Is Shoulder Imbalance a Useful Parameter in the Screening of Idiopathic Scoliosis? A Preliminary Study

Abstract Objective The present study aims to analyze the use of shoulder imbalance as a parameter for scoliosis screening as well as its relationship with other parameters of physical examination. Methods This study assesses a smartphone application that analyzes several parameters of the physical examination in adolescent idiopathic scoliosis. Medical and non-medical examiners applied the screening tool in students in a public school and in a private sports club. After data collection, interobserver correlation was done to verify shoulder imbalance and to compare shoulder imbalance with Adam's bending test and with trunk rotation. Results Eighty-nine participants were examined, 18 of whom were women and 71 of whom were men. Two subjects were excluded from the analysis. The mean age of subjects from the public school was 11.30 years and, for those from the sports club, it was 11.92 years. The examiners had poor-to-slight interobserver concordance on shoulder asymmetry in the anterior and posterior view. No significant statistical correlation was found between shoulder asymmetry and positive Adam's forward bending test. Conclusion Our preliminary study shows that the shoulder asymmetry has a poor correlation with the Adam's forward bending test and measuring trunk rotation using a scoliometer. Therefore, the use of shoulder imbalance might not be useful for idiopathic scoliosis screening. Level of Evidence III; Diagnostic Study
Resumo Objetivo O objetivo deste estudo é analisar o uso da assimetria de ombros como parâmetro para a triagem de escoliose e sua relação a outros parâmetros do exame físico. Métodos Este estudo avalia um aplicativo para smartphone que analisa diversos parâmetros do exame físico de adolescentes com escoliose idiopática. Examinadores médicos e não médicos utilizaram o instrumento de triagem em alunos de uma escola pública e de um clube esportivo privado. Após a coleta de dados, a correlação interobservador foi determinada para verificar a assimetria de ombros e compará-la ao teste de inclinação de Adam e à medição da rotação do tronco. Resultados Oitenta e nove participantes foram examinados, sendo 18 do sexo feminino e 71 do sexo masculino. Dois indivíduos foram excluídos da análise. A média de idade dos participantes da escola pública foi de 11,30 anos e do clube esportivo, 11,92 anos. Os examinadores apresentaram concordância interobservador baixa a branda quanto à assimetria de ombros em incidência anterior e posterior. Não houve correlação estatística significativa entre a assimetria de ombros e o resultado positivo no teste de inclinação do tronco de Adam. Conclusão Nosso estudo preliminar mostra que a assimetria de ombros tem baixa correlação com o teste de inclinação de Adam e assim como com a medição de rotação do tronco com escoliômetro. Portanto, o uso da assimetria de ombros pode não ser útil na triagem da escoliose idiopática. Nível de Evidência III; Estudo Diagnóstico

ABSTRACT OBJECTIVE To evaluate the quality of information on gestational syphilis (GS) and congenital syphilis (CS) on the Sistema de Informação de Agravos de Notificação (SINAN-Syphilis Brazil – Notifiable Diseases Information System) by compiling and validating completeness indicators between 2007 and 2018. METHODS Overall, care, and socioeconomic completeness scores were compiled based on selected variables, by using ad hoc weights assigned by experts. The completeness scores were analysed, considering the region and area of residence, the pregnant woman’s race/colour, and the year of case notification. Pearson’s correlation coefficients were used to validate the scores obtained by the weighted average method, compared with the values obtained by principal component analysis (PCA). RESULTS Most selected variables presented a good or excellent degree of completeness for GS and CS, except for clinical classification, pregnant woman’s level of education, partner’s treatment, and child’s race/colour, which were classified as poor or very poor. The overall (89.93% versus 89.69%) and socioeconomic (88.71% versus 88.24%) completeness scores for GS and CS, respectively, were classified as regular, whereas the care score (GS-90.88%, and CS-90.72%) was good, despite improvements over time. Differences in the overall, care and socioeconomic completeness scores according to region, area of residence, and ethnic-racial groups were reported for syphilis notifications. The completeness scores estimated by the weighted average method and PCA showed a strong linear correlation (> 0.90). CONCLUSION The completeness of GS and CS notifications has been improving in recent years, highlighting the variables that form the care score, compared with the socioeconomic scores, despite differences between regions, area of residence, and ethnic-racial groups. The weighted average was a viable methodological alternative easily operationalised to estimate data completeness scores, allowing routine monitoring of the completeness of gestational and congenital syphilis records.

ABSTRACT The SISBIOTA-BRASIL was a three-year multimillion-dollar research program of the Brazilian government to document plants and animals in endangered/understudied areas and biomes in Brazil. Distributional patterns and the historical events that generated them are extensively unknown regarding Brazilian fauna and flora. This deficiency hinders the development of conservation policies and the understanding of evolutionary processes. Conservation decisions depend on precise knowledge of the taxonomy and geographic distribution of species. Given such a premise, we proposed to research the diversity of Diptera of the Brazilian western arc of Amazon, Cerrado, and Pantanal in the states of Mato Grosso, Mato Grosso do Sul, and Rondônia. Three important biomes of the South American continent characterize these Brazilian states: Amazon forest, Cerrado (Brazilian Savannah), and Pantanal. Besides their ecological relevance, these biomes historically lack intensive entomological surveys. Therefore, they are much underrepresented in the Brazilian natural history collections and in the scientific literature, which is further aggravated by the fact that these areas are being exponentially and rapidly converted to commercial lands. Our project involved over 90 collaborators from 24 different Brazilian institutions and one from Colombia among researchers, postdocs, graduate and undergraduate students, and technicians. We processed and analyzed nearly 300,000 specimens from ~60 families of Diptera collected with a large variety of methods in the sampled areas. Here, we provide a detailed overview of the genera and species diversity of 41 families treated. Our results point to a total of 2,130 species and 514 genera compiled and identified for the three states altogether, with an increase of 41% and 29% in the numbers of species and genera known for the three states combined, respectively. Overall, the 10 most species-rich families were Tachinidae, Cecidomyiidae, Tabanidae, Psychodidae, Sarcophagidae, Stratiomyidae, Bombyliidae, Syrphidae, Tephritidae, and Asilidae. The 10 most diverse in the number of genera were Tachinidae, Stratiomyidae, Asilidae, Mycetophilidae, Syrphidae, Tabanidae, Muscidae, Dolichopodidae, Sarcophagidae, and Chloropidae. So far, 111 scientific papers were published regarding taxonomic, phylogenetic, and biogeographical aspects of the studied families, with the description of 101 new species and three new genera. We expect that additional publications will result from this investigation because several specimens are now curated and being researched by specialists.

Abstract Persea americana Mill., belonging to the family Lauraceae, is noteworthy for the large amount of ethnopharmacological information in its regard, attributing to it many and varied medicinal properties. The tea and alcoholic extracts made from its leaves are used in folk medicine to treat various ailments. This study was designed to analyze the cytogenotoxicity and underlying chemistry of aqueous and hydroalcoholic extracts of avocado leaves, using the Allium cepa and micronucleus tests. The results obtained by applying the experimental models demonstrate that the extracts did not have a genotoxic effect at any of the concentrations analyzed, and even demonstrated a certain protective effect, possibly due to the presence of flavonoids and phenols, both of which are antioxidant substances. However, the extracts did present a cytotoxic effect. There were numerous karyorrhectic cells and those with nuclear alterations related to cell death. At the highest concentrations, it was possible to observe cytoplasmic alterations and binucleated cells. The extracts also caused a significant reduction in the number of cells undergoing division. These effects can be a response to the phytochemical agents present in the extracts. The results suggest that the extracts contain bioactive components that deserve further studies related to cancer therapies.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Alguns comentários recebidos pelos organizadores e sugestões para os organizadores do próximo Monocots, na Costa Rica, também são fornecidos. Este artigo complementa os dados publicados no livro de resumos, disponível no Repositório Digital da UFRN.
Abstract In this paper we present an overview of the abstracts, scientific and social programs, field trips, and achievements in terms of participant number, represented countries, sponsorships, and themes treated during the Sixth International Conference on the Comparative Biology of Monocotyledons that took place in Natal, Brazil, October 7th -12th 2018. Some comments received by the organizers and a few suggestions for organizers of the next meeting, in Costa Rica, are also provided. The conference’s complete abstract book was published and provide additional information.

Abstract Purpose To evaluate the local effect of simvastatin (SVT) combined with deproteinized bovine bone (DBB) with hydroxyapatite/β-tricalcium phosphate biphasic ceramics (HA/TCP) and with collagen sponge (CS) on bone repair in critical size defects (CSDs) in rat calvaria. Methods Forty-two 5-mm diameter CSDs were made bilaterally in the calvaria of 18 rats. The animals were allocated according to the type of biomaterial and associations used to fill the CSD. After 8 weeks, the animals were euthanized, and their calvaria were evaluated for repaired tissue composition using histologic and histometric analyses. Results In the histometric analysis, the use of SVT showed to increase bone formation in the CSDs when combined with all the bone substitutes tested in this study (p<0.05). Greater bone formation was observed in the groups with SVT compared to the groups without SVT. Conclusions The use of SVT without the need for a vehicle and combined with a commercially available biomaterial may be a cheaper way to potentiate the formation of bone tissue without the need to produce new biomaterials. Therefore, SVT combined with DBB induced significantly greater new bone formation than did the other treatments.

Laurus nobilis L. is a large shrub belonging to the Lauracea family. Its leaves are widely used for food seasoning as well as in folk medicine. Various studies have demonstrated the antiproliferative, antifungal and antibacterial effects of Laurus nobilis, but no studies have investigated the genotoxic effect of the aqueous extract of the plant. The objective of this study was to analyze the genotoxic potential of an aqueous extract of leaves, using the Allium cepa assay and mouse peripheral blood cell micronucleus test. The results showed that the extract did not have any genotoxic activity, but cytotoxic activity was observed in the two experimental models used. The extract had an antiproliferative effect, detected through the reduction of the mitotic index and the polychromatic/normochromatic erythrocyte (PCE/NCE) ratio. The tests also demonstrated a large number of cells undergoing apoptosis and with nuclear abnormalities related to cell death processes. These results can be explained by the presence of phenolic compounds, saponins, flavonoids and alkaloids, detected in the phytochemical analysis of the extract. Therefore, the extract from L. nobilis in the form generally used by the population does not pose risks related to its genotoxic potential, and also contains components with apoptotic and antigenotoxic potential.

Resumo A modelagem do recrutamento em florestais tropicais é importante para estudos de sustentabilidade do manejo florestal, por dar subsídio adequado à recuperação do estoque de madeira. O objetivo do trabalho foi estimar o recrutamento após a colheita de madeira, empregando um modelo de rede neural artificial (RNA). A área de estudo está localizada na Floresta Nacional do Tapajós (55°00’ W, 2°45’ S), Pará. Em 64 ha da área de estudo, em 1979, foi realizada colheita intensiva de 72,5 m3 ha-1. Em 1981 foram instaladas, aleatoriamente, 36 parcelas permanentes de 50 m x 50 m. Essas parcelas foram mensuradas em 1982, 1983, 1985, 1987, 1992, 1997, 2007, 2010 e 2012. Para modelar o recrutamento foram consideradas as variáveis da subparcela-alvo e a sua vizinhança. As estimativas obtidas no treino e na generalização da RNA foram avaliadas pelas estatísticas: correlação () e raiz quadrada do erro quadrático médio (RQRQM), sendo obtido RQRQM 35,6% e 0,89. Foi possível modelar a tendência do recrutamento ao longo do tempo em florestas tropicais, após a colheita de madeira.
Abstract Recruitment models in tropical forests are important for studies on forest management sustainability because they provide adequate support to recovery of wood stocks. The objective of this work was to estimate recruitment after wood harvesting by using an artificial neural network (ANN) model. The study area is located at Tapajós National Forest (55° 00’ W, 2° 45’ S), Pará state. In 64 ha of the study area, in 1979, an intensive harvest of 72.5 m3 ha-1 was carried out. In 1981, 36 permanent plots of 50 m x 50 m were randomly installed. These plots were measured in 1982, 1983, 1985, 1987, 1992, 1997, 2007, 2010 and 2012. For recruitment modeling, the variables of the target subplot and its vicinity were considered. The estimates obtained in ANN training and generalization were evaluated by statistics: correlation () and root mean square error (RMSE) were determined: RMSE 35.6% and 0.89. Recruitment tendency could be modeled over time in tropical forests after wood harvesting.

RESUMO As doenças do fígado e das vias biliares são causas comuns de morbidade e mortalidade. Procedimentos invasivos com finalidade diagnóstica e terapêutica são frequentemente recomendados nos casos de doenças hepatobiliares. O reconhecimento das indicações e limitações das técnicas comumente empregadas é crucial para uma adequada seleção dos pacientes, maximizando os resultados positivos e reduzindo o risco de complicações. Em 2018, a Sociedade Brasileira de Hepatologia (SBH), em colaboração com a Sociedade Brasileira de Radiologia Intervencionista e Cirurgia Endovascular (SOBRICE) e a Sociedade Brasileira de Endoscopia Digestiva (SOBED) realizaram um encontro exclusivamente voltado para a discussão dos procedimentos invasivos nas doenças hepatobiliares. Este texto resume as principais recomendações discutidas durante o evento, e tem a intenção de auxiliar clínicos, gastroenterologistas, hepatologistas, radiologistas e endoscopistas no uso adequado dos procedimentos invasivos para manejo de pacientes com doenças hepatobiliares.
ABSTRACT Liver and biliary tract diseases are common causes of morbidity and mortality worldwide. Invasive procedures are usually performed in those patients with hepatobiliary diseases for both diagnostic and therapeutic purposes. Defining proper indications and restraints of commonly used techniques is crucial for proper patient selection, maximizing positive results and limiting complications. In 2018, the Brazilian Society of Hepatology (SBH) in cooperation with the Brazilian Society of Interventional Radiology and Endovascular surgery (SOBRICE) and the Brazilian Society of Digestive Endoscopy (SOBED) sponsored a joint single-topic meeting on invasive procedures in patients with hepatobiliary diseases. This paper summarizes the proceedings of the aforementioned meeting. It is intended to guide clinicians, gastroenterologists, hepatologists, radiologists, and endoscopists for the proper use of invasive procedures for management of patients with hepatobiliary diseases.

Resumo A procura intensa por essências madeireiras nativas consolidadas, que apresentam elevado uso, pode ocasionar a superexploração de espécies e diminuição do seu estoque na floresta. Uma alternativa para essa situação é a substituição dessas espécies por outras com propriedades da madeira semelhantes e com suficiente estoque de crescimento na floresta. O objetivo deste trabalho foi agrupar as espécies da Amazônia através das propriedades físico-mecânicas da madeira e realizar a análise discriminante para identificar quais características tecnológicas são mais importantes para o agrupamento. As espécies estudadas foram provenientes de oito localidades distintas da região amazônica. As propriedades físico-mecânicas utilizadas obtidas da literatura especializada nacional foram: densidade básica, contração (tangencial, radial e volumétrica), flexão estática, compressão paralela e perpendicular às fibras, dureza Janka paralela e transversal, tração perpendicular às fibras, fendilhamento e cisalhamento. Foi utilizada a técnica de análise multivariada de Cluster (distância euclidiana simples e o método de Ward) e a análise discriminante para avaliar o agrupamento. A análise de Cluster foi eficiente para agrupar as espécies, que foram separadas em três grupos distintos. As espécies que se destacaram foram a Helicostylis pedunculata Benoist. e Tachigali chrysophylla (Poepp.) Zarucchi & Herend., por se agruparem com as espécies mais comercializadas. Os menores valores de Wilks’Lambda foram da densidade da madeira (0,759053), cisalhamento (0,802960) e compressão paralela às fibras (0,825594). Essas características foram as mais determinantes para discriminar os grupos. A análise de agrupamento é eficiente para indicar a substituição de espécies consolidadas na Amazônia.
Abstract The intense search for consolidated native wood essences, which are highly used, can lead to the overexploitation of species and decrease their stocks in the forest. An alternative to this situation is the replacement of these species by others with similar wood properties and with sufficient forest growing stock. The objective of this work was to cluster the Amazon species through wood physical-mechanical properties and perform the discriminant analysis to identify which technological characteristics are more important for the clustering. The species studied came from eight different locations in the Amazon region. The properties used were: basic density, contraction (tangential, radial and volumetric), static flexion, compression parallel and perpendicular to the fibers, Janka hardness parallel and transversal, traction perpendicular to the fibers, cracking and shearing, all obtained from the national specialized literature. Multivariate Cluster analysis (simple Euclidean distance and Ward’s method) and the discriminant analyses were used to evaluate the clustering. Cluster analysis was efficient to cluster the species, which were separated into three distinct groups. The species that stood out were Helicostylis pedunculata Benoist. and Tachigali chrysophylla (Poepp.) Zarucchi & Herend., f or clustering with the most commercialized species. The lowest values of Wilks’Lambda were wood density (0.759053), shearing (0.802960) and compression parallel to the fibers (0.825594). These characteristics were the most determinant to discriminate the clusters. The clustering analysis was efficient for the separation of the species into marketing clusters.

RESUMO Objetivo: avaliar a eficácia da estratégia adotada e a qualidade do atendimento em trauma pediátrico na sobrevivência dos pacientes atendidos após desastre em uma cidade do interior de Minas Gerais, em comparação a resultados esperados por estudos sobre mortalidade infantil em grandes queimados. Métodos: análise retrospectiva observacional de dez pacientes que sofreram queimaduras e foram transferidos para um centro de referência de trauma. Utilizou-se o escore de R-Baux modificado para estimar a mortalidade esperada. Comparou-se a mortalidade esperada a partir do escore de R-Baux e a mortalidade real, a partir do teste de uma proporção. Comparou-se, também, tempo de admissão pós-trauma com mortalidade e grau de superfície corporal queimada com mortalidade. Resultados: o R-Baux médio foi de 75,2, o que significa uma mortalidade esperada para grandes queimados de 5%. No entanto, a mortalidade do grupo com grande área de superfície corporal queimada desse estudo foi de 60%, valor p=0,001. Observou-se neste caso uma mortalidade muito além da esperada pela literatura. Conclusão: apesar das inúmeras variáveis, aventa-se a hipótese de infraestrutura de atendimento em trauma pediátrico aquém da necessária no Estado. Este estudo sugere maior incentivo à políticas públicas para atendimento de trauma pediátrico, centro de referência preparado, acordos de transferência bem estabelecidos e otimização de planos de catástrofe para diminuição da morbimortalidade para os pacientes que sobrevivem à primeira hora após o trauma.
ABSTRACT Objective: to evaluate the effectiveness of the adopted strategy and the care quality for pediatric trauma in the survival of patients attended after a disaster in a city in the interior of Minas Gerais state, compared to the expected results of studies on infant mortality in major burns. Methods: retrospective observational analysis of ten patients who were burned and transferred to a trauma reference center. We used the modified R-Baux score to estimate the expected mortality. We compared the expected mortality predicted by R-Baux score and the actual mortality determined from one-ratio test. We also compared time of post-trauma admission with mortality and burned body surface area with mortality. Results: mean R-Baux score was 75.2, which means an expected mortality of 5% among major burn patients. However, in this study, mortality in the group of children with large burned body surface area was of 60%, p=0.001, a rate far beyond that expected in literature. Conclusion: despite the innumerable variables, we consider the hypothesis of the pediatric trauma care infrastructure being inferior than the one needed in the state. This study suggests a greater incentive for public policies concerning pediatric trauma care, prepared referral center, well-established transfer agreements, and optimization of catastrophe plans, in order to reduce morbimortality of patients who survive the first hour after trauma.

RESUMO No manejo florestal é importante projetar a distribuição diamétrica ao longo do ciclo de corte para subsidiar as técnicas silviculturais mais adequadas à estrutura da floresta. O objetivo do trabalho foi projetar a distribuição diamétrica ao longo do ciclo de corte utilizando Matriz de Transição Markoviana. Em 1979 foi realizada a colheita seletiva, com intensidade de 72,5 m3 ha-1, em uma área de 64 ha, na Floresta Nacional do Tapajós, Pará. Em 1981 foram instaladas, aleatoriamente, 36 parcelas permanentes de 50 m x 50 m medidas sucessivamente em 1982, 1983, 1985, 1987, 1992, 1997, 2007, 2010 e 2012. Para a projeção foram usadas duas matrizes de transição: uma com intervalo de tempo de um ano (1981 a 1982) e outra de dois anos (1981 a 1983). Todas as projeções apresentaram distribuição diamétrica em forma de “J” invertido, com pequena descontinuidade no último ano (2012). Houve aderência da distribuição diamétrica a 5% de significância entre os dados observados e estimados até aos 11 anos de projeção. ABSTRACT In forest management, it is important to project diametric distribution along the cutting cycle to subsidize the most suitable silvicultural techniques to forest structure. The aim of this work was to project diametric distribution along the cutting cycle using Markov Transition Matrix. In 1979, selective harvest was performed with an intensity of 72.5 m3 ha-1, in a 64 ha area in ‘Floresta Nacional do Tapajós’ - Pará state (PA). In 1981, 36 permanent plots of 50m x 50m were randomly installed and measured successively in 1982, 1983, 1985, 1987, 1992, 1997, 2007, 2010 and 2012. For the projection, two transition matrices were used: one with time interval of one year (1981 to 1982) and another of two years (1981 to 1983). All projections showed diametric distribution in inverted “J” form, with small discontinuity last year (2012). There was adherence of diametric distribution at 5% significance between observed and estimated data, up to 11 years of projection. Transition matrix can be used to project diametric distribution after harvest in short periods of time after the forest harvest.
