Resumo A linfohistiocitose hemofagocítica (LHH) é uma condição incomum e potencialmente fatal, caracterizada por importante ativação imunológica e produção maciça de citocinas por células mononucleares inflamatórias, devido a defeitos na função linfocitária citotóxica. É ainda mais incomum em receptores de transplante renal, nos quais está freqüentemente associada a infecções não controladas. A mortalidade da LHH é alta, e o diagnóstico diferencial com sepse é um desafio. A abordagem e o tratamento dependem do gatilho e das comorbidades subjacentes. Relatamos o caso de uma paciente transplantada renal com 50 anos de idade, admitida com febre e mal-estar 3 meses após o transplante, apresentando anemia, febre, hipertrigliceridemia, níveis elevados de ferritina sérica e antigenemia positiva para CMV. A urina mostrou positividade para células decoy e BKV-DNA. A biopsia do enxerto mostrou nefrite por CMV. Ambas as culturas de sangue e urina foram positivas para E. coli. A hemofagocitose foi confirmada pelo aspirado de medula óssea. A imunossupressão foi reduzida e a paciente recebeu altas doses de imunoglobulina intravenosa e dexametasona, com resposta completa após 3 semanas. Destaca-se a importância do diagnóstico precoce e do manejo adequado de uma condição rara e grave em um paciente transplantado renal, o que pode permitir um curso clínico favorável e melhorar a taxa de sobrevida.

Abstract Hemophagocytic lymphohistiocytosis (HLH) is an uncommon and life-threating condition characterized by major immune activation and massive cytokine production by mononuclear inflammatory cells, due to defects in cytotoxic lymphocyte function. It is even more unusual in renal transplant recipients, in which it is often associated with uncontrolled infection. The mortality is high in HLH and differential diagnosis with sepsis is a challenge. The approach and management depend on the underlying trigger and comorbidities. We report a case of a 50-year-old renal transplant female admitted with fever and malaise 3 months post-transplant and presenting anemia, fever, hypertriglyceridemia, high levels of serum ferritin, and positive CMV antigenemia. Urine was positive for decoy cells and BKV-DNA. Graft biopsy showed CMV nephritis. Both blood and urine cultures where positive for E. coli. Hemophagocytosis was confirmed by bone marrow aspiration. Immunosuppression was reduced, and the patient received high-dose intravenous immunoglobulin and dexamethasone, with complete response after 3 weeks. We highlight the importance of early diagnosis and proper management of a rare and serious condition in a renal transplant patient, which can allow a favorable clinical course and improve survival rate.

Ao longo dos anos, o rio Subaé na Bahia tem sido impactado por atividade industrial pelo lançamento de efluentes tóxicos em suas águas. O presente estudo analisou os efeitos citotóxicos e genotóxicos provocados pela água do trecho deste rio que banha o município de Humildes-BA, utilizando o teste do Allium cepa. Amostras de água foram obtidas em três pontos de coleta (PI, PII e PIII) para as análises de parâmetros fisico-químicos. Bulbos de A. cepa foram colocados por 72 h para germinação nas amostras de água em triplicata. As radículas foram submetidas por 12-h a 4 oC, fixadas em Carnoy por 12-h e coradas com técnica de Feulgen. Foram analisadas 6.500 células/tratamento. A citotoxicidade foi avaliada pelo índice mitótico (IM) e a genotoxicidade, por anormalidades cromossômicas (AC). Os testes fisico-químicos revelaram que PI e PIII apresentavam valores de OD, DBO5 e DQO alterados. O teste de toxicidade indicou que houve inibição do crescimento das raízes nos pontos PI (10,3 %) e PII (10,6 %), quando comparados ao controle negativo (11,9 %). No PI, foram encontrados 6,2 % de ACs, 6 % em PII e 5,4 % em PIII, valores acima do controle positivo (3 %), evidenciando o efeito genotóxico nos pontos estudados. A partir dos resultados, pode-se inferir que as águas do Rio Subaé podem conter substâncias que provavelmente estão afetando o ciclo celular do A. cepa, sendo importante que outras análises sejam realizadas em diferentes estações do ano, para melhor monitoramento das condições ambientais locais.

Over the years, the Subaé River in Bahia has been polluted by the discharge of toxic industrial waste into its waters. This study used the Allium cepa test to investigate the cytotoxic and genotoxic effects caused by the river water that that runs through the city of Humildes-BA. Water samples were collected in three points (PI, PII and PIII) for physical and chemical analysis. Onions bulbs were immersed in triplicate water samples for 72 h to induce germination. The root tips were kept at 4 °C for 12-h, then fixed in Carnoy for 12-h and stained using the Feulgen technique. We analyzed 6500 cells of each treatment. Cytotoxicity was assessed by the mitotic index (MI) and genotoxicity by chromosomal abnormalities (CA). The tests revealed that PI and PIII had DO values, BOD5 and COD changed. The toxicity test indicated that root growth was inhibited by the water drawn from PI (10.3%) and PII (10.6%) compared to the negative control (11.9%). CAs were found in 6.2% of the cells relating to PI, 6.0% relating to PII, and 5.4% relating to PIII, respectively. These values were higher than those found in the positive control (3%), indicating a genotoxic effect at the points studied. From the results, it can be inferred that the waters of the Rio Subaé may contain substances that are probably affecting the cell cycle of A. cepa. It is important that other analyses be conducted in different seasons, in order to better monitor local environmental conditions.

Um caso de síndrome purpúrico-papular em "luvas e meias" devido à infecção aguda por parvovírus B19 é descrito em um homem adulto que, além das manifestações clássicas de febre, lesões orais e púrpura em mãos e pés, evoluiu com icterícia e necrose cutânea superficial dos pés, características até então não descritas nesta síndrome. Acreditamos tratar-se do primeiro caso descrito no Brasil.

We present a case of papular-purpuric "gloves and socks" syndrome (PPGSS) in an adult male with acute parvovirus B19 infection. The patient displayed the classical features of fever, oral lesions, and purpura on hands and feet, but the purpuric lesions on the feet evolved to superficial skin necrosis, a feature not previously described in this syndrome. We believe this is the first reported case of PPGSS occurring in Brazil.

Por ocasião de um pequeno surto de leishmaniose tegumentar americana em localidade do município de Cordeiro (RJ) foram documentados 14 casos. Entre os fatos que chamaram a atenção dos autores estão a alta ocorrência em crianças (57%), a freqüência do componente linfangítico (40%), a tendência à cicatrização espontânea e a distribuição peri-domiciliar dos casos. Um inquérito pela intradermoreação de Montenegro na área de maior concentração de casos, realizado numa fazendo e numa escola, mostrou 17% de reações positivas, 17% de duvidosas e 66% de reações negativas, num total de 117 indivíduos testados. Não foram encontrados casos antigos cicatrizados, o que sugere um foco recente. Discute-se o significado das reações duvidosas.

During a small outbreak of American mucocutaneous leishmaniasis in a village of the municipality of Cordeiro, state of Rio de Janeiro, fourteen cases were documented. Outstanding features were the high incidence in children (57%), the clinical evidence of lymphangitis in 40%, the tendency towards spontaneous healing in the less recent cases and the peri-domestic distribution of the cases. A Montenegro skin-test survey carried out in 117 persons yielding 17% positive and 17% doubtful reactions. No positive reactors with old scars or signs of mucosal envolvement were noted, this being probably a recent focus. The significance of doubtjul skin reactions is discussed.
