Abstract Introduction: Aortic diseases are among the most serious cardiovascular diseases; the overall mortality rate due to diseases such as aneurysms and aortic dissections has been estimated at 2.78 per 100,000 persons in 2010, with a higher mortality rate in men than women. Our objective was to evaluate the epidemiological profile of patients with acute type A aortic dissection at a cardiology referral center. Methods: A retrospective cross-sectional study was performed at a public cardiac center with 24 patients hospitalized from 1/1/2016 to 12/31/2017 with a confirmed diagnosis of acute type A aortic dissection. Results: Twenty (83.3%) out of 24 patients underwent surgery and four (16.7%) did not undergo surgery. Among those who underwent surgery, 10 (50%) died and 10 (50%) were discharged, and all non-operated patients died (P=0.114) (Fisher's exact test). The male gender predominated (n=19, 79.2%), 86.7% (n=13) of the patients presented body mass index > 25 kg/m2, chest pain was found in 91.7% (n=22), and renal failure was present in 45.8% (n=11) of the cases. Hypertension predominated in 91.7% (n=22) and the main exam was aortic angiotomography in 79.2% (n=19) of the cases. Conclusion: The study presented a small sample size, making it impossible to associate the factors, although the service was considered a high-volume referral center. It is possible that the delay in arriving at the service and the accomplishment of invasive imaging with the use of contrast agents have aggravated the patients’ condition and have been decisive for the increase in lethality, which requires further studies.

RESUMO Introdução: o sarcoma primário do fígado é uma neoplasia rara, que acomete mais frequentemente crianças. Nos adultos, representa espectro de neoplasias de prognóstico reservado. Entretanto, em nenhuma faixa etária há consenso sobre a terapêutica de escolha dessas lesões, motivando revisão sistemática com objetivo de elencar opções de tratamento, fatores prognósticos e sobrevida. Material/Método: realizamos revisão sistemática dos artigos publicados nas bases de dados Pubmed, Medline, LiLacs e Scielo, de 1966 a março/2019, contendo as palavras-chaves: primary-liver-sarcoma e primary-hepatic-sarcoma. Foram incluídos estudos que incluíram pacientes com idade a partir de 18 anos e publicados em inglês, português e espanhol. Relatos de caso, tumores metastáticos e pacientes com múltiplos diagnósticos oncológicos foram excluídos. Foram encontrados inicialmente 1.318 artigos, desses, 1.206 foram excluídos por estarem fora dos critérios de inclusão. Dos 112 artigos analisados e discutidos, 15 foram incluídos nesse artigo (14 séries de casos e 1 estudo de coorte retrospectivo). Resultado: os tratamentos propostos para o sarcoma primário do fígado em adultos incluíram cirurgia e/ou quimioterapia e/ou radioterapia ou transplante hepático. Os tipos histológicos mais frequentemente relados foram angiossarcoma (32%), leiomiossarcoma (29%), hemangioendotelioma epitelioide (15%) e sarcoma embrionário (7%). Tipo histológico, grau de diferenciação e resseção R0 foram os principais fatores de bom prognóstico. A sobrevida média total variou de dois a 23 meses. A sobrevida em cinco anos variou entre 0 e 64%, em média 21%. Conclusão: ressecção cirúrgica R0 é a base do tratamento dos sarcomas primários do fígado. Esse grupo heterogêneo de tumores requer desenvolvimento de terapias sistêmicas efetivas para melhoria do prognóstico.

ABSTRACT Introduction: primary liver sarcoma is a rare type of tumor, more common in children. Among adults, it represents a spectrum of neoplasms with reserved prognosis. There is no consensus on the treatment of choice of these lesions, justifying a systematic review of the literature on treatment options, prognostic factors, and survival. Material/Methods: a systematic review of articles published in Pubmed, Medline, LiLacs e SciElo, from 1966 to March/2019, presenting the keywords: primary-liver-sarcoma and primary-hepatic-sarcoma was undertaken. Studies including patients older than 18 years, and published in English, Portuguese and Spanish were included. Case reports, metastatic tumors and multiple oncologic diagnosis were excluded. The initial search listed 1,318 articles. 1,206 did not meet the inclusion criteria. After reviewing 112 eligible articles, 15 were selected (14 case series and 1 retrospective-cohort). Results: proposed treatment modalities for primary liver sarcoma included surgery and/or chemotherapy and/or radiotherapy or liver transplantation. The most common histological types were angiosarcoma (32%), leiomyosarcoma (29%), epithelioid hemangioendothelioma (15%) and embryonal sarcoma (7%). Histology, degree of differentiation and R0 resection were mentioned positive prognostic factors. Median survival ranged from two to 23 months. Five-year survival rate varied from 0% to 64%, on average 21%. Conclusion: surgical resection (R0 resection) is the main treatment for primary liver sarcomas. Development of effective systemic therapies are required to improve prognosis of patients harboring this type of tumor.

ContextoO câncer de vesícula biliar ocasionalmente é descoberto após colecistectomias realizadas para tratamento da colelitíase. No entanto, a heterogênica prevalência de seu diagnóstico incidental é desconhecida entre as diferentes regiões de nosso país.ObjetivoExplorar a prevalência do câncer incidental da vesícula biliar em nosso centro terciário de saúde.MétodosRealizou-se estudo de coorte retrospectivo incluindo pacientes consecutivamente submetidos a colecistetomias para tratamento da colelitíase na Faculdade Pernambucana de Saúde, Instituto de Medicina Integral Professor Fernando Figueira - FPS/IMIP, de janeiro de 2007 a dezembro de 2010. Os dados dos pacientes com câncer incidental da vesícula biliar foram analisados visando estimar sua prevalência e descrever a experiência do serviço no manejo desta neoplasia.ResultadosEsta análise envolveu 2018 pacientes, com marcada predileção do gênero feminino (n=1,697; 84,1%) sobre o masculino (n=321; 15,9%). A prevalência em 3 anos para o câncer incidental da vesícula biliar foi estimada em 0,34% em nossa casuística. Ao estadiamento patológico inicial, observou-se um caso T1a, um T1b, e cinco T2, todos com diagnóstico histológico de adenocarcinoma. Colecistectomia laparoscópica isoladamente foi realizada para o tumor T1a, enquanto a complementação cirúrgica radical foi realizada em outros cinco. Em um caso, evidenciou-se doença metastática à reoperação. O estudo histopatológico após a cirurgia radical revelou doença residual/adicional em todos os tumores T2 e um paciente (T1b) foi submetido ao resgate cirúrgico (cirurgia de Whipple) para tratamento de colangiocarcinoma distal secundário. O caso estadiado como T1a está vivo após 3 anos de acompanhamento ambulatorial, enquanto todos os outros faleceram por recidiva/progressão neoplásica em até 12 meses.ConclusãoEste estudo confirma o mal prognóstico do câncer de vesícula biliar mesmo quando incidentalmente descoberto após colecistectomias e pressupõe a prevalência em 3 anos de 0,34% para seu diagnóstico incidental em nosso centro terciário de saúde.

BackgroundGallbladder cancer is sometimes incidentally uncovered following cholecystectomy for gallstones diseases. The supposed highly variable prevalence of incidental gallbladder cancer through our country is unknown.ObjectiveTo explore the prevalence of incidental gallbladder cancer in our tertiary-care hospital.MethodsA cross-sectional study was carried out on patients who consecutively underwent cholecystectomy due to gallstones disease at Faculdade Pernambucana de Saúde, Instituto de Medicina Integral Professor Fernando Figueira - FPS/IMIP, from January, 2007 to December, 2010. Data on incidental gallbladder cancer patients were explored for prevalence estimation and description of our experience with the management of this malignancy.ResultsOur analysis involved 2018 patients with a marked predominance of women (n=1.697; 84.1%) over men (n=321; 15.9%). The 3-year prevalence estimate of 0.34% was recorded for incidental gallbladder cancer in our sample. Regarding tumor staging, there were 1 T1a, 1 T1b, and 5 T2 adenocarcinoma tumors. Laparoscopic cholecystectomy alone was performed for the T1a tumor, and additional radical surgery was performed in five others. One patient presented metastatic disease at the time of repeat surgery. The final pathology revealed residual/additional disease in all T2 tumors after radical surgery whereas the T1b patient underwent a salvage Whipple’s procedure due to a secondary distal cholangiocarcinoma. The patient with T1a tumor is alive after 3-year follow-up but all of the others died because of disease recurrence/progression up to 12 months.ConclusionThis study confirms the poor prognosis of Gallbladder cancer even when incidentally diagnosed following cholecystectomy and supposes a 3-year prevalence estimate of 0.34% for incidental gallbladder cancer in our Center from Pernambuco State, Brazil.

OBJETIVO: Avaliar a utilização do fígado suíno como um modelo experimental para o ensino e treinamento da técnica glissoniana intra-hepática nas ressecções do fígado. MÉTODOS: Foram utilizados 10 fígados inteiros de porcos adultos brancos, pesando entre 35-45 kg. Os pedículos glissonianos de vários segmentos foram dissecados e ressecados para a realização de estudos histológicos e verificação da presença da bainha conjuntiva ao longo das tríades portais. RESULTADOS: Na microscopia, uma bainha de tecido conjuntivo foi encontrada envolvendo os pedículos glissonianos no fígado suíno. A utilização de preparações específicas para o tecido conjuntivo (Masson) ressaltou a presença dessa bainha em cada pedículo. CONCLUSÃO: As características morfológicas dos pedículos glissonianos suínos, fazem desse modelo experimental um método de treinamento da técnica glissoniana intra-hepática.

PURPOSE: The aim of the study is to evaluate the porcine liver as a teaching and training model for the glissonian approach. METHODS: Ten livers were removed from domestic adult white pigs weighting 35 to 45kg. Based on anatomical landmarks, the glissonian pedicles of each liver segments were dissected and biopsies were taken for histological examination, to analyze the presence of the glissonian sheath. RESULTS: During microscopic examination, a sheath of conjunctive tissue was observed wrapping each segmental pedicle in porcine liver. This could be clearly seen when histological preparation for connective tissue was obtained (Masson technique). CONCLUSION: The morphological arrangement of glissonian pedicles in porcine liver makes this model a useful tool for training the intra-hepatic glissonian approach.

RACIONAL: Cada vez mais se estabelece a correlação entre agentes infecciosos e doenças linfoproliferativas, sobretudo vírus e bactérias, através da ativação de linfócitos. OBJETIVO: Descrever 6 novos casos, de uma série de 254 pacientes (2,36%) com esquistossomose mansônica na forma hepatoesplênica. CASUÍSTICA E MÉTODOS: São descritos 6 pacientes, dentre os 254 portadores de esquistossomose mansônica na forma hepatoesplênica, acompanhados nos últimos 13 anos no Serviço de Cirurgia Geral do Hospital das Clínicas da Universidade Federal de Pernambuco, Recife, PE. RESULTADOS: Todos os seis casos ocorreram em mulheres. Os exames histopatológicos evidenciaram dois casos de linfomas de zona marginal esplênica, um de linfoma de grandes células com imunoblastos, um de linfoma difuso de grandes células, um de linfoma maligno de grandes células não clivadas, e um outro caso de doença de Hodgkin. Metade das seis pacientes evoluiu para o óbito entre 4 a 15 meses após o diagnóstico. As outras três persistem em acompanhamento no Serviço de Oncologia da mesma instituição. CONCLUSÃO: A incidência de linfoma nos 254 pacientes acompanhados foi de 2,36%. Pretende-se chamar a atenção para a ocorrência de linfomas nos baços de pacientes com esquistossomose mansônica, na forma hepatoesplênica.

BACKGROUND: Correlation between infectious agents and linfoproliferative diseases are more stablished, over all virus and bacteria, through the activation of linfocytes. AIM: To describe six new cases, of a series of 254 patients (2,36%) with mansonic schistosomiasis, in the hepatosplenic form. METHODS: Six patients will be described, amongst the 254 carriers of mansonic schistosomiasis, in the hepatosplenic form, followed in the last 13 years. RESULTS: All the six cases had occurred in women. The histopathologic examinations had evidenced two cases of marginal splenic zone lymphomas, one of great cells with immunoblasts lymphomas, one diffuse lymphomas, a great cells malignant lymphomas, a great not clivads cells, and another case of Hodgkin. Half of the six evolved for the death 4-15 months after the diagnosis. The others three persist in accompaniment in the Oncology Division of the Clinics Hospital. CONCLUSION: The incidence of lymphomas in the 254 mansonic schistosomiasis patients followed in our clinic was of 2,36%. At last, this article intends to call the attention, for the occurrence of lymphomas, in the spleen of patients with mansonic schistosomiasis, in the hepatosplenic form.

OBJETIVO: A origem dos abscessos esplênicos está relacionada a episódios de bacteremia após infarto esplênico. Recentemente, a SIDA, a tuberculose, a imunossupressão e as neoplasias têm mudado a apresentação clínica desse abscesso. Tradicionalmente, o tratamento de escolha é a esplenectomia. Entretanto, a drenagem percutânea pode ser uma alternativa útil em casos selecionados O objetivo desse trabalho foi determinar o perfil dos pacientes com abscesso esplênico tratados em um hospital terciário. MÉTODO: Dados referentes aos pacientes atendidos no Serviço de Cirurgia Geral HC-UFPE no período de novembro de1993 a fevereiro de 2004 foram analisados. Sete pacientes dentre os 73094 tratados no período constituíram o objeto desse estudo. RESULTADOS: Cinco pacientes foram tratados inicialmente com esplenectomia e dois por drenagem percutânea. Quatro pacientes (57%) apresentavam abscessos secundários à imunossupressão, tuberculose ou doença neoplásica. Três (43%) apresentavam abscessos decorrentes de infecção metastática (endocardite). A drenagem percutânea foi o único tratamento utilizado em um caso. CONCLUSÕES: Houve uma mudança no padrão clássico, e quase que exclusivo, de origem hematogênica do abscesso esplênico para abscessos decorrentes de uma condição de imunossupressão (câncer, tuberculose, transplantes etc.). Em casos selecionados, a drenagem percutânea pode ser a única forma de tratamento invasivo utilizada.

BACKGROUND: Splenic abscess remains a diagnostic challenge with a high mortality rate. Its origin is related to episodes of bacteremia after a splenic infarct. Last decade, the demographics of the population with this disease changed towards patients with immunosuppressive conditions (tuberculosis, cancer, transplant, etc). Traditionally, splenectomy has been the treatment of choice, with a high morbidity and mortality. Percutaneous drainage can be an alternative to avoid splenectomy. METHODS: A direct search of Hospital das Clínicas/UFPE records between November/1993 and February/2004 resulting in a total number of 73094 patients has been perfomed. RESULTS: Seven cases of splenic abscess are presented: five patients were treated with splenectomy and two with percutaneous drainage. Four patients (57%) had splenic abscess related to immunosuppressive conditions (steroids, cancer, tuberculosis and transplant). CONCLUSION: A change in clinical presentation towards imunosupression was observed. The diagnosis should be considered in a patient with fever, abdominal pain and immunosuppressive condition. The percutaneous drainage alone or preceding splenectomy can be efficient to solve splenic abscess in selected cases.

A perforated jejunal leiomyoma is a rare condition; only seven cases have been described in the literature. We report a case of a 45 years old man with a perforate acute abdomen. At laparotomy, a 12 cm multilobulated firm mass, located 20 cm distal from Treitz ligament, was removed. Histological sections disclosed a jejunal leiomyoma. The benign pattern of the lesion was confirmed with no mitosis and no atypical cellurarity. Five years later, the patient is free of the disease.

The authors report a case of acute gastric volvulus in a 20-year-old male, complicated by perforation near the gastroesophageal junction and generalized peritonitis. This is an uncommon and potentially lethal conditon although our patient has been handled successfully with a partial gastrectomy.

OBJETIVO: Analisar a experiência do Serviço de Cirurgia Geral - HC/UFPE com os tumores do intestino delgado, na tentativa de colaborar com a definição de metas para o manejo dessas lesões. MÉTODO: Constitui-se este estudo em uma análise retrospectiva de12 pacientes portadores de neoplasias intestinais atendidos no SCG-HC/UFPE, no período de cinco anos (1994-1999). Sete eram do sexo masculino (58%). A média de idade à admissão foi de 52 anos. A principal queixa referida foi dor abdominal (83%), seguida por sangramento digestivo e perda ponderal (42%). Massa abdominal palpável foi evidenciada em 50% dos casos. Sete (58%) apresentavam lesões malignas. Dentre as neoplasias benignas, quatro eram leiomiomas. Uma paciente, portadora de síndrome de Peutz-Jeghers, apresentou um hamartoma. Dez foram submetidos a tratamento operatório. Em seis, enterectomia com enteroanastomose foi empregada. Três foram operados em caráter de urgência (um por perfuração intestinal, um por enterorragia maciça e um por obstrução intestinal). RESULTADOS: A mortalidade relacionada à operação foi de 17%. Todos os pacientes que evoluíram para óbito apresentavam neoplasias malignas avançadas e perda ponderal superior a15% de seu peso. CONCLUSÕES: Tumores do intestino delgado são lesões incomuns, mesmo em serviços de referência. Um alto grau de suspeição deve ser mantido visando o diagnóstico precoce.

BACKGROUND: This study was aimed to identify the behaviour of small bowel tumours treated at the General Surgery Department of Hospital das Clínicas-UFPE. METHODS: From 1994 to 1999, 12 patients with small intestine tumours were retrospectively selected from the files of the General Surgery Department - Hospital das Clínicas/UFPE. Seven patients were male. The mean age at admission was 52 years. The most frequent complaint was abdominal pain (83%), followed by bleeding and loss of weight (42%). The tumour could be felt at the abdominal examination in 50% of the patients. Seven patients harboured malignant lesions (58%). Among benign tumours, four patients had leiomiomas. One female patient, with Peutz-Jeghers syndrome, presented with hammartomas. Ten patients were submitted to surgical treatment. Three, in an urgent basis. RESULTS: The mortality related to the surgery was 17%. All patients that eventually progressed to death had malignant lesions, and were admitted with marked loss of weight (> 15% of the ideal body weight). CONCLUSIONS: Small intestine tumours are rare lesions even in major treatment centers. Those treating these patients should keep a high clinical suspicion. Earlier diagnosis, particularly in patients with malignant lesions, can improve prognosis.
