between January 2002 and August 2013. The survey was limited to studies on humans published in English. Selection criteria Repeated studies (determined by overlapping keywords), case studies, literature reviews, letters to the editor and studies not directly related to the topic were excluded. Results A total of 17 studies matching the inclusion criteria were identified. More than half of the studies evaluated the deglutition of healthy adults with no preference for gender. The parameters adopted for image analysis were not unanimous, and there was considerable variation among studies. Conclusion US proved to be a fast, non-invasive, low-cost method for evaluating objective parameters of deglutition. As a further advantage, US may be performed at bedside because the equipment is typically easy to handle and transport.

OBJECTIVES: This study evaluated the effects of facial stimulation over the superficial muscles of the face in individuals with facial lipoatrophy associated with human immunodeficiency virus (HIV) and with no indication for treatment with polymethyl methacrylate. METHOD: The study sample comprised four adolescents of both genders ranging from 13 to 17 years in age. To participate in the study, the participants had to score six or less points on the Facial Lipoatrophy Index. The facial stimulation program used in our study consisted of 12 weekly 30-minute sessions during which individuals received therapy. The therapy consisted of intra- and extra-oral muscle contraction and stretching maneuvers of the zygomaticus major and minor and the masseter muscles. Pre- and post-treatment results were obtained using anthropometric static measurements of the face and the Facial Lipoatrophy Index. RESULTS: The results suggest that the therapeutic program effectively improved the volume of the buccinators. No significant differences were observed for the measurements of the medial portion of the face, the lateral portion of the face, the volume of the masseter muscle, or Facial Lipoatrophy Index scores. CONCLUSION: The results of our study suggest that facial maneuvers applied to the superficial muscles of the face of adolescents with facial lipoatrophy associated with HIV improved the facial area volume related to the buccinators muscles. We believe that our results will encourage future research with HIV patients, especially for patients who do not have the possibility of receiving an alternative aesthetic treatment.
