ABSTRACT Understorey herbs form a diverse and understudied plant assemblage in tropical forests. Although several studies and research teams have long been dedicated to the study of this conspicuous vegetation component in Amazonia, no effort to unify the data has been undertaken to date. In contrast to trees and other life forms for which major data compilations already exist, a unified database dedicated to herbs is still lacking. Part of the problem is in defining what is a herb and how to effectively sample herb assemblages. In this article, we describe the database HERBase, an exhaustive compilation of published and unpublished data on herb inventories in Amazonia. We also describe the structure, functioning, and guidelines for data curation and integration in HERBase. We were able to compile information from 1381 plots from all six Amazonian geographic regions. Based on this dataset, we describe and discuss sampling and knowledge gaps, priority areas for new collections, and recommend sampling protocols to facilitate data integration in the future. This novel database provides a unique biodiversity data repository on understorey herbs that will enable new studies on community ecology and biogeography.

RESUMO As ervas do sub-bosque formam um componente diversificado e pouco estudado em florestas tropicais. Embora vários estudos e grupos de pesquisa tenham se dedicado ao estudo desse componente conspícuo na Amazônia, nenhum esforço foi feito até o momento para unificar essas informações. Em contraste com árvores e outros grupos de plantas para os quais já existem grandes compilações de dados, uma base de dados unificada dedicada às ervas ainda não existe. Parte do problema está em definir o que é uma erva e como amostrar comunidados de ervas de forma eficiente. Neste artigo descrevemos a base de dados HERBase, uma compilação exaustiva de dados publicados e não publicados sobre inventários de ervas na Amazônia. Também descrevemos a estrutura, funcionamento e diretrizes para curadoria e integração de dados na HERBase. Conseguimos compilar informações de 1381 parcelas de todas as seis regiões geográficas amazônicas. Com base nesses dados, descrevemos e discutimos lacunas de amostragem e conhecimento, apontamos áreas prioritárias para novas coletas e recomendamos protocolos de amostragem para facilitar a integração de dados no futuro. Essa nova base de dados fornece dados únicos de biodiversidade sobre ervas do sub-bosque que permitirão novos estudos sobre ecologia e biogeografia de comunidades.

Everolimus as a therapeutic option in refractory epilepsy in children with tuberous sclerosis: a systematic review

Abstract Background Tuberous sclerosis (TS) is a multisystem genetic disease in which epilepsy is a frequent manifestation and is often difficult to control. Everolimus is a drug with proven efficacy in the treatment of other conditions related to TS, and some evidence suggests that its use benefits the treatment of refractory epilepsy in these patients. Objective To evaluate the efficacy of everolimus in controlling refractory epilepsy in children with TS. Methods A literature review was conducted in the Pubmed, BVS, and Medline databases, using the descriptors Tuberous sclerosis, Children, Epilepsy, and Everolimus. Original clinical trials and prospective studies published in Portuguese or English in the last decade that evaluated the use of everolimus as an adjuvant therapy in the control of refractory epilepsy in pediatric patients with TS were included. Results Our search screened 246 articles from electronic databases, 6 of which were chosen for review. Despite the methodological variations between the studies, most patients benefited from the use of everolimus to control refractory epilepsy, with response rates ranging from 28.6 to 100%. Adverse effects were present in all studies leading to dropouts of some patients; however, the majority were of low severity. Conclusion The selected studies suggest a beneficial effect of everolimus in the treatment of refractory epilepsy in children with TS, despite the adverse effects observed. Further studies involving a larger sample in double-blind controlled clinical trials should be performed to provide more information and statistical credibility.

Resumo Antecedentes A esclerose tuberosa (ET) é uma doença genética multissistêmica na qual a epilepsia é a manifestação neurológica mais frequente, sendo muitas vezes de difícil controle. O everolimo é uma droga com eficácia comprovada no tratamento de outras condições relacionadas à ET, e indícios sugerem benefícios de seu uso também no controle da epilepsia refratária nesses pacientes. Objetivo Avaliar a eficácia do everolimo no controle da epilepsia refratária em crianças com ET. Métodos Revisão de literatura nas bases de dados Pubmed, BVS e Medline, utilizando os descritores Tuberous sclerosis, Children, Epilepsy e Everolimus. Incluíram-se ensaios clínicos originais e estudos prospectivos publicados em português ou inglês na última década e que avaliassem o uso do everolimo como terapia adjuvante no controle da epilepsia refratária em pacientes pediátricos com ET. Resultados Nossa busca rastreou 246 artigos nas bases de dados, dos quais 6 foram escolhidos para a revisão. Apesar das variações metodológicas entre os estudos, a maioria dos pacientes tiveram benefício no uso do everolimo para controle da epilepsia refratária, com taxas de resposta variando entre 28.6 e 100%. Os efeitos adversos estiveram presentes em todos os estudos, levando à desistência de alguns pacientes, contudo a maioria foi de baixa gravidade. Conclusão Os estudos selecionados sugerem efeito benéfico do everolimo no tratamento da epilepsia refratária em crianças com ET, apesar dos efeitos adversos observados. Novos estudos envolvendo uma amostra maior em ensaios clínicos controlados duplo-cegos devem ser realizados para fornecer mais informações e credibilidade estatística.

ABSTRACT Objective To evaluate the effectiveness of the levonorgestrel intrauterine system in the treatment of patients with heavy menstrual bleeding and an enlarged uterus and to compare satisfaction and its complications with hysterectomy. Methods This was a comparative cross-sectional observational study of women with heavy menstrual bleeding and an enlarged uterus. Sixty-two women were treated and followed up for four years. Insertion of the levonorgestrel intrauterine system was performed in Group 1, and laparoscopic hysterectomy was performed in Group 2. Results In Group 1 (n=31), 21 patients (67.7%) showed improvement in the bleeding pattern, and 11 patients (35.5%) had amenorrhea. Five patients (16.1%) remained with heavy bleeding and were considered to have experienced treatment failure. There were seven expulsions (22.6%); in five patients, bleeding remained heavy, but in two patients the bleeding returned to that of normal menstruation. No relationship was found between treatment failure and greater hysterometries (p=0.40) or greater uterine volumes (p=0.50), whereas expulsion was greater in uteri with smaller hysterometries (p=0.04). There were 13 (21%) complications, seven (53.8%) in the group that underwent insertion of the levonorgestrel intrauterine system (all were device expulsions), and six (46.2%) in the surgical group, which were the most severe ones (p=0.76). Regarding satisfaction, 12 patients (38.7%) were dissatisfied with the levonorgestrel intrauterine system and one (3.23%) was dissatisfied with the surgical treatment (p=0.00). Conclusion Treatment with the levonorgestrel intrauterine system in patients with heavy menstrual bleeding and an enlarged uterus was effective, and when compared with laparoscopic hysterectomy, it had a lower rate of satisfaction and the same rate of complications, although less severe.

Abstract Objectives This is a double-blind, split-mouth, randomized clinical study that aims to evaluate the influence of bulk-fill composite packaging presented in syringes (BSy) and capsules (BCa), and the effect of selective enamel etching (SEE) on the clinical performance of class I and II bulk-fill resin composite restorations after 24 months. Methodology A total of 295 class I or class II restorations were performed on 70 patients. One universal adhesive was applied in all restorations. SEE was used in 148 restorations and self-etching mode (SET) in 147 restorations. After the adhesive application, cavities were restored with Filtek Bulk-fill Posterior Restorative in syringes (BSy), Filtek One Bulk-fill in capsules (BCa), or Filtek Supreme Ultra in syringes with the incremental technique (In). All restorations were evaluated using the FDI criteria after one week and after six, 12, and 24 months. Kaplan-Meier survival analysis and Pearson’s Chi-square test were used (α=0.05) for statistical analysis. Results After 24 months, 62 patients were evaluated and four restorations were lost due to fracture (one for SEEBSy, two for SEEIn, and one for SETIn). No significant differences in the fracture and retention rate were found between groups (p>0.05). SEE showed significantly fewer marginal adaptation defects than SET (p<0.05). BCa and BSy groups showed fewer marginal discrepancies compared to In (p<0.05). Restorations performed with BCa showed less color mismatch than BSy or In (p<0.05). Conclusion Although all restorations exhibited satisfactory clinical performance after 24 months of clinical service, the clinical behavior of class I and II restorations’ improved when performed with a bulk-fill composite in capsules, mainly when associated with a universal adhesive applied with SEE.

ABSTRACT In the Cerrado, the palm swamps ( veredas ) are characterized by being humid and stable environments that lead to the formation of Histosols ( Organossolos ). and soils with surface horizons of organic constitution, which are fragile and sensitive to anthropic action. This study aimed to evaluate the impact of anthropization (recurrent forest fires and livestock farming) on the chemical, physical and morphological properties of soils in two palm swamps in the Environmental Preservation Area (EPA) of Pandeiros River, Minas Gerais, namely: Água Doce, in preserved condition, and Taboa, in anthropized condition. Four soil profiles were morphologically described, two profiles in each palm swamp, with subsequent chemical and physical analyses, calculations of organic carbon stock and identification of the origin of organic matter. The results were analyzed using descriptive statistics and Pearson’s correlation coefficient. Soil morphological properties were influenced by vegetation cover, drainage and anthropization conditions. As for the physical and chemical properties, adequate values were observed in the preserved palm swamp, including lower bulk density values and higher cation contents. Anthropic actions in the anthropized palm swamp caused degradation of soils, revealed by subsidence, reduction in organic carbon content, increase in bulk density and decrease in fertility. Changes promoted in the soils of the palm swamps compromise ecosystem services, indicating that actions at either local or governmental level should be stimulated for the preservation and conservation of these environments.

ABSTRACT Objective: To identify the lingual foramina in a Brazilian population of 210 individuals through Cone-Beam Computed Tomography images, in order to guide the installation of dental implants in the region of the mental symphysis. Methods: After identifying the lingual foramina on a parasagittal section, four measurements were taken: distance to the alveolar ridge, distance to the mandibular lower border, distance to the limit of the vestibular cortical, and alveolar ridge inclination angle. The measurements were compared in relation to gender by the Mann-Whitney test. The association between gender and the number of foramina was done by the chi-square test. Pearson’s correlation analyzed the linear relationship between age and number of foramina. Results: Lingual foramina are present in 99.6% of the research participants. In the upper-inferior direction, a mean distance to the alveolar ridge of 21.4 mm was noted, while the mean distance to the lower mandibular border was 13.1 mm. In the bucco-lingual direction, a mean distance from the lingual foramina to the vestibular cortical of 15.2 mm is noted. The inclination angle of the alveolar ridge had a mean of 25.4º. Regarding gender, the test was not significant only for the distance to the vestibular cortical. Conclusion: The lingual foramina had a descending way in 100% of cases, being positioned mainly in the middle thirds (66.5%) and lower thirds (32.7%) of the mandibular ridge height. The use of dental implants up to 13 mm in length and up to 4 mm in diameter in the region of the symphysis mentualis is recommended.

RESUMO Objetivos: Identificar as foraminas linguais em uma população brasileira de 210 indivíduos, por meio de imagens de tomografia computadorizada de feixe cônico, a fim de orientar a instalação de implantes dentários em região de sínfise mentual. Métodos: Após identificação das foraminas linguais em corte parasagital, foram realizadas quatro medidas: distância até à crista alveolar, distância até à borda inferior da mandíbula, distância até o limite da cortical vestibular e ângulo de inclinação do rebordo alveolar. As medidas foram comparadas em relação ao sexo pelo teste Mann-Whitney. A comparação entre os sexos e o número de foraminas foi feito pelo teste qui-quadrado. A correlação de Pearson analisou a relação linear entre idade e número de foraminas. Resultados: As foraminas linguais estão presentes em 99,6% dos indivíduos. No sentido súpero-inferior, nota-se uma distância média até à crista alveolar de 21,4 mm, enquanto a distância média até à borda inferior da mandíbula foi de 13,1 mm. No sentido vestíbulo-lingual, nota-se uma distância média da foramina lingual até a cortical vestibular de 15,2 mm. O ângulo de inclinação do rebordo alveolar apresentou uma média de 25,4º. Em relação ao sexo, o teste só não foi significativo para a distância até a cortical vestibular. Conclusão: As foraminas linguais apresentam um trajeto descendente em 100% dos casos, posicionando-se, principalmente, nos terços médios (66,5%) e terços inferiores (32,7%) da altura do rebordo mandibular. Recomenda-se o uso de implantes dentários de até 13 mm de comprimento e de até 4mm de diâmetro em região de sínfise mentual.

ABSTRACT Astrocaryum aculeatum Mart. Moreover, Astrocaryum acaule Mart. are palm trees with ecological and extractive importance in the Amazon. These are hearty species that have been associated with archaeological sites and thrive in the presence of humans in certain areas. This work aimed to verify the effect of global climate change on the potential geographic distribution of A. acaule and A. aculeatum in the current period and future climate scenarios using ecological niche modeling in Brazilian phytogeographic domains. The modeling was based on 19 bioclimatic variables obtained from the Worldclim website and four algorithms (Climate space model, Envelope Score, Niche Mosaic, and Environmental Distance). Additionally, the Environmental Distance algorithm showed greater similarity regarding species distribution with potential occurrence in the five Brazilian domains (Amazon, Pantanal, Caatinga, Cerrado, and Atlantic Forest). The dispersion patterns were very similar between the two Astrocaryum palms though A. aculeatum was more sensitive to climatic variations. A. acaule may be more resilient to changes, as demonstrated by being able to recolonize in the southern portion of the Amazon in future scenarios in the year 2070. The modeling helped to delimit potential areas for A. aculeatum and A. acaule, indicating the need for the conservation of the species in more sensitive regions.

RESUMO Astrocaryum aculeatum Mart. e Astrocaryum acaule Mart. são palmeiras que apresentam importância ecológica e para o extrativismo na Amazônia. São espécies rústicas e que foram associadas a sítios arqueológicos e à presença de humanos em determinadas áreas. O objetivo deste trabalho foi verificar o efeito das mudanças climáticas globais na distribuição geográfica potencial de A. acaule e A. aculeatum, no período atual e em cenários climáticos futuros, utilizando a modelagem de nicho ecológico nos domínios fitogeográficos brasileiros. As modelagens foram baseadas em 19 variáveis bioclimáticas, obtidas no site do Worldclim, e quatro algoritmos (Climate space model, Envelope Score, Niche Mosaic e Enviromental Distance), dos quais o algoritmo Environmental Distance apresentou maior similaridade quanto à distribuição das espécies com ocorrência potencial nos cinco domínios brasileiros (Amazônia, Pantanal, Caatinga, Cerrado e Mata Atlântica). Os padrões de dispersão foram muito similares entre as duas palmeiras de Astrocaryum, porém A. aculeatum é mais sensível às variações climáticas. A. acaule tem potencial para ser mais resiliente às mudanças, podendo recolonizar a porção sul da Amazônia em cenários futuros, no ano de 2070. A modelagem auxiliou a delimitar as áreas potenciais de A. aculeatum e A. acaule, indicando a necessidade de conservação das espécies em regiões mais sensíveis.

ABSTRACT Objective: To describe contents, structure and origin of social representations about falls by elderly people, the peridomiciliary structural conditions that predispose to falls, and to relate the implications of these empirical evidence on the routine of the elderly in the architectural context. Method: Convergent mixed method by triangulation. Qualitative approaches(structural, n=195 and procedural, n=40of the Theory of Social Representations) and quantitative (descriptive sectional, n=183) were used. Elderly people enrolled in primary care were interviewed at home in 2018. Analysis techniques: categorical-thematic, prototypical, statistical, and deductive according to Leininger. Results: Categories of analysis: 1) Peridomicile: fall scenario and 2) Aging and vulnerability: risk of falls in peridomicile. The following environmental characteristics were precursors to falls: uneven floors, holes, unevenness and objects in the pathway. Feelings and behaviors allocated in the possible central core are associated, justifying falls, and determining their causes. Conclusion: There was an association between the peridomiciliary architectural environment and the predictive characteristics of the risk of falls.

RESUMEN Objetivo: Describir contenido, estructura y origen de las representaciones sociales sobre las caídas de los ancianos, las condiciones estructurales peridomiciliares que predisponen a las caídas; y relacionar las implicaciones de estas evidencias empíricas sobre el cotidiano de los ancianos en el contexto arquitectónico. Método: Mixto convergente por triangulación. Enfoque cualitativo: estructural (n=195) y procedimental (n=40) de la Teoría de las Representaciones Sociales y cuantitativo: descriptivo seccional (n=183). Se entrevistó en domicilio a ancianos inscritos en atención primaria (2018). Técnicas de análisis: categórico-temático, prototípico, estadístico y deductivo según Leininger. Resultados: Categorías de análisis: 1) Peridomicilio: escenario de caída y 2) Envejecimiento y vulnerabilidad: riesgo de caídas en peridomicilio. Las siguientes características ambientales fueron precursoras de caídas: pisos irregulares, agujeros, desniveles y objetos en el camino. Se asocian sentimientos y comportamientos alojados en el posible núcleo central, justificando las caídas y apuntando a sus causas. Conclusión: Hubo asociación entre el ambiente arquitectónico peridoméstico y las características predictivas del riesgo de caídas.

RESUMO Objetivo: Descrever conteúdos, estrutura e origem das representações sociais sobre queda apresentadas por pessoas idosas e condições estruturais peridomiciliares que predispõem à queda e relacionar as implicações dessas evidências empíricas na rotina de idosos no contexto arquitetônico. Método: Misto convergente por triangulação. Utilizaram-se as abordagens qualitativa (estrutural, n=195 e processual, n=40 da Teoria das Representações Sociais) e quantitativa (seccional descritiva, n=183). Foram entrevistados idosos no domicílio adscritos à atenção primária, em 2018. Técnicas de análise: categorial-temática, prototípica, estatística e dedutiva segundo Leininger. Resultados: Categorias de análise: 1) Peridomicílio: cenário de queda e 2) Envelhecimento e vulnerabilidade: risco de queda no peridomicílio. Apresentaram-se como características ambientais precursoras de quedas: pisos irregulares, buracos, desníveis e objetos no percurso. Sentimento e comportamentos alocados no possível núcleo central se associam, justificando as quedas e objetivando suas causas. Conclusão: Evidenciou-se a associação entre o ambiente arquitetônico peridomiciliar e as características preditoras do risco de quedas.

ABSTRACT The importance of non-conventional food plants has been evidenced due to their great potential for phenotypic plasticity, resilience and resistance to permanence in inhospitable places. This study aimed to evaluate the natural distribution of two of these species (Eryngium foetidum and Fridericia chica) in the present period (2009-2019) and the projection for two future climate scenarios (RCP 4.5 - “less pessimistic” and RCP 8.5 - “more pessimistic”) in two-time intervals (2020-2050 and 2051-2070), in the six Brazilian phytogeographic domains. Nineteen bioclimatic variables obtained from the WorldClim database and four algorithm models were tested: Climate Space Model, Envelope Score, Niche Mosaic and Environmental Distance. The Environmental Distance algorithm presented the best discrimination of the models adjusted for the two species. From the projections, it is possible to perceive that the species are severely affected in the phytogeographic domains of the Amazon, Pantanal and Pampa, becoming practically extinct in the RCP 8.5 scenario, for the period of 2051-2070.

RESUMO A importância de plantas alimentícias não convencionais tem sido evidenciada devido ao seu grande potencial de plasticidade fenotípica, resiliência e resistência à permanência em lugares inóspitos. Objetivou-se estudar a distribuição natural de duas espécies dessas plantas (Eryngium foetidum e Fridericia chica) no período presente (2009-2019) e a sua projeção para dois cenários climáticos futuros (RCP 4.5 - “menos pessimista” e RCP 8.5 - “mais pessimista”) em dois intervalos de tempo (2020-2050 e 2051-2070), nos seis domínios fitogeográficos brasileiros. Foram utilizadas 19 variáveis bioclimáticas obtidas da base de dados do WorldClim e quatro modelos de algoritmos foram testados: Climate Space Model, Envelope Score, Niche Mosaic e Environmental Distance. O algoritmo Environmental Distance apresentou a melhor discriminação dos modelos ajustados para as duas espécies. A partir das projeções, é possível perceber que as espécies são severamente afetadas nos domínios fitogeográficos da Amazônia, Pantanal e Pampa, tornando-se praticamente extintas no cenário RCP 8.5 para o período de 2051-2070.

Abstract This study aimed to apply selection indexes in situations in which F2 populations and their parents are evaluated simultaneously in field trials to predict population genotypic values and selecting the best populations. Fifteen F2 soybean populations were evaluated for number of days to flowering and to maturity and grain yield. The data was analyzed using restricted maximum likelihood/best linear unbiased prediction (REML/BLUP) to obtain genotypic effects, variance components and accuracies. Four selection methods were compared: genotypic values of the F2 populations (gF2), combined index with the mean genotypic value of parents and F2 population (Ia) and two indexes that add (to gF2 and to Ia) and consider genetic variability within populations (Ib and Ic). The index Ia presented a result similar to that obtained with the gF2 selection method. Selection indexes (Ib and Ic) based on several sources of information were more efficient than selection based on gF2 values only.

Abstract The Brazilian Practice Guidelines for Stroke Rehabilitation – Part II, developed by the Scientific Department of Neurological Rehabilitation of the Brazilian Academy of Neurology (Academia Brasileira de Neurologia, in Portuguese), focuses on specific rehabilitation techniques to aid recovery from impairment and disability after stroke. As in Part I, Part II is also based on recently available evidence from randomized controlled trials, systematic reviews, meta-analyses, and other guidelines. Part II covers disorders of communication, dysphagia, postural control and balance, ataxias, spasticity, upper limb rehabilitation, gait, cognition, unilateral spatial neglect, sensory impairments, home rehabilitation, medication adherence, palliative care, cerebrovascular events related to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, the future of stroke rehabilitation, and stroke websites to support patients and caregivers. Our goal is to provide health professionals with more recent knowledge and recommendations for better rehabilitation care after stroke.

Resumo As Diretrizes Brasileiras de Reabilitação do Acidente Vascular Cerebral (AVC) - Parte II, desenvolvida pelo Departamento Científico de Reabilitação Neurológica da Academia Brasileira de Neurologia é voltada para intervenções específicas de técnicas de reabilitação de déficits neurológicos e incapacidades. Seguindo o mesmo modelo da Parte I, a Parte II também se baseia em estudos randomizados, revisões sistemáticas, metanálises e outras diretrizes sobre o mesmo tema. A segunda parte aborda os distúrbios da comunicação, disfagia, controle postural e equilíbrio, ataxias, espasticidade, reabilitação do membro superior, marcha, cognição, negligência espacial unilateral, déficits sensoriais, reabilitação domiciliar, aderênciaao usode medicamentos, cuidados paliativos,ofuturodareabilitação no AVC, e websites de orientação sobre AVC para pacientes e cuidadores. Nosso objetivo é fornecer aos profissionais envolvidos na reabilitação conhecimento atualizado e recomendações para um melhor cuidado no pós-AVC.

Resumo Objetivo O presente estudo teve como objetivo desenvolver um modelo matemático útil que prediz a idade na qual a insuficiência ovariana prematura pode ocorrer após a radioterapia externa (teleterapia). O diagnóstico de menopausa prematura ou precoce tem consequências físicas e psicológicas; portanto, as mulheres podem precisar de apoio e acompanhamento médico de longo prazo. Métodos Para correlacionar a dose de radiação ovariana com a função ovariana, foi usada a fórmula descrita por Wallace et al.: √g(z) = 10(2-0,15z), na qual “g(z)” e “z” representam a taxa de sobrevivência do oócito e a dose de radiação (em Gray), respectivamente. Ao simular diferentes idades e doses, observamos um padrão que poderia ser usado para simplificar a relação entre a dose de radiação e o tempo restante da função ovariana. Resultados Obtivemos uma função linear entre a dose de radiação ovariana e a perda da função ovariana (LOF, na sigla em inglês) que é a porcentagem de diminuição no tempo até a falência ovariana em relação ao tempo esperado para uma mulher da mesma idade sem exposição à radiação. Para pacientes<40 anos de idade e com doses de radiação ovariana < 5 Gy, a equação LOF = 2,70 + (11,08 × Dose) pode ser aplicada para estimar a redução no tempo até a insuficiência ovariana. Conclusão O presente estudo relata um método teórico viável para estimar a perda da função ovariana. Estes achados podem melhorar potencialmente o manejo e o aconselhamento de pacientes jovens submetidas à radioterapia durante seus anos reprodutivos.

Abstract Objective The present study aimed to develop a useful mathematical model that predicts the age at which premature ovarian insufficiency might occur after teletherapy radiation. A diagnosis of premature or early menopause has physical and psychological consequences, so women may need support and long-term medical follow-up. Methods To correlate ovarian radiation dose with ovarian function, we used the formula described by Wallace et al.: √g(z) = 10(2-0,15z), where “g(z)” and “z” represent oocyte survival rate and the radiation dose (in Gray), respectively. By simulating different ages and doses, we observed a pattern that could be used to simplify the relationship between radiation dose and remaining time of ovarian function. Results We obtained a linear function between ovarian radiation dose and loss of ovarian function (LOF) that is the percentage of decrease in the time to the ovarian failure compared with the time expected for a woman at the same age without irradiation exposition. For patients <40 years old and with ovarian radiation doses < 5 Gy, the equation LOF = 2.70 + (11.08 × Dose) can be applied to estimate the decrease in time to premature ovarian insufficiency. Conclusion The present study reports a practicable theoretical method to estimate the loss of ovarian function. These findings can potentially improve the management and counseling of young women patients submitted to radiotherapy during their reproductive years.

Abstract Objective: To provide guidelines on the coronavirus disease 2019 (COVID-19) vaccination in patients with immune-mediated rheumatic diseases (IMRD) to rheumatologists considering specific scenarios of the daily practice based on the shared-making decision (SMD) process. Methods: A task force was constituted by 24 rheumatologists (panel members), with clinical and research expertise in immunizations and infectious diseases in immunocompromised patients, endorsed by the Brazilian Society of Rheumatology (BSR), to develop guidelines for COVID-19 vaccination in patients with IMRD. A consensus was built through the Delphi method and involved four rounds of anonymous voting, where five options were used to determine the level of agreement (LOA), based on the Likert Scale: (1) strongly disagree; (2) disagree, (3) neither agree nor disagree (neutral); (4) agree; and (5) strongly agree. Nineteen questions were addressed and discussed via teleconference to formulate the answers. In order to identify the relevant data on COVID-19 vaccines, a search with standardized descriptors and synonyms was performed on September 10th, 2021, of the MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, ClinicalTrials.gov, and LILACS to identify studies of interest. We used the Newcastle–Ottawa Scale to assess the quality of nonrandomized studies. Results: All the nineteen questions-answers (Q&A) were approved by the BSR Task Force with more than 80% of panelists voting options 4—agree—and 5—strongly agree—, and a consensus was reached. These Guidelines were focused in SMD on the most appropriate timing for IMRD patients to get vaccinated to reach the adequate covid-19 vaccination response. Conclusion: These guidelines were developed by a BSR Task Force with a high LOA among panelists, based on the literature review of published studies and expert opinion for COVID-19 vaccination in IMRD patients. Noteworthy, in the pandemic period, up to the time of the review and the consensus process for this document, high-quality evidence was scarce. Thus, it is not a substitute for clinical judgment.

Resumo Este artigo analisa como a Política Nacional de Saúde Integral da População Negra (PNSIPN) tem sido implementada na atenção à saúde em três municípios do Estado da Bahia, Brasil. Trata-se de um estudo qualitativo que faz parte da primeira etapa de uma pesquisa-ação, na qual foram realizadas entrevistas semiestruturadas com 27 profissionais de unidades de Saúde da Família, Atenção Básica e Sede do Distrito Sanitário. Após análise de conteúdo, emergiram quatro categorias: Atenção Primária à Saúde (APS); Saúde da População Negra; Atenção à saúde na diversidade; e PNSIPN. Como resultado, os profissionais apresentaram entendimento superficial sobre a APS, apesar de a reconhecerem como porta de entrada. A relevância de uma atenção à saúde específica para a população negra foi desconsiderada, sob o argumento de que todos são iguais. A diversidade racial não foi reconhecida dentro do cotidiano, sendo o termo atrelado à diversidade LGBTQIA+ e aos ciclos de vida. O dado comum nos municípios foi o desconhecimento da PNSIPN e dos meios práticos para inseri-la no cotidiano do trabalho. A ausência da política para essa população no processo de planejamento e trabalho dos serviços revela a urgência da educação permanente em saúde para a apropriação do princípio de equidade pelas gestoras e profissionais.

Abstract This study analyzes how the National Policy on Comprehensive Health of the Black Population (PNSIPN) has been implemented in three municipalities in the state of Bahia, Brazil. This qualitative study is part of an ongoing action research, in which semi-structured interviews were performed with 27 professionals from family health, primary health care, and health district headquarters. Content analysis presented four categories: primary health care (PHC); Black health; health care in diversity; and PNSIPN. Health professionals showed a superficial understanding of PHC, despite recognizing it as a gateway. A specific health care for the Black population was considered irrelevant, on the grounds that everyone is equal. Diversity was linked to the LGBTQIA+ population and life cycles, but not to ethnicity or skin color. All municipalities lacked knowledge about the PNSIPN and the practical means to implement it in their daily work. Its absence in the services’ planning and work processes shows an urgent need for permanent education in health so that managers and professional can appropriate the principle of equity.

RESUMO Objetivos: refletir sobre a compreensão global do conceito de Processo de Enfermagem, com ênfase no contexto brasileiro. Métodos: ensaio reflexivo, alinhado à visão e expertise de pesquisadores membros da Rede de Pesquisa em Processo de Enfermagem. Resultados: a reflexão se apresenta em dois tópicos principais: A evolução dos conceitos de Sistematização da Assistência de Enfermagem X Processo de Enfermagem e a sua consonância com as práticas nacional e internacional, e a legislação brasileira; Realinhamento do conceito Processo de Enfermagem na legislação brasileira em consonância com as práticas assistenciais, de ensino e pesquisa atuais. Considerações Finais: as reflexões se orientaram às questões conceituais, normativas e legais do Processo de Enfermagem, incluindo elementos de sua evolução histórica, e, com isso, apontaram para a necessidade da modificação da regulamentação brasileira sobre o Processo de Enfermagem.

ABSTRACT Objectives: to reflect on the global understanding of the Nursing Process concept, with emphasis on the Brazilian context. Methods: a reflection article, aligned with the vision and expertise of researchers who are members of the Nursing Process Research Network. Results: the reflection is presented in two main topics: The evolution of Systematization of Nursing Care X Nursing Process concepts and its consonance with national and international practices, and Brazilian legislation; The Nursing Process concept realignment in Brazilian legislation in line with current care, teaching and research practices. Final Considerations: the reflections were oriented to the Nursing Process’ conceptual, normative and legal issues, including elements of its historical evolution, and, with that, pointed to the need to modify the Brazilian regulation on the Nursing Process.

RESUMEN Objetivos: reflexionar sobre la comprensión global del concepto de Proceso de Enfermería, con énfasis en el contexto brasileño. Métodos: ensayo reflexivo, alineado con la visión y experiencia de investigadores integrantes de la Red de Investigación del Proceso de Enfermería. Resultados: la reflexión se presenta en dos temas principales: La evolución de los conceptos de Sistematización de la Atención de Enfermería X Proceso de Enfermería y su consonancia con las prácticas nacionales e internacionales, y la legislación brasileña; La realineación del concepto de Proceso de Enfermería en la legislación brasileña de acuerdo con las prácticas actuales de atención, enseñanza e investigación. Consideraciones Finales: las reflexiones se orientaron a las cuestiones conceptuales, normativas y legales del Proceso de Enfermería, incluyendo elementos de su evolución histórica, y, con eso, apuntaron para la necesidad de cambiar la regulación brasileña sobre el Proceso de Enfermería.
