RESUMO Objetivo: Realizar a tradução e validação do inglês para o português do Brasil da escala Modified Checklist for Autism in Toddlers, Revised with Follow-Up para triagem precoce do autismo, respeitando a equivalência transcultural. Métodos: Foi obtida permissão da autora da escala e realizaram-se tradução, retrotradução, avaliação da equivalência referencial e geral, avaliação de especialistas em autismo infantil, elaboração da versão preliminar, pré-teste, elaboração da versão final e validação com população-alvo. Resultados: Apenas uma das questões não foi 100% semelhante na avaliação da equivalência referencial. Das 10 pessoas interrogadas no pré-teste, todas compreenderam a maior parte do instrumento, contudo houve algumas sugestões de substituição de termos e exemplos para facilitar a compreensão. Após a incorporação das sugestões pertinentes, foi elaborada a versão final, que, submetida à validação com a população-alvo, indicou sensibilidade de 88,2% para ponto de corte maior que 2 pontos. Conclusões: O estudo torna disponível a versão em português da escala Modified Checklist for Autism in Toddlers, Revised with Follow-Up, considerada adequada por especialistas e compreensível pela população.

ABSTRACT Objective: To translate and validate the Modified Checklist for Autism in Toddlers, Revised with Follow-Up (M-CHAT-R/F) from English to Brazilian Portuguese, taking transcultural differences into account. Methods: Permission for the translation was obtained from the author of the scale. Translation and back-translation were performed, and the document was then assessed for reference and general equivalence. Specialists in childhood autism evaluated the scale. A preliminary version was prepared and pre-tested in a sample population, and a final version was validated with the target population. Results: Only one question had issues relating to referential equivalence. The 10 individuals questioned in the pre-test all understood most of the instrument, although some suggested substituting certain terms to improve comprehension. The final version was reached following inclusion of pertinent suggestions and was submitted to validation with the target population, indicating a sensitivity of 88.2% for a cutoff point greater than 2 points. Conclusions: A Brazilian version of the M-CHAT-R/F scale, approved by specialists and understandable by the target audience, is now available for use.

The aim of this study was to describe the partial cross-cultural adaptation of the ABC scale to Brazilian Portuguese. The process included conceptual and item equivalence, two translations (T1, T2) and their back-translations (R1, R2), evaluation of referential and general equivalence, expert evaluations, a pre-test, and elaboration of the final version. Conceptual and item equivalences of the ABC were considered pertinent to Brazilian culture. Semantic equivalence showed good correspondence between R1 items and ABC. Reasonable correspondence was obtained between ABC items and R2. All of the professors understood 94.8% of the items in the scale, while relatives understood 87.9%. Tanto o DSM-IV quanto a CID-10 excluem o diagnóstico de transtorno de déficit de atenção e hiperatividade (TDAH) em pacientes autistas. A literatura, no entanto, sugere que o autismo pode estar associado a outras doenças, dentre elas o TDAH. OBJETIVO: Estimar a freqüência de sintomas de TDAH em autistas. MÉTODO: As crianças foram selecionadas de uma escola especializada em crianças com transtorno do espectro autista (TEA); todas tinham um diagnóstico ou suspeita diagnóstica prévia de autismo. O instrumento de avaliação aplicado para o diagnóstico de TDAH foi a versão brasileira do KIDDIE-SADS PL. O diagnóstico de autismo de acordo com os critérios do DSM-IV foi feito através de entrevista com os pais e observação da criança. RESULTADOS: 32 crianças foram incluídas no estudo. Os resultados indicam que 53,1% dos pacientes apresentaram sintomas compatíveis com o diagnóstico de TDAH e de autismo, enquanto que 56,9% dos autistas não preenchiam critérios para TDAH. CONCLUSÃO: Os achados sugerem uma elevada freqüência de sintomas de TDAH em pacientes com autismo. Elucidar se estamos diante de uma comorbidade ou de um fenótipo diferente de autismo pode contribuir para uma melhor abordagem farmacoterápica dessas crianças.

BACKGROUND: Both DSM-IV and the ICD-10 exclude diagnosis of attention deficit/hyperactivity disorder (ADHD) when autism diagnostic is present. Some authors suggest, however, that autism can be associated to other comorbidity amongst which the ADHD. OBJECTIVE: To estimate prevalence of ADHD in children with autism. METHOD: Children were selected from a specialized school, all of then had previous diagnosis or diagnostic suspicion of autism. The Brazilian version of the KIDDIE-SADS PL was applied to parents for diagnostic of ADHD. DSM-IV diagnostic of autism was based on parents' interview and child observation. RESULTS: 32 children were included in the study. Results show that 53.1% of the ASD child had ADHD symptoms enough to fulfill DSM-IV diagnostic criteria, whereas 56.9% did not fulfill DSM-IV criteria for ADHD. CONCLUSION: Results suggest a high frequency of ADHD symptoms in ASD patients. Elucidating if we are facing a comorbity or an autism distinct phenotype can contribute for a more adjusted pharmacotherapy approach for these children.

OBJETIVO: Este estudo visa realizar uma revisão de literatura sobre a terapia farmacológica na depressão pós-AVE. MÉTODO: Foi feita uma revisão nos bancos de dados MedLine e SciELO, utilizando-se como descritores primários "stroke", "depression" e "treatment", incluindo artigos publicados entre 1996 e 2008. RESULTADOS: Treze artigos foram selecionados. Foram encontrados dez artigos que apresentaram terapias farmacológicas eficazes no tratamento da depressão pós-AVE e três em que as terapias farmacológicas utilizadas não trouxeram benefício para a depressão dos grupos em estudo. CONCLUSÃO: O manejo farmacológico da DPAVE pode ser realizado de maneira profilática ou terapêutica. Em ambas as modalidades, os inibidores de recaptação seletiva são as medicações mais adequadas, destacando-se a fluoxetina e, em pacientes adequadamente selecionados, a reboxetina e o citalopram. A nortriptilina, antidepressivo tricíclico, é uma alternativa com relativa eficácia na conduta da DPAVE.

INTRODUCTION: Poststroke depression (PSD) is one of the most frequent psychiatric sequelae in populations affected by stroke. Effective drug therapy is essential in the proper management of patients. OBJECTIVE: This study aims to conduct a review of the literature on pharmacological therapy in PSD. METHOD: A review was made in databases MedLine and SciELO using as primary descriptors "stroke", "depression" and "treatment", including articles published between 1996 to 2006. RESULTS: Thirteen articles were selected. Ten trials were found that described effective pharmacological therapies in the treatment of the PSD. CONCLUSION: The pharmacological management of PSD can be done prophylactic or therapeutically. In both methods, selective reuptake inhibitors, particularly fluoxetine, and in some instances, citalopram and reboxetine, seem to be the most appropriate medications to be used in PSD. Alternatively, nortriptyline, a tricyclic antidepressant, may be employed in some cases of PSD.

INTRODUÇÃO: A escala Modified Checklist for Autism in Toddlers (M-CHAT) é um instrumento de rastreamento precoce de autismo, que visa identificar indícios desse transtorno em crianças entre 18 e 24 meses. Deve ser aplicada nos pais ou cuidadores da criança. É auto-aplicável e simples, e apresenta alta sensibilidade e especificidade. Foi desenvolvida no idioma inglês e ainda não está disponível uma versão em português. Na literatura não existe consenso quanto à técnica de tradução, sendo a adaptação transcultural uma das formas possíveis. O objetivo do presente estudo foi realizar a tradução do inglês para o português do Brasil da escala M-CHAT para rastreio precoce do autismo, respeitando a equivalência transcultural. MÉTODO: Foi obtida permissão da autora da escala, realizada tradução, seguida de sua respectiva back-translation, avaliação da equivalência referencial, primeiro pré-teste em amostra da população-alvo, avaliação da equivalência geral, avaliação de especialistas em autismo infantil, elaboração da versão preliminar, segundo pré-teste em pais de crianças de ambulatório de pediatria do SUS e elaboração da versão final. RESULTADOS: A avaliação da equivalência referencial demonstrou que 78% das questões eram semelhantes, 13% aproximadas e 9% diferentes. Das 20 pessoas interrogadas no primeiro pré-teste 9 entenderam 100% das questões. O segundo pré-teste demonstrou boa aceitação e entendimento pela população-alvo, com 70% dessa sem nenhuma queixa. Após as avaliações pormenorizadas foi elaborada a versão final. CONCLUSÃO: O estudo torna disponível a versão em português da escala M-CHAT, considerada adequada por especialistas e compreensível pela população.

INTRODUCTION: The Modified Checklist for Autism in Toddlers (M-CHAT) is a screening instrument for autism that can be applied to parents of children aged 18-24 months. It is self-applicable, simple, and has high sensibility and specificity. It was developed in English and it is still not available in Portuguese. In the scientific literature there is no agreement as to the translation technique, cross-cultural adaptation being one of the possibilities. The objective of the present study was to translate the M-CHAT scale from English into Brazilian Portuguese to scan early autism, preserving cross-cultural equivalence. METHOD: After obtaining permission from the author of the scale, its translation was performed, followed by its back-translation; evaluation of reference equivalence; first pretest in a sample of the target population; evaluation of general equivalence; evaluation by specialists in infantile autism; development of a preliminary version; second pretest with parents of autistic children treated in a Pediatric Ambulatory of the Brazilian Public Health System; and development of its final version. RESULTS: Evaluation of reference equivalence showed that 78% of the questions were similar, 13% approximate, and 9% different. Of the 20 people interviewed in the first pretest, nine understood 100% of the questions. The second pretest demonstrated good acceptance and understanding by the target population, with no complaints in 70% of the sample. After detailed evaluations, the final version was developed. CONCLUSION: The study makes the Portuguese version of the M-CHAT scale available, which was considered adequate by specialists and understood by the population.
