Resumo: Introdução: As Diretrizes Curriculares Nacionais para os cursos de Graduação em Medicina (DCN) de 2014 definem que o(a) graduando(a) deve ser formado(a) para abordar a diversidade biológica, étnico-racial, de gênero e orientação sexual. Entretanto, essa temática costuma ser invisibilizada, reiterando a LGBTI+fobia institucional que perpetua desigualdades e iniquidades no ensino e cuidado em saúde. Essa situação se tornou mais explícita durante a pandemia da Covid-19. Objetivo: Analisar as DCN sob a ótica das diversidades de gênero e sexual, de modo a problematizar “o que” e “como” pode ser aprimorado na educação médica em relação às questões LGBTI+. Desenvolvimento: Considerando que a aprendizagem de adultos ocorre a partir da problematização de vivências do cotidiano, serão apresentados três casos relativos ao atual momento da pandemia da Covid-19, com base nos quais se podem abordar temas sobre a saúde LGBTI+ no curso médico. Após, serão problematizadas algumas competências específicas sobre saúde LGBTI+ que podem ser aprimoradas durante a formação médica, inclusive no período da pandemia da Covid-19, a fim de visibilizar a temática LGBTI+ no currículo como uma estratégia de combater a LGBTI+fobia na escola médica e promover o cuidado integral em saúde. Conclusão: É possível abordar sobre a saúde LGBTI+ a partir de situações vivenciadas durante a pandemia da Covid-19. Entretanto, é necessário ter clareza de como as DCN se traduzem em competências específicas sobre saúde LGBTI+. Essa pode ser uma das estratégias a fim de tornar os currículos mais acolhedores e compromissados com as necessidades de saúde dessa população.

Abstract: Introduction: The 2014 National Curriculum Guidelines (“DCNs”) determine that undergraduates should be trained to address biological, ethnic-racial, gender and sexual orientation diversity. However, academic medical training only occasionally addresses this theme, which perpetuates inequality and prevents physicians from being adequately trained to serve this population. The reorganization of curricular activities due to the Covid-19 pandemic may have aggravated this situation and this invisibility emphasizes institutional LGBTI+phobia. Objective: To shed light on the LGBTI+ theme in academic medical training is one of the strategies to tackle LGBTI+phobia in medical school. This essay will analyze the “DCNs” from the perspective of gender and sexual diversity, recommending related competencies on LGBTI+ health that should feature in the course, including the Covid-19 pandemic period. Development: We will present three cases related to the current moment of the Covid-19 pandemic, from which topics on LGBTI + health could be discussed in academic medical training. After analyzing the DCNs and the needs arising from these cases, some competencies in LGBTI+ health will be proposed for undergraduate medicine students; considering that adult learning results from critically questioning everyday experiences. Conclusion: we believe that it is possible to approach LGBTI+ health from situations experienced during the Covid-19 pandemic. However, it is necessary to be clear how the DCNs translate to LGBTI+ health-related skills. This might be one of the strategies to make academic medical training more welcoming and committed to the health needs of this group.

Resumo: Introdução: A atual pandemia da Sars-CoV-2 (Covid-19) deixou ainda mais evidentes a desigualdade social e o impacto dos determinantes sociais nos indicadores de saúde. Por conta disso, os profissionais de saúde, para cuidar das pessoas e populações, devem desenvolver ações que promovam a equidade, papel que também cabe às instituições de ensino e saúde. As Diretrizes Curriculares Nacionais para os Cursos de Graduação em Medicina trazem em seu artigo 3º que a formação do(a) graduando(a) deverá ocorrer com “responsabilidade social e compromisso com a defesa da cidadania”. Relato de Experiência: Este relato mostra como o debate da determinação social do processo de saúde e doença vem ocorrendo ao longo dos eventos nacionais de educação médica, especificamente nos Congressos Brasileiros de Educação Médica (Cobem), e como isso resultou na construção coletiva e colaborativa do grupo de trabalho Populações (In)Visibilizadas e Diversidades da Abem (GT-PIVD). Discussão: Os Cobems têm ampliado as oportunidades para debates sobre determinantes sociais por meio de atividades relacionadas às questões LGBTI+, racial, da mulher e indígena. O GT-PIVD, criado em 2019, reúne um coletivo de educandos(as) e educadores(as) com o objetivo de promover esse debate na formação médica e nos eventos da Abem, além de ser uma comunidade de trocas de experiências sobre as populações (in)visibilizadas e diversidades. Conclusão: Algumas iniciativas, como a “Carta de Porto Alegre”, sobre equidade de gênero e diversidade sexual, e a criação do GT-PIVD, são produtos dos Cobem. O GT-PIVD, por meio de sua comunidade de práticas virtuais, tem se tornando um espaço importante de trocas de experiências entre educandos(as) e educadores(as) sobre racismo, sexismo, LGBTIfobia e outras violências, inclusive durante a pandemia da Covid-19.

Abstract: Introduction: The current SARS-COV 2 pandemic (Covid-19) has made social inequality and the impact of social determinants on health indicators even more explicit. It is evident that health professionals, as well as educational and health institutions, must develop actions to promote equity in order to care for people and populations. The National Curricular Guidelines for Undergraduate Courses in Medicine bring in their third article that the training of the undergraduate student must occur with “social responsibility and commitment to promote citizenship”. Experience Report: This report presents how the debate on the social determination of the health and disease process has been taking place throughout the national Medical Education events, specifically in the Brazilian Medical Education Congress (“Cobem”), and how this resulted in the collective and collaborative initiative of the (In)Visible Populations and Diversities of the ABEM Working Group (“GT-PIVD”). Discussion: The “Cobems” have expanded opportunities for debates on social determinants through activities related to LGBTI+, race, women and indigenous issues. The “GT-PIVD”, created in 2019, gathers students and educators with the aim of promoting this debate in medical settings and at ABEM events, constituting a community for exchanging experiences about (in)visible populations and diversities. Conclusion: Some initiatives, such as the Porto Alegre Letter, on gender equality and sexual diversity, and the creation of the GT - PIVD, are products of the “COBEMs”. The GT-PIVD, through its community of virtual practices, has become an important space for exchanging experiences between students and educators on racism, sexism, LGBTIphobia and other violence, including during the COVID-19 pandemic.

RESUMO Bactérias promotoras de crescimento em plantas (BPCP) são encontradas em tecidos vegetais e promovem o crescimento vegetal por meio da secreção de hormônios e enzimas, ou auxiliando a absorção de nutrientes. Avaliaram-se quarenta isolados de BPCP para determinar seus efeitos sobre o crescimento de milho e sorgo. Estes isolados foram também comparados com plantas não inoculadas, como controles negativos (-N; sem fertilização nitrogenada) e positivos (+N; com fertilização nitrogenada). A altura, diâmetro de caule, massas secas da parte aérea e raízes, acúmulo de N e conteúdo de clorofila foram avaliados. Em ambos o milho e o sorgo, a altura, diâmetro do caule e a massa seca da parte aérea em plantas inoculadas com BPCP foram similares aos das plantas não inoculadas e fertilizadas com N, e as respostas da massa das raízes foram maiores que das plantas fertilizadas com N. Entretanto, os isolados de BPCP não promoveram acúmulo de N e conteúdo de clorofila semelhantes aos das plantas não inoculadas e fertilizadas com N. IPACC26 e IPACC30, ambos identificados como Bacillus subtilis, apresentaram melhores respostas para o crescimento das plantas e acúmulo de N do que os outros isolados.

ABSTRACT Plant growth-promoting bacteria (PGPB) are found in plant tissues and promote plant growth by secretion of hormones and enzymes, or by facilitating the nutrient uptake. This study assessed forty PGPB isolates to determine their effects on maize and sorghum growth. These isolates were also compared with uninoculated plants, as negative (-N; without N fertilization) and positive (+N; with N fertilization) controls. Plant height, stem diameter, shoot and root dry mass, leaf N accumulation and chlorophyll content were evaluated. For both the maize and sorghum, the height, stem diameter and shoot dry mass in plants inoculated with PGPB were similar to those of uninoculated plants supplied with N, and the responses for root mass were higher than in plants supplied with N. However, the PGPB isolates did not promote N accumulation and chlorophyll content similar to those of uninoculated plants supplied with N. The IPACC26 and IPACC30 isolates, both identified as Bacillus subtilis, resulted in better responses for plant growth and N accumulation than the other isolates.

The oral language development depends on the effective development of the hearing system. In cases of children presenting with hearing loss, a cochlear implant is an electronic device indicated to (re)habilitate the hearing function. Thus, it is of paramount importance to assess and follow the oral language development of children fitted with a cochlear implant (CI) to measure the effectiveness of the electronic device and support the therapeutic planning of these children. Questions are currently being raised about the instruments to assess the oral language of children using a CI, and, seeking the answers, this systematic review aimed at surveying these instruments. Searches were performed in three different databases utilizing six different descriptors to select articles published from 2004 to 2009 that performed an oral language assessment of children with a CI. Initially, 373 articles were found, and, after the application of inclusion criteria, 47 articles were analyzed, resulting in a survey of 74 instruments for oral language assessment, including tests, questionnaires and inventories. In analyzing the articles, it was realized that the studies included in this systematic review presented varied methodologies and low levels of evidence, with a greater concentration of instruments assessing receptive and expressive language, emphasizing the survey of the child's vocabulary and questionnaires. Thus, it can be verified that other linguistic skills, such as morphosyntactic, semantic, and narrative-pragmatic ones that are important in structuring speech and language for the effectiveness of the child's speech, are not being focused on. Just one of the instruments cited, a questionnaire, was specific for the oral language assessment of children with cochlear implants.

A integração curricular, atualmente, é apontada como importante estratégia de ensino, processo que envolve várias etapas, trabalho e compromisso de grupo. Este artigo relata a experiência de integração das disciplinas de humanidades médicas da Faculdade de Medicina da Universidade de São Paulo (FMUSP). Ao longo de dois anos, constituiu-se um grupo composto pelas disciplinas da área de humanidades médicas, pesquisadores do Centro de Desenvolvimento da Educação Médica Professor Eduardo Marcondes (Cedem), alunos e professores de outras disciplinas interessados na área. Desenvolveu-se a integração temática e prossegue-se na integração metodológica.

Currently curricular integration is considered an important educational strategy, a process involving several stages, work and group commitment. This paper relates the experience of the Faculdade de Medicina da Universidade de São Paulo (FMUSP) of integrating medical humanities disciplines. During two years a group was composed involving professors of medical humanities disciplines, researchers from the Center of Development of Medical Education Professor Eduardo Marcondes (Cedem) as well as students and professors of other disciplines interested in this area. The thematic integration has already been concluded and the methodological integration is in process.

OBJETIVO: Descrever as características clínicas e terapêuticas de pacientes com insuficiência cardíaca (IC) secundária a miocardiopatia chagásica crônica, bem como avaliar se estas são diferentes nas demais etiologias. MÉTODOS: Foram analisados prospectivamente pacientes atendidos no período de agosto de 2003 a junho de 2004, em um ambulatório de referência para IC. RESULTADOS: Foram incluídos 356 pacientes com o diagnóstico de IC. Miocardiopatia chagásica foi a etiologia mais freqüente, (48% dos casos). Outras etiologias foram miocardiopatia hipertensiva em 19%, dilatada idiopática em 11%, e isquêmica em 9%. Pacientes com IC secundária a miocardiopatia chagásica tinham com maior freqüência etnia não-branca (88 x 75%; p = 0,002), história familiar de doença de Chagas (57 x 21%; p = 0,001), maior tempo de doença (71 x 56 meses; p = 0,034), menor escolaridade (4,4 ± 4,1 x 5,7 ± 4,2 anos de estudo; p = 0,004), menor freqüência cardíaca (69 ± 12 x 73 ± 13; p = 0,03) e pressão arterial sistólica (121 ± 25 x 129 ± 28 mmHg; p = 0,006). Utilizavam com maior freqüência amiodarona (22 x 13%; p = 0,036) marcapassos artificiais (15 x 1%; p = 0,001) e com menor freqüência drogas betabloqueadoras (39 x 59%; p = 0,001). CONCLUSÃO: Nessa amostra de pacientes ambulatoriais com IC, em um estado com alta prevalência de doença de Chagas, miocardiopatia chagásica foi a etiologia mais freqüente, apresentando algumas características clínicas e terapêuticas diferentes dos demais pacientes.

OBJECTIVE: Describe the clinical and therapeutic characteristics of patients with heart failure (HF) secondary to chronic chagasic cardiomyopathy and evaluate if these characteristics are different from those found in other etiologies. METHODS: A prospective analysis of the patients treated between August 2003 and June 2004 at a HF referral outpatient clinic was conducted. RESULTS: Three hundred and fifty six patients diagnosed with HF were included in the study. Chagasic cardiomyopathy was the most common etiology (48% of the cases). Other etiologies included hypertensive cardiomyopathy in 19% of the patients, idiopathic dilated in 11% and ischemic in 9%. Patients with HF secondary to chagasic cardiomyopathy were more frequently from non-white ethnic groups (88 vs. 75%; p = 0.002), had a family history of Chagas disease (57 vs. 21%; p = 0.001), had the disease for a longer length of time (71 vs. 56 months; p = 0.034), had lower levels of education (4.4 ± 4.1 vs. 5.7 ± 4.2 years of study; p = 0.004), had a lower heart rate (69 ± 12 vs. 73 ± 13; p = 0.03) and a lower systolic blood pressure (121 ± 25 vs. 129 ± 28 mmHg; p = 0.006). There was also a higher incidence of the use of amiodarone (22 vs. 13%; p = 0.036) and artificial pacemakers (15 vs. 1%; p = 0.001). There was a lower usage of beta-blockers (39 vs. 59%; p = 0.001). CONCLUSION: In this sample of HF outpatients, in a state with a high prevalence of Chagas disease, chagasic cardiomyopathy was the most common etiology and they presented some unique clinical and therapeutic characteristics in comparison to other heart failure patients.

Resumo: As Diretrizes Curriculares Nacionais do Curso de Graduação em Medicina reforçam a articulação entre a Educação Superior e a Saúde. A faculdade de Medicina da Universidade de São Paulo definiu, em 1998, princípios muito semelhantes àqueles das Diretrizes formação generalista e humanista , crítica e reflexiva , com capacitação para atuar baseando-se em princípios éticos no processo de saúde doença em seus diferentes níveis de atenção , com ações de promoção , prevenção , recuperação e reabilitação , na perspectiva da integridade da assistência , com senso de responsabilidade social e compromisso com a cidadania , como promotor da saúde integral do ser humano . Este artigo documenta as impressões dos alunos do curso médico da FMUSP sobre algumas das propostas curriculares de 1998 presentes nas Diretrizes Nacionais Na FMUSP ocorre maior concentração didática em ambiente hospitalar. Esse contato não sensibiliza o estudante a conhecer os fundamentos do Sistema Único de Saúde. A metodologia de ensino predominante é a tradicional, com aulas expositivas e laboratórios de atividades. A integração entre as disciplinas tem sido limitada. A inserção dos alunos em projetos de iniciação científica e em atividades de monitoria pode estimular tal integração.

Abstract: The Brazilian National Curriculum for Medical Education emphasizes the interaction between the fields of health and education. Based on a review of its medical education program in 1998, the University of São Paulo Medical School (FMUSP) defined principles that are similar to those of the National Curriculum: a generalist humanitarian, judicious and reflexive education, fostering the capacity to work under ethical principles in the health-disease process at its different levels, acting to prevent disease and promote and rehabilitate health, considering the comprehensive nature of health care with social responsibility and a commitment to citizens' rights as factors for integral human health. This report discusses opinions by undergraduate medical students from FMUSP concerning key innovations in the 1998 reform that share the orientation of the National Curriculum. FMUSP has a relatively greater focus on hospital-based activities. This hospital-centered contact generally leaves students unaware of the underlying principles in the Unified National Health System (SUS). The teaching methodology is traditional: teacher-centered, with lectures and laboratory activities. There is limited integration among courses. Students participation in ongoing science projects and as tutors for beginning students could help stimulate greater integration among courses.

Despite the fact that majority of OCD patients responds to conventional treatments, about 40% remain symptomatic after appropriate trials with serotonin uptake inhibitors (SRIs) or behavioral therapy. A patient is considered treatment-resistant after failing to respond to three pharmacological trials with SRIs as well as to behavior therapy. When all somatic strategies have been tried, including augmentation with other medications, and the patient remains symptomatic, he or she is considered treatment refractory. The concepts of resistance and refractoriness are briefly revised, and the current augmentation strategies, as well as non-pharmacological approaches such as neurosurgery and transcranial magnetic stimulation, are addressed in this paper. Finally, issues of predictive response factors, pharmacogenetics, and the pathophysiology of resistant cases are suggested as targets for further researches.
