Abstract A persistent sciatic artery is an embryological remnant of the internal iliac artery that occurs in 0.03% to 0.06% of the population and may develop aneurysmal degeneration. Aneurysms can lead to distal embolization with increased risk of limb loss, especially if the sciatic artery is the main arterial supply to the limb. A sciatic artery aneurysm must be treated whenever diagnosed, because of the high risk of complications. Treatment options include open, endovascular, or hybrid repair. This manuscript describes a patient with bilateral persistence of the sciatic arteries, both with aneurysmal degeneration, who underwent endovascular repair with Covera® (Bard Medical, Georgia-USA) covered stents.

Resumo A persistência da artéria isquiática é um remanescente embriológico da artéria ilíaca interna que ocorre em 0,03% a 0,06% da população, podendo evoluir com degeneração aneurismática. A presença do aneurisma pode levar a embolização, com aumento de risco de perda do membro, principalmente se a artéria isquiática for seu principal suprimento arterial. O tratamento do aneurisma de artéria isquiática está indicado sempre que diagnosticado, devido ao alto risco de complicações. Entre as opções de tratamento, estão o tratamento aberto convencional, o tratamento endovascular e o tratamento híbrido. No presente estudo, será descrito o caso de um paciente apresentando persistência completa das artérias isquiáticas bilateralmente, com degeneração aneurismática de ambas, corrigida de forma endovascular com stent recoberto Covera® (Bard Medical, Geórgia, Estados Unidos).

<sec><title>OBJETIVO:</title><p>avaliar se a administração sistêmica de cilostazol reduz a hiperplasia neointimal nas artérias ilíacas de suínos submetidas à angioplastia com cateter balão.</p></sec><sec><title>MÉTODOS:</title><p> vinte suínos foram submetidos à angioplastia com cateter balão 6x40 mm na artéria ilíaca comum direita, guiada por ultrassonografia com Doppler. Os animais foram randomizados em dois grupos: grupo 1 (n=10), no qual foi administrado cilostazol 50mg em duas doses diárias, e grupo 2 (n=10), considerado controle. Após 30 dias, os animais foram mortos, e as artérias ilíacas preparadas para análise histológica. Os cortes histológicos foram digitalizados e analisados por morfometria digital. A análise estatística foi realizada com o teste t de Student e o de Mann-Whitney.</p></sec><sec><title>RESULTADOS:</title><p>comparando as artérias ilíacas submetidas à angioplastia com as artérias não submetidas à angioplastia, houve hiperplasia neointimal significativa (0,228 versus 0,119 mm²; p=0,0001). Nas artérias submetidas à angioplastia, não houve diferença entre o grupo 1 (cilostazol) e o grupo 2 (controle) na área do lúmen (2,277 versus 2,575 mm²; p=0,08), área da íntima (0,219 versus 0,237 mm²; p=0,64), área da média (2,262 versus 2,393 mm²; p=0,53) e no percentual de obstrução neointimal (8,857 versus 9,257 %; p=0,82).</p></sec><sec><title> CONCLUSÃO:</title><p>O uso de cilostazol 50mg administrado em duas doses diárias durante 30 dias não reduziu a hiperplasia neointimal em artérias ilíacas de suínos submetidas à angioplastia com cateter balão.</p></sec>

<sec><title>OBJECTIVE:</title><p> to evaluate whether systemic administration of cilostazol reduces neointimal hyperplasia in iliac arteries of pigs submitted to balloon catheter angioplasty.</p></sec><sec><title>METHODS:</title><p> twenty pigs underwent angioplasty with a 6x40 mm balloon catheter in the right common iliac artery, guided by Doppler ultrasound. The animals were randomized into two groups: group 1 (n=10), which received 50mg cilostazol twice a day, and group 2 (n=10), control. After 30 days, the animals were killed and the iliac arteries prepared for histological analysis. The histological sections were digitized and analyzed by digital morphometry. Statistical analysis was performed using the Student t and Mann-Whitney tests.</p></sec><sec><title>RESULTS:</title><p> when comparing the iliac arteries submitted to angioplasty with those not subjected to angioplasty, there was significant neointimal hyperplasia (0.228 versus 0.119 mm²; p=0.0001). In arteries undergoing angioplasty, there was no difference between group 1 (cilostazol) and group 2 (control) as for the lumen area (2.277 versus 2.575 mm²; p=0.08), the tunica intima (0.219 versus 0.237 mm²; p=0.64), the tunica media (2.262 vs. 2.393 mm²; p=0.53) and the neointimal occlusion percentage (8.857 vs. 9.257 %; p=0.82).</p></sec><sec><title>CONCLUSION:</title><p> the use of cilostazol 50mg administered in two daily doses did not reduce neointimal hyperplasia in iliac arteries of pigs submitted to balloon angioplasty catheter.</p></sec>

OBJETIVO: Descrever um estudo experimental avaliando através da morfometria digital o espessamento intimal na parede arterial após o implante de "stents" metálicos auto-expansíveis recobertos ou não com politetrafluoroetileno (PTFE) na aorta de suínos. MÉTODOS: Em três grupos de suínos jovens uma bainha introdutora de 12 F foi inserida na aorta abdominal distal. Os animais do grupo I (n=5) foram considerados controle. Os animais do grupo II (n=10) receberam o implante de um stent metálico auto-expansível não recoberto. No grupo III (n=10) um stent auto-expansível recoberto com PTFE foi inserido. Após quatro semanas os animais foram sacrificados e os espécimes arteriais foram retirados, sendo o espessamento intimal quantificado pela análise morfométrica. RESULTADOS: Na comparação entre os grupos I, II e III quanto às áreas da íntima, média e índice intimal, não foi observada variação estatisticamente significativa. Diferenças foram observadas entre os grupos em relação às áreas luminais proximais (p=0,0036) e distais (p=0,044). Através dos testes de comparação múltipla para Kruskal-Wallis foi identificada uma diferença entre os grupos I e II. Entretanto, quando essas variáveis foram controladas pelo fator peso (relação área luminal/peso), a diferença não foi mais observada. CONCLUSÕES: Nesse estudo a curto prazo, o revestimento de PTFE não esteve associado a adicional espessamento intimal além daquele promovido pelo dispositivo metálico em artérias de grande calibre e condições de alto fluxo.

PURPOSE: To report an experimental study evaluating, through digital morphometry, the intimal thickening of the arterial wall after the implant of auto-expandable stainless steel stents covered or not with polytetrafluoroethylene (PTFE) in the pig aorta. METHODS: In three groups of pigs a 12 F sheath was inserted in distal abdominal aorta. Group I animals (n=5) served as control. Group II animals (n=10) received an auto-expandable uncovered metallic stent. On group III animals (n=10) an auto-expandable stent covered with PTFE was inserted. After four weeks the animals were killed, the specimens were harvested and the intimal thickening was quantified by morphometric analysis. RESULTS: In the comparison among groups I, II and III referring to intimal, medial area and intimal index, it was not observed statistically significant variation. Differences were identified among groups referring to luminal proximal (p=0,036) and distal areas (p=0,044). Through multiple comparison tests for Kruskal-Wallis it was identified a difference between groups I and II. However, when these variables were controlled by weigh factor (luminal area/weigh relation), this difference was not observed anymore. CONCLUSION: In this short term study, the PTFE covering is not associated to further intimal thickening besides that promoted by the metallic mesh in large arteries and in high flow conditions.
