Abstract The objective was to understand experiences of double motherhood during antenatal, childbirth and postpartum healthcare, using a qualitative method involving individual online interviews and asynchronous, online focus groups of cisgender women, mostly in same-sex relationships. The results revealed how these women’s experiences of parenting were marginalised, highlighting institutional violence in Brazilian healthcare services, which are presented here in two thematic dimensions: 1) Cisheteronormativity and its impact on experiences of double motherhood; and 2) Institutional violence in healthcare services: from curiosity to LGBTQIA+phobia. It was concluded that cisheteronormativity hinders healthcare for these experiences, especially by rendering the non-gestational mother invisible. This underscores the urgent need to train healthcare personnel, rethink and challenge cisgender and heterosexual norms and promote inclusive policies to ensure equitable care and combat institutional violence.

Resumo Objetivou-se compreender as experiências de dupla maternidade na atenção à saúde durante o pré-natal, parto e puerpério a partir de um método qualitativo, utilizando entrevistas individuais on-line e grupo focal on-line assíncrono com mulheres cisgêneros, a maioria em relacionamentos homoafetivos. Os resultados revelaram a marginalização das vivências parentais dessas mulheres, destacando a violência institucional presente nos serviços de saúde brasileiros, sendo apresentados em dois eixos temáticos: 1) Cisheteronormatividade e seu impacto nas experiências de dupla maternidade e 2) Violência institucional nos serviços de saúde: da curiosidade à LGBTQIA+fobia. Conclui-se que a cisheteronormatividade prejudica a atenção à saúde para essas experiências, especialmente ao invisibilizar a mãe não gestante, destacando a urgência de capacitar profissionais de saúde, repensar e desafiar as normas cisgênero e heterossexuais e promover políticas inclusivas para garantir cuidados equitativos e combater a violência institucional.

Abstract This meta-analysis aimed to investigate the effects of propolis on the severity of coronavirus disease symptoms by reducing periodontal disease. PubMed, EMBASE, SciELO, Web of Science, and SCOPUS databases were systematically searched. Studies have been conducted analyzing propolis’s effects on COVID-19 and periodontitis. The study was conducted according to the PRISMA statement and registered in PROSPERO. Risk of Bias (RoB) assessment and meta-analysis of clinical studies were performed (Review Manager 5, Cochrane). The certainty of the evidence was assessed using GradePro (GDT). Studies have shown propolis flavonoids inhibit viral replication in several DNA and RNA viruses, including coronaviruses. Propolis components have an aminopeptidase inhibitor activity that can inhibit the main proteases of SARS viruses and seem to inhibit protein spikes, which are sites of most mutations in SARS-CoV strains. The meta-analysis showed favorable results with the use of propolis on probing depth (95%CI: 0.92; p < 0.001), clinical attachment level (95%CI: 1.48; p < 0.001), gingival index (95%CI: 0.14; p = 0.03), plaque index (95%CI: 0.11; p = 0.23), and blending on probing (95%CI: 0.39; p < 0.001). The antibacterial activity of propolis could be mediated through its direct action on microorganisms or the stimulation of the immune system, activating natural defenses. Thus, propolis inhibits the replication of SARS-CoV-2 as well as its bacterial activity. Treatment with propolis improves general health and facilitates the activation of the immune system against coronavirus.

Abstract Clinical practice guidelines (CPG) are developed to align standards of health care around the world, aiming to reduce the incidence of misconducts and enabling more effective use of health resources. Considering the complexity, cost, and time involved in formulating CPG, strategies should be used to facilitate and guide authors through each step of this process. The main objective of this document is to present a methodological guide prepared by the Epidemiology Committee of the Brazilian Society of Rheumatology for the elaboration of CPG in rheumatology. Through an extensive review of the literature, this study compiles the main practical recommendations regarding the following steps of CPG drafting: distribution of working groups, development of the research question, search, identification and selection of relevant studies, evidence synthesis and quality assessment of the body of evidence, the Delphi methodology for consensus achievement, presentation and dissemination ofthe recommendations, CPG quality assessment and updating. This methodological guide serves as an important tool for rheumatologists to develop reliable and high-quality CPG, standardizing clinical practices worldwide. Resumo O artigo traça um panorama da produção recente voltada para a historicização dos sons e das escutas, presente nos textos que compõem o dossiê História e Cultura Sonora. De modo geral, é possível indicar que três grandes correntes predominam nesses trabalhos: os Sound Studies anglófonos, a História das Sensibilidades de tradição francesa e os estudos latino-americanos que, embora inspirados nos dois anteriores, procuram não se limitar às discussões clássicas para refletir sobre as especificidades da produção sonora e dos modos de escuta de suas culturas. Abstract The article provides an overview of recent production focused on the historicization of sounds and listening, present in the texts that compose the dossier História e Cultura Sonora. In general, it is possible to indicate that three major currents prevail in these works: the anglophone Sound Studies, the History of Sensibilities of French tradition and Latin American studies, which, although inspired by the previous two, strive not to confine themselves to classical discussions but to reflect on the specificities of sound production and modes of listening within their cultures. Resumen El accidente cerebrovascular (ACV) constituye la principal causa de discapacidad de origen neuro- lógico en los adultos mayores a 40 años y la cuarta causa de muerte en Argentina. En los últimos diez años las publicaciones indexadas relacionadas al tratamiento del ACV isquémico fueron más numerosas que las de ACV hemorrágico. El objetivo de este material es proporcionar recomendaciones locales y actualiza- das del abordaje de pacientes con hematoma intraparenquimatoso espontáneo durante la internación. Para la redacción de este manuscrito se convocó a especialistas en esta enfermedad que conformaron grupos de trabajo. Se plantearon 10 tópicos centrales expresados como epidemiologia, atención inicial, imágenes, tratamiento de la presión arterial, reversión de antitrombóticos, indicación de cirugía, profilaxis anticonvulsivante, pronóstico, prevención de complicaciones y reinicio de antitrombóticos. De cada tópico se plantearon mediante preguntas PICO los interrogantes más frecuentes de la práctica diaria. Luego de una revisión sistemática de la literatura, se generaron recomendaciones evaluadas mediante sistema GRADE y consensuadas entre autores y pacientes.

Abstract Stroke is the leading cause of neurological disability in people over 40 years of age and the fourth leading cause of death in Argentina. In the last ten years, the indexed publications related to the treatment of ischemic stroke were more numerous than those of hemorrhagic stroke. The objective of this material is to provide local and updated recommendations for the management of patients with spontaneous intracere- bral hemorrhage during hospitalization. For the writing of this manuscript, diferent specialists were convened to form working groups. Ten central topics expressed as epidemiology, initial care, imaging, blood pressure treatment, reversal of antithrombotics, indication for surgery, seizure prophylaxis, prognosis, prevention of complications and resumption of antithrombotics were raised. For each topic, the most frequent questions of daily practice were raised through PICO questions. After a systematic review of the literature, recommendations were generated, evaluated using the GRADE system and agreed between authors and patients.

ABSTRACT Introduction. Different environmental hazards associated with water converge in Santiago de Cuba, which constitutes a permanent threat to human life and to the socioeconomic development of the province (heavy rains, floods, tropical cyclones, among others). Its study in terms of disaster reduction is a priority for the Cuban State. Objective: Doing a group of research and development actions in order to assess the hazard, vulnerability and risk caused by water in nature, and in water supply systems of Santiago de Cuba province, in the face of the impact of environmental phenomena. Methods. Investigations of a multidisciplinary group structured in three parts are integrated: Studies of hazard, vulnerability and risks (HVR) in water systems of Santiago de Cuba; studies of HVR in watersheds and vulnerability behavior due to the impact of capillarity humidity in buildings from the historical center of Santiago de Cuba city. Results. Tools and solutions to strengthen forecasting and early warning systems for the population and institutions involved. Design of a new procedure for risk management based on the theory of processes, vulnerability studies in water supply systems, and water as a phenomenon of capillarity humidity in buildings. Watersheds diagnostics and flood modeling; the monthly rainfall series for the province was updated, as well as the design of professional software for early warning of extreme hydrometeorological events and community actions to improve risk culture.

RESUMEN Introducción. En Santiago de Cuba convergen distintos peligros ambientales asociados al agua que constituyen una amenaza permanente a la vida humana y al desarrollo socioeconómico de la provincia (intensas lluvias, inundaciones, ciclones tropicales, entre otros.). Su estudio en función de la reducción de desastres constituye una prioridad para el Estado cubano. Con el objetivo de realizar un conjunto de acciones de investigación y desarrollo encaminadas a determinar el peligro, la vulnerabilidad y el riesgo que causa el agua tanto en la naturaleza, como en las obras de infraestructura de la provincia Santiago de Cuba, ante el posible impacto de distintos fenómenos ambientales. Métodos. Se integran investigaciones de un grupo multidisciplinario estructurado en 3 partes: Estudios de peligro, vulnerabilidad y riesgos (PVR) en obras hidráulicas de Santiago de Cuba; estudios de PVR en cuencas hidrográficas de Santiago de Cuba y comportamiento espacial de la vulnerabilidad por el impacto de la humedad por capilaridad en edificaciones del centro histórico de la ciudad de Santiago de Cuba. Resultados. Herramientas y soluciones para fortalecer los sistemas de pronóstico y alerta temprana a la población e instituciones involucradas. Diseño de un nuevo procedimiento para la gestión de riesgos basado en la teoría de procesos, estudios de vulnerabilidad en obras hidráulicas y el agua como fenómeno de humedad capilar en las construcciones. Diagnósticos en cuencas hidrográficas y modelaciones de inundaciones; se actualizó la serie de precipitaciones mensuales de la provincia, así como diseño de software profesionales para la alerta temprana ante el impacto de eventos hidrometeorológicos extremos y acciones comunitarias para mejorar la cultura de riesgos.

Resumo Em junho de 2013, emergiram no espaço público brasileiro diferentes grupos de direita rebelados contra o Partido dos Trabalhadores, que governava o Brasil naquele momento liderando uma grande coalizão de centro-esquerda. Desde então, uma nova direita se posiciona no debate político nacional, militando nas universidades e em outros contextos institucionais e mobilizando uma retórica agressiva contra o discurso de esquerda. O objetivo deste estudo é identificar os diferentes atributos da identidade da direita formulados em narrativas de militantes de direita de uma universidade pública brasileira, as contradições no conteúdo dessa identidade e os recursos retórico-discursivos mobilizados em sua construção. Com o uso do método de análise de discurso, foram analisadas transcrições de entrevistas realizadas com nove militantes de direita, alunos de uma universidade pública nordestina. Esses militantes mencionam três grupos políticos como constituintes da direita (os conservadores, os liberais e os libertários), mas somente dois são usados como categorias de autoidentificação: os conservadores e os libertários. Atributos identitários como o apreço pela verdade, a defesa da liberdade, a desconfiança em relação às mudanças abruptas e a valorização da família e da religiosidade cristã foram afirmados como definidores da identidade da direita. Em vários momentos, no entanto, os militantes produziam enunciados que contradiziam alguns desses atributos, revelando o caráter ambíguo e contraditório do discurso da militância estudada.

Abstract In June 2013, different right-wing groups emerged in the Brazilian public space rebelling against the Partido dos Trabalhadores, which ruled Brazil at the time leading a large center-left coalition. Since then, a new right-wing has positioned itself in the national political debate, militating in universities and other institutional contexts, and mobilizing an aggressive rhetoric against the leftist discourse. This study aims to identify the different attributes of the right-wing identity formulated in the narratives of right-wing militants in a Brazilian public university, the contradictions in the content of that identity, and the rhetorical-discursive resources mobilized in its construction. We analyzed transcripts of interviews conducted with nine right-wing activists, students from a public university in the Northeast using the speech analysis method. These militants mention three political groups as constituents of the right-wing (conservatives, liberals, and libertarians), but only two are used as categories of self-identification: conservatives and libertarians. Identity attributes such as an appreciation for the truth, the defense of freedom, a distrust of abrupt changes, and the appreciation for the family and Christian religiosity were affirmed as defining the identity of the right-wing. At various times, however, the militants produced statements that contradicted some of these attributes, revealing the ambiguous and contradictory character of the studied militancy discourse.

Resumen En junio de 2013, diversos grupos de derecha rebelaban contra el Partido de los Trabajadores, que gobernaba Brasil en ese momento liderando una gran coalición de centro-izquierda. Desde entonces, una nueva derecha se ha posicionado en el debate político nacional, militante en universidades y otros contextos institucionales, y movilizando una retórica agresiva contra el discurso de izquierda. El objetivo de este estudio es identificar los diferentes atributos de la identidad de derecha formulados en las narrativas de activistas de derecha en una universidad pública brasileña, sus contradicciones y los recursos retórico-discursivos movilizados. Como método el análisis del discurso, se analizaron las transcripciones de entrevistas realizadas a nueve activistas de derecha, estudiantes de una universidad pública del Nordeste (Brasil). Estos militantes mencionaron tres grupos políticos como constituyentes de la derecha (los conservadores, los liberales y los libertarios), pero solo dos utilizan las categorías de autoidentificación: los conservadores y los libertarios. Los atributos identitarios como el aprecio a la verdad, la defensa de la libertad, la desconfianza ante los cambios bruscos y el aprecio a la familia y la religiosidad cristiana fueron considerados definitorios de la identidad de derecha. En varias ocasiones, los militantes contradijeron algunos de estos atributos, revelando el carácter ambiguo y contradictorio del discurso de la militancia estudiado.

RESUMO: A mastite ocupa lugar de destaque entre as doenças que acometem o rebanho leiteiro, em virtude de problemas econômicos e de saúde pública. Staphylococcus spp. são os agentes infecciosos mais envolvidos na etiologia das mastites caprinas, principalmente Staphylococcus coagulase negativo. Dezenove isolados de Staphylococcus spp. foram obtidos a partir de mastite caprina subclínica. Todos os isolados foram caracterizados por MALDI-TOF MS, sendo 47,36% (9/19) identificadas como S. epidermidis, 15,78%(3/19) como S. warneri, 10,52% (2/19) como S. caprae e S. aureus e 5,26% (1/19) tanto para S. lugdunensis, como para S. simulans e S. cohnii. Todos os isolados caracterizados pelo MALDI-TOF foram submetidos a reação em cadeia da polimerase (PCR) para o gene 16rRNA de Staphylococcus spp. para a confirmação do gênero. Após a determinação da espécie, foram realizadas as provas para a detecção fenotípica de resistência aos beta-lactâmicos: difusão em disco simples de oxacilina, cefoxitina, penicilina G e amoxacilina +ácido clavulânico, ágar “screen” de oxacilina e microdiluição em caldo (MIC) de cefoxitina. O teste de difusão em disco demonstrou resistência de 58% (11/19) para penicilina G, 26,31% (5/19) para cefoxitina e 26,31% (5/19) para oxacilina. Todas as amostras foram sensíveis a amoxicilina + ácido clavulânico e ao ágar “screen” de oxacilina. Pelo MIC, 63,15% (12/19) das amostras foram resistentes a cefoxitina (MIC >4,0μg/ml). Em seguida os isolados foram submetidos a detecção dos genes de resistência mecA e seus reguladores (mecl e mecRI), mecC e blaZ. Duas amostras de S. epidermidis apresentaram o gene mecA. Todos os isolados foram negativos para a variante do gene mecA, mecl, mecRI, mecC e blaZ. Tais achados reforçam a importância deste grupo de microrganismos na etiologia da mastite subclínica em caprinos e abre perspectivas para futuras pesquisas para a investigação da epidemiologia da doença.

ABSTRACT: Mastitis occupies a prominent place among the diseases that affect dairy herds due to economic problems and public health. Staphylococcus spp. are infectious agents more involved in the etiology of caprine mastites, especially coagulase-negative Staphylococcus. Nineteen isolates of Staphylococcus spp. were obtained from subclinical caprine mastitis. All isolates were characterized by MALDI-TOF MS, being 47.36% (9/19) identified for S. epidermidis, 15.78% (3/19) for S. warneri, 10.52% (2/19) for S. aureus and S. caprae and 5.26% (1/19) for S. lugdunensis, S. simulans, and S. cohnii. All isolates characterized by MALDI-TOF were subjected a to polymerase chain reaction (PCR) for the 16S rRNA gene of Staphylococcus spp. to confirm the gender. After determining the species, tests for phenotypic detection of resistance to beta-lactams were carried out simple disk diffusion oxacillin, cefoxitin, penicillin G and amoxicillin + clavulanic acid, agar “screen” oxacillin and microdilution (MIC) cefoxitin. The disk diffusion test showed a strength of 58% (11/19) for penicillin G, 26.31% (5/19) for cefoxitin and 26.31% (5/19) for oxacillin. All strains were susceptible to amoxicillin + clavulanic acid and agar “screen” oxacillin. In the MIC, 63.15% (12/19) of the samples were cefoxitin resistant (MIC >4.0μg/ml). Then isolates were subjected to detection of the mecA resistance genes and regulators (mecl and mecRI), mecC and blaZ. Two samples of Staphylococcus epidermidis had the mecA gene. All isolates were negative for the mecA gene variant, mecl, mecRI, mecC and blaZ. These findings reinforce the importance of this group of microorganisms in the etiology of subclinical mastitis in goats and open perspectives for future research to investigate the epidemiology of the disease.

RESUMO: Embora raros, os micoplasmas estão incluídos entre as causas de doenças respiratórias em répteis e, na ordem Squamata, já foram realizados três relatos destes microrganismos causando doença em pítons. Este estudo teve como objetivo avaliar a ocorrência de espécies de Mycoplasma em serpentes em cativeiro. Foram avaliadas 26 serpentes das famílias Pythonidae (13), Boidae (7), Viperidae (5) e Colubridae (1) do RioZoo, Brasil. Os animais foram examinados para determinar sinais clínicos consistentes com qualquer doença infecciosa. Amostras de swab traqueal de cobras foram coletadas em meio Frey e analisadas por isolamento microbiológico e pela técnica da PCR para identificar Mycoplasma spp. As amostras positivas para o gênero Mycoplasma spp. foram submetidas ao sequenciamento genético para identificação das espécies. No isolamento, 19,23% (5/26) foram positivos, enquanto 65,38% (17/26) dos animais foram positivos por PCR. Com base nas análises das seis sequências obtidas, houve similaridade com o Mycoplasma spp. descrito anteriormente em um píton e M. agassizii e M. testudineum encontrados em quelônios. Este é o primeiro relato de Mycoplasma spp. em animais das famílias Boidae e Viperidae. Mycoplasma spp. foi detectado em serpentes com e sem sinais clínicos. Os micoplasmas encontrados apresentaram semelhança genética com outros já descritos em répteis. Não houve relação entre a presença de Mycoplasma spp. e sinais clínicos.

ABSTRACT: Although rare, mycoplasmas are included among the causes of respiratory diseases in reptiles and, in the order Squamata, three reports of these microorganisms causing diseases in pythons have already been reported. This study aimed to evaluate the occurrence of Mycoplasma species in captive snakes. A total of 26 snakes of the families Pythonidae (13), Boidae (7), Viperidae (5) and Colubridae (1) from RioZoo, Brazil, were evaluated. Animals were examined to determine clinical signs consistent with any infectious disease. Tracheal swab samples from snakes were collected in Frey medium and analyzed for the presence of Mycoplasma spp.by isolation and a genus-specific PCR. DNA sequencing analyses of six positive samples by PCR were carried out to identify the species. Using isolation 19.23% (5/26) was positive, while 65.38% (17/26) of the animals were positive by PCR. Based on the analyses of the six sequences obtained, there was similarity with a Mycoplasma spp. previously described in a phyton and, M. agassizii and M. testudineum reported in chelonians. This is the first report of Mycoplasma spp. in animals of the families Boidae and Viperidae. Mycoplasma spp. were detected in snakes with and without clinical signs. The mycoplasmas reported resented identity (range, 95% to 100%) to others already described in reptiles. There was no relationship between the presence of Mycoplasma spp. and clinical signs.

Resumo: O objetivo deste trabalho foi avaliar a variabilidade genética de Araucaria cunninghamii, em teste de procedência, e estimar ganhos genéticos para a produção de sua madeira, com base em diferentes intensidades de seleção. O experimento foi estabelecido em Luís Antônio, no estado de São Paulo, Brasil, com sementes de polinização aberta coletadas em Queensland, Austrália. O delineamento experimental utilizado foi o de blocos ao acaso, com seis repetições e quatro plantas por parcela. O diâmetro à altura do peito (DBH), a forma do fuste e a sobrevivência foram medidos 29 anos após o plantio. Diferenças significativas foram detectadas entre as procedências. O coeficiente de variação genotípica foi alto (60%) para DBH. Os valores médios de DBH e forma do fuste foram de 23,21 cm e 4,50, respectivamente. As procedências Langkelly Creek, Brooweena L.A., Yarraman e St. Agnes apresentaram melhor desempenho em DBH, e Imbil Seed Orchard, Brooweena L.A., St. Agnes, St. John e Yarraman, em forma do fuste. Os ganhos estimados com a seleção variaram de 0,5 a 24,8%, para DBH, e de 0,9 a 3,5%, para forma do fuste.

Abstract: The objective of this work was to assess the genetic variability of Araucaria cunninghamii in a provenance test and to estimate the genetic gains in its wood production, based on different selection intensities. The trial was established in Luís Antônio, in the state of São Paulo, Brazil, using open-pollinated seed from Queensland, Australia. The experimental design used was a randomized complete block with six replicates, and four plants per plot. Diameter at breast height (DBH), stem form, and survival were measured at 29 years after planting. Significant differences were detected among provenances. The coefficient of genotypic variation between provenances was high (60%) for DBH and stem form. The mean values for DBH and stem form were 23.21 cm and 4.50, respectively. The provenances Langkelly Creek, Brooweena L.A., Yarraman, and St. Agnes showed the best performance for DBH, and Imbil Seed Orchard, Brooweena L.A, St. Agnes, St. John, and Yarraman, for stem form. Genetic gains ranged from 0.5 to 24.8% for DBH, and from 0.9 to 3.5% for stem form.

RESUMO Objetivo analisar a dimensão prática das representações sociais de profissionais de saúde sobre as doenças negligenciadas. Método estudo qualitativo, apoiado na Teoria das Representações Sociais, desenvolvido entre março e outubro de 2018, em unidades de atenção primária e secundária de um município do interior da Bahia, Brasil. Os dados foram coletados por meio da entrevista em profundidade e submetidos à análise lexical utilizando o software IRAMUTEQ. Resultados as representações sociais foram constituídas por uma dimensão prática que descreveu o processo assistencial dos profissionais de saúde com suas dificuldades e potencialidades no enfrentamento das doenças negligenciadas. Conclusão a dimensão prática das representações sociais apontou a necessidade de políticas públicas direcionadas as doenças negligenciadas, práticas de cuidado na educação permanente em saúde e formação profissional com integralidade do cuidado.

ABSTRACT Objective to analyze the practical dimension of health professionals’ social representations about neglected diseases. Method a qualitative study, supported by the Theory of Social Representations, developed between March and October 2018 in primary and secondary care units in the inland of Bahia, Brazil. The data were collected by means of in-depth interviews and submitted to lexical analysis using the IRAMUTEQ software. Results the social representations consisted of a practical dimension that described the care process of health professionals with their difficulties and potentialities when facing neglected diseases. Conclusion the practical dimension of the social representation indicated the need for public policies directed to neglected diseases, care practices in permanent education in health and professional training with comprehensive care.

RESUMEN Objetivo analizar la dimensión práctica de las representaciones sociales de los profesionales de la salud sobre las enfermedades desatendidas. Método estudio cualitativo, con base en la Teoría de las Representaciones Sociales, desarrollado entre marzo y octubre de 2018 en unidades de atención primaria y secundaria de un municipio del interior del estado de Bahía, Brasil. Los dados se recopilaron por medio de entrevistas en profundidad y se los sometió a análisis lexical utilizando el programa de software IRAMUTEQ. Resultados las representaciones sociales se constituyeron por una dimensión práctica que describió el proceso asistencial de los profesionales de la salud con sus dificultades y potencialidades en el afrontamiento de enfermedades desatendidas. Conclusión la dimensión práctica de las representaciones sociales señalo la necesidad de implementar políticas públicas direccionadas a las enfermedades desatendidas, a prácticas de atención en educación permanente en salud, y la capacitación profesional con integralidad de la atención.

Abstract This study was undertaken to investígate the resistance phenotypes to macrolide-lincosamide-streptogramin B (MLSb) antibiotics and their associated genotypes in isolates of Staphylococcus aureus. We analyzed one hundred, consecutive, non-duplicate isolates (methicillin-susceptible MSSA, n = 53 and methicillin-resistant MRSA, n =47) obtained from var-ious clinical samples between July 2012 to December 2013. The resistance profile to MLSb antibiotics was determined by phenotypic methods and the resistance genes were detected by PCR assays. All of the isolates were subjected to pulsed-field gel electrophoresis (SmaI-PFGE). The overall prevalence of resistance to MLSb antibiotics was 38% and the resistance phenotype distribution was as follows: cMLSb, 22%; iMLSB, 10%; MSb, 5% and L, 1%. We detected ermA, ermC, ermB and mrsA/B genes in these resistant isolates. The single ermA gene was commonly observed mainly in those with a cMLSb R phenotype, whereas the combination ermA and ermC was more commonly observed in isolates with inducible expression. The patterns of SmaI-PFGE suggest a great genetic diversity in both MRSA and MSSA resistant to MLSb antibiotics. The results demonstrate the local presence of S. aureus resistant to MLSb antibiotics and its most frequently described responsible genes. Some of these isolates, especially those with the iMLSB phenotype, may be associated with therapeutic failure. Therefore, efforts should be directed to the correct detection of all MLSb resistant isolates using appropriate laboratory tests. PFGE results reveal a wide spread of resistance genes rather than the circulation of S. aureus clones resistant to MLSb antibiotics.

Resumen Los objetivos de este estudio fueron investigar en Staphylococcus aureus la presencia de fenotipos resistentes a los antibióticos macrólidos, lincosamidas y estreptograminas tipoB (MLSb) y conocer sus genotipos responsables. Analizamos 100 aislamientos consecutivos, no duplicados (53 sensibles a meticilina [MSSA] y 47 resistentes a meticilina [MRSA]), obtenidos entre 2012 y 2013 a partir de diferentes muestras clínicas. El perfil de resistencia a los antibióticos MLSb fue determinado por métodos fenotípicos y los genes de resistencia se detectaron por PCR. Todos los aislamientos fueron comparados por SmaI-PFGE. La prevalencia global de resistencia a los antibióticos MLSB fue del 38% y la distribución de los fenotipos de resistencia fue la siguiente: cMLSB, 22%; iMLSB, 10%; MSB, 5%; L, 1%. Se detectaron los genes ermA, ermC y mrsA/B en los aislamientos resistentes. El gen ermA se observó, sobre todo, en aislamientos con fenotipo resistente constitutivo R (cMLSB), mientras que la combinación ermA y ermC se detectó principalmente en aislamientos con resistencia inducible (iMLSB). Los patrones de Smal-PFGE sugieren una gran diversidad genética en los aislamientos resistentes a los antibióticos MLSb, tanto MRSA como MSSA. Los resultados demuestran la presencia local de S. aureus resistentes a los antibióticos MLSB y de sus genes responsables más frecuentemente descritos. Estos cultivos, especialmente aquellos con fenotipo resistente iMLSB, pueden asociarse con fallas terapéuticas. Por lo tanto, los esfuerzos deben dirigirse a la correcta detección de todos los cultivos resistentes a MLSB utilizando pruebas de laboratorio adecuadas. Los resultados de Smal-PFGE sugieren una amplia diseminación de genes de resistencia, más que la circulación de clones resistentes a los antibióticos MLSB.

RESUMO: Campylobacter spp. é um agente bacteriano causador de gastroenterite em humanos e associado à síndrome de Guillain-Barré, sendo a campilobacteriose considerada uma das principais enfermidades de origem alimentar. Aves e suínos são importantes reservatórios desses microrganismos e seus produtos derivados crus ou mal cozidos são muitas vezes incriminados como fonte de infecção humana. A primeira escolha para o tratamento em casos humanos são os antimicrobianos da classe dos macrolídeos como à eritromicina. Dentro desse contexto, o objetivo deste estudo foi isolar Campylobacter jejuni e C. coli a partir de 20 amostras de conteúdo intestinal de frangos de corte e de 30 de suínos ao abate e investigar a resistência à eritromicina das estirpes obtidas e os possíveis mecanismos moleculares envolvidos nesta resistência. A concentração inibitória mínima foi determinada pela diluição em ágar e a técnica MAMA-PCR foi utilizada para detecção de mutações nas posições 2074 e 2075 da região 23s rRNA, foi pesquisado também a presença do gene erm(B) pela PCR. A partir do conteúdo intestinal de frangos de corte foram isoladas 18 estirpes de C. jejuni e duas de C. coli, enquanto de suínos foram obtidas 14 estirpes de C. coli e nenhuma estirpe de C. jejuni. Todas as estirpes de C. coli de suínos foram identificadas como resistentes e três estirpes de C. jejuni de frangos foram caracterizadas com resistência intermediária. A CIM das estirpes variou de ≤0,5mg/μL a ≥128mg/μL. Todas as estirpes resistentes tinham a mutação A2075G e uma cepa com resistência intermediária também apresentou a mutação A2075G. Não foi detectada a mutação A2074C ou a presença do gene erm(B) em nenhuma das estirpes obtidas. Os resultados revelam um alto nível de resistência em estirpes de C. coli isoladas de suínos frente a eritromicina. A técnica MAMA PCR utilizada se constitui em uma ferramenta prática para detecção da resistência à eritromicina em estirpes de C. jejuni e C. coli.

ABSTRACT: Campylobacter spp. is a bacterial agent that causes gastroenteritis in humans and may trigger Guillain-Barré Syndrome (GBS) and is also considered one of the main foodborne diseases in developed countries. Poultry and pigs are considered reservoirs of these microorganisms, as well as raw or undercooked by-products are often incriminated as a source of human infection. Treatment in human cases is with macrolide, such erythromycin, that inhibits the protein synthesis of the microorganism. This study aimed to isolate Campylobacter jejuni and Campylobacter coli from intestinal content samples of broiler chickens (n=20) and swine (n=30) to characterize the erythromycin resistance profile of the strains and to detect molecular mechanisms involved in this resistance. The minimum inhibitory concentration was determined by agar dilution. The Mismatch Amplification Mutation Assay-Polymerase Chain Reaction (MAMA-PCR) was performed to detect mutations at positions 2074 and 2075 of 23S rRNA region, in addition to PCR test to detect the erm(B) gene. From the intestinal content of broiler chickens, 18 strains of C. jejuni and two strains of C. coli were isolated, whereas, from swine samples, no C. jejuni strain and 14 strains of C. coli were isolated. All C. coli strains were resistant, and three C. jejuni strains from broilers chickens were characterized with intermediate resistance to erythromycin. The MIC of the strains ranged from ≤0.5mg/μL to ≥128mg/μL. All resistant strains had the A2075G mutation, and one strain with intermediate resistance had the A2075G mutation. However, the A2074C mutation and the erm(B) gene were not detected. High resistance levels were detected in C. coli strains isolated from swine. The MAMA-PCR is a practical tool for detecting the erythromycin resistance in Campylobacter strains.

Abstract Objective: The preparation of parents of children who should undergo cardiac surgery requires special treatment such as the explanations about the event. This study aims to compare the effects of standardized nursing guidelines with routine institutional orientation on the anxiety of parents of children undergoing cardiac surgery. Methods: Randomized clinical trial. The sample consisted of parents of children who underwent cardiac surgery from December 2010 to April 2011. Twenty-two parents were randomized to the intervention group (IG) and received the standard nursing guidelines and 22 participated in the control group (CG) and received the routine guidelines from the institution. Anxiety was assessed by the State-Trait Anxiety Inventory (STAI) applied in the preoperative period, between 12 and 20 hours before surgery and before receiving standard or institutional guidelines and 48 hours after surgery. The analysis of variance (ANOVA) for repeated measures was performed to evaluate the differences between the variations in STAI scores between the groups during the studied period. The level of significance was 0.05. Results: There were no significant differences in baseline anxiety scores between groups with regard to trait anxiety as well as state anxiety: STAI-trait (CG 42.6±4.9 vs. IG 41.4±6.0, P=0.48); STAI-state (CG 42.3±5.7 vs. IG 45.6±8.3, P=0.18). Likewise, the variation in score after 48 hours was similar between groups (STAI-trait P=0.77; STAI-state P=0.61). Conclusion: There were no significant differences in the parents’ anxiety levels when comparing the two types of guidelines: the standard nursing and the institutional orientation.
