Abstract Background: Atherosclerosis is a serious health problem, and several factors contribute to its occurrence. Longitudinal and qualified monitoring of primary health care (PHC) may contribute to the management of atherosclerosis and reduction of avoidable hospital admissions. Objectives: To estimate the trend in hospitalizations for atherosclerosis and the impact of PHC coverage on its evolution from 2008 to 2018 in Brazil. Methods: An ecological time series analytical study based on the outcomes of hospital admissions for atherosclerosis in Brazil. Time in years, PHC coverage, and Family Health Strategy (FHS) services were considered independent variables. A Prais–Winsten model was used to estimate the outcome trend, and α < 0.05 was adopted. Results: We observed a mean increase of 1.81 hospitalizations for atherosclerosis per 100 000 inhabitants annually (p = 0.002) in Brazil. This growth was evidenced in the Northeast (p < 0.001), Southeast (p = 0.003), and South (p < 0.001) regions, being stable in the North (p = 0.057) and Midwest (p = 0.62) regions. Men presented twice the growth in hospitalizations from the fifth decade of life on (p < 0.01). An inversely proportional relationship was observed for PHC coverage (B = -0.71; p < 0.001) and the proportion of FHS services (B = -0.59; p < 0.001) with the rate of admissions due to atherosclerosis in Brazil. Conclusions: Although hospitalizations for atherosclerotic complications are increasing in Brazil, they present regional and individual gender and age discrepancies, as well as a mitigating effect exerted by PHC coverage.

Abstract Background Stroke and acute myocardial infarction (AMI) are cardiovascular diseases commonly characterized by the development of atheromatous plaques associated with major complications and high mortality rates. Objective To identify an epidemiological trend in hospitalizations due to stroke and AMI and to analyze the relationship between health programs applied in Primary Health Care, gender and the Federative Unit. Methods Ecological study with a time series design between 1998 and 2018, collecting data from all federal units in Brazil stratified by, gender and place of residence. There were analyzed Hospitalization Authorizations (AIH) for stroke and MI, consulting the Hospital Admissions System (SIH) of the Informatics Department of the National Health Service with p <0.05. Results From 1998 to 2018, the rate of hospitalization for AMI increased in Brazil approximately 42.58 events per 100 thousand inhabitants annually (p<0.001), while hospitalizations for stroke declined 32.17 cases (p=0.03). This pattern was observed in both sexes in AMI and stroke. There is also evidence of the effect of the Hiperdia (p<0.001) and Mais Médicos (p=0.001) program in reducing stroke and Hiperdia cases in mitigating the evolution of AMI cases (p = 0.0001). Conclusion Although these diseases remain as an important cause of death, stroke hospitalization has reduced significantly in the period evaluated. National programs as the Hiperdia and Mais Médicos showed an impact in the acute cases of strokes and AMI.

Resumo Fundamento A íntima relação entre a regulação do sono e os eventos cardiovasculares é um dos principais focos de investigação na medicina contemporânea. Hábitos e características do sono interferem na ritmicidade cardíaca e também na expectativa de vida, principalmente em idosos. Objetivo Estimar o risco de óbito e de eventos cardiovasculares em idosos comunitários que apresentam queixa de insônia e sonolência excessiva diurna ao longo de oito anos de seguimento. Método Foi desenhada uma coorte prospectiva com 160 idosos, a primeira onda em 2009 e a segunda em 2017. Os grupos de seguimento foram determinados pela exposição ou não às queixas de insônia primária e a sonolência excessiva diurna, com ou sem ronco. As covariáveis sexo, estado conjugal, depressão, hipertensão e diabetes foram controladas. O desfecho primário foi o óbito e o secundário, os eventos cardiocerebrovasculares (ECV). As probabilidades dos desfechos foram estimadas pelo risco relativo (RR), através da regressão de Poisson, adotando-se α ≤ 0,05. Resultados Registraram-se 40 mortes no período (25,97%:19,04-32,89) e 48 ECVs (30,76%:23,52-38,01). Os homens apresentaram maior risco (RR = 1,88;1,01-3,50) de óbito. A depressão (RR = 2,04;1,06-3,89), a gravidade da insônia (RR = 2,39;1,52-4,56) e a latência do sono entre 16-30 minutos (RR = 3,54;1,26-9,94) e 31-60 minutos (RR = 2,23;1,12-4,47) aumentaram independentemente o risco de óbito em idosos comunitários. Os ECVs foram preditos apenas por idosos hipertensos e/ou diabéticos (RR = 8,30; 1,98-34,82). Conclusão A mortalidade em idosos é influenciada pelo estado emocional e pela dificuldade de dormir, diferentemente dos ECVs, condicionados apenas pelas condições pressóricas arteriais e metabólicas.

Abstract Background The close relationship between sleep regulation and cardiovascular events is one of the main focuses of research in contemporary medicine. Sleep habits and characteristics interfere with the cardiac rhythm and also with life expectancy, especially in the elderly. Objective To estimate the risk of death and cardiovascular events in community-dwelling elderly individuals complaining of insomnia and excessive daytime sleepiness over eight years of follow-up. Method A prospective cohort was designed with 160 elderly, with the first wave occurring in 2009 and the second in 2017. Follow-up groups were determined by exposure or not to complaints of primary insomnia and excessive daytime sleepiness with or without snoring. The covariates gender, marital status, depression, hypertension and diabetes were controlled. The primary outcome was death and the secondary outcome was cardio-cerebrovascular events (CCV). Outcome risks were estimated by relative risk (RR) through Poisson regression, adopting α≤0.05. Results There were 40 (25.97%: 19.04-32.89) deaths over the period and 48 (30.76%: 23.52-38.01) CCV. Men had a higher risk (RR = 1.88; 1.01-3.50) of death. Depression (RR = 2.04; 1.06-3.89), insomnia severity (RR = 2.39; 1.52-4.56) and sleep latency between 16-30 minutes (RR = 3, 54; 1.26-9.94) and 31-60 minutes (RR = 2.23; 1.12-4.47) increased the risk of death independently in community-dwelling elderly. CCV were predicted only in the hypertensive and / or diabetic elderly (RR = 8.30; 1.98-34.82). Conclusion Mortality in the elderly is influenced by the emotional state and difficulty in falling asleep, unlike CCVs, which are conditioned only by arterial and metabolic blood pressure conditions.

Resumo Objetivo Determinar a concordância entre os instrumentos de mensuração short test of functional health literacy in adults (S-TOFHLA) e short assessment of health literacy for Portuguese-speaking adults (SAHLPA-18) como estratégia para estimar a validade concorrente. Métodos Estudo transversal, com usuários do Sistema Único de Saúde. Para testar a validade concorrente, aplicou-se abordagem de concordância com teste de Kappa ponderado para dados qualitativos. Resultados Participaram 372 indivíduos, dos quais 66% e 62% não apresentaram nível de letramento adequado, segundo o SAHLPA-18 e o S-TOFHLA, respectivamente. Observou-se correlação forte entre os instrumentos (p<0,001; r=0,60); e a concordância de acertos encontrada, 65,3% (Kappa=0,35; p<0,001), foi considerada fraca. Conclusão Os instrumentos SAHLPA-18 e S-TOFHLA apresentam constructos diferentes e fraca concordância. An agreement approach using a weighted Kappa test for qualitative data was applied in order to test for concurrent validity. Results 372 individuals participated. It was found that 66% and 62% of them did not have an adequate level of literacy according to SAHLPA-18 and S-TOFHLA, respectively. There was strong correlation between the instruments (p<0.001; r=0.60), although the 65.3% agreement of correct answers found was considered weak (Kappa=0.35; p<0.001). Conclusion The SAHLPA-18 and S-TOFHLA instruments have different constructs and poor agreement. Use of different instruments is indicated in research intended to measure level of literacy, as is the development of instruments specific to health conditions that allow results close to the real context of individuals to be obtained.

Resumo Os objetivos foram confirmar a validade do Guedes Tool, instrumento para avaliação do apoio social informal (ASI) para idosos, e estimar sua acurácia. Foi realizada análise fatorial confirmatória (AFC) com avaliação dos índices CFI, RMSEA e relação do qui-quadrado (X2) com graus de liberdade (gl). Para análise de acurácia foi verificada a área sob a curva ROC, valores de sensibilidade (S), especificidade (E), valor preditivo positivo (VP+), valor preditivo negativo (VP-) e o Índice J de Younder (J) para confirmar o melhor ponto de corte. A coleta de dados ocorreu com idosos de Natal e região metropolitana em 2018. Os critérios de inclusão foram: ter 60 anos ou mais, com níveis cognitivos preservados. Duzentos e seis idosos participaram do estudo de AFC e 197 participaram da análise de acurácia. As estimativas dos índices avaliados na AFC foram: X2/gl=1,33, RMSEA=0,04 (IC95% 0,025-0,054) e CFI=0,91. A curva ROC obteve área de 0,78 (IC95: 0,72-0,85; p<0,001) para a determinação de idosos com baixo ASI. O J de maior valor foi de 0,44 para pontuação ≤34, o qual tem uma S de 59,76% e E de 84,96%. O instrumento apresentou um modelo bem ajustado com quatro dimensões, segundo critérios da AFC. Apresentou boa área sob a curva ROC e boa E e S moderada para o valor de corte de 34 pontos ou menos para o diagnóstico do ASI insuficiente. Os bons indicadores de VP+ e VP- reforçaram sua acurácia desejável.

Abstract The objective of the present study was to confirm the validity of the Guedes Tool, an instrument for assessing informal social support (ISS) for older adults, and to estimate its accuracy. Confirmatory factor analysis (CFA) was performed with the CFI and RMSEA indexes and the chi-square relationship (X2) with degrees of freedom (gl). For the analysis of accuracy, the area under the ROC curve, sensitivity (S) and specificity (SP) values, positive predictive value (PV+), negative predictive value (PV-) and the Younder’s J Index (J) were verified to confirm the best cut-off point. Data collection was carried out with older adults from Natal and metropolitan region in 2018. The inclusion criteria were: be 60 years old or older, with preserved cognitive levels. Two hundred and six older adults participated in the CFA study and 197 participated in the accuracy analysis. The estimates of the indexes evaluated in the CFA were: X2/gl =1.33, RMSEA=0.04 (95% CI 0.025-0.054) and CFI=0.91. The ROC curve obtained an area of 0.78 (CI95: 0.72-0.85; p<0.001) for the determination of older adults with low ISS. The highest value J was 0.44 for a score ≤34, with an S value of 59.76% and an SP value of 84.96%. The instrument presented a well-adjusted model with four dimensions, according to CFA criteria. It had a good area under the ROC curve and good to moderate S and SP values for the cutoff value of 34 points or less, for the diagnosis of insufficient ISS. Good PV+ and PV- indicators confirmed the desirable levels of accuracy of the tool.

Resumo: Introdução: Este estudo teve como propósito estimar a prevalência de sintomas de estresse, depressão e ansiedade dos estudantes de Medicina da Universidade Federal do Rio Grande do Norte (UFRN), associando-os com outros fatores. Métodos: Trata-se de um estudo quantitativo epidemiológico, do tipo transversal. Participaram desta pesquisa estudantes de Medicina da UFRN, distribuídos equitativamente entre os diferentes períodos do curso. Entregaram-se 288 questionários, no entanto validaram-se 279, os demais não foram respondidos corretamente. Este estudo foi aprovado pelo Comitê de Ética em Pesquisa da UFRN (Parecer nº 2.009.026) e todos os voluntários assinaram o Termo de Consentimento Livre e Esclarecido (TCLE), após serem informados sobre os objetivos da pesquisa. Para a coleta dos dados, utilizaram-se os seguintes instrumentos: 1. ficha de Identificação para os dados sociodemográficos, 2. Inventário de Sintomas de Stress para Adultos de Lipp (ISSL), 3. Inventário de Depressão de Beck (IDB) e 4. Inventário de Ansiedade de Beck (BAI). Para análise dos resultados, realizaram-se análises descritivas a partir da média, do desvio padrão e da frequência dos dados coletados. A análise inferencial foi realizada para verificar a associação entre as variáveis de desfechos e os atributos dos estudantes. Resultados: Dos estudantes entrevistados, 66,3% tinham estresse e a maioria estava na fase de resistência (58,4%) com predominância de sintomas psicológicos (42,3%). Em relação à sintomatologia depressiva, 28% dos estudantes apresentavam sintomas: 51,3% com sinais de depressão de leve a moderada, 35,9% com sinais de depressão moderada e 12,8% com sinais de depressão severa. No que se refere à sintomatologia ansiosa, 66,3% dos estudantes apresentavam sinais de ansiedade em seu grau mínimo; e 33,7%, sinais de ansiedade leve, moderada ou severa. Desses últimos, 21,9% tinham sinais de ansiedade leve; 10,8%, sinais de ansiedade moderada; e 1%, sinais de ansiedade severa. Conclusões: Diante disso, acredita-se que as instituições de ensino superior devem se comprometer com o desenvolvimento integral dos seus estudantes apresentando estratégias institucionais para o enfrentamento dessa realidade.

Abstract: Introduction: The purpose of this study was to estimate the prevalence of stress, depression and anxiety symptoms among medical students of the Federal University of Rio Grande do Norte, associating them with other factors. Methods: This is a quantitative cross-sectional epidemiological study. Medical students from the Federal University of Rio Grande do Norte participated in this study, distributed equally among the different periods of the course. 288 questionnaires were delivered; however, 279 were validated, while the others were not answered correctly. This study was approved by the Research Ethics Committee of UFRN (Opinion n. 2,009,026) and all volunteers signed the free and informed consent (FIC) form, after being informed about the research objectives. The following instruments were used to collect the data: (1) Identification card for the socio-demographic data, (2) Inventory of Stress Symptoms for Adults of Lipp (ISSL), (3) Beck Depression Inventory - BDI, (4) Beck Anxiety Inventory (BAI). For the analysis of the results, descriptive analyzes were performed based on the mean, standard deviation and frequency of the collected data. Inferential analysis was performed to verify the association between outcome variables and student attributes. Results: Of the interviewed students, it was observed that 66.3% had stress symptoms and the majority was in the resistance phase (58.4%) with a predominance of psychological symptoms (42.3%). In relation to depressive symptomatology, 28% of the students had symptoms, with 51.3% showing signs of mild to moderate depression, 35.9% signs of moderate depression and 12.8% showing signs of severe depression. Regarding the anxious symptomatology, 66.3% of the students showed minimal signs of anxiety and 33.7% showed signs of mild, moderate or severe anxiety. Of the latter, 21.9% had signs of mild anxiety, 10.8% signs of moderate anxiety and 1% signs of severe anxiety. Conclusions: In view of this, it is believed that higher education institutions should commit themselves to the integral development of their students, offering institutional strategies for students to cope with this reality.

RESUMO: Introdução: O sono é uma dimensão do bem-estar e da saúde. Um sono não reparador relaciona-se com disfunções de saúde, principalmente em populações vulneráveis, à medida que os fatores relacionados mudam contextualmente. Assim, objetivou-se mensurar a magnitude da redução da função reparadora do sono (FRS) e os fatores biopsicossociais relacionados em idosos brasileiros. Método: Foram analisados dados secundários da Pesquisa Nacional de Saúde de 2013, de delineamento transversal. A amostra constituiu-se de indivíduos a partir de 60 anos. O desfecho considerado foi a prevalência de redução da função restaurativa do sono (FRS) autorreferida. Características de saúde e sono, comportamento emocional, hábitos de vida, suporte social e urbanização tiveram suas relações investigadas. A associação com desfecho foi medida pela razão de prevalência (RP) e estimada pela regressão de Cox, assumindo α ≤ 0,05. Resultados: A FRS estava reduzida em 29,2% (intervalo de confiança de 95% - IC95% 27,2 - 30,6%). Ela está relacionada à depressão (RP = 3,37; IC95% 2,87 - 3,97), insônia/sonolência (RP = 2,45; IC95% 2,14 - 2,79); oscilação comportamental (RP = 1,75; IC95% 1,53 - 1,99), percepção negativa de saúde (RP = 1,50; IC95% 1,23 - 1,82), computador e internet (RP = 1,44; IC95% 1,01 - 2,07) e dificuldade funcional (RP = 1,13; IC95% 1,01 - 1,27). Viver em áreas urbanas (RP = 1,32; IC95% 1,14 - 1,52) e ter condição crônica (RP = 1,58; IC95% 1,11 - 2,40) associaram-se apenas à pior situação de redução da FRS. Conclusão: A redução da FRS atinge um terço dos idosos no Brasil e está intimamente relacionada a fatores biopsicossociais, exigindo abordagens de políticas públicas intersetoriais de promoção de saúde.

ABSTRACT: Background: Sleep is a dimension of well-being and health. Non-restful sleep is related to health dysfunctions, especially in vulnerable populations, considering that related factors change contextually. Thus, the objective of the present study was to measure the magnitude of the reduction in sleep restorative function (SRF) and related biopsychosocial factors in Brazilian elderly. Method: Secondary data from the 2013 National Health Survey of cross-sectional design were analyzed. The sample consisted of individuals as from 60 years old. The outcome considered was the prevalence of reduced self-perceived sleep restorative function (SRF). Health and sleep characteristics, emotional behavior, lifestyle, social support, and urbanization were all investigated. The association with outcome was measured with the prevalence ratio (PR) and estimated with Cox regression, assuming α ≤ 0.05. Results: SRF was reduced by 29.2% (95%CI 27.2 - 30.6%). It is related to depression (PR = 3.37; 95%CI 2.87 - 3.97), insomnia/sleepiness (PR = 2.45; 95%CI 2.14 - 2.79); behavioral oscillation (PR = 1.75; 95%CI 1.53 - 1.99), negative health perception (PR = 1.50; 95%CI 1.23 - 1.82), computer and internet (PR = 1.44; 95%CI 1.01 - 2.07) and functional difficulty (PR = 1.13; 95%CI 1.01 - 1.27). Living in urban areas (PR = 1.32; 95%CI 1.14 - 1.52) and having a chronic condition (PR = 1.58; 95%CI 1.11 - 2.40) were only associated to the worst situation of reduced SRF. Conclusion: The reduction in SRF affects one third of the elderly in Brazil and is closely related to biopsychosocial factors, requiring intersectoral public health promotion approaches.
