Abstract To date, hydroxyurea is the only effective and safe drug that significantly reduces morbidity and mortality of individuals with Sickle cell disease. Twenty years of real-life experience has demonstrated that hydroxyurea reduces pain attacks, vaso-occlusive events, including acute chest syndrome, the number and duration of hospitalizations and the need for transfusion. The therapeutic success of hydroxyurea is directly linked to access to the drug, the dose used and adherence to treatment which, in part, is correlated to the availability of hydroxyurea. This consensus aims to reduce the number of mandatory exams needed to access the drug, prioritizing the requesting physician's report, without affecting patient safety.

RESUMO Objetivo. Descrever a implementação e a atuação pioneira do comitê de farmacovigilância em vacinas do estado de São Paulo, Brasil, na análise de eventos supostamente atribuíveis à vacinação ou imunização (ESAVI) no período da pandemia de covid-19. Método. Trata-se de um estudo de caso retrospectivo, qualiquantitativo, com enfoque na atuação do comitê de farmacovigilância nos anos de 2021 e 2022. A partir dos registros das reuniões do comitê, foi descrito o modo de funcionamento do comitê e contabilizados os casos de ESAVI discutidos e resolvidos nas reuniões de especialistas. O número de doses de vacinas aplicadas no período de interesse, assim como o número de ESAVI notificados nesse período, foi obtido de sistemas de informação estaduais. Resultados. Em 55 encontros realizados em 2021 e 2022, foram discutidos 118 casos de ESAVI nas reuniões do comitê de farmacovigilância, majoritariamente relacionados às vacinas de covid-19. Contabilizou-se a aplicação de 126 778 252 doses de vacinas de covid-19 nesse período, tendo havido 42 893 notificações de ESAVI. Entre os casos elencados para discussão, 71,2% ocorreram após a primeira dose da vacina, 32,8% representaram eventos neurológicos e apenas 5% dos casos apresentaram associação causal com os imunizantes. Conclusão. O comitê de farmacovigilância teve um papel importante no sistema de vigilância de ESAVI no período analisado, auxiliando na avaliação de casos de maior complexidade, com potencial para contribuir positivamente no fortalecimento do programa de imunização, em especial no âmbito do sistema de vacinação segura.

RESUMEN Objetivo. Describir la puesta en marcha del comité de farmacovigilancia de vacunas del estado de São Paulo (Brasil) y su actuación pionera en el análisis de eventos supuestamente atribuibles a la vacunación o inmunización (ESAVI) durante la pandemia de COVID-19. Método. Se trata de un estudio de caso, retrospectivo, cualitativo y cuantitativo, centrado en la actuación del comité de farmacovigilancia en el período 2021-2022. A partir de las actas de las reuniones del comité, se describió su funcionamiento y se contabilizaron los casos de ESAVI examinados y sobre los que se llegó a una conclusión en las reuniones de expertos. El número de dosis de vacunas administradas en el periodo de interés y el número de ESAVI notificados en ese periodo se obtuvieron de los sistemas de información estatales. Resultados. En 55 reuniones celebradas en el período 2021-2022, se analizaron 118 casos de ESAVI en las reuniones del comité de farmacovigilancia, la mayoría de ellos relacionados con las vacunas contra la COVID-19. Durante este periodo se administró un total de 126 778 252 dosis de vacunas contra la COVID-19, y hubo 42 893 notificaciones de ESAVI. De los casos seleccionados para el examen por el comité, el 71,2% ocurrieron después de administrar la primera dosis de la vacuna, el 32,8% fueron eventos neurológicos y solo el 5% presentaron una asociación causal con los productos vacunales. Conclusiones. El comité de farmacovigilancia desempeñó un papel importante en el sistema de vigilancia de ESAVI en el periodo analizado y ayudó a evaluar los casos más complejos, y mostró su potencial de contribuir positivamente a fortalecer el programa de inmunización, sobre todo en el contexto del sistema de vacunación segura.

ABSTRACT Objective. To describe the implementation and pioneering work of a vaccine pharmacovigilance committee in the state of São Paulo, Brazil, for the analysis of events supposedly attributable to vaccination or immunization (ESAVI) during the COVID-19 pandemic. Methods. This is a retrospective, mixed-methods (qualitative and quantitative) case study focusing on the work carried out by the pharmacovigilance committee in the years 2021 and 2022. The minutes of committee meetings were used to describe how the committee operates. ESAVI cases discussed and addressed at expert meetings were recorded. The number of vaccine doses applied during the period of analysis, as well as the number of ESAVIs reported within that period, was obtained from statewide information systems. Results. Over 55 pharmacovigilance committee meetings held in 2021–2022, 118 ESAVI cases—most related to COVID-19 vaccines—were discussed. A total of 126,778,252 doses of COVID-19 vaccines were administered during this period, with 42,893 ESAVI reported. Among the cases selected for discussion, 71.2% occurred after the first dose of vaccine, 32.8% represented neurological events, and only 5% were deemed to have a causal relationship with the vaccine. Conclusions. The pharmacovigilance committee played a significant role in the ESAVI surveillance system during the period of analysis by assisting in the evaluation of more complex cases. It shows potential to contribute positively to strengthening the immunization program, especially within the safe vaccination system framework.

ABSTRACT OBJECTIVE To evaluate the evolution of the dietary patterns of adolescents in the northeast region of Brazil. METHODS Secondary analysis of data from the Pesquisa de Orçamentos Familiares (POF – Household Budget Surveys), collected by the Brazilian Institute of Geography and Statistics (IBGE) in the years 2008–2009 and 2017–2018. A total of 3,095 adolescents were evaluated in 2008–2009 and 3,015 in 2017–2018. Food consumption was assessed using two dietary records in 2008–2009 and two 24-hour recalls in 2017–2018, applied on non-consecutive days. Based on these data, principal components factor analysis (PCFA) was performed, followed by orthogonal rotation of the varimax type, to derive dietary patterns, stratified by sex. The results were described as means or percentage frequencies, with their respective 95% confidence intervals. RESULTS Three main dietary patterns were identified among adolescents from the northeast region of Brazil. Among boys, in 2008–2009, the patterns were called snacks, traditional Brazilian, and coffee; and in 2017–2018, traditional Brazilian, snacks, and mixed, in this order of representativeness of the group’s eating habits. Among female adolescents, in 2008–2009, the patterns were snacks, traditional Brazilian, and coffee; and in 2017–2018, traditional Brazilian, snacks, and processed meats. CONCLUSION The dietary patterns identified in 2008–2009 and 2017–2018 were similar in both genders; however, the snacks pattern, which explained most of the data variability in 2008–2009, was replaced by the traditional Brazilian.

Aim The main purpose of this study was to conduct a narrative review investigating the possible relationship between permanent maxillary anterior teeth and anthropometric facial parameters in different populations. Methods Searches were performed in the PubMed, BVS (Biblioteca Virtual em Saúde) and SciELO databases to identify relevant scientific articles using the following search terms: “maxillary anterior teeth”, “facial measurements” and “anthropometry”, in such a way that 218 publications were found. After application of inclusion and exclusion criteria, 13 publications remained for full-text reading. Results All studies involved male and female samples and it was notorious that male measurements were unanimously higher than those obtained in the opposite sex. The age of the analyzed participants ranged from 17 to 60 years; however, a mean age of 18 to 25 years was the most investigated in literature. In addition, facial measurements including the bizygomatic width, interpupillary distance, intercanthal distance, interalar width and intercommissural width have been proposed to help determine the correct size of anterior teeth. Conclusion It was concluded that despite the limited number of studies, some factors that influence dental and facial dimensions, such as sex and age, can be identified. However, there is no standardization of the facial or dental parameters used in the studies, a fact that makes it difficult to establish a universal ratio for clinical dental practice.

RESUMO O presente artigo põe em foco a pesquisa que deu origem ao livro A produção do fracasso escolar: histórias de submissão e rebeldia, baseado em memórias das pesquisadoras. Tal obra é considerada um marco, pois tece incisivas críticas às práticas hegemônicas da Psicologia e da Educação e propõe rupturas teóricas e metodológicas radicais na pesquisa sobre a escola pública brasileira. O artigo partilha reflexões teórico-metodológicas que orientaram a realização de encontros-entrevistas com Maria Helena Souza Patto e as três pesquisadoras auxiliares do clássico livro. Retomada a trajetória pregressa das participantes, a costura de lembranças das pesquisadoras com reflexões registradas no livro referentes ao procedimento desenvolvido traz detalhamentos não ressaltados na publicação e que foram fundamentais para o estudo das práticas cotidianas.

ABSTRACT The present article focuses on the research that originated the book The Production of School Failure: Stories of Submission and Rebellion, based on the memories of the researchers. This work is considered a landmark because it makes sharp criticisms of the hegemonic practices of Psychology and Education and proposes radical theoretical and methodological ruptures in research on Brazilian public education. The article shares theoretical and methodological reflections that guided the meetings and interviews with Maria Helena Souza Patto and the three auxiliary researchers of the classic book. Once the participants’ previous trajectory is retrieved, a sewing of the researchers’ memories with the reflections registered in the book regarding the procedure developed brings details that were not highlighted in the publication and that were fundamental for the study of daily practices.

RESUMEN El presente artículo enfoca la investigación que dio origen al libro La producción del fracaso escolar: historias de sumisión y rebeldía, a partir de las memorias de las investigadoras. Dicha obra es considerada un hito, ya que hace críticas incisivas a las prácticas hegemónicas de la Psicología y de la Educación, y propone rupturas teóricas y metodológicas radicales en la investigación sobre la escuela pública brasileña. El artículo comparte reflexiones teórico-metodológicas que orientaron la realización de encuentros-entrevistas con Maria Helena Souza Patto y las tres investigadoras auxiliares del libro clásico. Retomada la trayectoria anterior de las participantes, la costura de memorias de las investigadoras con reflexiones registradas en el libro referentes al procedimiento desarrollado trae detalles no destacados en la publicación y que fueron fundamentales para el estudio de las prácticas cotidianas.

ABSTRACT Objectives: To assess the molecular profile of follicular cell-derived thyroid carcinomas (FCDTCs) and correlate the identified mutations with the clinical and pathological features of the affected patients. Materials and methods: Cross-sectional study of tumor samples from 100 adult patients diagnosed with FCDTC between 2010 and 2019. The patients’ clinical and pathological data were collected. Genomic DNA was extracted from formalin-fixed, paraffin-embedded (FFPE) tumors using the ReliaPrep FFPE gDNA Miniprep System. Genotyping of target genomic regions (KRAS, NRAS, BRAF, EGFR, and PIK3CA) was performed using the AmpliSeq panel, while sequencing was performed on the iSeq 100 platform. Results: The patients’ mean age was 39 years. In all, 82% of the tumors were classic papillary thyroid carcinomas. Overall, 54 (54%) tumor samples yielded satisfactory results on next-generation sequencing (NGS), of which 31 harbored mutations. BRAF gene mutations were the most frequent, with the BRAFV600E mutation present in 10 tumors. Seven tumors had BRAFNON-V600E mutations not previously described in FCDTCs (G464E, G464R, G466E, S467L, G469E, G596D, and the T599Ifs*10 deletion) but described in other types of cancer (i.e., skin/melanoma, lung, colorectal, and others). One tumor had a previously reported BRAFA598V mutation. EGFR gene mutations were found in 16 (29%) and KRAS or NRAS alterations in 8 (14%) of the 54 tumors analyzed. Conclusion: We described herein seven non-hotspot/novel variants in the BRAF gene, highlighting their potential role in expanding our understanding of FCDTC genetics.

Abstract Objectives: Patients with post-traumatic stress disorder (PTSD) present a variety of symptoms, with different intensities, causing impairment in individual, social, and occupational functioning. The aim of this study was to understand the psychodynamic functioning of patients with PTSD, exploring relationships between symptom severity, quality of life, subjective suffering, conflicts, and psychic structure and sociodemographic characteristics, styles, and defensive mechanisms. Methods: This is a cross-sectional quantitative study with 60 participants. The following instruments were used: a sociodemographic questionnaire, the Operationalized Psychodynamic Diagnosis-2 (OPD-2), and the Defensive Style Questionnaire (DSQ-40). Results: Participants had moderate to high symptom severity, with significant subjective suffering and isolation. The main conflict was need for care vs. self-sufficiency and the level of total structure was moderate/low. Use of immature, neurotic, and mature defensive styles was observed. More primitive personality structures, more rigid defenses, and greater dependence were found in patients with history of past trauma. Other mental disorders were also associated. Conclusion: The OPD-2 was effective for assessment of the psychodynamic functioning characteristics of patients with PTSD. Therapeutic treatment should focus on the psychic structure and not only on symptom control. Prevention strategies should target vulnerability factors while strengthening protective factors.

Abstract The present article analyzed the process of precarious work relations in the health sector, demonstrating how this situation generated illness and suffering among workers. The text sought to register how the institutional adoption of neoliberal economic conceptions, of the reduction of State social policies, coupled with a history of cultural rejection of labor rights, provided the attraction of mechanisms of precarious work for the public sector, notably, the outsourcing of labor and the provision of services. In a second moment, the text points out the legal improprieties committed, presenting the consequences of the weakening of the legal bond in the working conditions of these workers, mainly regarding working hours and remuneration, also highlighting how much the dissemination of these links represented a factor of a general reduction in wages and labor and social security rights in the sector. Data from Fiocruz surveys on working conditions during the pandemic were also presented. The article concludes by showing the urgency of rescuing the legal labor institutes that were abandoned in the historical path studied in order to guide the public health policies in a new direction.

Resumo O artigo analisou o processo de precarização das relações de trabalho no setor da saúde, demonstrando como essa situação gerou sofrimento dos trabalhadores. O texto buscou registrar como a adoção institucional das concepções econômicas neoliberais, de redução das políticas sociais de Estado, acopladas a um histórico de rejeição cultural aos direitos trabalhistas, proporcionou a atração dos mecanismos de precarização do trabalho, notadamente, a terceirização da mão de obra e da prestação de serviços. No segundo momento, o texto aponta as impropriedades jurídicas cometidas, apresentando as consequências da fragilização do vínculo jurídico nas condições de trabalho desses trabalhadores, principalmente no que se refere às jornadas de trabalho e a remuneração, destacando-se, também, o quanto a disseminação desses vínculos representou um fator de rebaixamento geral dos salários e direitos trabalhistas e previdenciários no setor. Foram apresentados, ainda, dados das pesquisas da Fiocruz sobre condições de trabalho durante a pandemia. Conclui mostrando a urgência de se resgatarem os institutos jurídicos trabalhistas que foram abandonados no percurso histórico estudado, para direcionar a novo rumo as políticas públicas na saúde.

Abstract The present article addresses the work conditions in health in the context of the COVID-19 pandemic in Brazil. This is a cross-sectional study that used data from the surveys “Working conditions of healthcare professionals in the context of Covid-19 in Brazil” and “Invisible healthcare workers: work conditions and mental health in the context of Covid-19 in Brazil”, seeking to better understand the working conditions and biosafety of these two distinct and socially unequal professional contingents. Data analysis proves that work conditions were extremely affected due to inadequate infrastructures, strenuous work, biosecurity at risk, exhaustion, fear of contamination and death, strong signs of physical and mental exhaustion, among workers. It also points out the discrimination and inequalities of social rights and professional development that mark the worlds of work highlighted in the surveys, emphasizing the profound inequalities that exist in Brazil and in its regions. It concludes by showing the importance of formulating public policies within the scope of work management in SUS, which ensures the protection, appreciation and reduction of inequalities pointed out in this article.

Resumo O artigo versa sobre as condições de trabalho na saúde no contexto da pandemia no Brasil. Trata-se de estudo transversal que utilizou dados de recorte das pesquisas “Condições de trabalho dos profissionais de saúde no contexto da Covid-19 no Brasil” e “Os Trabalhadores invisíveis da saúde: condições de trabalho e saúde mental no contexto da Covid-19 no Brasil”, objetivando conhecer as condições de trabalho e a biossegurança desses dois contingentes profissionais distintos e desiguais, socialmente. A análise dos dados comprova que as condições de trabalho foram extremamente afetas em função da infraestrutura inadequada, trabalho extenuante, biossegurança em risco, exaustão, medo da contaminação e da morte, fortes sinais de esgotamento físico e mental entre os trabalhadores. Aponta também para discriminação e desigualdades de direitos sociais e de valorização profissional que demarcam os mundos do trabalho apontados nas pesquisas, enfatizando as profundas desigualdades existentes no Brasil e em suas regiões. Conclui-se mostrando a importância de formulação de políticas públicas no âmbito da gestão do trabalho no SUS que assegurem a proteção, valorização e redução das desigualdades apontadas no artigo.

Abstract This article addresses the world of healthcare work, especially in the Brazilian Unified Healthcare System (SUS) in the context of the COVID-19 pandemic in Brazil. This study used data from the following surveys: “Working conditions of health professionals in the context of COVID-19 in Brazil” and “The Invisible health workers: working conditions and mental health in the context of COVID-19 in Brazil”. Data analysis proves that the pandemic highlighted existing structural problems within SUS, involving the issue of healthcare workforce (HWF) management, which can be interpreted as another reflection of the socioeconomic inequalities that already exist in the country. This article highlights: the reduced provision of permanent education, the regulation of hybrid care, precariousness, a lack of protection in the work environment, as well as fragile biosecurity leading to tragic rates of illness and death of health workers. Our study concludes by showing the importance of formulating public policies in the scope of education and work management in SUS that ensure the discussion on hybrid care as a new way of acting without losing quality, together with the need to review issues related to permanent education, protection, valuation, and reduction of inequalities pointed out among the professional contingents analyzed in this article.

Resumo O artigo versa sobre o mundo do trabalho da saúde, especialmente no SUS no contexto da pandemia no Brasil. O artigo utilizou dados das pesquisas “Condições de trabalho dos profissionais de saúde no contexto da COVID-19 no Brasil” e “Os trabalhadores invisíveis da saúde: condições de trabalho e saúde mental no contexto da COVID-19 no Brasil”. A análise dos dados comprova que a pandemia evidenciou problemas estruturais existentes no âmbito do Sistema Único de Saúde, envolvendo a gestão da FTS o que pode ser interpretado como mais um dos reflexos das desigualdades socioeconômicas já existentes no país. Destacam-se: a reduzida oferta de educação permanente, a regulação do cuidado híbrido, precarização, desproteção no ambiente de trabalho, frágil biossegurança levando a trágicas taxas de adoecimento e mortes de trabalhadores da saúde. Conclui mostrando a importância de formulação de políticas públicas no âmbito da gestão da educação e do trabalho no SUS que assegurem a discussão sobre cuidado híbrido como nova forma de atuar sem perder qualidade, a necessidade de se rever questões referentes a: educação permanente, proteção, valorização e redução das desigualdades apontadas entre os contingentes profissionais analisados nesse artigo.
