Resumen: El Pediatric Index of Mortality 3 (PIM3) es una escala que estima el riesgo de mortalidad en niños ingresados a Unidad de Cuidado Intensivo Pediátrico (UCIP) en la primera hora del ingreso. Objetivo: validar la escala PIM3 en población pediátrica que ingresa a UCIP en alturas sobre los 2.500 metros sobre el nivel del mar (m.s.n.m.), y evaluar si el lactato y el índice Saturación de Oxígeno/Fracción inspirada de oxígeno (SpO2/FiO2) mejoran su capacidad de predicción de mortalidad. Pacientes y Método: Estudio prospectivo multicéntrico realizado en diez UCIP de Colombia entre noviembre de 2016 y junio de 2017. Se recolectaron variables para cálculo del PIM3, lactato sérico y el índice SpO2/FiO2. Se validó su capacidad de discriminación con el área bajo la curva ROC y la calibración mediante la prueba de bondad de ajuste de Hosmer Lemeshow. Resultados: Se incluyeron 2.803 registros de admisiones, con una mortalidad global de 4,8%, y razón estandarizada de mortalidad de 1,1 (IC95%; 1,00-1,3). El PIM3 mostró una baja calibración (p < 0,001) y una adecuada discriminación para la población aplicada (ROC 0,84; IC95% 0,83-0,86). La adición de lactato sérico y SpO2/ FiO2 permitieron una buena calibración (p = 0,75), y mantuvo una alta discriminación de mortalidad (ROC 0,86; IC95% 0,84-0,87). Conclusiones: El comportamiento de la escala PIM3 tiene adecuada discriminación, pero una baja calibración en niños admitidos a UCIP sobre los 2500 m.s.n.m.. Las variables lactato y SpO2/FiO2 mejoraron su habilidad predictiva.

Leclercia adecarboxylata, a rare cause of soft tissue infections in immunocompromised patients, case report and review of the literature

Resumen Leclercia adecarboxylata es un miembro de la familia Enterobacteriaceae que ha sido aislada tanto de muestras de animales como medioambientales, sin embargo raramente produce enfermedad en seres humanos. Tiene resistencia natural a varios antibióticos y se ha encontrado que tiene enzimas capaces de hidrolizar la mayoría de los antibióticos utilizados en la práctica clínica, lo cual hace un verdadero tratar una infección por este microorganismo en humanos. Aquí reportamos el primer caso de infección por Leclercia adecarboxylata luego de un trauma con material vegetal, en un paciente de 69 años y con una diabetes tipo 2, mal controlada.
Abstract Leclercia adecarboxylata is a member of the Enterobacteriaceae family that has been isolated from several environmental and animal specimens, however it rarely causes diseases in human beings. It has natural resistance to several antibiotics, and has shown the ability to harbor and produce enzymes capable of hydrolyzing most of the antibiotics used in daily clinical practice, making its treatment a challenge when a strain with such characteristics causes disease. Here we report the first known case of infection by Leclercia adecarboxylata after a trauma with plant material, in a 69-year-old male patient, with poorly controlled Diabetes Mellitus type 2.

Resumo Até 2010, o conhecimento sobre a diversidade de fungos do Brasil estava registrado em publicações esparsas de taxonomia e ecologia e em algumas poucas listas de espécies. Com a publicação do Catálogo de Plantas e Fungos do Brasil, e a disponibilização da lista online, tem sido possível agregar o conhecimento disperso. A versão ora apresentada acrescenta 2.111 nomes de espécies aos 3.608 listados em 2010. São citadas 5.719 espécies de fungos distribuídas em 1.246 gêneros, 102 ordens e 13 divisões, consistindo em considerável aumento em relação a 2010, quando estavam registrados 924 gêneros e 78 ordens. Predominam os Basidiomycota (2.741 espécies, em 22 ordens) e Ascomycota (1.881 espécies, em 41 ordens). A Mata Atlântica possui a maior quantidade de registros, com 3.017 espécies, seguido pela Amazonia (1.050), Caatinga (999), Cerrado (638) e Pampa e Pantanal com 84 e 35 espécies, respectivamente. A região Nordeste tem a maior riqueza (2.617 especies), seguida pelo Sudeste (2.252), Sul (1.995), Norte (1.301) e Centro Oeste (488 espécies). Em relação aos Estados da Federação, São Paulo (1.846 espécies), Pernambuco (1.611) e Rio Grande do Sul (1.377) são os mais diversos.
Abstract Knowledge about the Brazilian fungal diversity was, until 2010, recorded in few taxonomy and ecology publications, as well as in a handful of species lists. With the publication of the Catálogo de Plantas e Fungos do Brasil and the continued availability of an online list, it has been possible to aggregate this dispersed knowledge. The version presented here adds 2,111 species names to the 3,608 listed in 2010. A total of 5,719 species of fungi distributed in 1,246 genera, 102 orders and 13 phyla represents a considerable increase over the last five years, when only 924 genera and 78 orders were registered. Basidiomycota (2,741 species in 22 orders) and Ascomycota (1,881 species in 41 orders) predominate over other groups. The Atlantic Rainforest has the largest number of records, with 3,017 species, followed by Amazon Rainforest (1,050), Caatinga (999), Cerrado (638) and Pampa and Pantanal with 84 and 35 species, respectively. The Northeast region has the greatest richness (2,617 species), followed by Southeast (2,252), South (1,995), North (1,301) and Central-West (488 species). Regarding the States of the Federation, São Paulo with 1,846 species, Pernambuco with 1,611 and Rio Grande do Sul with 1,377 species are the most diverse.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Chemical modification of cassava starch was conducted through an acylation reaction by using pyridine and propionic anhydride to replace the functional groups of starch. Polyurethane elastomers were prepared using suspensions of the mixture obtained from castor oil and yucca starch that was modified by a propionic anhydride reaction. The suspensions were characterized by means of tests based on The Fourier Transform Infrared Spectroscopy and the Hydroxyl Index. The castor oil-AMP suspensions were used for the PU synthesis. The PUs were characterized by their physical-mechanical properties like tension- deformation and Shore A. hardness, thermal gravimetric analysis and swelling test. The density cross-linking of from swelling tests was determined by applying the Flory-Rehner equation.

Um catalisador baseado em sílica quimicamente modificada com tetrametilguanidina (TMG) foi sintetizado pela co-condensação de tetraetilortossilicato (TEOS) com um novo agente sililante, preparado pela reação entre a molécula TMG e (3-cloropropil)trimetoxisilano. O direcionador n-dodecilamina foi utilizado para organizar a polimerização do catalisador inorgânico-orgânico. A termogravimetria mostrou que o número de sítios ativos do catalisador foi de 1,35 mmol g-1, com uma área superficial de 811 ± 75 m² g-1. A espectroscopia na região do infravermelho e de ressonância magnética nuclear no estado sólido para os núcleos 29Si e 13C confirmou a obtenção do produto desejado. Este material foi usado para catalisar a adição de nitrometano em ciclopentenona. 98% de conversão foi observada depois de 3 h de reação. O catalisador foi recuperado e reutilizado por mais catorze vezes, mantendo uma eficiência catalítica em torno de 98%.
A catalyst based on silica chemically modified with tetramethylguanidine (TMG) was synthesized by the co-condensation of tetraethylorthosilicate (TEOS) with a new silylant agent derived from the reaction between the TMG molecule and (3-chloropropyl)trimethoxysilane. A neutral n-dodecylamine template was used to organize the polymerization of the inorganic-organic catalyst. Thermogravimetry showed that the number of active pendant groups in the catalyst was 1.35 mmol g-1, with a surface area of 811 ± 75 m² g-1. Infrared spectroscopy and 13C and 29Si nuclear magnetic resonance data are in agreement with the proposed structure. This material has been used to catalyse the addition of nitromethane to cyclopentenone. The catalytic efficiency was followed and the nitromethylcyclopentanone conversion presented a yield of 98% at 3 h of reaction. The catalyst was recovered and reused 14 times, maintaining about 98% of its catalytic efficiency.

RACIONAL: Ainda não está estabelecida a melhor terapêutica anti-H. pylori. OBJETIVO: Avaliar a erradicação de H.pylori usando tetraciclina e furazolidona versus amoxicilina e azitromicina em terapia tríplice com lansoprazol no nordeste do Brasil. PACIENTES E MÉTODOS: Cento e quatro pacientes infectados por H. pylori, diagnosticado através do teste rápido da urease e histologia, foram selecionados aleatoriamente para receber: lansoprazol (30 mg q.d.), tetraciclina (500 mg q.i.d.), furazolidona (200 mg t.i.d.) por 7 dias (LTF; n = 52); ou lansoprazol (30 mg b.i.d.) e amoxicilina (1 g b.i.d.) por 1 semana, mais azitromicina (500 mg q.d.) nos primeiros 3 dias (LAAz; n = 52). A erradicação de H.pylori foi avaliada 3 meses após término da terapia através do teste da urease, histologia e teste respiratório usando uréia marcada com 14Carbono. RESULTADOS: A erradicação do H. pylori foi atingida em 46 de 52 (88.4%, 95% CI: 77.5%-95.1%) pacientes no grupo LTF e em 14 de 52 (26.9%, 95% CI: 16.2%-40,1%) pacientes no grupo LAAz. Na análise per-protocolo, a taxa de erradicação foi de 91.8% (95% CI: 81.4%-97.3%) e 28.5% (95% CI: 17.2%-42.3%), respectivamente no grupo de LTF e LAAz. CONCLUSÃO: O esquema com LAAz ofereceu taxas de erradicação inaceitáveis. Por outro lado, o esquema com LTF representa alternativa adequada para erradicação de H.pylori.
BACKGROUND: Optimal anti-Helicobacter pylori treatment has not yet been established. AIM: To evaluate H. pylori eradication using tetracycline and furazolidone versus amoxicillin and azithromycin in lansoprazole based triple therapy in northeastern of Brazil. PATIENTS AND METHODS: One hundred and four patients with H. pylori infection, as determined by rapid urease testing and histology, were randomly assigned to receive either: lansoprazole (30 mg q.d.), tetracycline (500 mg q.i.d.), and furazolidone (200 mg t.i.d.) for 7 days (LTF; n = 52); or lansoprazole (30 mg b.i.d.) and amoxicillin (1 g b.i.d.) for 1 week, plus azithromycin (500 mg q.d.) for the first 3 days (LAAz; n = 52). H. pylori eradication was assessed 3 months following completion of therapy by means of rapid urease testing, histology and a 14C-urea breath test. RESULTS: H. pylori eradication was achieved in 46 of 52 (88.4%, 95% CI: 77.5%-95.1%) patients in LTF group and in 14 of 52 (26.9%, 95% CI: 16.2%-40,1%) patients in LAAz group. On a per-protocol analysis, eradication rates were 91.8% (95% CI: 81.4%-97.3%) and 28.5% (95% CI: 17.2%-42.3%), respectively in LTF and LAAz groups. CONCLUSION: The LAAz regimen yielded unacceptably low eradication rates. On the other hand, the LTF scheme represents a suitable alternative for H. pylori eradication.

Relações granulométricas no processo de brunimento de arroz

O brunimento é uma das etapas de beneficiamento do arroz que tem grande importância sobre o rendimento de engenho. Neste trabalho, analisaram-se relações granulométricas de frações de grãos na massa de arroz e seus efeitos sobre o rendimento de engenho, durante o processo de brunimento. As amostras usadas foram 78 g de arroz descascado, tendo essas partido de amostras originais de 100 g. Foram testadas cinco relações de composição de amostras a serem brunidas, variando as quantidades de grãos quebrados e de quirera que acompanhavam a massa de grãos inteiros, que foram: 1) somente grãos inteiros; 2) 6,16 g de quebrados e 1,04 g de quirera, conforme amostra original; 3) 3,60 g de quebrados e 3,60 g de quirera adicionados aos inteiros; 4) 7,20 g de quebrados adicionados aos inteiros, e 5) 7,20 g de quirera adicionada aos inteiros. Os procedimentos foram executados com uso de engenho de provas e com classificação manual. O rendimento de engenho foi significativamente superior, após a operação de brunimento de arroz, quando existem, na massa a ser brunida, grãos quebrados e quirera, variando entre 3,60 g a 7,20 g e 1,04 g a 7,20 g, respectivamente, comparada ao brunimento da massa composta por 100% de grãos inteiros.
Milling yield using five different rates of broken and head rice was tested. The samples were arranged to have different amounts of big and small broken kernels in the mass of grains. The size of each fraction of broken grains was established according to the Brazilian Rules of Rice Classification. The paddy sample had 100 g and the brown rice sample had 78 g. The brown rice was milled by adding different proportions of broken kernels to make five treatments as follows: 1) only whole kernels; 2) 6.16 g of big broken kernels and 1.04 g of small broken kernels were used as they appear in the original lot; 3) 3.6 g of big broken kernels and 3.6 g of small broken kernels were added to the whole kernels; 4) 7.2 g of big broken kernels were added to the whole kernels and 5) 7.2 g of small broken kernels were added to the whole kernels. A laboratory mill was used. The classification was made manually. The percentages of broken kernels varied from 3.6% to 7.2% in the fraction of big broken kernels and from 1.04% to 7.2% in the fraction of small broken kernels, respectively. The results showed that, in the milling process using broken kernels in the mass, the head rice yield was significantly higher than when 100% of head kernels were milled.
