ABSTRACT Some psyllids transmit 'Candidatus Liberibacter solanacearum' (Lso), the causal agent of devastating plant diseases of cultivated Solanaceae and Apiaceae. The recent detection of Bactericera cockerelli and Lso in Ecuador seriously threatens these crops in South America. There, neither the role of native psyllids in the Lso epidemiology nor the psyllid fauna of vegetables are known. With the aim to identify potential vectors and risk scenarios for the spread of Lso in South America, a survey of the psyllid fauna of Solanaceae and Apiaceae crops and associated weeds was conducted in Brazil. Samples were taken at 29 localities in four states. A total of 2857 specimens were sampled, representing at least 37 species of 23 genera and seven families. The most frequent species on carrot, chilli pepper and potato were Russelliana solanicola, R. capsici and Isogonoceraia divergipennis, respectively. Immatures of R. capsici were found on chilli pepper and of R. solanicola on carrot and potato, confirming these plants as hosts. The two psyllid species have been suspected previously to transmit plant pathogens of unknown identity. Russelliana solanicola is one of the few polyphagous species. Here the species is reported for the first time from carrot. Recent collections in Rio Grande do Sul suggest that Solanum laxum represents the original host of R. capsici, which subsequently shifted to chilli pepper. Both, adaptation to agricultural crops and the possibility of ability to transmit pathogens, make the two Russelliana species dangerous potential vectors of Lso and other plant pathogens in South America.

ABSTRACT Chrysoperla externa (Hagen) and Hippodamia convergens (Guérin-Meneville) are voracious generalist predators, and important aphid control agents. In an environment containing a complex of species, the occurrence of intraguild interactions can interfere in the predator behavior and consumption. The aim of this work was to know the number of nymphs of Rhodobium porosum (Sanderson) and Macrosiphum rosae (Linnaeus) consumed by larvae of C. externa and H. convergens, and the interaction between these predators when confined together. First, second and third instar nymphs of R. porosum and M. rosae were provided in Petri dishes containing rose leaflets and second instar larvae of the predators. Intraguild interaction was studied in Petri dishes containing first instar nymphs of both aphid species and a second instar larva of C. externa plus one of H. convergens. A third treatment consisted of dishes containing a second instar larva of both predators maintained in the absence of prey. The evaluations took place throughout the entire instar of the predators. C. externa consumed a greater number of R. porosum nymphs and H. convergens a greater number of M. rosae nymphs. For both species of prey, the highest consumption was verified on the last day of evaluation. There was a positive interaction when the predator’s larvae were confined in the presence of aphid nymphs, with no mortality observed for any of them. In the absence of prey, there was 70% mortality of H. convergens larvae due to intraguild predation.

Abstract Forests play a crucial role in the global carbon cycle. Among the factors that can affect carbon stocks in forest stands, planting spacing stands out. This study verified the effect of spacing on biomass and carbon stock in a stand of Anadenanthera peregrina (L.) Speg at 56 months of age. They were planted with 3 x 2 m, 3 x 3 m, 4 x 3 m, 4 x 4 m and 5 x 5 m spacings. The biomass of the above-ground compartments (stem, branches, leaves and bark) and root biomass was quantified, totalling 16.42 and 6.68 Mg ha-1, respectively. The largest amount of biomass and carbon stock occurred in the denser spacings. Total biomass was twice as large at 3 x 2 m spacing compared to 5 x 5 m spacing. The root biomass represented 40.93% of the total biomass. The order of participation of the components in the total aboveground biomass was branches (44.99%), stem (40.77%), leaves (13.99%) and bark (4.90%). The denser spacings (3 x 2 m and 3 x 3 m) showed higher values of biomass and carbon stock than the wide spacings (4 x 3 m, 4 x 4 m and 5 x 5 m). There was no effect of planting spacing on carbon stock in necromass and soil.
Resumo As florestas desempenham um importante papel no ciclo do carbono. Dentre os fatores que podem afetar os estoques de carbono nos povoamentos florestais, o espaçamento de plantio tem grande relevância. O objetivo foi verificar o efeito do espaçamento na biomassa e no estoque de carbono em um povoamento de Anadenanthera peregrina (L.) Speg aos 56 meses de idade. O povoamento foi implantado nos espaçamentos 3 x 2 m, 3 x 3 m, 4 x 3 m, 4 x 4 m e 5 x 5 m. Quantificou-se a biomassa dos compartimentos acima do solo (madeira, galhos, folhas e casca) e da raiz, totalizando 16,42 e 6,68 Mg ha-1, respectivamente. A maior quantidade de biomassa e estoque de carbono ocorreu nos menores espaçamentos. A biomassa total foi duas vezes maior no espaçamento de 3 x 2 m em comparação com o espaçamento de 5 x 5 m. A biomassa abaixo do solo representou 40,93% da biomassa total. A ordem de participação dos componentes na biomassa aérea total foi galhos (44,99%), madeira (40,77%), folhas (13,99%) e casca (4,90%). Os espaçamentos mais densos (3 x 2 e 3 x 3 m) apresentaram maiores valores de biomassa e estoque de carbono do que os espaçamentos mais amplos (4 x 3 m, 4 x 4 m e 5 x 5 m). Não houve efeito do espaçamento de plantio sobre o estoque de carbono na necromassa e no solo.

This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.nbsp;
Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.

Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.nbsp;
The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.nbsp;
The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.nbsp; nbsp;nbsp;
Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.
Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidências científicas que as suportam, quanto ao diagnóstico e tratamento da CDC, com especial ênfase na base de racionalidade que a embasou.
A DC no século XXI mantém padrão epidemiológico de endemicidade em 21 países da América Latina. Investigadores e gestores de países endêmicos e não endêmicos indigitam a necessidade de se adotarem políticas abrangentes, de saúde pública, para controle eficaz da transmissão inter-humanos da infecção pelo T. cruzi, e obter-se nível otimizado de atendimento aos indivíduos já infectados, com foco em oportunização diagnóstica e terapêutica.
Mecanismos patogênicos e fisiopatológicos da CDC foram revisitados após atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistência parasitária tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distúrbios autonômicos e microvasculares. Alguns deles recentemente constituíram alvos potenciais de terapêuticas.
A história natural das fases aguda e crônica foi revista, com realce para a transmissão oral, a forma indeterminada e as síndromes crônicas. Metanálises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade após instalação da cardiomiopatia crônica. Condutas terapêuticas aplicáveis aos indivíduos com a FIDC foram abordadas especificamente. Todos os métodos para detectar alterações estruturais e/ou funcionais com variadas técnicas de imageamento cardíaco também foram revisados, com recomendações de uso nos vários cenários clínicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por métodos que detectam fibrose miocárdica.
A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.nbsp;nbsp;nbsp;
Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.nbsp;

Bell pepper rootstocks with multiple resistance to soilborne diseases

ABSTRACT Grafting has been increasingly used to overcome soilborne pathogens. The commercial rootstock hybrids AF-8253 (Sakata) and Fortaleza (Takii), as well as Embrapa’s new hybrid, BRS Acará, were evaluated as rootstocks for the bell pepper hybrids Margarita (Syngenta) and Pampa (Clause) as scions, under natural infestation of Ralstonia pseudosolanacearum, as well as upon artificial inoculation with a) ten Ralstonia isolates, b) one Phytophthora capsici isolate, c) Meloidogyne incognita race 1, and d) Meloidogyne enterolobii (only rootstocks for c and d). Grafted bell peppers consistently showcased a higher productivity than did non-grafted ones under natural R. pseudosolanacearum infestation, and the rootstock genotype affected fruit size distribution. Rootstocks presented higher resistance than did commercial bell pepper hybrids (scions) for 6 out of 10 Ralstonia isolates. AF-8253 and BRS Acará displayed an immune-like response to P. capsici, while Fortaleza was moderately resistant (25% symptomatic plants) and the bell pepper hybrids (scions) were susceptible (100% symptomatic plants). Rootstocks were immune to M. incognita race 1, but slightly (AF-8253 and BRS Acará) or moderately (Fortaleza) resistant to M. enterolobii. Grafting with BRS Acará, as well as with AF-8253 or Fortaleza, is recommended for managing bacterial wilt, Phytophthora blight, and root-knot nematodes.

Abstract The crude oil that reached the Brazilian coast in 2019 was the most significant environmental disaster ever recorded in Brazilian marine waters, with severe ecological and economic repercussions not fully dimensioned and understood. One consequence of this kind of oil spill is the absorbed dose delivered to marine organisms. The biota exposure to radiation can introduce consequences that range from fertility decrease to death of exposed population. Therefore, the current study aims to use the ICRP reference organisms (flatfish, crab, and brown seaweed) to assess marine biota exposure due to uranium and thorium series radiation. The oil spill scenario, as well as the reference biota, were simulated in MCNP. Thorium series stood out for presenting a significant contribution to the dose. Furthermore, it was noticed that a remedial action capable of removing Tl-208 would significantly decrease the effects of radiation on marine biota. Finally, dose conversion coefficients for both uranium and thorium series were obtained as oil activity concentration functions. The results obtained here can be used in an oil spill event along with other worldwide recognized models. In addition, the dose coefficients can be used strategically to assess the maximum exposure time for emergency oil control, removal, and mitigation.

ABSTRACT Droplet size is one of the most important factors that affect spray deposition and weed control through the use of herbicides. The aim of this study was to evaluate the influence of the droplet size on the spray deposition and control of Euphorbia heterophylla and Urochloa ruziziensis by using glyphosate. The treatments included spraying glyphosate with fine, medium, coarse, very coarse, and ultra-coarse droplet sizes to determine the deposition, uniformity of distribution, visual control, and dry weight reduction (DWR). The treatments were compared with the values of the dose required to achieve 50% DWR or 50% visual control (C50) at 7, 14, 21, and 28 days after herbicide application. Fine and medium droplet sizes afforded high deposition values and low distribution uniformities in E. heterophylla . Fine and medium droplet sizes yielded the lowest C50 on visual control and DWR in E. heterophylla, respectively. The droplet size did not affect the spray deposition on U. ruziziensis plants; however, the larger droplets had less uniformity of distribution. In U. ruziziensis plants, spraying glyphosate with coarse and medium droplet sizes yielded the lowest values of C50 to visual control and DWR, respectively.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Resumo Fundamento As características histopatológicas da doença de Chagas (DCC) são: presença de miocardite, destruição das fibras cardíacas e fibrose miocárdica. A Galectina-3 (Gal-3) é um biomarcador envolvido no mecanismo de fibrose e inflamação que pode ser útil para a estratificação de indivíduos com DCC por risco. Objetivos Nosso objetivo foi avaliar se níveis elevados de Gal-3 estão associados a formas graves de cardiomiopatia chagásica (CC) e são preditivos de mortalidade. Métodos Estudamos doadores de sangue (DS) positivos para anti-T. cruzi: não-CC-DS (187 DS sem CC com eletrocardiograma [ECG] e fração de ejeção do ventrículo esquerdo [FEVE] normais); CC-Não-Dis-DS (46 DS com CC e apresentando ECG anormal, mas FEVE normal); e 153 controles negativos correspondentes. Esta amostra foi composta por 97 pacientes com CC grave (CC-Dis). Usamos as correlações de Kruskall-Wallis e Spearman para testar a hipótese de associações, assumindo um p bicaudal <0,05 como significativo. Resultados O nível de Gal-3 foi de 12,3 ng/mL para não-CC-DS, 12,0 ng/mL para CC-Não-Dis-DS, 13,8 ng/mL para controles e 15,4 ng/mL para CC-Dis. FEVE <50 foi associada a níveis mais elevados de Gal-3 (p=0,0001). Em nosso modelo de regressão linear ajustado, encontramos associação entre os níveis de Gal-3 e os parâmetros do ecocardiograma em indivíduos positivos para T. cruzi. Nos pacientes CC-Dis, encontramos uma associação significativa de níveis mais elevados de Gal-3 (≥15,3 ng/mL) e morte ou transplante cardíaco em acompanhamento de cinco anos (Hazard ratio – HR 3,11; IC95% 1,21– 8,04; p=0,019). Conclusões Em pacientes com CC, níveis mais elevados de Gal-3 estiveram significativamente associados a formas graves da doença e maior taxa de mortalidade em longo prazo, o que significa que pode ser um meio efetivo para identificar pacientes de alto risco. This cohort was composed of 97 patients with severe CC (CC-Dys). We used Kruskall-Wallis and Spearman’s correlation to test hypothesis of associations, assuming a two-tailed p<0.05 as significant. Results The Gal-3 level was 12.3 ng/mL for Non-CC-BD, 12.0 ng/mL for CC-Non-Dys-BD, 13.8 ng/mL for controls, and 15.4 ng/mL for CC-Dys. LVEF<50 was associated with higher Gal-3 levels (p=0.0001). In our linear regression adjusted model, we found association between Gal-3 levels and echocardiogram parameters in T. cruzi-seropositive subjects. In CC-Dys patients, we found a significant association of higher Gal-3 levels (≥15.3 ng/mL) and subsequent death or heart transplantation in a 5-year follow-up (Hazard ratio – HR 3.11; 95%CI 1.21–8.04; p=0.019). Conclusions In ChD patients, higher Gal-3 levels were significantly associated with severe forms of the disease and more long-term mortality, which means it may be a useful means to identify high-risk patients. Alguns comentários recebidos pelos organizadores e sugestões para os organizadores do próximo Monocots, na Costa Rica, também são fornecidos. Este artigo complementa os dados publicados no livro de resumos, disponível no Repositório Digital da UFRN.
Abstract In this paper we present an overview of the abstracts, scientific and social programs, field trips, and achievements in terms of participant number, represented countries, sponsorships, and themes treated during the Sixth International Conference on the Comparative Biology of Monocotyledons that took place in Natal, Brazil, October 7th -12th 2018. Some comments received by the organizers and a few suggestions for organizers of the next meeting, in Costa Rica, are also provided. The conference’s complete abstract book was published and provide additional information.

Radiomic analysis of MRI to Predict Sustained Complete Response after Radiofrequency Ablation in Patients with Hepatocellular Carcinoma - A Pilot Study

OBJECTIVES: To investigate whether quantitative textural features, extracted from pretreatment MRI, can predict sustained complete response to radiofrequency ablation (RFA) in patients with hepatocellular carcinoma (HCC). METHODS: In this IRB-approved study, patients were selected from a maintained six-year database of consecutive patients who underwent both pretreatment MRI imaging with a probable or definitive imaging diagnosis of HCC (LI-RADS 4 or 5) and loco-regional treatment with RFA. An experienced radiologist manually segmented the hepatic nodules in MRI arterial and equilibrium phases to obtain the volume of interest (VOI) for extraction of 107 quantitative textural features, including shape and first- and second-order features. Statistical analysis was performed to evaluate associations between textural features and complete response. RESULTS: The study consisted of 34 patients with 51 treated hepatic nodules. Sustained complete response was achieved by 6 patients (4 with single nodule and 2 with multiple nodules). Of the 107 features from the arterial and equilibrium phases, 20 (18%) and 25 (23%) achieved AUC >0.7, respectively. The three best performing features were found in the equilibrium phase: Dependence Non-Uniformity Normalized and Dependence Variance (both GLDM class, with AUC of 0.78 and 0.76, respectively) and Maximum Probability (GLCM class, AUC of 0.76). CONCLUSIONS: This pilot study demonstrates that a radiomic analysis of pre-treatment MRI might be useful in identifying patients with HCC who are most likely to have a sustained complete response to RFA. Second-order features (GLDM and GLCM) extracted from equilibrium phase obtained highest discriminatory performance.

Resumo Fundamento A intervenção percutânea em pacientes com disfunção de prótese biológica mitral apresenta-se como uma alternativa ao tratamento cirúrgico convencional. Objetivo Relatar a primeira experiência brasileira de implante transcateter de bioprótese valve-in-valve mitral via transeptal (TMVIV-via transeptal). Métodos Foram incluídos pacientes portadores de disfunção de bioprótese cirúrgica submetidos ao TMVIV-transeptal em 12 hospitais brasileiros. Foram considerados estatisticamente significativos valores de p<0,05. Resultados Entre junho/2016 e fevereiro/2019, 17 pacientes foram submetidos ao TMVIV-via transeptal. A mediana de idade foi 77 anos (IIQ,70-82), a mediana do escore STS-PROM foi 8,7% (IIQ,7,2-17,8). Todos os pacientes tinham sintomas limitantes de insuficiência cardíaca (CF≥III), tendo 5 (29,4%) sido submetidos a mais de uma toracotomia prévia. Obteve-se sucesso do TMVIV-via transeptal em todos os pacientes. A avaliação ecocardiográfica demonstrou redução significativa do gradiente médio (pré-intervenção, 12±3,8 mmHg; pós-intervenção, 5,3±2,6 mmHg; p<0,001), assim como aumento da área valvar mitral (pré-intervenção, 1,06±0,59 cm2; pós-intervenção, 2,18±0,36 cm2; p<0,001) sustentados em 30 dias. Houve redução significativa e imediata da pressão sistólica de artéria pulmonar, com redução adicional em 30 dias (pré-intervenção, 68,9±16,4 mmHg; pós-intervenção, 57,7±16,5 mmHg; 30 dias, 50,9±18,7 mmHg; p<0,001). Durante o seguimento, com mediana de 162 dias (IIQ, 102-411), observou-se marcada melhora clínica (CF≤II) em 87,5%. Um paciente (5,9%) apresentou obstrução de via de saída de ventrículo esquerdo (VSVE), evoluindo para óbito logo após o procedimento, e outro morreu aos 161 dias de seguimento.Conclusão: A primeira experiência brasileira de TMVIV-transeptal demonstra a segurança e a efetividade dessa nova técnica. Abstract Background Percutaneous intervention in patients with bioprosthetic mitral valve dysfunction is an alternative to conventional surgical treatment. Objectives To report the first Brazilian experience with transseptal transcatheter bioprosthetic mitral valve-in-valve implantation (transseptal-TMVIV). Methods Patients with surgical bioprosthetic dysfunction submitted to transseptal-TMVIV in 12 Brazilian hospitals were included. The significance level adopted was p<0.05. Results From June/2016 to February/2019, 17 patients underwent transseptal-TMVIV. Their median age was 77 years (IQR,70-82) and median Society of Thoracic Surgeons predicted risk of mortality (STS-PROM) score was 8.7% (IQR,7.2-17.8). All patients had limiting symptoms of heart failure (FC≥III) and 5 (29.4%) had undergone more than one previous thoracotomy. Transseptal-TMVIV was successful in all patients. Echocardiographic assessment showed a significant reduction in mean mitral valve gradient (pre-intervention, 12±3.8 mmHg; post-intervention, 5.3±2.6 mmHg; p<0.001), in addition to an increase in mitral valve area (pre-intervention, 1.06±0.59 cm2; post-intervention, 2.18±0.36 cm2; p<0.001) sustained for 30 days. There was a significant and immediate reduction in the pulmonary artery systolic pressure, with an additional reduction in 30 days (pre-intervention, 68.9±16.4 mmHg; post-intervention, 57.7±16.5 mmHg; 30 days, 50.9±18.7 mmHg; p<0.001). During follow-up (median, 162 days; IQR, 102-411), significant clinical improvement (FC≤II) was observed in 87.5% of the patients. One patient (5.9%) had left ventricular outflow tract (LVOT) obstruction and died right after the procedure, and another died at 161 days of follow-up. Conclusion The first Brazilian experience with transseptal-TMVIV shows the safety and effectivity of the new technique. The LVOT obstruction is a potentially fatal complication, reinforcing the importance of patients’ selection and of procedural planning. (Arq Bras Cardiol. 2020; [online].ahead print, PP.0-0)
